Letter from Schweiger Dermatology Group’s Founder Welcome to the Spring 2015 edition of The SkinNY! We are happy to present to you the second edition of The SkinNY, our quarterly magazine covering all aspects of beauty, health and wellness. While The SkinNY focuses primarily on beauty and cosmetic dermatology, I also want to briefly discuss skin health. Springtime marks the beginning of the warmer months, which means daily exposure to the sun’s dangerous UV rays. May is also Skin Cancer Awareness month. One in five Americans will

develop skin cancer at some point in their life. The key to our fight against skin cancer is prevention, early detection and treatment. For prevention, we must focus on sun avoidance and proper use of sunscreen: Avoidance of peak sun times of 10 am – 4 pm, seeking the shade while outdoors and using a broadspectrum SPF of 30 on all exposed areas should be your top priorities this season. In addition, early detection is paramount in the fight against skin cancers, and this means regular self-skin exams and annual full body screening by a dermatology provider. I strongly urge you to make an appointment for a full skin exam if you have not had one in the past 12 months. If you are found to have skin cancer or pre-cancer, early treatment can make all the difference. On a lighter note, we’re incredibly fortunate to live in a region where we have the best options for total body

BRIDAL BEAUTY BOOT CAMP Vow to get your skin in the best shape of its life before your “I do’s” with our expert advice. Here, our providers share their bridal skincare action plan. It’s wedding season and whether you’re walking down the aisle or watching the nuptials from the audience, a gorgeously clear and glowing complexion will be your best accessory. But, getting your complexion to look wedding-ready takes time and patience. Ideally you should start prepping your skin for the big event 3-6 months in advance. “To have the best complexion for your wedding, the earlier you adapt basic skincare habits the better,” says Dr. Sandra Kopp, who is getting married this spring. Here is her pre-game strategy for dreamy, creamy and clear skin. At-Home Skincare Rx As an overall skin beautifying strategy, Dr. Kopp urges her bride-to-be patients to increase water consumption, obtain adequate sleep, use a daily SPF, eat a diet rich in antioxidants and green leafy vegetables and limit processed and high glycemic index foods. Basic topical regimens should include a twice daily eye cream, hydrating serum or moisturizer, daily sunscreen and a topical retinoid/retinol, which will help

decrease fine lines and make your skin appear more brilliant. If your skin can’t tolerate a retinoid/retinol product, you can substitute glycolic acid preparations. An exfoliating mask can be used as much as twice weekly (less if you have dry, sensitive skin) to keep pores clear. Lasers, Lights and Peels A series of superficial peels can also help with pigmentation, acne, and sun damage, but plan on at least 2-3 treatments a month apart. “For significant pigmentation reduction or faster results, I would recommend Fraxel 1927 about 3 months before the wedding to allow proper healing time and leave room for more treatments if necessary,” says Dr. Kopp. Redness associated with rosacea or past acne breakouts can be effectively treated with lasers and light therapy. Laser Genesis, a laser facial treatment, can decrease pore size, help with redness, and smooth fine lines. Timing: Laser Genesis: 4-6 treatments 2 weeks apart; Fraxel 1927 about 3 months before the wedding.

Injections For those “11 lines”, crow’s feet, or forehead lines, consider neurotoxin (Botox, Dysport or Xeomin) treatments,” says Dr. Kopp. “If it’s your first time, have a treatment more than three months before the wedding as a trial. Once you are happy with results, re-treatments or touch-ups can be done about a month before the wedding,” advises Dr. Kopp. For volume loss, such as tear troughs, or lines around the mouth, hyaluronic acid fillers will do the trick. Timing: Hyaluronic acid fillers: 2-3 months prior to the big day; Neurotoxins: 3 months before wedding and then one month for touch-up. Body Contouring Other treatments a bride may consider are Coolsculpting or Vanquish to decrease hard to reach pockets of fat to ensure a smooth fit into their wedding dress. Timing: Best to start 4-6 months prior. It can take 2-3 months to see results and it may take two treatment sessions.

3 MONTHS OR LONGER BEFORE THE WEDDING

Make sure you have a topical regimen that works for you, including a good eye cream, SPF, moisturizing serum or Months cream, and an anti-aging topical treatment (retinoid/ retinol, glycolic acid, Vitamin C preparation). Be careful with Vitamin C products as they are not standardized and can be difficult to keep stable in packaging. All acne, laser, and injectable treatments should have been started by this point. Mature brides may consider Ultherapy to help define the jawline and tighten the jowls for flawless photos on their big day. If you want to get rid of any facial or body hair, you should plan at least 6 months ahead of time for laser hair removal, as you will likely need more than one treatment. If you’re envisioning luscious lashes then turn to Latisse, which can take 16 weeks to show full effect, however most people notice a difference within a few weeks.

2 MONTHS BEFORE THE WEDDING You should be continuing the topical regimens and lifestyle changes made above, but can also add in maintenance treatments such as superficial peels for surface Months irregularities or Laser Genesis to help with pore size and brilliance of skin so your makeup can lie nicely on the big day. If the acne is almost clear but not perfect you can add in blue/red light treatments to enhance healing.

Dr. Kopp’s Personal Pre-Wedding Skincare Plan “I am using Revision Intellishade daily, which has antioxidants and SPF with a light tint for coverage. I also use their Vit C serum and hydrating serum in the morning and Neocutis Biocream with Retin A gel, which is prescription, at night. I use Neocutis Lumiere eye cream twice a day. I also am planning on having IPL and Laser Genesis for redness and pore size, as well as maintenance Botox touchups to help with dynamic lines. Make sure to try any new skincare products at least one month before the wedding, as you do not know how your skin will react.”

Glossary of Bridal Treatments Isolaz: Is a pore-purification laser that involves vacuum suction to clean out pores and pimples and then uses a broadband light that helps to directly kill acne bacteria. There is some redness immediately after the treatment that resolves within the hour. This helps to calm active acne and reduce residual red marks from healing acne lesions. PDT: Photodynamic Therapy is a combination therapy that directly kills acne causing bacteria and modifies oil glands that harbor the acnecausing bacteria which results in long-lasting remissions of breakout. There is also a reduction of the residual redness from old acne spots.

Red/Blue Light: Blue Light works to directly kill acnecausing bacteria and Red Light aids in the reduction of inflammation and residual redness/red marks from acne. It is a 30 minute treatment with no downtime. Fraxel Dual: Non-Ablative resurfacing laser is a treatment for acne scars and markings such as pitted acne scars and pigmentationhyperpigmentation. It will help production of collagen and improve texture of skin. There is 3-7 days of downtime, depending on settings and target treatment.

Laser Genesis: “Laser Facial” used to improve the appearance and texture of the skin. It is a 1064nm laser that targets microvasculature and stimulates collagen production, while protecting the surface of the skin. Used to reduce size of large pores, overall redness of skin and mild acne scars. There is no downtime. 2D: Two-Dimensional treatment that targets two areas: deeper and superficial treatment. First uses the Cutera Excel V Coolglide 532nm laser to diminish redness of acne marks and improves overall complexion of skin. Then the Laser Genesis is performed, a 1064nm laser to improve texture of the skin

and to reduce pore size with further redness reduction. Expect about 1-2 days of downtime. VI-Peel: A chemical peel that contain TCA, Tretinoic Acid, Salicylic Acid, Phenol, Vitamin C and a Mineral Blend. Can be used on all skin types. It improves the tone, texture and clarity of the skin as well as reduces hyperpigmentation and freckles, including melasma. It helps to clear acne skin conditions and can reduce the appearance of acne scars. This peel helps to stimulate collagen and elastin. But be warned, this peel will make you peel. There is 7 days of downtime.

RASH GUARD

Spring is in the air and on the skin. We get the skinny from Dr. Scott Hammerman on how the warmer season can make us a little itchy and what to do about it.

The SkinNY: What are the most common skin allergens you see in the spring? What are the major culprits? Dr. Scott Hammerman: In the spring, the most common skin allergens patients present to me are typically reactions to plants, such as poison ivy, poison oak, and sumac. These stem from direct contact with the plant itself. Less common skin allergens I see a lot of in the spring are due to ingredients in sunscreens. Although rare, patients can react to fragrances and preservatives in sunscreens. The most common of which are oxybenzone and padimate O, and less commonly avobenzone. Patients who have had skin reactions to these ingredients should avoid sunscreens that contain them. The SkinNY: How do skin allergies typically present on the skin? Dr. Hammerman: Skin allergies typically present as a well demarcated itchy pink rash. The rash can consist of blisters and drain clear fluid. The reaction is typically localized to the area of skin that comes into contact with the allergen. For example, nickel is one of the most common allergens worldwide and patients come with a dermatitis localized to exposure to the nickel. Nickel is a strong silver based metal that is commonly used in jewelry, buckles, and snaps. Patients allergic to nickel often present with dermatitis on the stomach where their belt buckle or the snap from

their jeans rests on the body. In some cases, the distinction between eczema and skin allergies can be difficult. The key difference is the distribution of the rash. Skin allergies classically occur in areas exposed to the allergen. Certain parts of the body are more prone to eczema in adults, such as the hands, face, and skin folds. Psoriasis usually looks very different from both skin allergies and eczema. Although the lesions are often well demarcated like skin allergies they often have a silvery looking scale and are seen on the scalp, elbows, and knees. The SkinNY: Are people who get seasonal allergies in the spring more susceptible to skin allergies as well? Dr. Hammerman: The short answer is yes, as many people with allergies usually have more than one type. However, the two are not usually related. Seasonal allergies as a whole are more common than skin allergies. There is more of an association between eczema and seasonal allergies. A condition known as the atopic triad is the common association between eczema, asthma and seasonal allergies in children. Exposure to seasonal allergens such as pollen and dust can trigger flares in eczema. The SkinNY: What should be avoided this time of year to help prevent skin allergies in the spring?

Dr. Hammerman: In general, the most common fabric related to skin allergies is formaldehyde and formaldehyde resins. Formaldehyde is ubiquitous and found in many cosmetics and, to some degree, in basically all fabrics. The resins are used as a finish on many wrinkle-resistant clothes. Clothing that is 100% polyester is believed to have the least amount of formaldehyde. Clothing with pure cotton and nylon also tend to be lower in formaldehyde. Washing clothes several times before wearing can decrease the amount of formaldehyde present but will not eliminate it. As the weather gets warmer, it’s best to avoid contact with certain fruits and vegetables directly on the skin before sun exposure. Contact with lime/lemon juice or celery prior to sun exposure can cause a skin allergy known as a phytophotodermatitis. A classic example of this is commonly known as “beer drinker’s dermatitis.” It occurs in those who squeeze lime wedges for beer and margaritas, or other drinks before being exposed to the UV rays from the sun. The SkinNY: What are the best ways to treat skin allergies? Dr. Hammerman: The best way to treat skin allergies is to identify the inciting agent if possible and treatment is aimed at preventing contact with the allergen. Skin allergies are identified by a process called patch testing. Patch testing is a relatively simple office procedure done by a dermatologist in which the most


common skin allergens are applied to the back. Results are interpreted 48 and 96 hours later. At the end, your dermatologist will discuss the results with you and advise you on what allergens to avoid. In our practice, we place the most common 80 allergens on the patient. During a flare, symptoms are controlled with topical steroids and antihistamines. Occasionally, oral steroids are needed to control symptoms. The SkinNY: What are the most common misconceptions about skin allergies? Dr. Hammerman: One that I hear often from my patients is that their diet is responsible for their skin allergy. In the

vast majority of cases, diet is not related to skin allergies. An allergic contact dermatitis (or skin allergy) is usually seen about 48 hours after direct contact with a specific allergen. The most common allergens causing skin allergy, overall, are nickel and poison ivy. Patients who react to poison ivy can actually have a skin allergy to certain foods such as cashews, mango peels, and the supplement ginkgo. However, such associations between food and skin allergens are rare. Other common allergens in my patient population are Fragrance Mix I and II, Balsam of Peru, and preservatives such as formaldehyde, which are in many cosmetics. Another misconception is that if a product is labeled “fragrance-free”

they can’t have a skin reaction to it. The label “fragrance-free” can be misleading because these products can contain various preservatives that can still produce a skin allergy. The above is also true if a product is labeled “all natural.” My best advice is that if you think you may have a skin allergy, consult with your dermatologist.

Regardless if you’ve got a hot date or you’re going stag, you’ll still want to look breathtaking at prom. Here, the best beauty tips and product recommendations to make your big night legen - wait for it - dary! “Prom is such an exciting event for teenagers, and it’s a huge motivator to look and feel your best,” says Dr. Rachel Nazarian. But getting your skin in its best condition for prom takes more than just a few hours of primp time before the big night. “Girls looking for a regimen to get their skin glowing should start the discussion with their dermatologist early, as there are an overwhelming number of options,” says Dr. Nazarian, who has created this prom beauty boot camp for teens looking to get their skin into prime condition before the big event. 2-3 Months Before Prom: Fix the basics Nothing is more beautiful than healthy clear skin. If you’re still breaking out with pimples and blemishes it’ll be a wasted effort. Although most dermatologists offer topical creams and lotions to help with breakouts, remember to start

early. These medications need a few months to really kick in and clear your skin. If you’re looking for results with less effort, red light/blue light therapy and PDT light therapy help dry up pimples and decrease redness. These procedures are easy and can fit into a quick lunchtime break or after-school visit. 4-6 Weeks Before Prom: The chemical peel Chemical peels are a great way to brighten up your skin, even out the tone, and get gorgeous glowing complexion. Try the VI Peel—it’s easy to do with your dermatologist, great for sensitive skin and most skin types. I would recommend about three sessions, spaced about three weeks apart. Medicated wipes and pads are also available that can help you slowly achieve smoother looking skin at home. Pads soaked with glycolic acid and acne-fighting ingredients are

great for girls who want something to apply at home to help treat their skin a few weeks before the big event. It’s important to remember that your skin is a reflection of how you take care of yourself and what you’re eating. Keep your nutrition well-balanced and focus on drinking plenty of water, staying away from high-sugar foods or drinks, and filling your plate with antioxidant rich items, such as fruits and vegetables. Hopefully, starting early and making the right choices will give you the best skin of your life, just in time for prom.

Dr. Lotika Singh lets us in on her beauty must-haves and skincare secrets. What’s your beauty product secret weapon? I recently started wearing navy blue eyeliner instead of a basic black one. The navy blue hue helps the whites of the eyes appear brighter and gives a more refreshed look overall. Favorite lip product? I use the Bite Beauty Agave Lip Mask every night. It contains lanolin and other great emollients, which really keep the lips hydrated and well moisturized. Plus the agave gives it a touch of sweetness! Best beauty advice you’ve ever heard? Train yourself to sleep on your back. Side sleepers tend to have more facial volume loss on their preferred sleeping side, which becomes more noticeable over time and leads to asymmetry.

What advice do you give your cosmetic patients who want to turn back the clock? In office procedures such as Neurotoxins, fillers and lasers are very effective, but it’s also important to have a good at-home regimen to maintain results and prevent further damage. Daily sun protection and the use of a retinoid at night really help with day to day UV damage.
