TODAY WE ARE influenced by consumerism more than ever before. We are becoming more desensitized to quality and longevity of materials and services as the next latest and greatest is six months away. Dental insurance has paved the way at reducing and robbing dentistry from truly helping patients receive much of the care they need. It is becoming much harder for patients to “hear” the truth about their options for dental care. The conflict occurs when oral health and consumerism collide. The disease processes in the body do not predictably respond to the “quick fix” today. How does one take control of his or her oral health rather than let it be dictated by outside factors? First, it starts by realizing that we will spend a part of our income investing in the quality of our oral health. Are we searching for the quick fix or are we planning for our future? There are times when the quick fix is an appropriate option. But, will 10 to 20 years of quick fixes provide the comfort and longevity we need as our lifespan continues to increase? Is there a way to spend money and control the disease process or does the disease process control the U.S. and our finances? By creating a “master plan” with your dentist, you can take control of your oral health. Risk factors can be identified and a plan set out to guide you in achieving your

goals. Much of the anti-aging we hear today truly does apply to our oral health. Worn teeth, receding gums, missing teeth, spaces between teeth and discoloration of teeth can all be controlled by properly evaluating and creating a plan to stop the deterioration. Has your dentist ever asked you what you wanted? Has a blueprint been presented to predictably get you there?

Are we searching for the quick fix or are we planning for our future? When the patient takes the control of his or her oral health and allows the dentist to enter into a long-term partnership with the dental insurance on the outside, the patient will then be free to experience the best and long-term care available to achieve their goals. There is no better time than right now to make this change. You have to take control before it controls you.

HEALTH IS OUR greatest gift, yet it is something so many of us take for granted until we have a health issue. I grew up in the National Health Service in England, where the waits for doctors are long and often unreasonable, where treatments and drug choices can depend on your postcode (zip code). When I moved to the United States, I was amazed by the freedom of choice and the quality of the health care offered. Here on the Northshore we have access to many physicians and health care facilities that offer not only choice but also a standard of care that is world-class. In this issue we showcase our Premier Physicians section and introduce you to many of the outstanding professionals and facilities that take care of our health care needs. We also meet our Enterprising Woman Laura Jacobs, owner of The Training Firm who is “5-feet of inspiration!” To read her story, please go to page six. Fashion Editor, Patty Beale has found stylish sunglasses to help us look our best when we are out enjoying the summer months and to help protect our eyes from harmful rays. To read more on eye health, this month’s Mentionable is Dr. Seán O’Sullivan who is an Ophthalmologist at the Retina Specialty Institute in Covington. One of our own is moving on. Our editor, Mara Beebe is exploring other endeavors; we wish her well and look forward to hearing about her new adventures. Have a safe July, enjoy the issue and Happy Birthday America!

WHEN COVINGTON NATIVE, Laura Jacobs eventually topped 179 pounds, she decided it was time to change her lifestyle. Always athletic and a horse lover who grew up competing in English riding and hunter jumping, her weight had never been a problem until college at Southeastern Louisiana University when she went from a size 5 to a size 14. Married with a young daughter, Laura realized that feeling good about herself and getting back into shape had to become a reality. She signed up with a personal trainer, began working out several times a week and changed the way she ate. “My husband, Jeff, encouraged me by repeatedly telling me, ‘If you don’t fear it, it’s not going to change you.’ Meaning that worrying about something is not going to help me move forward,” she says. A self-professed beach enthusiast, this 5-foot-tall dynamo made getting back to her athletic self a top priority. Five years later, Laura is now the owner of The Training Firm in Covington and is a certified trainer in kickboxing, injury prevention, nutrition, TRX Suspension Training®, kickboxing skills and choreography, and holds a certification from the Boxing Fitness Institute. She motivates her clients to take control of their bodies, both physically and mentally, and says that “We don’t sugarcoat the issue here; we teach clients how to push themselves to achieve the body they want. Our motto is, ‘Don’t wish for it, work for it.’” A family-centered business, her husband Jeff, owner of IEM Trucking, recently completed TRX Suspension Training® certification, and works as a personal trainer. Their 13-year-old daughter, Liv, a dance student for ten years, teaches children’s hip-hop classes, is CPR and baby sitter certified, and works in the “Kid Zone,” the on-site child care room for infants to age 11 years available during all classes.

“The Training Firm has totally transformed my entire life. It is not a typical gym, we are a family.” Having been in a place where she needed to be pushed, Laura says she is rewarded daily with text messages from clients who want her to know that they just put on a pair of jeans that have hung in their closet for years or that a workout was awesome and motivated them. “We tell our clients that the mind will quit a thousand times before your body will. It’s our job to get them to that next level,” says Laura. The Training Firm, 1958 Highway 190, Suite D, Covington, 985-871-5101, thetrainingfirmllc.com, facebook.com/TheTrainingFirm.

Sophisticated Woman | July 2013

With an emphasis on kickbox training with heavy weight MMA bags; there are three full-time trainers with multi-specialty certifications. There are group classes as well as separate women and men programs with trainers directing every class. “Many people are intimidated when they walk into a gym and have to figure out how to work a machine. Our programs are designed so that clients can jump right into their workout. ‘The Training Program’ is a month-long session that can be done in a group or at a personal level where a trainer varies the workouts to hit every aspect of the body. Our goal is to teach functional training of everyday movements that do not rely on machines,” she says. The TRX Suspension Training® workout is one example. This suspended harness system was created by a Navy Seal, uses gravity and resistance for a total-body workout, and is favored by trainers, professional athletes and the U.S. military. Modified for all fitness levels, a group class is now available on Mondays and Wednesdays at 6:30 p.m. Numerous cardio-circuits, treadmills, rowing machine, resistance bars, kettlebells and inventive dance classes are also available to members. Show off your dance moves in Zumba, Stiletto Strength, a BYOH (bring your own heels) workout in heels, Ballet Barre Boot Camp and Hip-Hop, with mood-boosting dance lights on a plyorobic rubber floor. Installed to ensure the long-term safety of clients, plyorobic flooring uses shock absorption to help prevent leg fatigue, shin splints and other injuries commonly experienced on concrete gym surfaces. Older kids can have fun also, in kickboxing, hip-hop and other cardio-circuits classes. For each child enrolled in the kickboxing program, a percentage is donated to Hope House, the St. Tammany and Washington Parishes’ Children Advocacy Center. Another special perk for parents needing a few hours out, is the drop-in Kids’ Night on Fridays from 6–9 p.m. With a 45-minute work out, pizza and a movie, children learn the elements of “fitness for life.” Fitness for life is centered on the entire well-being. Clients are guided through developed fitness programs, nutrition and meal counseling, biweekly body measurements to track progress and most importantly, “to stay off the scale.” “People can be tight in muscle and lose several inches but feel frustrated because they are not losing pounds. They learn that being healthy and looking leaner is achieved by eating wholesome food and having strong muscles,” says Laura. Brett Todd, a diabetic previously on insulin and other medications, is down by 40 pounds, is off all medications and now controls his condition by diet and exercise. Brett says, “Laura and the other trainers find ways to make the workouts interesting and fun. They are constantly bringing in new ideas and technology so that we are always changing and challenging ourselves.” Another client, Anjel Chatellier, has lost 30 pounds and 20 ½ inches since joining in December, and says, “The Training Firm has totally transformed my entire life. It is not a typical gym, we are a family.”

HAPPY INDEPENDENCE DAY! Even though our forefathers protested by throwing tea overboard to fight for freedom, it remains that a great cup of tea starts with high quality loose leaf, not dust (leftovers after processed). From another continent comes Rooibos (pronounced Roy-bus), a South African plant used to make tea infusions known as Red Bush. For centuries, native bushmen used the leaves from the Aspalathus Linearis plant for both medicinal and pure enjoyment. It has a rich, red color with a sweet nutty taste. Flavored Rooibos is available in Vanilla Almond, Belgian Chocolate, Nutcracker (a cherry-almond blend), African Autumn (cranberries and orange) and many other fruit and floral blends. Rooibos is reported to have amazing health benefits. Research shows up to 50 times more antioxidants than black tea, and studies indicate that it may aid in cancer prevention and increase the productivity of carcinogen-detoxifying enzymes, as well as protecting cell proteins and DNA. Studies may indicate that it also boosts the immune system to ward off illnesses and disease more effectively and the minerals in rooibos (manganese, calcium and fluoride) can promote bone strength and may reduce dental decay. Steeping time and temperature are dependent on the category of tea since the amount of processing of the tea leaves determines the flavor, appearance and health properties. Use gentle boiling water, steeping 2–3 minutes for black, 1/3 and as long and strong as Square you wish for Rooibos and herbals. 5.05” x 4.35”White and green teas are more delicate, so steep them for just 2–3 minutes (or they will turn bitter) with a water temperature just under boiling. I recommend Rooibos because the flavor and nutrition in my cup matters to me.

Protecting Healthy Vision in Aging Adults— Retina Specialty Institute

Sophisticated Woman | July 2013

WRITTEN BY JAMES HAGAN PHOTO BY JACK GARDNER

A GROWING NUMBER of older adults experience vision problems that turn simple daily tasks into difficult or impossible challenges. Many experiencing these challenges should know that significant vision loss, especially those caused by diabetes, can be prevented and—if they occur—treated. There are also easy preventative steps and accessible treatment options available for those with common vision-threatening diseases of the retina like age-related macular degeneration (ARMD) and diabetic retinopathy. The incidence of ARMD increases with age as it affects 30 percent of people over the age of 70, 40 percent of people over the age of 80, and 50 percent of people over the age of 90. Those most at risk of ARMD include smokers, individuals with high cholesterol, excessive sun exposure and those with a family history of the illness. However,

the last ten years have seen dramatic advances in treatment for the disease. Seán O’Sullivan, M.D., retinal expert at Retina Specialty Institute (RSI) explains, “As we learn more about age-related macular degeneration, new, less invasive, drug therapies are being developed and implemented in clinical trials. At RSI, we’re bringing these cutting edge treatments to the New Orleans community.” Even with these new advances in technology, attention to these risk factors, as well as regular retinal exams, can help prevent the occurrence of ARMD. Even more, as people age, their risk for developing diabetes also increases. According to the American Diabetes Association, about one in four people over the age of 60 has diabetes and nearly 45 percent of diabetics are at some stage of diabetic retinopathy as

A growing number of older adults experience vision problems that turn simple daily tasks into difficult or impossible challenges.

IF THERE’S ONE word that could easily describe Dr. Seán O’Sullivan, it’s “dynamic.” His friendly and magnetic personality, charisma, lively character, worldly experiences and abounding appreciation for stories—whether sharing or creating them—makes him as much of an endearing individual as it does a proficient, skilled and compassionate retinal surgeon. Dr. O’Sullivan’s own story began along the warm waters of the Mississippi Gulf Coast in Pascagoula. Here, he dreamed of colorful lands and distant cultures—and soon after graduating from Mississippi State as a dual major in Biomedical and Civil Engineering—made those dreams come true by joining the Peace Corps to help communities in Thailand preserve their resources and protect their wildlife. Upon his return home, Dr. O’Sullivan pursued his medical degree, internship and residency from The University of Mississippi Medical Center in Jackson, Mississippi. It’s here that he fell in love with ophthalmology and, specifically, the intricacies and biology of the retina. Following his residency, he later completed his fellowship in vitreoretinal surgery at Louisiana State University. Core to Dr. O’Sullivan’s beliefs are charity, compassion and research. Dr. O’Sullivan takes great pride in giving back to his community, and is actively involved in The Studies of Ocular Complications of AIDS (SOCA) and performs screenings to underserved populations including the homeless and those suffering from HIV/AIDS. Dr. O’Sullivan finds particular enjoyment at improving quality of life for those suffering from vision loss and believes whole-heartedly in the idea of treating the person, and not the disease. In his free time, Dr. O’Sullivan spends his time with his wife, Jennifer and their three sons, Finnbarr, Sebastian and Patrick.

Sophisticated Woman | July 2013

reported by the National Center for Chronic Disease Prevention and Health Promotion. Dr. O’Sullivan explains, “Blindness caused by diabetes is preventable with early detection and treatment. And like ARMD, there are new treatment options that are available to patients.” Because a dilated eye exam is the only way to detect changes in the retina, people with diabetes should schedule yearly comprehensive retinal exams for early detection and treatment. Regulating blood glucose levels, a diet rich in fruits and vegetables, as well as quitting smoking can help prevent diabetic retinopathy or its progression if you already have it. While preventative measures can ease the onset of vision loss, Dr. O’Sullivan explains that it is important for a person to seek medical help before their vision worsens. “When people cannot perform the daily activities that we take for granted, like writing a check, or even noticing faces or watching television, their quality of life suffers.” Many retinal researchers are working to develop less invasive technologies to treat vision-threatening diseases. Among them is Retina Specialty Institute, a nationallyrecognized clinical trial research and treatment center with offices in Covington and Metairie. If you are a diabetic, or feel you’re at risk or are showing symptoms of vision loss, schedule an appointment with your doctor or ophthalmologist for a comprehensive, dilated eye exam and start protecting your healthy vision. Retina Specialty Institute is located at 1421 Ochsner Blvd. in Covington, 985-590-5339 and 2800 Veterans Memorial Blvd., Suite 160, Metairie, 540-264-9438, retinaspecialty.com.

A PLACE OF OUR OWN

A Place of Our Own—A Pediatric Day Healthcare Center for Medically Fragile Children WRITTEN BY JILL WILLIE

MOST WORKING PARENTS of young children start their days in a very similar way ... get kids ready, get out the door, drop off at day care or school and go on to the office. Yet, parents of medically fragile children have a drastically different routine. In fact, most often it is necessary for one parent to stay home and care for their child. With the opening of A Place of Our Own, these parents can pursue career or educational opportunities, with the peace of mind that their children have the medical services they need. An integrated, multi-disciplinary service for medically complex, technology dependent children, A Place of Our Own features full-day medical care in an ageappropriate, loving environment. The program will serve medically fragile children, six-months to five years of age. These children are not eligible for enrollment in regular day care facilities or preschools because they require medical intervention throughout the day, such as tracheotomy care, tube feedings, IV, intramuscular and subcutaneous infusions. Pediatrician referrals are required for admission. Dr. Kimberly Williams, a Covington Nephrologist, and Tammy Oser, a Medical Administrator, have developed A Place of Our Own as a support program not just for medically fragile children, but their entire families as well. Dr. Williams explained that they wanted to merge the high-tech equipment and features of a medical facility into an atmosphere specifically designed for infants and preschoolers. “Our approach is to provide for the well-being of the entire child, providing opportunities for peer socialization, outdoor activities and group learning in addition to tending to medical needs,” she said. As the aunt of a medically fragile child herself, Mrs. Oser brings a particular insight into the family dynamic and stresses the importance of providing support services to both the children and their caregivers. “We plan to host events like Saints tailgating and viewing parties for our families throughout the year,” she said. “It is our hope that they can share experiences and become

Medical Director Dr. Kathryn Oubre, a pediatrician at Pontchartrain Pediatrics in Covington, will be responsible for management and implementation of on-site care. “Dr. Oubre is a key member of our professional team,” Dr. Williams said. “In addition to reviewing all physician orders, she will oversee the plan of care developed by our Director of Nursing.” Medical Directors in centers similar to A Place of Our Own have actually seen a decrease in ER visits and hospital readmissons. “Our children will receive medical supervision from the moment they arrive until the moment they leave, every day they are here,” said Dr. Oubre. “Because of our documentation standards, we’re able to identify changes in vital signs, physical demeanor or mood that may signify a medical issue. We are able to be proactive in our treatment approach.” A Place of Our Own provides a monthly report to each child’s referring pediatrician to provide fully integrated care. The physician can look at progress on a regular basis and determine if additional medical therapies need to be added to a child’s treatment regimen. Expected to open in early fall, A Place of Our Own is currently accepting children for the program. In order to maintain a high nurse-to-child ratio, enrollment is capped at 22 students. Dr. Williams and Mrs. Oser decided to concentrate on the infant/preschool segment before considering other age groups for their medical facility. Dr. Williams explained that school-aged children who are medically fragile can access services through the public school system. “We wanted to fill in the gap. To provide services where there was the greatest need.” Based on feedback they’ve received so far, Mrs. Oser said Northshore residents are embracing their efforts. “People are doing more than expressing interest,” she added. “They are helping make A Place of Our Own a reality.” “We cannot thank Don McMath of McMath Construction enough for the work he has done in transforming our building to meet our functional needs and our aesthetic desires. We are overwhelmed by his generosity,” Mrs. Oser said. Dr. Williams said she expects A Place of Our Own to set the standard-bearer for

pediatric day health care centers throughout the country. “We will have a Physician’s Advisory Committee that will help ensure that our high quality standards are being met,” she said. “We are here to care for and to support medically fragile children and their families in the most loving way possible.” A Place of Our Own, 39 Starbrush Circle, Covington, 985-902-8853.

Cellulite Be Gone

14

CELLULITE HAS BECOME symbolic for many women as the last vestige of stubborn fat deposits that cannot be exercised, starved, or treated away. For the cosmetic surgeon, it is the equivalent of a car’s hood dimpled with golf ball sized hail, but unlike the body shop guy, we can’t just order a new hood. However, if we understand the causes of cellulite and identify which cases are “fixable” with modern technology, substantial improvement is possible. Cellulite affects up to 90 percent of women, but rarely men. Its cause is a combination of genetic, hormonal, physical, and even psychological factors. What we can change among these causes is the actual physical architecture of the thighs and buttocks in three distinct components: namely, the “tufted” areas caused by fibrous bands which have lost their elasticity and dimple the skin; the extra fat that “overstuffs” a given area; and the general loss of elasticity and tightness which causes skin to sag. It is this combination of push and tug that prompts names such as “cottage cheese skin” and “Orange Peel Syndrome.” Improvement depends upon breaking the taut bands, precisely removing some of the stuffing, and strengthening the entire covering of skin. Every dimple, every bulge must be addressed. Massage, suction, injection of fatshrinking chemicals and enzymes, external sources of light and laser—all have been tried and failed to provide lasting improvement.

Having performed laser liposculpture on thousands of patients, I can attest to the reduction of cellulite due to the general removal of fat in a given area. But, not until the FDA’s approval of Cellulaze™ in 2012 have cosmetic surgeons had the necessary laser technology to pointedly address and markedly improve all three architectural components of cellulite. Cellulaze™ is an office procedure performed under local anesthesia which precisely addresses each “ding” separately, whether dimple or bulge, through a tiny needle-stick incision. After a short period of healing, the great majority of patients are satisfied, and the result is long lasting because the old structural skeleton of the skin is remodeled, not just temporarily shrunken or manipulated. As with laser liposculpture, ideal patients for Cellulaze™ are those who are close to their optimal weight but just can’t seem to lose those focal deposits which have been genetically bestowed. Larger deposits respond well to laser lipo, and now, with the already proven record of Cellulaze™, the last vestige of unsightly fat deposits can be successfully treated.

WHILE MANY HAVE never heard of endometriosis, chances are it has touched your life or that of someone you know. Endometriosis is a condition in which the cells that normally line the uterus grow in other places in the body. Up to 10 percent of all women have this condition. Symptoms can vary widely from infertility to painful menses. Treatment can be challenging, so finding the right doctor is critically important. The average age of diagnosis is 30. This is unfortunate as most women begin to experience symptoms up to a decade before knowing the cause. In women who are unable to get pregnant, 40 percent have endometriosis. Of those with chronic pelvic pain, up to 80 percent share the diagnosis. All too often, I see patients who have gone through multiple doctors without knowing what is wrong or what to do about their symptoms. Symptoms can range from painful periods to pain with intercourse, chronic pelvic pain and difficulty conceiving. The average patient with endometriosis will see three to four doctors before knowing the diagnosis. Treatment often requires several components. A good first line therapy for women not ready to have children is birth control. By suppressing the lesions, symptoms are minimized. This does not cure the illness, but can slow or stop progression. Lesions can also be identified and removed using the da Vinci® Surgical System. For more advanced cases, there are medications which stop estrogen production altogether. Women with severe cases may choose to have a hysterectomy. As difficult as it can be to hear that you or your loved one has endometriosis, the right doctor can tailor the treatment to your condition and your desired goals.

ST. TAMMANY PARISH HOSPITAL DEPARTMENT OF HOSPITAL MEDICINE

Advancing Medicine Through Technology— St. Tammany Parish Hospital Department of Hospital Medicine THE DEPARTMENT OF Hospital Medicine at St. Tammany Parish Hospital (STPH) is advancing the use of information technology to foster a healthier community in west St. Tammany and surrounding areas. The team of highly skilled and credentialed physicians known as hospitalists, use IT resources to ensure patients under their care in the hospital receive the highest quality of care and quality of service possible. Dr. Patrick Torcson, director of the hospitalist group, is also the hospital’s Chief Integration Officer. Dr. Joseph Landers is

vice chief of the medical staff and a leader in the clinical integration program. Dr. Craig Seicshnaydre, a founding member of the hospitalist program, is current Chair of Medicine at the hospital. Dr. Chris Darcey spearheaded last year’s effort to integrate computerized physician order entry at the hospital, using computer tablets at the bedside. Along with their partners Andrew Baier, M.D.; Sun Chaney, M.D.; Leslie Copeland, M.D.; Mae Dumlao, M.D.; Kyle Joyce, M.D.; Daniel Ilie, M.D.; Irfana Khan-Salam, M.D.; and Gray Napier M.D., the hospitalists care for patients referred to the hospital from primary care, emergency specialists and other physicians. By specializing in hospital medicine, they are able to devote all of their daily activities to caring for hospitalized patients. As STPH implements a strategy to reduce readmissions among patients at high risk for rehospitalization, hospitalists lead the way. This multifaceted transitions-of-care effort includes educating patients about the powerful role they can play in their own care after they leave the hospital. “Coaching patients on how to communicate their symptoms during follow-up visits

with their primary physician is extremely important,” says Dr. Landers. “Coaching helps patients understand their medications and how to recognize and respond to triggers.” Electronic health records provide the hospitalists and their care partners including primary physicians and the home health team with a complete picture of the patient’s condition, medications and treatments, while reducing opportunities for error. STPH is also expanding its patient coaching program through participation in a federally funded project to reduce readmissions. The Capital Area Agency on Aging will oversee the two-year project, which will measure the benefit of coaching of Medicare patients before and after discharge at four regional hospitals, including STPH. The hospitalists’ efforts are showing results. STPH’s readmissions for patients treated for pneumonia were already below the national average and continue to decrease from 17.12 percent in 2010 to 14.5 percent last year, hospital figures show. Readmission rates for patients treated for heart attack and heart failure have likewise declined over the past three years. “Putting information technology to work in our care system makes patients safer,” Dr. Torcson says, “If our orders are more precise, if we coordinate with other care providers and follow evidence-based best practice guidelines, then our patients heal faster and get back to their lives sooner. Our patients deserve the very best quality of care that we can deliver.” St. Tammany Parish Hospital, 1202 South Tyler Street, Covington, 985-898-4000, stph.org.

Technological Innovations Geared to the Individual— Northlake Neurological Institute WRITTEN BY MARA BEEBE

“WE KNOW THAT for many patients, a visit to the doctor can often be an uneasy experience. That’s why we strive to make the process as pleasant and productive as possible,” explains Dr. Michael Becker about the underlying philosophy of patient care at Northlake Neurological Institute. Founded in 1982 as a two-physician practice, NNI has grown to become the Northshore’s preeminent neurology center with 11 providers and offices in Lacombe, Hammond, Metairie and Lafayette. Specializing in a variety of neurologic disorders of the brain, spine and nervous system, the doctors at NNI have earned a reputation for tenacity in solving challenging diagnostic puzzles while always remaining sensitive to patient needs. Keeping up with the latest research is a daunting task. With more than 1,000 neurological disorders and tens of thousands of clinical trials in progress, NNI physicians are

constantly evaluating treatment options to ensure that their patients receive the best possible care. According to Dr. James Houser, “Seeing patients on a daily basis is just a small part of what we do at NNI. Parsing the ever increasing number of studies for effective new therapies is becoming more important than ever.” Two examples of the latest treatments in the neurological field are BOTOX® and Genomic DNA sequencing. BOTOX®, the new “wonder drug” of migraine headaches, involves a series of injections to the neck and head. Approved by the FDA in 2010, it is used for migraine sufferers who experience debilitating headaches 10–15 days in a month and who have not been helped through medication and dietary changes. Approximately 15 percent of the general populous (two to three times more women than men) suffer from migraine headaches. According to Dr. Michael Happel,

“While Botox treatment for chronic migraine isn’t appropriate for every patient, it has been very effective for many who don’t respond well to traditional drug therapies.” Genomic DNA sequencing is a technological innovation that enables NNI physicians to develop a customized treatment plan for each patient and avoid therapies that are unlikely to be effective. These tests utilize biomarkers of proteins and genetic patterns in blood, urine, skin tissue and saliva to reveal a patient’s predisposition to specific diseases or their ability to metabolize specific drugs. Referred to as “personalized medicine,” it could increase the survival rates for many illnesses, especially cancer, depression and coronary and peripheral artery disease. According to Dr. Michael Fischer, “DNA sequencing has the potential to be a real game-changer in the practice of medicine allowing physicians to take a targeted approach to patient care based on genetic makeup.” With advanced technology, excellent patient care from diagnosis through rehab and esteemed physicians, NNI offers extraordinary care right here on the Northshore. To contact Northlake Neurological Institute call 985-882-4500 or 504-831-6760 or visit northlakeneuro.net.

In October 2012, a text was sent: "I am ready." The reply was simple: "Well, come on." No other words needed to be written. From a casual colleague to serving as best man in his friend's wedding, Dr. Jeffrey Miller welcomed the opportunity to have his longtime friend, Dr. Paul Gordon, partner with him to expand his chiropractic clinic. Although Dr. Miller’s practice focuses on wellness patients and Dr. Gordon’s practice focuses on personal injury patients, their philosophy is the same: By treating the patients’ body as a whole, they are able to put the patients’ lives back together toward optimal health and toward pre-injury health status. If you are a patient of Dr. Miller, you are familiar with his statement, “Power’s On!”, after you have received an adjustment. Dr. Miller, CEO of Integrated Spine & Disc, is committed to teaching his patients how to live healthy, happy, productive and pain-free lifestyles, and does so by combining chiropractic care with corrective exercise and nutrition counseling. He believes that in order to maintain “power” to your body, you treat the body as a whole, not just specific symptoms.

“The human body has powerful restorative capabilities to heal itself when allowed to function the way it was designed,” says Dr. Miller. In order to achieve this level of wellness, Dr. Miller believes in teaching his patients the four fundamentals of empowered living: education, function, nutrition and fitness. To help his patients understand each of these concepts, Dr. Miller’s practice takes on a comprehensive approach. First, he educates the patient about their health through a series of consultations. Dr. Miller explains: “An educated patient can effectively participate in his or her care and help build a better rapport within the patient-doctor relationship." Once the patients become educated, then Dr. Miller teaches them how chiropractic care through spinal adjustments will help treat musculoskeletal complaints, including but not limited to back pain, neck pain, pain in the joints of the arms or legs, and headaches. A healthy spine and nervous system helps to balance the body's structure and assists the body in its ability to improve physically.

For Dr. Gordon, this means dedicating himself to each patient from beginning to end. After taking the time to properly understand and document his patients’ initial complaints, examining them and diagnosing their infirmaries, Dr. Gordon, like Dr. Miller, goes the extra mile in order to educate every patient on the complexities of their injuries, as well as to break down each step in the treatment process and how it will most benefit them. Visitors to Integrated Disc & Spine should not be surprised to see Dr. Gordon on his hands and knees teaching active care exercises to his patients.

Dr. Miller offers monthly nutritional workshops for his patients and members of the community who are interested in learning about a healthy diet in a world of fast foods and quick meals. “We do these workshops because I feel it is very important for patients to learn that healthy foods do not have to cost a fortune or take hours to prepare. There are options available for every person whether he is rich or poor, has plenty of time to cook or only a few minutes," Dr. Miller maintains. "Moreover, there are so many misconceptions about what is good for you and what is bad. It’s time to remedy the myths and get our population from being unhealthy to being healthy.” When most people hear "fitness," excuses quickly follow: I don’t have time! I can't find classes that fit my schedule! I’m too tired! It’s too hot! “Fitness,” Dr. Miller argues, “does not necessarily mean buying Lulu Lemon clothing and an expensive gym membership. It can simply mean taking a 30 minute walk after dinner, joining a local pilates or yoga program, or doing an at-home body workout. No one has ever regretted getting hooked on fitness.”

Dr. Gordon has made it a priority to walk his patients through each step in the healing process, which often means his day is not complete once the patient leaves. “That is just half of my day. The other half is putting pen to paper and relaying the injuries, severity of injuries, and treatment plans for each patient to their chosen attorneys,” says Dr. Gordon. “It is just as important to be able to relay the medical damages to the attorney as it is to teach the patients how to recover from their injuries.” Dr. Gordon’s practice espouses the same philosophies as Dr. Miller’s practice; however, they each carry out those philosophies in different ways. Dr. Gordon not only considers the treatment of his patients, but he must also report the patient’s full medical history to a third party. And Dr. Gordon takes his role as his patients’ advocate very seriously. “This takes a lot of time, effort, concentration, and accountability,” says Dr. Gordon. He then chuckles and says, “If you are going to remember Dr. Miller as saying ‘Power’s On!’ to his patients, then you should remember me by this tag line: ‘When your clients are injured, we are there with comprehensive healthcare.’”

While Dr. Miller is proficient in the wellness aspect of chiropractic, his business partner and longtime friend excels in treating patients injured in collisions, slips and falls or workers compensation matters. Dr. Paul Gordon relocated his practice from the Southshore to join Dr. Miller in practice. “I enjoyed working with my patients in New Orleans and do miss the constant excitement at times. However, my decision to move was based on my need to build a practice based on my philosophies. I wanted to spend time with patients to understand and treat their injuries,” says Dr. Gordon. He focuses on the development of comprehensive programs for his patients that will help them put their lives back together after a traumatic injury. “I can treat the patient functionally, socially, emotionally and vocationally rather than masking the immediate pain,” says Dr. Gordon.

Dr. Katie W. Carpenter and Dr. Jeffrey P. Franco are associate chiropractors of Integrated Spine & Disc.

Sophisticated Woman | July 2013

Now that he is the Chief Operating Officer of Integrated Spine & Disc, Dr. Gordon has been able to implement extensive changes in the way he handles personal injury claims. He is fortunate to have the staff to compartmentalize the administrative duties so he is better able to spend time being a doctor. “We are lucky to have a great, knowledgeable and friendly staff. Each person has a job and that person takes pride in what he or she does,” boasts Dr. Gordon. “This allows me the time to spend focusing on the patient.”

І CHILD SAFETY

Bouncer Danger

1/6 Horizontal

PLAYING ON MOONWALKS other inflat5.05” xand 2.12” able bouncers is great fun for kids. However, there is data that indicates there are risks too, and parents need to be aware of these risks. A study that was published in the journal Pediatrics last year showed that since 1995 the number of children being injured using inflatables has increased by 1500 percent. This is mostly due to increased use of inflatables. But, inflatables are also getting bigger with more variety in shape and activity, so these factors could also play a role. Most of the injuries on inflatables that require ER visits are broken bones and sprains. Approximately 3 percent of those children had to be1/2 hospitalized. Vertical So, what should our response be? 5.05” x 6.56” Well, I feel that playing on an inflatable is generally a great way to exercise. If you have ever jumped around in one, you know that it’s about the fastest you’ve ever worked up a sweat. It’s great for kids, too. It includes cardiovascular exercise, leg and core strengthening and balance training. If we can follow a few easy practices, our kids will be much safer: 1. Minimize their use by kids under six. 2. Limit the number of kids using an inflatable at one time. Ensure there is plenty of space for them to jump around without having to bang into each other. 3. Always have one parent paying close attention to notice any overly aggressive behavior. 4. Promptly remove any child who is playing in such a way that poses a risk to others. So, keep your kids active, let them play at the Bounce House. Just keep an eye on them.

Insomnia—The Nuts and Bolts INSOMNIA IS COMMONLY described as difficulty initiating or maintaining sleep, waking up too early and poor quality of sleep. Your daytime function may be compromised by fatigue, malaise, lack of energy and poor motivation. Insomnia is categorized as acute or chronic and has many potential causes. Acute insomnia applies to symptoms lasting less than three months and can relate to an identifiable stressor, such as a major life event, acute or chronic illness, medications or illicit drugs and changes in environment such as a nursing home or a hospital admission. Usually acute insomnia will improve when stressors go away. Chronic insomnia applies to symptoms lasting more than one month but usually lasting more than two years and potential causes include: (a) inadequate sleep due to irregular sleep, routine use of sleep-disturbing substances (alcohol, caffeine and nicotine) and stimulating activities before bed; (b) psychophysiological insomnia related to worry and racing thoughts that prevent sleep; (c) Idiopathic insomnia which begins during infancy or early childhood and many patients have a history of learning disabilities or ADHD during childhood; (d) Paradoxical insomnia (subjectively complaining of insomnia) but sleep study reveals normal EEG sleep stages; (e) medical conditions such as heart failure, diabetes, asthma and chronic pain syndrome; and (f) psychiatric disorders (depression, anxiety and Post Traumatic Stress Disorder) as well as Parkinson’s Disease and Restless leg syndrome. If insomnia affects your daytime or nighttime functions, keep a sleep diary with details of the onset and duration of sleep and sleep environment. See your physician for evaluation and suggestions. Have a good night.

AHH, SUMMER and family reunions. The memories made...the insecurities reignited, which support my work as a therapist. I understand. I have1/2 family, too. Vertical Did you know that I sleep with my mouth 5.05” x 6.56” open? I did not know. But, everyone at our family reunion confirmed that I do. After hearing that, I really tried to stay awake, especially because I saw a fly. Families have a way of reminding us that we are “failable” (I mean this just as I spelled it). Some of the family look like they missed the mark, and I really hope that they were adopted, but, apparently not. Seeing them go from pranksters to geezers—scary. Why I love the reunions is that they ground me to knowing hope face to face. I see that one person recovered from life-shattering news, and has regrouped and grown-up in remarkable ways. I hear the aged ones talk and their recollections remind us all to take time to laugh again and appreciate anew their love through the years and the time ahead. It amazes me to hear the ones of us left behind repeating, let’s face it, embellishing the stories told to us. I love the questions, “Did you eat? Did you eat enough?” If you haven’t been in a while and get the chance, go to a family reunion. Go as a guest. Then, maybe call a bunch of folks of your own, and start your own family reunion tradition. Family is the love that binds and abides. Bless your heart and mine.

Q: What is Neurofeedback?

A: It is a non-invasive learning process that balances and strengthens the connections in the brain to improve focus, attention, mood, anxiety, sleep, and behavior.

Q: How does it work?

A: It trains the brain to self-regulate by using a learning process that strengthens synaptic connections in the brain.

Q: Who can benefit?

A: It is an option for anyone whose symptoms associated with brain injury, strokes, ADD/ADHD, autism, anxiety, insomnia, and other disorders.

Q: Where do we begin?

A: A QEEG (Quantitative Electroencephalogram) or brain mapping is performed to determine how well each section of the brain is communicating. It pinpoints the areas that need adjustment and allows each client to have an individualized treatment plan.

Q: What is done during each session?

A: Seven sensors are placed on the scalp in the specified areas designated on the QEEG. The patient plays a game or watches a movie on the television controlled by the client’s brain activity, as the practitioner adjusts the thresholds or goals. When the brainwaves are balanced, the movie or game plays; likewise when the brainwaves are not balanced, the movie or game pauses or dims. The patient learns to control the brain so over time the brain retrains itself, and functions more efficiently.

Q: How long and how often are the sessions?

A: Sessions are usually 45 minutes long, twice a week.

JILL GIBSON, M.D.

The Mother’s Advocate— Dr. Jill Gibson, Obstetrician and Gynecologist EVERY WOMAN KNOWS that finding the right gynecologist is crucial for females from adolescence to menopause. Finding the “perfect” obstetrician to deliver your baby is a whole other level. Dr. Jill Gibson, an Obstetrician and Gynecologist, says that, “Delivering babies, of course, is my favorite part but I am also a huge advocate of listening to what the mother wants and incorporate my care to her beliefs and needs.” Dr. Gibson is excited to have her own practice and recently changed her location from Lakeview Regional Medical Center (LRMC) to the Doctors for Women facility in Covington. A graduate of the Department of Obstetrics and Gynecology at LSU Health Sciences Center in Shreveport, Dr. Gibson treats all stages of the female cycle: adolescent education, preconception counseling, routine prenatal care, birth control and hormone replacement therapy.

Dr. Gibson understands that women, especially new mothers, need to feel involved with treatment options and pregnancies. “I want my patients to have a voice in their care,” says Dr. Gibson. She believes it is possible to deliver vaginally after a previous cesarean and is happy to discuss each case individually. She also believes it is essential to teach young girls about their transition to womanhood. She recently led a mother/daughter presentation at LRMC and Our Lady of the Lake Catholic School. “I should call it ‘What to expect when you are expecting your cycle,’” she laughs, and was pleased that many of the girls opened up and asked questions. A mother herself, Dr. Gibson is married to Todd Gibson, an engineer, and they have a four-year-old son, Cade. She is a Louisiana girl, originally from Opelousas and now lives in Covington where Todd’s family is from. The Gibson family loves to travel, especially to the beach, but really enjoy the fishing and water on the Northshore. “There is nothing like south Louisiana. I am glad to be home,” she says. Food: Fats, Fads, Facts IN THE YEARS following WWII, there emerged a disturbing awareness that countries in the Mediterranean Basin—Spain, Italy, Greece and the Middle East—were experiencing far lower rates of chronic cardiovascular and lung disease, diabetes and obesity. At the same time, they were leading longer and healthier lives, even though medical services were limited. Following the initiative of Minnesota researcher Dr. Ancel Keys, studies began in the late 1940s, as to why this was so. A major breakthrough was his work with cholesterol; in the 60s and 70s, he even earned the nickname “Monsieur Cholesterol.” He recognized that plant-based fats, primarily olive oil; replacing animal-based fats, (primarily red meat) was a key component in the difference between the Mediterranean and the typical U.S. lifestyle. Why are 75 percent of Americans so obese? Why do Americans have such high rates of cardiovascular and lung disease, diabetes, cancer and other such killers? Enough research has been done now that we know the answer; we just do not like the answer. It is hard work giving up old familiar lifestyles and developing new. In 1993 the Harvard School of Public Health, Oldways and the World Health Organization introduced the Mediterranean Diet with its now classic pyramid. The base is whole grains, along with beans and nuts. Next come fruits and vegetables, cheese and yogurt from sheep and goats. Fish are recommended, as are fowl, several times a week. Delete butter altogether; use a fine olive oil for dipping bread or cooking. At the very top of the pyramid, recommended for occasional consumption, is red meat. A food plan like this is a guarantee of good health and will help in reducing obesity.

Time for Adventure WRITTEN BY ANNA RIBBECK

FIRECRACKERS WILL BE POPPING SOON to celebrate America’s glory. Why not plant “firecrackers” in your yard and celebrate the arrival of hummingbirds and butterflies? Two plants are referred to as firecracker plants. One (crossandra infundibuliformis) is called the firecracker plant because its seedpods tend to explode when presented with high humidity or rainfall. This helps the plant create new seedlings. It will come back year after year but only if covered in the winter or moved inside. It does well as a houseplant, but also is excellent outside in container gardens. Flower colors range from salmon or coral to red, yellow and even turquoise and have no scent but will bloom continuously for years. The bright colors are 1/3 Square what attract flying favorites to the garden. 5.05” x 4.35” They do like full sun or a bright window indoors. This plant is actually an evergreen shrub that grows up to 3-feet tall and propagates with cuttings as well as seeds. The second firecracker (cuphea ignea) is an annual that is native to Mexico. This plant only grows to a height of one foot but spreads over an equal area. They do great as border plants, in rock gardens and in hanging baskets or other planters. Seeds can be sown as soon as the soil is warm outdoors. Cuttings can be rooted in 10–14 days and then planted. Pinching out growth tips

will encourage bushiness and trimming the plant will give you the look and shape you want. This plant blooms in red—thus the name firecracker. If moved inside (no frost is tolerated) you may gain another season or two of enjoyment. New plants are tested by our Louisiana agricultural agencies continuously to find plants that will withstand our weather conditions. Most of these are available at local nurseries. Read the labels on plants to see if they fit your growing area. Do not be afraid to ask for advice or to call the LSU Ag Center. Here are a few other plants that will add pops of color to your garden and attract winged beauties: “Sparks Will Fly” begonia “Pineapple Pie” coreopsis “Pardon My Pink” bee balm “Butterfly Kisses” purple coneflower “Peek-a-Blue” Russian sage “Everlast” dianthus “Sweet Summer Love” clematis “Midnight Marvel” hibiscus Delight in a new plant as a new adventure along your pathway of life, liberty and the pursuit of happiness.

THE CITY OF MANDEVILLE’S mission is to “Provide citizens a safe, well-planned city with a strong long-term infrastructure foundation built from sustainable revenues and economic opportunity to benefit the health and well-being of its residents.” In the last three years, significant progress has been made and that mission is being accomplished. The city continues a strong financial position, approaching each year with cautious optimism for future revenues

and looking for ways to stretch tax dollars through state and federal grants. Public Works does a great job in making Mandeville a desirable place to live. At the present time, the city has just completed $1,025,000 in capital projects; $1,580,000 is under construction; $4,345,000 is under design; $315,000 is out to bid; and $4,150,000 is being evaluated. More road improvements are planned each year. “Road Soft,” a new software, was acquired for the streets

program, allowing for a 1 to 10 assessment of all roads and methods of repair to extend roadway life. A study was just completed on over 60 lift stations using hand-held data recorders to provide better tracking information. The City is in the process of completing significant drainage projects and will soon initiate a tidal study. Mandeville has always been known for its low crime rate and its police force continues to provide a safe community and peace of mind. Their rapid response and quick resolution of crimes sends a message to those looking to prey on local citizens. Cultural development is alive and well, making an impact on the lifestyle of our citizens. There is programming at the Trailhead and the lakefront, and interfacing with business, historic and citizen groups. The city is now enjoying a Historic District and all the benefits it brings.

Mandeville’s jewel and one of a kind on the Northshore is its pristine lakefront, which has never looked better. The gazebo and historic water fountain have been landscaped and renovated, swings and arbors have been installed, and the spectacular sunsets which are second to none, are enjoyed every day by residents and visitors from far and wide. All in all, Mandeville has steadily moved ahead and pride in the city has been restored. Mayor Donald Villere, the Administration and the City Council have worked hard to maintain the mission, but ultimately, the city is rich in resources, its greatest being Mandeville residents who are conscientious and focused on building a community with strong family values. The City has recently been named by Bloomberg Businessweek as one of America’s best places to raise kids for 2013. And, they are the key to our future.

ESTATE PLANNING

Do I Need a Power of Attorney?

WRITTEN BY RONDA M. GABB

YES! EVERYONE NEEDS a Power of Attorney (Louisiana Mandate). A Power of Attorney (POA) gives someone of your choosing the legal authority to act on your behalf (for finances and/or medical decisions) without Court intervention. The person you choose is called your agent (mandatary). It is important to know that a POA ends at the moment of your death (conversely, a Last Will does not take effect until the moment of your death). Many people have the misconception that they do not need a POA because they have a Last Will & Testament (Will). Remember, a POA and a Will are two completely different documents that take effect at totally different times—one before your death, and the other after your death. A comprehensive estate plan affords you the ability to protect your loved ones during your incapacity and after your death. Assume that Bill and Mary have been married for 50 years and Bill suffers a massive stroke and is now mentally and physically incompetent. Mary now needs to sell their vacation condo and/or pull funds from Bill’s IRA in order to have money to pay for Bill’s additional health care needs. Without a POA can Mary sign on Bill’s behalf to sell

the condo or speak to the IRA administrator? The answer, unfortunately, is no. Without a POA Mary would need to get specific court approval to be appointed as Bill’s curator (guardian) in order to be able to sell or access Bill’s assets. This legal proceeding is called an “interdiction.” The person petitioning the Court needs to actually sue Bill and declare him incompetent in court. An attorney needs to be appointed to represent Bill and Mary needs her own attorney, too. Once approved by the court, the curator needs to post a bond and the court will also appoint an “undercurator” to make sure that the curator is properly doing his/her job. As you can imagine, this process can be humiliating, expensive and extremely time-consuming. Additionally, everyone needs a Health Care Power of Attorney (unless these powers are specifically stated in your general POA) and a Living Will (Advance Directive).

Purchase your tickets today at estchamber.com.

Make Your Voice Heard

WRITTEN BY PAT BRISTER PARISH PRESIDENT

RECENTLY, I JOINED several other parish presidents, representatives from around the state and members of the Greater New Orleans, Inc. (GNO, Inc.) Regional Economic Alliance, as we met with our congressional delegation in Washington D.C. Our purpose was simple, to urge them to devise and support legislation like the Cassidy amendment that passed the U.S. House of Representatives in early June, which will address imbalances within the Biggert-Waters Flood Insurance Reform Act of 2012. This act—designed to bring the National Flood Insurance Program into solvency— has the potential to raise the rates of current policyholders in special flood hazard areas above and beyond the scope of

affordability. Despite the fact that these homeowners built their homes in compliance with previous flood elevation maps, many are now facing new elevation rates because of remapping by FEMA; their previous rates will not be grandfathered in under the new law. The ripple effect of this new law will eventually impact the overall economy of coastal regions such as ours. As citizens of St. Tammany, the state of Louisiana and the United States of America, you can exercise your right to be heard. You will find an appeal letter that you can copy, paste and email to your Congressman at stpgov.org/floodmaps. Together, our many voices will become one, as we urge Congress to address the insufficiencies in Biggert-Waters.

Emergency Planning

38

WRITTEN BY DONALD VILLERE, MAYOR OF MANDEVILLE

THE CITY OF MANDEVILLE is well prepared for hurricane season and we want to make sure our residents are, too. Please shop now for any items you need to have on hand for an emergency both for in-house and on the road. Make plans on what you will do and where you will go in case of an event. In last month’s water bill, every citizen received a magnet with emergency and non-emergency numbers. Please place that in a handy spot. Sign up for E-briefs for email updates by going to our website at cityofmandeville.com. Sign up for ALERT St. Tammany for phone updates by registering on their website at stpgov.org, and sign up for traffic updates texted to your phone at nixle.com. Make a

mental note of what has to be taken indoors or tied down when winds increase to prevent projectiles from creating further damage. If you need medical prescriptions to be refilled, now is the time to do so. By having all of this in place, staying tuned to the radio, television news and our website, you can relax and enjoy the summer, knowing if anything occurs, you have a plan in place. My office will remain open during any storm; I will not leave the city. Our police and public works departments will be on call 24/7. If you have any questions, please do not hesitate to contact my office at 985-626-1082. In the meantime, enjoy your summer and let’s pray for a safe and sunny season!

With a tailored 30 minute program to meet my needs, it takes the guesswork out and exercising no longer feels like a chore! I’ll be running my first half marathon in March and could not have gotten to this point without Koko!”

“Koko maximizes my workout in a short period of time, focusing on what I specifically need, so that it does not take away time from my family or work. This makes it possible for me to work out without feeling like I have to live at the gym. I started out one year ago in a size 40 waist, and today I am in a size 34. Koko has made it easy and convenient to be in shape.”
