The 340B drug pricing world can be difficult to understand. It’s easy to get lost between the requirements and needs of the many different 340B stakeholders: manufacturers, wholesalers, distributors, state Medicaid programs, and all of the qualifying unique safety net provider types. Transparency is especially important in the health care industry, particularly in complex programs like 340B.

What are the 340B Drug Pricing Program and the 340B Prime Vendor Program? The 340B Drug Discount Program is a federal drug pricing program that allows qualified health centers and hospitals to purchase outpatient drugs at deeply discounted prices. The intent is for these health care providers to reinvest the savings into programs that benefit the very communities they serve. The 340B statute requires a Prime Vendor Program (PVP). The PVP acts as an intermediary through which covered entities may enter into contracts for the distribution of drugs and also provides additional valuable services to covered entities. The Health Resources and Services Administration (HRSA) administers the 340B Drug Pricing Program through its Office of Pharmacy Affairs. Apexus is in its third five-year contract term with HRSA. Since 2004, Apexus has served as the federal 340B Prime Vendor, providing nearly 40,000 covered entities with discounts on drug distribution services, branded and generic medications, vaccines, women’s health products, and diabetic supplies. In that time, Apexus has generated $5.5 billion in value to PVP participants. Since HRSA first implemented the PVP in 1999, it has awarded the PVP contract to a prime vendor. The utility of a prime vendor for administrative simplicity, negotiating

Pyrosate has been developed as an easy-touse Limulus Amebocyte Lysate (LAL) gel-clot test that enables rapid testing and does not require special laboratory supplies. The easy-to-follow, step-by-step illustrated instructions allow the user to set up assays within minutes. In addition to FDA licensure, the Pyrosate® Kit is now available in sensitivities of 0.03 EU/mL, 0.125 EU/mL and 0.25 EU/mL, in both 10-pack and 30-pack formats. With the introduction of this licensed version, in addition to research and water testing, Pyrosate is ideal for dialysis centers, compounding pharmacies, and pharmaceutical and medical device companies.

Performing the Test

The Pyrosate® Kit is a rapid gel-clot test that contains a 2λ* endotoxin tube (PPC) matched to the sample (SPL) tube for each sensitivity. This feature is unique to the Pyrosate assay. The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.

Reconstitution Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance. Stability Pyrosate® is stable at room temperature and does not require refrigeration for shipping or storage. Hospitals and health care systems in the United States handle more than 35 million patients each year. These large operations are finding that the cost savings associated with effective pharmacy supply-chain management can have a big impact on the bottom line, as well as on patient care. Supplylogix offers a suite of software products that enables pharmacies to gain competitive advantages. THE CHALLENGE UW Health, an integrated health system associated with the University of Wisconsin-Madison, serves 600,000 patients each year in the Upper Midwest region of the country. With six hospitals and 15 dispensing sites, UW Health processes upwards of 44,000 prescriptions per month. A large portion of their services is related to specialty pharmacy items, including diabetes, transplant, and HIV care, making their inventory costs exceptionally high compared to standard retail pharmacy.

THE DECISION Until 2016, the pharmacy inventory management at UW Health was largely a manual process. “The pharmacy software we were using did not give the whole picture of what we needed to know for inventory management,” said Carrie Boeckelman, RPh, BCACP, Manager of Ambulatory Pharmacy Services for UW Health. Staff members routinely made decisions regarding minimums and maximums, often ordering large amounts of a drug in response to running out of stock a single time. “They went for the worst-case scenario, and their choices contributed to overstocking,” said Melissa Ngo, PharmD, BCACPA, Manager of Ambulatory Pharmacy Services for UW Health. “As a result, we had a lot of inventory dollars on our shelves that we did not need to have there. We knew we had to dig deeper.” Boeckelman attended a conference where she was introduced to Supplylogix and was able to learn more about the software. “It sounded like it made great sense for us,” she said. Supplylogix later provided a demo including an overview of the specific opportunity benefits to UW Health, and the decision was made. “We were doing the best we could manually, but Supplylogix really spoke to us,” Boeckelman said.

THE RESULTS Among Supplylogix's software solutions, UW Health uses Pinpoint Order for replenishment management, Pinpoint Transfer for redistribution intelligence, Pinpoint Audit for assessing risk, and Pinpoint Monitor for tracking controlled substances. “The Pinpoint Order software allows us to set maximums and minimums systematically based on industry best practices,” said Ngo. This control prevents unauthorized changes to ordering amounts and has seriously streamlined their inventory costs. “In the first six months of using the software, we reduced our managed inventory dollars by 23%,” Boeckelman said.

Even with a leaner inventory, UW Health distributes partial fills much less often than they had been before implementing Supplylogix. “What’s interesting is that we had more inventory on hand before, but it wasn’t the right inventory,” Ngo said. Pinpoint Transfer is another powerful technology tool that UW Health uses. “Prior to Pinpoint Transfer, each of our pharmacies used to send out emails to the other locations with a list of overstocked items to ask if they were in need,” Ngo said. “The system now shows us which drugs should be sent to which site.” The cost savings involved in redistributing pharmacy stock are very significant. “When it was a manual process we had very few transfers because no one had time to run each drug code to see if their site was using a particular medication,” said Boeckelman. “We now redistribute every week, and we sell through 90% of those drugs. Annualized, that’s $1.2 million not being lost to expirations.” Pinpoint Audit and Pinpoint Monitor offer UW Health a new level of security around sensitive drugs. “Our internal audit department conducts routine narcotic audits, and some drugs came back highlighted,” Ngo said. “When we dug into the reports, we were able to explain the issue. We have a high number of pediatricians who treat ADHD, and those were the drugs that were flagged.”

THE CONCLUSION UW Health implemented Supplylogix in 2016. Compared to manually managing inventory for 15 sites, the new tools have completely changed the way UW Health runs its pharmacies. Inventory costs are down, redistribution and sell-through rates are up, and workflow is optimized through automatic ordering and analytics report generation.

The following will briefly explain our procedures and service capabilities. n What to Expect On Inventory Day Upon arrival the Capital Inventory team will be introduced to the pharmacy and staff. Our team leader and a pharmacy representative will map the pharmacy and its respective areas. Team members are then assigned to the designated areas and proceed entering each NDC number and QTY into a hand-held data gathering computer. Once all areas are completed, the raw inventory data is then transmitted to an independent team for review and analysis. n What to Expect Post Inventory Day Items will be priced according to GPO/ Wholesaler Cost Data and Direct Purchase/ Secondary Wholesaler Cost Data. Upon review the data will be adjusted, compiled, and valuated. Finally, the inventory data will be delivered electronically as an Excel file. n What to Expect After Receipt of Inventory Data If it is identified that adjustments are needed we have an easy step-by-step way to apply the indicated changes. When submitted the adjustments can be made and returned within 24-48 hours. Unlike other companies that do not allow adjustment to the inventory data we will make adjustments to the data as requested to ensure the final value is a true representation of the on-hand inventory.

“I was a new and eager yet nervous APPE student that lacked the confidence in my abilities to interview patients, intervene and recommend therapies. Dr. McCafferty assured me on the first day of the rotation that I may observe her interactions with patients, review relevant content, and participate when I felt ready. After observing her confidence, clarity and compassion with patients for just one day, I immediately wanted to practice, and practice the way she did. She allowed me to see multiple patients each day, all who presented with diverse health conditions. Her instinct was to always hear my interventions and recommendations first, and would follow it up with her own reasoning and advice. She encouraged and empowered me to use knowledge and skills I gained in the classroom and gave me a comfortable platform to apply it. Regardless of how busy Dr. McCafferty’s day was, she never hesitated to stop what she was doing to answer any of my curiosities. She is a preceptor that students enjoy having, because she challenges them to think about the whole patient. Dr. Natasha Niedbalec is recognized by her students as someone who motivates and provides helpful feedback. Because of her influence and involvement through her practice site, her students were able to experience many different settings during their rotation, including management, inpatient, and outpatient. The University of New Mexico College of Pharmacy Recognizes Outstanding Preceptor, Anh T. Tran, PharmD, PhC, BCGP Dr. Anh T. Tran’s commitment to the pharmacy profession, and to training the next generation of pharmacists, is obvious to New Mexico’s student pharmacists who learn at her side during rounds at the New Mexico VA Healthcare System’s Community Living Center. Dr. Tran is not only a responsive leader and preceptor who tailors her rotations to meet her students’ goals, but also a devoted professional who positively impacts her patients and their family members. During the geriatrics/hospice and palliative medicine rotation, Dr. Tran teaches student pharmacists the veteran centered care approach by demonstrating and guiding compassionate pharmaceutical care of elderly patients. Dr. Tran, a 2007 graduate of the UNM College of Pharmacy, clinical pharmacy specialist and pharmacist clinician of 11 years, appreciates the opportunity to give back to New Mexico communities, serve veterans and work with UNM’s professionally-oriented student pharmacists. CONGRATULATIONS PRECEPTORS OF THE YEAR Harjeet “Micki” Singh, RPh (IPPE 1- Community Pharmacy Practice)

Dr. Singh is a partner and pharmacist in charge at Medical Arts Pharmacy in Bell, California. Medical Arts Pharmacy is an independent community pharmacy that services patients of all ethnicities and backgrounds with chronic conditions and acute self-care ailments. He received his pharmacy training from the University of Delhi, India. Dr. Singh is an active member of the California Pharmacists Association (CPhA) and National Community Pharmacists Association (NCPA). He has also served as a board member for nine years with the Indian Pharmacists Association and two years as the secretary. Dr. Singh is the former President of South East Pharmacy Association. He remains active in the community by providing monthly blood pressure screenings to senior centers and conducting health fairs. When he is not working, Dr. Singh enjoys traveling and spending time with his wife and two beautiful daughters.

Sofia Gezalyan, PharmD, BS, RPh, APh (IPPE 2- Institutional Pharmacy Practice)

Dr. Gezalyan received her Bachelor of Science in Health Promotion and Disease Prevention from USC in 2002 then earned her Doctor of Pharmacy degree from USC in 2006. Upon completion of her PharmD in 2006, Dr. Gezalyan started her career at Hollywood Presbyterian Medical Center (HPMC) as a Clinical/Quality Assurance Pharmacist. In 2007 she was promoted to Clinical Coordinator of Pharmacy Services. In that position, she aggressively expanded the Clinical Pharmacy program. Dr. Gezalyan was promoted a dual role, as the Manager of Inpatient Pharmacy Services and the Clinical Coordinator where she developed and implemented the Bedside Pharmacy Program. This concept was well received and adapted by many hospitals since then. In 2017, she was further promoted to Director of the Inpatient Pharmacy Services. In 2018, she became a licensed Advance Practice Pharmacist.

Lawrence Thompsen, BSc, RPh, MBA (IPPE 3- Selective Elective Pharmacy Practice)

Dr. Thompsen is a licensed pharmacist in multiple states and has more than 40 years of experience in hospital practice, management, home infusion, hospice care, nuclear pharmacy, and state and federal drug law enforcement. He has been a pharmacy lecturer and preceptor for more than 35 years. Dr. Thompsen is a member of numerous national, state and local pharmacists associations. He is also a dedicated member of the California Disaster HealthCare volunteers, and LA County EMS Agency. He is currently the Sr. Clinical Director and Vice President of Operations at a national home infusion provider in Los Angeles. Dr. Thompsen received his pharmacy degree from the University of Illinois, and later earned his Nuclear Pharmacist certification at Purdue University. Dr. Thompsen went on to earn MBA in International Health Care Finance from Keller Graduate School of Management.

Angela Villasenor, PharmD (APPE- Advanced Pharmacy Practice)

Dr. Villasenor graduated from Cal State University Long Beach in 2009 with a Bachelor of Science in Microbiology with a Minor in Chemistry, and a Bachelor of Arts in Spanish Language. She went on to work as a pharmacy technician for one year at County LAC+USC before returning to school to pursue her doctor of pharmacy. In 2014, Dr. Villasenor earned her PharmD from Loma Linda University with aspirations to enter into the ambulatory care field. Dr. Villasenor is also a certified pharmacist in the American Academy of HIV Medicine. She is currently working as the Pharmacist in Charge at Aids Healthcare Foundation. In her spare time, Dr. Villasenor teaches Pharmacotherapy in Infectious Disease as an adjunct professor at Charles Drew University in the Physician Assistant program. She joined West Coast University School of Pharmacy as an Advanced Pharmacy Practice Experience preceptor in 2016.

The 340B drug pricing world can be difficult to understand. It’s easy to get lost between the requirements and needs of the many different 340B stakeholders: manufacturers, wholesalers, distributors, state Medicaid programs, and all of the qualifying unique safety net provider types. Transparency is especially important in the health care industry, particularly in complex programs like 340B.

What are the 340B Drug Pricing Program and the 340B Prime Vendor Program? The 340B Drug Discount Program is a federal drug pricing program that allows qualified health centers and hospitals to purchase outpatient drugs at deeply discounted prices. The intent is for these health care providers to reinvest the savings into programs that benefit the very communities they serve. The 340B statute requires a Prime Vendor Program (PVP). The PVP acts as an intermediary through which covered entities may enter into contracts for the distribution of drugs and also provides additional valuable services to covered entities. The Health Resources and Services Administration (HRSA) administers the 340B Drug Pricing Program through its Office of Pharmacy Affairs. Apexus is in its third five-year contract term with HRSA. Product Overview Our cost-effective disposable products are available for various needs in the compounding pharmacy. Aseptic technique validation kits are available for low, medium, and high risk settings. The EDGE™ family of sensors and software provide wireless temperature tracking, mobile connectivity, and data-sharing for temperature-sensitive products. EDGE W-200 Wireless Temperature & Humidity Sensors Pharmacists are taking on an ever-growing responsibility of assuring that highly valued, temperature sensitive medications are received by the patient per the manufacturer’s temperature specifications. In a growing number of states, the requirements of the state board of pharmacy are also generating a need for effective monitoring and reporting tools. The EDGE W-200 sensor (with LoRaWAN™ technology) and EDGEConnect™ are innovative solutions that make facility monitoring simple. With EDGEConnect, the W-200 modules can be configured, managed and monitored. The wireless sensor modules automatically collect data from integrated or connected sensors and transmit the information to the cloud so it can be accessed remotely from anywhere.

EDGE M-300 Mobile Temperature Sensors

Temptime’s EDGE M-300 makes it easy for pharmacies to conduct accurate performance qualification (PQ) testing of an insulated shipper. EDGE M-300 is a wireless, Bluetooth-enabled mobile temperature sensor that can be placed in the package and will capture and transmit temperature data throughout transportation and storage. The M-300 can be monitored from a smartphone or tablet with the free, easy-to-use EDGEVue mobile app.

TransTracker® Shipment Indicators

TransTracker® temperature indicators are placed inside shipments of temperature-sensitive medications to monitor heat, freeze, or dual heatfreeze exposures during shipping, providing an easy-to-read visual signal of these events. TransTracker® visual temperature indicators are easyto-read and let patients see if a shipment has been exposed to potentially damaging temperatures.
