for Parenthood? STATEPOINT—The cost of raising a child is on the rise, according to the “2013 U.S. Cost of Raising a Child” report, conducted by BabyCenter. com. The report finds that parents spend an average of $13,000 annually on their kids. And while about two-thirds of moms feel that parents are spending too much, the vast majority of them are optimistic about their future -- the number of moms worried about having enough money to raise their children has declined approximately 10 percent in the past year. “Children are expensive, but couples aren’t letting that hold them back from starting a family,” says Carmen Wong Ulrich, a BabyCenter.com financial expert. “They’re exploring different ways of saving, such as moving back in with their parents or accepting money from inlaws. These can be good short-term solutions for couples getting started.” To gain insight into their finances and take steps toward financial independence, parents can use a free budgeting tool, such as BabyCenter.com’s Cost Calculator.

GETTING A BIRTH CERTIFICATE Be at your best before, during and after pregnancy.

our big day has come and gone, the baby has arrived, and you have named your little one. Now it’s time to get a birth certificate. There’s no deadline and no rush, but it’s simple to obtain a birth certificate from the Florida Department of Health (DOH), and having one will establish your baby’s identity for future needs such as enrolling in school and receiving health care. “It took 15 minutes. It was very easy,” said Nefertia Jones, a Dania Beach mother who came to the DOH Broward office in Fort Lauderdale to get a birth certificate for her 10-month-old daughter. The hospital takes care of notifying the DOH of each birth. Parents can check a box on the hospital birth form to automatically get a Social Security number for the baby. Next, parents can get the official birth certificate for as little as $15 ($20 in MiamiDade County). A plastic cover costs $3. You can get the document in person, by mail or online (for an extra charge). “Coming in person is the fastest way,” said Lilly Salazar, chief of vital statistics at the DOH Broward. “We try to give people full service in 20 minutes.”

ou don’t need a magic formula to enjoy a smooth pregnancy that brings you a beautiful, healthy
baby. You only need some advanced planning, self-discipline and stick-to-it-ness to help you dodge the pitfalls that can spoil your good health – and your baby’s. A healthy pregnancy starts well before conception, says Deborah Hill, executive director of community nursing at the
Florida Department of Health in Broward County. “If you plan that you want to get pregnant next year, you’ll start doing good things for healthy living that will help your baby when it is time,” Hill says. “You’ll quit smoking, you’ll eat good foods, you’ll exercise. “Often, women don’t plan their pregnancy ahead of time,” Hill says. “She may go out on the weekend with her friends
and have a few drinks, smoke a few cigarettes, eat some unhealthy food. If she is pregnant and doesn’t know it, she is off to a bad start.” The early weeks of pregnancy are crucial to your baby. Experts say most birth defects occur in the first three months. The federal Centers for Disease Control and Prevention has developed a game plan to help women deliver the healthiest possible babies.

Surviving the Pregnancy

W
hen I was about three months pregnant with my son, I stared at my mirrored reflection in my bedroom in complete dismay. “Where’s that glow that everyone you know who was pregnant talks about?” I wondered to myself. “You look like you just lost a fight against Mike Tyson!” Whether you are pregnant with your first child, or this is your second (or third or more), skin problems are nothing unusual for moms-to-be. The expected glow on your face shows you how your hormones are working in overdrive, making an increase in oil secretions. Some women suffer from oily skin throughout the entire course of their pregnancy; other women experience dry, itchy skin; still others experience a combination of both. Moms-to-be with pre-existing skin conditions may have to alter their treatments. All these factors can make a pregnant woman feel extremely frustrated. But don’t despair. There are an array of treatments and skin care products available to help ease your new facial changes and leave your skin feeling and looking fresh and relaxed.

Some moms-to-be may notice a dark line that runs down the middle of their belly. This line, referred to as the linea negra (black line), is quite common and usually disappears after the birth of your baby. But Steven D. Shapiro, MD, board certified dermatologist and director of Gardens Dermatology and Cosmetic Surgery Center in Palm Beach Gardens, warns that the time it takes to disappear may “differ from woman to woman. During pregnancy, sunscreen can slow the development of the line.” The area around your nipples may become darker in pigmentation, but this too, will disappear after your baby arrives. Some pregnant women may also develop brownish, uneven marks around their nose and cheeks. This condition is called chloasma and is commonly referred to as the “mask of pregnancy.” Prolonged exposure to the sun may increase the mark, and dermatologists recommend that moms-to-be should always use sunblock and wear a hat. Chloasma will also dissipate after delivery. The American Academy of Dermatology (AAD) states that Hyperpigmentation affects more than 90 percent of pregnant women in the United States. This skin disorder, which causes dark spots to
appear on the surface of the skin, is the result of an overproduction of a natural substance called melanin, which gives color to your skin, hair and eyes. Hyperpigmentation can occur anywhere on the breasts, nipples and genitals. Once again, this skin condition will disappear after the birth of your baby. Stretch marks (striae) are reddish streaks that may appear on your stomach and/or breasts as your skin stretches as your baby grows. Although there is no cure to make stretch marks disappear completely, Dr. Shapiro recommends that expectant women heavily moisturize their bellies throughout the course of their pregnancy. Moms-to-be who develop dry skin should consider switching to a non-soap facial wash. During your first trimester, there is a massive shift in your hormones, and androgen levels (those pesky hormones that cause breakouts during your premenstrual period) peak to extremely high levels, causing an increase in oil production on your face. “Gentle cleansing with a neutral cleanser may help to reduce oil as well, “ Dr. Shapiro says. If you suffer from pre-pregnancy skin condition such as acne, continued treatment during pregnancy must be evaluated on a case-by-case basis, as several types of prescription medication and topical creams, such as retinoid and isotretinoin (marketed as Accutane), can have severe effects on a developing baby. Once you have learned that you are pregnant, contact your dermatologist/ primary care physician as soon as possible, as you will need to steer clear of both these types of prescription as well as over-the-counter medications that contain chemical exfoliants. Look for gentle, nonchemical, organic exfoliants that will help to clear and remove dead cells that clog your pores. Before you throw away those mirrors in frustration, take a moment to stop and reflect on this special time in your life. After all, your wild pregnancy hormones will not be raging out-of-control forever.

Belly Bands are available in several different colors, and you can find them at most maternity stores and online. Not only is your belly expanding, but your breasts will change size and shape. Every mom-to-be’s body is different; however, if you find your cup size remaining the same and you are not ready to shell out $30 for a maternity bra, a bra back extender may become your new best friend.

Recent research has begun to uncover a myriad of chronic health problems which may be associated with impaired sleep in children and adults, including school and behavioral problems, obesity, diabetes, heart disease and even Alzheimer’s disease. Getting your newborn off to a good start by establishing proper sleep habits may help prevent some of these sleep problems later on. In addition, the better your child sleeps, the better you sleep, which will help you have the energy and patience to better care for your child.
0-3 months
For the first several months, infants typically sleep an average 14 hours a night, although up to 16 hours or more is not unusual. However, they do not sleep more than several hours at a time. Unfortunately, there is nothing you can do about this. It is normal and necessary as their metabolic demands and small stomachs require that they feed frequently. Many infants initially seem to be more awake during the night and sleepier during the day, which can be even more frustrating. Your main role during this stage is to keep them safe. Always put them to sleep on their back, as this has been shown to significantly reduce the incidence of sudden infant death syndrome (SIDS). Most experts discourage “co-sleeping,”

i.e. having your child sleep in the bed with you, in part because of the potential risks of rolling over on top of them. Make sure that your crib meets the latest safety standards, such as no drop side rails, and has not been recalled. Read all instructions and never tamper with or alter the design of the crib. Do not keep large and/or soft pillows or toys, loose bedding or bumpers, cords, strings or ropes in their crib, which can cause suffocation or strangulation. Do not let your infant sleep with the bottle, which can lead to cavities and create poor sleep habits. Sleeping with a pacifier is probably OK but NEVER tape or in any way attempt to secure the pacifier in a child’s mouth. Colic (incessant crying in a otherwise healthy infant for more than 3 hours a day, 3 or more days a week for at least 3 weeks) may start about 2 weeks of age and usually resolves by 3-4 months. Swaddling can help, but be careful not to “overdress” your infant which may lead to overheating and never cover up their face or mouth. Stop swaddling once they are able to roll over, and do not wrap them so tightly that they cannot move their legs as this may predispose them to hip dysplasia. If you like to take your infant for a drive to help them sleep, never leave them alone in the car or leave your car running in the garage. If your child seems too sleepy to wake and feed at least every 4-5 hours in the first 6 weeks of life, notify you physician.
3-6 months
Your main role during this stage is to start to establish good sleep habits and routines, which will facilitate their natural sleep abilities and hopefully prevent the sleep problems that often arise later. By 3-6 months, children are still sleeping an average of 13 hours per night. However, they begin to develop a circadian rhythm which allows them to “consolidate” their sleep, meaning they can sleep for longer periods of time and start to sleep through the night. The age at which this happens can be highly variable, but fortunately there may be some things you can do to help this along.
Put your infant to bed drowsy
This can be helped by learning to recognize the signs that they are getting sleepy, such as rubbing their eyes, droopy eyes, yawning, etc. Putting them to bed when they are still wide awake is an obvious recipe for failure, but so is waiting until they are overtired and more irritable.
Limit bedtime interaction
It has been shown that infants of parents who spend more time interacting with their infants at bedtime and throughout the night are more likely to have trouble sleeping through the night. Specifically, whether or not parents are present at the time the child falls asleep is the most important factor correlating with whether they are able to sleep at least 6 consecu-

tive hours during the night. Thus, it appears that many things parents do to soothe their child, such as rocking, feeding or allowing them to sleep in bed with them, might actually lead to poorer sleep. One reason for this may be that waking at night is a normal feature of sleep. Whether a child is able to return quickly back to sleep, on the other hand, often depends on what routines the infant has become accustomed to. If they have learned to fall asleep only when they are rocked or fed, for instance, they will have a difficult time falling back asleep on their own in the middle of the night. Of course, it is difficult to say whether parents who interact with their children more do so because those children have more difficulty soothing to begin with. The answer probably lies somewhere in between. A common sense approach for children who have a difficult time settling down at bedtime is to keep interactions to a mini-

mum and try to have them in their own crib/bed alone before they fall asleep. A similar strategy should be used for middle-of-the-night wakings. Once a child is old enough to start sleeping through the night (3-4 months for full-term infants), try to gradually wean them off night-time feeds. Whether you should allow your child to “cry themselves to sleep” remains controversial and is a topic you should discuss with your pediatrician or sleep specialist.
Signs that there may be a problem Fortunately, most sleep issues in newborns and infants are relatively minor, however, there are times when medical advice should be sought. These include:
» Infants less than 6 weeks of age who do not wake to feed every 4-5 hours
» Prominent snoring » Choking, prolonged pauses or other abnormalities of breathing during sleep
Establish a good bedtime routine
It is never too early to establish a consistent bedtime routine. This starts with “winding down” with calm, non-stimulating activities and dimming lights during the hour or so before bedtime. Bathing and other hygiene should be consistent and in the same order. Save favorite activities such as music, bedtime stories, etc. for last. Again, make sure the routine ends with the child in bed, alone before they fall asleep.

Frequent awakenings with incessant crying or abnormal behaviors
» Prominent or rhythmic movements in sleep If your child experiences any of the issues, or if their sleep issues do not respond to these measures, discuss it with your pediatrician or sleep specialist. The good news is that most infants and children are natural sleepers, and most problems can be prevented or solved with good habits and sleep hygiene.
