You might think that having a disability would erase the possibility of participating in competitive sports, but it doesn’t have to. Having a disability does not have to change one’s desire to excel in physical activities. In San Joaquin, you’ll find many opportunities for people with disabilities to remain every bit as competitive as their able-bodied counterparts.

Beep Baseball Relying primarily on their sense of hearing, even the visually-impaired can enjoy participating in America’s favorite pastime. Eight years ago, the Community Center for the Blind and Visually Impaired in Stockton helped form the first official beep baseball team in California, the Stockton Stingrays.

The official rules of the game are outlined in great detail by the National Beep Ball Association (NBBA). As visual acuity may vary, all fielders and batters are blindfolded to maintain a level playing field. Players bat a beeping ball into play, and then run towards a base that buzzes. They score if they reach it before a fielder gains full control of the ball. Meanwhile, a sighted pitcher, catcher, and field spotters round out the lineup. The NBBA currently has 19 registered teams who are playing each other during this year’s regular season, March 6-August 28. During the season, the Stingrays have also set aside time to challenge sighted (also blindfolded) teams to come out and give it a try. Do you have what it takes? »

The ABCs of Autism

Autistic Spectrum Disorders (ASDs) are defined as developmental disorders that impair one’s communication and other interpersonal skills, as well as the ability to learn. Under the ASD umbrella, so far five specific disorders have been identified in terms of their severity: Autistic Disorder, Asperger’s Disorder, Rett’s Disorder, Perceptive Developmental Disorder Not Otherwise Specified (PDDNOS), and Childhood Disintegration Disorder. Twenty years ago, the prevalence of autism in children was about 1 in 10,000. According to the Center for Disease Control (CDC), today that number has skyrocketed to 1 in 150. There is no explanation for the leap, nor is there a cure for the disease or even a clear-cut explanation as to how the disease occurs in the first place. Research and clinical studies to find answers continue at a brisk pace. For example, the

UC Davis MIND Institute in Sacramento is currently conducting the Autism Phenome Project (APP) to further distinguish subgroups, or phenomes, within autism, as well as the CHARGE (Childhood Autism Risks from Genetics and the Environment) study to determine its cause. The search continues. In the meantime, families with autistic children are left feeling powerless and wondering what to do and where to go. So I began my own search to discover what resources and tools are available to parents and children in San Joaquin and beyond to try to fight back against this disease. Starting with the Family Resource Network, which offers referral services and support for families of special-needs children in five counties, Ann Cirimele said that while there is not yet a cure for autism, there are certain promising therapies/interventions being employed to minimize its effects.

She then referred me to Tara Sisemore-Hester, Coordinator of Autism Services at Valley Mountain Regional Center, who elaborated to say that those dealing with autism in San Joaquin actually have better access to beneficial services than anywhere else in California. This is due primarily to the sense of cooperation instilled into the San Joaquin educational system by Dr. Howard Cohen and associates back in 1994. After various court battles between parents and the school district over the need for special education for autistic children, Sisemore-Hester says, “We stepped back and decided to take all the money we’d been spending on lawyers and instead put it towards the children.” The result is that San Joaquin now offers more than forty free, comprehensive, and individualized home-based and school-based programs through innovative interagency agreements and public-private partnerships. The most promising of these programs is the Early Intensive Behavioral Treatment (EIBT) program. The basic premise is to catch autism young—preferably by the age of 2—and treat it aggressively to minimize or even reverse its effects. The teaching method used with EIBT is called applied behavior analysis (ABA). With this, teachers break both simple and complex skills down into small tasks that students practice repeatedly, ideally on a one-to-one basis with the instructor. A child is rewarded when they perform a task successfully or prompted to try again when they do not succeed. Parents help this process along by continuing such education at home. The key to this is intensity. Depending on the assessment of individual needs, children under age 3 receive 20-30 hours of ABA a week, 47 weeks out

of the year. Over three years, 35-40 hours a week could be expected. The ultimate goal is to transition into a regular school environment. According to Sisemore-Hester, 75 percent of the participants in ABA treatment have improved in their behavior and communication skills, most of them enough to matriculate into elementary school. For even greater insight into the progress being made, Sisemore-Hester directed me to speak with Allyson Moore, Site Coordinator for The Kendall School in Tracy. Together with ABA, The Kendall School has also instituted their “Kids Helping Kids” program—facilitated play dates where children from public schools act as peer helpers to aid in the socialization process. Moore has worked with autistic children in various capacities for fourteen years. Despite the growing number of autistic children over that time, she sees a silver lining, especially in terms of awareness. “Published reports are making parents aware of what to look for,” Moore says. “So children that weren’t getting diagnosed at an early enough stage are now.” This also puts a positive spin on the cost. Institutionalizing an autistic adult can run into the millions over their lifetime. Spending earlier on intensive early intervention will help eliminate those costs by eliminating the need for institutionalization altogether. All in all, in my search I discovered that we still have a long way to go in terms of understanding autism and finding a cure. But I also learned that when public schools, private institutions, and parents work together, autism can be overcome. In that, there is hope.

Nine years ago, the Decision Medicine program was created to introduce the world of medicine to young, gifted high school students in San Joaquin. Today, as back then, one of the primary goals of the program is to motivate young students to With additional reporting by become physicians so that in the future they will come back NIssa Hallquist and help serve the San Joaquin community, which is in constant need of quality medical professionals. Through the program, students can truly see what it is like to work in the medical field by allowing them to shadow and ask questions of various physicians and health care professionals. The program has changed drastically since its inception in 2001. When first introduced, only six students were selected from one local high school to participate. Later, the program grew to serve twelve students from about five different high schools. During the most recent session in the summer of 2009, twenty-four students were selected to participate in the two-week program, representing almost all the high schools in San Joaquin County. In order to not disrupt the daily functioning of the

accommodating hospitals and other medical facilities, the twenty-four students were divided up into three groups. Each day, two different students from each team assumed the day’s reporting and photo-taking responsibilities to both serve as a reinforcement of their learning experience as well as archive events for the 2009 program. The program has also grown in the number of participating facilities. One such addition this past year was the Community Center for the Blind. There the group was able to meet Christopher Sanchez, who is visually impaired. He told everyone that the blind learn to “do the same things, but just in different ways.” Students were able to observe Center clients making ceramic chimes and statues, weaving intricate blankets and quilts, using different computer software programs, and they even had a chance to learn about how the Stockton Stingrays play beep baseball. (For more details on the Stingrays, see “Adaptive Sports” on page 10.) High—were able to watch two live surgeries performed by Dr. Rawson at St. Joseph’s Medical Center. Excitement over the prospect wore off as the time for the first incision crept closer. “The anxiety didn’t really hit me until I was getting dressed up in the surgery scrubs,” says Dougherty. Fortunately, since the surgery was a laparoscopic gallbladder removal, there was not a large amount of blood, which put both Cwick and Dougherty at ease. They also felt very welcomed by the surgical team during the surgery as the doctor described and explained his actions throughout the entire procedure. It left such a lasting impression that Cwick, who was initially interested in research and microbiology, found himself very interested in becoming a surgeon. Both agreed that if it was not for the Decision Medicine Program, they never would have gotten to experience watching a live surgery. Cwick defined the pivotal experience by saying that it “gave [him]

more motivation to continue pursuing [his] dream of becoming a physician.” After visiting many local facilities, the group traveled to San Francisco to visit Sutter Pacific Hospital and tour the California Pacific Regional Rehabilitation Terrain Park, which helps patients practice everyday obstacles such as crossing streets and walking on different terrains like sand, bark, and asphalt. At the end of the day, the students enjoyed a boat ride in the San Francisco Bay and had dinner at Pier 39. To celebrate the conclusion of the 2009 Decision Medicine program, the students were honored at a beautiful banquet at Stockton Golf and Country Club. The three groups performed a prepared skit in front of their families and visiting doctors to demonstrate what they learned throughout the program. The skits ranged from flashbacks to first-aid scenarios, all different but evident of the skills the students learned throughout the weeks.

The program has been so successful at the high school level that it has expanded to target students at a younger age. Starting this past September, fourth graders at Hamilton School in Stockton participated in the first Decision Medicine Jr. Program. Each month over the course of the school year, health care professionals visited their class and introduced them to the field of medicine. After seeing such a positive impact in San Joaquin, the California Endowment has also given a grant to explore the possibility of implementing the Decision Medicine in other counties. A decade ago, it was only an idea of a few dedicated doctors, but through the support of many partners and organizations, Decision Medicine has become a successful program that continues to grow and impact the lives of many of San Joaquin’s future doctors. Last year Health Volunteers Overseas/Orthopaedic Overseas inquired if I wished to volunteer for two weeks at the Angkor Children’s Hospital in Siem Reap, Cambodia. I was already scheduled to spend two weeks volunteering in Phnom Penh, Cambodia, and it seemed natural to add the Children’s Hospital in Siem Reap. I agreed. In the interim, long before the trip was to begin, the assignment in Phnom Penh was cancelled because of local holidays and a conference that conflicted with my scheduled time in Phnom Penh. Nonetheless, I wanted to volunteer at the Angkor Children’s Hospital, and I decided to proceed. »

The hospital arranged for a driver, who held a sign with my name on it at the airport and drove me to my guesthouse. I was immediately aware of the heat and humidity in Siem Reap. The average temperature at this time of year is above 90 degrees, and the humidity 100 percent. The first week in Cambodia I stayed at a small guesthouse near the hospital, which was convenient and inexpensive. The second week, when my wife arrived, we checked into the Angkor Palace Resort and Spa, a five-star Cambodian-owned resort. Cambodia is extremely inexpensive. At the guesthouse, an air-conditioned room with bath was $12 a night. The luxury resort hotel was $85 per night. The day after arrival I arranged a guide to take me to visit the temples of Angkor Wat. The Khmer civilization was extremely advanced. These spectacular temples and cities were built in the tenth and eleventh centuries and were abandoned approximately 200 years later. They were rediscovered by a French botanist in the early 1800’s. They are remarkably wellpreserved, and anyone who makes a trip to Siem Reap should plan on spending a few days visiting these magnificent structures. My first day at the hospital was Monday,

October 19. After an introduction and tour with the volunteer coordinator and some very minimal paperwork, I was introduced to the surgical staff with whom I spent the majority of my time. There are three surgeons at the Angkor Hospital for Children. They are all general pediatric surgeons who have had no formal residency training. They learn on the job, and my role was to help them to learn to do orthopaedic procedures. Their surgical skills are remarkably good, but they don’t have enough orthopaedic training to appreciate how to approach some of the various orthopaedic conditions with which they are faced. The orthopaedic surgical volunteer supply at Angkor Hospital for Children seems to be sufficient. The hospital consists of a number of onestory buildings which, for the most part, are not airconditioned. There are four wards. The patients all stay in the same room. The parents stay with the children. There is one single operating room at the Angkor Children’s Hospital, and the

operating room and recovery room are airconditioned. The operating room is run quite well. The equipment is a potpourri of primitive and relatively modern, although outdated, equipment. Nonetheless, it gives an opportunity to improvise and work with less than we are accustomed to at home. They do have an x-ray machine and a portable x-ray machine, so single-shot x-rays can be taken in the operating room. Good quality x-rays are available on all the patients. They also have ultrasound, which seems to work reasonably well. There are no other advanced imaging systems, such as CT scan or MRI scan. Those are essentially not available to the average patient in Cambodia. Because there is only one operating room, only one volunteer can be present at any given time. The volunteers come from numerous specialties including pediatric general surgery, urology, cardiothoracic surgery, plastic surgery, and orthopaedic surgery. During the two weeks I was at the Angkor Children’s Hospital the operating room schedule was fairly light with routine cases, so that I could do orthopaedic cases every day. Nonetheless, the local surgeons had occasional urgent cases that would take precedence over my schedule. The anesthesiologists are very highly skilled and do anesthesia well with few complications. They do general anesthesia on very tiny infants. Almost every orthopaedic surgical patient received some type of a nerve block along with the general anesthetic. They do caudal anesthetics, femoral blocks, ankle blocks, and sciatic blocks.

The surgical turnover is extremely efficient, so fast you barely have time for a cup of tea before the next patient is asleep in the operating room. Nonetheless, the number of cases is not enough for a twoweek volunteer stint. Most days I did two or three operations and was done by noon. The entire operating room shuts down from 12 to 2 p.m. for lunch. The first day there was a screening clinic. The local surgeons had assembled approximately 40 patients for me to examine and determine whether or not I could help them surgically. The cases were interesting and challenging. I tried to allow the Cambodian surgeons to do the operations, with me assisting. I felt they would learn a great deal more than by watching me operate. That seemed to work out very well because they are very highly skilled and pick things up quickly. The present rate is about one pediatric orthopaedic surgical volunteer about every three months, and that does not allow accumulation of enough surgical patients for a two-week stint. I might advise other doctors that visit to spend one week working at the hospital, and the second week traveling in Cambodia and seeing

the spectacular Angkor Wat and associated temple ruins. The town of Siem Reap is a little bit touristy. A lot of young tourists come here, and a lot of Asian tours pass through this area. We ran into people from all over the Far East, including the Philippines, Japan, and Korea, as well as a few groups from Western Europe. Most of them passed through Siem Reap in a period of two or three days, and then moved on. We were easily the longest tenants at the resort hotel. Everyone on the staff got to know us. The hospital is run by a group called Friends without a Border, and there is an American pediatrician who is the full-time education coordinator. He and the chief of surgery coordinate the volunteer visits. That way, a plastic surgeon, urologist, general surgeon, orthopaedist, and ENT don’t turn up at the same time and have to compete for operating room space. I would recommend that those wishing to volunteer contact Friends without a Border directly, as I think they are quite happy to have volunteers for a week at a time. Once they know you at the hospital, I believe they would be eager to have you return.

They see a lot of chronic musculoskeletal infection which often needs to be treated surgically. There are also many malunited displaced fractures that had been treated by traditional bone healers. These have healed in malunited positions and are beyond the ability of the surgeons in Siem Reap, or for that matter, the volunteer surgeons, to correct with the type of equipment that is available. There is little choice but to accept the deformity, as the children seem to function reasonably well. I saw no acute fractures, although I am told they are seen from time to time. The femur fractures are treated in traction. The Ponseti method of treatment for club feet has caught on here. There is a group called Handicapped International which does all the casting of club feet (by physiotherapists). The percutaneous tenotomies are done by the surgeons at the hospital, and then the additional casting and construction of the Denis Browne bar and corrective shoes is done at Handicapped International. The splints are extremely innovative. They look like they are much more comfortable than what we use in the United States. Travel around Siem Reap is usually by Tut Tut, a small carriage pulled by a motorscooter that can easily pull two adults. While I was staying at the guesthouse I could walk to the hospital in one minute. While staying out at the resort hotel I traveled by Tut Tut each morning, which took about ten minutes and usually cost $3. The restaurants in Cambodia are outstanding and extremely inexpensive. It was not unusual for my wife and I to dine out in a nice restaurant, have three Khmer dishes and beer, and walk away from the restaurant with a bill of about $12. Some of the restaurants even offered live Cambodian entertainment, which did not add to the price. The de facto currency of Cambodia is the U.S. dollar. There is a Cambodian currency called the Reil, but no one uses it. The prices in all the restaurants, shops, and hotels are in U.S. dollars. The transactions, even on the street, are in U.S. dollars. The ATM machines (which were plentiful) give out U.S. dollars and not the local currency. There is no reason to change money at all. HAVING TROUBLE HEARING? bills, which are very useful (perhaps 100 for a 1-2 week stay). Credit cards are useful only in a few places. There are numerous choices of accommodations in Siem Reap. These range from small guest houses, where I stayed the first week, to numerous, rather luxurious hotels. The area is gearing up for what I believe they anticipate is a huge influx of tourists in the next few years. The attraction, of course, is Angkor Wat. There are easily 100 new hotels that have been built in the area. They range from comfortable hotels to luxury resorts. The Angkor Palace Resort and Spa offered special rates to various groups, including people affiliated with the United Nations and to people like myself, volunteers at the Angkor Children’s Hospital. The information brochures provided by the Friends without a Border provide a list of available accommodations, but it is easy to go online and look at the many choices, which is what we did. We contacted the hotel directly, asking if they had arrangements with the Angkor Children’s Hospital. The volunteer coordinator at the hospital also is extremely helpful in arranging accommodations. My take on the needs of the Angkor Children’s Hospital, as far as pediatric orthopaedic volunteer consultants, would be the need for one surgeon approximately every two to three months for one week at a time. That way, on the first day accumulated cases can be screened and an operating room schedule set up for the rest of the week. In that way, each surgical day would be a little bit longer, and I believe all of the cases could be completed. On the first visit to Siem Reap I think it is important to spend a little bit more time to get the lay of the land and enjoy some of the incredible history and architecture of the area. After seeing that on your first visit, one can accomplish almost everything one has to do in a week at the hospital. Procedures can even be staged if there is enough sequential pediatric orthopaedic coverage. This is an excellent opportunity to teach, as the local surgeons are extremely skilled and only need some direction as to handling many of the difficult problems. My visit was extremely gratifying and I highly recommend this type of experience to all physicians and surgeons. [SJ MED]

Cosmetic surgery gives a better body image, and a better body image grants confidence. Someone who looks better—and who knows it—is more likely to take better care of themselves to keep it that way. The following outlines today’s most commonly requested cosmetic procedures, including current techniques, recovery times, and what you can do to maximize the results, to become and remain the confident, healthy person you were meant to be. »

Wrinkle Reduction

LipoSuction Liposuction is meant to slim and reshape the body through the strategic removal of fat deposits. Through small incisions, fat is vacuumed out with a cannula, or thin tube. The fat is liquefied for easy removal, either through manual emulsification with the cannula (the traditional method), fluid injection, or UltrasoundAssisted Lipoplasty (UAL), which uses ultrasonic waves to break down the fat. The latter two techniques take a bit longer to perform, but tend to reduce trauma and recovery time afterward. According to Dr. Hugh L. Vu, a board-certified plastic surgeon practicing in Stockton, the best candidate for this procedure is healthy, of overall normal weight with pockets of excess fat only in specific areas, and has firm, elastic skin that can rebound. There are two reasons for this. First, such a body type results in a more proportional result. Second, if you’re too overweight, even with no history, there’s still a concern regarding potential heart, lung, or diabetes-related issues. Maybe the most important thing is that the patient should also have realistic expectations of the outcome. In order to discuss your individual needs, desires, and the best technique to achieve them, Dr. Vu recommends lengthy conversation with your surgeon before the procedure. After surgery, the time needed to fully recover depends on the technique used. In general, you should be able to get back to work within a few days. While most swelling, bruising, and discomfort should dissipate in a few weeks, some residual swelling may exist for months. Afterward, proper diet and exercise is the most effective way to maintain your new body. SAN JOAQUIN MAGAZINE

Wrinkles are initially caused by repetition of muscle movement under the skin, and then become deeper as the skin loosens over time. Minimally-invasive injections that reduce the effects of both have become exceeding popular and successful. Ideal for new and/or smaller wrinkles, Botulinum Toxin Type A, more commonly known as Botox, inhibits the contractions of the muscles responsible. Dr. Shaukat Shah of Total Body Medi Spa in Stockton finds that Botox remains one of the most popular treatments since, in addition to smoothing out ones that already exist, “it may actually prevent new lines and wrinkles from forming.” For larger and more established wrinkles, soft tissue fillers like collagen, fat, hyaluronic acid (Restylane or Juvederm), and microspheres (Radiesse) can be injected into the skin to help temporarily restore volume and structure to the face. As one of the fastest growing areas in the cosmetic surgery industry, new injectables are coming out all the time, with constantly improving and longer-lasting effects. One in particular, Artefill is actually meant to last permanently. Anyone considering this type of treatment should not have any active skin infections or open sores, and need to be aware of any drug or food allergies from which certain injectables are derived. Very severe wrinkles may be dealt with more effectively with traditional facial surgery.

BreastAugmentation In order to increase size and/or help to reshape the breast, breast augmentation surgery currently uses saline or silicone-filled implants. Based on body type and implant size, there are a few different ways a surgeon may decide to insert the implants. An incision may be made in the crease where the breast meets the chest, around the areola, or in the armpit. From there, a pocket is made directly behind the breast tissue or under the pectoral muscle to accommodate the implant. Certain risks, including infection, bleeding, or hardening of the breast may occur. Under normal circumstances, however, the patient is up and around in a couple of days, stitches come out in two weeks, sensitivity lessens in about three weeks, and then swelling should subside within five weeks or so. Following up with regular breast checks ensures that the implant is functioning properly. According to Dr. Vu, as with liposuction, the ideal patient for this procedure should be relatively healthy and have realistic expectations regarding the outcome. “The best candidates for breast augmentation,” he says, “are women who are looking for improvement, not perfection, in the way they look.”

SkinResurfacing There are a number of procedures that can improve skin’s texture and appearance with little to no downtime. Microdermabrasion renews the skin with exfoliation from tiny crystals. Chemical peels (glycolic or lactic acid, Obagi Blue Peel, etc.) improve the skin by penetrating the surface to reduce discolorations, scarring, and fine lines while at the same time stimulating collagen production to firm the skin. Laser/light resurfacing offers a more targeted approach. Treatments such as Genesis reduce wrinkles, rosacea, and other imperfections without any side effects, even immediately afterward. Other treatments like Intense Pulse Light (IPL) therapy, or Titan, help tighten the skin to such a degree that they are touted as non-invasive face lifts. Factors that determine the ideal candidate for these procedures include skin type, coloring (darker skin can become discolored after a resurfacing treatment), and whether or not there’s a history of allergic reactions. In order to avoid infection, you’ll also likely need to hold off if you have an active case of acne, or if you’ve recently received radiation treatments or been burned. Any possible side effects, such as redness, generally disappear within hours of treatment. For maximum results, most of these treatments are completed as a series, with periodic follow-up treatments recommended as needed. Although temporary, the effects of the initial treatment can be maximized through proper washing, moisturizing, and regular sunscreen use.

Teeth Whitening Professional tooth whitening is the simplest way to go to improve your smile. The most popular in-office treatment right now is called ZOOM!, which uses a combination of whitening gel and UV light to produce noticeable results in a very short time. Other options include tray whitening systems, which are a bit more cost-effective and controllable sensitivity-wise. Dr. Todd Franklin, a cosmetic dentist in Lodi, prefers hydrogen peroxide gel treatments such as Ultradent’s Opalescence Extra Boost. “It offers results comparable to ZOOM,” he states. “And there are no concerns about potentially negative tissue interaction with the UV light.” For professional whitening at-home, Dr. Ronald Noriesta of One Appointment Dentistry in Stockton has been impressed by the popularity and effectiveness of take-home systems like Crest Whitestrips Supreme. Since it uses a particularly high concentration of hydrogen peroxide to whiten teeth, such kits are currently available only through a dentist’s office. In order to avoid pain, inconsistent bleaching, and other complications, anyone considering tooth whitening should take care of other existing dental problems first, including active cavities, tooth or gum recession, extreme sensitivity, etc. It should also be kept in mind that, while tooth whitening has come a long way, there are still some stains/ discolorations that may be too deep to be completely removed. After any professional whitening treatment, results can be maintained through proper cleaning, lowering the intake of the worst staining offenders (coffee, red wine, smoking), and using over-the-counter whitening strips, trays, or pastes as needed.

Orthodontics For more severe structural problems, orthodontics have also come a long way. Gone are the days of painful and unsightly wire braces. Today, dentists that specialize in orthodontic procedures, such as Cameron R. Wheeler, DDS, MS of Wheeler Orthodontics have the Invisalign System and Clarity Braces to correct bites. To determine the proper plan of attack to create a perfect smile, Dr. Franklin offers a complete “smile makeover,” which entails a thorough analysis of the entire face. From there, photographs, models, and ongoing examination will help formulate a cosmetic blueprint that will enhance the final result. In addition to whitening and braces, veneers, implants, crowns, and bridges are just some of the other procedures that could be recommended to get a white, straight smile.

But good dental care extends far beyond just having a great smile. According to Anureet Sohi-Thadwal, DDS of All About Smiles in Stockton, taking good care of your teeth can actually save your life. Gum disease is a bacterial infection and, therefore, can spread and cause other problems throughout the body. Stockton general and cosmetic dentist Maywell L. Baby boomers approaching their fifties and sixties are showing no signs of slowing down. Instead, they are seeking ways to be healthy, active, and do all they can to reach their seventies and eighties and beyond. While it’s true you can’t fight time indefinitely, lifestyle choices such as the following can help ensure a long, healthy life. »

Eating Well As one grows older, nutritional requirements change. The types of foods that were good for you when you were younger are the same when older: fruits, vegetables, lean meats, grains, and low-fat dairy, but the amount of food you should eat may change. Every year over 40, metabolism continues to slow down. So, if weight is a concern, portion sizes should be smaller than when younger.

Your system may also become less adept at absorption of vital vitamins and minerals, so you may need to increase your intake of foods rich in calcium and vitamin D for bone health, B-vitamins and folic acid for mental acuity. Taking a multi-vitamin specific for the nutritional needs of seniors can help make sure you’re getting all the nutrients you need. Physical Activity In order to remain flexible, strong, and wellbalanced, you need to keep your muscles working. Regardless of age, thirty minutes of moderate exercise each day is always recommended. For flexibility, periodic stretching exercises can maintain proper range of motion and alleviate the pain of osteoarthritis and other forms of joint pain. When it comes to strength and endurance, some form of weight training and/or aerobic exercise like walking or jogging improves muscle mass, cardiovascular function, and has also been shown to improve bone density for those prone to osteoporosis. Falls are the leading cause of accidental death and serious injury for older adults. In order to avoid this, one can improve their balance by doing exercises requiring focus and slow controlled movements, such as tai chi or yoga. In addition to staying physically fit, the firing of neurons and release of chemicals in the brain during exercise can also help improve mood and cognitive function over the long-term. Reducing Vices Basically, drink alcohol only in moderation and quit smoking altogether, which is a good idea at any age. Positive Relationships As hinted at above, one’s physical health and mental health are very strongly connected.

Although difficult to measure in any concrete manner, if you feel connected and happy, it seems that you’re more likely to be healthier than those without such connections. Having a strong support system of friends and family is essential to maintaining a positive outlook. Stress Control A calm mind is a healthy mind. Finding proper ways to cope with anger, stress, or depression can greatly improve one’s quality of life. Things like meditation, yoga, or tai chi can help relieve stress and refocus your spiritual center. If you feel blue or lethargic, and meditation just isn’t doing the trick, don’t be afraid to reach out for professional help. Talk to Your Doc Regardless of your general health, age is itself a risk factor for any number of health conditions. The chance of acquiring heart disease, macular degeneration, arthritis, stroke, Alzheimer’s, and other forms of dementia increases as one ages. Family history is another way to predict potential health problems. That is, if your father passed away from a heart attack, you should consider keeping a close eye on your heart and arteries. Keep in mind that prevention is the best way to fight illness, so be sure to keep up with regular maintenance and see your doctor for your annual physical. For women, a mammogram is recommended every one to two years over 40, and screening for osteoporosis should begin at 65 (unless risk factors dictate earlier). For men and women over 50, be sure to get regular screening for colorectal cancer. Apart from your regular check-ups and screenings, if you feel a bit off, don’t make excuses or ignore your body. Don’t let pride be the reason you were not able to, as the saying goes, live long and prosper.

Kaiser Permanente Modesto Medical Center The Kaiser Permanente Modesto Medical Center is the first new hospital constructed in the Central Valley in the past forty years. As such, it was built using the most environmentally-friendly techniques possible to minimize the hospital’s footprint while also subsequently improving patient comfort and care. The new facility features a full-service hospital, pharmacy, and more than 100 practitioners’ offices in over fifty departments. It also has a 36-bay Emergency Department; 112 licensed beds; twenty-four labor, postpartum, and delivery beds; and six neonatal intensive care beds. If a stay is necessary, the main goal is to make it as comfortable as SAN JOAQUIN MAGAZINE

possible. The hospital features all private rooms with natural sunlight and garden views, an acoustic design that reduces noise, and a clean air filtration system that limits airborne bacteria. It’s also a family-friendly environment with pullout beds for overnight stays, wireless Internet access, large visiting areas, and a healing garden for quiet reflection. At the adjacent Modesto Medical Offices, members have access to Adult Medicine, Women’s Health, and other specialty departments including Cardiology; Ear, Nose, and Throat; Endocrinology; and Gastroenterology. Lodi Memorial Hospital Lodi Memorial Hospital (LMH) is a not-for-profit, acutecare hospital owned by the Lodi Memorial Hospital Association. For 58 years the hospital’s mission has been to provide quality medical care, education, and support services to the community and to improve community members’ quality of life. LMH is licensed for 270 acute-care beds, with services including emergency, maternity, nursery, pediatric, intensive care, acute physical rehabilitation, and transitional, surgical, and medical care. Several satellite clinics are used to provide a variety of inpatient and outpatient services, including urgent care, primary care, pre-natal, pediatric, pulmonary medicine, occupational-medicine, and wound clinics, as well as a free clinic for the uninsured. Three community clinics offer care in Lodi, as does another in North Stockton. A Lodi endoscopy center and an outpatient-surgery center in North Stockton are among LMH’s affiliated businesses, with services including a clinical laboratory, cardiac-catheterization laboratory, ultrasound, CT scan, nuclear medicine, x-rays, respiratory, pharmacy, and dietary services. Physical, occupational, speech and cardiac rehabilitation are among the other outpatient-treatment programs. The hospital is also very involved in offering community services. They provide adult-day care, pre-school care, a community-fitness center, free physician referral, parish-nurse programs, and free health screenings at a variety of public events throughout the year, and speakers to community groups upon request. A Healthy Plan for San Joaquin

When it comes to health, many people have a trusted community physician, a neighborhood pharmacist, and a favored area hospital. With so much of healthcare being local, why wouldn’t somebody want a local health plan that works closely with these providers in coordinating that care? That was the thinking in 1994 when San Joaquin County decided to develop its own community health plan— Health Plan of San Joaquin. Now, over 15 years later, HPSJ, as it’s commonly called, has reached a membership of 100,000 area

residents through a focus on customer service, quality, and community involvement. Health Plan of San Joaquin was initially launched to serve local families in the State Medi-Cal program, providing health benefits for low income and vulnerable families. Since its inception, HPSJ has expanded to serve more Central Valley residents. In addition to being the leading health plan among San Joaquin County Medi-Cal participants, HPSJ is now the largest health plan in the area serving children through the State’s Healthy

Families program. In 2009, Health Plan of San Joaquin began offering AIM (Access for Infants and Mothers), a health coverage program for uninsured or underinsured women who are pregnant. “There’s no question that being local has had a positive impact on our success,” says David Hurst, the Health Plan’s vice president of marketing. “For members, the idea of being served by a plan that is highly visible in the community and easily accessible is unique. We’re able to be involved with our members in a more direct way because we’re right here.” Some high touch efforts to work with members include personal case management, face-to-face customer service, community outreach, and a special member advisory committee that provides feedback and suggestions to the Health Plan regarding its programs. Hurst notes that many of HPSJ’s efforts to engage with members are particularly tailored to the community. “We’ve developed health improvement programs that allow us to work with trusted community agencies, schools, churches, non-profits, and other trusted organizations that serve our clients to promote preventive care, wellness, and the importance of health coverage for families,” says Hurst. Beyond customer service, the Health Plan’s commitment to quality extends to the health services members receive. Quality measurements among health plans have placed HPSJ among the highest in the state in areas such as immunizations and well-child exams. HPSJ has emphasized preventive care for members by developing an incentive program that rewards members for getting appropriate care with gift cards for adults and movie tickets for kids. HPSJ’s focus on access has resulted in an expansive physician and provider network, including nearly 200 primary care physicians and over 600 area specialists. HPSJ is also the only plan participating in the Medi-Cal and the Healthy Families programs with relationships with all hospitals in San Joaquin County. Beyond the benefits and services provided to members, the Health Plan also places a priority on investing in community health initiatives. In 2009, the Health Plan provided $150,000 in grants to community-based organizations for their health programs focused on access to healthcare, nutrition and physical activity, and other key community needs. HPSJ also launched the Health Careers Scholarship Program, providing funds to graduating high school students that plan to pursue health professions and bring those skills back to San Joaquin County. HPSJ’s success in San Joaquin County and unique model as a community health plan prompted its expansion to Stanislaus County (Healthy Families and AIM) and Merced County (Healthy Families). Outpatient General Anesthesia is recommended for apprehensive children, very young children, and children with special needs that would not work well under conscious sedation or I.V. sedation. General anesthesia renders your child completely asleep. This would be the same as if he/she was having their tonsils removed, ear tubes, or hernia repaired. This is performed in a hospital or outpatient setting only. While the assumed risks are greater than that of other treatment options, if this is suggested for your child, the benefits of treatment this way have been deemed to outweigh the risks. Most pediatric medical literature places the risk of a serious reaction in the range of 1 in 25,000 to 1 in 200,000, far better than the assumed risk of even driving a car daily. The inherent risks if this is not chosen are multiple appointments, potential for physical restraint to complete treatment and possible emotional and/or physical injury to your child in order to complete their dental treatment.
