Q: Should people in their 20s get skin cancer screenings? A: Absolutely! We’ve spotted skin cancer in patients under 20 and have saved many lives in the process. We are experts in detecting skin cancers. Although there are various scanning techniques, a full skin exam by an experienced dermatologist cannot be overemphasized.
A: Dermatologists have the most experience in medicine in treating and diagnosing skin disorders, and in my case, with 35 years of dermatologic practice I have seen and treated almost every skin disorder. As dermatologists, we have all the most up to date technology to assist us in diagnosing and treating patients when necessary. which is a procedure that can be done quickly and painlessly and yield results in a matter of days. From there a treatment plan will be recommended based on current and acceptable medical protocol. Q: What are your thoughts on teledermatology? A: I’ve been aware of the field of teledermatology since the 80s. Although it has great potential, the field has not been developed yet, in my opinion, to proceed with this type of practice. I can see that at some point the accuracy will be acceptable and the supervisory mechanisms will be improved. But for now seeing a licensed provider face-toface would yield the highest quality. Q: I have a wart. Why do I need to see a dermatologist? A: Warts are viral and don’t necessarily respond to topical OTC treatments. A multi-disciplinary approach is most effective. We treat warts with a combination of nitrogen gas, lasers, and electric cautery needles. We also used sophisticated prescription topical treatments that stimulate your own body’s immune system to destroy the way by harnessing your body’s own defense mechanisms.
“The cause of hair loss is different with every patient, most importantly we need to know what’s causing the hair loss and that enables us to offer a more successful potential treatment.” Q: There’s a lot of info about men and hair loss, but what about women who are suffering from hair loss? A: The cause is different with every patient, most importantly we need to know what’s causing the hair loss and that enables us to offer a more successful potential treatment. Our investigation may include scalp biopsy and blood work including hormone testing. I find our success rate for regrowth of hair is much higher when an actual diagnosis is found. If there is a hormone issue, such as a thyroid disorder, we’d treat that. In other instances an underlying disease may be present and treating that can achieve regrowth. Steroid injections can be very helpful in certain instances to achieve a rapid regrowth of hair. Q: What do you do for patients who have skin tags around their face and neck? Should they be removed? A: They can always be removed for cosmetic reasons and should always be removed if they are causing irritation or discomfort. Before removing these growths I always like to take a small laboratory sample for diagnosis so I can determine the best procedure to employ. Many techniques are available for removal including laser, cauterizing, nitrogen gases and various topical preparations. With experience, I choose the method that will give the best cosmetic results and cause the least amount of discomfort.
Q: What about all of the people who complain about deformed or yellow nails? A: The first step is to diagnose the disorder. Some nail issues are due to internal disorders, while some are due to external infections, which may even be acquired by manicures. One of the most common diseases that cause nail deformities is psoriasis. By treating the underlying psoriasis, one can achieve a more normal looking nail. The predominant cause of infectious nail deformities are fungal infections and these can be treated with external prescription products or with oral antibiotics. It should be noted that it’s always safer to treat nail infections externally since oral treatments have a higher incident of side effects. With my experience over many years, I try and determine which therapy will have the most impact.

Diana is also helping Nicholas, her husband-to-be, with his skin care strategy for the big day. Here, Diana and Nicholas share their insider tips and tricks to making their wedding day a showstopper.

SKIN HOMEWORK I have kept a great skincare regimen at home or what I call my “skin homework”. Keeping my skin clear is a top priority. I use a mixture of cleansers including Neutrogena Ultra Gentle Foaming Cleanser, Neostrata Foaming Glycolic Cleanser and Revision Brightening Cleanser. I tone my skin with Revision Soothing Rinse. My facial creams include the Neocutis line; Lumiere Eye cream, Journee, Biocream, BioSerum and Hyalis. For any acne breakout needs, I use our Clear Clinic “Vanished” for spot treatment and ClearME Softly pads 1-2x per week as needed.

My bridal prep has been very simple and natural. My focus has been to only maintain pure, organic beauty and glow as I advise all my patients, especially those that are brides. My approach in my very own bridal rejuvenation includes prevention and maintenance. My twin sister, Laura Palmisano, PA-C, has played an integral role in my bridal prep as she has performed my treatments and has

our wedding date. Keeping those lines away and an open, vibrant eye is done best with Botox in the upper 1/3 of the face. I had “light and gentle” Botox with one treatment seven months prior to the wedding, I will do it again three months prior and lastly two weeks prior to our wedding date. I adore lip rejuvenation and having a nice pout for my future husband-to-be is a must! I had some Juvederm Ultra for my lips eight months prior and will do it again one month prior to our wedding date so I’m perfectly pout ready again!
THE HAIR AND BODY PL AN Hair and body are two major factors in my bridal prep. As for my hair, I want to keep shiny, healthy and strong hair. I make sure my diet is rich in vitamins and nutrients essential for my hair and my body’s needs. Biotin, Calcium and Vitamin D are essential to me. My goal is to add one red light hair treatment monthly starting six months prior to the wedding for better circulation and scalp rejuvenation. Keeping a strong exercise program is always a part of my lifestyle. Endurance training includes spinning (SoulCycle, of course) and a running program (with my fiancé or my twin sister is always best) as well as strengthening and exercise for my mind and soul with hot yoga.
THE FINAL TOUCH The big and final question always is, “what do you do right before your wedding?” The last week before becoming a “Mrs.” I will make sure to have a Laser Genesis treatment and a red & blue light session. You will find me on the morning of our wedding day drinking Fiji Water while resting with our famous Derm Duo Ice Mask for ultra wedding day hydration. My skin will be supple and ready to take on the big day!
THE GROOM’S GAME PL AN My prep has been through Diana’s words and guidance. Aside from managing a crazy work schedule and our personal life, exercising and racing are a huge part of my life. My major concern is keeping my skin clear for the wedding as my physical activities and racing is harsh on my skin.
HEALTHY HAIR AND BODY Exercise and a proper healthy diet will continue as we approach our wedding day. I will continue my multi-discipline high endurance and strengthening program including CrossFit training, running, competing in my obstacle course racing, Spartan Races, and NYRR races. I want my skin to keep up as well. Keeping my hair full and strong for our wedding day is important as well. Six months prior to the wedding I had PRP (Platelet Rich Plasma) injections to my scalp and then a second PRP treatment to my scalp one month later, five months prior to the wedding.
SKIN HOMEWORK My skincare regimen includes facial cleansing with Neutrogena Ultra Gentle Cleanser and Clear Clinic’s T2 cleanser. I use Clear Clinic’s O2 Cleanser on my chest and back 2-3x per week as needed to keep skin acne clear. I use Neocutis products including Lumiere Eye Cream (this cream really works!) and Biocream. I find these help with my lines around my eyes and making me look less tired. If I need added hydration after a workout I carry Neutrogena Oil-Free Moisturizer for
quick light moisture that won’t break me out. I use Clear Clinic AttackBack Pads on my face or body as needed when I have acne and to prevent acne, 1-3x per week. It controls oil production, especially with intense exercise.
IN-OFFICE SKIN TREATMENTS To keep my pores tight and improve the overall appearance of my skin I will have 2-3 Laser Genesis treatments starting two months before the wedding with the last treatment one week prior to the wedding.
THE FINAL TOUCH I have noticed staying hydrated is an important factor for my skin health both inside and out. I will make sure to stay richly hydrated prior to the wedding day, especially the day prior and morning thereof. I will then only have to put on my Lumiere eye cream and be ready to marry the love of my life!

DIANA + NICHOL AS’S TIPS FOR BRIDES AND GROOMS We suggest picking the goals you have for your skin, hair and body for your wedding day. If acne is a concern, the sooner you start taking control and getting your skin clear, the better. This goes for body preparation as well. For general skincare concerns, it is always good to check in with your dermatology provider six months prior to your wedding day. Having a skincare regimen that you follow and are consistent with will help you to reach your wedding day skin goals.

SECRET WEAPON BEAUT Y PRODUCT Ultimate lash booster Latisse along with lash extensions by Courtney Akai. It helps to shorten my beauty routine in the morning because I wake up looking like I am already wearing makeup. This is key as a busy working mommy of two. The Courtney Akai boutique in Soho is my one of my favorite beauty addictions. Other than the whole studio being hot pink adorned with glitter and feather boas, they make lash extensions look fabulous and natural. The lashes are lightweight, but look voluminous because of a special multi-lash technique that they use to mix it up. It is the latest method in lash extensions. Some of the old methods many extension places used at the beginning of this lash craze put more stress on the natural lash causing alopecia or loss of your natural eyelashes. The latest ones do not harm or weigh down the natural lash, so there is never a time you would need a break. I also experience no irritation or itching from the adhesive they use and I have very sensitive skin.

me just the right glow all year long. WHICH COSMETIC PROCEDURE DO YOU THINK EVERYONE CAN BENEFIT FROM? Botox! It’s non-invasive, has little if any downtime, and effectively treats dynamic wrinkles, which appear when we move our face—like when we frown or smile which we have been doing since we were babies. Botox typically lasts 3-4 months and then lines begin to reappear and need to be treated again. It helps maintain a refreshed, well rested look and fits into anyone’s active lifestyle, from a busy mom to a Wall Street banker. my beauty routine every three months. It’s a quick and easy procedure and I love to look better rested than I am since I chase after two toddlers a lot and work full time. I personally like to keep my injections natural looking where I still have some movement in my face and I am not completely frozen. I find that to be more of a trend with patients now. They want to look natural. They don’t want anyone to know they have had anything done. I also have injections with one of the HA (hyaluronic acid) fillers usually one or two times a year. Most women begin to experience loss of volume in their face in their 20s and it is all downhill from there. Combining a neurotoxin to help those frown lines with a filler can take years off the face. Other cosmetic procedures that are part of my beauty routine are chemical peels to help with my hyperpigmentation, aging skin and rosacea. The VI peel precision plus done in my office is a favorite. It contains many different ingredients such as trichloroacetic acid (TCA), salicylic acid, retinoic acid, kojic acid,
hydroquinone, hydrocortisone L-ascorbic acid.

I also try to keep up with treating spider veins on my legs once a year before the spring and summer months. Many factors can contribute to the development of spider veins on the legs such as pregnancy, genetics, weight gain, occupation or activities that require long periods of sitting or standing. Most spider veins can be treated with a combination of sclerotherapy injections and laser therapy to get legs in shape for wearing skirts and dresses.

TREATMENT: K YBELL A BY ALLERGAN What it is: Kybella is being touted as the anti-double chin drug. Kybella injections are made up of a nonanimal substance called deoxycholic acid, which is naturally found in our bodies. How does it work: “When injected into tissue, Kybella physically destroys the cell membrane and can treat moderate to severe fat pockets,” says Julie Zuckerman, PA-C. And the good news is that it destroys the fat cells for good. In other words, once your double chin has disappeared after Kybella injections, it won’t return.
How many treatments needed: Approximately 50 injections are needed per treatment session. Most patients see results from Kybella after two to four treatment sessions. Treatments should be spaced about one month apart. Each Kybella treatment takes about 15-20 minutes in the office. While some of their techniques might differ, both mom and daughter know the right path to reaching complexion perfection.

Dr. Hammerman says: Correcting sun damage at home for skin that has become dry from the sun, use a moisturizer that contains glycerin, urea, sorbitol, lactic acid, or AHAs, which are exfoliants that will get rid of dry, flaking skin. Avoid hot baths or hot showers and wash only with lukewarm water using mild, gentle cleansers.
Dr. Flamenbaum says: Apply sunblock 30 minutes prior to sun exposure, the total amount should be a shot glass. Make sure to reapply every 1-2 hours. I like Revision Intellishade, which has a very nice tint to it as well as full coverage protection with SPF 45. Also, wear a physical cover, such as UPF clothing from Coolibar. Avoid the sun between 10:00am-2:00pm and avoid reflective surfaces, such as sand and water. Get a blanket and an umbrella, so you’re not directly on the sand. I’m a big fan of antioxidants for sun protection. Eating
For painful sunburns, find a cool compress or mist to the area with cool water. You can take ibuprofen or aspirin. For brown spots, use a prescription hydroquinone or as an alternative, look for a product containing kojic acid, malic acid, or arbutin, which are all ingredients that will help with skin lightening. A vitamin C-based antioxidant skin care product will help treat uneven brown pigmentation, but any type of antioxidant will help boost the skin’s appearance if it’s sun damaged.

Correcting Sun Damage in the Office For treatment of sun damaged skin, I use topical agents Fluorouracil and imiquimod, which will rev up the body’s defense mechanisms and treat sun-damaged cells. You can also spray liquid nitrogen or treat with chemical peels. It’s not possible to reverse most of the effects of long-term sun exposure, but a dermatologist may be able to improve the appearance of the skin by prescribing tretinoin, which is a derivative of Vitamin A. You can do laser resurfacing, chemical peels, LED or IPL lasers to treat brown spots and visible blood vessels. Microdermabrasion is also great for rubbing away the

superficial layer of the skin. Then there are fillers and Botox to temporarily reduce wrinkles that have already formed. To correct actinic keratosis (tiny bumps that look like sandpaper and have a pink or reddish tint), we use Photodynamic Therapy (PDT), where a light sensitizing solution is absorbed by the AK and then activated by a blue light which destroys it. Products to Treat and Correct Sun Damage: Revision Vitamin C Lotion to protect and provide antioxidant effect, it also has moisturizers in it. ANTI-AGING PREVENT Dr. Flamenbaum says: Be aware of lifestyle factors. The sun, sleeping on your stomach or side (the best position is to sleep on your back!) and squinting, can lead to frown lines. Skincare Product Recommendations I recommend exfoliation twice a week. I love Crystal Peel Microdermabrasion Exfoliating Creme, with micronized crystals. Exfoliation is very important because it stimulates collagen and elastin and it helps reveal a healthier, younger looking skin. Avoid skincare products containing sulfates, which are harsh to the skin, alcohol, benzoyl alcohol, they strip away skin’s natural oils and lead to early aging and irritation. Mineral oil, which is in many products, can form a film over the skin and clog the pores, inhibiting the skin’s natural ability to get rid of toxins. The other thing to avoid is chemical sunscreens, they can encourage free radical damage.
CORRECT Dr. Hammerman says: Anti-aging at home. A retinol cream will stimulate collagen, increase cell turnover and get rid of dead skin cells on the surface. That new collagen ends up puffing out the skin and filling in those fine lines and wrinkles. The OTC retinols may be a good place for a beginner to start. You are supposed to get a little bit of irritation, that’s how you know it’s affecting your skin, so if you feel like an OTC isn’t producing any irritation, you can move onto a prescription strength. If someone has been on prescription topical treatments for acne and their skin is not responding, then I’ll start them on antibiotics. We do the topical for about two to three months before we say it’s not working. Or if they come into the office and their acne is moderate to severe, then I’ll start them on antibiotics immediately. Hormonal acne refers to women who are getting acne in their 30s, 40s, 50s. They never had acne before, but are now breaking out. Cysts around the jaw and lower face usually point to hormonal acne. For those patients I’ll recommend a birth control pill and Spironolactone as well. Adult acne in men is pretty similar to teenage acne, it’s inflammation in the hair follicles, a lot of time this happens from shaving. At Home Acne Treatments To correct acne at home, wash your face gently two times a day, do not squeeze pimples, which can lead to infection and scars. Use a moisturizer to keep skin from drying out, make sure it’s non-comedogenic. Use benzoyl peroxide, which helps unclog the pores and salicylic acid to dry up pimples and peel the top layer of skin.

Recent reports of celebrities undergoing the laser to get their tats removed has influenced others with ink to see the dermatologist for a tattoo removal session. But it’s important to understand the process beforehand. Marianne Pistilli, a physician assistant and tattoo removal specialist, answers some key questions about laser tattoo removal. What are the most popular reasons people come in for tattoo removal? It’s mostly regret – maybe an old partner’s name, some silly symbol or it’s huge. Some folks have religious exceptions or maybe they are getting married and their family never saw it.

Have you seen an uptick in tattoo removal in reaction to social media (for example, someone didn’t like how their tattoo looked on Instagram)? I haven’t really seen those types of regrets – but there is a lot of media around gang members or prisoners wanting to change their lifestyle and wanting to start with a clean slate. Others don’t like the way the tattoo has faded (some can have a “dirty” appearance as they fade). Others just don’t like the idea of a “tramp stamp.” I also have lightened tattoos for folks

Many patients somehow believe it can be removed in one session and that’s just not true. Will the tattoo disappear completely after treatment? There is sometimes a slight shadow of the tattoo after it is finished being removed but that normally recedes over time. Who receives the best results? Definitely the lighter the skin, the better the result – because pigment in the skin can also be lightened by the laser. The less professional the tattoo is, the easier it is to remove. Black is the easiest color and red or yellow can persist for a long time. Advice to those who are thinking about tattoo removal? Be prepared to need 8-20 treatments spaced about two months apart. It will probably cost more to remove than it cost to put on!

Today, science is demonstrating that exercise can improve skin health too, making your skin look younger and clearer.
THE CORTISOL CONNECTION

Exercising outdoors increases your exposure to UV radiation, which increases your risk for skin cancer and premature aging. Exercising without proper hydration can also lead to a drier, less firm appearance of your skin. Some skin conditions may be made worse by exercising. Rosacea can flare with the increased circulation that occurs during exercise. Those with this condition may need to keep exercise intensity lower or participate in exercise that can keep the body
Here are a few tips that can help to further improve the benefits that exercise can have on your skin health. • Don’t wear make-up when you exercise. This can lead to clogged pores and increased breakouts. • Don’t rub, pat. When you sweat pat your face dry with a clean towel, don’t rub. The friction caused by rubbing can lead to increased breakouts and strips your skin of natural oils leading to drier skin. • Do thoroughly cleanse your skin immediately after working out. Bacteria can flourish on unclean skin and lead to breakouts or infection. Fungal and bacterial infections can be picked up where you work out. Washing up immediately after your workout greatly reduces the possibility of getting these infections. • Do drink plenty of water. Proper hydration is essential when you exercise for normal overall function. Failure to do so will also have negative effects on the appearance of your skin. • Do moisturize after your shower. The
evaporation of sweat from your skin can cause dryness. • Do protect yourself from the sun if you exercise outdoors. A good quality sunblock/sunscreen with zinc or titanium oxide is best. Use a product that has an SPF rating of 30 or higher, is water resistant and provides UVA and UVB protection.
