Somerset County Council (SCC) offers, directly or indirectly, a comprehensive range of support and care for its residents. They maintain, in partnership with the Residential Care Providers Association Somerset, a list of all care homes within the county, and also all of the independent care agencies which provide a wide range of support services for those who wish to remain in their own homes, but need some form of occasional - or regular - care and support. In addition, SCC provides a wealth of information to residents on services available to them.

Every care service is given a quality rating, from 0 to 3 stars. This makes it easy to see what the inspectors think of the overall quality of care the service provides. A three-star rating means ‘excellent’, two stars ‘good’, one star ‘adequate’, and zero stars ‘poor’.

The Care Quality Commission replaced the former social care regulator, the Commission for Social Care Inspection, in April 2009. Just as CSCI did, CQC regulates and inspects all care services for adults in England. Its inspectors inspect thousands of residential care homes and home care agencies every year and write a detailed report each time, setting out the strengths and weaknesses of that service.
When you look at CQC’s website, www.cqc.org.uk, you can search for care services in your area by postcode, type of service or their quality rating. You can view and download inspection reports, as well as other useful information and advice about choosing the best care service for your needs. Somerset’s Community Information Database Somerset Gateway holds information on clubs, societies, community and voluntary organisations and self help groups. Information includes contact details, venue, timetable, links to websites and general notes on the purpose of the organisation. Organisations may also attach documents, such as minutes of meetings, annual reports, or leaflets/posters.

Somerset County Council Adult Social Care Adult Social Care arrange social care services for older people, younger people with physical disabilities and people with sensory loss. We aim to support people in their own homes in the community wherever possible. Where that is no longer possible, our aim is to ensure the availability of a good quality residential or nursing home. Services are based on the principles of promoting independence and maximising choice. Most services are purchased from the independent sector, rather than being provided in-house. Fair Access to Care Services; for older people and people with a physical disability or sensory loss Fair Access to Care Services Somerset County Council provides a range of services for adults with care and support needs to help them to live as full a life as possible. To make sure that the way people get our services is as fair as possible we use the following principles: • People with similar needs will be helped in similar ways. • People will have equal access to services regardless of their age, the type of disability that they have, or where in the county they live. Families often provide the most support for vulnerable people. We take the needs of carers into account. We do not take their willingness to continue caring for granted. Call Somerset Direct 0845 345 9133 Eligibility for services The decision about whether you are eligible for services is based upon an assessment of your needs and how they affect your life. The assessment will help us to understand the degree of need, according to four bands: Critical, Substantial, Moderate or Low. We can only provide services if the degree of need you are experiencing is Critical, Substantial or Moderate. What sort of help can we arrange? We may arrange: • Support with some of the caring tasks in your home • Special equipment or changes at home to help you manage essential tasks or activities • Day services • Meals services • Accommodation in a care home if you are unable to remain at home • Short term care to provide you with a different and enjoyable experience and allow your carers to take a break. • We work closely with other organisations that provide health, education, employment, housing and leisure services. The kind of help we organise in partnership with these organisations aims to: • Help to develop or maintain skills you need to be as independent as possible • Support to develop employment skills and recreational opportunities, particularly where these help you to make better use of facilities in your community • Support for carers. Is there a charge for services? There is no charge for the assessment of needs, but there is a charge for some services that we arrange. Where there is a charge for any of the services you need, we may have to carry out a financial assessment to see how much you have to pay. Putting People First A shared vision and commitment to the transformation of Adult Social Care Older people, disabled people, and people with mental health problems demand equality of citizenship in every aspect of their lives, from housing to employment to leisure. The vast majority of people want to live in their own homes for as long as possible. The values of Putting People First Ensuring older people, people with chronic conditions, disabled people and people with mental health problems have the best possible quality of life and the equality of independent living is fundamental to a socially just society. For many, social care is the support which helps to make this a reality and may either be the only non-family intervention or one element of a wider support package. The time has now come to build on best practice and replace paternalistic, reactive care of variable quality with a mainstream system focused on prevention, early intervention, enablement, and high quality personally tailored services. In the future, we want people to have maximum choice, control and power over the support services they receive.

We will always fulfil our responsibility to provide care and protection for those who through their illness or disability are genuinely unable to express needs and wants or exercise control. Over time, people who use social care services and their families will increasingly shape and commission their own services. Personal Budgets will ensure people receiving public funding use available resources to choose their own support services – a right previously available only to self-funders. It might have been to offer an opinion on how things could be improved, or talk about an experience – good or bad. Well, now you have the opportunity. A LINk (Local Involvement Network) is a network of local people and groups. It listens to what people like you have to say about health and social care services in your area, and tells councils and the NHS what it has discovered. They are legally obliged to give the LINk any information it asks for and to respond to any recommendations the LINK makes, so that services can get better. The Somerset LINk wants to hear from you. You may want to comment on GP services, hospitals, mental health services, sexual health, day care, residential homes or older people’s services. Help and Care has been appointed the host organisation for the Somerset LINk. The charity’s role is to support the LINk to be an effective voice for people in the area.

Somerset NHS Trusts are responsible for addressing the health needs of people who live in the county of Somerset. They provide some of these services by employing our own teams of nurses and allied health professionals, treating patients in their own homes and in community hospitals. A&E (Accident and Emergency Departments) Assess and treat patients who have serious injuries or illnesses. Generally, you should visit A&E or call 999 for emergencies such as; loss of consciousness, pain that is not relieved by simple analgesia, acute confused state, persistent/ severe chest pain, or breathing difficulties. MIU (Minor Injuries Unit) If your injury is not serious, you are usually better off visiting a minor injuries unit (MIU) saving yourself unnecessary long waiting times and allowing staff in A&E departments to concentrate on people with serious and life-threatening conditions.
Patient Advice & Liaison Service - PALS As a patient, relative or carer sometimes you may need to turn to someone for help, advice and support. PALS aim is to help sort out any problems or concerns you may have when using health services. PALS works with patients and staff to improve our health service. They will do this by: • Offering advice/support to patients, their families and carers • Providing information on NHS services • Listening and responding to concerns, suggestions or queries • Helping sort out problems quickly on your behalf. PALS will always ask for your permission before personal information is discussed with others. They can talk to staff, management and other organisations on your behalf. The service is free and confidential. Only in exceptional cases where your safety or that of others must be considered will we have to pass on information. PALS will always tell you if they have to do this. Our aim is to agree the best way forward to deal with a problem and get it resolved quickly - if possible on the spot. NHS: Help and care beyond the hospital Continuing Healthcare What is continuing care? NHS continuing healthcare is the name given to a package of services which is arranged and funded by the NHS for people outside hospital with ongoing health needs. You can get continuing healthcare in any setting, including your own home or in a care home. NHS continuing healthcare is free, unlike help from social services for which a charge may be made depending on your income and savings. In your own home, this means that the NHS will pay for healthcare (e.g. services from a community nurse or specialist therapist) and personal care (e.g. help with bathing, dressing and laundry). In a care home, the NHS pays for your care home fees, including board and accommodation. Who is eligible for NHS continuing healthcare? Anyone assessed as requiring a certain level of care need can get NHS continuing healthcare. It is not dependent on a particular disease, diagnosis or condition, nor on who provides the care or where that care is provided. If your overall care needs show that your primary need is a health one, you should qualify for continuing healthcare. What if I am not eligible for NHS continuing healthcare? If you do not qualify for NHS continuing healthcare then you may have to pay for some or all of your care, although the NHS will still provide for your medical needs. You may have to take a local authority means test to decide how much you should pay towards your personal care and accommodation if you are in a care home. However, if you are a resident of a residential home and require care from a registered nurse, this will be provided free of charge through NHS-funded nursing care.
How will the assessment framework operate? The NHS will make the decision on eligibility for NHS continuing healthcare in collaboration with the local authority through a multi-disciplinary team and with the full and active involvement of you and your carers. A multi-disciplinary team is made up of two or more professionals who are involved in your care.
NHS-Funded Nursing Care What is NHS-funded nursing care? By law, local authorities cannot provide clinical services because the NHS is responsible for any care that must be provided by a registered nurse. For people in care homes, registered nurses are usually employed by the care home itself and, in order to fund this nursing care, the NHS makes a payment to the care home to cover the cost of providing the care for those who are eligible. Who is eligible for NHS-funded nursing care? You should receive NHS-funded nursing care if you: • have been assessed as requiring the services of a registered nurse • do not qualify for NHS continuing healthcare but have been assessed as requiring the services of a registered nurse • are not receiving registered nursing care in any other way What is happening to the nursing care bandings? Previously, there were three bands for NHS-funded nursing care, and the NHS had to determine which of these each individual would fall into. Under the new National Framework, the NHS will still have to assess your need for registered nursing and decide what services to provide, but there will not be any bands, nor will there be a need to have a separate assessment to decide the right band.

The Home From Hospital Service What is the Home from Hospital Service? It helps people to go back home after a stay in hospital. The service can give short term practical and emotional support to help regain confidence and live independently. We can help by: • Taking you home and helping you to settle in. • Shopping for you. • Collecting prescriptions and pensions. • Sitting with you if your carer has to go out. How can I get this help? If you are in hospital, the1 person who is arranging for you to go home and they can contact us for you.
Who gives the help? The service is provided by our trained volunteers. Our volunteers follow the seven principles of the Red Cross. These are Humanity, Impartiality, Neutrality, Independence, Voluntary Service, Unity and Universality. What does it cost? There is no charge for this service. Some people like to make a donation to the Red Cross after they have used the service but you do not have to do this. Why England needs a new care and support system Society is going through huge change. People are living longer than ever before, and the proportion of older people in our society is growing. We have different social values, and we expect more choice and control over all areas of our lives, including public services. And too often the existing system does not live up to the expectations of those who depend upon it. We have an ageing population. In the next 20 years, the number of people over 85 in England will double and the number over 100 will quadruple. Similarly, advances in medical knowledge and practice mean that disabled people can live longer and can lead healthier lives. We expect over 1.7 million more people to have a need for care and support in 20 years’ time. A radical rethink of the care and support system is needed to address these challenges. Otherwise, it is likely that families, including dependent children within the family, will be under pressure to provide inappropriate levels of care, and in some cases people will go without support. If we fail to get to grip with these long-term issues we will fail to provide quality of life for potentially large groups of people, and consequently demand for NHS services will increase inappropriately. The Power of Choice A dramatic expansion of patient choice in the NHS got under way in April 2008. Surveys have consistently shown that patients want choice. The 2005 British Social Attitudes survey revealed that 65% of patients said they wanted choice of treatment, 63% wanted a choice of hospital and 53% welcomed a choice of appointment time. The introduction of free choice means that patients referred to see a specialist can choose where they will be treated. They can choose from any hospital that meets NHS standards. The list includes many private hospitals as well as all NHS providers. Between them they offer everything from treatment for cataracts to open-heart surgery. Under free choice, if you and your GP decide that you need to see a specialist, you’ll be able to choose the hospital that best suits your needs. Perhaps you’ll want to go somewhere away from home but close to your family. Perhaps good parking facilities are vital for you. Waiting lists, cleanliness, reputation... these can all be considered in your decision. The choice is now yours. NHS Choices will help you to make your choice, allowing you to select a wide range of different hospitals and compare them on the criteria that matter most to you. Find and compare hospitals by visiting NHS Choices website - www.nhs.uk In many cases, you’ll also be able to see how other patients rated the hospital. You can also add your own comments. It may not always be possible for you to choose which hospital you go to. In the case of accidents and other emergencies, for example, ambulance crews may have to decide for you. Also, your GP may take the view that, although your case is not an emergency, you need to see a specialist quickly. Here, the GP may have to advise you. There are two areas of healthcare where the location in which you are treated is not subject to free choice: mental health services and maternity. Questions to ask when choosing a Care Home... Everyone will have different views about what they think is a ‘good’ home, and each person’s needs will differ. Here are some questions to consider. Some may be more important to you than others. • Does the home encourage residents to do as much as possible themselves and to make choices about as many aspects of their daily lives as they can? • Can residents bring personal possessions - pictures, plants, furniture? • Can the home meet the resident’s communication needs - for example, through a language other than English, or non-verbal method such as sign language or large print? • Do residents choose what and when to eat? How are special diets catered for? Can Residents eat privately with guests from time to time? Can residents prepare any food and drinks themselves? • Are residents free to see visitors when and where they choose? Can visitors stay overnight at the home, if they have travelled long distances? • Can residents use a telephone in privacy, for incoming and outgoing calls? • Do residents rise and go to bed when they choose? If not, do you consider the arrangements to be reasonable? • What provisions does the home make for taking residents out - on outings, to the shops, to the theatre, place of worship, or entertainment? • What physical activities are available for residents? • is there more than one living room, so that there is a quiet room as well as one with a television? • Are there books, newspapers available for residents? • Do residents visit the library, or does a mobile library come? • Do the managers of the home ask about how the resident Strip2 15/5/07 11:59 Page 1 would like to handle money or medicines?

• How does the home ensure valuable items are kept secure? • Do the residents have their own GP’s? • Is there a residents committee? • Does the home encourage residents to say how they feel about living there, and provide written information to residents and/or their families about how to discuss a problem or make a complaint? • Are toilets available in all parts of the home, fully equipped with handrails and other helpful equipment? • An wheelchairs go everywhere within the home, and easily in and out? Is there a lift? • If the resident needs help bathing, does the home have suitable facilities? Who will help bathing, and can the resident choose how often they have a bath? • Do you have to pay extra for toiletries? • Are there any areas for smoking and non-smoking? • What happens if residents require more or less care than they currently have? Paying for Services in a Care Home We understand making a move to a care home can be a daunting task. This article will be helpful if you are an adult who needs help with paying for services in a care home. You may already be living in a care home, paying your own fees and find that your savings are reducing, or you may be considering moving into a care home. Each person requiring funding must have their care needs assessed by Somerset County Council and must meet the eligibility criteria for funding. The amount a person has to pay will depend on their individual financial circumstances. The rules for deciding how much people pay towards service in care homes, and the capital limits for funding are laid down by the central government in the ‘Charging for Residential Accommodation Guide’. If you have been assessed as needing continuing NHS care services, or you have been placed in a care home under section 117 of the Mental Health Act 1983, you will not be charged for your care whilst you continue to be eligible for funding. *Rate as of April 2009
Where do I start? Paying for Residential Care If you are moving to a care home and think that Somerset County Council may be able to help you with the fees, they will have to look at your finances and work out how much you can afford to pay and how much help you might be able to have. A member of staff from the Financial Assessment and Benefit (FAB) Team will contact you, or the person who looks after your finances, within four weeks of the date of your move to a care home. They will arrange for a FAB officer to visit, at a time that suits you, to work out how much you will have to pay. At the visit they will be able to help you with any questions you have about exactly how much you will have to pay. Most people have to pay something towards their care fees. For someone over sixty on Pension Credit (Guarantee Credit) this will normally be £108.10 a week. For someone under sixty on Income. Support this will normally be £69.90. If you have moreStrip2 income the weekly charge will be higher than this. If you 15/5/07 11:59 Page 1 have savings of over £23,000 you will have to pay the full cost

of the care. The FAB officer will look at your income and savings and when they visit they will ask to see evidence of these.
Top-Ups Somerset County Council sets different weekly fee levels for different types of care. If you choose to go to a home that charges more than these levels a third party will have to pay the difference. This could be a family member, friend or charity. The resident is not allowed to pay the top-up from their income or savings, unless they own a property that is taken into account in the financial assessment. For example if you are an older person and choose to go to a residential home with quality rating charging £350 each week: • The fee level set by Somerset County Council is £328.20 • A third party will have to pay £21.80 each week – the difference between £350 and £328.20 • The resident will pay as much as they can afford • Somerset County Council will pay the difference between the amount the resident can pay and £328.20. For example if the resident could afford to pay £120 • £120 - resident to pay directly to the home • £208.20 – Somerset County Council to pay to the home • £21.80 – third party to pay directly to the home • £350 – total fee Weekly Fee Levels vary depending on the type of care you need call Somerset County Council on 0845 345 9133 for details.
Income Most income is taken into account including: • Occupational pensions / State retirement pensions • Income Support • Minimum Income Guarantee • Pension Credit • Other state benefits including Attendance Allowance / Disability Living Allowance (care component)

Savings You can only get help with care fees from Somerset County Council if you have less than £23,000 in savings. This includes things like; bank and building society accounts, including current accounts, shares, premium bonds, unit trusts, PEPs / ISAs, cash, national savings certificates, bonds. If you have more than £14,000 you will be asked to pay a contribution from your savings of £1 each week for every £250 you have over £14,000.
Types of Care Home Residential homes You may choose to go into a care home (sometimes called residential home) if you can no longer manage in your own home. Provisional Charge You will be asked to pay a provisional amount for the first four weeks until your contribution has been agreed. This amount will depend on your age and whether you receive Attendance Allowance (AA) or Disability Living Allowance (DLA), care component. If ASC work out that this amount is too high then they will arrange to give you back any overpayments you have made. Community Teams There is a Community Team for Adults with Learning Disabilities (CTALD) based in one of the major towns in each of the four localities. These teams are the first point of contact and will assess needs and arrange services if appropriate. Services may be provided by Social Services or purchased from the Independent Sector. CTALDs are a group of Health, Social Services and Connexions professionals (Transitional Personal Advisors – TPAs) who can help people decide what services they need and help to arrange them. The CTALD may do a number of things:• Give information and advice • Arrange for a Social Worker to visit to discuss and assess needs People may be offered a Person Centred Plan or a Community Care Assessment. They both involve getting to know the person and their carers to find out what is important for them. When this has been done a plan will be made for the best way to meet wants and needs – this is called a Community Care Plan. NHS Learning Disability Assistance The main task of the Health Facilitator is to advocate and facilitate to ensure that people with learning disabilities have access to primary and secondary health services in order to maintain their health and reduce identified health inequalities. ‘The Health Facilitator role will be vital in helping people with learning disabilities navigate their way around the health service’ (DoH 2001) The Clinical Lead for Safeguarding Adults (Duncan Marrow) and PALS Manager (Sandy Killick) assist with facilitating for Learning Disabilities at the Musgrove Park Hospital in the Taunton and Somerset NHS Foundation Trust. he role of the carer is a very important one within the local community. 55,000 people recognised themselves as carers in Somerset in 2001, and there is a wide range of support to people carrying out this valuable role.
Are You a Carer? If you give up time, without pay, to look after a family member, friend, or neighbour who is ill, frail, or has a disability, then you are a carer. You may be a parent carer, or a young carer. You and the person you care for can get help, advice, and support from many health and social care professionals and voluntary support groups. Carers Support Workers are based in doctor’s surgeries across Somerset. Carers Support Workers can help you by: Giving information and advice, making sure your needs are assessed, helping you to get practical support, introducing you to other carers through a local carers group, telling you about how to get a break, get advice about money matters and benefits. Call Somerset Direct for more, 0845 345 9133. A Carers Assessment is an opportunity to talk about your needs. It is your chance to tell about the things you think could make caring easier even if the person you care for refuses help. To work out how to help you best, a social worker or Carers Support Worker will ask you about: • The help the person you care for needs • The help you are providing at present • Any difficulties that you may have • What you think would make life easier for you. You, and the person you care for may qualify for benefits. For Carers; Carers allowance, Carers Premium, and for the person you care for; Attendance Allowance (over 65), Disability Living Allowance, Independent Living Allowance. Local Support Services SomersetCarers.org is a website network offering good advice and information for carers. Here you can find details on independent support groups across areas in Somerset and details from authorities such as PCT, NHS and Somerset County Council, as well as info on education for carers. There is also a newsletter you can subscribe to, visit www.somersetcarers.org for a great starting point on help for carers. Carer Support St John Ambulance with support from Somerset County Council provide a carer support programme to help people in a caring role. The programme is provided free of charge and includes lunch. Open to all carers over the age of 18 who provide voluntary care. Would you like the opportunity to gain more skills and knowledge to help you in your caring role? Do you need training in stress management, basic first aid, safer moving/handling and how to prevent falls? Do you want to meet other carers to share experiences, exchange advice and ideas and at the same time make friends? For anyone finding it hard to leave their ‘charges’ free, sitting can be arranged by request. Nursing Agencies
Nurses agencies send nurses to visit you at home when you need specialist or medical care, usually after leaving hospital. The frequency of the visits can vary from several times a day to once a week. About 95% of the nurses supplied by nurses agencies are provided to people in hospital or a care home. However, some nurses are provided to people in their own home. Care Quality Commission website: www.cqc.org.uk There are two types of nurses agency; the first type simply acts as an employment or introductory agency which places self-employed nurses into employment with people who need nursing help at home. The other type of agency employs, trains and supervises its own staff.

Minimum Standards
A set of guidelines to help providers, inspectors and service users understand the expected minimum level of service. These guidelines are not enforceable by law, but providers should abide by them. They also give service users the ability to judge care providers to ensure the correct service is employed. You could expect a nursing agency to provide a wide range of services for example; • Ad hoc, • Emergency cover, • Joint planning to manage seasonal shortages, • Bank management, • Permanent recruitment. Assistance from your Council What is Care at Home? Care at Home used to be called homecare, it can help people with personal care and some practical household tasks that will help you to remain in your own home and be as independent as possible. Who are Care at Home Services for? Services are for older people, adults and children with a physical disability or sensory loss. You will need to have an assessment of your needs to decide whether you qualify for the service. The Fair Access to Care Services Information Sheet explains who can have services, if you would like a copy of this ask your Social Worker or contact Somerset Direct on 0845 345 9133 and they will send you one. What is an Assessment? An assessment is a discussion to make sure that we are clear about your needs. It includes: • What difficulties do you have • What can you do for yourself • What things you cannot manage at all • Who helps you now • What do you think your needs are • What is the best way of meeting your needs
Who provides Care at Home? Somerset County Council arranges for Care at Home to be provided by a number of private organisations. They are all registered with the Commission for Social Care Inspection to provide Care at Home and they all have Contracts with the Council. The care staff are experienced and trained in caring for people in their own homes. Care workers will help you to do the things that you used to do for yourself and will help you to be as independent as possible. For the first few weeks that you have Care at Home you will be helped by a specialist team who will agree the times of their visits with you. This team will carry on assessing your needs and looking at the care you get to make sure that it is right for you. They will change the level of care you have to meet your needs. You can get help with things like: • Washing and using the toilet • Getting out of bed and dressing • Undressing and going to bed We do not normally provide domestic help like bed making, cleaning, laundry or shopping. After a few weeks, when the team are sure that you are getting the right amount of help, they will arrange for you to receive your care from your own regular group of carers. The care you have will change as your needs change and will end when you no longer need help, or if you do not meet the Fair Access to Care rules. Is there a Charge for the Service? For the first few weeks if you receive your Care at Home from the specialist team, there will not be a charge. There may be a charge for an on-going service, your Social Worker will ask someone from the Financial Advice and Benefit (FAB) Team to contact you to do a Financial Assessment to find out how much you may have to pay.

UKHCA Homecare, also known as domiciliary care, enables people who need assistance in their personal care to remain in their own home, rather than move into a care home or care home with nursing. Sometimes homecare is temporary, after a stay in hospital, and sometimes it is needed on a longer-term basis or to supplement the care already provided by a member of the family or friend. Homecare is intended to help people remain as independent as possible and enables them to stay in familiar surroundings, close to their friends, family and the local community. It can be purchased from a homecare agency independently, provided directly by the local authority or purchased by social services from local homecare providers. Homecare workers are specially trained to work in people’s homes and assist with getting up and going to bed, washing and dressing, shopping, meal preparation and assisting with medication. All care workers receive basic training and many study for a vocational qualification in care and have specialist training on caring for people with learning disabilities or dementia. Homecare workers can visit for as little or as long and as often as needed, up to a ‘live-in package’. Working hours are geared to the normal daily routine from early morning to late evening. Eat Well, Live Well Eating well is a positive choice to make. A healthy balanced diet contains a variety of types of food‚ including lots of fruit‚ vegetables and starchy foods such as wholemeal bread‚ protein rich foods such as meat‚ fish‚ eggs and pulses and some dairy foods. Fat is a good source of energy, but eating too much fat can make us more likely to put on weight. Eating foods that are high in saturated fat increases the risk of developing heart disease by raising your cholesterol levels. Oily fish contain ‘omega 3 fatty acids’, which are good for your heart and vitamin D which is important for healthy bones. You should aim to eat at least two portions of fish a week, one of which should be oily. Vitamin D is also found in fortified cereals and soft spreads such as margarine. If you do not eat any of these foods, or you rarely get any sunshine you should consider taking a daily supplement. Base your meals on foods that are high in starch and fibre,

such as wholemeal bread, pasta, rice, cereals and potatoes. Oats, fruits and vegetables, pulses, such as kidney, butter or baked beans are all good sources of fibre. Eating fibre rich foods and drinking plenty of fluid – around 6 to 8 glasses throughout the day – can help prevent constipation, which also reduces the risk of some common disorders in the intestine. Have some protein such as meat, eggs, pulses (peas, beans and lentils) and fish every day. Many of these protein rich foods also include iron, which is important to prevent anaemia. Other foods that contain iron include bread, dark green vegetables and fortified breakfast cereals. Because having too much vitamin A (more than 1.5mg per day) might increase the risk of bone fracture you should avoid taking any supplements that contain vitamin A, including fish oils which are have a high content. ne in four people suffer from mental health problems a year, depression and anxiety are the most common. Help and support is available for all types of mental health problems. Many care homes offer support for people with dementia, see ‘Care Homes’ section page 14.
National Dementia Strategy The NDS was launched on 3rd February 2009 to transform the quality of dementia care, including improved community personal support services and domiciliary care services. There are about 700,000 people with dementia in the UK. Most people with dementia are over 65 years old, but there are at least 15,000 people under 65 who have the illness. The strategy aims to develop services for people with dementia and their carers that are fit for the 21st century, with services that meet the needs of everyone, regardless of age ethnic group or social status. Admiral Nursing DIRECT The Admiral Nursing Direct Helpline, part of the charity For Dementia, has been set up to provide people with the opportunity to talk through their worries and concerns about themselves, friends or relatives with dementia. Lines are manned by experienced Admiral Nurses and responds to requests for help with diagnosis, medication, support and care at home, professional carer support, and finance.The lines are open on Tuesday 10am - 4pm, and Tuesday and Thursday evenings between 6pm and 9pm. The programme is based around the individual and the maximum stay is six months minimum of three. We will give every individual the opportunity to express themselves in a variety of ways to allow clients to move on in their lives. The program is person centred and takes into consideration the individuals needs, we introduce a variety of models to expand the choice of our residents and also draw on the experience of our staff. Whilst residing at Somewhere House the individual is given the opportunity to explore both past and present issues, giving them both insights to their own patterns of addiction and behaviours. Volunteers needed to develop Disability Football Employment for disabled people Around one in five people of working age have a disability or long term health condition and over one million disabled people want to work but don’t have a job. Pluss offers disabled people a wide range of services to help them train for, find and keep work. Having a job offers much more than just an income. It offers independence, dignity and self esteem. At Pluss, we believe this opportunity should be available to everyone and offer a full range of services designed to meet an individual’s needs and readiness to work. We currently support over 2,000 people to develop their skills, find and keep work. Our services include the following: Training and learning: We offer information and advice, and will help you put together a development plan for learning and work. We also deliver a wide range of courses to help you achieve nationally recognised qualifications. Work experience: We will help you build your confidence, self esteem and find work experience in a job of your choice. Volunteers needed to develop Disability Football Somerset FA are looking for individuals/Coaches who are keen to support young people and adults with a special need and/or disability to play football! Disability Football is developing quickly in Somerset. To ensure sustainability of disability football, volunteers are the key! If you are a coach linked to a club or someone interested in supporting individuals with a special need and/or disability Somerset FA want to hear from you! Somerset FA host a substantial coach education programme with many courses for individuals to learn new skills. There are two disability specific courses which support coaches and individuals within football to learn more about disability football and how players with disabilities can be supported. There is a disability player pathway which Somerset FA is working hard to establish. This pathway is; to develop grassroots football by increasing opportunities for players at school and club level; develop more adult club teams to enter into the adult disability league - the ‘Ability Counts League’; set up impairment specific teams e.g. Cerebral Palsy football team, Partially Sighted football team; develop a Centre of Excellence for elite players which is the National Talent ID programme for players to be selected for the National England Disability teams. One of the clubs members, Dave Cooke, is the Disabled Persons Representative on the South West Region Committee of the BCU and also a disabled paddler. The club house in Priory Bridge car park has disabled access and toilets, and a ramp down to water level on the River Tone. We have several kayaks suitable for disabled beginners, including a Topolino Duo (a two seater with large cockpits) and several Open boats.
The Frome group is assisted by a rota of professional tutors and is run by two volunteers and a committee of members. The group meet weekly in the local community college. The meetings give an opportunity for artistic expression and socialisation for people of all ages from Frome and its surrounding villages, who have physical disabilities which make it difficult for them to attend standard day and evening classes. The age range of our members is wide and they have a variety of physical problems resulting - for example - from strokes to multiple sclerosis.
Disabled people are welcome to attend the open sessions, running from June to September on Thursday evenings from 6:30pm to 8pm. y the mid 1970’s most households in the country owned a car. But disabled people were missing out. As a result, many people were housebound for long periods and often dependent on others for their mobility. The Scheme is available to anyone who receives either of the following benefits, and has at least 12 months’ award length remaining when they apply:
Before the introduction of the Mobility Allowance in 1976, only disabled people who could drive themselves got any government help with personal transport. It was usually in the form of a small car provided to disabled couples, to people responsible for the sole care of a disabled child and to a category called “war disabled”. An allowance was paid to disabled people who actually owned a car. But most people were supplied with a small, blue, single-seat, three wheeled, motorised “invalid trike”, which was incapable of carrying passengers. Today, The Motability Scheme enables disabled people to use their government funded mobility allowances to obtain a new car, powered wheelchair or scooter. Some 450,000 people currently use the Scheme and in 2006 Motability celebrated providing two million vehicles since it was set up by the government in 1977.

Similarly, parents and carers can apply on behalf of a child aged three or older. Some customers need extra financial help to meet their mobility needs, for example, to help towards the cost of the advance payment for a suitable car, adaptations, driving lessons or perhaps a wheelchair accessible vehicle. The blue badge scheme provides a national range of parking concessions for disabled people with severe mobility problems who have difficulty using public transport. You are automatically eligible to apply for a badge if you are over two years old and either: • receive the Higher Rate of the Mobility Component of the Disability Living Allowance • are registered blind • receive a War Pensioner’s Mobility Supplement You may also be eligible for a badge if you are over two years old and either: • have a permanent and sustainable disability which means you cannot walk, or which makes walking very difficult • drive a motor vehicle regularly, have a severe disability in both arms, and are unable to operate all or some types of parking meter If you are a parent of a child who is less than two years old, you may apply for a badge for your child must be accompanied by bulky medical equipment or needs to be kept near a vehicle at all times. The concessions provided under the scheme apply to onstreet parking only. Badge holders may park for free and for as long as they need to at on-street parking meters and pay-anddisplay machines, unless there is a traffic sign specifying a time limit for holders of Blue Badges. Where a time limit is in force you must also display the special blue parking disc showing time of arrival.
Car/Road Tax Exemptions Who is eligible? You may claim exemption from vehicle excise duty (road tax) if you’re receiving one of the following: • The higher rate mobility component of the Disability Living Allowance • The War Pensioner’s Mobility Supplement If you’re not the driver, you can nominate someone else (a nominee) to drive the car for you. But the car must: 1. Be registered either to you or your nominee 2. Only be used for your benefit 3. Be insured for the nominee to drive If you don’t own a car, you can nominate someone else’s vehicle but it must be used only for your purposes for example shopping or getting prescriptions. Vehicle excise duty (road tax) exemption can only apply to one vehicle. Once you get your tax exemption status and once you have your tax exemption disc, you can apply for a refund for any whole months remaining on your existing tax disc at the day the DVLA gets your application. Motability keeps Aaron on the move! go as I please. I’m currently studying for my A-Levels, so I use it all the time for getting to college and meeting with my friends. I love fishing and playing tennis and having my car means I can do all these things and not have to think about how I’ll get there - I can just get in the car and go whenever I want to.
Aaron Morgan, 18, from Hampshire, is paralysed from the waist down following a motocross accident at the age of 15. “I’ve always been a massive fan of motor sports and would love to become a racing driver,” says Aaron. “After the accident, there was no way I was going to give up on my dream. After passing my driving test first time, I got a brand new VW Golf through the Motability Scheme. It’s an automatic with hand controls instead of foot pedals and is great to drive. “My Motability car means I can live an independent life. I don’t need to rely on my parents for lifts and I can come and

Wet and Well The Blue Gym project aims to promote physical and mental well being and strengthening of communities through encouraging people to get out and get active in the coastal and water environments. The Blue Gym will act as an umbrella and a portal to the amazing range of activities that are already taking place in, on,

under or around water. These range from walking on the coastal footpaths and beaches through to rock pool rambles and full immersion sports like swimming, sailing and surfing. The NHS and PCTs will be encouraged to direct patients through the Blue Gym to activities they can take part in as an alternative or supplement to other forms of treatment. The South West will be a test bed for the scheme especially in terms of research to prove that using the Blue Gym works! The

Green Gyms provide a fun, socially interactive environment that offers a great way to get fit and connect with people in your community who are interested in doing something great for the planet. BTCV is a conservation charity that runs Green Gyms, local programs whereby you are led through some warmup exercises followed by conservation or gardening work at your own speed, with the guidance of a trained leader. Green Gyms are a great way to take care of yourself and the planet at the same time. We’re Keeping Somerset Fit and Well Somerset Primary Care Trust (PCT) is the NHS organisation that funds your local health services. It spends over £700 million each year funding hospital treatment and operations, managing the counties 13 community hospitals, delivering community services and working with doctors, dentists, pharmacists and opticians. Free Cinema for Carers
Carers who look after a disabled person may be entitled to go to the cinema for free. The Cinema Exhibitors’ Association card (CEA) is a national scheme, however not all cinemas subscribe. To qualify for the card the person must be getting DLA and AA or registered blind, or the holder of a disabled persons rail card. Based at Taunton Conquest Centre the Riding for the Disabled Association (RDA) is open to all disabled people who would like to horse ride. epresentation for someone who, for whatever reason, is unable to speak up for themselves. This could be support for questions, decisions, or making a complaint. People who might need advocacy may be elderly, frail, physically disabled or have learning disabilities or mental health needs.
Advocacy for people with physical disabilities or sensory impairments Somerset County Council has arranged for an organisation called A4e to provide an advocacy service for people of working age with physical disabilities and sensory impairments. A4e will help people with: • Representation when their health and social care needs are assessed, including for continuing healthcare. • Employment issues • Support with benefits appeals and tribunals • Housing issues • Getting the services they need. For example leisure, education, and transport. • Making complaints against public bodies like the NHS or the County Council You should be able to see an advocate from A4e somewhere in the area where you live. Age Concern Advocacy for Older People Advocacy services has two components – we are pleased to be able to offer a general information and advice service as well as one to one support via our advocacy service. Somerset Advocacy helps people to speak up for themselves and speaks for people who are unable to represent themselves. Somerset Advocacy can help people who are not happy about home, care, work, bullying, school/college, health care or transport. Somerset Advocacy can help people who want to move house, look for work, have relationships, and with leisure, day services, and choice. Care Advice for Older People Their Families and Carers In response to demand, four of the UK’s leading advice services have come together to launch the first ever joined up advisory service to provide through a single telephone number and website a free, independent and good quality advice and information service about housing and care options for older people, their families and carers. Through a web based customer management system all four partners will be able to complete and access client records to have some input into the advice and information process as and when required. Day Services
Day Services offer help and support to older people and to people with disabilities. They offer social contact and stimulating activities for people who are finding difficulty living at home. Day care also enables carers to have a break from their caring. Centres are run by local authorities, the NHS, voluntary organisations and commercial businesses such as care homes (see page 14). Day Services offer care for adults with physical and/or learning disabilities and people recovering from mental illness. Day Services aim to give them the skills they need to live as independently as possible. Most are run by the Adult Services department and are for people who have been assessed as needing this kind of help.
Respite Care
Caring for someone at home may sometimes feel like having a full time job and you might feel that you need to take a break from time to time. Respite Care for the person you look after may give you the opportunity to do just that. It may be possible to admit them to a residential or nursing home for a short period of time, giving you time at home or perhaps enabling you to take a holiday away. You may also like regular short breaks for a few hours to pursue a hobby or take time for yourself, or perhaps you would just like a good night’s sleep. There are a range of services that can provide trained people to look after the person you care for just such purposes, during the day or night.

Sheltered Housing S
heltered Housing (Retirement Housing) in Somerset is listed in this section. It is likely that there will be a waiting list for rents and purchasing, it is advisable to contact the preferred scheme early.
Schemes and options Sheltered housing is often called retirement housing or warden-assisted housing. There are many different types of sheltered or retirement housing schemes, both to rent and to buy. Schemes usually consist of between 15 and 40 dwellings which may be bedsitting rooms, self contained flats, bungalows or luxury apartments. Usually they are designed with older people in mind and to mobility standards, for wheelchair access for example. Some schemes are simply housing designed to meet the needs of older people and linked to a community alarm. But the majority of schemes have a Scheme Manager (or ‘Warden’) as well as a community alarm service. There are often communal facilities such as a lounge, laundry, guest flat and garden. Meals are not normally provided but a very few schemes include a restaurant, and quite a number arrange one hot meal a day. There is usually a minimum age, often 55 or 60, but occasionally 50. Sheltered or retirement housing appeals to people who like living independently but want the reassurance of knowing that assistance is on hand if there is an emergency. The duties of the Scheme Manager can vary considerably between schemes. Some schemes have a resident manager and a 24 hour service, others have visiting/part time managers. Scheme Manager (Warden) Over the last few years there have been many changes to the Scheme Manager’s duties. Most Scheme Managers are now expected to: • Manage the scheme • Summon help in an emergency. • Build up a relationship with older people living in their schemes; giving residents information on availability and access to services and encouraging them to ask for additional support from statutory and voluntary organisations when appropriate. • If help is needed the centre will alert relatives or friends, or contact your doctor or the emergency services. • Other facilities Renting Most housing for rent is provided by local councils or nonprofit making housing associations (often called Registered Social Landlords or RSL’s). Local councils and RSL’s generally aim to offer their housing to people in the greatest housing need, at rents which are affordable - perhaps with the help of housing benefits. The criteria that applicants are generally expected to show include: • Some degree of housing need because of the physical condition of their present home; or • Medical and/or social reasons for wanting to move, e.g. poor health, disabilities, loneliness, fear or isolation from friends & family; and • That they are not easily able, for whatever reason, to buy rather than rent; and • That (if they are not local already) they have good reason for wanting to move to the area they apply for. In most parts of the country, councils and RSL’s recieve applications from more people than they can assist. The cost of renting will be made up of a rent plus service, a charge to cover the services provided. Service charges in the majority of ordinary sheltered or retirement schemes is in the range of £20 to £40 a week. People wanting to rent sheltered housing and who have a low income should contact local authority to check their eligibility for housing benefit. Buying You may also consider purchasing property. Prices and types of property vary enormously. A small second-hand flat may be found for under £70,000 in parts of the country. Brand new properties cost more. Luxurious homes on sites with every amenity - swimming pools, golf courses and restaurants - can cost well into six figures. Service charges are likely to apply as will ground rent which could vary from £50 per year to £300. HELPING PEOPLE WITH A HELPING HAND The STARS charity (Syncope Trust And Reflex anoxic Seizures) provides information and support to people who suffer with syncope and unexplained loss of consciousness. If someone loses consciousness for a short time they are often said to have experienced a ‘blackout’. There are three main causes of blackouts: dysfunction of the cardiovascular system (syncope), dysfunction of the brain (neurological, as with epilepsy) or psychogenic dysfunction (mental psychological factors). Different Types of abuse? Physical abuse. This is when someone physically hurts you. This can be, hitting, kicking, pulling hair, pinching, scratching, shaking, giving someone too much or not enough medicine. Sexual abuse. This is when someone is making you do sexual things that might make you sad, angry or frightened. This can be being touched where you do not want to be touched, in your private parts being made to touch other people in their private parts, undressing or having sex when you don’t want to.

Financial abuse. This is when someone takes your money or things. This can be stealing your money or things, making you buy something you don’t want to buy, using your money to pay for their things, when you don’t have a choice in how your money is spent or when someone tells you that you have to give them your money or your home.
Neglect. This is not giving you the things you need. This can be not having your prescribed medicine when you need it, not being taken to the doctor if you are ill, being cold a lot of the time or hungry a lot, only having dirty clothes to wear, or not having your equipment when you need it (like hearing aids, walking frames, wheelchairs).
Emotional abuse. This is when someone says things to you that hurt your feelings or scare you. This can be calling you names, threatening you, laughing at you, treating you like a child, not letting you spend time with other people, or go out, Strip2 15/5/07 11:59 Page 1 ignoring you.
Discrimination. This is when people are treating you unfairly because you are different. This could be because of the colour of your skin, your religion, your sex, your age, your disability, your language, or because you are lesbian or gay.
