StandingCommitteeonProfessionalExchange(SCOPE) is oneofthesix(6)standingcommitteesoftheCenterfor IndonesianMedicalStudents'Activities(CIMSA)thatengagesin thefieldofprofessionalexchangeandhas17localsspreadin variousfacultiesofmedicinethroughoutIndonesia.Twonational officersleadSCOPECIMSA;NationalExchangeOfficerfor Incoming(NEO-In)andNationalExchangeOfficerforOutgoing (NEO-Out),aswellasmaintainednationallybytheNational ExchangeCommittee.

SCOPEisastandingcommitteefocusingon incominghostingandoutgoing sending,exchangepromotions,globalhealthawareness,andcultural understanding. TheCOVID-19pandemichas highlightedthedailychallengesthat hospitalsandhealthsystemsface: highcosts,infectioncontrol,and patientsatisfaction,tonamejusta few. Althoughourcountrieshavemade stridestotransitiontovalue-basedcare andbettermanagepopulationhealth, Indonesiahasspentmoreon healthcare.Still,itranks11thoutof27 inqualitycomparedtoothernationsin South-EastAsianandthePacific.Aswe allknow,allhealthsystemshave strengthsandweaknesses,andmany reflecttheircountry'shistory,culture, andvalues.Healthcareisacomplex environment,anddealingwithgrowing demandrequireschangingperceptions ofdelivery.Integrationofprimaryand communitycareisdifficultbutessential forreducingpressureontheacute sector.

It'shardtoimaginethataperfect healthsystemcouldexist--onethat wouldmeetallpatients'needsat reasonablecosts. So,whatmusta countrydotoachievethis? Hereare somecomponentswemustacquire, andlet'stakeinspirationfromother countries.

1.Universalhealthcare

TheNHSintheUnitedKingdom offersthebestuniversalhealthcare. TheNHSwasthefirstintheworldto createauniversalhealthcaresystem thatisavailabletoallcitizens,

regardlessofwhethertheycanpay forit.

2.Primarycare

Indeed,Israelhasoneofthehighest lifeexpectancyratesintheworldand oneofthelowestsharesofgross domesticproductspentonhealth. Primarycareissupportedbyfour healthmaintenanceorganizations thatactasbothpurchasersand providersofpreventive,immediate, andcommunityservices.Out-ofhourscareisprovidedaroundthe clockandintegratedwithevening carecenters,urgentcarecenters, andhomevisitservices.

3.Communityservices

Forinspiration,wecanlookatBrazil, wherecommunityteamsmadeupof doctors,nurses,nurseauxiliaries, andcommunityhealthworkersvisit householdseverymonth,whetheror nottheydemandorneedit.These teamsofferimmunizations,chronic diseasemanagement,and screenings.

4.Mentalhealthandwell-being Australiahasmadethemost progressinthisarea;theyoffer publicfundingtoinvestincrisisand hometreatment,earlyintervention, andassertiveoutreach.

7.Researchanddevelopment Researchanddevelopmentare wheretheU.S.shines,notingthe numberofhigh-impactdrugsand medicaldevicestheyhave developed.

8.Innovation,flair,andspeed InIndia,severalorganizationshave createdahub-and-spokemodelthat focusesoncost-effectivenessrather thancost-cutting.Examplesincluded standardizedcarepathways,making iteasiertoshifttasksanddomore withfewerstaff;referralnetworks thatchannelpatientstothecorrect settingsandevenhospitals manufacturingtheirowndevicesor implantswhensuppliersrefusedto reduceprices.

9.Information,communications,and technology

Therearenoout-of-network providersinFrance.Patientscango toanydoctororhospitaltheywish. Patientsatisfactionisexceptionally high,asarethecountry'squality.

11.Funding

Nocountrydoesitbetterthan Switzerlandinfunding,whichspends 11.5percentofitsGDPonhealth, accordingtoBritnell.The governmentalsoboastshighpatient satisfaction,goodclinicaloutcomes, andalifeexpectancyof82.7years. ButBritnellnotesthatthecountry canaffordtospendsomuch becauseitseconomyisglobally competitiveanddynamic.

12.Agedcare

Japanoffersitscitizenscompulsory long-termcareinsurance,which providessocialcareforallthose olderthan65basedsolelyonneed. Theabilitytopayisseparatefrom theassessmentprocess.Theplan offershomehelp,community-based services,andresidentialandnursing care.Thecountryhasalsocreated dementiahomeswheregroups ofpeoplelivetogetherina supportive,home-like environment.

Thisexchangestartedin NovemberlastyearwhenIwas selectedforourNationalLanguage test. Sincethen,Ihaveworkedhardas anurseinahospitaltosaveenough moneytotraveltotheothersideofthe worldtoexperiencesomething differentthanIhaveevertried.Ihadno ideawhatcitytochoose,somychoice wasrandom.IgotSurabaya,my numberonechoice,althoughIdidn’t knowanythingaboutthisplace.WhenI lookedattheinternet,everything seemedsointerestingtome.Traveling toIndonesiawaslovely,andmyarrival toSurabayawaswithoutproblems.

Iwassupposedtobeinthe InternalMedicine–Cardiology department,butintheend,Iwas placedintheEndocrinology department,whereImetnicedoctors, namelydr.DeasyArdiynyanddr. HermawanSusanto.Theyprovidedme withplentyofknowledgeabout endocrinology.Igotthechanceto entertheendocrinologyward,the endocrineICU,andendocrine polyclinics.ImustmentionthatIam thankfulforallthestudentsand residentsinthisdepartment.Theyheld theirmorningreportinEnglishforme tounderstand,whichwasterrific.They arealsoverycommunicativeandvery

curiousastheywantedtoknowalot aboutmycountryandtoldme reciprocallysomeexcitingthingsabout theirs.Ihadtheirnumbersincaseofany emergency,andtheywerealways helpfulwhenIdidn’tknowtheplace whereIwassupposedtobe.

Onthelastweekofmyexchange, I askedthedoctorsiftheycould arrangeformetoworksomehoursin theInternalMedicine–Cardiology departmentsinceInternalMedicine–Cardiologyismyfutureresidency. Theythenintroducemetodr.Yusuf.He wasafantasticmentortome.Hestayed intouchevenafterthehospital,asking meifeverythingwasfineandifhecould doanythingforme.Heisamassive inspirationtome.Healsodidamorning reportheldonlineviaZOOMinEnglish sothathecouldshowmesome interestingcases.Ihopetoseehim againonedayatsomeinternational congressofinterventioncardiology.

Ialsomentionsomethingaboutmy experiencewiththecityofSurabaya.As Icomefromasmallvillageandhave neverlivedinatownbiggerthan 300.000people,thisissomething differentforme.Traffichereis unimaginable,andafterthreeweeks here,Icanthoroughlycrossthestreets withoutfear.Ispendsomeexcellent timeinlovelyplacesandsometimein thesuburbofthecity.Thereisa massivedifferencebetweenthem.

Yogyakarta,Indonesia.Ibecameoneof theparticipantsintheIFMSA-SCOPE ProfessionalExchangeProgram. ParticipatinginanExchangeProgram hasbeenmydreamsinceIwasakid. WhenIwaslittle,Ialwaysimagined beingabletoliveabroadinthousand milesfromIndonesia. discoveredthatmyuniversitycould offertheSCOPEProfessional ExchangeProgram,which cooperateswithCIMSAandIFMSA, itboostedmyspiritstoalways believeinandalwaysdreamabout joininganExchangeProgram.This exchangewillbeanenormous additiontomystudiesandan excellentonce-in-a-lifetime opportunitythatwillcontributeto myadvancementinmanyways.

Iwasplacedinanot-too-smallcity, butanexcellentonecalledTours.Ilive inanapartmentprovidedbythelocal committee.Thecondowasvery comfortableandclean.Thenthe apartmentisalsoclosetothebus stationandsupermarket,which makesitveryeasyformetolive mydaysthere.Istartedmy

surgeries.Inaddition,Ialsometdoctor RamySamargandi,whoisfromSaudi Arabia.Heisanorthopedicdoctor interestedinthehipandknee.Iamalso oftenaccompaniedbyresidentswho areveryfriendlyandkind.Theyhelped mewithmydaysinthehospital.

Ihavewitnessedcountless orthopedicsurgeriesfromtopto bottomduringmytimethere.I'm estimatingthatmaybeI'veseen30-40 surgeriesinamonth.Inaddition,Iwas alsoallowedtobeanoperating assistantfivetimes.Oneofthebest surgeriesIwilleverforgetwaswhenI hadsurgerywithdoctorRamy Samargandi.Thesurgerywas oncologicalresectionofthebone tumor(osteo-liposarcoma)andthe insertionofaprosthesis.Apartfrom that,Ialsofoundvariousothercases, suchasosteosarcoma,osteoarthritis, rotatorcufftendonrupture,hallux valgus,anteriorcruciateligament rupture,andbrokencartilageofthe shoulder.

Besides,Idovariousotherfun activitiessuchasbowling, pizzaparty,andplaying lasergames.Ialsohad timetovisitsome magnificent cathedralsandthe oldcitycenter, whichisvery exotic.Itmade meveryhappy, andIwillnever forgetit.

Studyingabroadisanopportunity thatonlycomesonceinlife. In addition,workingwithdifferentgroups ofpeoplewithdifferentcultureshas improvedmyabilitiesasaperson,and beinganindependentpersonisalso oneoftherecognitionsIhavegained throughthisprogram. Inaddition,this programhasbeenamassive milestoneformebyprovidingan overviewofthediversityinforeign healthcare.Ialsogaindeepinsight intootherculturesandsystemsthat candevelopmeandgivemea differentperspective. It'saprocessto getandexpandasmucheducationas possiblefromothercountrieswith tremendousadvancesinmedical science,especiallyinorthopedichealth sciences.Theseresultshaveprovideda newperceptionofadvancing Indonesianhealthservices.Onthe otherhand,studyingabroadhas improvedmyskillsandeducationin medicalscience,primarilyorthopedic healthsciences.

Afundamentalpartofbeinga physicianorresearcheristheconcept ofbioethics.Itisthefirstthingto consideraswe,medicalstudents,will workonthehumanbodywiththeir singlelives. Inpreparationfordoing so,medicalstudentsmusthaveenough informationandcommongroundin bioethics.

PREBIOTHICS3.0(SCOPExSCORE BIOETHICSDAY)washeld, commemoratingWorldBioethicsDay. Thisyear,weencouragedmedical studentstoactivelycontributetoour activities:socialmediacampaign, publicdiscussion,andpublicposter competition.Wekickedoffthisyear’s PREBIOTHICS3.0byopening registrationforcollegestudentsofany backgroundwithaninterestinbioethics torecordavideowithaconceptcalled “MiddleGround.”Thecampaignset outaspacetotalkandlistento volunteers’argumentsonthegiven promptwithoutjudgment.Sothatthe discussionbecameinteractive,we selectedvolunteerswithnot-so-exact perspectiveseachother.OnOctober 16th,2022,thevideowastakenoffline, withfourvolunteerssigningawritten informedconsent statementbefore.

Inourpublicdiscussion,wedivided participantsintoseveralbreakout roomsforafocusgroupdiscussion.In eachroom,thereweremoderatorsto leadtheparticipantstosharingtheir opinionsoneightquestions.After60 minutes,wereturnedtothemainroom andlistenedtothreespectacular speakers,NabilaPuspakesuma,S.Psi, M.ScofCBMHFK-KMKUGM,Prof.Dr. RiantoSetiabudy,Sp.FK(K),Presidentof theGoverningBoardofMKEKIDI,and Prof.EryatiDarwin,PA(K).,Lecturerof FacultyofMedicineFKUnand,Member ofMKEKIDIWilayahSumateraBarat. However, concernsarisefromsocial,cultural, legal,andethicalpointsofviewofsuch progress,finallyleadingtooneofthe mostsignificantdebatesofthepast century.Anewwordhasbeencoinedto encompasstheseconcerns:bioethics. Andso,theUniversalDeclarationof BioethicsandHumanRightswas agreeduponduringtheGeneral Conferenceon19thOctober2005.

WorldBioethicsDayisacelebration ofthedeclaration’sadoptionduringthe GeneralConference,whichtheCenter forIndonesianMedicalStudents’ Activitiesannuallycelebrates.This year’sWorldBioethicsDaythemewas ‘SocialResponsibilityandHealth.’And so,MedicalStudents’Committeefor InternationalAffairsUniversitas Brawijaya(MSCIAUB)hasalso celebratedWorldBioethicsDayto increaseawarenessoftheethicaland legalissuesrelatingtomedicineand healthsciences.

Thisincludes variousessentialhealthservices,from healthpromotiontoprevention, treatment,rehabilitation,andpalliative care.Countriesexperiencingrapid populationgrowthhaveyoung populationsthatcanpromote economicgrowthandreducepoverty. However,tobenefitfromthe demographicbonus,governments mustinvestinthehealthandwell-being oftheirpeopletobuildhumancapital andpromoteinclusivegrowth. Improvingreproductivehealth, mothers,newborns,children,and adolescents,andovercomingmental healthdisorders,areessentialto achieveUHC. InrealizingUHC,oneof thesignificantstepsthatIndonesia hastakenistousetheNational HealthInsurance,anintegrationof variousformsofhealthinsurancethat havebeenpreviouslyimplemented.

Enteringthemainevent,wehave twoevents:offlinetalkshowsand educationalvideos.First,therewasan OfflineTalkshow,wherewegot speakersfromtheHeadofHealth Services,DinasKesehatanJawaTimur, andmoderatedbytheFinalisDutaFK Unair2022.Thetopicdiscussedwas "UniversalHealthCoveragefromthe publicpointofview."Aswellas educationalvideosarevideosfilledwith talkshowparticipantsaboutdefinitions, opinions,andhopesforUHCinthe future.

Inthepost-event,wechose infographicsandafterthemovie becausewewantpeopletoremember thematerialwe'vediscussedandsee andappreciatesomeoftheflashbacks ofoureventsfornearlythreeweeks.

Thequestionis,a14-year-oldgirl washospitalizedforanabortion.She wastwomonthspregnantfromwhat sheclaimedwascausedbyrape.The familywasdesperateanddemanded thatdoctorsandmedicalpersonnel initiateanabortionforthegirl immediately.Butthegirl’sconditiondid notallowforanabortiontobe executedbecauseofmedicalreasons. Whatwillyoudoasmedicalpersonnel?

Asmedicalpersonnel,wewould beaskedforopinionsandquestions relatedtoourpatient’sconditionsin thenearfuture. Thoseanswers, responses,andsolutionsmustbebased onbioethicsprinciples.

SCOPEandSCOREofCIMSAUPH wantedtointroduceandexpose knowledgeandeducationabout bioethics,healthpractice,research fields,andbioethicsincultural differencestoyourfuturedoctors, nurses,andpharmacistsinthisevent. Alloftheparticipantsfromthis eventwerealsogivenapre-event challenge,whereparticipantsshared postersabouttheimportanceof bioethicsthroughInstagramStoryin howtosolvemoralconflictssothatwe canmakewisedecisionsandhowit couldincreasethequalityofhealthcare, hopingthatpeopleinoursurrounding couldalsounderstandaboutthe purposeofbioethics.Itcanalsobe helpfulandappliedinthefutureasthe participantsareaspiringprofessional medicalpersonnel.

WealsoinvitedDr.dr.TheoAudiYanto, Sp.PD,FINASIMasourspeakerforthis eventwithmanyexcitingmaterials.We weregivenaninteractivelectureand comprehensiveinsightsabout bioethics,whatweshoulddo,whatto expect,andhowtoexpressitto patients.That’snotjustthe excitingpart.Through thisevent, enjoyable activitieswere done,suchasgroup challenges. Participantswere dividedintogroups,and eachgroupwasgiven casesandpresentedtheir discussionina10-minute dramaroleplays. Eachgroupwas veryenthusiastic andcouldexpress thevaluesof bioethicsvery well.

IntheSummerof2018,Iwas fortunatetoparticipateintheSCOPE exchangeprogramasanoutgoing studentatWroclawMedicalUniversity inPoland.There,Icompletedaclinical clerkshipintheSurgeryDepartment, doingroundsandassistingmedical proceduresintheoperatingroomwhile learningtocomprehendthePolish languageandcustoms.Igotthechance tomeetotherincomingstudentsfrom Greece,Hungary,Taiwan,Brazil, Mexico,Jordania,Kazakhstan,Korea, Iraq,andPortugal,creatingadiverse andenrichinginternationalexperience. Everydaywasanewadventure,from theclinicalsettingtoexploringthecity withmyexchangematesandeven travelingtonearbycountrieslike Germanyduringtheweekends.This experiencewillforeverholdaspecial placeinmymemories,andIyearnfor theopportunitytorelivethose moments.
