The company’s success is rooted in its community-centered philosophy, which is what led them to open their first in-store retail pharmacy more than 40 years ago. Fast forward to today, and you’ll find the vast majority of Giant Eagle’s 227 supermarkets now offer comprehensive pharmacy services. Their community focus is combined with an aggressive growth strategy and tradition of innovation. With this, the company’s leadership is always looking for new ways to add efficiency and better serve their customers. That’s why they decided to act when series of acquisitions put them in an ideal position to develop a long-term care (LTC) pharmacy to support assisted living facilities, nursing homes, and other similar healthcare providers. Recognizing that an LTC pharmacy had unique parameters and regulations to meet, Giant Eagle brought Senior Manager for Long-Term Care Pharmacy Operations, Michael Ashmore, on board to build out a scalable, future-proofed organization. Q Christine Lee-Wilson, PharmD, Owner, Professional Pharmacy Reverse Distribution Outsourcing Q. Can you give us a little background on why reverse distribution or drug returns are important for pharmacies? The main objective of reverse distribution or drug returns program is to minimize the risk of expired medications being dispensed and getting into the hands of patients accidentally. So above anything else, it’s a public health and patient safety issue. That’s what the regulations are about — and as pharmacy owner, I want to know we are in compliance at all times. That means my shelves are always clean, and everything in order no matter when the State Board of Pharmacy visits. There’s financial benefit. Pharmacies can earn rebates for drug returns that are processed correctly and within a specific time frame. Q. Why did you decide to outsource your drug returns program? For Professional Pharmacy, it comes down to resources and expertise. If we were to process our own returns, that would mean taking technicians away from what they do best. It’s difficult enough to find strong technicians today — they are the pharmacy’s most valuable resource. The last thing want to do is have them spending hours on tasks that aren’t central to day-to-day patient care. Drug returns also have to be processed in a specific way to be compliant, which is not something I’m thoroughly trained to do. Having an expert third-party partner manage the process gives me one less thing to worry about. It’s what they do every day, so I can be confident the process will be handled in both the most efficient and environmentally friendly manner. More than anything, working with a partner means the job will get done. For example, a 55,000-cards-per-month pharmacy serving mental health patients is producing 35,000 cards per month in DOSIS automation while providing different filling and labeling formats for their various customer facilities. Another partner pharmacy is filling 250,000 cards per month

for SNF patients with DOSIS Systems filling over 200,000 cards every month (dailies and cycles). DOSIS automatically prints color-coded med-pass information on every label in this system based on the prescription data. For a pharmacy filling 97,000 on-demand cards each month, the pre-pack manager feature extends the productivity of the DOSIS Systems by automatically restocking pre-pack card inventory after hours — DOSIS produces 58,000 cards each month, and the pharmacy is open less than 60 hours per week.

AUTOMATING LONG-TERM CARE PHARMACY SERVICES FROM THE GROUND UP

INTRODUCTION AND INTENDED OUTCOME

Established in the 1930s, Giant Eagle has grown from a single grocery store into a trusted supermarket brand with locations across Pennsylvania and Ohio. The company’s success is rooted in its community-centered philosophy, which is what led them to open their first in-store retail pharmacy more than 40 years ago. Fast forward to today, and you’ll find the vast majority of Giant Eagle’s 227 supermarkets now offer comprehensive pharmacy services.

Their community focus is combined with an aggressive growth strategy and tradition of innovation. With this, the company’s leadership is always looking for new ways to add efficiency and better serve their customers. That’s why they decided to act when a series of acquisitions put them in an ideal position to develop a long-term care (LTC) pharmacy to support assisted living facilities, nursing homes, and other similar healthcare providers. Recognizing that an LTC pharmacy had unique parameters and regulations to meet, Giant Eagle brought Senior Manager for Long-Term Care Pharmacy Operations, Michael Ashmore, on board to build out a scalable, future-proofed organization.

LAYING THE FOUNDATION

The first step, Ashmore says, was setting up a state-of-the-art system that could handle the initial prescription volume but that was also flexible enough to grow with them as they expanded their scope of services. “A critical part of designing the right system is deciding how you want to deliver medications to the facilities.”

While there are several options, Ashmore knew automated strip packaging technology with its multi-dose capabilities would be the best option for Giant Eagle and their customers. “What this allows an LTC pharmacy to do is sort a patient’s various medications into individual packages based on what dose needs to be taken and when,”

he explains. “This is especially helpful for elderly patients who are often on several different medications.” For nurses and staff in long-term care facilities, it simplifies and speeds the process of administering prescriptions while also improving medication compliance.

“Giant Eagle has always been a technologically-advanced company. We know what it takes to gain market efficiencies. Automated strip packaging is the way of the future and exactly where we need to be,” says Ashmore. With this decision, Ashmore began researching and evaluating what equipment and services different vendors across the country offered. “It’s a major investment — and when you’re filling prescriptions for large numbers of beds in long-term care facilities, you need a machine that will run flawlessly for eight or even 12 hours a day, every day.” More than that, he was looking for a true partner — one who was flexible and innovative and cared about Giant Eagle’s vision. Ashmore says he found everything he wanted with Noritsu.

THE NORITSU DIFFERENCE

“What struck me about Noritsu immediately is that they were genuinely interested in what we were trying to achieve,” Ashmore explains. “There was a real openness and commitment to helping us design the system in a way that would work for us.” That includes spending a great deal of time with Giant Eagle’s IT department, which was trying to write middleware to tie the Noritsu equipment into the retail system to manage the LTC pharmacy operations.

When it came time to install the equipment, Giant Eagle’s dedicated Noritsu sales representative was on-site with three technicians. “Once the automated packager was in place, Noritsu stayed for days helping us test and train to make sure everything worked and our staff was comfortable.” This was especially complicated because, at the time, they were also merging the retail software with the system. Noritsu collaborated with them for months as they worked through it. “I’m not sure another vendor would have done that for us,” Ashmore says.

The result? Giant Eagle formally opened its long-term care pharmacy in mid-July of 2021. “Today, we’re running the LTC operation in a closed pharmacy setting using Noritsu’s automated strip packager."

Aside from the fact that their equipment is so incredibly well built, Ashmore believes the two companies also have shared goals. “I feel Noritsu truly cares about patient outcomes just as we do at Giant Eagle,” he explains. “I’m all about patient care, especially geriatric patient care, because these people are part of the generation that got us all here. We owe them a debt of gratitude.” That’s why Giant Eagle’s primary objective from the start was to make administering meds faster and easier, so nurses and staff have more time to spend on care. “Noritsu helped us realize that goal.”

LOOKING AHEAD: AN EVOLVING PARTNERSHIP

Based on the success of the partnership, Ashmore is looking to leverage what they’ve done with Noritsu on the LTC pharmacy side and apply it to other areas. To begin, Giant Eagle is already building out a compliance pharmacy for the retail side. “The beauty of it is we already know the software our IT team and Noritsu built at the start of the project, while not ideal for LTC operations, works seamlessly for the retail pharmacy.”

Down the road, Ashmore also wants to explore opportunities of partnering with a nutraceutical provider and adding supplements to customers’ medication packages to increase convenience and adherence. Having previously helped with hospital discharges during his training, Ashmore also sees an application for using the Noritsu system to help patients synchronize their medications after a hospital stay. “Sometimes when a patient leaves the hospital, they are not always clear about how to combine medications they were taking previously with new medications,” he explains. “If we can play a role in simplifying that process with compliance packaging, I think we can help reduce readmissions — which can happen when medications aren’t taken properly after discharge.”

With Noritsu’s highly-scalable system, Ashmore knows he can do all he envisions and more. “Our central fill pharmacy is already filling close to 50,000 prescriptions daily to support our stores,” he says. As Giant Eagle adds new services — like compliance packaging within the retail pharmacy, that number will likely grow. “But I know some operations in the United Kingdom and Europe are running as many as 30 or 40 Noritsu packagers. That means no matter how much we grow — they’ll be able to keep pace.”

The Pyrosate® Kit is available in sensitivities of 0.03 EU/mL, 0.125 EU/mL, and 0.25 EU/mL, in both 10-pack and 30-pack formats. In addition to research and water testing, Pyrosate® is ideal for dialysis centers, compounding pharmacies, and pharmaceutical and medical device companies.

Performing the Test

The Pyrosate® Kit is a rapid gel-clot test that contains a 2λ* endotoxin tube (PPC) matched to the sample (SPL) tube for each sensitivity. This feature is unique to the Pyrosate assay. The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.

Reconstitution

Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance.

Q. Can you give us a little background on why reverse distribution or drug returns are important for pharmacies?

The main objective of a reverse distribution or drug returns program is to minimize the risk of expired medications being dispensed and getting into the hands of patients accidentally. So above anything else, it’s a public health and patient safety issue. That’s what the regulations are about — and as a pharmacy owner, I want to know we are in compliance at all times. That means my shelves are always clean, and everything is in order no matter when the State Board of Pharmacy visits. There’s also a financial benefit. Pharmacies can earn rebates for drug returns that are processed correctly and within a specific time frame.

Q. Why did you decide to outsource your drug returns program?

For Professional Pharmacy, it comes down to resources and expertise. If we were to process our own returns, that would mean taking technicians away from what they do best. It’s difficult enough to find strong technicians today — they are the pharmacy’s most valuable resource. The last thing I want to do is have them spending hours on tasks that aren’t central to day-to-day patient care.

Drug returns also have to be processed in a specific way to be compliant, which is not something I’m thoroughly trained to do. Having an expert third-party partner manage the process gives me one less thing to worry

about. It’s what they do every day, so I can be confident the process will be handled in both the most efficient and environmentally friendly manner.

More than anything, working with a partner means the job will get done. As I said, we’re all busy — and processing returns just isn’t a priority. I can see how it would be easy to say, “I’ll do it tomorrow” or “I’ll do it next week” — and then you run the risk of not processing returns in the appropriate time frame to get the full financial benefit. For me, having a reverse distributor cycle just makes sense.

Q. What was most important to you when you began looking for a drug returns outsourcing partner?

Finding a partner was easier for me because I had worked with the company I chose in my previous position. Still, I spoke with several other pharmacies using their services, and no one ever had a problem. So it was a trust factor as much as anything. I wanted someone who was accessible, local, and could deliver an end-to-end turnkey solution. We’ve worked with our partner for six years, and everything has been seamless.

Q. Can you walk me through your partner’s process of returning expired pharmaceuticals?

My representative comes in every six months and scans the prescription shelves, pulling any outdated medications we

haven’t already removed and stored for them to process. They work with both controlled and non-controlled substances. Controlled substances are broken down, and then everything is separated so they can be packaged properly for return to the respective manufacturers. My representative prepares all the documentation and arranges for pick up, typically the next day. When the courier arrives — everything is ready for them, and I have copies of each form to keep on file. They also take care of compounded expires for me, which is not always easy to find. At the end of it all, we get one check from them that lists credits from each manufacturer they’ve worked with as well as some smaller rebates directly from other manufacturers.

Q. Have you found any other benefits from working with a returns company that you didn’t consider when you began?

In some ways, I see them as a double-check for us. Since they only come in every six months — we pull expired medication from the shelves between visits. The fact that they come in and scan the shelves again gives me added peace of mind. In that process, they are able to give me a complete inventory of what’s being returned along with a credit estimate. With this information, I can see what drugs are expiring and in what quantities on an ongoing basis. This historical data helps me make better-informed business decisions about purchasing inventory.

They also provide a tremendous amount of guidance. For instance, they keep me up-to-date on regulations. In 2019, for example, the DEA changed Order Form for Schedule I and II Controlled Substances (DEA Form 222) — implementing a new single sheet format instead of the old triplicate forms. The next time my representative came in, they had the new forms and walked me through what I needed to do differently, what I needed to keep on file, and even where I should be keeping the forms. I didn’t have to worry about researching the change; they did it all for me.

Not all patients inject the same. Some may apply excessive force, which can increase the risk of injecting deeper than intended, increasing the risk of intramuscular (IM) injections.1** Shorter needles (i.e. 4 mm pen needles) may lower the risk of inadvertent IM injection, which is associated with unpredictable insulin absorption and variable effects on glucose.7 Educating patients on structured injection technique can help improve glycemic control.8^ A few steps to deliver structured injection technique training include using a new needle with every injection, rotating injection sites, and injecting within sites at least one finger width apart, and shifting to short needle lengths.9 Join the movement and help ensure your patients have enough pen needles or insulin syringes to use a new needle with each injection.

Both outcomes were considered statistically significant if the 95% confidence interval (CI) for the mean VAS score was either positive (XTW preferred) or negative (current PN preferred). Significant differences favoring XTW pen needles were seen for perceived thumb force and confidence that the full dose was delivered by 28.4 mm (95% CI, 23.7-33.2), and 24.4 mm (95% CI, 19.729.1), respectively; (all, P < 0.001).

^ 116 patients with diabetes on insulin were randomized to three intervention groups to assess the change from baseline in A1C at six months following structured injection technique training and changing to a shorter needle length (4 mm or 5 mm pen needle). Baseline A1C for all groups were similar (Mean: 8.5 – 8.8% [± 1.4 – 1.9%]).

Pharmacy Product Lines

Our Closed Rx Shelving (CRX) units feature slide in shelves with smooth side panels. This eliminates the need for expensive laminate side panels. The CRX clean white shelving is easy to assemble and ships flat to save on freight. Our shelving sizes will fit right in with your existing Classic Rx shelving.

Our Open Rx Shelving (ORX) features open backs and sides. The ORX three position shelves have two mounting tabs that allow the shelves to install flat, 12° down slope, or upside down to provide a flat surface. They come in 16" or 24" wide, and 7.5" or 12" deep. The shelves have an upturned lip on all sides, this allows you to reposition them without having to remove the product. Our shelving is packaged with care to avoid shipping damage.

We have a local cabinet maker building our pharmacy undercounter cabinets. If you have existing Lozier metal undercounter cabinets we can use a black recessed handle pull to match, the sizes are the same so they will fit right in with your existing cabinets. These cabinets come in a variety of units to meet your individual needs. Choose from narcotics cabinets, bottle dispenser units, open and closed storage cabinets, and more! Need a special size? No problem! No need to add filler panels, we’ll just make a cabinet a little wider to fill that space.

Franklin Eyewear evolved as Al Underwood drove from drugstore to drugstore in his home state of Mississippi in the early 1980s.
