Student Sport Heath Declaration This document is in place to help safeguard and provide a safe environment for all members to participate in their chosen sport. At all times, every measure is put in place by the University, including all staff and coaches, to ensure a safe environment for the activity. Please read it carefully and take any actions if necessary. As part of the membership process you agree you have read and understood the information contained within this document.

Questionnaire Please read the questions below. If YES is the answer to any of the question, you must notify the coach leading your session before taking part. They may request more information. If you are unsure who to notify before the session, please contact a member of the Sport Development Team.            

Have you ever had any sign fact medical conditions? Have you ever had any significant surgical operations? Have you ever had any sign fact injury? Do you, or have you ever, suffered from back pain? Do you, or have you ever, experience chest pains? Is there a history of heart disease in your family? Have you had your cholesterol levels tested in the last 12 months? Do you regularly take any drugs or medication? Do you have any allergies? Do you suffer from headaches, dizzy or fainting spells or fits? Do you get out of breath at rest, at night or after slight exercise? Do you smoke? Do you know of any reason why you should take part in physical activity?

Your responsibilities As detailed above, if you answer YES to any of the questions, it is your responsibly to notify the coach leading your sessions. As a sports club member, you are responsible for participating at a level suitable to your fitness level. If you have any concerns you should discuss with the coach leading your session.

Declaration Any student who joins a sports club agrees they have read and understood their responsibilities as outlined in this document.
