Two year pilot –  1.6 WTE Discharge Liaison 9:00-5:00 7 days a week  1 WTE Project Co-ordinator 9:00-5:00 Monday to Friday  8 WTE Healthcare Assistants 8:00-22:00 7 days a week Available to bridge gaps of care support All of these will actively work together to reduce the amount of bed days within an acute Hospital and enable people to get to their preferred place of care in an organised and timely manner.
