Thank You Preceptors! MCPHS University Thanks Timothy Aungst, PharmD Tim is an Assistant Professor at MCPHS University who maintains a Transitions of Care practice site at the University of Massachusetts, Ambulatory Care Clinic. Tim graduated in 2011 from Wilkes University Nesbit School of Pharmacy. Post-graduation he completed a PGY1 residency at St Lukes University Hospital in Bethlehem, PA and a Geriatric Fellowship at MCPHS University. Tim has published a book, authored multiple articles, and has made numerous presentations dealing primarily with Mobile Timothy Aungst, PharmD Medical Applications. His clinical focus is Congestive Heart Failure (CHF). Tim explains that the literature shows the positive impact of having a pharmacist on a multidisciplinary team to treat CHF. Pharmacists play an integral role in medication adherence by customizing therapy for each patient through careful review of the patient’s education level, potential language barriers, diet, cost of therapy, and timing of medications. It is also important to consider the patient’s lifestyle when making treatment decisions.

Tim views “Lifestyle Coaching” as an important facet to the transitions of care when working with CHF patients. Educating patients on diet and fluid restrictions is important. Students on rotation with Tim complete medication reconciliation on each new CHF discharge, ensuring that patients understand their treatment plans and individualized goals for managing CHF. Tim coaches his patients on medication therapies as well as appropriate diet and lifestyle modifications, which can impact the patient’s ability to achieve their overall treatment goals.

2013 Preceptor of the Year Award The Ernest Mario School of Pharmacy would like to thank Frank Iannarone III, for his hard work and dedication. On May 20, 2013, Frank Iannarone III was recognized for his outstanding service and commitment the students of the Ernest Mario School of Pharmacy by being awarded the 2013 the Ernest Mario School Preceptor of Year Award at Rutgers, The State University of New Jersey’s commencement ceremony in New Brunswick, NJ. As Preceptor of the Year, Mr. Iannarone was selected from hundreds of pharmacy preceptors Frank Iannarone III and acknowledged by the school for his dedication to developing and mentoring pharmacy students at his independently owned Madison Pharmacy. Having graduated from the School of Pharmacy at Rutgers University, Mr. Iannarone has seen the value of practice experience and has continued to be a tremendous advocate of student development through community outreach programs and one-on-one interactions with his patients. During the four or five week rotation segments, students are given opportunities to apply information learned in school and gain hands-on experience through patient interactions and prescription processing. Students have created and delivered a variety of programs over the year covering specific disease states such as diabetes and high blood pressure to more general programs on medication disposal and Medicare prescription program information. In addition to training students at his pharmacy and in the community, Mr. Iannarone has also supported student learning during student organization meetings at the school by providing his insights on the value of owning a pharmacy. Based on his commitment to the students of Rutgers University and excellence in the profession of pharmacy, Mr. Iannarone was awarded the prestigious designation of Preceptor of the Year for 2013.

In addition to FDA licensure, the Pyrosate® Kit is now available in sensitivities of 0.03 EU/mL, 0.125 EU/mL and 0.25 EU/mL, in both 10 pack and 30 pack formats. With the introduction of this licensed version, in addition to research and water testing, Pyrosate® is ideal for dialysis centers, compounding pharmacies, pharmaceutical and medical device companies.

Performing the Test

The Pyrosate® kit is a rapid gel-clot test that contains a 2λ* endotoxin tube (PPC) matched to the sample (SPL) tube for each sensitivity. This feature is unique to the Pyrosate® assay. The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate® is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.

Reconstitution Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance. Stability Pyrosate® is stable at room temperature and does not require refrigeration for shipping or storage. that when followed confirm FDA acceptable sterility assurance levels for components being placed into the direct compounding zone of the isolator. The Sterility Assurance Levels That are Validated Include:

•  The standard MIC Isolator configuration incorporates procedural spray down of components in the static airlock. Providing a defined area, decontamination solution, plus entry and exit protocols results in exceeding published data contamination rates expected in a USP <797> compliant clean room with open primary engineering controls. •  The standard MIC configuration can be modified to incorporate an automated decontamination airlock. The vapor phase hydrogen peroxide is an effective decontaminate with a broad range of capabilities that leaves no residue. The validation of this onsite modification demonstrates a sterility assurance level that meets the FDA expectation for sterility assurance (10-4) of product produced by aseptic processing in approximately two minutes. •  The MIC-EDU System is our most advanced closed system isolator. It uses batch decontamination technology that allows for continuous compounding with no downtime. The vapor phase hydrogen peroxide provides a repeatable method of reaching all interior surfaces contained within the closed system isolator. This unit has been validated to show sterility assurance levels at 10-5.

Drug Shortages

Every healthcare institution is sure to feel the effects of shortages in the coming months; in fact, the amount of shortages has more than tripled since 2005. Over 75% of these shortages were for sterile injectable medications.
