BoxPicker Automated Storage and Retrieval by Swisslog BoxPicker is the innovative medication storage and retrieval solution providing secure storage for bulk medications and supplies with the capability of completing up to 200 transactions per hour. President:

Charlie Kegley Founded: 1915 Employees: >250 Phone: (800) 525-1841 Address: Swisslog Healthcare Solutions 10825 East 47th Avenue Denver, CO 80239 Website: www.swisslog.com Email: healthcare@swisslog.com

Company Background Swisslog Healthcare Solutions began in 1915 as The Grover Company, a provider of pneumatic tube systems for local Detroit businesses. Over the years the company has transformed into a leading provider of integrated logistics solutions for healthcare facilities around the world. Its comprehensive product portfolio ranges from pharmacy automation to medication management and materials transport systems. Swisslog’s solutions optimize customers’ production, logistics and distribution processes in order to increase flexibility, responsiveness and quality of care while minimizing logistics costs. Decades of experience in the development and implementation of integrated logistics solutions have led more than 2,000 hospitals throughout North America to install Swisslog products. The North American Division of Swisslog Healthcare Solutions is based in Denver, CO.

Product Overview BoxPicker is an automated pharmacy warehouse unlike anything else on the market today. It provides high-density storage and retrieval of medications and supplies in non-refrigerated, refrigerated and dual temperature models. The modular and scalable system can meet the needs of any pharmacy and offers a cost-effective alternative to vertical carousel storage. It’s ideal for pharmacies looking to improve picking times and accuracy as well as achieving perpetual inventory. This unique solution to medication storage and retrieval allows the pharmacy to pick medications at a rate up to 3 times faster than a carousel, averaging 5 picks per minute and up to 200 transactions an hour. BoxPicker utilizes scan in and out practice, and as a result provides extremely accurate inventory control while optimizing pharmacy workflow.

Orders to be filled are submitted directly to BoxPicker through the hospital’s IT system. After entering their password into the secured system, the technician uses the touch screen interface to fulfill the orders. The internal robotic arm quickly picks the necessary bin and delivers it to a secured drawer. Once in place, a light indicated which drawer the bin was delivered to and unlocks the drawer. Access is limited to a single drawer at a time in order to eliminate picking errors. The technician scans the barcoded item for verification and inventory tracking. For storage, items are scanned and loaded into their assigned bin. Once filled, the drawer is closed and the bin is automatically placed onto a shelf until retrieval is requested. BoxPicker can also be utilized as a clean pass-through for USP 797 Clean Room applications.

Features and Benefits • Secure and limited access to medications • Limits access to one medicine at a time • Picks more than 3 bins to 1 when compared to carousels • Software tracks inventory, access and interfaces directly with hospital information systems • Unique single-opening loading feature reduces wrong location placement • Refrigerated, non-refrigerated and dual temperature options • Modular for easy expansion

Vented SEVA Convenience Kit by IntraVena IntraVena’s cost-effective products offer improved safety and convenience in facilities where hazardous drugs are prepared and administered. CEO:

Gale H. Thorne Jr. Founded: 2007 Employees: <10 Phone: (801) 510-8733 Address: 1898 Chokecherry Drive, Bountiful, UT 84010 Website: www.intravenaonline.com

Company Background

IntraVena, LLC (“IntraVena”) was founded in 2007 by Gale H. Thorne Jr. and Gale H. Thorne Sr. IntraVena focused primarily in the area of hazardous drug safety. In 2008, IntraVena launched its first product called Sequence SDS. Sequence SDS proved useful when delivering BCG for the treatment of bladder cancer by delivering a saline flush behind the injection of the BCG solution. In Q4 2010, IntraVena launched Vented SEVA, SEVA Direct and SEVA Direct with Texium. SEVA is an acronym for Safety Enclosed Vial Adapter. SEVA is designed to minimize or eliminate accidental exposures to hazardous drugs. SEVA’s unique and simple approach helps prevent accidental exposure from a number of sources that are not addressed by any other known product on the market. These include: 1.) Residual hazardous drug found on vial exteriors 2.) Vapors released during use and disposal 3.) Vial breakage SEVA is part of IntraVena’s overall program for Arrival to Disposal safety. In third quarter 2011, IntraVena introduced an exciting new integral tethered alcohol swab feature that provides easier, more efficient product use.

Product Overview

Studies indicate a widespread occurrence of hazardous drug residue on the exterior surface of vials, and the mere act of spiking a vial can release hazardous drug aerosols outside the vial. To address these and other common hazardous drug transfer issues, IntraVena developed SEVA convenience kits. Vented SEVA is a low cost solution for closed system hazardous drug transfer. Each Vented SEVA convenience kit provides an enclosed vial adapter with tethered alcohol swab inside a medical grade, 4 mil, transparent, polyethylene bag. SEVA creates a barrier against hazardous drug on the exterior of the vial which may spread outside a laminar flow hood via touch contamination. The barrier also helps contain fluid and aerosol from release into a work environment.

exterior without compromising sterility of bag contents. An easy tug of the tether exposes the swab, thereby reducing steps in vial preparation and handling. Vented SEVA uses the bag as an air reservoir, permitting high volume gas transfer without a need to exchange air from the exterior environment. With the aid of a vent on the vial spike, diluent may be added to lyophilized powder for easy drug reconstitution. The SEVA bag is clear, permitting clinicians to easily see vial contents and labeling.

Additional Product Modules

Mini SEVA is a vial adapter for accessing hazardous drug vials requiring needle access. Similar to Vented SEVA, IntraVena’s Mini SEVA convenience kit offers a low cost, closed, needle based transfer system for hazardous drug requiring needle access. SEVA Direct provides transfer of hazardous drug from a vial directly to an IV bag without detaching a syringe from the vial access pathway. IntraVena’s SEVA Direct convenience kit offers a low cost, closed system hazardous drug transfer product, which delivers measureable doses of hazardous drug directly from a vial to an IV bag. SEVA Direct is even more cost effective when considering use of SEVA Direct for multidose vial applications. Fluids are delivered from the vial to a measuring syringe and there from to the IV bag through an integral stopcock.

i.Series® and Horizon Series™ Pharmacy Refrigerators from Helmer Helmer Refrigerators offer both a controlled temperature environment with superior temperature uniformity AND a dry storage environment excellent for storing boxes and other containers with our custom storage options. The reduced humidity level inside Helmer Pharmacy Refrigerators results in an environment that is well-suited for storage of water-permeable materials such as cardboard packaging, making these models ideal for use by Clinical and Research Laboratories, Pharmacies, Health Departments, and for any application that requires a drier storage environment.

Company Background Helmer is a manufacturer and worldwide distributor of cold chain storage and processing laboratory equipment. We have over 35 years of experience in providing high-quality temperature controlled environments. Our products are used in over 100 countries throughout the world. Reduced Humidity Storage Medical grade refrigerators offer a controlled temperature environment with superior temperature uniformity throughout the chamber. Unfortunately, refrigeration systems that achieve acceptable levels of uniformity also result in a humid storage environment. Typical scientific refrigerators run at humidity levels of 70% to 80%. For products stored in cardboard packaging, such as reagents, medications, and vaccines, the humid environment can create problems that lead to damp boxes that may become unreadable, contaminated, or fall apart. Helmer has developed a proprietary approach to refrigeration that allows us to reduce the humidity level of our pharmacy refrigerators while maintaining optimal temperature uniformity. • Relative humidity levels of 50% to 60%, with a 55% average relative humidity.

PCCA Lipoderm® Family of Transdermal Bases The industry’s most studied transdermal bases now proven to deliver four drugs simultaneously. CEO:

David Sparks Founded: 1981 Employees: 250+ Toll Free Phone: (800) 331-2498 Phone: (281) 933-6948 Fax: (800) 874-5760 Address: 9901 South Wilcrest Drive Houston, TX 77099 Website: www.pccarx.com

Company Background

Three decades ago, a physician encountered a patient who required an anti-nausea medication that was no longer commercially available. The physician challenged a Houston-area pharmacist to compound the medication. The prescription was a success, and the pharmacist saw that others in his field faced similar demands to help patients who required compounded medications. This network of pharmacists, united by a commitment to meet patient needs, was the foundation of PCCA (Professional Compounding Centers of America). Today, PCCA has become the independent compounding pharmacist’s complete resource for fine chemicals, equipment, devices, flavors, ACPEaccredited training and education, pharmacy software, marketing, business and pharmacy consulting assistance. Our membership includes more than 3,900 independent community pharmacists in the United States, Canada, Australia, and other countries around the world.

Product Overview

Lipoderm Transdermal Bases Available exclusively from PCCA, the Lipoderm family of transdermal bases has been independently tested, using the Franz Skin Finite Dose Model, and proven to deliver up to four drugs simultaneously through human skin in vitro. Lipoderm is an elegant alternative to traditional Pluronic Lecithin Organogel (PLO) and contains a proprietary liposomal component to increase the permeation of a variety of active pharmaceutical ingredients (APIs). Also tested for allergic reactions, no adverse reactions were reported in an extensive two-month study. The Lipoderm family also includes: •L ipoderm ActiveMax™ for higher API concentrations without the separation or liquification typically seen in other transdermals. • Anhydrous Lipoderm for APIs unstable in water or for patients who require a soy-free base, for example, autistic patients. •L ipoderm High Molecular Weight™ (HMW) for APIs with higher molecular weight making it ideal for HCG delivery or for autistic patients as it does not contain lecithin, phospholipids or eggs.

Other PCCA Exclusives Designed to Improve Your Patient Care and Help Your Bottom Line: •T 3/T4 Sodium Dilutions (1:1000) – These 1:1000 dilutions of Liothyronine Sodium USP and Levothyroxine Sodium USP help members meet the specialized needs of their patients and are extensively tested to ensure accurate potency and content uniformity, so members can save time and help reduce the risk of compounding errors. Each batch is manufactured to tighter tolerances than USP and available in a 25gm pack size for less frequent T3/T4 compounding. • Rapid Dissolve Tablet Technology – The Rapid Dissolve Tablet (RDT) is one of the most unique dosage forms available today. Active ingredients are mixed with the PCCA Rapid Dissolve Tablet Powder™ Base, sweetened and flavored with one of the many powdered flavors available. The tablet takes approximately 30 seconds to dissolve and allows members to differentiate themselves from the competition with a unique dosage form that’s ideal for pain management, hormone therapy, pediatric and hospice patients. • Unparalleled Quality Control and Quality Assurance – With more than 4,500 chemicals (active and non-active) in stock, PCCA’s priority is to deliver to members the highest quality products. From the moment a chemical arrives to the time it ships, it is tested and verified a minimum of 14 times. Only FDA-registered and GMPcertified manufacturers are used for the purchase of APIs, and PCCA does not solely rely upon the USP or manufacturer’s label to ensure the quality but does additional testing of APIs specific to actual formulations. • Broadest Range of Educational Options – PCCA provides the broadest range of in-person and web-based training and educational options. Our in-house, hands-on compounding course includes discussion of quality and safety procedures, legal issues, and marketing techniques. PCCA also conducts Accreditation Council for Pharmacy Education (ACPE) - accredited continuing education programs to provide pharmacists and physicians with opportunities to earn CE credit, practice compounding techniques according to USP <795> and <797> standards, learn effective methods of building patient and practitioner relationships, and network with colleagues. • Leading Pharmacy Consulting – With access to more than 30 consultant pharmacists, pharmaceutical chemists, and training personnel, no other company offers the experience and technical depth the way PCCA does. On average, PCCA’s Pharmacy Consulting Department answers more than 600 consulting calls a day from member pharmacists who have technical questions about preparing medications for patients. Members and consultants also have access to a database of more than 8,000 proprietary formulas, many of which have been pre-tested with PCCA’s fine chemicals and are continuously reviewed and updated. The Pharmacy Preceptor of the Year Award at MCPHS is presented to six preceptors from all campuses that have made an outstanding contribution to pharmacy experiential learning. One preceptor is selected from each campus in each of the following three categories: Community Pharmacy Practice, Institutional Practice, and a Massachusetts College of Pharmacy and Health Sciences Faculty Member. Each recipient is honored annually at an Awards Ceremony in May, and is acknowledged with a plaque. Throughout the current academic year, students on rotation are expected to complete a site/preceptor evaluation at the conclusion of each rotation and have the opportunity at that time to nominate their preceptor for the award. The votes are then tabulated by the Office of Experiential Education. Students evaluate the preceptor on the following criteria: Preceptor encourages questions and comments; Preceptor is genuinely interested in each student’s progress; Preceptor challenges each student’s academic and experiential skills; Preceptor is a good role model; Preceptor provides periodic feedback throughout the rotation; Preceptor provides a positive learning environment.

MedEx – Advanced Medication Tracking and Distribution Solutions by Aethon TM

Applying best practice principles from industries specializing in the management of distribution systems, MedEx provides hospital pharmacies with systems that bring advanced tracking and delivery technology to the hospital.

CEO:

Aldo Zini Founded: 2001 Employees: 65 Toll Free Phone: (866) 873-9796 Phone: (412) 322-2975 Address: 100 Business Center Drive Pittsburgh, PA 15205 Website: www.aethon.com Email: inquiries@aethon.com

Company Background Aethon is a leader in healthcare supply chain logistics, chain of custody and workflow solutions. At the core of Aethon’s pharmacy solution is MedExTM, which tracks the delivery and receipt of all medications whether they are delivered by courier, tube or other mobile robots. MedEx improves the throughput, management and accountability of the medication distribution process while ensuring regulatory compliance, improving patient safety and reducing costs related to missing medications.

These MedEx functions provide a real-time delivery status/ disposition allowing the pharmacy to address the common delivery problems of late, missing, divergent deliveries and overall productivity. MedEx is a web-based solution that gives both pharmacy and nursing access to real-time medication status information, while providing the pharmacy with the ability to record electronic signatures, archive data, and search for trends and divergence. The MedEx system is inherently flexible and can be applied without regard to the current or future pharmacy distribution model. The system is equally effective in fully decentralized, centralized, hybrid or satellite distribution systems. MedEx can be easily integrated with already acquired technology, such as unit-based cabinets and pneumatic systems.

Ordering Information To discuss how MedEx can help your pharmacy, please contact us at inquiries@aethon.com or (866) 873-9796 to reach one of our consultants.

Product Overview The MedEx product suite gives hospital pharmacies the ability to improve the efficiency and effectiveness of their internal and external distribution systems using medication tracking software, innovative technology and an efficient design approach. Utilizing barcodes, passive RFID and biometrics, MedEx tracks medications that are destined for patient-specific or unit-based storage locations. The MedEx medication prep and checking function allows the pharmacy to document who prepared the medication and which pharmacist checked the medication prior to dispensing. In addition to providing and archiving the data for quality purposes, this tool will allow the hospital pharmacy to be compliant with their State Board regulations requiring such documentation. The MedEx tracking function can work on its own, or can be combined with other sophisticated technologies, such as mobile robotics, secure carts/cabinets, nurse servers, pneumatic tube delivery and hand-held devices (e.g., smart phones), which provides an automated closed-loop delivery system for every type of delivery method and destination, bringing further efficiency and accountability to the medication delivery process. American Health Packaging Unit Dose Products AHP provides a full line of pre-packaged unit dose products to health systems nationwide to support BCMA and promote efficiency in hospital pharmacy operations. General Manager:

Rick Knight

Business Division of AmerisourceBergen Toll Free: Phone: Fax: Location: Website:

(800) 707-4621 (614) 492-8177 (614) 492-0448 Columbus, OH americanhealthpackaging.com

Product Overview Patient safety is a critical issue for health systems. As a result, many health systems seek bar coded products to dispense to patients. Bar coded products used in conjunction with bar code medication administration (BCMA) systems, automate the “five rights.” American Health Packaging supports this health systems’ priority with its growing line of pre-packaged unit dose products. Safety in Every Dose Bar coded to the dose level, our unit dose line has grown substantially in the last couple of years. In fact, we have the fastest growing line in the industry. And our products support all the major therapeutic groups. Recent product introductions include: Meclizine HCl, Morphine Sulfate ER Cll, Sotalol HCl and Bisoprolol Fumarate. A specific niche in unit dose that American Health Packaging targets is exclusive unit dose items - products not offered in a bar coded packaging format by other suppliers. About one-fourth of the line is comprised of exclusive items. Therefore, providers look to American Health Packaging when seeking niche or “difficult to find” unit dose items. Exclusive UD items include: Dronabinol SGC Clll, Nateglinide, Isosorbide Mononitrate ER, Vitamin D and Zidovudine. As an additional safety measure, our packages are color-coded to better distinguish them on the pharmacy shelf prior to dispensing. We also use “tall man” lettering (TML) to differentiate very similar product names.

Pharmacy Efficiency - Health systems are experts in the care of patients. Oftentimes, however, they’re not as well suited or staffed for packaging tasks. Packaging can become an added burden to a department already at capacity. Packaging in-house requires close supervision and a number of checkpoints to ensure it is done correctly. Cost Savings - Establishing a packaging area within the hospital pharmacy is costly. Staff must be hired and trained to complete the associated packaging tasks. Pre-packaged unit dose products minimize both fixed capital equipment costs and the associated increase in staff overhead costs to package in-house. Liability Management - Packaging in the hospital setting shifts the legal burden of accuracy in packaging and dispensing to hospital associates and dispensing caregivers. Pre-packaged product, however, maintains a legal burden to be correct as delivered to the hospital. Pre-packaged UD helps to protect pharmacy personnel and dispensing nurses from additional legal exposure.

About AHP A subsidiary company of AmerisourceBergen, American Health Packaging’s manufacturing facility is registered with the FDA and fully adheres to current Good Manufacturing Practices (cGMP). Industry veterans with nearly 100 years of collective experience lead the quality and manufacturing departments. American Health Packaging is licensed by the Drug Enforcement Administration (DEA) to package Schedule II to V controlled substances. Visit our website, americanhealthpackaging.com, for more information.

There are many benefits to AHP pre-packaged unit dose products:

Safety - Companies that specialize in pharmaceutical packaging are highly regulated and scrutinized by the FDA. cGMP compliance ensures that products are packaged in a regulated and safe environment. Packaging professionals are adept at ensuring the product’s lot number, NDC and expiration date are recorded correctly and legibly. W i n t e r 2 0 11 i s s u e

Coumadin® Management Software by Pharmafile Solutions Inc. Pharmafile’s Coumadin® Management Software saves time and improves patient safety by employing priority patient settings and electronic communications with patients and their healthcare providers. President:

Claude Farrier Founded: 2004 Employees: <50 Phone: (866) 944-0854 Address: 440-8250 Decarie Boulevard Montreal, Canada H4P 2P5 Website: www.pharma-file.com Email: cfarrier@pharma-file.com

Company Background

Pharmafile Solutions Inc is a technology company serving healthcare providers in North America and Asia since 2004. Our areas of expertise include: • Coumadin® dosing software • Patient education videos which cover the top 18 chronic diseases • Patient education videos which demostrate how to properly use devices such as metered-dose inhalers, Epipen®, Handihaler®, spacers, nasal sprays, nitro pumps etc. • Our Coumadin® software is used by more than 300 hospitals and clinics in North America, is EMR compatible, and HIPAA compliant.

Product Overview

Our software follows the logical steps of any Coumadin® clinic: Step 1: Access the list of patients who have an INR test due for a given day. •T his list provides all patients due that day, as well as any patients who had an appointment in the past but who did not come. This provides a safety net by making sure that patients who forgot their appointment are flagged. Step 2: PT/INR Value entered, either manually or electronically via HL7 interface. •A n entry can be done by staff/electronically, OR by the patient via the web if self-testing. •E ach patient can be given a unique access and any value provided is entered directly into the patient file to be processed. The patient then sees what the clinician determined as new dosage. Step 3: Dosage Adjustment •E asy-to-use adjustment tools can be used, and even automatic entries programmed by users. Protocols are available and custom ones can be added. Step 4: Communication of New Dosages •D aily dosage regiments in PDF format can be created and sent to the patient, caregivers or other healthcare providers directly from the application either by email or using our internal fax service. Not paper to print, fax to send, acknowledgements to wait for, all done digitally.

Key Product Benefits & Features • SMS/Text reminder service (reminds what tablet to take each day, as well as appointment dates) • Web-enabled and can be accessed from any computer which has an Internet connection. • HL7/EMR interface • Installation on hospital servers • Internal fax service (price per page sent) • Custom reports • Simple to learn, simple to use - One 90-minute training session is all which is needed to learn how to use the software. • Excellent support - Our experienced staff is using state-ofthe-art technologies to provide phone support and training. Using remote desktop management applications we can show different functionalities or train new staff easily. Testimonials “Our Coumadin clinic follows approximately 1000 patients. This program takes the guesswork out of calculating doses, making it safer for our patients. By automating much of the information it makes us more efficient, and shortens the time we need to care for patients. We particularly like the daily dosage sheets, which patients also like a lot as they find them easy to read and follow.’’

–– Charles Pollick, MD, LA Cardiology Associates

“This software cut our patient management time by more than 50%. It is intuitive and best of all it takes us no time to train new staff. The Pharmafile team is very professional, answers all our questions and constantly proposes solutions.’’

Company Background

The International Academy of Compounding Pharmacists (IACP) is a professional association founded in 1991 to protect, promote, and advance personalized medication solutions. The association represents more than 2,100 pharmacists and pharmacy technicians, licensed and regulated by their individual state boards of pharmacy and located throughout North America, South America, Europe, Australia and Asia. In addition, IACP currently represents 159,527 patients, physicians, nurse practitioners, and veterinarians through its ally grassroots organization, Patients & Professionals for Customized Care (P2C2). IACP is committed to ensuring the rights of physicians to prescribe, of pharmacists to prepare, and of patients to take personalized medication solutions that meet their unique, individual health needs. IACP offers a free Compounding Pharmacy Locator Service which can be accessed toll free at (800) 927-4227 or via IACP’s website at www.iacprx.org.

Overview

IACP has created a new tool to help hospital-based pharmacists, prescribers, and others who are trying to find means to alleviate the ever growing number of prescription products in short and long-term shortage situations identify compounding pharmacies with whom to work. IACP’s Compounding Pharmacy Assessment Questionnaire (CPAQ™) is designed to provide non-compounders with the necessary questions to ask a compounding pharmacy when considering working with them as a referral partner. IACP’s Compounding Pharmacy Assessment Questionnaire (CPAQ™) includes evaluation points in the following areas: • Regulatory compliance • Licensing – permits • Internal controls and quality assurance • Testing & verification • Site visits

When originally conceived by IACP President-Elect Scott Karolchyk, RPh, FIACP, the CPAQ was to be a springboard to facilitate conversations between hospital pharmacists and compounding pharmacists about meeting drug shortage needs. As IACP’s Hospital Shortage Workgroup – led by Scott Karolchyk, Lisa Ashworth, RPh, and Linda McElhiney, PharmD, RPh, FIACP, FASHP – discovered during the month-long creation and review of this document, there are similar assessment tools in the market. Unfortunately, those are not comprehensive and miss many of the critical points which should be discussed about pharmacy compounding. In some instances, those tools were created with sponsorship from companies with a vested interest in positioning themselves as the “solution” to hospital drug shortages. IACP believes every compounding pharmacy that wants to pursue a professional relationship with a hospital – or other institution, clinic or practice group – should be able to do so based upon an unbiased dialogue without a preconceived conclusion. CPAQ helps ask and answer the right questions at just the right time in the right way.

Testimonials “Drug shortages have been an ongoing problem for healthcare facilities and the number of drug products that are unavailable due to manufacturer backorders and discontinuation is on the rise,” says, Linda F. McElhiney, PharmD, RPh, FIACP, FASHP, compounding pharmacy operations coordinator, Indiana University Health. “These facilities are often trying to find substitutions for the drug products or alternative treatments and there is often a delay in the patients’ treatment. Compounding pharmacies may be able to prepare these medications using bulk active pharmaceutical ingredients; however, it may be difficult for the healthcare pharmacy administrators to know if the compounding pharmacy can provide quality compounded medications to meet their facilities’ needs because they are unfamiliar with the USP standards for compounding sterile and non-sterile preparations. The International Academy of Compounding Pharmacists (IACP) has developed a checklist of standards and criteria to assist the administrators in finding a suitable compounding pharmacy to meet these needs.” “IACP leadership recently participated in FDA’s conference on prescription drug shortages,” says Scott Karolchyk, R.Ph., FIACP, IACP president-elect. “While there were many good intentions expressed during the conference, there were no real solutions for now. Each day, pharmacists and physicians are grappling with making sure our patients get the medicines they need. What was clearly overlooked was how compounding pharmacists are providing solutions today as they work with U.S. hospitals, surgery centers, and practitioners to make those medicines available.”
