Contents Case Study Treating Acute Uncomplicated Influenza with a Novel, One-Dose Intravenous Neuraminidase Inhibitor THE CHALLENGE: During the influenza season, which typically peaks in the winter months, doctors’ offices and emergency rooms see a great influx in patients presenting with influenza-like illness. Typically, the only available treatment option is to prescribe a multi-dose antiviral neuraminidase inhibitor which can alleviate the symptoms of influenza (headache, myalgias, fever, fatigue, cough, sore throat, and nasal congestion), on average, about a day sooner than standard of care. Many people are able to quickly recuperate from influenza; however, some segments of the population are at a higher risk of developing serious complications from the flu, leading to significant morbidity, hospitalizations, and even death.
THE CASE: A 62-year-old male smoker with chronic bronchitis who was doing well prior to “catching a virus” presents with 12-hour history of fever, myalgia, sore throat, cough, and nausea. He states that he was feeling well the day before, but upon awakening that morning he “felt hot, had a cough, was nauseated and felt like every single muscle in his body was achy”. He called his doctor’s office but they were closed and the answering service advised him to go directly to the emergency department of his local hospital. Upon presentation to the emergency department, the patient denied any chest pain, dizziness or shortness of breath and reported that he had not been vaccinated this year for the flu. He states that he had the flu a couple years ago and was given an oral medication but admits he started feeling better a couple days after getting the prescription and did not complete the treatment course. Patient’s COPD is controlled with medication, and he does not require supplementary oxygen. His exam was notable for high fever and muscle aches and his chest X-ray was consistent with chronic COPD but showed no evidence of pneumonia.

Important Safety Information Rapivab™ (peramivir injection) is indicated for the treatment of acute uncomplicated influenza in patients 18 years and older who have been symptomatic for no more than 2 days. • Efficacy of Rapivab was based on clinical trials in which the predominant influenza virus type was influenza A; a limited number of subjects infected with influenza B virus were enrolled. • Influenza viruses change over time. Emergence of resistance substitutions could decrease drug effectiveness. Other factors (for example, changes in viral virulence) might also diminish clinical benefit of antiviral drugs. Prescribers should consider available information on influenza drug susceptibility patterns and treatment effects when deciding whether to use Rapivab. • Efficacy could not be established in patients with serious influenza requiring hospitalization.

Product Overview
Many central pharmacies have adopted medical-grade refrigeration to ensure medications are stored within specified temperature ranges, reducing the risk of lost product and supporting regulatory compliance. These same principles apply to medications and vaccines stored on patient floors and in clinics. There are significant differences between medical-grade and non-medical-grade refrigerators and freezers in terms of their ability to maintain the required temperature ranges. Nonmedical grade units are not designed to maintain uniformity throughout the storage space. While medications stored in some locations may stay within the recommended range, in other locations the temperature may routinely fluctuate outside of the acceptable limits. These temperature excursions are likely to go undetected because not all locations within the unit can be monitored, increasing the risk of administering compromised medications to patients. Medical-grade refrigerators and freezers are designed and validated to maintain stringent temperature ranges. They utilize powerful, forcedair refrigeration systems. This enables uniform temperatures throughout the cabinet and quick recovery after door openings. These units are also designed for reliable performance, which minimizes disruptions and eases the strain of ongoing repair costs. In addition, they reduce the financial risk of lost product, which can easily amount to thousands of dollars. They also help facilities maintain compliance with The Joint Commission and state Board of Health regulations.
In order to protect public health, the Centers for Disease Control and Prevention (CDC) developed recommendations for vaccine storage and handling. Failure to properly manage vaccine inventories can reduce potency, leading to ineffective vaccinations and poor protection against preventable diseases. The CDC’s Vaccine Storage and Handling Toolkit states that the storage of any vaccine in a dormitory-style refrigerator is not recommended under any circumstances. For facilities that participate in the Vaccines for Children (VFC) program, dorm-style refrigerators are not allowed. The CDC developed these recommendations to prevent the administration of mishandled vaccines, and to protect against the financial risk of lost vaccines. The same principles apply when considering medications stored on patient floors and clinics. Helmer Scientific routinely conducts temperature mapping studies to verify the uniformity of laboratory and pharmacy refrigerators. To assess uniformity throughout the storage space, a Helmer Scientific medical-grade refrigerator (undercounter model iLR105) was evaluated by measuring probe bottle temperatures at multiple locations inside the cabinet. The evaluation showed that the refrigerator, configured with a set point of 4ºC, maintained tight uniformity across all storage locations. Probe bottle temperatures never approached the lower or upper limits of the acceptable range (2ºC to 8ºC). The results of this evaluation indicate that the Helmer Scientific unit maintains the required temperature range for refrigerated medications and vaccines. When selecting refrigeration for sensitive vaccine or medication storage, true medicalgrade refrigerators support patient safety by protecting these expensive and life-saving products.

Case Study Treating Acute Uncomplicated Influenza with a Novel, One-Dose Intravenous Neuraminidase Inhibitor THE CHALLENGE: During the influenza season, which typically peaks in the winter months, doctors’ offices and emergency rooms see a great influx in patients presenting with influenza-like illness. Typically, the only available treatment option is to prescribe a multi-dose antiviral neuraminidase inhibitor which can alleviate the symptoms of influenza (headache, myalgias, fever, fatigue, cough, sore throat, and nasal congestion), on average, about a day sooner than standard of care. Many people are able to quickly recuperate from influenza; however, some segments of the population are at a higher risk of developing serious complications from the flu, leading to significant morbidity, hospitalizations, and even death.
THE CASE: A 62-year-old male smoker with chronic bronchitis who was doing well prior to “catching a virus” presents with 12-hour history of fever, myalgia, sore throat, cough, and nausea. He states that he was feeling well the day before, but upon awakening that morning he “felt hot, had a cough, was nauseated and felt like every single muscle in his body was achy”. He called his doctor’s office but they were closed and the answering service advised him to go directly to the emergency department of his local hospital. Upon presentation to the emergency department, the patient denied any chest pain, dizziness or shortness of breath and reported that he had not been vaccinated this year for the flu. He states that he had the flu a couple years ago and was given an oral medication but admits he started feeling better a couple days after getting the prescription and did not complete the treatment course. Patient’s COPD is controlled with medication, and he does not require supplementary oxygen. His exam was notable for high fever and muscle aches and his chest X-ray was consistent with chronic COPD but showed no evidence of pneumonia. Important Safety Information Rapivab™ (peramivir injection) is indicated for the treatment of acute uncomplicated influenza in patients 18 years and older who have been symptomatic for no more than 2 days. • Efficacy of Rapivab was based on clinical trials in which the predominant influenza virus type was influenza A; a limited number of subjects infected with influenza B virus were enrolled. • Influenza viruses change over time. Emergence of resistance substitutions could decrease drug effectiveness. Other factors (for example, changes in viral virulence) might also diminish clinical benefit of antiviral drugs. Prescribers should consider available information on influenza drug susceptibility patterns and treatment effects when deciding whether to use Rapivab. • Efficacy could not be established in patients with serious influenza requiring hospitalization.

THE RESULTS: Patient was treated with one dose of Rapivab™ (peramivir injection), the complete treatment course for uncomplicated flu; this was deemed preferable due to his nausea and previous inability to comply with a multi-day oral antiviral regimen. The patient was also given IV hydration since he was not eating or drinking. The patient was then observed for the next ten hours and began to feel better, with diminished fever, and improvement in fatigue. His vital signs were stable, po intake was initiated, and he was able to ambulate. He was discharged from the emergency department’s observation unit with instructions to call his outpatient physician should symptoms not resolve completely over the next few days.
THE CONCLUSION: The CDC and IDSA recommend that among outpatients presenting with influenza-like illness during flu season, early (within the first 48 hours of onset of symptoms) antiviral treatment with a neuraminidase inhibitor is recommended for all persons who are at higher risk for influenza complications because of age or underlying medical conditions. Some selected high risk conditions include: • persons with chronic pulmonary (including asthma), cardiovascular (except hypertension alone), renal, hepatic, hematological (including sickle cell disease), metabolic disorders (including diabetes mellitus) or neurologic and neurodevelopment conditions (including disorders of the brain, spinal cord, peripheral nerve, and muscle such as cerebral palsy, epilepsy [seizure disorders], stroke, intellectual disability [mental retardation], moderate to severe developmental delay, muscular dystrophy, or spinal cord injury); • adults aged 65 years and older; • persons with immunosuppression, including that caused by medications or by HIV infection; • American Indians/Alaska Natives; • persons who are morbidly obese (ie, BMI is 40 or greater); and • residents of nursing homes and other chronic-care facilities. Important Safety Information Contraindications None Warnings and Precautions • Rare cases of serious skin reactions, including Stevens-Johnson syndrome and erythema multiforme have occurred with Rapivab. Appropriate treatment should be instituted if a serious skin reaction occurs or is suspected. • Patients with influenza may be at an increased risk of hallucinations, delirium and abnormal behavior early in their illness. There have been post-marketing reports (from Japan) of delirium and abnormal behavior leading to injury in patients with influenza who were receiving neuraminidase inhibitors, including Rapivab. Because these events were reported voluntarily during clinical practice, estimates of frequency cannot be made, but they appear to be uncommon. These events were reported primarily among pediatric patients. The contribution of RAPIVAB to these events has not been established. Patients with influenza should be closely monitored for signs of abnormal behavior. • Serious bacterial infections may begin with influenza-like symptoms or may coexist with or occur as complications during the course of influenza. Rapivab has not been shown to prevent such complications.

Neuraminidase inhibitors are the most effective treatment for acute uncomplicated influenza when given within 48 hours of onset of symptoms. Rapivab™, specifically, has been clinically shown to reduce the total time to alleviation of symptoms of influenza by a median of 21.5 hours sooner and resolution of fever a median of 12 hours sooner when compared to placebo. Also, the safety profile of RAPIVAB is comparable to placebo. The single dose, intravenous delivery of Rapivab makes it optimal for patients presenting to the emergency room with symptoms precluding them from taking oral antiviral therapy or for patients requiring IV hydration. Additionally, a high percentage of patients do not adhere to oral, multi-day neuraminidase inhibitor treatment making a one dose option like Rapivab™ the most fitting choice. Important Safety Information Adverse Reactions The most common adverse reaction was diarrhea (8% Rapivab vs 7% placebo). Lab abnormalities (incidence ≥ 2%) occurring more commonly with Rapivab than placebo were elevated ALT 2.5 times the upper limit of normal (3% vs 2%), elevated serum glucose greater than 160 mg/dL (5% vs 3%), elevated CPK at least 6 times the upper limit of normal (4% vs 2%) and neutrophils less than 1.0 x 109/L (8% vs 6%). Concurrent use with Live Attenuated Influenza Vaccine Antiviral drugs may inhibit viral replication of a live attenuated influenza vaccine (LAIV). The concurrent use of Rapivab with LAIV intranasal has not been evaluated. Because of the potential for interference between these two products, avoid use of LAIV within 2 weeks before or 48 hours after administration of Rapivab unless medically indicated. PRIMARY INTENDED OUTCOME(s) Our goal was to reduce the error rate during IV infusion and create standardization of line tracing protocol on infusion pumps by implementing line tracing labels.
CASE STUDY THE CHALLENGE As one of the first hospitals in the nation to integrate their smart pumps with EHR (Epic), Lancaster General Hospital was using stickers, masking tape, and other variations to handwrite and label IV lines and infusion pumps. Using more than 900 pumps (2-6 channels/ pump), this caused confusion, creating the need for LGH to reduce their error rate when pushing and switching lines. Medi-Dose Line Tracing Labels
“Line trace itself is never something that technology is going to be able to do,” says Tina Suess, Manager of Medication Safety for Lancaster General Hospital. “The technology cannot make sure that the tubing is placed through the correct cassette or channel in the pump.”

“The line trace labels actually help us draw attention to an area in the process where technology can exist, but we still need the human to do the right thing which is follow the tubing from the bag through the pump to the patient,” Suess says. After identifying a number of errors within their internal error report, Lancaster General Hospital realized with proper line trace, errors could be prevented.

We established a line trace process, which began with the question, “how are we going to label the line?” Lancaster General Hospital consulted Medi-Dose, where the two helped establish a label standard concept. At LGH, line trace needs to be done a minimum of every eight hours. Protocol dictates after the nurse hangs the bag in the patient room, the label is placed on the bag, at the top of the line, as well as the distal end of the line. The nurse’s hand should not leave the line during the trace process, which allows for rapid identification of the correct line. Medi-Dose Line Tracing Labels are flexible, offering color-coded or pre-printed customized options. Effectively, the risk of misreading a label due to penmanship is eliminated. Prusch continues, “after putting in several technologies, mostly manual-type processes, the [Medi-Dose Line Tracing Labels] were part of the overall rate in dropping that number.”
THE CONCLUSION Medi-Dose Line Tracing Labels is the tool that allowed Lancaster General Hospital to standardize their line labeling process, according to Suess. While looking to drive consistent workflow, the line label facilitates an easy-to-use fashion for the nurse. “We’re very interdisciplinary here. The pharmacist facilitates those labels for nursing. The nurse doesn’t have to go run someplace else to get the labels. So our approach that the product actually gets dispensed with the bag when the nurse pulls it from Pyxis, is actually saving the nurse a few footsteps and we always appreciate that,” said Suess. Pharmacist oversight is now protocol for adjustments made by the nurse, including pharmacy validation and confirmation. “Line trace is something technology will never be able to do. Technology can’t make sure tubing is placed through the correct channel of a pump. [Medi-Dose] Line Tracing Labels helped us draw attention to area where technology exists but we still need humans to practice following the line from pump to bag to human,” Suess said.

Performing the Test
The Pyrosate® kit is a rapid gel-clot test that contains a 2λ* endotoxin tube (PPC) matched to the sample (SPL) tube for each sensitivity. This feature is unique to the Pyrosate assay. The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.
Reconstitution Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance. Stability Pyrosate® is stable at room temperature and does not require refrigeration for shipping or storage. MCPHS University Thanks Saumil J. Patel., PharmD. Saumil J. Patel graduated from MCPHS University with his PharmD degree in 2007 and has developed a successful Medication Therapy Management (MTM) service for Walgreens in Roslindale, Massachusetts. He is currently the District MTM Leader for Walgreens in the Boston district. He is energetic and passionate about providing MTM throughout his tenure at Walgreens and has been an advocate of changing our culture to relational from transactional. The formation of Medication Therapy Management has been the niche of his practice. He realizes as the pharmacy profession continues to evolve, and as pharmacists keep pushing to become recognized as health care providers, MTM will very likely prove to be at the foundation of such growth. Saumil is offering MCPHS University students, both IPPE and APPE, the opportunity to be involved in MTM delivery while completing community rotations. He also offers a specialty APPE elective rotation in MTM and depending on the ability of the APPE student, to be involved in the completion of the entire MTM case. 2015 Preceptor of the Year Rosina M. Stamati The Preceptor of the Year award is bestowed upon one exceptional volunteer faculty preceptor. Recipients are superlative role models who exhibit a genuine interest in teaching, developing trusting relationships, fostering respect, and encouraging learning. Preceptors are selected through nominations received from students and by having outstanding evaluations. The 2015 Preceptor of the Year, Rosina M. Stamati is Clinical Coordinator, Pharmacy Program; Director of the PGY-1 Pharmacy Practice and PGY-2 Geriatrics Pharmacy Practice Residency Programs; and Coordinator of the Pharmacy Palliative Care Fellowship Program at the James J. Peters Veterans Affairs Medical Center in Bronx, NY. She has been a Certified Geriatric Pharmacist since 1997 and a boardcertified Pharmacotherapy Specialist since 2004. Ms. Stamati has presented national and local education programs in geriatrics and medication-related issues for pharmacists, physicians, nurses, and other health care providers. She has also served as a poster reviewer for the American Society of Consultant Pharmacists and a journal reviewer for Gerontology and Geriatrics Education. Ms. Stamati has been a preceptor for LIU Pharmacy students in both IPPEs and APPEs for over 20 years. Dr. Cauthon attended St. Louis College of Pharmacy where she received her Pharm D. Following graduation she completed her residency at the Madison, Wisconsin William S. Middleton VA. Dr. Cauthon was at Midwestern University College of Pharmacy in Glendale, Arizona for seven years prior to joining the faculty at the University of the Incarnate Word in 2012. Dr. Cauthon’s courses include teaching patient assessment in the second year and coordinating pharmacotherapy seminar, a capstone course, in the fourth year. She is a Certified Geriatric Pharmacist and is serving as a Board of Commissioners member. This may be accomplished by retaking the examination or by the Professional Development Program, with continuing education of 75 credit hours over the five-year period. ASHP is one of the approved Professional Development Providers for CCGP. Benefits of CGP Credential With the rapid growth in numbers of older adults in hospitals, and changes in Medicare reimbursement, the need for hospital pharmacists with the Certified Geriatric Pharmacist credential is growing rapidly. Hospitals need pharmacists who understand the unique medication-related needs of the elderly, including Beers criteria, geriatric syndromes, prescribing cascades, and issues related to transitions of care and medication reconciliation.
Hospitals also need pharmacists to participate in geriatric interprofessional teams, such as ACE, HELP, and NICHE programs. A pharmacist with the CGP credential should be a part of every hospital pharmacy team.
Self-Assessment Examination CCGP offers an online practice test, or Self-Assessment Examination, with the same number of items as the actual examination. Many candidates like to use this SAE as a way to help prepare for the actual examination. In addition, CCGP has compiled a list of resources that some candidates have found useful in exam preparation. The list is on the CCGP website at www.ccgp.org/exam/prepare. Students and Residents Two years of experience as a pharmacist is required for eligibility to take the CGP examination. One year of residency counts as one year of practice experience. Pricing Information The fee to register for the initial examination is $600, and the fee for periodic recertification is $400, either by exam or by CE. In addition, a certification administration fee of $250 per five years is required, payable after meeting requirements for certification or recertification. n Exchange Trays H+H System exchange trays give hospital personnel the ability to standardize their medication storage in all medical carts and drawers. Standard heights include 1 3/8", 2 5/16", 4", and 5". Standard size trays are available but custom sizes can be produced for any quantities. Flexible dividers on the inside of the tray allow the end user to compartmentalize the tray based on their needs. If the need for more storage space arises for a particular item, just slide the divider and a larger storage compartment
is now created. Trays can be produced in different colors which allow it to be quickly identified for different hospital departments. n FlexModul The FlexModul product line is designed to give the user ideal storage for bulk goods. Our side panels can be mounted inside existing casework and allows the trays to be hinged 10 or 30 degrees for viewing while still being stored horizontally. This allows for the storage to be optimized in the existing space. Trays are available in 2", 4", and 8" heights in either gray ABS plastic or clear polycarbonate plastic. FlexModul dividers can be added to the inside of trays to further divide the space. Labeling options are also available so the items are easily and quickly identified. n FlexShelf The newest innovative product line offered is our H+H System FlexShelf. FlexShelf is an expandable open rack storage system that allows for products to be stored in a viewable manner. When accompanied with the FlexModul trays, the system can be adjusted as your storage needs change. Accessories such as fixed shelves, pull-out shelves, pull-out drawers, and solid worktops allow for numerous configurations and the perfect solution for any need. Additional storage columns can be added at any time, as needed. For a mobile storage option, castors can be used instead of the adjustable leveling feet. This can be used as a distribution cart when restocking exchange trays on your medical carts.

The physical inventory process is seamless and unobtrusive as team leaders strategically place the team in locations throughout the pharmacy to ensure accuracy and efficiency. Before completion of an inventory service, our teams actively work with finance and/or independent auditors while on site to validate the accuracy of the inventory data collected. Beyond the physical inventory, the data is then received, analyzed, and formatted into a report that includes all pertinent information to allow for accurate and precise analysis of the inventory data. Once all areas are completed, the raw inventory data is then transmitted to an independent team for review and analysis. n What to Expect Post Inventory Day Items will be priced according to GPO/Wholesaler Cost Data and Direct Purchase/Secondary Wholesaler Cost Data. Upon review and analysis the data will be adjusted, compiled, and valuated. Finally, the inventory data will be delivered electronically as an Excel file. n What to Expect After Receipt of Inventory Data If it’s identified that adjustments are needed we have an easy stepby-step way to apply the indicated changes. When submitted the adjustments can be made and returned within 24 to 48 hours. Unlike other companies that do not allow adjustment to the inventory data we will make adjustments to the data as requested to ensure the final value is a true representation of the on-hand inventory.
