Your Cornwall Introduction from Cornwall Adult Care & Support’s Director

From 1 April 2009 the Directorate of Adult Care and Support became part of the new Cornwall Council. The new Council brought together services that had previously been provided by the six District Councils and the County Council, into a new Unitary Council. Social Care services to adults continues to be provided as before, but has a slightly different name. The Directorate of Adult Care and Support is independent of the organisations listed in this Directory, but we are pleased to distribute it widely. We are able to offer a Community Care assessment to anybody who requests it, whether or not they pay for their service privately and we can help them access services, information or support. We are also keen to assess the needs of carers whether or not they pay for their service privately and we can help them access services, information or support. We are also keen to assess the needs of carers whether or not the people for whom they care receive a service from us. Our assessments look at the whole person and focus on what they want to achieve. We will encourage and assist people to contribute to their assessment, which means talking through with them how to best meet their needs and choices. To determine a person’s eligibility we use a framework called ‘Fair Access to Care Services’ (FACs). This is a national requirement. We use it to determine whether people’s needs are ‘critical’, ‘substantial’, ‘moderate’ or ‘low’. At the moment we offer funded services to people with ‘critical’ or ‘substantial’ needs only. However, if we are unable to offer funded services, we endeavour to help people find other ways of achieving outcomes by redirecting them to other organisations, by helping them to maximise their own resources or by giving them advice whether they are publicly or privately funded. We want to encourage you to play a bigger part in deciding and planning how you can direct your own care choices and services. We aim to offer a service to promote independence to the widest possible group of people in Cornwall whilst ensuring proper use of the Council’s resources. If you have concerns about care or support or need advice, please talk to us.
In the coming years the care sector faces some of the most difficult challenges of recent times. Changes in the commissioning of social care provision by the County, the introduction of Independent Budgets and Direct Payments coupled with the volatile economic climate will offer both opportunities and challenges to the industry as a whole and both the Charitable and the Independent sectors will need to respond with vigour and enthusiasm to meet them. Whilst, as individual providers our concerns over these changes and our fears for the future of the Care sector are frequently ignored, collectively our message is heard by those who can make changes that affect the sustainability and quality of care provision. Cornwall Partners in Care as the voice of the independent sector. Over the past year CPIC has increased its membership and now offers effective and proactive representation for Care providers throughout the county. CPIC remains committed to the continued improvement of quality Care services and the development of our valued workforce. CPIC recognises that, in order for this to be achieved, there has to be sustained investment by the members’ individual companies as well as by the County itself. CPIC endeavours to convey the importance of this front line service to both the public and the statutory bodies. By working in partnership with Cornwall’s Department of Adult Care and Support, the Primary Care Trust and the voluntary sector we can, as an industry, continue to provide a high quality service for the people of this county and ensure worthwhile employment for our staff.

0300 1234 131 Adult Care and Support (ACS) is a single, countywide generic access service that deals with all referrals, requests, and enquiries; the vast majority of which are currently received by telephone. A multi disciplinary and professional team is there to help identify how enquiries can be resolved at the point of referral. This include an initial stage of information gathering and an assessment of needs and risk using the Fair Access to Care Services eligibility screening (see blue box). The access service can also give financial information. In situations where referrals do not appear to meet eligibility (currently set at Critical and Substantial) the service will offer to sign-post to other community based services, including the voluntary and independent sectors, and provide any available support. Once it is identified that a person requires a full assessment or immediate onward referral to a specialist service or worker, the referral will be allocated to the appropriate fieldwork team working with older people, people with physical disability or people with learning disability. The intention of ACS is to quickly and appropriately resolve more than 40% of the referrals and enquiries it currently receives at the point of first contact. More information can be found in ACS leaflets, inparticular their revised leaflet “Looking for Social Care in Cornwall”. This is for people who are not eligible for local authority services, or who wish to pay for their own services. Copies or any leaflets can be obtained through your local ACS office or by calling 0300 1234 131. Assessing Your Needs Adult Care and Support (ACS) can ask for an assessment. This means that someone from ACS may visit you, usually at home or in hospital, once your referral has been subject to initial receipt by the access service. When ACS does come out to meet with you and any family members or representatives you may wish to have present, they will discuss your Page situation, and work out with Strip2 15/5/07 11:59 1 you what help would be appropriate.
ACS use eligibility criteria laid down by the Government to assess people wishing to receive services. This assessment process is called Fair Access to Care (FACS, see below), and its aim is to focus services on those people in greatest need. Fair Access to Care Services (FACS) 4 Bands: Critical, Substantial, Moderate, Low 5 Criteria: • Autonomy and freedom to make choices • Health and safety, including freedom from harm, abuse and neglect • The ability to manage personal and other daily routines • Involvement in family and wider community life • Carer’s circumstances
Should I Ask For An Assessment? Adult Care and Support will assess your need for services if you are an adult with:• physical disability, or a long term illness • sight or hearing loss • learning disability • mental health problem • an older person and frail • a carer You can ask a friend, relative or your GP to contact us on your behalf if you are in hospital, the nursing staff can arrange for Adult Social Care to contact you, either during your stay in hospital, or on your return home. For more information: Call Cornwall Council on 01872 322532 and ask for ‘Assessing Your Needs’ leaflet, ref: ASC14 With all the services Adult Care Support provide, they will: • assess your needs • give you as much choice and independence as they can • listen to what you say about the service you want, and do their best to provide it. • make sure the services you get meet your needs • review the services you receive at least every 6 months to make sure they are still right for your. • keep your details confidential, and only share them with people who work with you. • if you have to wait for a service, they will tell you when you can have it and identify any alternative forms of support.

Cornwall’s Single Access Service: Referrals, Requests and Enquiries
Out Of Hours Service, Adult Care & Support The Out-of-Hours service is for: • older people • people with physical disabilities or sight/hearing problems • people with a learning disability • people with a mental health problem • vulnerable children and families Non-urgent requests for services should always be referred to the appropriate local Adult Care and Support office during normal working hours, telephone 0300 1234 131. You can also find useful information about services on the Council’s website, and request help via the page called ‘On-line Forms for Adult Care and Support’. The Out-of-Hours team can only deal with the urgent needs of people requiring adult care and support services; health emergencies, for example, will be dealt with by the medical services, and housing problems by Cornwall Council.

Adult Care and Support emergencies will be dealt with by 3 duty care managers across the county. Mental Health emergencies will be dealt with by the 24 hour Crisis Resolution Service. Children and Young People under 18 years emergencies will be dealt with by Children’s Service specialist officers. Anyone phoning the emergency number will be asked for their telephone number, location and which specialist out of hours service they think they need.The operator will then pass the call to the appropriate specialist worker for the area. If there is uncertainty about which specialist worker is required, one of the team will make contact and discuss the problem. Wrongly directed calls, eg. Health, Social Security or Housing, will be referred back to the organisation best able to help. Emergency needs relating to Social Care will be fully assessed in partnership with other agencies, where appropriate.

The Care Quality Commission replaced the former social care regulator, the Commission for Social Care Inspection, in April 2009. CQC regulates and inspects all care services for adults in England. Every care service is given a quality rating, from 0 to 3 stars. This makes it easy to see what the inspectors think of the overall quality of care the service provides. A three-star rating means ‘excellent’, two stars ‘good’, one star ‘adequate’,
and zero stars ‘poor’. When you look at CQC’s website, www.cqc.org.uk, you can search for care services in your area by postcode, type of service or their quality rating. You can view and download inspection reports, as well as other useful information and advice about choosing the best care service for your needs. Direct Payments If you are entitled to a service from Adult Care and Support, you will be able to receive an Individual Budget. This is an amount of money paid to you by Adult Care and Support so that you can organise and pay for the support you need. Our vision is that: “People will have easy access to services, real choice in the help they receive, and maximum control over the way they live their lives. They will be treated with dignity and respect.” Stages of an individual budget 1. Eligibility A Fair Access to Care Assessment will indicate if you are entitled to Adult Care and Support services. 2. Assessment You need to let us know when and why you need help through an Assessment. We hope that people will be able to use SelfAssessment in the future to tell us what they need. 3. Allocation You will be able to see the amount of money you can receive based on your needs. • This will be fair - because the amount of money will be Strip2 15/5/07 11:59 Page 1 based on your level of need.
• This will be equitable because people with the same level of needs will receive the same amount of funding. 4. Your contribution We will need to work out how much you can afford to contribute towards your support before we provide your budget. This is called a financial assessment. 5. Plan your support Now you can write your own plan! This should preferably be written with the people who know you best, for example: • Your family • Friends • Support workers • Advocate • Social worker This plan is for your whole life. It is about your work, leisure, travel, housing, relationships, and your hopes for the future. The plan will show how your Individual Budget will contribute towards meeting your outcomes. The plan has to be: • Safe • Within your allocated Budget 6. Getting your money Your money can go: • To your bank account

• To your carer’s bank account • To your circle of friends and family • To your care provider directly • Or it can stay with Adult Care and Support if you do not want to manage the money 7. Start living your life But tell us if there are any problems. We hope to put people in touch with a buddy, so you can talk to people who are going through the same thing for support and advice. 8. Primecare ‘8 to 8’ GP service
Newquay Drop-In Clinic
An innovative GP service, believed to be the first in Cornwall now makes it easy to see a doctor or a nurse at the Cardrew Health centre without having to join the practice.
A free, confidential health advice service for young people opens for the first time in Newquay. The drop in clinic will run on Thursdays beginning 5th November from the Youth Centre at 1 Whitegate Road. It will open between 3.30pm and 5.30pm and will always be staffed by at least one medical professional.
The service will be available from 8am to 8pm, 7 days a week, and will be provided by Primecare and run from the PCT’s premises in Cardrew Redruth. Appointments will be booked through Primecare and referrals made from existing GP practices. The service will also allow for self-referral (‘walkin’) patients. Patients are requested to book appointments, but patients who ‘walk-in’ will be seen as soon as there is a available doctor or nurse. The scheme offers all GP services, but is not a minor injury unit. This service, although administered by Primecare, is in addition to, and separate from, the existing ‘Out of Hours – Urgent CarePage Service’ also run by the Strip2 15/5/07 11:59 1 Primecare team. Many services are free. There is no charge for an assessment by Adult Care and Support. Services supplied following a Rapid Assessment Team visit are not subject to financial charges for the first week. There are charges for other services, such as meals and personal care. Following assessment, Adult Care and Support workers will always advise about any charges before setting up a service. Is there anything else I should know? Adult Care and Support have to keep records about the people who use their services. These records are securely kept and remain confidential. If you are dissatisfied with any aspect of your assessment, or the services provided to you, Adult Care and Support will always seek to fix the problem. If you are a carer, you are entitled to an assessment of your needs, regardless of whether the person you care for receives services from Adult Care and Support or not.
Continuing Healthcare Continuing Healthcare
NHS continuing healthcare is a package of services arranged and funded by the NHS for people outside hospital with ongoing health needs. Continuing healthcare can be in your own home or in a care home. Unlike help from social services (where a charge may be made depending on your income and savings), continuing healthcare is free. This means that the NHS will pay for healthcare services in your own home (e.g. a community nurse or specialist therapist) and personal care (e.g. help with bathing, dressing and laundry). Or if you are in a care home, the NHS will pay for your care home fees, including board and accommodation. Who is eligible for NHS continuing healthcare?
If your overall care needs show that your primary need is a health need, you should qualify for continuing healthcare. It is not dependent on a particular disease, diagnosis or condition, nor on who or where the care is provided. Not eligible for NHS continuing healthcare?
You may have to pay for some or all of your care, although the NHS can still provide for your medical needs. You may have to take a local authority means test to decide how much you should pay towards the cost of your care. NHS-funded nursing care. How will the assessment framework operate?
The NHS will make the decision on eligibility for NHS continuing healthcare in collaboration with the local authority through a multi-disciplinary team and with the full and active involvement of you and your carers. A multidisciplinary team is made up of two or more professionals who are involved in your care. NHS-Funded Nursing Care
By law, local authorities cannot provide clinical services because the NHS is responsible for any care that must be provided by a registered nurse. In care homes, registered nurses are usually employed by the care home itself and, in order to fund this nursing care, the NHS makes a payment to the care home to cover the cost for those who are eligible. Who is eligible for NHS-funded nursing care?
You should receive NHS-funded nursing care if you: • have been assessed as requiring the services of a registered nurse • do not qualify for NHS continuing healthcare but have been assessed as requiring the services of a registered nurse • are not receiving registered nursing care in any other way What is happening to the nursing care bandings?

Cornwall Air Ambulance The air ambulance is free at the point of emergency need, and is there to benefit children and adults who are unfortunate enough to need very fast and smooth emergency transport to hospital because of the severity of their condition. The helicopter covers all areas of Cornwall and the Isles of Scilly, and is able to land at a variety of locations, including beaches, moorland, on roads and community playing fields, in order to air lift a patient to hospital. The Cornwall Air Ambulance is not government funded, nor does it qualify for National Lottery Funding. It is purely down to the goodwill of the people of Cornwall who have kept the helicopter flying for more than 22 years. During that time the crews have attended thousands of incidents. Sadly almost half the incidents are to life threatening medical conditions, but the air ambulance also gets called out to horse riding and other sports injuries, industrial and farm accidents, fires and medical emergencies, and trauma arising from road traffic collisions. The helicopter, an EC135-T2, always flies with a crew of three which includes the pilot and two paramedics, seconded from the South West Ambulance Service Trust. Why England needs a new care and support system Society is going through huge change. People are living longer than ever before, and the proportion of older people in our society is growing. We have different social values, and we expect more choice and control over all areas of our lives, including public services. And too often the existing system does not live up to the expectations of those who depend upon it. We have an ageing population. In the next 20 years, the number of people over 85 in England will double and the number over 100 will quadruple. Similarly, advances in medical knowledge and practice mean that disabled people can live longer and can lead healthier lives. We expect over 1.7 million more people to have a need for care and support in 20 years’ time. A radical rethink of the care and support system is needed to address these challenges. Otherwise, it is likely that families, including dependent children within the family, will be under pressure to provide inappropriate levels of care, and in some cases people will go without support. If we fail to get to grip with these long-term issues we will fail to provide quality of life for potentially large groups of people, and consequently demand for NHS services will increase inappropriately. Care Homes This section includes information on how to make a decision, types of home, care home fees, and questions to ask. Care Homes are listed alphabetically in areas East, West and Central. Introduction Choosing where to receive the care you require can be a daunting task, particularly as it often involves leaving a home with which you are familiar and in which you may lived for many years. Whenever we choose a new place to live, we do so on the basis of a mixture of logic and emotion. Logic dictates where it should be and how close to family and friends etc. The emotional element is based on whether or not it feels like home. Written information and brochures can tell you about the practical issues but the “feeling of the place” is something you must establish for yourself. The key advice is to visit the homes which meet your practical requirements. Prepare a list of the issues which concern you and don’t be afraid to ask questions. You will find care providers more than happy to discuss your needs and concerns. Many of us have preconceptions about care homes and sadly, the image of care provision has not been well presented in the media. Television programmes and press articles tend to focus on the sensational and all too often it is the occasional problems which are highlighted. The vast majority of homes providing high standards and first class care are simply not newsworthy. Everyone has their own idea of what is nice, what suits them and what meets their own preference. The question therefore needs to be “does it suit me?” and in this respect there is no right or wrong answer. Who Provides Care? There are three principal providers of residential and nursing care. Most local authorities own and operate homes of their own although the numbers of places they provide have declined over recent years. The voluntary and charitable sector provides a relatively small but Strip2 15/5/07 11:59 and Pagethe 1 largest group is the important provision independent or private sector.
How Do I Measure the Quality of a Home? The difficulty with quality is that it can mean different things to different people and therefore it is extremely difficult to measure. All establishments providing residential or nursing care have to be registered with the authorities and comply with the prevailing regulations. This ensures that the home complies with the requirements of health and safety, staffing levels, hygiene, drug administration and accepted good practice. Homes are inspected regularly and the latest inspection report is available by asking the owner, manager or via the Care Quality Commission. It should however be remembered that the inspection process tends to examine systems and procedures rather than caring. How Much Will It Cost? The cost of care will vary between homes and across each region of the country. Although cost is an important consideration, affordability is perhaps the more significant issue and getting advice on benefit entitlement and how best to manage finances is crucially important. The CareAware Helpline can assist in this regard. With the requirement for higher standards, increasing wage levels and rising operating costs, care fee inflation can be an issue and so it is also important to consider the future situation, particularly if the care need increases. Demographic trends suggest that more of us will need residential care in the future and with home closures continuing, the availability of care beds can be expected to reduce and this in turn may lead to higher prices. Are There Specialist Homes? Some homes offer specialised care services. This may range from homes which provide male or

female only accommodation to those which cater for particular problems and difficulties. How to Find Potential Homes Some people are aware of the home they would prefer, perhaps through a recommendation or because a friend was resident there. However, many face the prospect of making a selection from scratch. The local authority will provide a listing of all registered homes and there are the traditional information sources such as Yellow Pages etc. In addition, many homes are part of local regional or national care associations who produce directories and listings. Where do I start? Paying for Residential Care If you are moving to a care home and think that Cornwall Council may be able to help you with the fees, they will have to look at your finances and work out how much you can afford to pay and how much help you might be able to have. A member of staff from the Financial Assessment and Benefit (FAB) Team will contact you, or the person who looks after your finances, within four weeks of the date of your move to a care home. They will arrange for a FAB officer to visit, at a time that suits you, to work out how much you will have to pay. At the visit they will be able to help you with any questions you have about exactly how much you will have to pay. Most people have to pay something towards their care fees. For someone over sixty on Pension Credit (Guarantee Credit) this will normally be £108.10 a week. For someone under sixty on Income. Support this will normally be £69.90. If you have more income the weekly charge will be higher than this. If you have savings of over £23,000 you will have to pay the full cost of the care. Savings You can only get help with care fees from Cornwall Council if you have less than £23,000 in savings. This includes things like; bank and building society accounts, including current accounts, shares, premium bonds, unit trusts, PEPs / ISAs, cash, national savings certificates, bonds. If you have more than £14,000 you will be asked to pay a contribution from your savings of £1 each week for every £250 you have over £14,000.

Provisional Charge You will be asked to pay a provisional amount for the first four weeks until your contribution has been agreed. Please call Adult Social Care (ASC) on 0845 345 9133 for the amount you would have to pay. If ASC work out that this amount is too high then they will arrange to give you back any overpayments you have made. If ASC work out that you can afford to pay more than this, you will be asked to pay the new charge from four weeks after you went into residential care. If they find that your savings are over £23,000 they will ask you to pay the full fee backdated to the start of your stay. Independent Financial Advice Cornwall Council advise all residents to take independent financial advice, as there may be several different ways of funding care available. Care Homes
Top-Ups Cornwall Council sets different weekly fee levels for different types of care. If you choose to go to a home that charges more than these levels a third party will have to pay the difference. This could be a family member, friend or charity. The resident is not allowed to pay the top-up from their income or savings, unless they own a property that is taken into account in the financial assessment. For example if you are an older person and choose to go to a residential home with quality rating charging £350 each week: • The fee level set by Cornwall Council is £328.20 • A third party will have to pay £21.80 each week – the difference between £350 and £328.20 • The resident will pay as much as they can afford • Cornwall Council will pay the difference between the amount the resident can pay and £328.20. Types of Care Home Residential homes You may choose to go into a care home (sometimes called residential home) if you can no longer manage in your own home. Services you should expect at a care home include: • meals and accommodation
• personal care like bathing and dressing • staff available 24 hours a day • physical and emotional care • care through short illnesses
Care homes with nursing If you have a disability or illness that means you need nursing care on a frequent basis then a care home with nursing (sometimes called a nursing home) might be more appropriate. A qualified nurse will be on duty 24 hours a day. You have the right to choose your care home but it must meet your needs, so if you need a care home with nursing you may not be able to move into one that cannot provide nursing care. You may qualify for full NHS funding of your care home. You will have to meet your health authority’s criteria. You can ask them for a copy of these. Specialist care homes You may choose to move into a care home that provides specialist care for your particular disability or needs. This might include specially trained staff or adapted facilities. Specialist homes could be for: • Learning Disabilities • Mental Health • Dementia Your local council should have lists of specialist care homes or ones that might offer services appropriate to your needs. Merlin Project Here at last!
In a beautiful semi‐rural
and picturesque setting, sits Cornwall’s very own, brand new, Multiple Sclerosis Therapy Centre. The building stands proud, as do the people, who have worked so long and hard to plan, develop and deliver this amazing project.
But it has been no easy ride to get this far. Indeed, founder members of the project, Eileen and Derek Murphy, along with their band of truly committed supporters, embarked on The Merlin journey in 2001 with a capitol appeal of £2million, after realising that support for M.S. sufferers in the County was limited.
For some strange and curious reason, Cornwall has the highest incidence of Multiple Sclerosis on mainland UK with the Orkney Islands having the highest. The condition is usually diagnosed in the 20’s and 30’s age group and particularly affects women of childbearing age.
After years of fundraising, the long awaited Centre opened its doors to the public on the 14th April 2009 and its popularity goes from strength to strength, with more than 250 people and their carers visiting the centre on a regular basis.

Built with the specific aim of catering for the therapeutic needs of those in the County living with M.S., the Centre, which is still only 2/3rds operational, welcomes anyone, with any condition, who might benefit from the treatments and therapies that are on offer, of which there are many. However, there are those who simply use the centre as a place to meet with others – a therapy in its self!
Until this wonderful gift was given to Cornwall, sufferers of the incurable and degenerative condition were forced to travel out of County, in an effort to access the much needed treatments that would bring some quality and benefits to their lives.
Even though still in its infancy, there are already a wide range of orthodox and complimentary treatments and therapies, such as aromatherapy massage, reflexology, reiki healing, Indian head massage, spiritual healing, counselling and support groups, and much more.
In addition to the above, there is an Osteopath and a fully equipped Physiotherapy department with no waiting list – unlike the present overstretched NHS system. With this in mind The Merlin Project is able to offer appointments at a mutually convenient time when clients can be guided through the various techniques, which in turn improves strength and prolongs client’s independence.
By far the most popular therapy is the Hyperbaric
Oxygen Therapy Suite. This is the busiest of all the therapies and has proven to assist in the relief of a number of symptoms, not only by those with M.S., but a whole host of other conditions including M.E., Tinnitus, Sports Injuries, Fractures, Chronic Back Pain, Fatigue, the list goes on.
But Merlin’s journey still has some way to go with fundraising for Phase II of the building ongoing. It is estimated that a further £250k is needed to deliver a fully operational centre. This will offer so much more with the development of additional services giving opportunities and support to an ever growing client base.
Whilst the Centre has been built primarily for people with MS it is not exclusive and welcomes all. The Merlin Project is a registered charity (Number: 1093691) and relies on fundraising events and the generosity of the general public.

Help At Home
PIPs -Age Concern (Age UK)
If you are having difficulty coping at home, due to old age and/or illness, there are an increasing number of services available to help you continue living safely and independently at home. Alternatively, after a stay in hospital you may require extra help when you go home, especially if you live on your own. The independent agencies listed offer help at home, either with housework, or with personal care such as washing, dressing and using the toilet. Alternatively, Adult Social Care can arrange personal care for you, following an assessment of your circumstances. You may also be entitled to Direct Payments from Adult Social Care, see direct payments section, page 9. Adult Social Care and the NHS in Cornwall also have a range of schemes across the county specifically to help people whose health may have deteriorated, but who nevertheless wish to remain in their own homes. Some of the schemes include extra help from therapists in the community; help from trained workers to enable people to cope safely with daily living tasks; and care for people leaving hospital, who may not feel confident about returning home straightaway. Finally, for short-term help at home, following discharge from hospital, the British Red Cross offer the ‘Home from Hospital’ scheme. Whether you want help filling in a form, advice about your rights and choices, or need a bit of care and support, one of our experienced team can help you. If your circumstances have changed or if you have recently lost a service you think you need then one call to the PIPs can help. A PIP will visit you in your own home and talk to you about what would make your life better. We will then do our utmost to make it happen. The PIP will work with you to access one of our main services such as home care, aid call and befriending or a specialist service provided by another organisation and if you don’t think these services will meet your own needs, then we will develop a service around you and your lifestyle. Carers Carers provide unpaid, regular support for the care of someone who is unable to manage alone without help. This includes young carers of an adult and parents of a disabled child. From the census in 2001, in Cornwall alone, 55,000 people, 10% of the population are carers. Nationally, carers save the government £87billion a year. Many carers experience ill health, financial disadvantage and fatigue so it is essential that they can access information, advice and support at the earliest opportunity to enable them to get help at the right time and ease there caring role. Carers Assessment and Carers Grants Carers have a legal right to an assessment of their needs, a Carer’s Assessment. DACS have a legal responsibility to carry out this assessment which gives the carer an opportunity to discuss their needs and the things that could help in their role as a carer. It could also influence the care services provided to the “cared for”. It may also lead to a Carer’s Grant which could provide, for example, a break. All the Carers Support Services listed, can assist carers in making a referral for a Carers Assessment. Benefits for Carers The person you care for may qualify or be in receipt of Disability Living Allowance or Attendance Allowance. As a carer you may be entitled to Carers Allowance or Carers Premium if the person you care for is getting one of the benefits above. There are a number of agencies that can advise you on benefits as well as the Carers Support Services. Meal Provision Eat Well, Live Well Eating well is a positive choice to make. A healthy balanced diet contains a variety of types of food‚ including lots of fruit‚ vegetables and starchy foods such as wholemeal bread‚ protein rich foods such as meat‚ fish‚ eggs, pulses and some dairy foods. Fat is a good source of energy, but eating too much fat can make us more likely to put on weight. Eating foods that are high in saturated fat increases the risk of developing heart disease by raising your cholesterol levels. Oily fish contain ‘omega 3 fatty acids’, which are good for your heart and vitamin D which is important for healthy bones. You should aim to eat at least two portions of fish a week, one of which should be oily. Vitamin D is also found in fortified cereals and soft spreads such as margarine. If you do not eat any of these foods, or you rarely get any sunshine you should consider taking a daily supplement.
Base your meals on foods that are high in starch and fibre, such as wholemeal bread, pasta, rice, cereals and potatoes. Oats, fruits and vegetables, pulses, such as kidney, butter or baked beans are all good sources of fibre. Eating fibre rich foods and drinking plenty of fluid – around 6 to 8 glasses throughout the day – can help prevent constipation, which also reduces the risk of some common disorders in the intestine. Have some protein such as meat, eggs, pulses (peas, beans and lentils) and fish every day. Many of these protein rich foods also include iron, which is important to prevent anaemia. Other foods that contain iron include bread, dark green vegetables and fortified breakfast cereals. Because having (more than 1.5mg per day) might increase the risk of bone fracture you should avoid taking any supplements that contain vitamin A, including fish oils which have a high content. Recent days have been important for the Social Work profession. The government’s long awaiting Social Work Taskforce Final Report was published in early December. The Report has a range of recommendations that seek to improve the profession and to promote a more positive identity and image with the public. Many of us see at first hand the excellent work that Social Workers do and witness the real and positive differences they make in changing peoples lives for the better, often in circumstances where people most need help. Hopefully, it is this positive and optimistic view of Social Work that will be better promoted rather than just the blame when something goes tragically wrong. To do this, the Report recognises the vital role that training institutions, County Councils (as the major employers of Social Workers)as well as Social Workers themselves have in improving standards. The government have fully endorsed the Report and, I think, it’s positive outlook for the future.

SWAPS is a registered charity delivering Shared Lives services (formerly adult placements) across Cornwall, Plymouth, Devon and Torbay. SWAPS provides a range of long term and short break services to people with a variety of support needs. Support is provided by an approved provider, mostly in their own home, but also in the person’s own home if needed. Short breaks can be a few hours or a week or two. The service is individual and person centred. SWAPS is a three star excellent service registered with the Care Quality Commission. Launched on 3rd February 2009 to transform the quality of dementia care, including improved community personal support services and domiciliary care services. The strategy aims to develop services for people with dementia and their carers that are fit for the 21st century, with services that meet the needs of everyone, regardless of age ethnic group or social status. The Alzheimer’s Society is the UK’s leading care and research charity for people with or affected by dementia. Alzheimer’s Society is a membership organisation, which works to improve the quality of life of people affected by dementia. In over 250 locations across the UK, Alzheimer’s Society staff and volunteers work together to provide help and information to people affected by dementia in their communities. Our branch services include day care and home care for people with dementia, as well as support and befriending services to help partners and families cope with the demands of caring. Integrated Care Pilots (PCT) The projects, designed to improve services for people with dementia and mental health problems, are two of only sixteen Integrated Care Pilots from across the country, and they will be showing how health and social care services can join together to improve the quality of care people receive. The Newquay Pilot is designed to simplify access to all types of dementia services in Newquay. The pilot includes establishing a GP-led memory clinic and ensuring that every patient diagnosed is assigned a practice-based key worker to provide continuity of care from diagnosis until end of life. The North Cornwall Pilot involves ten GP practices in the area working together to integrate Mental Health community teams, based in a rural location, with a single point of access from GP practices. It will integrate Mental Health acute and social services. John Ingle GP Dementia specialist at Narrowcliff Surgery said, “We are delighted to have had our initiative in this area recognised. Dementia is a condition that we believe has been traditionally under-diagnosed and consequently some patients and their carers have missed out on appropriate care and support. SANEline
The Dementia Gateway is a new website on the Social Care Institute for Excellence’s (SCIE) website. The Gateway comprises a comprehensive set of resources on dementia aimed at staff, managers, carers, commissioners and people with dementia. The goal of the Gateway is to increase good practice in care homes and within domiciliary care, and in turn to improve the lives of people living with dementia. The website contains practical tips, tools and activities on key areas in an accessible and user-friendly format. Visitors can check for quick updates, or look in detail at dementia issues and the site also includes free e-learning programmes, developed in conjunction with the Alzheimer’s Society Sections of the site include: • Getting to know a person with dementia • Eating well for health and enjoyment • Tackling difficult situations • Keeping active and occupied • Making living places safe and enjoyable In addition, the site offers access to a series of new films developed by SCIE for its Social Care TV service. SANE is a charity set up in 1986 concerned with all mental illnesses including depression and manic depression, schizophrenia, anxiety, obsessions, phobias, as well as alcohol and drug addiction, and more. The three main objectives: • To raise awareness and respect for people and their families with mental illness, improve education and training, and secure better services. • To undertake research into the causes of serious mental illness. • To provide information and emotional support to those experiencing mental health problems, their families and carers through SANEline. The confidential SANEline number is 0845 767 8000, lines are open 6pm to 11pm every day, calls are charged at local rate. Help includes practical information, crisis care and emotional support to anybody affected by mental health problems, including family and carers.
Every year, around 1 in 4 people experience common mental health problems such as: • Low mood and depression • Stress, anxiety, worry, phobias and panic • Problems after trauma • Obsessive - compulsive disorder Outlook South West provides psychological therapies for all the common difficulties listed above. The service is provided to all GP surgeries across Cornwall & the Isles of Scilly. With around 100 trained therapists throughout the county, they provide a range of useful options including; self-help resources, advice about other resources and organisations, or planned sessions with a psychological wellbeing practitioner, cognitive behavioural therapist or counsellor. As well as delivering primary care services for Cornwall & the Isles of Scilly, Outlook South West operates a multitude of additional projects, which include Stress Buster courses (educational support on coping with and managing stress), the Brighter Outlook programme (for people with Seasonal Affective Disorder) and Dementia Carer Training (information, help and advice for people who care for loved ones with dementia).
Part of Outlook South West’s Brighter Outlook programme involves offering light therapy to people with Seasonal Affective Disorder.

Employment for disabled people Around one in five people of working age have a disability or long term health condition and over one million disabled people want to work but don’t have a job. Pluss offers disabled people a wide range of services to help them train for, find and keep work. Having a job offers much more than just an income. It offers independence, dignity and self esteem. At Pluss, we believe this opportunity should be available to everyone and offer a full range of services designed to meet an individual’s needs and readiness to work. We currently support over 2,000 people to develop their skills, find and keep work. Our services include the following: Training and learning: We offer information and advice, and will help you put together a development plan for learning and work. We also deliver a wide range of courses to help you achieve nationally recognised qualifications. Work experience: We will help you build your confidence, self esteem and find work experience
in a job of your choice. This gives you the opportunity to try new things, identify any support needs and build on your skills. Job search: We get to know you and your skills, strengths and aspirations. We then set about matching you to the right job with the right employer. We offer you support with your job search, practise interviews and liaise with employers on your behalf. In work support: Once you are in work we offer ongoing support, including job coaching, training, funding for equipment and regular contact. You will get as much support as you need because it’s your choice and your job. Hearing problems
If you are disabled or lack mobility, a meanstested grant may be available for adaptations to your home to enable you to continue to live there. The first stage is to contact Adult Care and Support, who will arrange for an occupational therapist to visit and assess you. They will recommend the type of adaptation that would be suitable, and how urgent it is. Please note that the length of time it takes to complete the adaptations process can vary considerably. Disability Cornwall aims to be the voice of disabled people in Cornwall and will challenge discrimination and inequalities and at the same time promote examples of good practice. This is achieved through managing a portfolio of projects & services and the development of partnerships to deliver services around access audits & advice, and training in both DDA & Disability Equality, as well as Cornwall’s Diversity Toolkit. This innovative document aims to help eliminate discrimination in the workplace along all seven of the equality strands (race,
disability, age, faith, gender, transgender & sexual orientation). Disability Cornwall provides consultancy services in connection with policy review and the provision of consumer audits or ‘mystery shoppers’. In addition, the DC100 comprises 100 Disability Cornwall members willing to be consulted for their views and experiences through e-based simple surveys. This type of consultation feedback can then be used to raise awareness and affect positive change. We produce Discover, Cornwall’s only disability aware lifestyle
magazine covering human interest stories, news, sport, services available etc. We hold conferences, workshops and meetings according to need. The DIAL service (see below). Day Services
Respite Care
Day Services offer help and support to older people and to people with disabilities. They offer social contact and stimulating activities for people who are finding difficulty living at home. Day care also enables carers to have a break from their caring. Centres are run by local authorities, the NHS, voluntary organisations and commercial businesses such as care homes (see page 14). Day Services offer care for adults with physical and/or learning disabilities and people recovering from mental illness. Day Services aim to give them the skills they need to live as independently as possible. Most are run by the Adult Services department and are for people who have been assessed as needing this kind of help.
Caring for someone at home may sometimes feel like having a full time job and you might feel that you need to take a break from time to time. Respite Care for the person you look after may give you the opportunity to do just that. It may be possible to admit them to a residential or nursing home for a short period of time, giving you time at home or perhaps enabling you to take a holiday away. You may also like regular short breaks for a few hours to pursue a hobby, to take time for yourself, or perhaps just a good nightâ&#x20AC;&#x2122;s sleep. There are a range of services that can provide trained people to look after the person you care for just such purposes, during the day or night.

Nursing Agencies
Minimum Standards
Nurses agencies send nurses to visit you at home when you need specialist or medical care, usually after leaving hospital. The frequency of the visits can vary from several times a day to once a week. About 95% of the nurses supplied by nurses agencies are provided to people in hospital or a care home. However, some nurses are provided to people in their own home. Care Quality Commission website: www.cqc. org.uk
A set of guidelines to help providers, inspectors and service users understand the expected minimum level of service. These guidelines are not enforceable by law, but providers should abide by them. They also give service users the ability to judge care providers to ensure the correct service is employed. You could expect a nursing agency to provide a wide range of services for example: • Ad hoc, • Emergency cover, • Joint planning to manage seasonal shortages, • Bank management, • Permanent recruitment. There are two types of nurses agency. The first type simply acts as an employment or introductory agency which places self-employed nurses into employment with people who need nursing help at home. The other type of agency employs, trains and supervises its own staff.

Cornwall Hospice Care runs the county’s only adult hospice service, providing specialist medical care to patients with life limiting conditions and offering practical, emotional and spiritual support to them and their families. There are a total of 24 in-patient beds, as well as day treatments and therapy services provided within the hospices and specialist medical input in hospitals and nursing homes throughout Cornwall. The clinical team includes doctors and nurses; occupational, complementary and physiotherapists; Lymphoedema nurse specialist; social worker and bereavement counsellors, offering an all-encompassing care to improve the quality of life of patients. Nearly 600 adults stayed at the hospices last year and there were around 2000 patient admissions for day treatments and therapies. Specialist equipment plus staff costs add up to around £5.6 million annually – Strip2 equivalent to nearly £11 a minute. Advocacy Advocacy is representation for someone who, for whatever reason, is unable to speak up for themselves. This could be support for questions, decisions, or making a complaint. The Advocacy Directory The advocacy directory is a comprehensive online database compiled by Age Concern Cornwall signposting advocates for a wide variety of issues and situations. Cornwall Council observed that people of all ages and groups can benefit from the help of advocates. These advocates can give advice, support and information. With this help, some people will be able to sort out issues that are important to them on their own, others will want somebody who can take up their battles on their behalf. Motability
Before the introduction of the Mobility Allowance in 1976, only disabled people who could drive themselves got any government help with personal transport. It was usually in the form of a small car provided to disabled couples, to people responsible for the sole care of a disabled child and to a category called “war disabled”. An allowance was paid to disabled people who actually owned a car. But most people were supplied with a small, blue, single-seat, three wheeled, motorised “invalid trike”, which was incapable of carrying passengers.
• Higher Rate Mobility Component of Disability Living Allowance. • War Pensioners’ Mobility Supplement. Around two thirds of our customers drive, but nondrivers can get a car as a passenger. Similarly, parents and carers can apply on behalf of a child aged three or older.
Today, The Motability Scheme enables disabled people to use their government funded mobility allowances to obtain a new car, powered wheelchair or scooter. Some 450,000 people currently use the Scheme and in 2006 Motability celebrated providing two million vehicles since it was set up by the government in 1977.
Some customers need extra financial help to meet their mobility needs, for example, to help towards the cost of the advance payment for a suitable car, adaptations, driving lessons or perhaps a wheelchair accessible vehicle. Every year Motability awards grants from both its own Charitable Fund and the Specialised Vehicle Fund which is administered on behalf of the government.

The blue badge scheme provides a national range of parking concessions for disabled people with severe mobility problems who have difficulty using public transport. You are automatically eligible to apply for a badge if you are over two years old and either: • receive the Higher Rate of the Mobility Component of the Disability Living Allowance • are registered blind • receive a War Pensioner’s Mobility Supplement You may also be eligible for a badge if you are over two years old and either: • have a permanent and sustainable disability which means you cannot walk, or which makes walking very difficult • drive a motor vehicle regularly, have a severe disability in both arms, and are unable to operate all or some types of parking meter • If you are a parent of a child who is less than two years old, you may apply for a badge for your child must be accompanied by bulky medical equipment or needs to be kept near a vehicle at all times. The concessions provided under the scheme apply to on-street parking only. Badge holders may park for free and for as long as they need to at on-street parking meters and pay-and-display machines, unless there is a traffic sign specifying a time limit for holders of Blue Badges. Where a time limit is in force you must also display the special blue parking disc showing time of arrival.
Car/Road Tax Exemptions You may claim exemption from vehicle excise duty (road tax) if you’re receiving one of the following: • The higher rate mobility component of the Disability Living Allowance • The War Pensioner’s Mobility Supplement If you’re not the driver, you can nominate someone else (a nominee) to drive the car for you. But the car must: 1. Be registered either to you or your nominee 2. Only be used for your benefit 3. Be insured for the nominee to drive If you don’t own a car, you can nominate someone else’s vehicle but it must be used only for your purposes for example shopping or getting prescriptions. Vehicle excise duty (road tax) exemption can only apply to one vehicle. Motability keeps Aaron on the move! for my A-Levels, so I use it all the time for getting to college and meeting with my friends. I love fishing and playing tennis and having my car means I can do all these things and not have to think about how I’ll get there - I can just get in the car and go whenever I want to.
Aaron Morgan, 19, from Hampshire, is paralysed from the waist down following a motocross accident at the age of 15. “I’ve always been a massive fan of motor sports and would love to become a racing driver,” says Aaron. “After the accident, there was no way I was going to give up on my dream. After passing my driving test first time, I got a brand new VW Golf through the Motability Scheme. It’s an automatic with hand controls instead of foot pedals and is great to drive. “My Motability car means I can live an independent life. I don’t need to rely on my parents for lifts and I can come and go as I please. I’m currently studying
“Driving myself around everyday is giving me plenty of road experience which is perfect if I want to be a professional driver. I have been going to track days at Silverstone and Brands Hatch recently, and I’m hoping that next year I’ll be able to take part in the Production BMW Championship in an adapted BMW E30 Series. I can’t drive my Motability car at racing speeds, but knowing I have it to get around is great. “I’m really lucky as both my family and X Sport Racing give me lots of support and encouragement. I don’t know what I’ll do after college, or if I’ll end up being the next Lewis Hamilton, but I’m looking forward to racing properly and seeing what my future holds.” Aaron was presented with the key to his Motability car recently by Her Majesty the Queen as part of Motability’s 30th anniversary celebrations.

Sheltered Housing Sheltered Housing (Retirement Housing) in Cornwall is listed in this section. It is likely that there will be a waiting list for rents and purchasing, it is advisable to contact the preferred scheme early. Schemes and options Sheltered housing is often called retirement housing or warden-assisted housing. There are many different types of sheltered or retirement housing schemes, both to rent and to buy. Schemes usually consist of between 15 and 40 dwellings which may be bedsitting rooms, self contained flats, bungalows or luxury apartments. Usually they are designed with older people in mind and to mobility standards, for wheelchair access for example. Some schemes are simply housing designed to meet the needs of older people and linked to a community alarm. But the majority of schemes have a Scheme Manager (or ‘Warden’) as well as a community alarm service. There are often communal facilities such as a lounge, laundry, guest flat and garden. Meals are not normally provided but a very few schemes include a restaurant, and quite a number arrange one hot meal a day. There is usually a minimum age, often 55 or 60, but occasionally 50. Sheltered or retirement housing appeals to people who like living independently but want the reassurance of knowing that assistance is on hand if there is an emergency. The duties of the Scheme Manager can vary considerably between schemes. Some schemes have a resident manager and a 24 hour service, others have visiting/part time managers. Scheme Manager (Warden) Over the last few years there have been many changes to the Scheme Manager’s duties. Most Scheme Managers are now expected to: • Manage the scheme • Summon help in an emergency. • Build up a relationship with older people living in their schemes; giving residents information on availability and access to services and encouraging them ask for additional support from statutory Strip2to 15/5/07 11:59 Page 1 and voluntary organisations when appropriate.

Local councils and RSL’s generally aim to offer their housing to people in the greatest housing need, at rents which are affordable - perhaps with the help of housing benefits. The criteria that applicants are generally expected to show include: • Some degree of housing need because of the physical condition of their present home; or • Medical and/or social reasons for wanting to move, e.g. poor health, disabilities, loneliness, fear or isolation from friends & family; and • That they are not easily able, for whatever reason, to buy rather than rent; and • That (if they are not local already) they have good reason for wanting to move to the area they apply for. In most parts of the country, councils and RSL’s recieve applications from more people than they can assist. The cost of renting will be made up of a rent plus service, a charge to cover the services provided. Service charges in the majority of ordinary sheltered or retirement schemes is in the range of £20 to £40 a week. People wanting to rent sheltered housing and who have a low income should contact local authority to check their eligibility for housing benefit. Buying You may also consider purchasing property. Prices and types of property vary enormously. A small second-hand flat may be found for under £70,000

See opposite page for facilities in your area. FREE SWIMMING FOR ADULTS OVER 60! Contact your local swimming facility to find out more. Proof of age required. Cornwall Council will be offering free of charge swimming at local authority swimming pools to anyone over 60 years of age. The scheme is subsidised by the Department for Culture, Media and Sport as the centrepiece of the government’s plan to ensure a lasting sporting legacy from the London 2012 Olympic Games and Paralympic Games. Simon Blamey, a senior leisure officer, explains, “We are delighted to be able to offer those over 60 the chance to improve their health and wellbeing by participating in totally free of charge swimming sessions. I hope that Cornwall’s silver swimmers take full advantage of the offer, which will not only help them keep fit, but also provide a chance to socialise with other people with similar interests.” Anyone over 60 who would like to take advantage of free swimming shall go along to the local authority owned leisure centre and fill in a registration form, taking a form of ID such as bus pass, passport, birth certificate or driving licence. Once registered, you will be able to access sessions that are designated ‘general swimming sessions’ at participating pools.

Activity & Therapy Wet and Well The Blue Gym project aims to promote physical and mental well being and strengthening of communities through encouraging people to get out and get active in the coastal and water environments. The Blue Gym will act as an umbrella and a portal to the amazing range of activities that are already taking place in, on, under or around water. These range from walking on the coastal footpaths and beaches through to rock pool rambles and full immersion sports like swimming, sailing and surfing. The NHS and PCTs will be encouraged to direct patients through the Blue Gym to activities they can take part in as an alternative or supplement to other forms of treatment. The South West will be a test bed for the scheme especially in terms of research to prove that using the Blue Gym works! The Blue Gym will work with all organisations such as local authorities and sporting organisations to improve ease of access and engagement. Green Gyms in Cornwall If gardening as a form of physical activity appeals to you or if you’re just looking for a way to connect more with the earth on a personal level, consider joining a Green Gym. Green Gyms provide a fun, socially interactive environment that offers a great way to get fit and connect with people in your community who are interested in doing something great for the planet. BTCV is a conservation charity that runs Green Gyms, local programs whereby you are led through some warm-up exercises followed by conservation or gardening work at your own speed, with the guidance of a trained leader. Green Gyms are a great way to take care of yourself and the planet at the same time. Free Cinema for Carers Carers who look after a disabled person may be entitled to go to the cinema for free. The Cinema Exhibitors’ Association card (CEA) is a national scheme, however not all cinemas subscribe. To qualify for the card the person must be getting DLA and AA or registered blind, or the holder of a disabled persons rail card. Mental health care What is Sea Sanctuary? A mental health charity based on-board a 43ft classic yacht ‘Leopard’, set-up to provide support with deliberate self-harm, depression and anxiety. We also offer a respite facility for up to 5 days at sea, far removed from the hustle and bustle of ordinary life. Sea Sanctuary is suitable for young people and adults as we are keen to be as inclusive as possible. Our service is commissioned by the NHS and our treatments are evidence based. We further offer educational programmes in the subjects of communication skills and conflict resolution, anger management, team building and improving self-esteem (these are particularly suitable for young people). Support in form of life coaching, meditation, pampering (organic of course) and nutrition advice – with lots of laughter and care thrown in. The days will all be sponsored by Spiezia and their spa business partners. This is in addition to the ongoing complementary therapy in two palliative care clinics in Cornwall provided by Spiezia’s Spa Development Manager. “We know that women with cancer are all individuals, not just a statistic, and laughter and care are vital to help them pull through. Lasting Powers of Attorney Anyone who faces the possibility of losing mental capacity should make a Lasting Power of Attorney. An LPA appoints someone you trust to make decisions for you. Separate LPAs may be made for both financial and for welfare matters.The process of making an LPA is quite involved because it incorporates safeguards minimising the risk of financial abuse. Deputyship Where someone has already lost mental capacity, it is often advisable to obtain a court order so that a trusted person, called a “Deputy”, can manage that person’s finances and take decisions on their behalf. Papers are required to make an application to the Court of Protection for a Deputy.
Deprivation Of Liberty Sometimes elderly people who have lost mental capacity are required to move into Residential Homes for their own best interests. But mistakes are sometimes made, and it is believed that many people are kept in residential homes against their wishes and without sufficient justification. Since April 2009 Local Authorities have had a duty to ensure that Deprivations of Liberty are justified.
Management of Affairs If you are elderly or disabled and struggling to cope, it is ideal to have a caring adult son or daughter to look Probate after your affairs.
