“While I was still at school and later at university, I worked with a friend’s mum who made beautiful bridal gowns. What she taught me set the foundation for a career as a couture seamstress,” Melinda says. “In London I worked for prestigious couture houses and helped create gowns for Princess Diana and the Queen. I used to make my own clothes when I was so big it was impossible to buy anything, now I am a shopaholic. I now wear a size 18, sometimes a 16. I have so many clothes but they are interim clothes. “As my weight loss journey continues, I buy smaller and smaller sizes. My regular support consultations at Weightloss Solutions Australia at the John Flynn Hospital have become something of a fashion exchange. I give my larger sizes to people who have just started on their weight loss journey. The clothes have hardly been worn and I am happy to pass them on knowing the women who wear them will enjoy them for a short time as their weight loss success leads to smaller sizes,” she says. I was thrilled to see the scales at 99 kilos. “I may reassess my goal and try to get below 85 kilos but I will jump that bridge when I come to it. Right now I am enjoying a new lease on life. I love buying clothes, I enjoy walking and going to the gym occasionally. I am not a sporty person and can’t say I love exercise but there is a big difference in walking with a spring in my step at my current weight than being out of breath and uncomfortable even on the shortest walk at my former weight,” she says.

A matter of taste Scientists have discovered some people have a taste for fat. Deakin researchers Dr Russell Keast and PhD student Jessica Stewart with colleagues from the University of Adelaide, CSIRO, and New Zealand’s Massey University found people with a high sensitivity to the taste of fat ate fewer fatty foods and were less likely to be overweight. Dr Keast says we already know the human tongue can detect five tastes – sweet, salt, sour, bitter and umami (a taste for identifying protein rich foods). “Through our study we can conclude that humans have a sixth taste – fat,” he says. “Fats are easily accessible and commonly consumed. This suggests our taste systems may become desensitised to the taste of fat over time, leaving some people more susceptible to overeating fatty foods. We are now interested in understanding why some people are sensitive and others are not, we believe this will lead to ways of helping people lower their fat intake and aid development of new low fat foods and diets,” Dr Keast says.

Put too much fuel in a car and it spills out, put too much food in the body and it turns to fat. A calorie is one unit of heat or fuel. A kilojoule is exactly the same thing in metric measurement. A calorie equals 4.18 kilojoules. Calories

The results were significant and components of behavioural therapy are now part of almost every treatment for moderate weight loss and obesity. Goal setting, cognitive restructuring, social support, self monitoring, and relapse prevention are important aspects of modern weight management. The 1950s saw the introduction of surgical weight procedures. Early operations involved intestinal bypasses; a large part of the small bowel was removed to reduce food absorption. Many forms of intestinal bypasses were tried but major complications including dehydration and electrolyte inbalance were common. Gastric bypass procedures were developed to counteract the problems and produce notable weight loss. Bariatric surgical procedures continue to evolve. Minimally invasive modern procedures use laparoscopic techniques. These safer procedures give excellent weight loss results. Professional weight loss management practitioners say bariatric surgery is the single most effective treatment for obesity.

There are numerous theories about weight gain but the fact is most of us now eat too much and don’t move enough
– a Modern disease Early man had no issues with weight management. Food had to be caught or found, hunting wasn’t a hobby, it was hard work and people ate only the amount of food they needed to survive. In ancient times, people fasted for religious or cultural reasons. This holds true today as Christians may choose to fast or give up a favourite food during Lent and Muslims fast during Ramadan. As people became more affluent and food more readily available, excess weight became a greater problem. In the days of the Holy Roman Empire, gladiators and their girls would finish a big night out with a trip to a vomitorium for a good chuck in the company of friends. The dangerous binge and vomit fad latest for centuries. The link between food and weight was not understood until the 1700s. Not many people had a weight problem as food was a scarce resource for the majority, only the wealthy ate well. Obesity was seen by some as a disease, a punishment for sinful behaviour, or a sign of affluence. Art favoured the fat, think of those voluptuous ladies classic artists painted – but only the rich could afford portraits. In 1727 Dr Thomas Short published Discourse Concerning the Causes and Effects of Corpulency. This English physician was ahead of his time. He advised overweight individuals to follow a diet of ‘moderate, spare and of the more detergent kind’. Some of his other ideas were a bit hit and miss. Dr Short recommended the avoidance of swamp like areas where the air was moist and soggy. Perhaps this theory was based on fluid retention; lots of people appear to enlarge in hot, humid conditions. In 1835 Belgian statistician Lambert Adolphe Jacques Quetelet developed the body mass index (BMI). This was the first mathematical calculation to measure body fat. The first commercial concerns about obesity and death were highlighted by the life insurance industry. Later the World Health Organisation developed a system of categorising BMI based on health risk. While other measures now exist, BMI is still widely used as a measure of weight related health issues.
Fads and fancies We could construct a diet dictionary from Atkins to Zen but it seems the first low carbohydrate diet book was Letter on Corpulence, Addressed to the Public published in the 1860s by English undertaker William Banting. Perhaps Mr Banting was tired of carrying heavy coffins but he treated his own obesity using a diet designed to treat diabetes

The majority of people regard most of these activities as torture but there is something to suit every body – plump, lean, obese or athletic. Exercising alone is more beneficial for people who want a high intensity workout, according to a study from the American College of Sports Medicine. The study shows people are more likely to work harder on their own, and even more so in a gym than outdoors. The study speculates this is due to the variety of strength and cardio equipment available in a gym. The study revealed people are more likely to exercise longer (but not necessarily harder) when working with a friend. Working with someone seems to prevent boredom. Get motivated with Emma Snowsill and Craig Walton The Gold Coast’s golden couple has an impressive collection of Olympic medals but that doesn’t mean motivation comes naturally. Emma says her greatest motivation is shopping but she knows it’s important to keep in shape and lead a healthy lifestyle. Craig says he loves to exercise. “It keeps me productive as well as fit,” he says. Emma and Craig start the day with a run on the beach and a swim. Emma says regular exercise is all about habit. “It doesn’t matter what you do as long as you do something every day,” she says. “Around 30 minutes of physical activity every day is all it takes. Start with something that is relatively achievable, push it a bit harder each day. “That might be just adding an extra block to the morning walk or spending a few more minutes on a treadmill. Results aren’t instant but they are consistent with the effort put in. Even after the past 10 years of professional triathlon training, getting out of bed at 5am every day never seems to be easy but it is all about habit and consistency. The more you force yourself to do something, the easier it will get,” Emma says. Craig says working as a couple makes exercise easier. “We are both keen on health and fitness but not all couples share the same goals. If your partner won’t work out find a friend who will. It makes exercise fun and most people achieve better results with the help of a little friendly competition,” he says. If you know you have to meet someone at a certain place to exercise then you are more likely to do it rather than making excuses at to why you shouldn’t,” he says.

At the recent American Society of Aesthetic Plastic Surgery meeting in Washington DC, much attention was focused on advances in body contouring. This expanding field has seen a sharp focus on patient safety and improved contour after these significant procedures. After a significant stretch the skin’s capacity to rebound to its original size and shape may be seriously impaired. This can leave unwanted skin folds along the upper arms, the sides of the chest and the torso. The legs may also be markedly expanded and the breasts may have had a huge stretch and then a dramatic loss in volume. Innovations in the techniques are using some of these skin folds to be “recycled” in such a way that the tissues that would normally be excised are tunnelled to help recreate the curvature of the buttock and an appropriate shape to the breast.
Esteem Day Spa plastic surgeon Dr Terrence Scamp recently attended an American Society of Aesthetic Plastic Surgery seminar in Washington. Dr Scamp says it featured advances in body contouring
Further adaptations in techniques have been designed to reduce the incidence of delayed healing in lifts to the arms, trunk and thighs, as these long wounds are commonly troublesome and slow to heal. Of course the most common body contouring procedure, Liposuction, can have a role to play in restoring shape. However if the skin elasticity is poor, liposuction in isolation may give a deflated look and occasion a great deal of skin irregularity due to the loss of the capacity to shrink and smooth the contour down. Thus Liposuction may be required to be combined with one of the more extensive techniques to create good contour. Another area where major advances have been made is in abdominoplasty or “tummy tuck”. The abdomen is the area most commonly ravaged by weight fluctuations and pregnancy. The stretch to the skin is obvious, but there are usually significant pressures put on the abdominal muscle wall from the expanding uterus or even weight gain. Thus an abdominoplasty combines removing this excess skin and the attached fat with tightening of the muscle sheath to return it to its youthful tone. This has a dramatic effect to narrow the waistline and flatten the abdomen. The scar is designed to be situated as low as possible to make it easier to conceal beneath most clothing. The narrowing of the waistline can be further accentuated by limited liposuction to the flank region which helps to taper and lengthen the waistline. Adaptations in the abdominoplasty technique pioneered by the French have enhanced the improvement in contour achieved with this procedure and at the same time reduce the complication rate and shortened the recovery. The procedure is most commonly performed under a general anaesthetic and hospitalization for two or three nights is normal. Fine catheters may be left in the abdomen to trickle local anaesthetic around the abdominal wall repair for two days after the procedure which greatly enhances the comfort during recovery. Much like a caesarean, the muscle wall repair is the feature that tends to inhibit mobility after surgery. Most people will have one to two weeks away from work but it takes four to six weeks to get back to full strength there.

However their effects on body contour can be quite dramatic and they really deal effectively with problems that are not amenable to more conservative procedures. Remember that your surgeon is basically adjusting your skin and muscle wall to fit you, so the slimmer you are, the more dramatic the improvement in shape there.

Carbohydrates Carbohydrates are a great source of energy. Most foods contain carbohydrates, fibre, vitamins and minerals. Very low carbohydrate diets are not recommended for people with diabetes. Regular amounts of carbohydrate foods help maintain energy levels without causing dramatic rises in blood glucose levels. People taking insulin or diabetes tablets may need between meal snacks. Researchers are discovering better ways to ease pain and suffering, enhance quality of life and allow people to return home sooner. Wesley Research Institute director Professor Julie Campbell is a renowned scientist who heads a team of dedicated researchers in an outstanding facility. Wesley now includes a health and medical research centre, a new clinical trials centre and a tissue bank. Professor Campbell says they aim to achieve success through faster and more accurate diagnosis.
Better nutrition doubled the life expectancy of pancreatic cancer patients and improved the quality of their lives “We look for treatments with the least side effects and new treatments that lead to quicker recovery,” she says. “The close link with one of Australia’s largest private hospitals is crucially important. Hospital wards are essential to an institute focussed on clinical research. “Wesley Research Institute funds research projects for doctors, scientists and allied health professionals. These projects include research into cancer, rehabilitation, diabetes, cardiovascular and respiratory disease, neurology, seniors’ health, obesity, tissue banking, wound and bone healing,” she says. Professor Campbell says the health and medical research centre enables a broader area of research. “The metabolic laboratory and kitchen researches nutrition and obesity, diabetes, and cancer,” she says.

“This is essential work as obesity is now in epidemic proportions in Australia and most of the world,” she says. “Many people benefit from Wesley Research Institute nutrition research. A study showed better nutrition doubled the life expectancy of pancreatic cancer patients and improved the quality of their lives. “Another project tackled the problem of malnutrition in hospitals and other healthcare facilities. The results are now being used around the world to identify patients at risk of malnutrition,” she says. Other major projects include a neurological study that aims to develop new treatments for stroke recovery and a three-year project on improved home care for Alzheimer’s patients. Professor Campbell says the clinical trials centre has an excellent reputation with international pharmaceutical and other companies for high quality medication and treatment trials. “The tissue bank has tissues and blood samples stored under liquid nitrogen for researchers studying the causes and progress of disease. Wesley is now researching ways of minimising and repairing damage caused by radiation treatment. The institute has established a rural and remote health centre to help people in the Australian outback. The Mitsubishi Development sponsored centre will research the use of communication technology to provide psychological and physical health support for people in remote areas,” Professor Campbell says.

Babies grow at about seven centimetres every three months. Toddlers grow at a much slower rate (about 7 to 10 centimetres a year) but good nutrition is still a priority. Breast milk and formula provided adequate nutrition for infants but toddlers need a variety of foods. This is the time to leave bottles behind and teach kids to eat and drink independently. Toddler time (between 12 and 24 months) is crucial to long term healthy eating habits. Children learn to accept new tastes and textures. A wide variety of nutrients should ensure satisfaction and normal development. Depending on age, size, and activity levels, toddlers need between 3200 to 4400 kilojoules a day. Milk is an important part of a toddler’s diet. Milk provides calcium and vitamin D to help build strong bones and teeth. Toddlers should have 500 milligrams of calcium and five micrograms of vitamin D a day. Australian kids usually get enough Vitamin D from a varied diet and outdoor play in the sun.

After age two most kids can switch to low-fat or non-fat milk. Discuss this with a doctor or health professional. Children may reject cow’s milk because it doesn’t taste like breast milk or formula. If the child is at least 12 months and rejecting cow’s milk try mixing the milk with formula or breast milk. Gradually adjust the mixture until it is 100 per cent cow’s milk. Some kids don’t like milk, some may have an intolerance to dairy products but this is relatively rare. Discuss the options with a health professional. Toddlers need nine milligrams of iron each day. Iron deficiency can affect growth and may lead to health, learning and behaviour problems. A child who has too much milk may reject iron rich food as their appetite is already satisfied. Limit milk to about 600 ml a day. Serve iron-rich meat, poultry, fish, grains, beans, and tofu. Include foods that contain vitamin C (tomatoes, broccoli, oranges, and strawberries); these improve the body’s iron absorption. Keep high fat, deep fried, spicy and sugary foods off the menu for as long as possible. Eventually peer groups pressure will help cultivate a longing for unhealthy foods but tastes formed in early childhood are likely to last a lifetime. People who prefer the natural flavours of fresh wholesome foods have the healthiest outlook. Talk to a doctor about dietary concerns and don’t give children a vitamin, mineral or food supplement without first discussing the products with a doctor.
