My dog will be current on their vaccinations (DHPP, Bordetella, and Rabies) and copies will be kept on file at Virginia Woof, with a completed approved application. I will only bring my dog to daycare when they are healthy and display ongoing temperament characteristics that are conductive to a fun and safe enviornment for all. I will not bring my dog if they show signs of illness (vomiting, diarrhea, coughing, limping, ect.) Initials

All About Virginia Woof DAYCARE INFORMATION
• No calls/no shows for scheduled daycare will be charged. • Payment is due upon pickup or in advance. • Dogs must be dropped off or picked up during posted business hours. Unless prior arrangements have been made, a late fee will be assessed or your dog will be boarded comfortably overnight and boarding fees will apply. Late fee is $5 for every 15 minutes. • We often fill to capacity during the day so it is recommended that you call to reserve your spot. • All dogs must have a quick-release buckle type collar to be worn in the play area.
