Dear friends and colleagues,

I hope that everyone was able to enjoy the holiday season. I wish all of you the best for 2023.

The CAPD/ACDP Annual Conference in Whistler BC was a great success and I want to thank everyone involved with the meeting, especially Dr. Anita Gärtner and her team. The Scientific Program was excellent, and all attendees enjoyed the social programs. It was good to reconnect with colleagues at the conference after the pandemic interruption. It was also great to see a number of new and younger faces in attendance.

Thank you to the residents for presenting their research projects. Congratulations to Dr. Alexandra Rabalski for receiving the 2022 3M Oral Care Graduate Student Research Award.

Of great concern to all Canadian Pediatric Dentists is the introduction and administration of the Canada Dental Benefit Program. We will continue to monitor the program and advocate for Pediatric Dentists and keep the membership informed of details as they become available.

For the more senior CAPD/ACDP members, a Life Membership category was approved at the AGM. Members 65 years old will automatically have their CAPD/ACDP membership fee reduced by 50%. Please make sure all your membership data fields are updated on the CAPD/ACDP website.

I would like to welcome Dr. Terry Farquhar to the CAPD/ACDP as our new Secretary-Treasurer. Terry will be a great addition to our team. Many thanks to Dr. Ray Lee for his many years of service on the executive. His contributions and humorous interjections will be missed.

The New Life Membership Category

A message from Dr. Duy-Dat Vu, Chair, Membership Committee

CAPD/ACDP Members,

At the Annual General Meeting on October 28 in Whistler, BC a motion was passed to introduce a new Life Membership category. Eligible members of the Academy who have been active for 30 years or more and/or have reached the age of 65 by September 30 of the current membership year and maintain an active license to practice will be automatically transferred to this new category.

It was further determined that the annual dues will be 50% of the CAPD/ACDP membership dues Life Members shall pay all dues and assessments and are eligible for all the privileges of the Academy including the right to vote, nominate, and to hold office.

2022-2023 Life Membership Dues:

CAPD/ACDP Life Member: $168.00 CAD

Joint CAPD/AAPD Life Member: $530 CAD (Just a note that in order to be eligible for AAPD Life Membership, a member needs to be 65 years of age or more AND an AAPD member for 30 years or more.)

CAPD Life / AAPD Active Member: $890 CAD

CAPD/ACDP members are reminded that they enjoy a 25% discount on the AAPD portion of their membership dues when they pay their Joint Membership through CAPD/ACDP.

Eligibility:

To assess eligibility for the new Life Membership category, we would ask each member to update their profile on the website with their month, date and year of birth at their earliest convenience.

Once profiles are updated, we will inform members automatically when they become eligible for CAPD/ACDP Life Membership.

As the CAPD/ACDP Life Membership category is retroactive to July 1, 2022, a number of members have already transferred to this new category.

Update on the Canada Dental Benefit

CAPD/ACDP members continue to receive information on the interim Canada Dental benefit from CDA or their provincial dental organization. However, CAPD/ACDP wanted to let members know that as an organization we are actively engaged and considered an equal stakeholder by Health Canada in the development of the Canada Dental Benefit. As such we continue to be invited to the Minister of Health’s Round Tables with Oral Health Care Associations, hosted by the Minister of Health the Honorable JeanYves Duclos and the Deputy Minister of Health Stephen Lucas.

Round tables were held in May/June of 2022 and December 14, 2022. I represented CAPD/ACDP at these meetings to ensure that the national voice of children’s dentistry is heard. The most recent round table was to seek input from Oral Health Care Associations on how the next iteration of the Plan should be structured. This included discussions on measuring outcomes and how the National Plan would interact with already established dental plans.

In addition to these round tables Dr. Paul Andrews, Dr. Jennifer MacLellan and I attended the information/question and answer session presented by Health Canada and the Canada Revenue Agency on the unveiling of the Canada Dental Benefit.

At the CAPD/ACDP Annual Conference in Whistler BC, the CAPD/ACDP Executive Council received an update from CDA President Dr. Lynn Tomkins on CDA’s interactions with Health Canada. The Executive council let Dr. Tomkins know that Pediatric Dentists across Canada are called upon to care for the most vulnerable children with ECC/SECC. Dr. Tomkins received suggestions and guidance to ensure that the specialty of Pediatric Dentistry is considered as the plan develops further.

EXCITING OPPORTUNITY FOR CAPD/ACDP!

Bid for the 31st IAPD Congress 2027

CAPD/ACDP has submitted a Letter of Intent to host the 31st Congress of IAPD in Vancouver, BC in June 2027. CAPD/ACDP joined the IAPD as a member organization in 1973 and last hosted the IAPD Congress in Toronto in 1982. A formal bid will be presented by Dr. Ross Anderson and Dr. Sarah Hulland on behalf of CAPD/ACDP in Maastricht, Netherlands on June 12, 2023.

Hosting the 31st IAPD Congress is a move for greater social opportunity and responsibility and will place the CAPD/ACDP on the world stage. In addition, the potential revenue will allow CAPD/ACDP to fulfill a long-standing goal of creating a legacy fund.

How can you support the bid?

UPDATE AND CLARIFICATION ON THE NDSE, AND MRCDC & FRCDC DESIGNATION GUIDELINES

I was honoured when Ross Anderson asked me to prepare this update and clarification regarding the history and current activities of the Royal College of Dentists of Canada (RCDC, College). I was asked to report specifically on two topics, 1) the status of the delivery of the National Dental Specialty Examination (NDSE), 2) the historical context and current use of the designation of MRCDC and FRCDC.

National Dental Specialty Examination

It is not possible to provide an update on the National Dental Specialty Examination (NDSE) without first delving into the history of the College.

In 1962, the Board of Governors of the Canadian Dental Association (CDA) approved a recommendation from its Committee on Specialists and Specialization to establish a Royal College of Dentists with four clear objectives in mind:

To promote high standards of specialization in the dental profession.

To set up qualifications for and provide for recognition and designation of properly trained dental specialists. 

To encourage the establishment of training programs in the dental specialties in Canadian schools; and 

To provide for the recognition and designation of dentists who possess special qualifications in areas not recognized as specialties.

Three years later, on March 8, 1965, The Royal College of Dentists of Canada was established by an Act of Parliament. Since its inception, RCDC has administered a high-calibre examination that has been a requirement to gain Fellow status within the College and helped in establishing standards for the recognition of nationally recognized dental specialties.

Initially, interim examinations were offered to practitioners who were already registered as specialists in their province. Successful completion of this initial interim examination allowed the candidate to use the designation Fellow in the Royal College of Dentists of Canada (FRCDC).

Throughout the years, legislations evolved and with it, so did the administration of the RCDC examination for fellowship status. In 1981, the first part of the examination was modified to include a clinical component and was designated as sufficient for “Membership” (MRCDC) in the College. Completing the previous second part of the examination was designated the expert or, “Fellowship” level (FRCDC). The MRCDC category served a useful purpose in that certain of the provincial Dental Regulatory Authorities (DRAs) used this designation as one criterion for specialty licensure. However, based on the medical model of obtaining staff privileges in most hospitals, obtaining Fellowship status became critical for dental specialists providing care in hospitals.

From 2001 onwards, to satisfy the unique requirements of each provincial DRA, the College was asked to develop and administer a standardized “one-step” examination, renamed the National Dental Specialty Examination (NDSE). Consequently, candidates who successfully completed the NDSE could apply for registration and licensure as a specialist in their chosen province. These same candidates were also eligible to apply for Fellowship within the College.

Update on the NDSE and Clarification of the MRCDC and FRCDC Guidelines (Cont’d)

In December 2018, RCDC was informed by the DRAs, through the Canadian Dental Regulatory Authorities Federation (CDRAF), that they were investigating alternatives to having an RCDC administered NDSE as the certification process for licensure of dental specialties. The CDRAF determined that the public interest could only be served if the NDSE was administered by the National Dental Examining Board of Canada (NDEB), the governance structure of which would allow direct oversight of the examinations by the DRAs.

RCDC and other stakeholders, continued to promote the importance of the administration of a national assessment as part of the certification process for licensure for dental specialists in Canada. Ultimately, as requested by CDRAF, RCDC entered into an agreement with NDEB to support the transition of the administration of the NDSE from RCDC to NDEB. The NDEB successfully took over the administration of the NDSE in 2020.

In 2021, the NDEB informed all stakeholders that it wished to withdraw its involvement from the administration of the NDSE for several reasons. The NDEB committed to administer the NDSE until 2023, while a decision was made on how to proceed with the assessment as part of the licensure process for dental specialists.

With the support of the Canadian Dental Specialty Association (CDSA) and the nationally recognized Dental Specialty Associations, including CAPD, the College took the lead on the development of a proposal for the administration of the NDSE in 2024 and beyond. As part of this proposal, the NDEB would collaborate with RCDC during a transition period to help with a variety of aspects of the examination process. An agreement was structured to ensure that all sides are aware of expectations and outcomes, including collaborating with other key stakeholders such as the Commission on Dental Accreditation of Canada (CDAC) and the Association of Canadian Faculties of Dentistry (ACFD).

Significant discussions have taken place between the College and CDRAF to ensure that this proposal would address the concerns of the DRAs for a sustainable model moving forward. The College was pleased to inform its membership at its Annual General Meeting in September 2022 that RCDC will administer the NDSE in 2024 moving forward based on acceptance by the DRAs of the governance model of the current proposal as well as the determination of reasonable examination fees. The structure of the NDSE must carry forward as a written examination to satisfy the DRAs.

The RCDC Board of Directors recently presented a governance model that appeared to meet the needs of the DRAs and is currently in the process of hiring a Director of Examinations to lead this project.

RCDC’s relationship with Dental Specialty Associations is of paramount importance and therefore, I will continue to routinely update our members as work progresses. Although RCDC is committed to the development and administration of the NDSE as part of the licensure process. This will have no effect on the continued provision of the fellowship examinations for specialists to achieve Fellow status in the College.

MRCDC and FRCDC Designation Guidelines

Members of The Royal College of Dentists of Canada are entitled to use the Fellowship or Membership designation of the College. This designation sets RCDC members apart as dental specialists with a demonstrated commitment to excellence in their profession.

New Fellows can use the designation of Fellow of the Royal College of Dentists of Canada (FRCDC) as soon as Fellowship is conferred, which occurs once their name is called by the Registrar at the annual Convocation Ceremony despite their licensure status.

Clarification of the MRCDC and FRCDC Guidelines (Cont’d)

The designation should appear entirely in capital letters after their name (e.g.: Dr. John Smith, DDS, FRCDC or Dr. Jane Smith, MRCDC), and should not be italicized, placed in parentheses, nor contain any hyphens, periods, commas, or other punctuation marks. The use of parentheses around the C in FRCDC is not consistent with the name of the College we are not the Royal College of Dentists of (Canada).

A spelling using parentheses (i.e.: FRCD(C)) was previously used but is no longer the preferred orthography. More so, letting go of the parentheses in the designation is more in line with the healthcare community and provides a more uniformed and standardized way of using the designation.

In addition, to continue to use the respective designations as Fellows FRCDC and Members MRCDC you must remain as a member in good standing with The Royal College of Dentists of Canada.

Whistler Annual Conference

The Medical Model for Managing Dental Caries in

the Pediatric Population

When the 2020 Annual Conference in Whistler had to be cancelled due to COVID Health restrictions, the only option at the time was to re-schedule for October, 2022. The organizing committee thought that a conference late in October, at a travelling destination (at least a 2 hour shuttle from Vancouver) would be a challenge. In the end, with 207 attending the Scientific Sessions, our attendance expectations were greatly exceeded. Attendees knew they were in for something special when the weather cleared, the mountains shone and the Welcome Greeting by President Jennifer MacLellan included a personal appearance by the Mayor, Jack Crompton, but also by the Squamish Lil’Wat Cultural Ambassadors.

The Thursday Conference pre-courses were sold out, as was the President’s Dinner. The new format of a short Welcome Reception (drinks and hors d’oeuvres), encouraged attendees to explore the local restaurants for dinner. And the first major use of the Whova Conference App allowed for social interaction amongst attendees, as well as business interactions with the Sponsors and Exhibitors at the Trade Show.

The National Annual Conference Committee will review and act on the suggestions made in the surveys. Thank you for taking the time to comment!

CAPD/ACDP – AAPD Joint Conference Theme:

Embracing Facial Differences by Harnessing the Power of Interdisciplinary Teams

Partnering with AboutFace Canada, this joint CAPD/ACDP - AAPD conference will focus on a life course of caring for patients with Facial Differences, both congenital and acquired. From early diagnosis, community based Pediatric dentists are often one of the first points of contact for these patients and become team leaders in the management of these often complex patients. Dynamic speakers from Canada and the US will guide community based Pediatric Dentists through this life course from prenatal care to harnessing the power of interdisciplinary care teams. Presenters, many of whom have direct experience with the topic of facial differences, will present and participate in panels that will discuss trauma informed care and how Pediatric dentists can transition patients when the timing is right, through the lens of the patient, the family and the profession.

Please talk about your initial career choices and why you eventually choose to specialize in pediatric dentistry?

I grew up in North Bay, Ontario. After high school, like most, I was not sure how to proceed. I had tried various jobs, my favourite being ski instructor at the local ski hill. I could wrangle kids with the best of them! This was the starting point I think (although dealing with kids was second nature, as the oldest of 5 boys). I ended up studying Nursing in Brockville, ON, and moving to Alberta to work. I spent 5 years working at Alberta Children’s hospital as an RN, mostly in the emergency department, and considered applying to medicine. One day while working I met a Pediatric Dentist, Dr. Rory Vinsky, who came to the hospital for a dental consult, and we got to discussing pediatric dentistry (which I did not know was a career, at the time, having grown up in northern Ontario where there are no Pediatric Dentists).

My interest was piqued, so I went back to Mount Royal College in Calgary for 2 years to obtain the pre-requisites for dentistry via the University of Calgary transfer program, while working at ACH on the weekends. I was accepted to Dalhousie University in 2003 with a vision of moving straight to pediatrics, so my wife Tamara and I headed East.

In my first year I read an article in the JCDA by Dr. Alan Milnes outlining a technique using IV sedation for pediatric dentistry. I was hooked immediately, having been an IV sedation provider in the pediatric ER for a few years, and contacted Dr. Milnes to discuss using pediatric IV sedation for dental care as my 3rd year table clinic topic. Dr. Milnes was very kind, and gently told me to cool my jets (I am paraphrasing) and focus on getting through dentistry first.

In 2005 I approached Dr. Ross Anderson to mentor me on the path to specializing in pediatrics. Dr. Anderson was animated, knowledgeable, and firm in his guidance through my 2nd, 3rd and 4th years at Dal, offering me the opportunity to volunteer/learn as a summer student at the IWK dental department for 2 years. The Team there was amazing, and I have a special place in my heart for Dr. Anderson, Dr. Heather Dyment, and Dr. Jen MacLellan who all offered their time and expertise, helping in part to shape me to be the practitioner I am today.

After graduating I spent the next year as the 2nd pediatric GPR resident at the IWK, deeply immersed in hospital dentistry with the IWK Team, which felt like home. After 5 years in Halifax (and 2 kids, Perrin and Tallula, later) we uprooted our new family and moved to Rochester, NY to complete my pediatric training at the Eastman institute for Oral Health. It was there that I once again contacted Dr. Milnes to talk turkey… ”Hello Dr. Milnes, I’m not sure if you remember me…” He did, 7 years later. In 2010, we moved our family once again, this time to Kelowna, BC. I associated with Dr. Milnes for 1 year and then bought his practice, and had another son, Jude, in the meantime. Dr. Milnes and I practiced together from 2010 to 2018. After mentoring me for 8 years, he retired to pursue his many other interests. I’ve been here ever since! I do still miss working with Alan though, an incredible man with an astounding list of accomplishments, breadth of knowledge, and ability to pass on his pediatric dental wisdom to others.

What do you enjoy the most about your career?

I enjoy technical things, problem solving, and am diplomatic by nature. I find working with children, developing relationships, guiding behaviour, and communicating with families interesting, and am constantly being challenged to develop ways to help parents understand the value of prevention.

What do you find most challenging?

Dr. Google. It is not uncommon for families to desire changes to a recommended treatment plan based on research that they have done regarding restorative materials and techniques. I learned long ago from Dr. Anderson and Dr. Milnes that active listening, asking questions and ensuring parental engagement while communicating with families will prevent a lengthy soapbox sermon from falling on deaf ears.

What is the most misunderstood aspect about Pediatric Dentistry?

I am frequently asked about how we (in the office) can work on crying kids all day. I think that there is a general perception that going to the dentist is challenging, based on individual prior experience. These experiences shape how people, parents in particular, think that their children may feel at the dentist. We explain that generally the only crying that happens in the office is during oral examinations of infants and toddlers, while on their parent’s lap. The odd exception exists for noises made by individuals with special health care needs or emergency situations (i.e.: extractions following trauma for infants/toddlers). The goal is to ensure that 1) children are always safe, 2) that they always have a positive experience (except in the cases of emergencies as mentioned), and 3) there is an environment conducive to allowing for excellent treatment to be provided. If this is done awake, great! If sedation or anesthesia is required to achieve these three goals, then that is the appropriate way to provide care.

Industry Involvement. Please summarize the various volunteer positions/activities you have undertaken.

Working in the office is great, however, as I get older and become more experienced, I have learned that becoming involved in organized Dentistry can add an element of fulfilment that cannot be gained simply from working in private practice. When I moved to Kelowna, Dr. Milnes encouraged me to join the Canadian Pediatric Society Oral Health Section Executive. This was an 8 year commitment that allowed me to learn more about working in an organization, provided unique network opportunities, expanded my continuing education exposure, and got me more involved with the CAPD by acting as a liaison between the CPS and CAPD. Locally I have been involved with the Kelowna District Dental Society, also moving through the 3 year executive track. This also allowed for networking, and provided opportunities to speak to other groups in the Interior of BC.

I have recently been appointed the rank of Clinical Instructor in the Department of Surgery with the University of British Columbia, Faculty of Medicine. My goal with this position is to improve the exposure of our medical colleagues to dentistry, specifically pediatric dentistry, thus strengthening the dental/medical relationship. Hopefully in a small way I can help future medical practitioners recognize the importance of excellent oral health, and the link to overall health.

Recently I’ve taken the position of Treasurer for the CAPD/ACDP. This is a decision that did not come lightly. I have always felt that I would move through the Executive track, but was not sure when the ‘right’ time was. After a nudge from Dr. Jennifer MacLellan, and now that I have some years under my belt, I feel that now’s as good a time as ever! I am excited to work with the CAPD/ACDP Team, all of whom I know, having been to every meeting since 2010, BEFORE the arrival of Steve and Claire - arguably one of the best moves our organisation has made to launch into a new era, allowing for improved communication and development of our administrative Team.

Please talk a bit about your role as CAPD/ACDP Liaison for Canadian Graduate Students studying in the US. What kind of issues or concerns do they have?

This position has been challenging in that there is no direct contact between the CAPD/ACDP and residents training abroad. I have struggled to find a way to link the CAPD/ACDP to US trained Canadian pediatric dental residents. Traditionally, contact with the AAPD allows us to reach all of the US pediatric dentistry program directors indirectly via a mail server. In this way we can invite residents to meet at the annual CAPD/ACDP gathering at the AAPD meeting. It also allows us to provide information to residents planning on returning to Canada regarding accessing CAPD/ACDP and its members, how to join, benefits of membership, and an avenue for networking and becoming involved in pediatric dentistry in Canada.

What is your message to Pediatric Dentists (Students, Graduates and those Active in the Profession) about the value of volunteering?

When we begin working after training it’s all about the kids and the teeth. Next is learning the business aspect of dentistry, human resources, paying down the debt that has built up over the years. Eventually, the kids and the teeth come naturally, the business runs, human resources never gets any easier, and the debt shrinks. Throughout this time I encourage pediatric dentists to make an effort to attend the national CAPD meetings, become involved, even minimally, with organized dentistry by being part of the AGMs, and getting involved in local dental societies. Get to know your local MDs and pediatricians. As you gain momentum, great satisfaction can be gained from sharing your experiences with others, whether it is part of a study group or giving local CE. I am early in the game, so I plan to follow my own advice. I am excited to teach more, volunteer more, and give back to our profession. Every little bit counts!

Gazing at the Crystal Ball, what innovations in the profession do you foresee (or dream about) over the next 30 years.’

Everything we thought we knew has been turned on its head in the last 3 years, since Covid took over. There have been some major challenges for so many, but on the flip side there has been a great deal of growth. I think that a strong focus on infection control has helped practices get out of the mode of doing things the way they have always been done, and there have been new innovations as a result, too many to count. Personally, the focus on emergency care and minimally invasive dentistry by further developing and adopting the medical management of caries has been a strong area of growth. I am not a believer in SDF as a ‘silver bullet’ and “‘SMART’ restorations for all” with a move away from definitive treatment. Don’t get me wrong, I am a huge SDF cheerleader. I AM a believer in carefully managing the disease of caries by evaluating risk, instituting a preventive strategy and restoring a child’s dentition back to heath in whatever form that comes. If it is SDF and brushing and flossing, that’s great! If it is full mouth rehabilitation under GA to get a child back to a healthy baseline, that’s great too. I think that the innovation we need to see is to develop more of a medical management approach to caries, but maintain the understanding that the need for surgical care is not going anywhere.

As you seem to lead a fairly busy life, what do you do to relax and unwind?

With Tamara. She’s tolerated me for 22 years

Family is first, for sure. The reality is, I work a lot. In the times between those two responsibilities, I tend to spend my time mountain biking, skiing, reading books on mindfulness and communication, and enjoying a beautiful glass of wine. I try to start each day with meditation that seems to set the stage for a more balanced day.

The International Association of Paediatric Dentistry (IAPD) is a non-profit organization with the objective to contribute to the progress and promotion of oral health for children around the globe. IAPD includes 60 National Member Societies and represents more than 15,000 dentists

The Fellowship Examination Committee (FEC) is responsible to maintain or enhance the organization’s high standards through the administration of a Fellowship Examination; policy development; and volunteer engagement. Responsibilities include ensuring the Fellowship Examination meets recognized psychometric principals and is a valid evaluation instrument; overseeing the development of policies, processes and best practices to maintain or enhance the quality and effectiveness of the Fellowship Examination; and oversee the volunteer management program.

The Commission on Dental Accreditation of Canada (CDAC) is the body responsible for accrediting dental, dental specialty, dental residency, dental hygiene and dental assisting education programs in Canada. CDAC also accredits dental services. In Québec, dental services accredited by ODQ are recognized by CDAC.

To review all of the accredited hospital dental services and internships (dental residencies in Canada) to insure that standards that are reflective of quality service and education are met in accordance with CDAC.
