Welcome to your March edition of The Dental Technician Magazine. Most of you will have got some sort of break during the half term holidays, maybe you managed to escape the horrendous storms and weather we had of late. Either way, I do hope you were unaffected by the storms. In this edition we will have some information about the Chicago Mid-Winter meeting. Ashley Byrne will be giving us a bit of information about why he visits and why he feels it is so important to his lab. We have yet another great piece from Nicki Rowland discussing business and leadership development. The subject of business development came up in the DTGB (Dental Technicians Great Britain) Facebook group just a few days ago. I must say Nicki is fantastic on this subject. I have attended many a meeting and networking event with Nicki, if you are thinking of growing your business or wanting to become a better leader, she is someone you need to speak to.
This month we will also see the return to face to face exhibitions and dental shows, I certainly hope to be at one or two of them for sure. I do hope to meet a few of you, do come and say hello if you see me. Last month I made a plea to readers, I wanted to encourage you to write to me as the Editor of your magazine. On the back of this it has provoked a reader to contact me and we plan to meet to discuss a few issues or concerns they have with the GDC. I hope to report back to you all on this and I do hope this is the start of things to come. I really want to encourage you all to send in your emails, letters or private messages. You may want to discuss a previous article; you may want to raise a concern or you may want to share some good news with us all. Please do get in touch, I would be delighted to hear from you. We thought we would introduce a book corner for readers of The Dental Technician Magazine.
We want to invite fellow Technicians to share your reading interests with us. Have you read a good business book, do you have a favourite author, have you read a great fictional book that you just couldn't put down?
THE FOUR AGREEMENTS REVIEWED BY MATT EVERATT
To start things off, I wanted to share a book that I just could not put down, it is quite a small book and a relatively quick read.
Second Agreement Don’t take anything personally This was a huge life lesson for me. I had been suffering and was incredibly angry by a few things that had been said to me by someone quite close to me. I took this so personally, it was making me rage and each day just seemed to be consumed by constantly over thinking and trying to understand why someone would be so hurtful. Then this section of the book just released me from this negative thinking. I cannot and don’t have to change the way people think or portray themselves, if they want to spend their days being bitter, resentful or jealous and say horrible things, then that’s up to them. I just chose not to listen and not to take anything personally. After all, they see the world the way they want to see it. This was life changing, truly!
I picked up this book on recommendation after attending a Dental lecture. The book has nothing to do with Dentistry I may add! The book is called The Four Agreements: A Practical Guide to Personal Freedom. The author, Don Miguel Ruiz writes about limiting self-beliefs and how we can easily and rapidly change our negative thoughts and transform our lives. Now, ‘transforming our lives’ by reading a ‘small book’ sounds a bit crazy! Trust me I wouldn’t blame you for thinking I was a bit whacky! I started reading the Four Agreements at a time when I was going through some difficult times personally. First Agreement Be impeccable with your words After reading the first agreement I was totally hooked and I cannot tell you the weight that was lifted off of my shoulders. I stopped with the ranting and wasting my time with moaning about others on which I have no control and only use my words in a good way and with love.

Fourth Agreement Always do your best Another great piece of advice. I am one for self-deprecating talk, ‘am I doing my best?’, ‘I could have done that better’ or ‘I’m not quite at my best today’. This is OK, ‘our best’ changes from time to time. When you’re ill, your best might be ‘just getting up, having a shower and opening the curtains’ or you might be having a tough time at work but you manage to get through the day and look forward to cuddling up to your spouse in the evening, you got through the tough day and enjoyed the evening with your spouse. Just do your best, whatever the circumstances, don’t beat yourself up and fill yourself with regrets.
Third Agreement Don’t make assumptions It is so easy to make assumptions and so easy to wrap yourself up in knots by second guessing situations, circumstances and what people are saying. This section of the book really helped me gain the courage to ask questions and to communicate better. Things just get resolved quicker if we communicate and stop assuming.
I can hand on heart recommend this book to everyone, whatever your background, age, sex, religion. It has some gems of advice that are just gold dust and can be helpful in your personal life as well as your working life. I bought the book on Amazon for about £5, I downloaded the audible version too. I pick it up from time to time and have a little read, just when I need a few reminders. In last month’s edition, Nicki Rowland, Director of the Exceptional Leadership Academy (ELA), explored how you can steer your ship to new horizons in 2022 by adopting an executive leadership mindset. In this article, she discusses why the world of business owners is evolving at a fast rate of knots and the importance of having the ability to adapt to change. BE AGILE
It is more important than ever for laboratory owners and their management teams to be agile and adapt to the ‘New World’. The word agile comes for the Latin word ‘agere’ and in conceptual terms means ‘being able to think and comprehend quickly’. You have probably heard the word ‘pivot’ bounced around recently and ultimately leaders need to be able to flex and bend at any point. When they are passed the ball, a basketball player needs to choose a ‘pivot foot’ to swivel on to avoid a defensive player and change direction at speed. In a similar way, dentistry has been turned on its head and the change and ambiguity continue every day. When ‘the only thing that is certain is uncertainty’, we need to be quick thinking to find an opening in the court to get the ball to the basket.
ESTABLISH A SENSE OF URGENCY
A sense of urgency is crucial for those of us who are driving change in dental laboratories. John Kotter, a professor at Harvard Business School, talks about establishing a sense of urgency in his book, Leading Change. Kotter argues that “Establishing a sense of urgency is needed in the change process and helps managers of companies and leaders of change to fight against complacency”. When team members become complacent, it is often because they are happy with the status quo. Questions crop up such as ‘Why do we have to change when the old way still works?' or 'Why change what we do when we do not know if the new way will work?' Urgency is the polar opposite of complacency. Retail establishments use the ‘urgency’ tactic in their marketing – ‘Don’t miss out. Only 5 left in stock’. This approach instils a sense of ‘missing out’ if the buyer does not act quickly. Equally, this works well in the workplace. It helps teams to recognise why change is essential to avoid being left behind. Always sell the
benefits of change to your team to get their ‘buy-in’. I would suggest drawing a vertical line down the middle of a flipchart and comparing your ‘old world’ with your ‘new world’. You can then directly demonstrate the benefits of change by comparing the two.
ADOPT A BALANCED PERSPECTIVE
Laboratory owners are spinning so many plates right now – clients, staff, new ways of working, revenue, cash flow, the list goes on. When I talk about balance, leaders need to acknowledge that a plate may be dropped here and there. Revealing your vulnerability to your team is central to achieving a well-balanced culture as it helps to build trust. Do not see asking for support as a flaw. Instead, see it as an opportunity to work in a collaborative way with your team. In this way, you are setting a precedent whereby your staff also feel that they can ask for help and are allowed to be human. As American businessman, Michael Bloomberg, says “Working collectively and collaboratively is the difference between mediocrity by yourself or success as a team. You have to share the pain and the responsibility and if you do then you will also share in the rewards”. Decision-making is part of the balancing act and the defining art of leadership. Our decision making is not always spot on but is usually for the greater good of the team and the business. As leaders, we need to be careful about making decisions on our own. Remember, your team will hold you to account for the outcome of any decision if you make it by yourself. If you have made it jointly, and something goes wrong, accountability does not rest purely on your shoulders and you can all hatch plan B together. In this way, you are far more likely to gain staff ‘buy-in’ and nurture commitment within your team.

PERSEVERE
Change management can be exciting, challenging and difficult. Implementing change in your laboratory can be a rough ride. In most cases, a large dose of perseverance is required. Don’t give up! You maybe be tired by the continuous battle of getting your team on side or dealing with a problematic client. You may be exhausted introducing new policies and procedures in line with Covid-19 restrictions and directives and training your team to deliver them. However, the battle is worth it. As businessman, Jim Watkins, says “A river cuts through rock, not because of its power, but because of its persistence.” Whether you can see tangible results instantly or they creep in slowly, perseverance is character building. Facing challenges teaches us a lot about our own abilities. Guiding your team through change develops leadership skills. And driving improvements in your laboratory is good for business growth. Isn’t that good for everyone?
LOOK TO THE FUTURE
So, as you face 2022, make a promise to yourself. Be resolute, stand firm and do not become distracted. Plan out what you want to achieve in your organisation and involve your team in decision-making. A team approach generates trust, commitment, accountability and attention to detail – a formula for success! In the fourth of Nicki’s articles, she will be discussing the need to be a great listener to nurture your team and drive results. I started in the plaster room, as we all did then, at Riley Dental Laboratory. I worked for 30 years as a ceramist. Remember PJC, VMK, Captek, allceram - you name it! The first scanner I used was a piccolo! I am now Digital Manager at Metrodent, who would have thought that! Dental born and bred and still in dental! And still learning! I love to get out on my bike and chill with a glass of red - SIMPLES! Dental has changed a lot, is still changing and will carry on changing!

Well after being a Dental tech/ ceramist for 30 years and now moving on into the dark side of being a rep for Metrodent! Covering technical digital installations, training well almost anything really, as like mucking in! These are the things I could not live or work without...
1 TEAM VIEWER
I would say or start by saying the first thing I cannot live with out now especially after all this covid malarky would be Team Viewer. This fantastic piece of software has saved me hundreds of miles and also gives the opportunity to help customers when they need most being a dental technician as most work long hours! Then boom something goes wrong, probably when you have
that rush job to finish, and I probably make a rod for my own back by answering the phone and jumping on Teamviewer late into a Friday evening or even a weekend but when you have a glass of red in your hand! It doesn’t seem so bad, and always good to lend a helping hand when needed the most. Easy to connect via laptop/pc or even phone to give support when required even stopping at Mcdonalds to pick up wifi to connect to that customer in need!
2 EXOCAD SOFTWARE
So after covering Teamviewer, you need some software to support? So exocad software would be my next thing as without this I may not have a position as a digital/ technical Manager at Metrodent, and have no customers to support! But have to say love exocad software - user friendly, stable and as easy or complex as you want it to be, a bit like learning and playing a video game really, and easy to configure to your lab requirements. Not just exocad the software is now seamless with exoplan and chairside for all your planning designing then straight to print or mill! Who as a tech would want to live without it now? Suppose this sums up exocad in a few words - Seamless digital workflow.
3 RED WINE
Well suppose we all have a vice so next thing is going to be a nice bottle of red! One needs to chill and this certainly helps after a days driving or sat at a screen, so a couple I would recommend would be Jam Shed or Jammy Red Roo, both help with a session of team viewer or exocad! Let´s be right, it can´t be all work no play!!
4 MOBILE PHONE
If we are all being honest next thing which most of us utilise would be a mobile phone! Could be classed as being anti social to some but we all use them everyday in some way shape or form. Come on we all use one for communication social media even to the point of emails, booking restaurants ordering drinks and food! Well anything you want you can order through this anti social device! But how would you cope with out it? How do you feel when you think you may have lost it? All those contacts and personal details! These things are not just a phone, they are a mobile computer, and we all rely on them more than we care to admit!
FAMILY
So what’s the most important thing we can not live without, forget money, cars, gadgets, we all need that love and support of our family and friends to get us through those long hours and hard stressful times. We all need that person to turn to, to drink with to rant and rage at! Yep you have all done it! Memories, good times! Not all good, but that makes you realise what and why, and why you put in those long hours, many miles and stress of dental software! I’ll say no more, I think you all know! Sometimes we take the simple things for granted! Life is too short, enjoy it while you can! David North (RDT Digital Manager at Metrodent) and nearly 40 years in Dental but you have to love it!

our journey through life as a dental lab technician will be filled with challenges and opportunities on both a professional and personal level. These begin as soon as you receive your qualification or embark on your apprenticeship, and will include a number of significant milestones. The transition into the working world after graduation/finishing your apprenticeship and finding your first dental lab technician role is just the start. Other significant decisions such as buying a home, getting married and starting a family, progressing in your career, specialising and maybe even opening a lab of your own one day are all potential avenues to be explored. Throughout these life events you’ll have to navigate tax, pensions, working towards retirement and even making a will – all of which can be confusing, especially if you don’t know where to begin.
As such The IFA’s will explore ways to tackle these challenges by providing advice and guidance to fictional character, Bridget Crown. In this initial article, we will look at the first step in the character’s journey and what needs to be considered when she goes out on her own.
FINDING A JOB
Bridget chose to do a Bsc (Hons) in dental technology and go to university instead of choosing to do an apprenticeship. This means that her journey is inevitably going to be very different from someone who chose the apprenticeship route, as these individuals are typically offered a position at the lab in which they received their training. For Bridget, on the other hand, her journey begins by finding a lab position somewhere that can benefit from her newly learned skills. So, what advice could be given to someone in Bridget’s position? The first step for dental technicians entering the field is to register with the GDC. This will involve completing an online registration questionnaire and submitting associated documents such as proof of qualifications. Following this, Bridget will have to consider not only what positions are available, but also what she wants to achieve during this first step into her career. For example, should she aim to work for a smaller, family-feel dental lab or opt for a bigger conglomerate? What about commuting times, accommodation, salary and training opportunities? These initial challenges will have a significant impact on her career and are definitely worth consideration. As she is new to the profession, signing up to a recruitment agency may be a smart way to begin, as doing so will make her aware of multiple graduate opportunities throughout the country.
MANAGING A STUDENT LOAN Once Bridget has found and accepted a role, she will need to give thought to paying back her student loan. Depending on where she studied, her accommodation needs and any family support, the amount of money that Bridget borrowed could vary by a substantial amount. In this case, she studied away from home, at a London university, and her parents couldn’t give her any financial aid. This means that Bridget is now potentially in over £30,000 of debt, especially if you take into account both tuition and maintenance loans that she may have needed during her time studying.

However, as we are assuming that Bridget has finished her time at university, it’s likely that she would have been able to borrow less than this amount each year, as the maximum loan available in the 2021/2022 year was £12,382. So, if we add three years of this loan up – we’re looking at a almost £37,000 that needs to be paid back. Add in the yearly £9,250 tuition fee, and in the worst-case scenario, Bridget now probably owes almost £65,000.i The repayment of the loan is managed through the tax system and only begins if Bridget is earning above £27,295 per year. I
was 21 years old, working for a family run Dental Lab in Rome, where I have basically spent all of my youth since the age of 14, by casting plaster and processing jobs for the senior technicians in both departments of C&B and Prosthetics. God only knows how much I have loved that place but as the time was passing by, my frustration was escalating with my age - I wanted more, I wanted to learn more, I wanted to be able to finish the jobs, and not just be processing them and passing into someone else hands.
It becomes power only when, and if, it is organised into a definitive plan of action, and directed to a definitive end.
I was stuck, busy, too young maybe… but I was determined, and I will never forget that day of July. “Luigi?!” He was my manager. “When will you teach me how to finalise the work?”
“Before I will get retired” he said. “Why you said that?” “Because If I teach you everything you will leave me” He was wrong, I would never have left. However, from that moment I sadly understood that the only way to acquire more knowledge was to leave, but I couldn’t do it without a definite plan of action. It took me nearly 2 years to gain the experience and the courage to leave Luigi. Without his knowledge, every Saturday I was working for free for another Lab just to learn enough to feel confident in myself. I have met many “Luigi’s” in my career. All of them failed to build a successful team, by neglecting to foster knowledge to those who are eager to learn and enhance their skills. Maybe for fear of losing them once the “power” had been given to their hands, or maybe just to avoid an uncomfortable pay rise. In any case they lost many valid and talented people. The same goes for Team members that are not sharing their knowledge with other colleagues. It can be even worse for a team than a “Luigi", as it will create fractures between colleagues and a lack of trust which can passively destroy a team that is not well-bonded.
FOR THE TEAM
Invest in your knowledge: Your brain is the most profitable asset you will ever own and the one that will never stop paying you back. Read books, magazines and listen to podcasts: Nothing is better than starting to build your knowledge by studying books, reading magazines or listening to some podcast. It will help you to achieve your goals, improve your skills and develop your mindset.

Go to conferences and courses: Conferences and expos are a great place to meet professionals and learn from them. The purpose of these events are not just based on marketing, but also to share valuable information and experiences. Courses are what you need to sharpen your skills and refine your techniques. Use your own time: I really want to focus on this aspect. We all are busy in many ways - hobbies, friends, family, children and so on - but if you are not committed to do that extra mile or spend at least one hour a day of your free unpaid time for yourself to be learning and focusing on your personal growth, guess what? You probably will never achieve your goals and will slowly give up on them. I recently started training some of my colleagues into Digital, and my goal and happiness is to see them succeed. They are all at different ages and experience, and I am honestly really impressed to see them committed to learn by staying later or coming in earlier for training in Digital.They are not paid extra for that time, it is their choice and this is their right path for successful results.
FOR THE MANAGERS AND LAB OWNERS
Reward who is giving more I am quite sure you all have a member of your team who is always ready to do that extra mile for your business. Do not forget to appreciate their work. I am not just talking about bonus and pay raise (which are important) but
most of all recognise their hard work with compliments and sincere appreciation. We are all waiting for that magic moment when your boss says “well done, you have done a great job”. American philosopher William James said: “The deepest principle in human nature is the craving to be appreciated”. Unfortunately, sometimes we are only called out when we have done something wrong. As the old couplet says: “Once I did bad and that I heard ever. Twice I did good, but that I heard never.” Invest in your team Your team is one of the main reasons for your business success. I have been working in some places where the main concern was where to cut expenses, even on the milk and tea for the staff... It is right to not waste resources but do not forget to invest in your team by offering courses, materials and equipment. Ask their opinion and take time to listen and consider their requests. Obviously there is a limit to anything - if tomorrow I asked my boss to pay for me to attend a 3 day course in New York, he would probably choke while drinking his tea (but I am sure he would consider if the course would be worth the cost). Take time for yourself Do you remember the last time you had a break form work to take part in a course for sharpening your managing skills or your business knowledge? As for the team its fundamental for the superior to keep their managing and mindset skills up to date.

Time is money, nothing is truer than this in business, but don’t forget to take time off for yourself and literally turn your brain off from work, especially when you are at home. Having a business to run and provide everything it needs to run smoothly is an endless challenge, this could drain you, and sometimes ruin your work-life balance. Delegate more. Building trust with your team will make you feel safer to be away for few days and will improve your team’s managerial responsibilities and problem solving skills. Just ask yourself: How long could your business run without you? If the answer its only few months, you might should delegate more.
CONCLUSION
The purpose of this article is not to make you leave your workplace, don’t take me the wrong way, but a continuous search for knowledge is key to helping you reach your career goals. For managers & lab owners, it’s about how to make all of this happen too, as most of the time you are the reason of our success, sadly sometimes our failure. If you are feeling stuck doing something you are not really enjoying, or maybe desiring to move into a different department, I wish you well in finding your path and remember that it is better for you to start thinking about a definite plan of action now, that will take you there.

or many years I had heard the legends of the ‘Chicago Midwinter’ conference, held each February in the USA. It wasn’t until 2015 that I would take the plunge and see it for myself. Weirdly, the US dental market is very different to ours and sadly, the technical and clinical shows are in separate locations, so the ‘Chicago Midwinter’ I knew of is actually just for dentists. Technicians need to book onto the LMT LAB DAY® Chicago, which is run by LMT Communications Inc., publishers of LMT - Lab Management Today - magazine in the US. I’ve been going for seven years now, and it is my go-to conference. I prefer it over anything else, including IDS. 100’s of lectures, an incredible trade show, and some of the most amazing people you could ever meet - all in one incredible city. Sadly, cancelled in 2020 due to the pandemic, LAB DAY® returned this year with only a mask mandate (although there was a vaccine mandate across the City of Chicago). I was thrilled to be heading to the show, an open show with face-to-face contact! We arrived a few days early to get ourselves used to the time change and acclimatise - Chicago is cold, really cold. We landed in -5C with a windchill of -10C and believe me, if you are not wrapped up well, it chills to your bones. My favourite run takes in the Riverwalk, then alongside Lake Michigan and back through Millennium Park – it’s beautiful and exhilarating but be prepared to have frozen eyebrows and ears. LAB DAY® runs from Thursday to Saturday and this year delivered a fantastic line up of 200+ lectures and a ton of product launches. Industry leaders like 3Shape and exocad use LAB DAY® to show off the latest CAD advances and sneak peaks of what is coming in the following year. I saw lectures from dentists, technicians, and sales reps on all sorts of products. This year was dominated by digital dentures and 3D printing, and the US market is way ahead of us on this topic. I spoke to a great guy called Jimmy Stegall who was showing how digital dentistry had revolutionised his lab. His deep Southern drawl and highly entertaining lecture left me itching to get back to the lab and see what we can do. We

watched lectures on IOS, milling, 3D printing, business management, software, resins, digital dentures, bar design…. the list goes on and on. If you crave some quality education and lectures, you need to be at LAB DAY® Chicago. The exhibition hall is also refreshing - let me explain - most exhibitors are using wooden tables instead of expensive stands, enabling incredible 1:1 access to techs, software engineers, and product specialists. Our American colleagues are also delightfully positive and honest. I relish my talks here, with some of the most powerful people in dentistry, and yet you are talking over a wooden table like at a car boot sale. It’s refreshing because it offers all technicians, salespeople, CEO’s, and business owners an opportunity to engage on a level playing field.
Of course, the City of Chicago has an incredible social scene so when the lectures stop and the trade show closes, you could find yourself at a secret Smoked Bourbon cocktail party; or a lively blues bar knocking back a bucket of cold beers with new friends; and let’s not forget the incredible steaks and seafood that Chicago is known for. Without a doubt, this is the best show in the diary for me. I love it and every February I get excited that I’m booked to go again. If you want to some great lectures, a fantastic tradeshow, and a refreshingly different industry perspective to banter about, then get rugged up and head out to Chicago next February - you’ll enjoy an incredible experience.

Founded by Dr Johann Malawana (Course Director), former leader of the Junior Doctor Dispute, and a team of like-minded professionals (Our Faculty and Assistant Course Directors) the Healthcare Leadership Academy launched on 20th December 2016.
This summit will be the first for the HLA’s WIHL group, it will be a prerecorded event and held virtually on the 9th and 10th of April 2022. The Summit theme is: Developing change & pushing boundaries. This theme will be kept broad and we invite speakers to present their ideas on areas such as new innovations and directions in the healthcare field by prominent females, stereotyping/breaking the mould, Leadership journeys, barriers/perceptions of, developing and leading change, advocacy and mentorship to name but a few possible topics. ABOUT THE WIHL In 2021/22 gender and opportunity equality remain topics of fervent debate. The National Health Service (NHS) is facing the challenges of poor staff retention, ever increasing demand and changing demographics of service users. Balanced clinical leadership is required to ensure the NHS can continue to deliver patient centred care and support its employees. As part of the Healthcare Leadership Academy (HLA), a team of current scholars and faculty

members are running a project in support of Women in Healthcare Leadership (WIHL). This project provides opportunity to pose questions, invite open discussion and create a bank of resources for women aspiring to become leaders in healthcare. Though the process of identifying barriers can feel virtuous, the time to act must really be now. In summary our objective is simple; to ensure all who desire have the opportunity, training and support to pursue a leadership role to ensure we equip the NHS with representative and discerning leaders of the future. O
n the 21st of February 2022 all DLA Members received an email highlighting that the HMRC had come about a ruling that VAT must be charged on bleaching trays. I will begin by saying that I am no VAT expert and each lab should speak with their accountant or VAT specialist in regards to their own circumstances.
£85k Threshold
It was pointed out by the DLA that the majority of labs are not likely to be effected by this new ruling. There is a threshold of £85,000 of sales before VAT needs to be applied. The assumption would be that this would be specifically £85,000 worth of bleaching trays as (for now) all other custom made dental appliances are VAT exempt or zero VAT rated. In my role as Technical Director at S4S Dental Lab we've been in a similar discussion before with the HMRC, on more than one occasion I may add. Back in 2005, when we opened our lab, we predominantly made Mandibular Advancement Splints for Snoring and Sleep Apnoea. The HMRC ruling at the time, and on at least two other occasions, confirmed that such appliances are subject to VAT and if sales exceed £85,000 per year, VAT must be charged. So we are relatively 'seasoned pros' when it comes to VAT. We charge VAT already and it is now just a matter of changing our accounting system to add VAT to all bleaching tray sales. What it does mean though, any lab that has sales above the threshold of £85,000 per year in sales of Bleaching Trays and Snoring Appliances for that matter, must register for VAT and begin charging VAT.
Is this the start of things to come?
Now, here is the question. Is this the start of things to come? I personally think it is. I can't help but feel we've been a little hidden, almost invisible profession and so far we have been left untouched by the HMRC. In some ways this is good, in other ways it is to our detriment. I foresee that once HMRC begins looking at our sector, they will come about rulings for other products we provide. For example, take mouthguards or sportsgaurds. We have been told and it has been discussed recently, a sports guard is classed as PPE (Personal Protective Equipment). All other forms of PPE are Vatable. Surely it is a matter of time before HMRC dump this on us too? It is not deemed essential dental treatment, it's not provided on the NHS, will this be next?
Then we get into the realms of cosmetic treatments. Could the HMRC decide that only essential dental treatments can be exempt from VAT? What would be deemed as 'non-essential'? Would anything that has any cosmetic element come into question? Any orthodontic treatment that isn't seen at complex enough for NHS funding, would that be classed as cosmetic and become Vatable?
What does it mean for our profession? Again, these are just my own personal opinions and thoughts on what the reaction will be.
I think the HMRC will begin asking questions directly to lab owners and sole traders. Particularly if they have already had some big wins with some of the larger labs, they will see these new VAT rules as easy wins. If you do get a letter asking for information, you may wish to speak to the DLA if you are a member, and you should certainly speak to your accountant or Tax advisor. In terms of the physical application of adding VAT to those Vatable products, it should be relatively straight forward if you use an accounting system. As a suggestion, having been in many discussions with Dentists, they hate being quoted prices excluding VAT. If you can, show them an invoice that includes the VAT. It is unlikely they will be able to claim the VAT back and will generally pass the costs onto the patient. My thoughts are that most dentists won’t particularly care too much for this exact reason. Will they choose a different lab for their lab products based on whether the lab charges VAT or not? Of course, this does mean that the labs that are now having to charge VAT will be 20% more expensive, unless they swallow up the 20% price difference, I cannot see why any lab would do this though. This does pose as an opportunity for other labs that sit under the threshold to increase their prices in line with those of us in the industry who have to add 20% VAT. I doubt many will do this either. Do remember, if you are charging VAT to products, you can claim back the VAT on the items you buy to manufacture those products. Again, do check with your accountant as to what you can and cannot claim for.

would be to have a chat with your accountant or a VAT specialist. Yes, there are such a thing as VAT specialists. A dear friend of mine is one. He helps individuals or businesses that have fallen fowl of the HMRC and their, often ambiguous' rules. I immediately called him when we received the letter from the DLA. He confirmed much the same as the DLA did. He did say that the HMRC will most likely perform a blanket exercise across the profession by writing to all and ask how much revenue you receive from bleaching trays. It is then likely they would seek to backdate any VAT claims, they have the power to do this and they can go back many years.
Do you keep an account of types of different appliances and devices sold?
It never occurred to me that some labs do not break down the type of sales into categories and will simply invoice an amount on a monthly statement. It was only when a colleague said they simply invoice by patient or number as a reference and the amount charged that it dawned on me that he would have no way of being able to produce a comprehensive report of how many bleaching trays he had done over the year. The only way he could do this, would be to go back through piles of prescriptions and add them all up. I guess this may be an isolated case and most of us are able to produce sales reports via our accounting software or spreadsheets.

I would be keen to hear from any labs or individuals that have already been contacted by the HMRC on this subject. Together we may all be able to help each other and defend any potential back dated claims the HMRC may try to enforce. I must say the DLA have been very helpful so far and they have acted quickly in letting all members know what information they had. If you think you may be exceeding this threshold, or may have previously, I would suggest you get professional advice from your account and a VAT specialist.

ancer, the big ‘C’. I don’t know a person who has not been affected by this in some way, either through their own personal battle or through a close friend or relative. I personally have lost 6 family members in the last 5 years due to various different cancers and more besides in previous years, I have lost work colleagues who have been taken away far too young due to this terrible disease, my mum has survived one cancer and is now battling another type, I have close friends fighting this and in the last year, I too have had my own battle with breast cancer. I am one of the lucky ones who has fought and survived and that is in no small part due to the amazing medical care provided by our NHS and the amazing advances made in different treatments offered over the years. Much of these advances and the increased survival rate of people like myself owes a huge thanks to charities such as Macmillan. Macmillan Cancer Support is one of the largest British
charities that provides specialist health care, information and financial support to people affected by cancer. For the entire month of March I will be participating in the Macmillan plank every day challenge to help raise funds for this amazing charity to play my part in ensuring that they can continue to help the millions of others needing effective cancer care and support. Maybe you or your family at some point? The stats certainly indicate that even you reading this may be affected in some way during your own life. Oak View Restorations was established in 1992 by Nick Powers andJason Foster renting bench space in Chris Cowell’s chrome laboratory. In 1994 the lab moved into its own premises on a farm in Cookham Dean, in the beautiful Berkshire countryside. Over the next 12 years Oak View grew into a progressive laboratory, keeping up with new technologies and materials liked pressables and indeed were the 1st commercial laboratory to take on the Cerec In-Lab system . It was at this time that Oak View amalgamated with Jim Webb, who had run his own successful crown & bridge laboratory in Beaconsfield, giving the business more flexibility to share resources and grow. Expansion plans meant that Oak View would then move again into its own much larger premises in Saunderton, Bucks where it is still today. A prosthetic department was set-up offering our clients a much broader range of services while also managing a consistent high quality. With the majority of the work now being implant-based and technology and materials changing, the Ceramill CAD/CAM system was brought in, giving the opportunity to offer even more to our clients.

ooth whitening is very common practice in dentistry today, made famous by celebrities in the 1980s, and was initially thought to have been discovered by accident by a group of dentists using peroxide to treat gum disease (Kurthy, 2016). The UK tooth whitening industry is believed to be worth over £40 million, as more consumers seek the white, healthy smiles endorsed by many celebrities, according to the 2018 National Smile Month campaign. Market research shows that 99.2% of us believe our smiles are an important social benefit, according to research firm Hexa Research, whilst a further study, published online, found that 48% of adults believe a person’s smile is the most memorable feature upon first meeting. Haywood (2003) suggests that the efficacy of whitening is greatly improved when the hydrogen peroxide level is around 6%, and custom-made dental trays are used to hold the gel close to the teeth for a period of around two hours. Carey (2014) also supports this approach, concluding that

hydrogen peroxide and carbamide peroxide tooth whitening is safe and effective. He felt that ‘home based bleaching (following manufacturer’s instructions) results in less tooth sensitivity than in-office bleaching.’ Cosmetic dentist, Zase (2009), lists sensitivity and compliance as the two main problems associated with tooth whitening. The whitening procedure could help reduce sensitivity, whereas tray design can directly help improve compliance. It is therefore important to consider the design and use of custom-made bleaching trays to achieve the optimal results in a timely and safe manner. BLEACHING TRAY DESIGN Bleaching tray designs have been debated amongst dental professionals over the years, with many opinions formed on the basis of very little scientific evidence to support any particular design. Many assumptions are made and logic used in the absence of science when discussing the specifics of tray design. There are several technical specifications used, such as: scalloped margins, straight cut margins, extension beyond the

gingiva, and reservoirs. The process of thermoforming is also debated; Mizuhashi and Koide (2017) found that vacuumformed appliances maintained material thickness, whilst pressure formed appliances obtained a better fit. Model thickness can also affect the fit of the finished tray. Due to the morphology of the oral tissues and dentition, it is difficult to provide a standardised model size in which to form the tray. Therefore, material thickness can vary from model to model. Mizuhashi and Koide (2017) also noted that the marginal fit of the pressure formed appliances was significantly improved by pressure forming appliances. RESERVOIRS OR NO RESERVOIRS? Matis et al (2002) showed that there was little difference in results in terms of shade difference when comparing trays with and without reservoirs. Despite there being minor shade differences when using a colourimeter, the shade difference was below the threshold of visual differentiation. It would appear that the importance of using reservoirs is linked to how well the margins are sealed. Trays that are scalloped are inevitably more flexible and will allow more bleach to escape the tray, therefore requiring a larger amount of bleach present in a reservoir. There are clinicians, such as Kurthy (2016), who advocate the use of a reservoirs and finishing the tray exactly at the gingival margin. The margin for error from the impression stage to the dental laboratory finishing the trays is high. Further anecdotal evidence suggests that the hand trimming and finishing process deforms the thermoformed tray, leading to a tray that is not as well sealed at the peripheral margins.
Haywood (2008) also states: ‘Reservoirs are not needed to bleach; they merely reduce the tightness of the tray. Teeth bleach just as quickly without the reservoir as they do with.’ DOSING DOTS A relatively new method introduced in the tray design are ‘dosing dots’ or ‘dimples’. Not to be confused with reservoirs, these small areas in the trays are designed to help the patient add the correct amount of gel to each tooth and not overload the tray. Overloading the tray can lead to sensitivity, non-compliance, and gingival irritation (Zase, 2009). Dosing dots are therefore recommended to reduce the risk of complications associated with home tooth whitening. GINGIVAL CONTOURING OR STRAIGHT EXTENSION? Theory may suggest that gingival contouring or scalloping could reduce the amount of bleach having mucosal contact; however, there appears to be no cited references to the benefits to scalloping trays. One could argue that by closely following the gingival margin with a scalloped designed tray, the surface area covered is less and therefore leads to less mucosal irritation, although this theory is not supported by any literature. To the contrary, Curtis et al (1996) demonstrated in a group of 52 patients that there was no soft tissue damage in any of the sample group as a result of the bleaching regime. To further support that the extension of the bleaching tray beyond the gingival margin has no effect on efficacy or sensitivity, Morgan et al (2015) demonstrated in their group of 20 subjects that there was no statistical difference in how effective the bleaching was, nor did the extended or non-extended trays cause any more or any less sensitivity. There appears to be a lack of clinical evidence to support gingival contouring.
Scalloping is only needed with higher concentrations of peroxide. According to Haywood (2008), considering the 2013 laws in the UK that restricted the use of higher concentration ‘in office’ bleaching, the need for scalloped trays is no longer an issue in terms of gingival contact. Research by Cowley et al (2012) compares the fit and retention of thermoplastic retainers in the different design of finishing, these being: scalloped, straight cut at the gingival zenith, and a straight cut 2mm beyond gingival zenith. The best retention was found in the appliances finished 2mm beyond gingival margins, with those finished at the gingival margin the poorest. Although the materials cannot be compared like for like, finishing with a straight-line cut will increase the stability of any thermoformed appliance. This means that the bleach will likely remain in contact with the tooth surface longer, possibly increasing the efficacy of the bleaching material. To further support the benefits of cutting the trays straight in a line 2mm above the gingival margin, Cowley et al (2012) also suggests that the appliances should ‘be more comfortable than before, because there will be less risk of them impinging on the unattached marginal gingiva’. With the current literature stating that there is no benefit in using reservoirs, it would appear counter-effective to have gingivally trimmed trays. Trays with a straight cut margin of approximately 2mm beyond the margin will give better stability (Cowley et al, 2012). The additional flexibility that comes with scalloping a tray can also be problematic in bruxism patients, as the tray is more prone to bending and emitting bleach in comparison to straight cut trays. TRAY MATERIAL There are several materials that are commonly used for bleaching trays, with 1 or 1.5mm soft ethylene-vinyl acetate (EVA) being the most common. In recent years, there has been an introduction of material designed for use in bleaching. A foam-lined tray, for instance, was introduced, with the notion that the foam would have an advantage in holding the bleach over standard trays. However, Haywood et al (1993) proved there was no difference in the clinical results. Manufacturers have introduced a material that has a firmer feel to standard EVA material, commercially available as a ‘bleach tray material’, with the majority of the manufacturers offering 1.5mm semi-rigid as the most popular type for bleaching. CONCLUSION In conclusion, this review has highlighted that there is only a limited number of clinical studies available discussing tooth bleaching. Furthermore, there are even fewer studies to

support bleaching tray designs in relation to their effect on treatment. Further studies should consider tray designs in relation to compliance alongside the efficacy of the whitening gels.
be trimmed straight just beyond the gingival margin to improve seal and stability, and provide comfort and, therefore, increased compliance for the patient.
With the limited clinical evidence, a conclusion can be drawn that a bleaching tray should have some specific design features, whilst other features appear to be operator-led, without having any proven clinical benefit other than it being accepted by the clinician and patient as acceptable.
Although there is no published data to support ‘dosing dots’ or ‘dimples’ on the labial surfaces, it appears to be a useful tool in demonstrating to patients how much bleach to administer per tooth.
There is no evidence to show that reservoirs improve shade reductions, and there is little evidence to support their use other than in trays that are trimmed gingivally to aid the seal. Scalloping would also appear to be counterproductive in terms of patient comfort and tray stability.
