Putting People First is a government initiative with the ambition to reform adult social care by enabling people to live their own lives as they wish, confident that services are of high quality, are safe and promote their own individual needs for independence, well-being and dignity. Some of the key points are listed here: Local authorities to work in partnership with local NHS, providers and carers to create a new, high quality care system which is fair, accessible and responsive to the individual service user needs. • Agreed and shared outcomes which should ensure people, irrespective of illness or disability, are supported to: Live independently; Stay healthy and recover quickly from illness;

DoH Green Paper: Shaping the future of care together To create a new care and support system, one that enables everybody to access the high quality care they need, we need to make changes to the ways care and support is provided. More joined-up working: We need services that will keep people well for longer. They should also help people with ongoing care needs to stay at home, if this is what they want, and maintain their independence. One way of doing this is through better joined-up working between health, housing and social care services and between the care and benefits support systems. A wider range of services: As people begin to have greater choice over the care they receive, they will need to have a range of services to choose from. We need to help local authorities ensure that the care and support available in their area support innovative and high-quality care and understand how services will need to adapt. Partners in Care; Representing the Independent Sector
Partners in Care is an employer-led group established in 2000, whose activities have focused on workforce development within the social care field. They do this by working with voluntary, independent and statutory organisations throughout Bournemouth, Dorset and Poole. The Partners in Care group has representatives from all three local authorities, Learning and Skills Council, Business Link Wessex, Training Provider Representative (Care South), PCTs, Home Care Association, Dorset Care Association, Job Centre Plus, Skills for Care and the CQC. This not only ensures an equal representation across the county but also across a wide expanse of organisations. Therefore future development in social care can be considered in an effective manner. Over the last three years Partners in Care have supported over 1200 individuals in 300 organisations to gain a qualification in Health and Social Care. This not only helps to raise the standard of care delivered but also the standard of the social care workforce.

NHS Dorset was established on 1 October 2006, it covers the same geographical area as Dorset County Council, comprising six District/Borough Councils, Weymouth and Portland Borough Council, West Dorset District Council, North Dorset District Council, Christchurch Borough Council, East Dorset District Council, Purbeck District Council. The Trust is responsible for securing services for a population of 395,000 through commissioning and direct provision. Services are directly provided through Community and Specialist Health Services and Community Hospitals. The Trust is responsible for the direct management of and employment of staff in eleven community hospitals and supports the use of the specialist skills of the general practitioners and community nurses in the area using the facilities available at those hospitals.
NHS Bournemouth and Poole is the local NHS organisation tasked with improving the health and well-being of the local population and receives funding directly from the Department of Health. We are expected to invest our resources to tackle health inequalities, promote health and well-being and ensure a comprehensive and equitable range of health care services are available for local people. “Our vision for Bournemouth and Poole is of a healthy population, with informed individuals who are living independent healthy lives, supported appropriately to take care of themselves. When they do need treatment or additional support, we expect local people to be able to access a range of high quality services, at a time and place to suit them.” www.bournemouthandpoole.nhs.uk
District nurses and health visitors directly managed and employed by the Trust are based in 59 general practices and a range of other premises. The Trust provides Mental Health Services for patients in the North and South West of the Trust and commissions mental health services from Dorset Health Care Trust for the patients in the South and East locality. www.dorset-pct.nhs.uk/

Continuing Healthcare
NHS-Funded Nursing Care What is NHS-funded nursing care? By law, local authorities cannot provide clinical services because the NHS is responsible for any care that must be provided by a registered nurse. For people in care homes, registered nurses are usually employed by the care home itself and, in order to fund this nursing care, the NHS makes a payment to the care home to cover the cost of providing the care for those who are eligible. Who is eligible for NHS-funded nursing care? You should receive NHS-funded nursing care if you: • have been assessed as requiring the services of a registered nurse • do not qualify for NHS continuing healthcare but have been assessed as requiring the services of a registered nurse • are not receiving registered nursing care in any other way What is happening to the nursing care bandings? Previously, there were three bands for NHS-funded nursing care, and the NHS had to determine which of these each individual would fall into. Under the new National Framework, the NHS will still have to assess your need for registered nursing and decide what services to provide, but there will not be any bands, nor will there be a need to have a separate assessment to decide the right band.

What is continuing care? NHS continuing healthcare is the name given to a package of services which is arranged and funded by the NHS for people outside hospital with ongoing health needs. You can get continuing healthcare in any setting, including your own home or in a care home. NHS continuing healthcare is free, unlike help from social services for which a charge may be made depending on your income and savings. In your own home, this means that the NHS will pay for healthcare (e.g. services from a community nurse or specialist therapist) and personal care (e.g. help with bathing, dressing and laundry). In a care home, the NHS pays for your care home fees, including board and accommodation. Who is eligible for NHS continuing healthcare? Anyone assessed as requiring a certain level of care need can get NHS continuing healthcare. It is not dependent on a particular disease, diagnosis or condition, nor on who provides the care or where that care is provided. If your overall care needs show that your primary need is a health one, you should qualify for continuing healthcare. What if I am not eligible for NHS continuing healthcare? If you do not qualify for NHS continuing healthcare then you may have to pay for some or all of your care, although the NHS will still provide for your medical needs. You may have to take a local authority means test to decide how much you should pay towards your personal care and accommodation if you are in a care home. However, if you are a resident of a residential home and require care from a registered nurse, this will be provided free of charge through NHS-funded nursing care.
How will the assessment framework operate? The NHS will make the decision on eligibility for NHS continuing healthcare in collaboration with the local authority through a multi-disciplinary team and with the full and active involvement of you and your carers. A multi-disciplinary team is made up of two or more professionals who are involved in your care.

Dept of Health - Providing a National Care Service High Quality Care for All This time last year following the efforts of 2,000 frontline clinicians and the 60,000 people engaged in producing the NHS Next Stage Review, I published my report: High Quality Care for All. At its heart was one simple yet radical idea: to put quality at the heart of the NHS. A year on its right to reflect on where we are. Across each dimension of quality - patient experience, patient safety and clinical effectiveness there has been real progress and patients are already seeing the difference. There are many examples to choose from. To name but a few: we now have 50 new GP led health centres, 9.3 million patients with long-term conditions have a personal care plan, more people than ever before are quitting smoking and are able to benefit from keyhole surgery. The focus on quality has moved up the agenda for staff and organisations, supported by the promotion of innovation through the introduction of tools and programmes such as NHS Evidence, the Innovation Fund and the Challenge Prizes. Yet there is more to do. In order to drive the quality agenda further and free up clinicians time to do this my report includes proposals on.

Refining of targets based on evidence - We want to free up front line staff so they can focus on delivering high quality care. We will remove the obsolete 13 week outpatient and 26 week inpatient targets, as well as reviewing data collections across the board in order to reduce the burden; Clinician Budget Ownership - We will look at giving clinical teams in the acute sector ownership of their budget. This will promote entrepeneurship and innovative delivery of services built around the needs of the patient; Peer Review Accreditation System - We will create a new voluntary peer review system in which clinicians will judge the standard of their peers in order to drive up quality and achieve a ‘gold standard’ of care. The system will be developed through close collaboration and consultation with the relevant partners and stakeholders.
What does Change4Life do for you? The Change4Life advertising campaign will make the subject of weight and physical activity a hot topic and will urge us to make changes to our diet and levels of activity. The campaign talks about ‘fat in the body’ rather than ‘fat bodies’ and makes clear the link between fat and preventable illnesses. It also pins the blame on modern life, which affects all of us, rather than pointing the finger at parents. Shaping the Future of Care Together, the Green Paper on the reform of Adult Care and Support in England. The Green Paper sets out a vision for a new National Care Service. of care and support needs. In the Green Paper it suggests the following three funding options: Partnership You will be supported by the Government for around a quarter to a third of the cost of their care, more if you have a low income. This system would work for people of all ages. Insurance The Government will cover a quarter to a third of the cost of care, and also make it easier for you to take out insurance to cover remaining costs. This system would work for people over retirement age. Comprehensive Everyone pays into a state insurance scheme, whether or not they need care, everyone gets care free when they need it.
What is in the Green Paper? The Green Paper describes six things that the Government thinks every adult should be able to expect from the National Care Service. Prevention services The right support to help you stay independent and well for as long as possible and to stop your care needs getting worse. National assessment Wherever you are in the country, your care and support needs will be assessed in the same way and you will have the same proportion of your care paid for. Joined-up services All the services that you need will work together smoothly, particularly when your needs are assessed. Information and advice You can understand and find your way through the care and support system easily. Personalised care and support The services you use will be based on your circumstances need, preferences. Fair funding Your money will be spent wisely and everyone who qualifies for support will get some help meeting the cost
What happens next? The reforms envisaged in Shaping the Future of Care Together will represent a challenge for many organisations. There is no universal, one-size-fits-all solution. No two areas, organisations or people are the same. There will be variations in the kind of system needed in different parts of the country, just as each service user has their own preferences in how care and support is provided for them. The reforms we implement must be sensitive to this. We know that the current system is unsustainable. Often the front line is where the cracks in the system are most visible. The workforce see the system’s shortcomings and their results close up, and bear the brunt of complaints. The Power of Choice
A dramatic expansion of patient choice in the NHS got under way in April 2008. Surveys have consistently shown that patients want choice. The 2005 British Social Attitudes survey revealed that 65% of patients said they wanted choice of treatment, 63% wanted a choice of hospital and 53% welcomed a choice of appointment time. The introduction of free choice means that patients referred to see a specialist can choose where they will be treated. list includes many private hospitals as well as all NHS providers. Between them they offer everything from treatment for cataracts to open-heart surgery. Under free choice, if you and your GP decide that you need to see a specialist, you’ll be able to choose the hospital that best suits your needs. FACS Fair Access to Care: The Four Bands of Need The Department of Health has issued guidance to local authorities to help them decide who is eligible for social care services. Local authorities must decide what levels or bands of need they will meet. The guidance describes four bands of need: Critical, Substantial, Moderate, Low. Your health and social care needs We use a Single Assessment Process to help make sure you receive the right health and social care support at the right time to ensure that you don’t have to keep answering the same questions repeatedly. The local authority use the single assessment process in conjunction with the Health Service to assess your needs. For example: What can you do for yourself? What can you do if given some help? If you have someone looking after you, what do they feel you need? What is the best way of meeting your needs? Carers can ask for their own separate assessment. During the process we find out your health and social care needs. We then plan with you the best way for you to get support for you to live safely and independently wherever possible. Health or social care staff work with you to identify your needs using different levels of assessment. Our aim is to use the right levels of assessment for you. Who can carry out the assessment? One or more health or social care professionals can carry out an assessment with you. This includes a district nurse, social worker, occupational therapist, physiotherapist or specialist nurse. How are you involved? We discuss your needs with you and listen to what you have to say. We consider your views when planning the support you need. You can involve your family and/or other carers. What happens after the assessment? We agree a plan of action with you. This sets out what is required to meet your specific needs. Do I have to pay? A charge is made for most care services. Usually the amount you pay will depend on how much capital and savings you have, although this varies from service to service. Our staff will be as sensitive as possible when discussing your financial situation and any charges that apply will be in line with fairer charging. What if you are unhappy with your assessment? You have the right to complain or comment about the way your assessment has been undertaken. See below for an explanation of the legal services you may require. Lasting Powers of Attorney Anyone who faces the possibility of losing mental capacity should make a Lasting Power of Attorney. An LPA appoints someone you trust to make decisions for you. Separate LPAs may be made for both financial and for welfare matters. The process of making an LPA is quite involved because it incorporates safeguards minimising the risk of financial abuse. Deputyship Where someone has already lost mental capacity, it is often advisable to obtain a court order so that a trusted person, called a “Deputy”, can manage that person’s finances and take decisions on their behalf. Papers are required to make an application to the Court of Protection for a Deputy.
Management of Affairs If you are elderly or disabled and struggling to cope, it is ideal to have a caring adult son or daughter to look after your affairs. For those who are not lucky enough to have a carer, legal advisors can provide a good alternative. Such support mechanisms can; administer the person’s finances, ensuring that available state benefits are claimed, bills paid, and tax returns submitted; instruct outside agencies to assist, such as tradesmen, personal carers and social workers; do everything possible to enable an elderly person to remain at home, but if a move into residential care becomes inevitable; choose the best available residential home and manage the sale of the house. Probate Probate is the court’s authority; given to a person or persons to administer a deceased person’s estate. The document issued by the Probate Service is called a Grant of Representation. Funding The Cost of Care Caring for elderly and disabled people is expensive. There are several possible sources of funding. Sometimes you may be able to obtain all the care fully-funded by the NHS under Continuing Care. Sometimes funds will be available from your local authority, but on a means-tested basis. Some local authority services can be provided on a discretionary basis, and others are mandatory. It may be possible to obtain funds against the value of your house through an equity release. We will guide you through the continuing care maze but also help you investigate other means of obtaining funding. And perhaps help you keep the house. Deprivation Of Liberty Sometimes elderly people who have lost mental capacity are required to move into Residential Homes in their own best interests. But mistakes are sometimes made, and it is believed that many people are kept in residential homes against their wishes and without sufficient justification. Types of Care Home You may choose to go into a care home (sometimes called residential home) if you can no longer manage in your own home. Everyone will have different views about what they think is a ‘good’ home, and each person’s needs will differ. Here are some questions to consider. Some may be more important to you than others. • does the home encourage residents to do as much as possible themselves and to make choices about as many aspects of their daily lives as they can? • can residents bring personal possessions? • can the home meet the resident’s communication needs - for example, language other than English, Strip2 15/5/07through 11:59 aPage 1 or non-verbal method, eg. sign language or large print?

• do residents choose what and when to eat? How are special diets catered for? Can Residents eat privately with guests from time to time? Can residents prepare any food and drinks themselves. • are residents free to see visitors when and where they choose? Can visitors stay overnight at the home, if they have travelled long distances? • can residents use a telephone in privacy? • do residents rise and go to bed when they choose? If not, do you consider the arrangements to be reasonable? • what provisions does the home make for taking residents out - on outings, to the shops, to the theatre, etc? • what physical activities are available for residents? • is there more than one living room, so that there is a quiet room as well as one with a television? • are there books, newspapers available for residents? • Do residents visit the library, or is there a mobile library come? • do the managers of the home ask about how the resident would like to handle money or medicines? • how does the home ensure valuable items are kept secure? • do the residents have their own GP’s? • is there a residents committee? • does the home encourage residents to say how they feel about living there, and provide written information to residents and/or their families about how to discuss a problem or make a complaint. • are toilets available in all parts of the home, fully equipped with handrails and other helpful equipment? • can wheelchairs go everywhere within the home, and easily in and out? Is there a lift? • if the resident needs help bathing, does the home have suitable facilities? Who will help bathing, and can the resident choose how often they have a bath? • do you have to pay extra for toiletries? • are there any areas for smoking and non-smoking? • what happens if residents require more or less care than they currently have? Might they have to leave? What arrangements are made for funerals, and for payment if the resident dies? • can residents help in ordinary activities of the home? • are residents allowed to bring their pets?

Paying for Services in a Care Home We understand making a move to a care home can be a daunting task. This article will be helpful if you are an adult who needs help with paying for services in a care home. You may already be living in a care home, paying your own fees and find that your savings are reducing, or you may be considering moving into a care home. Before you move into a residential or nursing home it is worth considering your eligibility for NHS continuing health care. If you have a primary need for health care it is possible that the NHS Primary Care Trust will fund your care and accommodation. To find out more please contact your local Primary Care Trust, see page 8. Your local authority Bournemouth Borough Council, Borough of Poole Council, or Dorset County Council need to work out how much you will have to pay towards the cost of your care. Each person requiring funding must have their care needs assessed and must meet the eligibility criteria for funding. The amount a person has to pay will depend on their individual financial circumstances. The rules for deciding how much people pay towards service in care homes, and the capital limits for funding are laid down by the central government in the ‘Charging for Residential Accommodation Guide’. Currently people with more than £23,000 capital* will be required to pay the full care costs although they may be eligible for free nursing care. Please note that if you have been assessed as needing continuing NHS care services, or you have been placed in a care home under section 117 of the Mental Health Act 1983, you will not be charged for your care whilst you continue to be eligible for funding. *Rate as of April 2009

Financial assessment If the authority assess that you require services in a care home and you wish to apply for funding for your placement, they will work out how much you will be required to pay towards the care costs. This is called a financial assessment. A Benefits Officer will either visit you, or a person you have nominated to act on your behalf, to collect information about your income and savings. The visiting officer will also ensure that you are receiving all the benefits you are entitled to. Any information provided will be treated in confidence in accordance with the Strip2 15/5/07 Data Protection Act. 11:59 Page 1

What the Council will need to see It would be helpful if you could have the following information available for the visiting officer: Income • Letter from the Department for Work and Pensions giving details of all the benefits you receive, e.g. Retirement Pension, Income Support, Pension Credit, Guarantee and Savings Credit, Attendance Allowance, and/or a bank statement showing these payments • Notification of any private pension or annuity • Details of any other income you may have Savings • Bank Statements • Building Society Books • National Savings Books • Premium Bonds and National Savings Certificates • Stock and Share Certificates • Details of endowment insurance policies • The value of land or property (other than your own home) that you own • Details of any other savings and investments Calculating your charge Any savings below £14,000 are ignored. For savings of £23,000 or less but more than £14,000, we will assume an income of £1 per week for every £250 or part thereof. E.g. If you have savings of £16,250 we would ignore the first £14,000 leaving £2,250. We will assume an income of £9 a week. This is called ‘tariff income’. The charge you will be expected to pay will be your total weekly income, including any tariff income, less the statutory personal allowance, currently £21.90 weekly. If you are over 65 you may qualify for an additional savings disregard of up to £5.65 weekly. Please note that some income is disregarded from the assessment. For example, 50% of any personal pension you may have if you are maintaining your spouse at home, and if your stay at the care home is temporary, we will make extra allowances for the upkeep of your own home. Your home When you move into a care home on a permanent basis the value of your home is disregarded for the first 12 weeks. After that we may include the value of your home, as savings, but

placement. If your disposable capital runs out before you can sell your property, it is unlikely that the local authority will be able to make up the shortfall in your care costs. Your care manager should be able to advise you about this. Deferred payments If your home is included in the assessment of charge you do not have to sell it. If you do not want to sell your home or can’t sell it quickly enough to pay for your care you may apply for the deferred payment scheme. Under this scheme, the Council places a legal charge on your property and in return pay your care costs, less any charges that you can pay from your weekly income. The amount that the Council pay will accrue as a debt, just like a mortgage, although no interest is added while you remain in the care home. The debt must be repaid by you or your estate as soon as the property is sold, or within 56 days of you leaving care.
this will depend on your individual circumstances. We will not take the value of your home into account if someone else lives in it who is: • Your husband/wife or partner • A relative who is aged 60 or over • A relative who is disabled • A dependent child We do have discretion to disregard the value of your property in exceptional circumstances, for example if there is a person living in the property who has no other home and has been your long term carer. Information about choice of home If you would prefer to live in a care home and the fees are more expensive than the amount that has been agreed for your placement, you can still be funded in the home if the care manager agrees that the accommodation is suitable and providing that another person is able to meet the difference in costs. This is called a ‘top-up’. The legislation prevents residents ‘topping up’ their own fees if they choose more expensive accommodation, although there is one exception to this rule. The exception is that if you own your own home you will be allowed to live in more expensive accommodation and ‘topup’ your own fees during the first twelve weeks of a permanent

Learning Disability Services Dorset Learning Disability “Supporting people to be part of their community” Partnership Board Bournemouth Borough Council, Borough of Poole Council, and Dorset County Council, provide care where necessary and give support to enable and encourage people to reach their full potential in all areas of their lives (to contact your Local Authority see page 13). Daytime opportunities and services Resource centres provide a range of day services for people with varied abilities, covering personal needs, practical living skills, physical and leisure activities and community living. They are places where people can socialise, make friends and take part in paid and voluntary work. Training and employment The Stepping Stones scheme offers people with learning disabilities, who are aged 16 years+, access to training and employment activities in a supportive environment. Care services Care homes for people with a learning disability offer long term care for people with more intense needs. Our Adult Placement Scheme enables people to live in the community, in the homes of families or individuals, who can offer support and care. The scheme can provide long or short term stays. Short term and respite care We can arrange for people to have short breaks staying in our care homes from time to time, offering them different experiences and to give their carers a break. Supported living For people who can live more independently we can help arrange tenancies where support is provided. It is sometimes possible to arrange housing for individuals or groups of people through district councils and housing associations. The Direct Payments scheme People eligible for our community care services can choose and buy themselves services with an allocated sum of money. The LDPB board meet every 3 months with the focus of improving services for people with learning disabilities. The board is formed by people from the NHS, social services, Dorset People First, carers groups and many other organisations. LDPB work in the following areas Health, Housing, Person Centered Planning, Fulfilling Lives, Personal Budgets, Moving On, Employment, Transitions, Family Carers, Safeguarding. ‘Moving On’ with the NHS Dorset HealthCare NHS Foundation Trust provides mental health, learning disability, addictions, community brain injury and community dental services for the majority of Dorset, including Bournemouth and Poole. Dorset Health Care NHS Foundation Trust’s Learning Disabilities Intensive Support Outreach Team was established in the west of the county in 2006 and a similar provision will be provided to the east of the county from April 2010. The West Dorset service provides support, assessment and treatment to all adults with a learning disability and complex behaviour living in the community. Across the whole of Dorset, the Trust is providing specialist support for individuals as they move on into the community under the ‘Moving On’ programme and assisting them with accessing mainstream services. Are You a Carer?
Carers in Dorset
A carer provides help to someone else who needs it. This may be physical, emotional or social care. It may involve a lot of daily input or more occasional help. Care would be provided for more than 15 hours per week and be regular and substantial. The carers input will be essential for the person cared for to remain in their home environment. A carer may live with the person that they help, or they may be some distance away. Carers can be of any age but will provide the help voluntarily. The person they help may be a relative or non-relative (for example friend). Who is eligible for help? Carers are eligible for an assessment if it is considered that they meet the criteria to be registered as a carer. An assessment will be undertaken by a member of the Community Care team or a specialised carers case worker and may lead to a support plan for the carer. If the person the carer is looking after is known to Adult and Community Services then the carer should be consulted about the care plan for that person. However the person being cared for does not need to be known to Adult and Community Services for the carer to have an assessment. Support to carers is available from several sources. If you, or the person you care for, are already in contact with Social Care and Health, you can contact your social worker or care manager for further information or advice. The NHS locally identifies and supports carers through GPs and the primary care services they provide. Dorset Carers Strategy The Dorset’s Carers’ Strategy is a joint initiative involving Dorset County Council Social Care and Health Directorate, the Primary Care Trusts in Dorset and relevant voluntary sector partners. Carers were consulted on what they perceived to be the main themes for developing services and thirteen areas for action have been identified. An action plan details how these can be taken forward. Carer Support Programme St John Ambulance provide a carer support programme to help people in a caring role. The programme is provided free of charge and includes lunch. Open to all carers over the age of 18 who provide voluntary care. Would you like the opportunity to gain more skills and knowledge to help you in your caring role? Do you need training in stress management, basic first aid, safer moving and handling and how to prevent falls? Do you want to meet other carers to share experiences, exchange advice and ideas and at the same time make friends? These guidelines are not enforceable by law, but providers should abide by them. They also give service users the ability to judge care providers to ensure the correct service is employed. Nurses agencies send nurses to visit people at home when they need specialist or medical care, usually after leaving hospital. The frequency of the visits can vary from several times a day to once a week. There are two types of nurses agency: • The first type simply acts as an employment or introductory agency which places self-employed nurses into employment with people who need nursing help • The other type of agency employs, trains and supervises its own staff.

Obtaining Home Care Services Home Care Services provide a personal and practical service for people, to help them stay independent, and remain in their own homes. Your local authority’s own staff may provide Home Care, or they may ask other organisations to provide the service, to the same high standards. The Home Care Service operates 365 days and provides help according to needs. Is Home Care available to everyone? Your local authority, Bournemouth, Dorset, or Poole, have a duty to assess anyone who is eligible for care services. Due to resources they are not able to offer services to everyone. To ensure services are provided fairly ‘Fair Access to Care Services Criteria’ guidelines are used. Needs are assessed to find out if they qualify and at what level of priority.
Care at Home - Care in Your Own Home
You may be eligible for Home Care if: • You find personal tasks difficult and need essential help to stay independent • You need help because of a disability • You care for someone who is elderly, or disabled, and can’t look after themselves • You are a family with young children having temporary difficulties • You are living at home and will be at risk, if assistance is not provided to you What kind of help can you get? Care staff may provide help with any of the following: • Getting up and going to bed • Washing and dressing • Making sure you have something to eat • Going to the toilet If you are a carer, we may be able to give you support by sharing some of these tasks, or provide the back-up you need to be able to take a short break. We1are not able to provide household cleaning, or shopping, as services on their own.

How much help can you have? We will make an assessment of the level and type of help you need and discuss this with you when we first meet. We match the help we give as closely as possible to each person’s changing personal circumstances. How can you get help? If you think you could be eligible for Home Care, you (or a friend, or relative, on your behalf ) can contact your nearest social care local office (see page 58). Your doctor or health visitor may also contact us on your behalf. Making changes If you want to make a change to your Home Care programme, please give us at least 48 hours notice. (We realise that in an emergency this may not be possible). We ask you to keep us up-to-date with any permanent or temporary changes in your circumstances. We can quickly adapt the service to suit you. Having your say You can tell us what you feel about our services at any time, but at least once a year you (with your carer if you have one) have a formal opportunity to comment on the service provided/received and to review any changing needs. Care in Your Own Home - Care at Home
Homecare, also known as domiciliary care, enables people who need assistance in their personal care to remain in their own home, rather than move into a care home or care home with nursing. Sometimes homecare is temporary, after a stay in hospital, and sometimes it is needed on a longer-term basis or to supplement the care already provided by a member of the family or friend. Homecare is intended to help people remain as independent as possible and enables them to stay in familiar surroundings, close to their friends, family and the local community. It can be purchased from a homecare agency independently, provided directly by the local authority or purchased by social services from local homecare providers. Homecare workers are specially trained to work in people’s homes and assist with getting up and going to bed, washing and dressing, shopping, meal preparation and assisting medication. a vocational qualification in care and have specialist training on caring for people with learning disabilities or dementia. Homecare workers can visit for as little or as long and as often as needed, up to a ‘live-in package’. Working hours are geared to the normal daily routine from early morning to late evening. All UK-based agencies can join United Kingdom Home Care Association, the professional representative association committed to ensuring the highest standards of homecare. Meals At Home Appetising, healthy and nutritious meals are important, and if you are unable to prepare them yourself lunch clubs and day centres may provide meals. Another option is to have meals delivered to your door. Meals-on-Wheels providers are listed below. When organising a meals-on-wheels or meal provision service you should check that: • •
Meals are of a high standard. They will be enough for you as a main meal each day, and have a pudding as well as a main course. You can ask for a special type of food because you are vegetarian, or have a special diet because of your health, or because of your religion, beliefs or culture.

It may be possible to have hot and frozen meals delivered. Frozen meals can then be heated when necessary. Meals should be balanced and nutritious, as well as appetising and attractively presented. Protein, vitamins and minerals should be emphasised in the diets of older people.
Special dietary requirements related to health, culture and personal preference should be taken into account. This may involve providing, or buying in, specialised meals. Most needs can usually be catered for with careful planning and preparation. Identifying individuals’ needs before they start to receive meals is essential. Consultation is also important with the local community, for example, to find out if there is a need for provision for ethnic minority groups. A meals on wheels service usually delivers food but does not serve it. Frozen meals can be delivered once a week for the week then heated daily by the person or their carer. For hot meals, deliveries will have to be phased daily, bearing in mind how long the food needs to be kept heated, and by what time the meals need to arrive. • Malnutrition affects over 10 per cent of older people. (British Association for Parenteral and Enteral Nutrition, 2006) • Malnutrition is estimated to cost the UK over £7.3 billion a year. (BBC, 2006) • It is estimated that up to 90,000 people who receive home care services could be at risk of malnutrition. (Grove. K. (2008), Sutton: UK Home Care Association) • When malnourished older people are admitted to hospital they stay in for much longer, are three times as likely to develop complications during surgery, and have a higher mortality rate. (Age Concern, 2006, BBC, 2006) • Routine screening for risk of malnutrition across health and Social services is a key part of good nutritional care. • Giving older people the time, help and encouragement they need to eat can help tackle malnutrition. • Older people’s preferences and their dietary and cultural requirements need to be taken into account for planning meal times.

Dorset County Council has launched a campaign designed to raise awareness of malnutrition and the nutritional care of all older people who receive care services – in residential homes, within the community receiving domiciliary care or who live in sheltered housing. “Dorset has been in the forefront of national developments in nutritional care for older people. Malnutrition effects up to one in three older people living in the community, care homes or hospitals and costs the nation £7.3 billion per annum. Dorset raised these issues with central government and has played an integral part in the “Improving Nutritional Care Action Plan” produced by the Department of Health and Nutrition Summit stakeholders. Dorset County Council aims to pass on the information that we have learnt so far. The Nutritional Summit is going to affect us all so come along and find out how you may be able to develop your services. National Dementia Strategy The NDS was launched on 3rd February 2009 to transform the quality of dementia care, including improved community personal support services and domiciliary care services. There are about 700,000 people with dementia in the UK. Most people with dementia are over 65 years old, but there are at least 15,000 people under 65 who have the illness. The strategy aims to develop services for people with dementia and their carers that are fit for the 21st century, with services that meet the needs of everyone, regardless of age ethnic group or social status. 1. Assessment service for loan of equipment Following an assessment (Adult Access Team 01305 221016), if you qualify for funding under the Fair Access to Care Services (FACS) criteria, equipment can be loaned to you through the Integrated Community Equipment Service Advice on disability products from Dorset Council If you need information and advice on disability products, national and local suppliers, organisations and services, we can help you find it. A wide range of products relating to the following are available for demonstration and assessment purposes: Bathing and showering products; Armchairs and cushions; Wheelchairs and walking aids; Meal preparation; Feeding and drinking products; Beds and associated products; Clothing and continence; Moving and handling products; Telecare products. Call Alison Walker 01305 257164. Dorset Integrated Community Equipment Service The way aids for daily living will be supplied to people who meet the eligibility criteria has changed. The criteria levels will be explained during the assessment by a professional. Due to the continued pressure on resources it is likely that the equipment will be funded only if the full set of criteria is met. Dorset, without Bournemouth and Poole, will start supplying smaller items of equipment via a network of accredited retailers in the region. This will give the individual greater choice as they will be able to choose from a range of similar products. Community alarm services Telecare enables individuals, their carers and families to retain better control, privacy and peace of mind through community alarm systems, which most people know as Careline. Careline operates through a telephone line, alerting a 24 hour call centre when a pendant alarm is pressed. The Dorset Association for the Disabled is affiliated to R.A.D.A.R. and was set up in 1951 to provide additional services and social amenities for disabled people in the county of Dorset. Its aims were, and still are, to complement and supplement services provided by the statutory welfare authorities with whom the Association has enjoyed a long and harmonious relationship. From one small branch the Association has now spread throughout the county providing help and information to people with disabilities and their carers via fourteen branches.

y the mid 1970s most households in the country owned a car. But disabled people B were missing out. As a result, many people were housebound for long periods and often dependent on others for their mobility. The Scheme is available to anyone who receives either of the following benefits, and has at least 12 months’ award length remaining when they apply:
Before the introduction of the Mobility Allowance in 1976, only disabled people who could drive themselves got any government help with personal transport. It was usually in the form of a small car provided to disabled couples, to people responsible for the sole care of a disabled child and to a category called “war disabled”. An allowance was paid to disabled people who actually owned a car. But most people were supplied with a small, blue, single-seat, three wheeled, motorised “invalid trike”, which was incapable of carrying passengers.
Motability - Get Around in Dorset
Today, The Motability Scheme enables disabled people to use their government funded mobility allowances to obtain a new car, powered wheelchair or scooter. Some 450,000 people currently use the Scheme and in 2006 Motability celebrated providing two million vehicles since it was set up by the government in 1977.
• Higher Rate Mobility Component of Disability Living Allowance. • War Pensioners’ Mobility Supplement. Around two thirds of our customers drive, but non-drivers can get a car as a passenger. Similarly, parents and carers can apply on behalf of a child aged three or older. Some customers need extra financial help to meet their mobility needs, for example, to help towards the cost of the advance payment for a suitable car, adaptations, driving lessons or perhaps a wheelchair accessible vehicle. The Blue Badge Scheme provides a national range of parking concessions for disabled people with severe mobility problems who have difficulty using public transport. You are automatically eligible to apply for a badge if you are over two years old and either: • receive the Higher Rate of the Mobility Component of the Disability Living Allowance • are registered blind • receive a War Pensioner’s Mobility Supplement • you may also be eligible for a badge if you are over two years old and either: • have a permanent and sustainable disability which means you cannot walk, or which makes walking very difficult • drive a motor vehicle regularly, have a severe disability in both arms, and are unable to operate all or some types of parking meter If you are a parent of a child who is less than two years old, you may apply for a badge if the child must be accompanied by bulky medical equipment or needs to be kept near a vehicle at all times. The concessions provided under the scheme apply to on-street parking only. Badge holders may park on single or double yellow lines for up to three hours in England and Wales, except where there is a ban on loading or unloading. You must display both your Blue Badge and parking disc showing time of arrival. Badge holders may park for free and for as long as they need to at on-street parking meters and pay-and-display machines, unless there is a traffic sign specifying a time limit for holders of Blue Badges. Where a time limit is in force you must also display the special blue parking disc showing time of arrival.

Free Cinema for Carers
Activity & Therapy - Be Active, Be Healthy in Dorset
Carers who look after a disabled person may be entitled to go to the cinema for free. The Cinema Exhibitors’ Association card (CEA) is a national scheme, however not all cinemas subscribe (see map opposite). To qualify for the card the person must be getting DLA and AA or Registered blind, or the holder of a disabled persons rail card. Continuing the good work of Stevie B... The Steve Bernard Foundation was formed following the tragic passing of the affectionately named Stevie B on the 2nd November 2005 in a car crash near Chichester. Steve was killed along with two fellow students from Chichester University. They were all studying to become PE teachers. The foundation aims to mirror Stevie’s desire to bring sporting happiness to those around him and to distribute and support local sporting causes for youngsters in and around the Bournemouth area. So far we have raised over £50,000 through a variety of events such as comedy nights, valentine disco’s (Stevie was born on valentines day), sporting dinners and a fancy dress fun run on Bournemouth sea front to celebrate what would have been his 21st birthday. We have now helped and supported hundreds of youngsters in 14 different sports and we have also supplied sporting equipment for local groups to distribute in oversea’s projects. Our next money raising events are another fancy dress fun run/walk in February, a bike ride to Bayeux in France (and back!) in June and a boxing event at the Opera House in October. Tim understands the Sportability Experience from the inside. The first symptoms of his MS appeared in 2000, but it was not until 2001 that he was finally diagnosed. With his enthusiasm and positive approach we were delighted when Tim agreed to take on a more active role as our Area Organiser. Since then, Sportability in Dorset has had one of the most exciting and diverse programme of events - and this continues to develop. After shooting with Sportability – he now shoots regularly at the Christchurch Gun Club. He has also taken up a new sport - horse riding. Tim’s mantra is: One life. Live it. Riding for the Disabled
Physical activity is something that matters to all of us.
Riding for the Disabled Association (RDA) is open to all disabled people who would like to horse ride. All riders need a medical cetificate and a test ride, T: 0845 658 1082 . RDA and other riding centres are listed on page 47, these centres have stated that they are able to accommodate disabled riders but it is essential to make arrangements in advance to make sure that your requirements can be met.

Harbour Challenge is a charity providing sailing tuition based on Brownsea Island. It has received £13,100 from the Community Foundation to fund new boats and project costs to provide sailing and kayaking courses to children and young people from disadvantaged backgrounds locally. Funding from the Community Foundation’s Local Network Fund has allowed over 100 young people to benefit from this project last year. Open the door at Pebble Beach and discover a gem of a restaurant in Barton on Sea. Since Mike Caddy established Pebble Beach as a charitable trust in 2005 over £45,000 has been raised in donations. Fifty percent of the monies are given to local Hampshire and Dorset charities while the other half provides computers and educational equipment to schools and youth projects in Brazil. Dorset Advocacy Awareness of advocacy has grown considerably since Dorset Advocacy started in 1993. However it is still not always understood. We have written this leaflet to explain how advocacy works, and how you as a carer can contact us if you require further information. Here are some of the questions that relatives and carers ask about advocacy: What is Dorset Advocacy? Dorset Advocacy exists to create and support one to one partnerships between people who have a learning disability and volunteer advocates. Through these partnerships, people who have a learning disability can speak up, promote their interests, and play a fuller part in the life of their community.
Advocacy - Representation for health and care
How do you decide who needs an advocate? Dorset Advocacy staff will spend a lot of time talking to people who have a learning disability. If we think someone needs help in making their needs and wishes known, we ask if they would like an advocate. What do advocates do? Advocates have a range of roles which vary according to the needs of the person they help. They may attend ISP meetings and case conferences, making sure that their partner’s needs and wishes are clearly stated. They may help people try out new interests and opportunities. An advocate never imposes their own ideas on the person they help; rather, they support

that person in making their own choices. Where an individual cannot make their wishes known, an advocate may speak up for what they perceive that person needs. How long does it take to find an advocate? Advocates are volunteers, so we cannot guarantee a specific waiting time. We may find a suitable volunteer quite quickly, or it may take some months. We will keep in contact with the person who needs an advocate during this time. How are advocates chosen? Dorset Advocacy staff interview advocates and get to know their interests and background, two references are obtained and a Criminal Record Bureau disclosure is carried out. No one becomes an advocate unless we are satisfied they are reliable. The partnership does not go ahead if either person is not happy after initial meetings. Will I be introduced to a new advocate? Once an advocate has been found for the person you care for, we normally arrange for you to meet them at an early stage, so that you can get to know each other and exchange addresses etc. The only exception to this would be where the person you care for asked us not to make such an introduction. 01305 251033 | enquiries@dorsetadvocacy.co.uk

Help and Care is an independent charity who receive funding from Dorset County Council in order to provide support services to adult carers and carers groups. The service is open from Monday to Thursday (9am to 5pm) and on Friday (10am to 4.30pm). Outside of these hours callers can leave a message which will be dealt with when the service re-opens. A charity led by people who have learning difficulties. We support each other to “speak up and lead change”. We work across Bournemouth, Poole and Dorset. Our projects include: The Forum has over 100 members. We come from all over Dorset and we are all adults with learning disabilities. We have regional meetings, and a “Making Changes” committee, and we are represented on Dorset’s “Learning Disability Partnership Board”. Total Communication is about having lots of communication methods available, because everyone is different! Methods include; Signing, Photographs, Using Easy Words, Symbols, Objects, Intensive Interaction. Just Say runs Self-Advocacy groups where young people aged 13 - 25 years can meet and speak up about things that are important to them. There are 9 groups running at the moment. In the Speaking Up groups people can talk about anything that is important to them. Keeping Safe & Citizenship Lots of people with Learning Disabilities have told us that they do not feel safe when they go out. This is because many people experience harassment and crime. Dorset People First is running a citizenship project to look at ways we can make people be and feel safer when they are out. Some of the things we are doing are; Looking at what is being done elsewhere in the country; Helping people with learning disabilities to know their rights and to report crime; Working with the police to let them know what the problems are and how it makes people feel. On 1 April 2009 the four national Age Concerns in England‚ Scotland‚ Wales and Northern Ireland joined with Help the Aged to create four new national charities dedicated to improving the lives of older people. We all speak with one voice on behalf of all older people. Day Services Day services offer care for adults with physical and/or learning disabilities and people recovering from mental illness. They offer social contact and stimulating activities for people who are finding difficulty living at home, aiming to give them the skills they need to live as independently as possible. Day care also enables carers to have a break from their caring. Centres are run by local authorities, the NHS, voluntary organisations and commercial businesses such as care homes (see pg 15). Respite Care Caring for someone at home may sometimes feel like having a full time job and you might feel that you need to take a break from time to time. Respite Care for the person you look after may give you the opportunity to do just that. It may be possible to admit them to a residential or nursing home for a short period of time, giving you time at home or perhaps
enabling you to take a holiday away. You may also like regular short breaks for a few hours to pursue a hobby or take time for yourself, or perhaps you would just like a good night’s sleep. There are a range of services that can provide trained people to look after the person you care for just such purposes, during the day or night. Direct Payments Scheme An alternative way of providing care that Social Services would normally put in place. Instead of receiving a service, those eligible can receive a payment to purchase their own care. Direct Payments can be used to pay for agency home care, day activities or respite care. Many people prefer this as it gives more control, choice and flexibility in the way their needs are met. Carers can also receive Direct Payments to meet the eligible needs within their assessment.

However, when demands on the carer increase, it is often very helpful to have a way of organising short breaks. There are several ways that you can get a break from caring, from an hour or two, to several weeks; depending on what is appropriate to you and the person you care for. In order to qualify for this service, carers will need to meet set criteria and will then be registered. Those approved to receive the service will then be referred to Help and Care, a local independent charity, who have been commissioned to provide this service on behalf of Dorset County Council. The sitting service is provided through approved care agencies and there is a maximum hourly allowance and a small hourly administration charge payable for this service. Day care The person you care for may be able to attend a day centre, or a care or nursing home, depending on their needs. Transport for day care can sometimes be arranged. Longer breaks The person you care for may be able to spend time away in a care or nursing home, depending on the level of care they need. This could be for a weekend, week or fortnight as needed, and can be booked in advance to ensure both carer and cared for person receive regular respite. It may be possible for someone to come and ‘live in’ with the person you care for, to take over your caring tasks while you take a break for a few days or longer. Making choices Within the different approaches outlined above, there are various schemes tailored to meet the needs of people with particular needs, such as those with a learning or physical disability who may have specific care needs.

Day activities Where the cared-for person needs daytime activity we may be able to arrange for them to attend a day centre or other type of community activity. nformation on Sheltered Housing (Retirement Housing) in Dorset is provided in this section. It is likely that there will be a waiting list for rents and purchasing, it is advisable to contact the preferred scheme early.
Schemes and options Sheltered housing is often called retirement housing or warden-assisted housing. There are many different types of sheltered or retirement housing schemes, both to rent and to buy. Schemes usually consist of between 15 and 40 dwellings which may be bedsitting rooms, self contained flats, bungalows or luxury apartments. Usually they are designed with older people in mind and to mobility standards, for wheelchair access for example. Some schemes are simply housing designed to meet the needs of older people and linked to a community alarm. But the majority of schemes have a Scheme Manager (or ‘Warden’) as well as a community alarm service. There are often communal facilities such as a lounge, laundry, guest flat and garden. Meals are not normally provided but a very few schemes include a restaurant, and quite a number arrange one hot meal a day. There is usually a minimum age, often 55 or 60, but occasionally 50. Sheltered or retirement housing appeals to people who like living independently but want the reassurance of knowing that assistance is on hand if there is an emergency. The duties of the Scheme Manager can vary considerably between schemes. Some schemes have a resident manager and a 24 hour service, others have visiting/part time managers. Scheme Manager (Warden) Over the last few years there have been many changes to the Scheme Manager’s duties. Most Scheme Managers are now expected to: • Manage the scheme • Summon help in an emergency. • Build up a relationship with older people living in their schemes; giving residents information on availability and access to services and encouraging them to ask for additional support from statutory and voluntary organisations when appropriate. Community alarms • Offer reassurance that help is at hand if needed • By pressing a button on a pendant or pulling a cord, a message is relayed to a monitoring centre, staffed 24 hours a day Strip2

• If help is needed the centre will alert relatives or friends, or contact your doctor or the emergency services. • Other facilities Renting Most housing for rent is provided by local councils or nonprofit making housing associations (often called Registered Social Landlords or RSL’s). Local councils and RSL’s generally aim to offer their housing to people in the greatest housing need, at rents which are affordable - perhaps with the help of housing benefits. The criteria that applicants are generally expected to show include: • Some degree of housing need because of the physical condition of their present home; or • Medical and/or social reasons for wanting to move, e.g. poor health, disabilities, loneliness, fear or isolation from friends & family; and • That they are not easily able, for whatever reason, to buy rather than rent; and • That (if they are not local already) they have good reason for wanting to move to the area they apply for. In most parts of the country, councils and RSL’s recieve applications from more people than they can assist. The cost of renting will be made up of a rent plus service, a charge to cover the services provided. Service charges in the majority of ordinary sheltered or retirement schemes is in the range of £20 to £40 a week*. People wanting to rent sheltered housing and who have a low income should contact local authority to check their eligibility for housing benefit. Buying You may also consider purchasing property. Prices and types of property vary enormously. A small second-hand flat may be found for under £70,000* in parts of the country. Brand new properties cost more. Luxurious homes on sites with every amenity - swimming pools, golf courses and restaurants - can cost well into six figures. Service charges are likely to apply as will ground rent which could vary from £50 per year to £300*. Joseph Weld Hospice Trimar Hospice Community Specialist Nursing Service Weldmar Hospicecare Trust is a local charity providing specialist palliative care, advice and support for patients and their families in North, South and West Dorset. All our services are provided free of charge. Our care is available for patients with a life-limiting or terminal illness and we support patients at home through our specialist community nursing service and our day hospices in Weymouth, Dorchester and Shaftesbury. Patients needing in-patient care come to Joseph Weld Hospice in Dorchester for symptom control, respite or end of life care. We also provide respite services for people with long-term progressive conditions. As well as our specialist doctors and nurses we also offer specialist expertise and skills in physiotherapy, occupational therapy, complementary therapy, art and music therapy. We have a Family Support Team with social workers, chaplain and bereavement support workers for both adults and children. Additionally, over 250 patient care volunteers help us provide our care. STARS - THE BLACKOUTS TRUST The STARS charity (Syncope Trust And Reflex anoxic Seizures) provides information and support to people who suffer with syncope and unexplained loss of consciousness.
