CANNABIS (MARIJUANA) is the most commonly used illegal substance worldwide. About four percent of the world’s population between the ages of 15–64 years is estimated to have used cannabis at least once a year. The predictors of continued cannabis use include early onset and frequent use, male sex, using other illegal substances, and using to enhance positive feelings and reduce negative feelings. Long-term medical effects of cannabis use include the following. Cannabis smoke contains about three times the amount of tar found in tobacco smoke and 50 percent more carcinogens, irritating the airway causing obstructive lung disease (emphysema) and probably increased risk of lung and bladder cancer. It can cause memory deficits as well as decrease executive functionality and psychomotor speed. Also, it causes decreased testosterone secretion in men, which leads to lower libido, decreased sperm count and motility causing infertility; causes impotence, gynecomastia. There’s also substantial evidence linking cannabis use to psychosis with delusional thoughts. In addition to increased periodontal disease, cannabis causes adverse psychosocial issues such as school dropout, interpersonal problems with family and friends, and loss of jobs.

Varicose Veins Do Occur In Men! MOST PEOPLE are surprised to hear that varicose veins and vein problems in general can occur in men. They are surprised also when you tell them that it’s just as important to treat in men as it is in women. Much of these biases arise from the fact that many people think of vein disease as just a cosmetic problem and can be ignored. As you have heard me say many times in the past…nothing could be further from the truth! Let’s start out with the smallest of vein problems, spider veins. We always recommend that we check for the underlying cause of all vein problems, venous insufficiency. This can be done very simply and easily with an ultrasound examination done right in the office. If we do not see venous insufficiency then we treat the spider veins only. We have cosmetic procedures that are very good at doing this and they make people very happy. Have we done these procedures in men? Absolutely! Why shouldn’t men want to have their legs looking as good as they possibly can? The next category is varicose veins. Now the presence of varicose veins is a whole different ball game. If a person has varicose veins they will absolutely have some amount of that underlying problem we call venous insufficiency. This means that the one-way valves in the leg veins are not working properly and blood starts to move in the wrong direction,

downward. This is when the process becomes an important medical issue. Why? Well, varicose veins just by virtue of their presence can cause pain, achiness, skin irritation and fatigue. Also, sometimes clots can suddenly form in a varicose vein and the area then becomes intensely inflamed. You may have heard of this as something called “phlebitis” for short. This pain and irritation can take weeks to resolve and can also lead to the development of the dangerous type of blood clotting that in some cases leads to sudden death. Then there is just the ongoing process of venous insufficiency that with time can lead to skin discoloration and skin breakdown and chronic ulceration that can be very difficult to heal. Ok men, I don’t think we need to want legs that look like Jennifer Aniston but what’s wrong with wanting to look good? And…the health of your legs in general could be on the line!

health It Takes a Village WHEN HILLARY CLINTON, a zealot for government-run health care, entitled her book It Takes a Village, she unwittingly coined the new paradigm for the current practice of medicine. Now even the most minor of procedures, such as a skin biopsy, involves knowledge and adherence to a plethora of regulations from OSHA, HIPAA, CLIA, DHHS, and Medicare/ Medicaid/private insurances, requiring countless, ever-changing forms and protocols with specific staff members delegated to become the office experts on each entity. Removal of a suspicious mole that used to take two minutes with the assistance of one medical assistant now takes ten minutes with a collaboration of several individuals and specialized technology.

Jonathan Gruber and other social architects have made the rules and deserve responsibility. What are the effects of this ever-growing blob of activity? Staff numbers have quadrupled. Overhead expenses including labor, technology, training and equipment have as well. Fewer patients can be accommodated, so availability for appointments is less and waiting room times are longer. Frustration levels rise along with pressure on staff to meet the population’s needs as best they can. And who is the face of all of this? It is not the president or the senator or the insurance CEO or the high-level bureaucrat. It is the office receptionist, the medical assistant and the billing manager who so often catch the frustration that belongs to those we’ve elected and who have concocted this system. LAST YEAR

I traveled to New York and set out on a mission to find a neighborhood Chinese restaurant that supposedly had “the best” steamed crab and pork dumplings. When I arrived, I knew that the experience would be authentic. Fortunately, my Chinese friend had written the exact name of the dumplings, Xiao Long Bao, so I was only required to point to the item on the menu. When the dumplings arrived nestled in a bamboo steamer with unfamiliar utensils and sauces, I briefly panicked. After taking a moment to observe the more experienced diners and their approach to delicately and politely consuming the dumplings, I mustered the energy for my first attack. Briny, earthy flavors from the sea paired with salty pork and exotic spices filled my mouth and juices dripped down my chin and rolled down to my elbows. A food memory was made! I can tell the story of my life through food experiences and memories. My cousin, Tish, and I began experimenting in the kitchen at an early age with ovenfried chicken rolled in crushed potato chips. This was not our best effort and our skills have vastly improved over time. Years later, when we were in graduate school, my dear high school friends, Nancy Gaden Jackson, Debbie Duke Morvant and I formed a cooking and dinner club together, “Les Trois Petits Cochons.” We perfected traditional New Orleans Creole basics such as BBQ shrimp and Chicken a la Bonne Femme. Quite a step up from the fried chicken! I asked my foodie friends to share a few of their recent favorite food memories and bests: The best thing I cooked in 2014 has to be the strawberry pavlova for the Downton Abbey cooking with friends party. Peggy loves the truffle fries, fish tacos and pulled pork sandwiches from Lola Deux food truck. Carol Lapari loved the farm to table dinner at Bartlett Farms prepared by Ristorante Del Porto. Darnell had an orgasmic food experience at the newly reopened Brennan’s with the exquisite foie gras and squab soup. Carol Miles loves Carmo’s and Peggy’s recipe for Fish en Papilotte. THIS DISH was supposedly invented in New Orleans at Pascal’s Manale. This particular recipe was given to us by our dear friend and neighbor, Anne Morvant. Anne always serves the BBQ shrimp with boiled red new potatoes and of course, crusty French bread to soak up the buttery spicy seafood broth. Serves 8-10 people.

Ingredients: 8–10 pounds shrimp, heads on 1 pound butter 1 pound margarine 6 ounces Worcestershire sauce 8 tablespoons black pepper 4 lemons, sliced 8 garlic cloves, sliced Tabasco or Crystal hot sauce 2 teaspoons salt White wine Pinch of rosemary

legal Adoption and Inheritance Laws I AM OFTEN

asked by potential clients, “Why do you have to ask such detailed and personal questions about me during our consultation?” The answer is, “Because I really do need to know everything about you to be sure that I can legally accomplish your goals.” One of the most uncomfortable questions I have to ask is whether or not a client has ever had a child they put up for adoption. But this is one of the most important questions I can ever ask. Why? Under Louisiana law (La. C.C. Art. 199), the child you put up for adoption is still your legal heir for inheritance purposes. This means that if you die intestate (without a Will), the child you put up for adoption would inherit an equal share alongside your other children. Going even further, if you predecease your own parents, the child you put up for adoption becomes a legal heir to your own parents if they too die intestate. When the adoption is complete, it severs the tie from you being able to inherit from the child you put up for adoption, but not the other way around. This is why it

is so important in a Will, and in legal papers drafted for a Succession (Probate), to state that there are/were no other children whatsoever, born or adopted. It must be clear to the world that there are no other heirs that may arise and make a claim against a decedent’s estate. This is why we always recommend that our clients purchase owner’s title insurance when buying their homes because under Louisiana law, you really never know what “legal heirs” may still be out there. The above scenario is a perfect example. The best way to protect yourself in this situation is to have a well-drafted Will or Trust that states exactly to whom you wish to leave your estate. Do not let the State of Louisiana make that decision for you by dying intestate. I realize that this may be a very uncomfortable conversation to have with your loved ones now, but addressing this issue during your life is far cheaper and easier than having your family deal with a “stranger” after your death. IF YOU TRAVEL

north from the edge of the brackish water of Lake Pontchartrain, down the slow Bayou Bonfouca, the neoclassical, modern home of artist George Dunbar rises out from the marsh, on a literal hill. An entire wall of French doors overlooks an outdoor living area and the bayou below. A single heron perches on the periphery of the wetland grasses across the wide tributary while Mallard ducks gather in the infinity pool, seemingly enjoying the view. I am told there are many other animal visitors including nutria, alligators and even otters. This unencumbered view of the Louisiana bayou, ebbing, flowing and teaming with wildlife, is the pièce de résistance of Dunbar’s impressive home.

The landscape’s ever-changing nature serves as inspiration both for the home and Dunbar’s large portfolio of work. In fact, the life and career of George Dunbar is as “ever-changing” as the land surrounding his Slidell home. Born and raised in New Orleans, Dunbar’s accomplishments abound. His resumé includes: World War II veteran, where he served in the Navy as a salvage diver in the Philippines at the young age of 17; property developer: mostly on the Northshore, where he channeled his artistic visions into the land and is responsible for many of the canals, roads and neighborhoods in the area; and of course, visionary artist, where he helped usher in the contemporary art movement in the South during the late 50s and early 60s.

Dunbar’s works can also be described as ever-changing. In his formative years after the war, he attended Temple University’s Tyler School of Art in Philadelphia, traveled throughout Europe and studied at the Grand Chaumiere in Paris. His early work, inspired by such abstract expressionists as de Kooning and Kline, gave way to action paintings, sculpture and hard-edge paintings, to name a few. His works can be seen in such esteemed venues as the New Orleans Museum of Art and the British Museum in London. Dunbar’s periods, mostly named for regional bayous and terrain like Bonfouca, Marshgrass and Coin du Lestin, continue to develop and at 87 years old, he still works in his on-premise art studio on a daily basis.

His home, built on land that he developed many years earlier reflects his artistic vision of the textures and colors of his surroundings. The home was designed by architect Lee Ledbetter in the style of the classic, regional bayou homes with a simple modern twist. Dunbar describes his house as a “small home with great scale,” which is quite an accurate portrayal. With two bedrooms, a small kitchen and an open living and dining area, the home feels much larger than its small footprint due to soaring 22-foot ceilings and wall-to-wall French doors, letting in the afternoon light and idyllic views. Dunbar’s style is a hard-to-replicate mix of sleek, modern designs mixed with French antiques and laid-back, Louisiana charm. Mexican travertine floors flow throughout the entire space with brick and plaster walls, keeping the color of the space neutral while enhancing it with interesting textures. Two slipcovered sofas flank a low chrome and glass coffee table piled high with well-loved art and design books. The fireplace mantle, formerly a Mexican threshold that Dunbar brought home from San Miguel de Allende, serves as the mounting point for one of Dunbar’s 1998 paintings from his Coin du Lestin series. On the opposite wall, an enormous antique mirror anchors the space, juxtaposed with a glass, le Corbusier dining table and sleek but traditional dining chairs.

The bedroom boasts a less-expansive, yet equally impressive view of the bayou. A surveyor’s tripod sits in the corner, harking back to his days as a property developer, while the artwork over the bed comes from Dunbar’s collage series, this work, a 1984 monotone acrylic over rags. Dunbar explained that in hard-edge paintings, the color on the canvas must be put into the surface and brought back up, therefore becoming part of the landscape and not simply an afterthought. The same is true of his property; built literally from the ground up, his unique home is part of the bayou and the ever-changing landscape.

She said one of the biggest things she learned while moving from city to city in five states was the importance of home—the sense of being connected not only to where you’ve come from but also the experiences that bring you to where you are now. “Home can be a state of mind or a sense of belonging. It can also be a town or destination. And of course, it can mean the actual place where you live. The beauty of homeownership is that it allows a home to become all of those things at once, to become a true safe haven,” she said. “Finding a home you can afford, or finding a way to better afford the home that you already live in, can be stressful,” Raynah explained. “It is my job to educate the homeowner, which I believe helps alleviate some of that stress.”

HOMEGROWN & HOMEMADE There is no doubt that the area Raynah returned to five years ago is very different from the one where she grew up. Raynah credits her time spent in the piney woods of the Northshore as a child with her resourcefulness and “can do” attitude. She loves the outdoors and spends as much time as she can outside with her two Labrador retrievers, one-year-old Gunner and seven-year-old Reese. While hiking, walking and playing fetch are favorite activities with her four legged friends, Raynah also enjoys the peace and discipline of hot yoga. She is a certified group fitness instructor and one day hopes to offer complimentary yoga classes to clients and friends in the area. “I know how hard they work and would love to be able to help them achieve the physical and mental peace that yoga brings.” With her vision of home ownership extending to her own home, Raynah is an avid fan of Do It Yourself projects. She has stenciled and painted walls, changed light fixtures and is even working on custom cabinetry for her closets. She just recently installed an electronic dog door for her pups. “I think it runs in the family,” she said. “My dad is a carpenter, and his dad before him. I love making things with my own hands, building and then displaying them in my home.” Raynah admits that she usually has multiple projects going on at all times. But she matches her interest in home improvement with her desire for organization to create an environment she calls organized chaos. It seems like with a passion for her business, a bevy of friends and family, and a community in which she is actively involved, Raynah Cullen is exactly where she belongs. Set in Nazi Germany during World War II, The Book Thief follows the story of Liesel, a young German girl. What makes this narrative different is that it is told from Death’s perspective. You see, from the start of her journey in this war, Liesel caught Death’s eye, not because she was destined to die, but because she is a survivor. The first time Death sees Liesel is at the graveside of her younger brother. It is there she steals her first book: The Grave Digger’s Handbook. Through her foster father’s help, Liesel learns to read it, opening up a whole new world to her. She falls in love with books as they become her comfort in a quickly changing world. Through the eyes of this young girl, The Book Thief covers many profound themes in life and literature. You’ll laugh and you’ll cry as you fall in love with each character in this book. It is one of my personal favorites (though I do have many), and one you’ll read again and again discovering something different with each read.

ROBOTICS RUMBLE

Twenty-two teams from around the state competed in the VEX Robotics State Championships held at the University of Louisiana Lafayette. Five teams from the Northshore area qualified to compete in the high school division including: Archbishop Hannon, St. Paul’s School, Fontainebleau High School, Team Combustion (a joint team from Salmen and Northshore High School) and Pine High School.

Guests at Chef Soirée were treated to an enchanting evening under the stars at Bogue Falaya Park. The annual event, which benefits the Youth Service Bureau, is one of the most popular fundraisers on the Northshore. Sponsored by Chevron, the event featured the culinary creations of more than 85 area restaurants, along with cocktails, wine and spirits. From the opening ceremony, which saw the 610 Stompers leading a parade into the park to the fireworks show that capped off the evening, Chef Soirée is a feast for all of the senses. In addition to entertainment by Benny Grunch and the Bunch, Gypsy River, Louisiana Spice, Voodoo Funk & Soul, The Wagners and a guest appearance by fiddler Amanda Shaw, there was a raffle for a 2015 Ford Mustang courtesy of Banner Ford in Mandeville. This year’s event was dedicated to Judge Andy Clayton James Jr. (1932-2014), who was the first to assign a Court Appointed Special Advocates (CASA) volunteer in the 22nd Judicial District in 1989. Matt Cole, Marine Corps veteran, was also honored.

RECENTLY,

an altercation took place near my clinic. I was in session and learned about the incident within minutes. I can tell you the air was charged, faces tense, police sirens in the distance. Anger is a healthy and necessary emotion, which may prompt people getting rescued and healthy decisions made. Recall your first expressions of anger? Probably everything from pouting to hitting took place. We learned to pause, walk away, ask for what we want, go for a run. Unchecked anger is a different story. In session, I see people whom I sometimes urge to find their anger and learn how to manage the power of it. Untouched anger sometimes develops into deep and abiding depression, which can lie dormant, all of one’s life. Treating depression and not addressing the anger would be like not turning your phone on and wondering why your text messages aren’t getting through. How to cope with verbal conflict: 1. Excuse yourself for a moment; get a glass of water and, before returning, drink the whole glass. 2. Practice saying, “I can understand how you might see it like you do.” 3. If you wouldn’t mind, please help me see the steps you took getting to this conclusion (misinformed people come to wrong conclusions). 4. Ask to take the discussion outside; have a walkie-talkie. 5. Call a mutual friend who might help you objectively talk through your feelings. 6. Ask if the “discussion” may be resumed later-when you are both better rested. When people are directing anger at you or you are directing anger at others, remember that the present situation is influenced by your unresolved past incidents of anger. Your anger and harnessed expression of it can be powerful allies.

in March, but somehow April always seems to be the month we begin to crave that fresh start. The garden begins to really come back to life and the chores begin to loom over us! This is the perfect time to step outside and check out the first thing you see upon doing so, your entry door. Whether you use the front or side door, take some time to make that entry space truly say, “Welcome!” A can of paint, some new oversized pots and a few colorful plants can make all the difference to your entry. Think color. Maybe you haven’t repainted that entry door in years or maybe you did it yesterday, whatever the case may be, imagine surrounding the area with large pots of annuals or perennials. Remember you can always change your plantings as the seasons change. Shop around for the best buys in pots. The Northshore has an abundance of places to purchase these. to choose a color that coordinates with your door either color—a deeper or lighter shade of the same color or choose a complimentary color on the color wheel. The next thing to consider is the actual space around your entry. You don’t want to feel cramped as you enter or exit. If you have steps, you could also stagger the pots up and down the stairway. Maybe you have room for a chair or garden bench. You could also add new address numbers and of course, don’t forget the new welcome mat! If you have lots of sunlight, keep that in mind when choosing your plants. Lantana and sultana do great in containers and love the sun. Portulaca also thrives with direct sun and any of these would be a great accompaniment to a small citrus. Ask for help in choosing at your local nursery or check with the LSU Ag Center for container garden ideas. A little research

before you shop always makes the task more successful. Be prepared to water these containers more often than you would if these same plants were in the ground. Many evergreens will also do well in containers surrounded by spring and summer bloomers. Just try to get plants that appreciate the same amount of watering. If your choices fail, don’t be afraid to try something different. Things can always be moved to a permanent garden bed and there are always an abundance of plants to be had. Taking that first step will be the inspiration you need to make your whole garden say, “Welcome” to each and every person that comes your way.
