The Hoffman House, also known as Case Study No. 17, was constructed in 1955 in Hidden Valley Road, Beverly Hills, California. GENERAL NOTES: 1.

Scope of Work: The Project Scope shall include minor renovations to the existing twenty-first floor of 4-74 48TH AVE, 21ST FLOOR UNIT AB, QUEENS, NY 11109 The work to be undertaken will include: demolition of interior partitions and doors, demolition of existing plumbing fixtures, replacement of existing plumbing fixture, new doors and partition and new floor and ceiling finishes.

All work shall conform to the requirements of the New York City Building Code, Fire Department Rules and Regulations, utility company requirements, and the best trade practices.

3.

Before commencing work, the contractor shall file all required insurance certificates with the Department of Buildings, obtain all required permits, and pay all fees required by the governing New York City agencies.

4.

Minor details not usually shown or specified, but required for proper construction of any part of the work shall be included as if they were indicated in the drawings.

5.

The contractor shall coordinate all work procedures with the stipulations of local authorities, building management or board of directors.

GENERAL NOTES CONT'D: Spread Rating and Density Requirements of the applicable section of the NYC Building Code. Any and all insulation installed shall be certified by the Manufacturer to be compliant with the State of New York quality standards insulating materials.

28. General Contractor shall provide for paint, dust, debris and noise containment in accordance with the Alteration Agreement. All vents and exhaust ducts to be fully sealed to prevent infiltration of dust and dirt. Protection of corridor finishes during removal of debris and delivery of materials to be coordinated and approved by the resident manager. 29. No alteration or relocation of any utility, piping or conduit serving another apartment will be allowed.

OTHER RELEVANT PROJECT NOTES:

The contractor shall be responsible for the protection of all conditions and materials within the proposed construction area. The contractor shall design and install adequate shoring and bracing for all structural or removal tasks. The contractor shall have sole responsibility for any damage or injuries caused by or during the execution of the work.

The contractor shall coordinate millwork installation with construction of all partitions. provide additional reinforcement support, or blocking for millwork as required. provide control joints in all work as shown or specified. locations of control joints shall be reviewed by the architect prior to installation.

The contractor shall lay out his own work, and shall provide all dimensions required for other trades: plumbing, electricals, etc.

Manufacturers nameplates, trademarks, logos, or other visible identification shall not be visible in finished construction.

8.

Plumbing work shall be performed by persons licensed in their trades, who shall arrange for and obtain through the Department of Buildings all required permits, inspections and required sign offs.

Items indicated by owner shall be installed by the contractor.

Electrical work shall be performed by persons licensed in their trades, who shall arrange for and obtain through the Bureau of Electrical Control all required permits, inspections and required sign offs.

10. The contractor shall do all cutting, patching, reparing as required to perform all of the work indicated on the drawings, and all other work that may be required to complete the job. 11. All piping and wiring shall be removed to a point of concealment and shall be properly capped or plugged. 12. Typical details and general notes apply to all parts of the work except where specifically detailed or unless otherwise noted (U.O.N.) 13. The General Contractor shall verify all existing dimensions prior to ordering materials or commencing work. The Contractor shall be responsible for all dimensions and conditions which impact the work. Any and all revisions to the plan during construction shall be confirmed with the Owner or the Owner's Representative and the Architect. 14. No representation shall be made that the plans are accurate as to the scale or locations of the existing dimensions. The Owner reserves the right to add, delete, change or modify specifications as required to meet budgetary and time constraints. 19. Submit all plan changes to the Owner or the Owner's Representative along with a change order and an estimate for the cost of the work. 20. Prepare all shop drawings for review by the Owner or the Owner's Representative and the Architect. Allow 2-5 working days for review and resubmit. Submit material samples when required.

21. The General Contractor shall provide proof of General Liability Insurance and Workman's Compensation Insurance.

23. Prepare all surfaces adjacent to the proposed construction areas to protect the finishes of zones not scheduled for demolition. Use a standard of care relative to the preservation of the property and do not make additional work out of the minimal work shown on the plans. 24. Means of Egress: At all times in the course of construction provision shall be made for adequate egress as required by this code and the tenant protection plan shall identify the egress that will be provided. Required egress shall not be obstructed at any time except where approved by the commissioner. All existing means of egress for tenants of the building to be maintained clear and free of all obstructions, such as building materials, tools, etc.

Fire Safety: All necessary laws and controls, including those with respect to occupied dwellings, as well as additional safety measures necessitated by the construction shall be observed. 3.1. All building materials which are stored at the site or any area of the building are to be stored in a locked area. Access to the area to be controlled by the owner, or the General Contractor. 3.2. All materials to be stored in a orderly fashion. 3.3. All flammable materials to be kept tightly sealed in their respective manufacturers' containers. Such materials are to be kept away from heat. 3.4. All flammable materials to be used and stored in an adequately ventilated space. 3.5. All electrical power to be shut off where there are exposed conduits. 3.6. All electrical power in the construction area to be shut off after working hours. 3.7. Contractor, at all times, to ensure there is no natural gas leakage in the buildings, or any flammable gas to be used during construction.

Health Requirements: Specification of methods to be used for control of dust, disposal of construction debris, pest control and maintenance of sanitary facilities, and limitation of noise to acceptable levels shall be included. 4.1. There shall be included a statement of compliance with applicable provisions of law relating to lead and asbestos.

Compliance With Housing Standards: The requirements of the New York City housing maintenance code, and, where applicable, the New York state multiple dwelling law shall be strictly observed. 8. Dust Control: 8.1. Debris, dirt and dust to be kept to a minimum and confined to the immediate construction area. 8.2. Contractor to isolate construction area from occupied building area by means of temporary partitions or heavy weight drop cloths. 8.3. Debris, dirt, and dust to be cleaned up and cleared from the building site periodically to avoid excessive accumulation.

Smoke/Carbon Monoxide detectors or devices shall be installed in accordance with the Housing Maintenance Code. Multiple Dwelling law, the New York City Building Code, the New York City Electrical Code and NFPA 72.

9. Noise after Hours: 9.1. Construction operations will be confined to normal working hours: 9 am to 4 pm, Monday to Friday, except legal holidays or as stipulated by the Building Owner. 9.2. Contractor must obtain written permission from all affected parties to work other than regular hours.

Each Smoke/Carbon Monoxide Detector shall be installed in accordance with Reference Standard UL 2034 and UL 2075 of the New York City Building Code.

Each Smoke/Carbon Monoxide Detector shall be installed outside of each sleeping room in the immediate vicinity or within 15'-0” of the entrance to a sleeping room; inside each sleeping room; and on each story within a dwelling unit, including below grade stories and penthouses per 908.7.1.1.1

The provisions of Subchapter 19 of the New York CIty Building Code shall govern the conduct of all construction and demolition with regard to the safety of the public and property.

10. Construction operations will not involve interruption of heating, water, or electrical services to other tenants. 11. Construction work will be confined to the apartment interior, and will not create dust, dirt, or other such inconveniences to the other apartment units within the building. 12. All precautions will be taken to prevent dirt and dust from permeating other parts of the building during the progress of alteration. Materials and rubbish will be placed in barrels or bags before being taken out of the apartment and removed from the premises. Contractors to use the service elevator to remove all garbage.

Such smoke/carbon monoxide detector must be either the ionization chamber type or the photoelectric detector type as per New York City Building Code Sec. 907 and 908. 12. A “Certificate of Satisfactory Installation for Smoke/Carbon Monoxide Detectors” must be filed with the division of code enforcement H.P.D., 10 days after installation.
