I have celiac disease, and I don’t really cook much special food for myself. That’s how it is; we mothers just don’t dedicate the time and thought to ourselves as we would for our kids. I buy whatever challah and bread is available at the store and make do with the basics. I’m also not the type to research extensively and read up on the condition, so this week’s issue on celiac disease was a real treat for me. So much great information and so many ideas arrived right into my hands! A SMOOTH CUT (Re: Chocolate Rings and Glittery Things, Issue 60) I’m writing to thank you for the delicious chocolate log recipes. I tried the praline and the pecan chocolate logs recently, and they came out so pretty. However, I thought to mention that I sent one log to be served at a simcha, and since there were only cake knives provided, we had a hard time cutting the logs. (I then tried cutting the log at home with a good knife, and that worked well.) My question is: Did I do something wrong, or is it meant to be on the hard side? I would very much appreciate some clarification, since I really enjoy working with chocolate. Thanks in advance, F.F. PARSHAS BO A Yid and His Brother
In one of the final scenes on their stage of power, the Egyptians found themselves plunged into darkness. The Midrash says that during the second phase of Makkas Choshech, even a candle could not remain lit because of the intense darkness. The pasuk states that during these last three days of Choshech, a person could not see his brother nor stand up from his place, while in the homes of the Yidden, there was light. Why does the Torah describe the darkness this way? Perhaps it could have given the more detailed imagery of the candles that could not be lit, but extinguished on their own. Why are the words “Lo ra’u ish es achiv — A man did not see his brother” (Shemos 10:23) used to define this period?

IN A VILLAGE not far from the city of Vorka lived a Yid whose only son, born after many long years of waiting, was desperately ill. When he lifted his face, it was marked with pain. “The gates of Shamayim are locked,” the Rebbe said sadly. “There’s nothing I can do to help you.” The poor Yid burst into uncontrollable sobs and left the room. He cried inconsolably the entire way home. Not long after he arrived at his house, there was a knock at the door. To his surprise, the Rebbe, Reb Mendel of Vorka himself, was standing at his doorstep. The chassid welcomed the Rebbe inside and asked about this strange visit. “When you stood before me,” said the Rebbe, “I knocked on the Shaarei Rachamim, but they were locked. But when I saw your pain, I asked myself how I could possibly help. I couldn’t poel a yeshuah for you, but I can share your pain! I came to cry along with you.” And with that, Rebbe and chassid sat side by side sobbing and pleading with Hashem to have rachmanus and heal the poor child. Indeed, the illness took a turn for the better, and within a short time, the child recovered.

Such is the ko’ach of a Yid! A Yid feels the pain of his brother when he is broken and rejoices with him when he is happy. Not so the Egyptians. They did not feel along with one another; rather, each one lived in his own bubble, caring for no one other than himself. And that is why, the Netziv of Volozhin explained, the Torah emphasizes that in the darkness, an Egyptian could not distinguish even his own brother whom he knew well and should have been able to recognize even with little light. The Imrei Emes of Gur expounded that the Egyptians did not see one another’s pain, and therefore they could not stand up. But Bnei Yisroel, who saw and felt each other’s heartache and distress, merited having a bright glow light up their homes. The Chiddushei HaRim added that darkness is the worst when a person doesn’t want to see his brother in pain and refuses to stretch out his hand and offer his help. Klal Yisroel had already developed the art of caring decades earlier. Yaakov went down to Mitzrayim with 70 family members, but the pasuk says “shivim nefesh” in singular language. They had honed their quality of achdus and sensitivity to one another and were like one being. By noticing the feelings of another Yid, we demonstrate that being part of Klal Yisroel is being part of one whole. When a tooth hurts, the rest of the body feels the pain too. This is the essence of a Yid. When we feel the pain or joy of another part of our extended guf and reach out to help or share in the simcha, we are exercising our power to dispel the darkness and illuminate the world.
Did You Know? In one Egyptian tomb, sandals made of solid gold, presumably worn by queens who lived about the time of Galus Mitzrayim, were found. Already in that ancient period, goldsmiths developed methods to hammer sheets and strings from their country’s rich supply of gold. And while the Egyptians were immobilized in the darkness, the Yidden were scouting out these golden treasures, which they ultimately inherited on their way out of Mitzrayim.

Simplified COVID-19 Vaccinations for Holocaust Survivors Senior citizens living in New York City may be eligible to receive their COVID-19 vaccinations, but with the process often fraught with frustration, several communal organizations have teamed up to streamline it for Holocaust survivors. Boro Park 24 reported that the Boro Park Jewish Community Council, the Boro Park Y and Ezra Medical Center have been working hand in hand to facilitate the process, and vaccinations are being offered at Ezra Medical Center to any survivors who sign up for one with the BPJCC. Holocaust survivors can register for their shots by calling the BPJCC at 718-972-6600, extension 4602. Once a person is enrolled, caseworkers will contact them to arrange an appointment as well as transportation. “Our Holocaust survivors are really, truly survivors,” said BPJCC CEO Rabbi Avi Greenstein. “They need to be given one extra level of care and comfort, and what we are trying to do is making sure that they are treated nicely in the vaccination process. We want to give them special treatment.” Israel’s Ministry of Diaspora announced plans last week to vaccinate all Holocaust survivors worldwide, free of charge, reported The Jerusalem Post. It intends to work with Jewish philanthropists to fund the project, which will be conducted at worldwide vaccination centers, with home visits arranged for those who cannot travel. “During this global crisis, we have an opportunity to support Holocaust survivors, whose resilience continues to guide and inspire humanity today,” said Diaspora Affairs Minister Omer Yankelevitch. “It is our collective obligation to safeguard this treasured, yet vulnerable population in the spirit of mutual responsibility. Now is the time for all of us, Jewish institutions and leaders from across the world to come together in this operation. Together we can ensure that Holocaust survivors are efficiently vaccinated, wherever they live.”
Plasma Treatment Yielding Positive Results for COVID-19 Patients With vaccine rollouts proceeding at a frustratingly slow pace, widely available antibody treatment is good news for those testing positive for COVID-19. Yeshiva World News reported that both Flatbush

Hatzolah and the Sephardic Community Alliance have been touting the benefits of monoclonal antibody infusions, which were granted an emergency authorization by the Food and Drug Administration for high-risk individuals who receive positive results on a COVID-19 PCR test. Antibody infusions are done on an outpatient basis throughout the United States and have yielded excellent results when given to patients in the early stages of the virus, said Alan Esses of Flatbush Hatzolah. “These scientifically produced antibodies given in day two, three or four can stop the disease in its tracks,” explained Esses. “It is a simple procedure that can be done in just a few hours, and it is really keeping people out of the hospital.” Monoclonal antibody infusions are being given to those over 65 who are overweight and have chronic kidney disease, diabetes or are immunocompromised; those over 55 with heart disease, hypertension or chronic respiratory disease; and 12- to 17-year-olds with certain health conditions. The number of facilities offering treatment on a walk-in basis continues to expand and in NYC currently includes New York Community Hospital, Mount Sinai Hospital, Staten Island University Hospital and Richmond University Medical Center — and by appointment only at ODA-Wallabout Health Center in Williamsburg and Ezra Medical Center in Boro Park. In the Monsey area, Good Samaritan and Montefiore Nyack hospitals are offering walk-in infusions, with Refuah Health Center in Spring Valley offering treatment by appointment only. In New Jersey, walk-in treatment is available at Holy Name Medical Center in Teaneck; Monmouth Medical Center in both Long Branch and Lakewood; Community Medical Center in Toms River; St. Mary’s General Hospital in Passaic; St. Joseph’s University Medical Center in Paterson; and Newark Beth Israel Hospital. Multiple hospitals in South Florida are also on the list. Esses said that those undergoing the intravenous treatment typically find themselves feeling better within days, while previously, those who contracted COVID-19 could see their conditions worsen for more than a week, if not longer. With treatment available nationwide, Esses said that anyone testing positive for COVID-19 who meets the criteria should contact their local hospital or bikur cholim to find out where monoclonal antibody treatment is available nearby. “We want to get the word out to everyone in our community,” said Esses. “If you have COVID, and you meet the eligibility criteria, don’t wait until you are really sick to seek treatment. Speak up, go to the ER, get the infusion and then go home and get better.”

Maybe getting used to these kinds of things was superfluous, if they would be going back to the old house soon. Maybe they would soon be feeling that same confused sensation, like after a surprising dream that takes you to some far-off place… No, they could not go back there so soon. The contract that they had signed with the tenants who were renting their old apartment wouldn’t be up before a year. So what would they do? Start wandering from one rental to the next? “You’re not listening to me, Elisheva,” Blumi said. “Sorry,” Elisheva apologized. “I began arranging the shelves in my closet, and I was thinking about something. What did you say?” “Now I say that your shelves are probably very organized already, and that you don’t need to do any more organizing. And that they didn’t fix the broken windows, or the beds, or the stacks of hay, or whatever they had then? And that she wasn’t happy with all of this?” “Probably.” “She did exactly what she needed to do with the money she’d been given. And so did you. Is that clear?” “I think so.” It wasn’t always easy to follow Blumi’s train of thought, but now Elisheva actually felt like she did understand. And something about the simple — albeit deep — message was very encouraging. “Good. Just tell me, do you want me to try to convey a message to Ludmir that you know who he is? That you would want to make contact with him, or at least hear about his motivations?” “Or tell him thank you…” Elisheva murmured. “It’s an important question, Blumi.” She took a deep breath and stood up to close her open closet doors. “I can’t decide this myself, though. I’ll ask my husband, and we’ll see, all right?”

House Votes To Impeach President Trump for the Second Time THE PULSE: The House of Representatives voted 232-197 to impeach President Trump on one article of “inciting violence against the government of the United States.” Ten Republicans joined with House Democrats and voted to impeach the president. The charges now go to the Senate, where the upper chamber will decide if the evidence warrants conviction and if the president should be barred from holding public office in the future. The impeachment of the president has further divided Republicans. The party is now at a significant inflection point, with many actors jockeying for a role in setting the party’s future direction.

TRUMP’S DETRACTORS: The president’s strongest detractors consist of the ten Republicans who voted to impeach him. Most prominent among them is Liz Cheney (R-WY), who serves as House Republican Conference chair and holds the third-highest position in Republican House leadership. Adam Kinzinger (R-IL), a veteran of the wars in Iraq and Afghanistan, has emerged as a vocal House critic of the president. Perhaps the most surprising vote came from Tom Rice (R-SC), a hitherto staunch ally of the president representing a conservative South Carolina district. First-term Rep. Peter Meijer (R-MI) said that he is aware that his vote to impeach may spell the end of his political career, “But I think it’s also important that we have elected leaders who are not thinking solely about what’s in their individual self-interest, not what is going to be politically expedient, but what we actually need for the country.”

TRUMP’S SUPPORTERS: Trump’s strongest supporters in Congress remained steadfast in their support of the president. Congressman Doug LaMalfa (R-CA) rose to defend the president and said that his fellow congressmen are motivated to impeach the president because “you hate him for putting America first.” Jim Jordan (R-OH), an ardent Trump supporter, said, “They impeached him once. And today, they’re trying to do it again. Despite all that, he never stopped working for you.” Matt Gaetz (R-FL), Paul Gosar (R-AZ), Louie Gohmert (R-TX) and Mo Brooks (R-AL) all remain committed and outspoken members of the so-called Trump congressional caucus. Newly elected Republican freshman Majorie Taylor Greene (R-GA) and Lauren Boebert (R-CO), both of whom have previously espoused support for Qanon conspiracy theories, are expected to be reliable pro-Trump voices in the House.

THE FENCE STRADDLERS: Many members of Congress maintained that Trump’s rhetoric and actions were incorrect but that they did not rise to the level of an impeachable offense or that even if they did, impeachment would serve to deepen the partisan divide at a time when national unity is of utmost necessity. Republican Majority Leader Kevin McCarthy (R-CA) said, “The president bears responsibility for Wednesday’s attack on Congress by mob rioters,” but that “a vote to impeach will further fan the flames of partisan division.” In a New York Times op-ed, Representative Tom Reed (R-NY) made a similar point when he argued that Trump must face justice but that a partisan impeachment was the wrong path to take. First-term GOP Rep. Nancy Mace (R-SC) said, “I believe we need to hold the president accountable.” Still, she opposed the impeachment because it was rushed and violated “due process.” According to Congressman Jason Crow (D-CO), many Republicans feared physical harm should they vote for impeachment. Crow told MSNBC that “I had a lot of conversations with my Republican colleagues, and a couple of them broke down in tears — saying that they are afraid for their lives if they vote for this impeachment.” Tim Alberta, Politico’s chief political correspondent, said that his reporting confirmed that some Republicans were scared to vote for impeachment. In a similar vein, Congressman Peter Meijer (R-MI) said that some of his colleagues feared voting to confirm Biden’s electoral victory. He told Matt Welch of Reason Magazine that “I had colleagues who, when it came time to recognize reality and vote to certify Arizona and Pennsylvania in the Electoral College, they knew in their heart of hearts that they should’ve voted to certify, but some had legitimate concerns about the safety of their families.”

THE GOP IMPACT: Impeachment has deepened the
existing rift in the Republican Party. Republican leadership would like to move on from Trump, especially post the recent Republican losses of the presidency and the Senate. There is a divide among Republican leadership regarding whether it is best to have the party break from Trump by convicting him in the Senate impeachment trial or by allowing Trump to fade away by himself post the Capitol riot debacle. Despite opposition from Republican leadership, Trump is still highly popular among a substantial portion of the Republican base. He retains support from many rank and file members of Congress. Trump will likely wield significant political power for the foreseeable future. The big political question is if the current chasm in the Republican Party is too wide to bridge and if the party is inevitably set on a path toward fragmenting into competing conservative and populist movements. THE NATIONAL IMPACT: The U.S. Senate will decide whether or not to convict President Trump. A conviction would require a two-thirds majority vote of the Senate, so Democrats will need Republican support. Senate Majority Leader Mitch McConnell (R-KY) will allow senators to vote according to their conscience on the matter, and GOP leadership will not work to hold a party line. Democratic Senate Whip Dick Durbin (D-IL) also announced that he would not whip votes for this second impeachment trial. He told CNN’s Jake Tapper that “When it comes to an issue of this gravity and constitutional importance, members really have to follow their own conscience.” The president’s attorney, Rudy Giuliani, announced that he would likely be part of the team defending the president at the trial. However, Republican strategist Karl Rove said that Giuliani’s involvement would only increase the president’s chances of being convicted. The trial date has yet to be set, and many Democrats do not want it to interfere with Joe Biden’s presidential agenda. A vote to convict the president does not automatically bar him from holding office in the future. Still, the Senate may choose to disqualify the president from ever holding public office again. Should they decide to bar him, the decision is not subject to appeal.
Biden Outlines His Agenda THE PULSE: President-elect Joe Biden announced the
agenda for the first days of his administration. Biden unveiled a $1.9 trillion package to combat the COVID-19 virus and the current economic downturn. Biden also intends to issue executive orders on his first day in office, which will roll back some of the Trump administration’s more controversial measures. COVID-19 PACKAGE: The package includes $1,400 checks to top off the $600 stimulus checks approved in December. The package also includes a one-year ex-

pansion of the child tax credit. The credit will rise from $2,000 per child to $3,000 per child ($3,600 for those six and under). The credit will apply to all children under the age of seventeen. Additionally, the full amount of the credit will be refundable, instead of the refundable $1,400 of the current credit. The bill also includes provisions for expanding the earned income tax credit and larger tax credits for child care. These measures will bring much-needed relief to families with a large number of children. The bill also proposes a national $15 hourly minimum wage and includes additional funding for vaccine distribution and public transportation. The COVID-19 package is intended to serve as a first-step rescue package. In February, Biden will present his Build Back Better plan, which will focus on massive investments in American infrastructure. EXECUTIVE ORDERS: Biden intends to issue a flurry of executive orders in his first days in office. He plans to rejoin the Paris Climate Agreement and repeal the Trump travel ban, which restricts entry to the U.S. from several Muslim majority countries. Biden also intends to restore the Obama-era DACA program, which provides protections for illegal immigrants who were brought to America as children and grew up in the United States. Biden also intends to prioritize the work of reunifying children separated from their parents at the border under the Trump administration’s zero-tolerance program. Biden will also issue a directive mandating mask-wearing on Federal property and for interstate travel. Biden intends to use part of his inauguration speech to encourage Americans to wear masks to help slow the spread of the coronavirus pandemic. THE IMPACT: Biden’s executive orders will take ef-
fect as soon as they are issued. However, his COVID-19 relief package will face strong headwinds in Congress. It is doubtful that the bill in its current form will receive much, if any, Republican support. To avoid a Senate filibuster, Biden will need to retain full Democratic support and gain the support of at least ten Republican senators. Republicans will undoubtedly balk at such a large bill’s costs and its effects on the national deficit. Democrats will highlight the support of many economists who believe that historically low interest rates make borrowing funds to stimulate the economy a worthwhile venture. To gain Republican support, the bill will likely be subject to significant revisions. Should the Biden administration fail to gain Republican support, many of the bill’s provisions could pass the Senate with a simple majority through reconciliation, which is not subject to the filibuster. However, reconciliation takes time and only applies to budgetary items. It is unlikely that the Senate can use it to legislate a $15 national minimum wage. Biden would prefer to go the route of having Congress legislate in a bipartisan fashion and have them pass the bill in an expedited manner.

I love almost any goodie or dessert that involves apples — especially at this time of the year, when the days are cold and crisp and just right for a warm and cozy dessert. These apple-based recipes are so absolutely delicious, they will leave you wanting more. Happy baking!

APPLE CINNAMON CRUMB BARS These hardly lasted a day in my house. Apples, crumbs and cinnamon make these bars simply irresistible!
INGREDIENTS 1 cup all-purpose flour 1 cup graham cracker crumbs (10–12 crackers) ¾ cup brown sugar, divided 1 cup (2 sticks) melted margarine, divided ¼ cup sugar ½ tsp. cinnamon 2 cups chopped apples (about 2 apples) VANILLA GLAZE 2½ cups confectioners’ sugar 1 tsp. vanilla extract 3 T. plus 1 tsp. almond milk DIRECTIONS 1. Preheat the oven to 350°. Line a 9x13” pan with parchment paper and set aside. 2. Combine flour, graham cracker crumbs, ½ cup brown sugar, ¾ cup melted margarine, sugar and cinnamon in a bowl. Stir until well combined. Reserve 1 cup of the crumb mixture. 3. Press the remaining crumb mixture onto the bottom of the prepared pan. Bake for 10 minutes, then set aside. 4. Meanwhile, toss the remaining ¼ cup brown sugar with the apples. Spoon the apples over the baked crust. Stir the ¼ cup melted margarine into the remaining 1 cup of crumb mixture until it’s well incorporated. Sprinkle this mixture over the apples.

5. Bake for 40 to 45 minutes, until the apples are soft. Cool for 20 minutes. 6. Combine the glaze ingredients, and drizzle glaze over the apple bars. Let the bars cool completely to set. Cut them into squares and serve. Store leftovers in an airtight container. Refrigerating leftovers keeps them fresh longer, but it’s not necessary.

2. Prepare the crumble topping. In a medium bowl, whisk together the flour, oats, brown sugar, sugar, salt and cinnamon. Incorporate margarine into the mixture with two forks or with your hands until the mixture is crumbly. Set aside. 3. Peel and chop 2 apples. Place in a medium-sized saucepan. 4. Cut the tops off the remaining 6 apples. Use a spoon to scoop out the insides. (You don’t need to get out every little bit, just enough to create a “bowl” for the filling.) 5. Add the scooped-out insides of the apples to the chopped apples in the pot along with margarine, brown sugar and cinnamon, and sauté over medium heat for 8 to 10 minutes or until the apples are tender. Remove this mixture from the heat. 6. Fill the scooped-out apples with apple filling from the saucepan. Top with a generous amount of crumble. 7. Place the stuffed apples on a baking sheet, and bake for 10 to 15 minutes until the crumble topping is crispy. Serve warm, topped with ice cream and caramel sauce if desired. Enjoy!

APPLE FRITTERS These easy homemade apple fritters are packed full of fresh apples, easy to make, and ready in just 30 minutes! Expect them to become your new family favorite.
INGREDIENTS Oil, for deep-frying (about 3 cups) 1½ cups all-purpose flour 1 T. sugar 2 tsp. baking powder ½ tsp. salt ⅔ cup almond milk 2 eggs, beaten 1 T. oil 3 cups peeled, cored and diced apples 1 cup cinnamon sugar (1 cup sugar and 1 T. cinnamon) GLAZE 1¼ cups confectioners’ sugar ¼ tsp. vanilla extract 3–5 tsp. water DIRECTIONS 1. Heat the oil in a deep frying pan. 2. In a large bowl, stir together the flour, sugar, baking powder and salt. Pour in the milk, eggs and oil, and stir until well blended. Add the apples, and mix until they are evenly distributed. 3. Drop spoonfuls of batter into the hot oil, and fry until golden on both sides — about 5 minutes, depending on the size. Fry the fritters in small batches so the pot is not crowded. Remove them from the hot oil using a slotted spoon, and drain briefly on paper towels. Toss with cinnamon sugar while still warm. 4. To prepare the glaze, combine the sugar, vanilla and 3 teaspoons of water in a small bowl. Add more water, 1 teaspoon at a time, until a pourable consistency is reached. 5. Drizzle the glaze over the cooled fritters and let dry. Note: For an easier version of these apple fritters, you can try this: On a square of store-bought puff pastry, place a tablespoon of apple filling and some readymade custard. Wet the edges of the puff pastry with a little water, and seal tightly to form a triangle. Fry and coat in cinnamon sugar. Serve hot.

Scientist Andrew Pelling was at home one evening, cutting the ends off asparagus shoots for dinner, when he had a eureka moment. He noticed that inside the stalk of each plant was a vascular bundle — veining that served to transport water and nutrients throughout the plant, together with supportive tissue. Why can’t we use these vascular bundles to repair spinal cord injuries? he thought to himself, admitting that the idea seemed rather ludicrous. He imagined the microchannels in the vascular bundles becoming tiny pathways that could guide regenerating neurons, helping debilitated patients to gain normative feeling and even mobility. If his wild idea panned out, the quality of life for many could be drastically improved. Admittedly, there were plenty who scoffed at his idea, but Pelling forged on, teaming up with neurosurgeons, bioengineers, biochemists and clinicians to see if his idea had any merit. They stripped asparagus of its DNA and cells, leaving only fibrous material to be used as a scaffold that would be implanted in a patient’s spinal column. The first patients were nonhuman: rats who were intentionally paralyzed in the hind legs and then fitted with the asparagus scaffold in the hopes of restoring their mobility. The experiments that took place over a stretch of five years showed tremendous promise, with parapalegic animals regaining much of the ability to use their legs. 94 / THE BORO PARK VIEW / January 20, 2021 www.thebpview.com / 718.408.8770
THE STAGES OF APPROVAL When it comes to pharmaceutical drugs, specifically, the road to the market is long and complex, with the average drug being in development for twelve to fourteen years before eventually being released into the wider world of patients and prescribers. The primary reason for the lengthy process is concerns over safety. While there are always potential side effects of drugs, it is vitally important to understand precisely what risk each drug carries, and at what levels of drug exposure those risks become unacceptable. This can happen only through trials and testing. But long before risk versus benefit is even taken into consideration, the germ of an idea must be formulated.
Because of the significant impact this device could have on humans at a time when few other viable options are available, the FDA has now designated these asparagus-based implants a Breakthrough Device — a designation that helps fast-track the approval of medical devices. Even so, trials on humans are set to begin only in about two years. It will take even longer for the device to make it to market. Nothing happens quickly in the world of medicine.
Discovery and Development Unlike Pelling’s eureka moment, most drugs begin with the problem, not the solution. Before setting out to discover how to treat a disease, researchers first choose a disease to focus on, aiming to understand the underlying mechanism in order to pinpoint the molecule (or protein) that is causing the condition. At that point, the molecule — called “the target” — can be isolated and tested against various chemical or biological compounds to see if there is any reactivity that could prove therapeutic. Large pharmaceutical companies maintain libraries with a mind-boggling number of compounds. Novartis Pharmaceuticals, for instance, has a collection of more than 2 million compounds! Some occur naturally in nature, while others have been created in a lab. Running these tests used to be extremely time-consuming as they were historically done by hand. Nowadays, robotic engineering has drastically sped up the research with a process called high throughput screening. In this process, automated machines do most of the work, carefully combining tiny, meticulously managed samples of the target with hundreds of thousands of compounds. Any notable activity that results is reported to a computer system. In this manner, researchers can narrow down the “hits” to a handful of compounds that may have the ability to alter the course of a disease. These hits are then zoomed in on and screened more carefully. Once a promising compound is found, tests are run to discover the manner in which it would be absorbed and eliminated in the body, how it might interact with other drugs or foods, and the best method of introducing the drug into the body. Take, for example, biological drugs, which generally must be administered via injection or infusion, because the drugs contain proteins that are destroyed easily by stomach acid. Other drugs may be suitable for oral prep-

FUDGING THE FACTS Every pharmaceutical company wants its product to do well, to reach the market and save or enhance lives. But like it or not, money is a motivating factor in the drive to succeed. Trials cost a fortune, and by the time a drug hits the shelves, its manufacturers have laid out anywhere from $650 million to $5+ billion dollars! One of the factors that drives up costs for companies is that nearly 90% of tested drugs fail somewhere along the way when they prove themselves unsafe, resulting in a dead-ended waste of funds. There’s naturally a push to see one’s uber-expensive drug succeed, creating a conflict of interest that’s impossible to avoid when a company is conducting its own trials. Sadly, recent years have proven that pharmaceutical companies sometimes feel the pinch and aren’t above fudging the facts. Take OxyContin, for instance. Purdue Pharma, its manufacturer, was desperate for OxyContin to become the leading pain-relief prescription and therefore acted in an underhanded manner to get ahead. Despite heavily marketing the opioid as being a twelve-hour drug with low addiction potential, Purdue knew from original trials that the drug wore off far before the twelve-hour mark in many patients. This created a natural uptick in addiction as patients began to self-medicate more frequently to avoid pain. Or take Tamiflu. After convincing much of the world to stockpile massive amounts of the drug to combat flu epidemics, researchers began asking tough questions, such as: Why was there no reporting of neuropsychiatric events during the trials, when adverse neuropsychiatric events were noted in some patients around the world? Cochrane, a watchdog organization comprising medical and scientific professionals as well as laymen, realized that a huge percentage of trial data had never been made public. When they requested that data, they were refused access. Ultimately, Tamiflu was branded a product

arations, but may require enteric coating to delay their dissolution as they may be too harsh on the upper gastrointestinal tract or need to be released in the intestines for other reasons. The target population, too, is important to take into consideration. A pill being developed for those with arthritis, for instance, should preferably not be small in size, as grasping tiny items can be difficult for this population. Taste and smell are also taken into account in this initial stage; if patients refuse the drug due to one of these, it clearly won’t be successful! PHASE 1:
Preclinical Research Once the general properties of a potential drug are better understood, preclinical trials can begin, both in the test tube (in vitro) and in living beings (in vivo). Toxicity is of prime concern, and the question first addressed is not how to treat the disease, but simply: Will this compound be safe to put inside a human being? Though it tugs at the heartstrings, animal models are used to determine if a drug might prove dangerous for humans. But humans are not rats, and these tests don’t tell the full story, even when the animals are dosed at ten times or one hundred times the intended equivalent

that was marketed with too much pomp and circumstance, its benefits overplayed and its negative traits downplayed.
BUT WAIT! Recent years have seen a surge in “Big Pharma” distrust, which translates into distrust of medicine itself. People are running to “alternative” products, assuming they are safer. But the truth is that humanity is undoubtedly indebted to Big Pharma for the myriad life-saving and life-enhancing drugs that have come our way. The oversight that goes into making brand-name medications far exceeds anything that the alternative world offers. Due to lack of regulation, the alternative market has many unscrupulous individuals passing off medication-laden, useless, and sometimes even dangerous products, freely calling them “herbal” or “natural” at whim, with the goal of wooing customers. That’s not to say that there aren’t quality products there; it’s just extremely difficult to separate the safe from the unsafe.

Even generic pharmaceuticals lack the oversight of pharmaceutical mega-giants, with far too many manufacturers of generics being located in places such as China or India, where inspections are infrequent and the products are often tainted or produced in unsanitary conditions. That alone is a redeeming factor in Big Pharma’s favor. It’s also important to note that despite areas of murky data, even the aforementioned drugs OxyContin and Tamiflu have merit for those who truly need them. Medication is never perfect; it’s an imperfect solution for imperfect people. But it’s often the best we’ve got. With Big Pharma, it’s natural to have a love-hate relationship. As they say: You can’t live with ‘em, but can’t live without ‘em.
SO, WHAT TO DO? According to Dr. Ben Goldacre, an outspoken physician seeking to overcome the flaws in drug development, there has to be a better way. One recommendation is that negative results — not just positive ones — must

human dose. They can, however, often give a heads-up to researchers when a compound may prove dangerous. At this point, researchers begin to get an idea of potential human dosage, and they formulate different strengths of the medication to be used in a small group of volunteers, usually between 20 to 100 healthy, non-diseased volunteers. The medication is administered in varying doses to see what sort of side effects can be expected and what the uppermost limit of tolerable dosing would be. For example, in one experiment, a test drug was well tolerated up to a certain point, after which every volunteer began to vomit as certain processes in their neurological systems were activated. That was clearly the cut-off point of tolerability. At this point, some drugs prove themselves too dangerous or to have side effects that are too uncomfortable, and these are completely scrapped, sending the developers back to the drawing board. PHASE 2:
Clinical Research Once a drug is presumed safe for the human body, efficacy studies are begun on patients affected by the target disease. Usually a few hundred participants are given the drug at the dose chosen in the previous phase. The hopes are that the drug will prove itself therapeutic.
Only through careful monitoring of the patients does this information become available. PHASE 3:
Proof of Clinical Benefit Once knowledge of the safety and efficacy of a drug has been further established, the drug is administered to larger groups of volunteers who have the targeted condition. At this point, the main question being addressed is not Does this drug work?, but Does this drug work better than any treatment that is currently available? Because why replace the best possible option with something inferior? In order to receive unbiased results, a single-blind study may be conducted. This is when the researcher knows whether the volunteer is being administered the drug in question or a placebo — a non-medicinal fake — but the volunteer isn’t told what he is receiving. A double-blind study, which is sometimes used, is when both the researcher and the volunteer are kept in the dark, neither knowing if a placebo or the drug is being administered. The results of these studies are often (especially if positive, rarely if negative) published in medical journals to raise awareness of the up-and-coming drugs and the data points they’ve generated. After several years of studies are completed, an application is submitted to the FDA. The organization goes over the data carefully and either approves or rejects the drug for use in humans. Once approved, the drug can be launched, hopefully proving to be of benefit to mankind.

be published in medical journals, with each trial having been registered with an unbiased party before beginning. As it is, publication bias and the sometimes hidden data prevent doctors and their patients from learning about the downside of medications and making proper risk-benefit analyses. Another way to raise the bar is to require trials to be conducted as randomized controlled trials, in which volunteers are randomly divided into two groups — one that will receive the new medication and the other that will receive either an older medication or a placebo, depending on the trial’s goal. Randomization ensures that there isn’t an unbalanced division of patients, such as would occur should the healthiest volunteers be placed in one group and the most acutely ill patients in the other, thereby skewing the results. Such manipulation can lead to medications that are statistically proven but fail to live up to the hype when actually used outside the lab. While there certainly are kinks in the system that cannot be ignored, the general desire for a safe product is overwhelmingly the goal of the industry. Strong research guidelines paired with oversight agencies, watchdog groups, and thinking physicians mean that the pharmaceutical industry is a blessing in the lives of many, providing truly lifechanging products where there would otherwise be a void. Despite the flaws, it would be incredibly unwise to “throw out the baby with the bathwater.”
THE FINAL COG Our physicians face a constant challenge to not be blinded by medical marketing that doesn’t always accurately reflect the studies, and to instead go beyond surface literature and delve into the data, listen to their patients, and monitor for adverse events. The burden they carry is a heavy one, indeed, for they are the final cog in getting the medicine to market. We owe these physicians a debt of gratitude for carrying this burden, and we pray that they always be blessed with siyata d’Shmaya.

Post-Market Monitoring Even when a drug has made it to market, not everything is known about how the drug will affect the entire range of patients. Simply making the jump from a few thousand test subjects to millions of medication-using patients can reveal negative data that wasn’t picked up on during trials. If significant safety concerns are raised, a drug needs to undergo further testing. Additionally, certain populations may not have been represented in trials, and their unique circumstances may cause previously unknown complications to surface. Long-term side effects that can occur years after using a drug are also watched for — something that isn’t possible in the primary test stages. All in all, the data that is accumulated for each drug is a process of many years and many tests. There are currently over 20,000 FDA-approved prescription drugs, as well as more than 6,500 categories of approved medical devices. While we must be thankful that they exist, we pray that Hashem should help us to never need any of them! hen I was about six or seven, I noticed some loose, white powder that looked like Kool-Aid. I eagerly took a lick. To my dismay, it tasted more like Tide detergent. I was hysterical, but too terrified to admit to my mother what I’d done. In my juvenile mind, I had but a few hours left… This was my first memorable encounter with poison exposure — other than messages on the cereal box from Poison Control, that is.
If you’re like me, you probably don’t think much about poison control. I mean, how often do kids devour laundry detergent or other toxic stuff? Very often, actually. The U.S. Poison Control Centers field one poison exposure report every fifteen seconds! And here’s a second detail you may not have been aware of: Toxic exposures are not limited to children. Of course, kids are curious. They explore the world through touch and taste. That’s why it isn’t unusual to find little ones with a swallowed coin — the culprit in 80% of swallowed objects! — or other ingested toxins they could get their little hands on. But while children under age six account for a disproportionate number of cases, toxicity plagues adults, too. The adult version of poisoning can generally be attributed to an overdose or confusion of meds. Food poisoning is also prevalent in this age group. So what happens when someone suspects a toxic incident? Luckily, there’s help out there, and it comes in the form of a vast and highly reliable information resource named the U.S. Poison Control Center. Poison Control, as it is referred to in short, is the go-to address, be it by phone, text or via their online diagnostic tool. As it’s the absolute authority on this subject, even paramedics and ER staff reach out to Poison Control for guidance. Take the case of one little girl who pasted her eyelids shut using Krazy glue. Those treating her in the emergency room had no clue how to get rid of the superglue without damaging the child’s eyes. The mother listened as the nurse called Poison Control and got step-bystep guidance on how to remove it.
WHAT TO DO Help! You suspect a toxic exposure (which is, by the way, the term toxicologists use rather than “poisoning,” because most of the time, when someone swallows or comes in contact with a substance that might be toxic, the effects are not nearly as bad as imagined). What is your first step? Obviously, if the person loses consciousness or has trouble breathing, drop everything and call Hatzolah! But if the patient is in stable condition or exhibits no symptoms at all, reach out to Poison Control. They’ll want to know pertinent details about the substance, the amount exposed to, time since exposure and the age and weight of the person involved. They usually ask for an email address so their dedicated staff can follow up with you. (Every case is documented and used for further research.) According to Poison Control, the first-aid tips listed below can really make a difference. Ideally, they should be performed within seconds or moments of exposure — even before reaching for the phone to dial Poison Control. For SWALLOWED POISON, drink a small amount of water or milk right away. POISON IN THE EYE should be rinsed immediately. Every second counts when a delay can cause vision loss! Irrigate the eye for 15 to 20 minutes under running room temperature water. The water should hit the nose and run gently into the eyes, rather than being poured directly into the eyes. Blinking is encouraged. Sometimes it’s easier to hop into the shower. A child can be wrapped in a towel and held under the tap. Rinse POISON ON SKIN immediately. Remove any contaminated clothes first. Sticky materials can be removed with mild soap, then let water run on the area for at least fifteen minutes. Move to fresh air as soon as POISON IS INHALED. Stay away from toxic fumes and gases.

Indeed, most toxic exposures aren’t nearly as dangerous as people think. In 2017, 84% of poison exposures reported to the U.S. Poison Control Centers were actually nontoxic, minimally toxic or had only minor effects. But since there’s no way to know when an exposure is really serious, it’s smart to be on the safe side. Reach out to Poison Control; they have the information and knowledge to guide you. They know when a medical checkup is in order and when the situation can be safely monitored at home. Suri* was dismayed to find that her toddler had spilled the contents of her favorite reed diffuser. Little Yossi was holding a stick to his mouth, and the narrow-necked bottle sat near him, empty. The thought that there might be a poison issue didn’t cross her mind; Yossi seemed totally okay. But later that afternoon, Suri noticed that Yossi was unusually drowsy and that his skin was irritated, especially around his mouth. Poison Control advised that she have Yossi checked out at the ER for possible aspiration. He was discharged after six hours of observation… only to be back 24 hours later, at which time he was treated for lung inflammation.
TOXIN TRENDS The Poison Control Centers collect data in real time. This helps them catch hazardous products and follow substance abuse patterns quickly. They send out alerts when there is an unusually high number of cases in an hour or a high frequency of specific symptoms. One trending issue with an alarming number of life-threatening effects is the button battery. You know those little round

batteries, smaller than a dime? You may have noticed them in watches, toys, remote control devices or hearing aids. They are small enough to swallow, yet large enough to get stuck along the digestive tract. The danger is that the battery can form a full circuit and burn the insides of the digestive tract, causing tissue damage or worse. ENT specialist Chris Gindle says that “the problem with these injuries is that they’re not always apparent immediately. The battery gets in there and can start causing injury in about 15 minutes. It can cause severe injury in under two hours.” Poison Control warns parents to take prompt action even if no symptoms are apparent. The ER usually takes X-rays to determine whether the battery has already passed the esophagus. An interesting tip the Poison Con-
trol Center offers: Swallow some honey! It slows the damage. (It’s important to note, though, that honey may cause infant botulism in kids under one year of age and should only be given to older children.) Batteries that get stuck inside the ear or nose should also be removed immediately. Another rising concern involves spray bottles. Let’s face it — if you were a toddler who found that bottle, you’d probably also give a squirt. It’s an irresistible activity for little ones! Studies show that while household cleaner injuries have gone down, spray-bottle injuries are up. Make sure to keep your spray bottles on the “off” position, and ideally, use them only when little eyes and hands are not around…
COVID CURE? Panic causes people to do drastic things. As was the case with one healthy, symptom-free couple in their sixties during the first wave of COVID… This couple from Phoenix, Arizona, heard that scientists were exploring chloroquine and hydroxychloroquine as a possible cure for COVID-19. Unfortunately, they took matters into their own hands and ingested a different form of this drug, chloroquine phosphate, which is an additive used to clean fish tanks. Very soon they were rushed to the hospital, where the husband died of chloroquine poisoning. The wife survived. This tragic story is a reminder to the rest of us to take medical advice only from a physician or other medical authority. * * * * *

COUGH AND COLD SEASON With winter upon us, it’s time to be proactive. First, prepare the medicine chest. Throw out all outdated meds. Though there are usually no ill effects, such drugs are simply ineffective. Doctors usually advise over-the-counter acetaminophen or ibuprofen (not aspirin) for the common cold or flu. Ipecac syrup is not recommended for anyone. Protection against carbon monoxide poisoning is all the more important during the winter, as well, when windows are often closed and ventilation is down. Another issue to keep on top of is moisture-related. You guessed it! Mold. Wheezing, asthma, allergies and cough are among the ill health effects of time spent in damp, moldy environments. You can remove small amounts of mold on your own, but get professionals to take care of the issue when it is more widely spread; their expertise and protection can make all the difference.

Two, four, seven... The pink pills are small and round in my hands, and I drop them carefully into the bottle. It’s late and I’ve really got to arrive for this appointment on time. As I fumble to close the lid, I wonder what the doctor will say. I take this medication periodically for irregular heartbeat and palpitations. It’s the kind of thing I really don’t take often but need to have in the house, and the doctor wants to know how many I’ve used. It’s late and I’m tired and I can hear my baby whimpering from the dinette. Won’t this bottle lid just close so I can get my baby ready and leave? I glance at the clock — seven minutes late — when my hand slips and the bottle falls with a clatter to the ground, scattering little pink balls in all directions.
All while I’m late for my appointment. I gather them up, hoping, praying that I got all of them. A quick sweep of my kitchen floor shows me that it’s clean, and I squelch the little voice inside me that’s telling me to put my baby in his crib and check again. Gas off, oven off, window check, and I’m ready. I get my baby and plant a kiss on his head. He’s seriously cute, this kid. Then I notice it. It’s so small, barely discernible on my speckled floor, but it’s there. A little pink pill. A little pink pill, right next to my baby. When the initial panic fades, I force myself to think clearly. He’s sixteen months old, my
Chaim, and totally eats everything. Think raw cholent beans, fallen pretzels, Cheerios. And my pill was right next to him. Call the doctor. I have to call the do — I fumble for my phone, hating the way my fingers shake, my heart throbs. Answer, answer, please... My thoughts are wild, horrible side effects and scenarios filling my mind in all their gruesome detail. When I finally get through to the doctor, her voice is calm, almost soothing me. Almost. I fill her in on what happened and hope she doesn’t hear the catch in my throat, the wild beat of my heart. Question after question tumbles out of my mouth, frantic almost. How will I know if he swallowed a pill? What will the symptoms be? And one question I fear to voice

aloud: How dangerous is it? When she tells me she’s calling Poison Control, I know she’s worried. That five-minute wait is probably the most agonizing one in my life. I keep checking on Chaim, listening to his breathing, looking at his color. He seems okay. But is he? I lunge for the phone on its first ring, blood rushing to my head so fast I can feel it. “They said the main symptom would be if he gets sleepy within the next hour...” The doctor goes on, and I wonder how her voice is so unemotional and impassive when this is my baby we’re talking about. I scoop up Chaim, holding him close, hot tears prickling at the corners of my eyes. If he gets sleepy… But babies can always get sleepy, and Chaim’s a terrible napper, so how could I ever know? I peer desperately into his eyes, trying to see if he’s alert, normal, awake. Air. I need air. Stumbling over his toys on my way to the back door, I stagger outside and sag onto a garden swing, Chaim a bundle of warmth against my shivering body. I stroke his curls with trembling fingers, waiting, watching. Half an hour passes, and still, Chaim looks normal. I’d love to stay here, in the gentle caress of the wind, but it’s time to go inside, back to life: supper, buses, kids. When it happens, it’s so different from what I imagined. When the doctor said sleepy, I thought he might gently doze off, slipping quietly into oblivion. So when Chaim’s body suddenly drops, limp against mine, horrible panic curses through me. He swallowed the pill. I shake him with fingers that feel like butter, like honey, but still, he sleeps, my son. “Chaim!” My voice is shaky, frantic, horrible, and there are searing

tears streaming down my face as I stand, immobile, in my backyard. And this is how my daughter finds me when she dashes outside after hearing my panicked scream. The doctor tells us to go straight to the emergency room. When I arrive, a panicked woman clinging desperately to a limp baby, the staff is waiting for me. I learn that although the next twenty-four hours will be telling, the first twelve hours are really crucial. He looks so small and vulnerable, lost in a tangle of wires that connect him to the machines that monitor him. Most of his vitals are fine, but his blood pressure is a little low. The hospital we are in is small, with no pediatric unit, and we prepare to be transferred by ambulance to a bigger hospital farther away. Chaim is small and barely dis-
but stable, and I whisper choked tefillos. I hold my son’s hand as day fades slowly into night, as he fades in and out, opening his eyes drowsily and falling back into a heavy slumber. Time stands still in the ICU, all daily worries and thoughts dwindling before the stark reality that hangs here, almost heavy. And through it all, the chesed. The hand of Hashem, seen so clearly. I knew that such a story could be considered child abuse, but it had been like a throbbing worry, secondary to the life of my son. But Poison Control had called the hospital, and the personnel were very impressed that I had been so on top of things after dropping the bottle of pills and had been in touch with the proper experts. Twenty-four hours pass, and my son is still sleepy, looking at me with glazed eyes as I spoon some liquids into his mouth when the fatigue loosens its hold on him. “He’ll be fine,” they say. I breathe, a fresh wave of tears cascading down my face. He’ll be fine. They’re ready to send him home. He’s just another one of their patients, and he’s fine. But he’s my son. I refuse, argue, beg, plead, in my tired stupor, my brain a fog. I live two hours away from a good hospital. What if something happens? They let me stay, in the end, transferring me to another room. We end up spending almost another day there at the hospital. Chaim continues to be sleepy — but they tell me it’s residual and that I don’t have to worry. From beginning to end, it takes him three days to get back to himself. I can’t stop looking at him, my baby, as he sleeps peacefully, curls matted to his forehead. Hi, again. I’m Hendy Friedman, and I’m ready to continue my story. Last week, I shared with you readers that I’d started working at Ohr V’Daas and was becoming increasingly aware that something wasn’t right with my health. At that stage in my life, even with my growing awareness, I hadn’t yet reached the point of actively looking for help. It was more of a cognitive period, during which I started compiling lists of symptoms in my head and comparing them to what I saw in other people. The comparisons were startling, and I began to wonder if anyone else in the world shared my experiences. I kept searching, and sometimes found people who had some of my symptoms, but never all of them. I got married in June of 1993, and as my health fluctuated, so did my symptoms. At one point I was weak and running a low-grade fever, and my worried doctor sent me for many blood tests. They revealed that I was seriously deficient in many basic vitamins and minerals, but none of my doctors made the connection to gluten intolerance. I assumed it was due to my limited diet and made the effort to eat more nutritious foods. I moved to Boro Park shortly after my first son was born, and for a while, attributed any symptoms I was experiencing to the stress of moving. Over the next few years, as my family grew, I started reading everything I could find about gastro issues, skin rashes and joint pain. I read books about candidiasis and cut out all foods that contain yeast. I so nobly ate my matzah every Shabbos, switched from bread to pasta, and avoided fermented foods like pickles and yogurt. I was already eating sourdough bread long before it was fashionable, and decades before it was even remotely palatable. Impressive, right? I was automatically eating less gluten than before, so I did see some mild improvement, but never more than that. I tried cutting out MSG and food coloring, which

admittedly helped with my asthma, but didn’t touch my other symptoms. For years, every one of my doctors told me that I clearly had some sort of

My list of separate diagnoses kept growing. With all those letters after my name, I should have been awarded honorary degrees from every reputable institution of higher learning! I had asthma, allergies, chronic anemia, IBS, gastritis, GERD (acid reflux), too many skin diagnoses to list here, blood clotting abnormalities, gestational diabetes and hypertension, chronic fatigue, and idiopathic joint pain. Try listing those ten times, quickly. Can’t do it? Yeah, me neither. There is one period that stands out vividly in my mind. I was on bed rest for six months at that time, and as I
then had a private practice as a special educator, I was unable to work during those months. Following that period, my daughter was born. She was quite small, and I was instructed not to expose her to other people during her first year, as she was at a much higher risk of becoming seriously ill. That pretty much precluded my running a private practice out of my home, nor could I take her to a babysitter with other kids, which resulted in a financial crisis for me. Well, anybody who has ever attempted to live more healthfully can tell you that proteins and produce are very expensive. Starches are relatively cheap. A pack of spaghetti goes a long way, especially if you overcook it. I remember counting out enough change to buy everyone a ramen soup cup for dinner. They were cheap and satisfying, and after using up whatever we had received in the Tomchei Shabbos packages I depended on then, those Tradition soups became my lifeline. At least my family wasn’t hungry. I, however, felt worse than I had ever felt before. Every one of my symptoms got worse, and I developed new ones that greatly concerned my doctors. My blood test results were off the charts. My sedimentation rate, which measures inflammation and is considered high when above twenty, was consistently around one hundred. I was weaker and more tired than ever.

My white blood cell count was extremely high as well, but despite being poked, prodded, pricked and examined from head to toe, my doctors were unable to come to a diagnosis that would explain my symptoms. I went through so many scans and tests — and even had a bone marrow biopsy with bone marrow cultures done — and spent so much time in medical offices, I started to feel like I was living in an alternate universe, one that was full of pain and discomfort, but offered no answers, no lifeline. I often wondered if, perhaps, I was exaggerating my symptoms, if they only existed inside my head, but then I would have to deal with feeling so ill, and I knew that it was real. Pretending that it wasn’t was simply wishful thinking, because if I were indeed a hypochondriac, I’d have one problem to solve, not several. Then I began to think that this was the life I had been given, that there were no answers, and I resigned myself to it. My relationship with Hashem has always been strong, and if that was what He wanted, I would accept it with love, or at least without complaint. (Okay, without too much complaint.)

Because there’s nothing a sick person loves more than trying to push that bottle cap hard enough while twisting it. Or aligning arrows beyond perfectly so the cap would deign to open. Grr! Couldn’t they make these things a bit easier to handle? Well, only 39 years ago, it was easy as pie to twist that cap and pop that pill. But with equal ease, pharmacy patrons could cure their ailments by pilfering just one or two pills, right off the shelf. No need to purchase that full bottle anymore! Until the pill heyday came to a tragic halt in 1982.

CHICAGO TYLENOL MURDERS In the suburbs of Chicago, seven people died shortly after taking Tylenol. Investigation revealed that the capsules had been laced with potassium cyanide, and that the drugs had been tampered with in stores. The culprit, however, was never found. In quick action that was widely praised, Johnson & Johnson recalled $100 million worth of drugs and updated their packaging with a foil seal over the mouth of their drug bottles, a plastic ring over the neck, and a glued box. Just six months after the crisis, Tylenol became the first product in the industry to use the new, tamper-resistant packaging. An additional safety feature decided by Congress and the FDA: The lid of a medicinal jar should not be removable only by turning it into a special position.
IN HISTORY Prescription bottles have been around since the nineteenth century, when the germ theory became the prevalent theory as the cause for diseases, and society leaped forward on the path of modern medicine. The importance of our health is evident in how these bottles have been produced in thousands of varieties since then, gone from glass to plastic, and come with quite a few monikers too. Call them medicinal, chemical, druggist or prescription bottles and you’ll get the same definition: bottles for keeping medicines prescribed by a physician.
WHY ORANGE? Since some medicines are negatively affected by light, prescription bottles are colored to prevent a photochemical reaction from wreaking havoc on their contents. Orange and brown are most helpful in the damage control department, and incidentally, more cost-effective too. Orange’s vibrancy is also great when looking to find those meds in a mess! Other colors can also protect medication, and some companies personalize the bottles they manufacture by producing them in a different color. Target would be one example; they chose red.

WHY THE EXTRA SPACE?
COTTON, ANYONE?
Ever opened a medicine bottle and felt like you were opening a bag of chips? Yup, same 50-50 ratio of product to air in the package. Why all the extra space? The FDA requires ingredients, uses, directions, purposes, storage instructions and warnings to all be listed on the label of over-the-counter medications, and all this must fit onto the bottle and be large enough to read, too. Also, companies like to streamline the packaging of drugs as much as possible, so having fewer bottle sizes helps. The tools to make pill bottle molds are also very expensive, so that throws cost-effectiveness into the mix as well.
WHAT’S IN A LABEL? All over-the-counter medications must have a Drug Facts label on their outer package that tells you all about the med and when and how often to use it. The information is always presented in the same format: ACTIVE INGREDIENT/S: That’s a list of the therapeutic substance/s in an OTC medicine that makes the medicine work. PURPOSE: The action or category of this medicine (such as antacid, antihistamine and so on). USES: The desirable effects the active ingredients are likely to have and what symptoms or illnesses you can expect the medicine to treat. WARNING: Possible side effects. DIRECTIONS: How and when to take the medication. OTHER INFORMATION: Additional safety and storage instructions. INACTIVE INGREDIENTS: Any additions to the active ingredients
BOTTLE FUN! Did you know how amazingly nifty prescription bottles can be? Peel off the label, wash the bottle well, and have a grand time storing all that’s little. There is no end to the potential here; these bottles can hold bandages, beads, bobby pins, earrings, breath mints, buttons, cash, coins, glitter, rubber bands, safety pins, seeds, toothpicks and much more. Some even use a prescription bottle as a hideaway for a key! They glue a rock to the cover of a bottle, slip a key into said bottle, close it tightly, and push it into the earth. A stroke of genius, but one that is known to many thieves by now.
Cotton started keeping pills company in the early 1900s. Worried about incorrect dosage if consumers were to pop broken pill fragments, the pharmaceutical company Bayer was the first to add the fluffy white stuff to its medicine bottles in order to prevent the pills from jiggling around inside and breaking. With the introduction of pill coatings in the 1980s, cotton balls were no longer needed. Indeed, Bayer stopped using cotton in 1999. But you know how it goes with an old, comfy routine. Customers had no clue the cotton was useless and had come to expect it. Some rely on cotton stuffing as an indicator that their bottle hasn’t been tampered with, and some believe it somehow keeps the pills fresher. And so, many companies have kept the cotton. The National Institutes of Health says the cotton can actually draw moisture into the bottle, and too much of that could cause the pills to stick together, altering their potency. So the next time you manage to manhandle a cotton ball out of your med container, feel no shame in dumping it!
