WHEN WE ASKED our Enterprising Woman, Candice Knight, to check her schedule to reserve time for our January cover shoot, we could see that it was going to be a difficult task for her. Juggling her medical practice, Knight Integrative Medicine, family life with husband and two children and community service was going to be a daunting task. All fell into place. We slowed her down long enough to relax and enjoy the pampering she received at Paris Parker Salon and Spa in Mandeville before her Enterprising Woman photo shoot. Her hair was styled by Hair Designer, E. Lee and her makeup expertly applied by Makeup Artist, Dionna Deliberto. Healthy Living Keys to Achieving Your Wellness Goals
WE’VE ALL HEARD
the saying, “A new year, a new you.” While it may be meant to motivate people into taking control of their health and wellness, experts warn that trying to make too many changes may be counterproductive. When the clock strikes midnight, most people vow to improve their health over any other lifestyle changes. According to the University of Scranton’s Journal of Clinical Psychology, top resolutions include losing weight, staying fit and healthy and quitting smoking. Of those who make resolutions, only 8 percent claim to have achieved success in achieving their goals. One of the reasons for failure is that people try to do too much or make too many changes all at once. It turns out that moderation in all things—even in resolutions—may be the key. Jane Freudenberger, licensed clinical social worker at the Mary Bird Perkins Cancer Center at St. Tammany Parish Hospital said when people set unattainable goals, they not only set themselves up for short-term failure but they also minimize chances for future success. She recommends replacing negative feelings about past failures with positive, motivational
statements. “One of my favorites that keeps me from self-sabotage is from Ann Bradford: ‘Tell the negative committee that meets inside your head to sit down and shut up.’” For those who vow to lose a certain amount of weight in the coming year, Asahel Gridley, M.D., bariatric surgeon at Ochsner North Shore in Slidell said it is more important to focus on changing your lifestyle than changing the number on the scale. “It is easy to convince yourself to just eat less to lose weight, but chances are you will regain your weight lost when you start eating normal amounts. Picking a sustainable diet is key to losing weight and keeping it off.” Noting that most people have success when they choose activities they enjoy, Dr. Gridley recommends integrating a level of accountability into their exercise goals. Hope Clay, owner of Pure Barre in Mandeville has found that developing a set of mini challenges helps keep her clients accountable and motivated throughout the year. “We’ll do something like attend 30 classes in 40 days. The key is to make it difficult enough so they’ll
achieve a sense of satisfaction, while also making it attainable enough so that they don’t get discouraged,” she said. In addition to exercising the body, Freudenberger recommends exercising the mind as well—especially when it comes to creating healthy behaviors. “Just wishing for something will not make it happen; we have to engage our brains in the process of goal setting. Any new behavior, or change, requires time and practice. But ultimately, it comes down to a change in mindset. If the resolution is really not something you want to commit to then it will fail.” So when you begin making your resolutions this year, remember that just by changing your mindset, you are already ahead of the game. According to Dr. Gridley, “Every step that you make toward healthier choices for your mind, body and spirit is a success.”

WE CAN ALL RELATE to the frustration that comes from an occasional headache or back strain. While pain may range from a nagging sense of discomfort or the need to take a sick day, most of us are assured the inconvenience will be relatively short lived. For those with chronic pain, however, it may seem like there is no end in sight. The neurologists at the Northlake Neurological Institute (NNI) work with chronic pain sufferers to guide them down the road to recovery. “All pain has a neurological cause,” explained Dr. Rex Houser. “At NNI we have the qualifications, experience and technology to determine the source of the pain and recommend treatment options.” Staffed completely by neurologists, NNI can perform all testing on-site. Acne Alternatives THE TREATMENT
of acne, as with so many other medical disorders, has circled back on itself over the last few decades. Years ago, acne treatment depended upon “physical modalities,” such as scrubbing with harsh soaps, ultraviolet light, low dose X-ray sessions, and dietary changes. Prescription medicines, both topical and internal, revolutionized acne therapy with great success, culminating with Accutane in the 80’s, which is still by far the most effective medical agent for moderate to severe or stubborn acne. But the last few years have seen a rise in physical treatments, including photodynamic therapy (PDT), laser genesis, microdermabrasion, dermalinfusion, and dietary supplements, as alternatives in managing this unsightly, disfiguring condition.
Prescription medicines, both topical and internal, revolutionized acne therapy. PDT involves the application of a solution containing a light-sensitive chemical which first is absorbed into the pores where acne forms and then is activated by harmless visible light rays to quell the inflammation and infection therein. Microdermabrasion and dermalinfusion such as SilkPeelTM abrade the outer layers of skin to unroof clogged pores and stimulate removal of whiteheads and blackheads. Additionally, SilkPeelTM infuses acnespecific serum into the skin. Laser genesis treatments target the redness and pigmentation which often persist after flares of acne. And of late, dietary phytochemicals derived from certain vegetables have shown promise and efficacy by tweaking estrogen metabolism. health Advance Care Planning ADVANCE CARE PLANNING is an ongoing process that involves the patients, their family members and health care providers, making plans for the patients’ current and future medical care. It should be proactive, appropriately timed and integrated into routine care. Furthermore, it should be revisited every time a person’s medical condition changes. It reflects a patient’s goals, values and cultural beliefs, which drive his or her specific medical treatment decisions that can be recorded in an advance directive. There are various kinds of advance directives, but only two types are recognized by state law in the United States: (a) Living Will: Lays out patients’ preferences for life sustaining treatments and resuscitation. This may cover choice of hospitalization, artificial tube feeding, dialysis, etc. (b) Durable Power of Attorney for Health Care: Designates a choice of surrogate decision makers. Advance Directives (AD) are completed while a person can still make his or her own decisions with regard to his or her future medical care in the event he or she loses the capacity to make such decisions. ADs can be revoked orally by the patient at any time.

of important figures for 2015 that we use regularly in our practice (the Medicaid and VA figures are “projected” figures as the final figures do not publish until January or February). The amount you are free to give to each person for this year without either party having to file anything with the IRS: $14,000 (this means a married couple could give $28,000 to each person). The amount one spouse may gift for free to the other spouse who is not a U.S. Citizen: $147,000, and to a spouse who is a U.S. Citizen: an unlimited amount. The amount you are able to leave your loved ones at your death free of any estate taxes or state inheritance taxes: $5,430,000 (this means that a married couple may leave $10,860,000). For Medicaid purposes, the amount the “at home” community spouse may retain in countable resources if the other spouse is in a nursing home: $119,220 (the primary home is not considered as a “countable” resource, but after the death of both spouses Medicaid may seek estate recovery against the home). For Medicaid purposes, a single person who is in a nursing home may keep only up to $2,000 in countable resources, a married couple who both are in a nursing home may keep only up to $3,000 in countable resources.
Medicaid will not institute estate recovery if an heir’s adjusted gross income (AGI) is 300% or less of the Federal Poverty Guidelines, the following are annualized figures based on family size: (1) $35,010, (2) $47,190, (3) $59,370, (4) $71,550, (5) $83,730. For Medicaid purposes, the amount of monthly income the “at home” spouse may keep of the “institutionalized” spouse’s monthly income is up to $2,980.50 per month total (assuming the “at home” spouse had no other income). For the VA Aid & Attendance Improved Pension Program monthly figures: both Veteran and Spouse: $2,120, Veteran only: $1,789, Spouse of healthy Veteran: $1,371, Widow of Veteran: $1,149 and Veteran married to Veteran: $2,837. To have the assessed value of your Homestead “frozen” according to the “Senior Freeze” laws, an owner of the property must be 65 years of age or older and have an AGI of $71,563 or less.

at 2 a.m. in New Brunswick, Canada during a snowstorm and learned that I was one of 22,000 people without electrical power. I started to panic; my flight out was not only delayed, but cancelled until the next day. I learned this after a 20-minute hold on the phone during which I tried to save my cellphone battery. What to do? I couldn’t plan for the whole day because I didn’t know what was going to happen next. So, I made a plan for the next hour then the next. This comforted me. Bring a flashlight, make sure cellphone is charged up, have clothes ready to go. I did sleep. When daylight came, I went to another town, looked for lights and shared an impromptu Thanksgiving meal with family. I felt in better control. I couldn’t control the circumstances but I did control my responses. The bigger lack of control for me right now is that my Dad has lung cancer, which is why I was in New Brunswick. What powerlessness I feel about that! Part of empowering Dad is

His case is not curable and treatment is centered not on quantity of life but quality of life. Dad is having radiation treatments. The plans my siblings and I had before Dad’s diagnosis have changed. Our plans now are to be with Dad as often as possible and celebrate that hour and the next. As we start out 2015 together, let’s plan for quality in all that we do. God bless you my friends.

DR. CANDICE L. KNIGHT may have come back to her native New Orleans after graduating from Yale in 2001, yet the past 12 months are what really mark her return home. After six years of practicing in a more traditional hospital setting, Dr. Knight has reignited her passion for holistic wellness with Knight Integrative Medicine. “I wanted to do more than treat people’s symptoms. I wanted to bring them to true health,” Dr. Knight explained. The current medical model is more reactive than proactive. While it is great that we can provide relief from symptoms with medications, we really need to treat the root of the problem as well. Without that, it can be a slippery slope. People may take one medicine for one symptom, another medicine for another symptom, and yet another medicine to help with the side effects. All the while, they still haven’t addressed the underlying conditions that are perpetuating the symptoms in the first place.” Discouraged by the limitations of a traditional practice, Dr. Knight became more involved with integrative medicine—an approach that first intrigued her during medical school where both patient and practitioner work in partnership to optimize health and healing. Based on the principles of integrative medicine, Dr. Knight ultimately decided to focus on functional medicine, a subset of the integrative specialty. “Functional medicine is a therapeutic dynamic that allows me to really get to know my patients and assess the entire spectrum

Noting that her goal is to restore biochemistry, Dr. Knight said that functional medicine also allows her the flexibility to utilize a variety of treatment options such as prescription and botanical medicines, supplements, hormone replacement or modulation along with behavior modifications like exercise, diet and stress management techniques. “Holistic remedies are viable, not only in the treatment of disease, but they can also play a significant role in the prevention of disease as well,” she said. “These are natural alternatives that have not only proven the test of time for thousands of years but are also just as well researched as pharmaceuticals.” With a diverse array of “tools in her toolbox,” Dr. Knight said one of the most important tools is actually the patient. “We need to change this idea that the outcome is solely dependent on the physician, to one that values the therapeutic partnership. While the physician must give guidance with a prescription plan, the patient determines the full effect by having to execute it.”
A Supporting Environment— Inside & Out Dr. Knight is committed to making sure that her patients have all the support they need to get healthy and stay that way—so much so that the practice includes a Certified Health
Counselor (CHC) who provides patients with direction and support. CHC Dana LeDuff (who is also Dr. Knight’s sister) explained that she serves as coach, counselor and budding nutritionist. “I help our patients overcome obstacles while also serving as someone to whom they are held accountable on their health journeys.” Nurses Kelly Saucier, RN and Whitney Brumfield, LPN round out the staff. In addition to providing personal attention and care, they also share their experiences in blogs on the practice’s website. The blogs have provided them with an outlet to pursue their individual passions: Dana writes about nutrition, Kelly covers cooking and Whitney focuses on exercise. “We want to promote a sense of partnership and healing on every level—from investing in ongoing training for our staff to the actual physical design, décor and ambiance of the office,” Dr. Knight said. Prior to attending the LSU School of Medicine, she received a master’s degree in public health from the Tulane University School of Public Health and Tropical Medicine. “Having a background in public health has helped me see things from a global perspective. That insight, coupled with my personal experiences in rural, family medicine, is what propelled me to embark on this journey toward integrative medicine,” she said. After inviting her sister Dana to visit an integrative practice in Houston last year, the two decided to embark on their journey together. Dr. Knight explained, “Because of Dana’s extensive background in medical management, I wanted her counsel and guidance. She was so moved by the visit that Dana not only recommended that I pursue my dream, she also decided that she wanted to be a part of it.” Dana has since received her certification as a CHC from the Institute of Integrative Nutrition and is now pursuing a master’s degree in nutrition. “We saved each other,” Dana said. “We were both in corporate jobs and questioning our real purpose. Noting that he helps with the shopping, sporting events and music lessons with their boys Trey and Chase, Dr. Knight said she knew she loved Collins early in their relationship. “When we met, I had just started LSU and he had just finished his residency program. He essentially went to medical school a second time as he supported me through my studies. I couldn’t get through the day without him,” she said. One of the biggest lessons she is trying to teach her patients is that there is beauty in imperfection. “Learning to live in the moment is probably the hardest thing to do. It is something I am working on too. Yet, at the end of the day, I know that what we are doing here matters. sets the stage in January for new beginnings. While everyone worries about resolutions, she is in the process of recharging her battery for the coming year. We should all do the same—focus on what we WANT to happen in 2015! There is no better place to do that then right in your own backyard. Winter sunlight is the best stimulus for positive energy. Sit for a few minutes in the direct rays and let your eyes wander over your stripped down landscape. Amazing how the removal of summer leaves allows lots of light to dance into your garden. Use the bare winter bones to begin painting a picture of what you would like to see mid-summer. Remove the things that did not work and replace them in your mind’s eye with something new. This may not be the time to do all the new plantings, but a visual picture may be your canvas when 32 Sophisticated Woman | January 2015
spring arrives. Perhaps some written notes will be helpful reminders when the time for planting rolls around. The garden, just like us, also needs time to rest. That is exactly what winter provides. We see leafless trees and shrubs, bare flowerbeds and brown lawns. These things are not lifeless. They are merely recharging for what lies ahead. You can help nature out just a little by using this down time to prepare for spring’s rebirth. While looking at the bare bone landscape, decide what trees and shrubs need to be trimmed. If they are bare, it is certainly a great time to do so. Leafy plants can wait. Perennials definitely need to be cut back now. If you have no perennials (plants that come back each year), go to a local nursery and ask about plants that do well in this area. Find out when they will come in and where they might flourish in your garden. All of these things will help to
paint that mental picture of positive change in your garden. Don’t stop there. Think about adding garden paths and statuary to accentuate areas not utilized. You have all winter to shop around for the best deals on these ideas. Add this to your mental garden picture and then make it a reality. Use the winter garden as a place to find your own positive energy for 2015. Following nature’s cues instead of fighting them may be just the answer to recharging your own positive vibes for the year ahead. Drink in the warm winter sun whenever you can and rest when the darkness falls! As each day lengthens, you will be refreshed with positive energy and a new canvas in your own backyard. WHEN LISA AND ERIC Beloso purchased their house in Mandeville following Hurricane Katrina, they did not have much of a wish list or design scheme in mind— they simply wanted a place to call home. Almost ten years later, the couple realized that their house had truly become their home in every sense of the word. They adore their neighbors. They have family and friends that live nearby. And like most families who relocated after the storm, their daughter Erica has now lived in the new home almost as long as she lived in their house in St. Bernard. While the fixtures and furnishings reflected their personal style, the couple felt their home did not accurately reflect their lifestyle. In order to make the home more conducive to entertaining, they created an outdoor living space that has since become the heart of their home. Separated into four sections, the space contains a living area, an outdoor kitchen, bar and dining space. All flow seamlessly into one another under a pitched ceiling of cypress planks and decorative metalwork. Lisa said that on any given weekend, they have up to 20-30 people over to visit. Both she and Eric love to cook—and it doesn’t hurt that they do it really well. They often serve fresh seafood from the waters off St. Bernard. From chargrilled and raw oysters to a fish fry or crab boil, the Belosos probably serve as much seafood as some local restaurants, and it’s just as good! “We love having our family here,” Eric said. “One night we may be watching an LSU game and getting rowdy and the next night we’ll have our nieces over watching a movie and drinking hot chocolate. It doesn’t get any better than that.”

MANY ADULTS have been faced with a toothache at sometime in their lives. Root canal therapy was performed to remove the remaining nerve and any infection in the tooth. Most of these teeth become asymptomatic and heal. However, there are times when the tooth remains symptomatic or becomes reinfected. At that point, one is faced with the decision of removing the tooth or dealing with the infection. How does one make the decision? There are many reasons for a root canal to fail. There could be circumstances beyond the control of the dentist to thoroughly clean the roots. Many times the seal above the gum becomes compromised and the root canal system is compromised by bacteria. Teeth that are severely broken down pose a greater risk of fracture. The severity of infection prior to treating the tooth can also affect a root canal. The list goes on. One thing is certain; the most successful treatment is the FIRST treatment. All treatment following comes with a lower likelihood of success. With the success of dental implants, this may be a viable treatment choice for the failing root canal. Dental implants have become the gold standard today for tooth replacement.
Many teeth can be removed and the implant and temporary crown placed in the SAME DAY. State-of-the-art technology allows patients to have the temporary fixed to the implant at time of placement for most cases.
