FFECTING ANYWHERE BETWEEN 20 TO 45 PERCENT OF MIDDLE-AGED AND elderly women (according to the North American Menopause Society), vaginal atrophy may be the least talked about, yet most common, problem for women of a certain age. Now there is a solution! The Mona Lisa Touch is an FDA-approved medical laser available for use in the U.S., replacing past treatment options such as hormone therapies, which are often laden with side effects or contraindications. So what is the Mona Lisa Touch and how does it work? Here’s what you need to know. WHAT IS VAGINAL ATROPHY AND WHAT CAUSES IT?

Vaginal atrophy is a condition often coinciding with menopause in which the vaginal walls thin and dry out, creating issues with urinary functions, dryness and tightening as well as pain during sexual intercourse. Though often painful and frustrating for sufferers, few women ask for help –or know it exists.

with patients able to resume sexual intercourse after about 48 hours. After the three treatments, some patients may return for annual maintenance. HOW FAST WILL PATIENTS FEEL THE RESULTS?

Many patients experience symptom relief after just one treatment and almost all feel a difference after the second.

HOW DOES THE MONA LISA TOUCH WORK?

IS IT SAFE?

WHAT DOES THE PROCEDURE ENTAIL?

HOW CAN I GET MORE INFORMATION?

The medical laser works similar to other laser treatments, counteracting the atrophy by stimulating collagen production inside the vagina rather than altering hormone levels; essentially rehydrating, reviving dead skin and improving the condition of the vaginal walls. Patients will receive three treatments spaced about six weeks apart. Each procedure takes only a few minutes to perform. The procedure is done in-office (patients feel only a small vibration) and requires no anesthesia or painkiller,

A multisite study was conducted by two leading Urogynecologists who are experts in the field of postmenopausal gynecologic health; and recent results from a trial on postmenopausal women and breast cancer survivors showed positive results and high patient satisfaction. For more information and to see if you are a candidate for the Mona Lisa Touch, contact Ana Bueno, R.N. at South Miami Women’s Health / Drs. Phillips, Miller, Roca and Venditto at 305.665.9644

Spring is Here! I

T’S HARD TO BELIEVE THAT SPRING IS AROUND THE CORNER. AND -- ALONG with some gorgeous weather — spring brings with it the promise of a new start, making it the perfect time to clean house and get organized. Easier said than done, of course. This issue we delve into the dos and don’ts of spring cleaning, bringing readers expert advice and the latest tools and tricks to keep your closets clean and your sanity intact. Need a truck to haul your junk? There’s an app for that! Need a storage box that can double as a cooler? We found it. And when you’ve piled up your excess we have just the place to make a little extra cash. Elsewhere this issue we meet Neal Roth, whose status as a founding member of Pinecrest’s Temple Bet Shira spurred him to dedicate much of his free time to creating and building a legacy for not just his family, but other Jewish families in South Florida. His work to secure a safe, inviting and inclusive space over the past three decades is an inspiring story no matter where you worship. On a similar note, we were thrilled to cover the incredible achievements of our local schools—both public and private—in everything from sports to academics and to spotlight the pressures that go along with those successes in our Student Corner column thanks to Palmetto student Sabrina Garcia. Voting is well underway for The Florida Villager’s Best of the Best Reader’s Choice Awards. This is a Special Edition that will be coming out in May. LIVING LEGENDS: BRANFORD MARSALIS QUARTET WITH SPECIAL GUEST KURT ELLING

Although following the format of a standard jazz quartet with vocals, the collaboration between jazz’s most intense band and one of jazz’s foremost singer should prove to be anything but standard. Opening for the Quartet will be jazz vocalist, composer and Miami native, Carmen Lundy. The tight-knit working band featuring Marsalis on saxophones, Joey Calderazzo on piano, Eric Revis on bass, and Justin Faulkner on drums rarely invites other musicians into the folds of their nearly telepathically cohesive unit, and Elling’s deep jazz vocabulary, technical versatility, and outstanding intonation will enable the band to perform a variety of material in new ways. ZZ TOP a/k/a “That Little Ol’ Band From Texas,” lay undisputed claim to being the longest running major rock band with original personnel intact and, in 2004, the Texas trio was inducted into the Rock and Roll Hall of Fame.. The band’s music is always instantly recognizable, eminently powerful, profoundly soulful and 100% Texas American in derivation. The Fillmore Miami Beach at the Jackie Gleason Theater, 8pm; fillmoremb.com

Based on the famous memoir of Cuban poet Reinaldo Arenas, Jorge Martín’s “Before Night Falls” follows Arenas’ life from childhood poverty in the Cuban countryside to his emigration to the United States in the 1980 Mariel boatlift, and his last decade in New York City. Disillusioned by the Cuban Revolution and persecuted by the Castro regime as a dissident writer and homosexual, the opera follows his trials and tribulations as a political prisoner forced to smuggle his manuscripts abroad for publication. in English & Spanish with projected titles in both languages. arshtcenter.org

High-end kitchen showrooms Scavolini Store Miami and Miele were stops on the Tour as well as boutique condominium Laguna House. The Closing Event was hosted by Ferguson Bath, Kitchen and Lighting Gallery where tourists were treated to Martini Prosecco and a variety of desserts such as gelato from Dolci Peccato and cupcakes from Happy Sugar Drop. There was also a silent auction and a kitchen talk offered by award-winning kitchen designer Julia Johnston of Kitchens for Cooking.

Capillus laser therapy works through the process of photobiomodulation. Just like plants convert light energy to produce oxygen from carbon dioxide, cells on the scalp convert light energy from lasers into chemical energy to restore normal function in hair follicles. If caught on time, photobiomodulation can reverse the process of hair loss in people with androgenetic alopecia by restoring cellular function at the cellular level of the hair follicle, restoring the hair follicle.

Preservation Society Sets its Sights on Street Lights BY ANDREA CARNEIRO

I

N 1926, 500 ORNAMENTAL BASES WERE COMmissioned for the “White Way” streetlights in Coral Gables. These unique streetlights were designed by Phineas Paist, as supervising architect, and Denman Fink, as art director. Denman Fink, uncle of George Merrick, founder of Coral Gables, also designed the Douglas Entrance (1924), the Venetian Pool (1925) [in collaboration with Phineas Paist], and Coral Gables City Hall (1928)—all listed on the National Register of Historic Places. Unfortunately, most of the original 500 lights have been lost. The remaining lights are endangered and in dire need of saving. Now, The Historic Preservation Association of Coral Gables is working with City of Coral Gables officials to bring attention to the condition and need for maintenance, restoration and preservation of the few remaining historic street lights in the Riviera Section of Coral Gables. Why the White Way Lights? The White Way Lights project place George Merrick as an early advocate of "art in public places." In fact, these functional works of art were commissioned 90 years ago by city founder Merrick, not only to light the fledgling city but to beautify the newly built roads. Originally completed with 500 lights, less than ten percent of these historic lights still exist today, with the remaining lights, although still beautiful, endangered and in dire need of saving. These particular “White Way Lights”, designed in the early 1920s by Phineas Paist and Denman Fink, are located along University Drive from Granada Boulevard past Ponce de Leon Boulevard and along Riviera Drive from Granada to University Drive. At each of the four sides of the base, there is a head in relief symbolic of the life of Coral Gables. Each of these faces represents a different character. “Art and Architecture” and “Horticultural Planting” reliefs are of beautiful women with their appropriate symbolic implements. “Labor” and “Industry” reliefs are represented by men with strong features, also with symbolic implements and tools. Alternating between the heads are the Spanish castle and rampant lion, both important symbols used often in Coral Gables designs. Encircling the top of the

base, in raised letters are the words, “Coral Gables—The Miami Riviera, Fla.”. The posts are of cast iron construction and were originally painted verdigris green. Later, the posts and bases were painted silver. The original Coral Gables’ “White Way Lights” corridor made reference to New York City’s “The Great

White Way” which became one of the nicknames for Broadway in the late 1890s, back when the street was one of the first to be fully illuminated by electric light. The “White Way Lights” were designated as a City Historic Landmark Site in 1981 and included on the City Historic Landmark Inventory.

N A WORLD WHERE TECHNOLOGY IS AT OUR FINGERTIPS AND FINDING INFORmation at the local library is unheard of, students now, more than ever, are feeling the intense pressures that coincide with an increasingly educated and innovative society. TV shows enjoyed by students today such as “90210” and “Gossip Girl” deceptively depict the high school experience as a time of carefree bliss. Quite frankly, these shows are unrealistic. Recently, the line between excelling academically and enjoying what many adults fondly refer to as their “glory years” has become blurred. As a high school student taking five AP classes, an editor of the yearbook, and a cheerleader, I feel the pressure to excel in every activity. We all hope to be viewed as the epitome of a “well-rounded citizen” which, quite honestly, can become overwhelming. High school in many ways now is less of an experience and more of a four-year-long interview. Any adult can understand that interviewing for a job is nerve-wracking; imagine a student having this constant pressure to be perfect every day. According to the Child Mind Institute, “Over 17 million children in the U.S. under 18 have or have had a diagnosable psychiatric disorder, 32 percent of which are anxiety disorders. The median age at which a child is diagnosed with an anxiety disorder is 6 years old.” Students now are being taught from a young age that success is equivocal with quantitative values such as a 4.0 grade point average and a six figure salary. The end goal for many students is college, and among the top students at highly competitive schools like Palmetto, the Ivy League is the focal point. For this reason, these students tailor their schedules to impress their prospective schools and often choose classes that look better on their transcripts instead of classes that pique their interests. It seems as if they spend more time trying to get inside of the minds of an admissions officer and less time focusing on how to expand their knowledge bases. Additionally, students strive to be involved in as many extracurricular activities as possible. We all understand that within approximately five minutes, an admissions officer will make a decision that we have worked an entire four years towards. The uncertainty of whether our efforts will be rewarded is what keeps us going yet also holds us back. I personally understand the effects of late nights spent doing hours of studying when a 7:30pm basketball game goes into overtime the night before an AP Chemistry exam. To me and many others, this is a regular Tuesday night. According to a 2007 study in the Journal of Adolescent Health, “68.9 percent of teens get seven hours of sleep or less per night.” As a small fraction of this 68.9 percent, and a student who stays up late into the night studying advanced topics such as solubility equilibria, I can attest to the need for a break. It is difficult to find a balance between focusing on academics and spending time with my friends. As cheesy as it may sound, I truly love to learn. I am an extremely curious person, and to me, school is a place to feed that curiosity. I have also been known to be somewhat of a social butterfly. High school places an emphasis on preparing kids to compete at a collegiate level, but it is also a time for exploration and self-growth. Unfortunately, many students (including myself) tend to forget the latter. I wouldn’t change my high school experience for anything; it is where I first discovered my love of questioning everything placed in front of me. It has cultivated new understandings of the world around me and is the first step into my future. However, when exhaustion kicks in and the exams seem to be never ending, I find it hard not to wonder, is the prize worth the pressure? Sabrina Garcia is a student at Palmetto Senior High School.

Neal Roth

The Gift of Community

BY ANDREA CARNEIRO

TEMPLE BET SHIRA BACKGROUND:

Growing up on Miami Beach, Neal Roth knew early on that ties to his faith would have an impact on the man he would become. Roth vividly remembers attending services and religious school at Beth Torah Congregation, so his initial foray into philanthropy after returning to Miami post- law school should come as no surprise to those who know him. “A good friend, who I had known since high school, knew of my feelings about my Jewish identity,” says Roth, co-founder of Gables-based law firm Grossman Roth Yaffa Cohen “He asked me to make a donation to the Greater Miami Jewish Federation's annual campaign. I donated $500 even though I did not have the money back then. From that point forward I knew this was something I wanted to continue doing. “

20

THE FLORIDA VILLAGER

A NEW CONGREGATION:

In 1985, Roth became one of the founding members of Temple Bet Shira, a conservative temple in the heart of Pinecrest. The congregation was borne out of the need to serve the growing numbers of young conservative families moving south. The only problem? The property consisted of only a school building. “Being involved at the time, and knowing the importance of the capital campaign which was required to allow the synagogue complex to be built, we donated $350,000 to the capital campaign and the Bet Shira synagogue complex is named in memory of my mother, Letty Roth,” says Roth, who went on to become the temple’s President as well as the first Chair of Bet Shira's Endowment. Since then, Roth has led and contributed to endowment funds that emphasised everything from Jewish education to infrastructure needs of the synagogue. “The last endowment campaign commenced in 2013. We raised $2 million which was two times the amount our feasibility study suggested we could raise,” he says. “Endowments are critical to the survival of the synagogue. Currently, 150 families are participating in the third running campaign that ends this year.”

03.2017

CONTINUING A LEGACY

Roth says his dedication to the temple comes back to family--both the one he has at home and the one he has created within the community. “Having a place to both learn and teach adults and children was instilled in me at a young age, and I wanted to continue that tradition with my family,” he says. “Bet Shira is a true community in every sense of the word. We don’t just speak of ourselves as members—we speak of ourselves as family, where everyone is welcomed. We’re a very warm congregation and look to inspire those around us, especially in our immediate neighborhood.”

LOOKING AHEAD

Counting two children as graduates of the day school (it now houses up to Kindergarten) and three grandchildren who have been through the Early Childhood Education Center, Roth is proud of the past and looking positively into the future as he watches the next generation of leaders come up through the congregation. “There is a very nice group within the younger generation, who have become very active and share a lot of the ideals of the founding members,” he says. “I know the future will be secure in their hands.”

Bet Shira means ‘House of Song,’ and was named to reflect the upbeat and dynamic character of our membership. We all came together to form this congregation, and as a family, we made it happen, and we turned it into one of the strongest temples in South Florida.”

O NOW YOU’VE CLEANED YOUR CLOSETS like a pro… but what to do with all the, ahem, leftovers? Consign it, says Jennifer Kaloti, owner of Pinecrest’s The Recycled Closet. A longtime consignment shopper, Kaloti opened The Recycled Closet in 2008 and soon became a go-to source for South Florida fashionistas, earning accolades including AOL City’s Best, CBS 4 Miami Best Thrift Shop in South Florida 2010, and Miami New Times Best Thrift Shop 2013. “Consignment is a great way to make a little extra money without any hassles,” says Kaloti. Here are her tips for making the most of your closet. What are the best items to consign? We are a middle market store, so we take trend labels, everything from jeans, tops, jackets and formal gowns to handbags, shoes and jewelry. We like things from chain stores, department stores and boutiques. What is the criteria for a good consignment item? If you loved it at one time maybe someone will too! Just make sure it's freshly laundered or dry cleaned and free from stains and tears. We typically look at 20-50 items at a time. Just bring them neatly folded in a shopping bag. How does the consignment process work? Call or email us to make an appointment and we will look at your things on the spot and decide what we can sell. We enter everything in a database and if and when it sells you will receive 40 percent of the selling price. What items are you looking for right now? It's prom time! We are now accepting long gowns appropriate for high school girls, along with accessories such as shoes, evening bags and jewelry. We even have some men’s suits from J Crew, Zara and Calvin Klein... and will accept similar suits in perfect condition.

Ask the Expert: Organized by Aly

A

LY FINKELSTEIN HAS A PASSION FOR ORGANIZATION… BUT IT’S NOT EXACTLY what you think. The New Jersey-based organizational expert isn’t just driven by the visual satisfaction of de-cluttering, she’s onto something deeper, bringing her experience and eye for order to families near and far and allowing them to live with less, while creating more time for family, friends and, well, life. Taking inspiration from her “neat freak” Dad, Finkelstein has grown her business, Organized by Aly (organizedbyaly.com), to include clients from across the Northeast as well as in South Florida, helping them with everything from room transitions and home staging to executive office organization and wardrobe storage. She even offers clients moving support via “The Organized Move.” We spoke with Finkelstein to get her take on getting started, her favorite products and the one room we all seem to ignore. What’s the best way to start spring cleaning? Pick a designated spot. The closet is oftentimes too daunting for people, so pick one category: shoes or swimwear, for example. How do you decide what to keep, what to donate and what to toss? Once you’ve decided what you are going to focus on take all of those things out so you can see them. For example, remove every pair of shoes from the closet. Then clean the shelves. Once you have done that look at each pair and be very honest with yourself about what you will truly enjoy wearing again. If it's old, dirty or beat up give it away or throw it away! What’s your best advice for clients who have a hard time parting with their “stuff?” It’s often very hard to part with expensive or sentimental things, but if you are not going to wear or use the item again it is important to part with it. Space is limited (even in the largest of closets!) and making room for the things you love should be a main priority.

What are some of your “goto” products for organizing? The Container Store has a line of clear storage boxes called "our storage boxes." I am obsessed with how easy they stack, how clean they look and how inexpensive they are. I use them on every single one of my jobs! What’s the room most often neglected? The laundry room! It really should be home base for neatness and organization. I tell clients to start with a blank slate and try to keep only the essentials in the laundry room. What has been your most challenging job? The most challenging jobs are the ones where clients are reluctant to part with belongings but have a vision of a calm, serene space. Often, people have four-to-five times the amount of items in their home for the space they have. I never want anyone to feel like I am forcing them to part with their belongings, but I know that reduction is an essential component of organization.

MARCH 2017

What’s your best piece of advice for clients? Don’t overstock your home; it’s not a store. Keep the things you love and part with the rest. We live in a time where you can literally have anything you need within 24 hours... having 14 bottles of shampoo clog up your closet is unnecessary and very hard to keep neat. How did your company come about? I have always been a neat freak. My favorite activity is cleaning out my closet, basement and pantry. When my second child was 18 months old I had a strong itch to go back to work, but felt compelled to only leave my kids to do something I truly loved. When I realized being a professional organizer was a real thing I was amazed at my luck! I am so fortunate to spend my days doing something that does not feel like work to me!

BREAKING THE MYTH

CISCA FOUNDER FRANK BALLESTE BREAKS DOWN THE DO’S AND DON’TS OF HIRING A CONTRACTOR BY ANDREA CARNEIRO

W

ITH MORE THAN TWO DECADES OF EXPERIENCE IN THE CONSTRUCTION

business, Cisca Construction Founder and Owner Frank Balleste has seen his fair share of contracting disasters. In fact, some of Balleste’s clients have landed at Cisca’s door (often via referral) after wading through years of building or remodeling work with other vendors. “We worked on a house in Pinecrest that had gone through four contractors in their bathroom,” he says. “It was $40,000 of horrific work. We came in and cleaned up the mess in two weeks.” And though bathrooms are just a small part of the Cisca empire (they handle everything from commercial to residential construction), Balleste is more than happy to share his knowledge of a business he loves dearly. “I love this business, I cherish it,” he says with a laugh. “I think about it every five seconds of the day.” Here are Balleste’s tips on how to hire a reputable contractor.

DO

Check their background. Hiring an unlicensed contractor leaves homeowners little recourse should anything go wrong on their project, making a licensed contractor a must in protecting your home and finances. “Know the difference between a contractor’s license and a business license,” says Balleste. “A lot of people say they are contractors but most are just building contractors, we’re statecertified general contractors.” Doing a quick search can also help clients see if potential contractors have had a history of disputes with clients or subcontractors.

DON'T

Rely on results. Work off of referrals when you can, says Balleste, but be sure to use your own eyes (and judgment) before you make a hire. “Talk to friends and relatives, but do a drive-by of a current project,” he suggests. “See if they’re working safely and neatly and respecting the homeowner’s property.”

24

THE FLORIDA VILLAGER

DO

Know what you want. Come to the table with an idea and a plan, says Balleste. Starting the process by interviewing contractors may not only confuse you, but also cost you money in the long run. “If you have a strong idea of what work needs to be done and a good feel for the materials or function you need and want you’ll get a truer, more realistic estimate,” he says.”These days it’s very easy to educate yourself before jumping into a big project."

DON'T

Hire the cheapest guy. “Get rid of your lowest bid,” advises Balleste, who maintains that a low bid can mean they’re either desperate for the work or cutting corners. “I’m not the cheapest guy or the most expensive guy, but I’m the guy who’s going to do it right the first time,” he says. “Believe me, you would rather pay it once the right way.”

03.2017

DO

Get it writing. At Cisca, all client estimates go directly through Balleste, who says it’s important to know exactly what you’re getting into no matter what the scale of the project. “I have a set of project managers and superintendents,” he explains. “They measure and then they sit with the client to understand exactly what they want to do. We get all the information and create a plan. Our estimating team then sits down and plugs in numbers. I hate unknowns.”

DON'T

Rush into anything. Be sure to take your time and investigate all avenues, says Balleste. “It’s not just buying a coffee.” Once you’ve decided on someone be sure not to put down too much money right up front. In fact, Balleste says you shouldn’t part with more than ten percent of the job at the beginning of a renovation. “Incremental payments are completely fair to make sure no one goes running off with your money,” he explains. “You can even schedule the last payment for 30-or-so days after completion. It gives you a window to make sure nothing goes wrong and it’s completely reasonable to an experienced, professional contractor.”

DO

Understand the value of a connection. Building and renovation projects can be stressful, so it’s important to work with someone who not only understands your needs, but also matches up with your personality. “We treat each client like a family member,” says Balleste of Cisca’s outlook. “We sit down, we ask what your needs are. If anything comes up we’re there to bounce around ideas and come up with solutions. It’s one more piece of the puzzle I can give the client.”

What’s Happening in Our Communities? Jim Cason

MAYOR OF CORAL GABLES

F

OR THE PAST FEW YEARS, THE CITY OF CORAL GABLES has been working hard to reduce the instances of crime in the City Beautiful. Thanks to the new strategies that our Public Safety Team has implemented last year, Coral Gables has seen an overall 6.8 percent reduction in crime in 2016. Coral Gables has now the lowest incidents of reported vehicle and residential burglaries citywide in more than 15 years. We have also dramatically reduced traffic homicides and crashes. Last year, Coral Gables addressed the issue of speeding through our neighborhoods by giving more than 23,294 uniform traffic citations and written warnings in neighborhoods, school zones, and major roadways. So far, we have improved vehicle safety in our streets with a 60% reduction in traffic homicides, a 20 percent reduction in traffic crashes at five specific intersections along US1 and the Downtown area, and a 3 percent reduction in traffic crashes citywide. This good work continues every day. We would like for all of you to help us improve our public safety: If you see something, say something. Contact our Coral Gables Police or Fire Department non-emergency number at 305-442-1600 or dial 911 for any emergencies.

THE BENEFITS OF CRYOTHERAPY OPTIMAL RESULTS: 2-3 SESSIONS PER WEEK “I abuse my feet. Most mornings I run on concrete sidewalks, and then I run around work in heels (very high heels). The day after running in the last Miami Half Marathon, my legs, feet and hips were seriously aching. I tried cryotherapy and the very next morning, I felt so good, I was able to run pain-free! In fact, as long as I had a weekly session, my plantar fasciitis was gone. But driving far away for cryo was getting old, so I brought cryotherapy down to Pinecrest... And now we can all enjoy the benefits close to home. When you try it, you’ll understand why it’s so popular among not only professional athletes but also with regular people with aches and pains, like me. I’m sure you’ll enjoy it as much as I do.”

6 Tips for Facing the Flu BY DR. GUSTAVO FERRAR

W

ITH THE RECENT FLU EPIDEMIC SWEEPING OUR NATION AND THE closure of schools around the United States, we want to help you arm yourself against these nasty bugs! There are affordable OTC’s and easy steps you can take to help prevent and fight against these illnesses naturally, even before you get a sniffle.

We focus on “natural” in many of these tips because most over-the-counter (OTC) medicines are combined with other unnecessary medication. We tend to think that more is better so we go for the ones labeled “Max” or “Extra Strength” and those are the ones usually combined with medications such as acetaminophen (Tylenol). Too much acetaminophen can damage your liver. Every year over 80,000 people go to the emergency room due to overdose of acetaminophen and 25,000 get hospitalized. So, stay away from combination medications—you don’t need them.

»» Tip 1: Basic things first—consider the Flu vaccine and wash your hands frequently. »» Tip 2: Take a good blend of immune support herbs such as echinacea, goldenseal, astragalus, and andrographis, available in any health food store. The best one and my favorite is tropical sage, unfortunately it is not widely available. »» Tip 3: keep your nose clean moist. Use Xlear saline nasal spray with xylitol to wash away microbes right in the nasal passages and reduce bacterial adhesion! This one is a must. »» Tip 4: Probiotics, or “good bacteria”, help the immune system as well as the digestive system, so make sure you are taking those regularly. »» Tip 5: Drink ginger tea with lemon and honey. The best method is to simmer ginger slices to bring out the antibiotic effect. Add lemon and honey, each add to the cold and flu-prevention effect and makes it taste delicious. »» Tip 6: Garlic is also a strong antimicrobial and can be minced and sautéed to enhance a variety of meals. Or mince it and add it raw to salsa for a delicious and health-boosting treat. Working these six simple and easy to use tips into your regular health routine in the classroom, work, and at home will help prevent colds and flu, and keep you one step ahead this season.

My Dog was Diagnosed with Knee Problems: Now what?

S

O, YOUR DOG IS LIMPING FROM ONE OF THE BACK LEGS. You may have noticed limping on and off for a few months. Or perhaps, you have observed a sudden holding of the leg from the ground, just for a couple of days. Your veterinarian diagnosed a stifle problem, and you have been told that your pet needs surgery. You are not alone. Stifle conditions are one of the most common orthopedic problems seen in dogs. The stifle is a complex joint found midways in the back leg of your dog. It involves three bones: the femur, the patella (kneecap) and the tibia. This joint is equivalent to the human knee. Even though the stifles can be affected by a large number of conditions, by far the most common ones are patellar luxations and Anterior Cruciate Ligament (ACL) tears. There are a number of treatment options for these conditions, which keep expanding with advances in Veterinary Medicine. Treatment recommendations are not cookie-cutter, and must be formulated based on the individual physical characteristics of the dog, the lifestyle of the dog, and the specific stifle pathology of the joint. Treatment recommendations are also dependent on the level of expertise and knowledge of the doctor performing the evaluation and recommendation. Stifle orthopedic problems are one of the veterinary conditions that will produce the most confusion among dog owners, regarding its treatment. Often, owners seek second opinions for treatment options. For all cat owners, know that cats can also suffer from ACL tears and patellar luxations, even though not as commonly as dogs. Following is a short description of these two most common orthopedic problems of the dog’s knee. The orthopedic conditions of the stifle and the possible treatment approaches are complex and very patient-specific. The important point to remember is that, if your dog shows persistent lameness in one of the back legs, do not ignore it. Get a proper diagnosis by a licensed veterinarian as soon as possible. Conservative treatment should be initiated first (rest, anti-inflammatories, professional physical therapy, custom made orthopedic devices, etc). If your dog is not improving, then surgery should be considered. Remember that the surgical repair of these conditions is not an emergency that needs to be dealt with within one or two weeks. Surgery is not without complications, and when planning on it, an experienced veterinary orthopedic specialist is required. If your dog has problems on both stifles, avoid surgical repair of both limbs on the same day. It is best to have surgery on one leg at a time, because recovery is faster when there is a healthy leg to assist with strength and balance. Professional physical therapy, when provided by a veterinarian with accreditation in this field (Certified Canine Rehabilitation Practitioner, CCRP) will manage the stifle diseases without the need for surgical intervention, for certain patients. On the other hand, when surgery is required, it will maximize the level of recovery, and will shorten the recovery time. It is your responsibility to make the best medical decision for your canine best friend. Do not rush your decision for treatment. Always get second and third opinions, if you need to.

IV Infusions – The Next Big Thing? BY JOSE M. SOLER-BAILLO M.D., M.S, BOARD CERTIFIED PLASTIC SURGEON, SOUTH MIAMI

A

H, TO BE YOUNG AGAIN! THE QUICK REBOUND AFTER a night out, being able to go out night after night with no repercussions – the dream. While regaining a youthful appearance is something I specialize in as a plastic surgeon, I also recognize that people want to be able to enjoy their new body. While I will never say no to an invite out on the town, I know that I need to prepare properly and recover properly if I want to feel like myself again quickly. IV infusions with my own special “cocktails” work wonderfully to accomplish just that. In fact, many establishments all over the nation have recently sprung up offering just IV infusions. They will provide a menu of different blends each touting unique benefits. In general, they will all provide substantial increases in hydration as well as a welcome pick-me-up. Infusions can be targeted towards general wellbeing, immune defense, or even weight loss, depending on exactly what concoction you desire. After an IV infusion, you should expect to feel refreshed and even better than you did before your big night out. Although they are most commonly known as the “hangover cure” for young and spry twentysomethings, mature professionals can benefit just as much after a particularly exhausting work week or if they feel a sickness coming on and want to avoid it.

As the name implies, the benefits are administered intravenously. Moreover, the ingredients in these cocktails can be relatively potent. Ingredients include corticosteroids, amino acids which affect metabolism, as well as factors that can affect blood alkalinity. They are almost always well-tolerated and legitimately produce positive effects in the body, but only a medical professional should deliver them. Because of this, I generally advise patients to seek IV infusions once a week, max – even once a month if steroidal ingredients are infused. Too much of a good thing will always lead to trouble, and trained medical staff can help to avoid this or spot when someone undergoing IV infusions has reached that point. Even just applying an IV requires a certain technique. It’s not uncommon to hear complaints of a friend leaving the hospital of the nurse who “kept stabbing me and couldn’t find the vein!” If it’s hard for them, then you had better not leave it to Joe Blow in the IV wagon. If you’re looking for the next medical trend, you’ve found it. IV infusions can produce rapid changes in the body for recovery or prevention of sickness. They work – but trust your body to those trained to care for it. So, what’s the harm in partying hard, especially if you don’t have to deal with the hangover the next. Get out there. Live a little!
