Dear FXW Families, We hope you had a wonderful, relaxing summer filled with great fun and happy memories. Our teams were busy this summer preparing for another successful school year. You may see some new faces on both campuses this year (and some familiar faces in new places!), so we hope you will join us in welcoming everyone into our community. We are excited to jump into the first, full day of school for KindergartenGrade 8 on Tuesday, September 3. For Preschool families, please see page 18 for the first day of school schedule. In lieu of FunFest, we will be hosting a Homecoming celebration from September 5 through September 8. Please see page 11 for more details. As we celebrate our 30th Anniversary, the upcoming year will be filled with many opportunities to help commemorate this important milestone for our School. In addition to the information included in this packet, please note the following important announcements: 1. Teacher and classroom assignments will be available to view in the myFXW parent portal. We will send an e-blast when the assignments are live. 2. Registration for Fall Co-Curricular Programs at the Old St. Patrick’s Campus (OSP) and the Holy Name Cathedral Campus (HNC) started August 1 and goes through September 5. Registration for Athletics at OSP and HNC is open through Friday, August 9. 3. We will continue sending In the Loop (FXW e-newsletter) every Sunday this year beginning on Sunday, August 18. If you do not receive In the Loop, please contact communications@fxw.org. Throughout the year, there will be many opportunities to learn more about daily activities and upcoming events via social media. Below is information on our social media handles. NEW LEADERSHIP AT FXW Courtney Britton—Principal, OSP Courtney began her career at FXW in August 2009 as a First Grade teacher. Over the course of her tenure at FXW she has worked in several different capacities, including classroom teacher, literacy teacher, and assistant principal. Courtney is a graduate of the University of Michigan where she earned her B.A. She also holds two M.A. degrees in Elementary Education and in Educational Leadership, both from Loyola University Chicago. Autumn Perry—Assistant Principal, OSP Autumn has been teaching at FXW since 2008. She taught preschool, music and, most recently, Kindergarten. Autumn received her M.A. in Curriculum and Instruction from Concordia University, is SLANT certified, and is working towards her M.A. in Educational Leadership at Loyola University Chicago. She received her B.A. in Music Education and Church Music from Olivet Nazarene University. Lauren Fitchett—Principal, HNC Lauren has been at FXW since 2016 as a Fourth and Fifth Grade teacher. Before joining FXW, Lauren was the Assistant Principal at Agassiz Elementary School, and prior to that spent 11 years at Andrew Jackson Language Academy, both in Chicago. Lauren earned her B.A. from the University of Illinois, Urbana–Champaign and a M.A. in Curriculum and Instruction from National-Louis University. Norma Guzmán—Assistant Principal, HNC (Student Life) Norma joins FXW this year as Assistant Principal. Norma was a Principal, Assistant Principal, and teacher at several Catholic and charter schools for 12 years. She comes to us from Swedish Covenant Hospital, where she has been serving as the Director of the Child Care Center since 2014. Norma has a M.A. of Education in K-12 Curriculum and Instruction and a PhD of Education in Educational Leadership, both from National Louis University. Brad Mallers—Assistant Principal, HNC (Academics) Brad has been teaching since 2010 and has been with FXW since 2015, most recently as a 6th Grade Math teacher and Grade Level Chair. Brad is completing his M.A. in Educational Leadership from DePaul University and received his B.A. from Butler University in Early and Middle Childhood Education.

BACK TO SCHOOL NIGHT & OPEN HOUSES Back to School Night & Social and Open Houses are our first opportunity of the new school year to establish a partnership between teachers and parents. Teachers will share milestones as well as what to expect with a global overview. We will host a curriculum night later in the year to showcase student work that connects with curriculum specific to grade levels.

BACK TO SCHOOL NIGHT & SOCIAL: PRESCHOOL & KINDERGARTEN Wednesday, August 28, 6-8 p.m. at the Old St. Patrick’s Campus This parent-only event is an opportunity to meet your child(ren)’s teacher and learn more about our philosophy, routines, and parent partnership. A social will follow. OPEN HOUSES New this year, we will be hosting an Open House at each campus. On Late Start Wednesday Days ONLY, staff will be present for drop-off from 8:20-8:30 a.m. Before school care is available from 7:00-8:20 a.m. on Wednesdays.

ATHLETICS FXW’s Athletic Program is an integral part of our commitment to nurturing the whole child. We believe that physical activity promotes mental, physical, emotional, and spiritual wellbeing. The mission of FXW’s Athletic Program is to foster the growth of well-rounded, confident young adults in an age-appropriate manner. Our program emphasizes four key areas: • Social, emotional, and physical development through athletics • Meeting the individual needs of all students • Modeling a learning process that includes self-discipline, respect, success, and failure • Giving all students the opportunity to participate in athletics The Athletic Program, starting in Grade 2, offers students the opportunity to explore competitive sports in a safe way. Trained coaches emphasize sportsmanship, teamwork, integrity and self-discipline while commending individual effort and commitment. We also believe that activities on the playing fields and courts are an extension of the classroom. What you need to know to sign up your child/ren for athletics this year: 1. PRESCHOOL TIPS Tips for New Preschool Parents Welcome to FXW! Below are some insider tips shared by first-year preschool parents. DROPOFF/PICKUP LOGISTICS •

On Late Start Wednesdays, you can still drop your child off at the regular time and have them join before-school-care.

Check the In the Loop school newsletter to confirm preschool days-off because they sometimes differ from the big kids, especially around parent-teacher conferences.

If you ever use Uber to drop your kids off, the best place to get out is on Adams St west of Desplaines St (don’t drive in or in front of the car drop-off line).

If you have some time between drop-off and the start of a school meeting or activity, check out Big Shoulders Coffee (Jackson Blvd) or the Department of Coffee and Social Affairs (Madison St).

A fun park to walk to after pickup is Mary Bartelme Park.

ACTIVITIES •

The school masses generally do not include preschoolers. Instead, preschoolers have their own prayer services, which are adorable (you will probably want to make an effort to attend!).

Similarly, the preschool events and parties are really interactive and fun for parents to attend. Your room parents will reach out with more information.

Living the Legacy, FXW’s community service program, is a great way to get involved. Do not feel like you need to participate in every activity; instead, pick the preschool-appropriate ones like the Thanksgiving placement making event.

The OSP News at the end of the In the Loop school newsletters relate to the Old St. Patrick’s Church, NOT the FXW OSP school campus.

Start looking at summer camps in February! FXW has a great summer program (Camp Jaguar) and some teachers offer summer enrichment courses.

Feel free to coordinate with your room parents to plan parents/mothers/fathers nights out to get to know the other parents in your child’s class.

PARENTS AT THE HEART (PATH) MISSION Parents at the Heart (PATH) is a parent-run committee, which supports the mission of FXW by working with faculty, staff, and families to enhance and encourage spirituality through personal support and community-building events. PATH also provides an opportunity for children and parents to utilize their resources and talents to help others within the FXW community. PARTICIPATION We welcome all parents at both FXW campuses to participate in the many activities that PATH offers throughout the year. Parents can also volunteer to be a class representative or a general committee member. Sign ups will be available in the first In the Loop. PATH representatives work with the teachers in their child’s class to organize support to families during times of illness, death, births and any life-altering event where the family could use a Good Samaritan. All PATH representatives and committee members are invited to meetings throughout the year and receive information about volunteer opportunities for PATH events.

There are many opportunities for volunteer participation: serve on a planning sub-committee, assist with administrative tasks in the Advancement Office, work on data entry related to the auction and to attendance, sell raffle tickets, assist with day-of events, including set-up, registration, tear down and clean up, and post-event assistance with auction item distribution. CHILDREN AT THE CROSSROADS FOUNDATION (CATC) Children at the Crossroads Foundation (CATC) was created in 1990 on the belief that economic circumstances should not preclude any eager and capable child from receiving a quality education. The Foundation seeks to enrich our community by significantly improving the academic achievement, self-esteem, and life opportunities of school children by offering need-based scholarship support to students attending FXW. Over the last 29 years, CATC has awarded over $15 million in needbased scholarships to nearly 700 FXW students. However, all FXW students—whether they receive a scholarship or not—benefit from being a member of this special community filled with a diversity of backgrounds, perspectives, and experiences. The FXW Advancement Department organizes many events and initiatives through the school year that benefit CATC, including:

SOCIAL EMOTIONAL DEVELOPMENT AND TECHNOLOGY (SET) PROGRAM Dear Families, We look forward to seeing you in a few weeks for the start of school. As you look to September, you are invited to learn more about our Social Emotional Development and Technology (SET) Program. SET is an outreach program that brings discussions and resources to our FXW families through presentations, grade-level coffee talks, book discussions, and a resource library. We encourage you to participate in these talks and presentations over the course of the year. The SET Program, now in its fourth year, is the collaboration of FXW’s Director of Innovation, Library and Media Specialist, Social Worker, and Psychologists. The program represents our commitment to the charisms by engaging the FXW community to foster balanced technology practices that support optimal learning, healthy relationships, and emotional well-being. FXW SET events for the 2019-20 school year will focus on: •

Helping students develop a healthy and respectful approach to relationships in both the physical and digital spaces we use, as we interact and communicate with each other.

Providing opportunities that invite ongoing discussions between school and home, between teachers and parents, and between teachers and students.

Offering a teen panel discussion and grade-level specific parent coffees. Specifically, we would like to invite you to our Maggie Daley Speaker Series Event with Rosalind Wiseman on October 22, 2019 at 7 p.m. Look to In The Loop throughout the year for these dates and mark your calendars to join us!

Please take time to check out all the information on the SET Program page on the FXW website. Additionally, we encourage families to speak with their children about internet rules, expectations, and boundaries at home, particularly since 5th-8th graders will be bringing devices home. Our SET Program resource library, located on the FXW website, includes sample device contracts and a customizable family media contract. These contracts are good starting points for family discussions about device use, internet use, and participation in social media sites and apps. VOLUNTEER REQUIREMENTS Anyone interested in volunteering for FXW must comply with the Archdiocese of Chicago Protecting God’s Children requirements for volunteers. This includes attending Virtus Training, signing the Code of Conduct form, registering for CANTS (Child Abuse and Neglect Tracking System), and undergoing a background check. The requirements for volunteering apply to individuals who chaperone field trips, coach, moderate a club, assist in the lunchroom, gym or classroom, or participate in other volunteer opportunities. Anyone who wants to volunteer (parents, guardians, family members, friends, community members) are required to comply as follows: REQUIREMENTS FOR ALL NEW PARENTS, GUARDIANS, AND CAREGIVERS: 1. VIRTUS TRAINING Virtus Training is offered several times a month at various locations. Upcoming and available sessions are posted at virtusonline.org. Go to “First Time Registrant,” “View a List of Sessions,” and select “Chicago, IL (Archdiocese)” as the organization. Sessions can be found in both English and Spanish. You must register online for a session that works for you. 2. BACKGROUND CHECK (ONE TIME ONLY) The Background Check is completed online by visiting the Archdiocese of Chicago’s page here. Create a User ID and password, and follow the website prompts as directed. ALL PARENTS, GUARDIANS, AND CAREGIVERS MUST COMPLETE THE TWO FORMS BELOW ANNUALLY: 1. CANTS (Child Abuse and Neglect Tracking Form) The Child Abuse and Neglect Tracking System Form (pg 32) is included in this Welcome Kit. This single page must be completed and returned to school. Please do not mail or e-mail to DCFS. Please return this form to the front office. 2. CODE OF CONDUCT The Volunteer Code of Conduct (pgs 28-30) is also included with this Welcome Kit. The acknowledgement form (pg 31) must be completed and returned to school. Please return this form to the front office. VOLUNTEER REQUIREMENTS Code of Conduct for Volunteer Personnel As someone who volunteers with young people and/or vulnerable adults, I will maintain a professional role and be mindful of the trust and power I possess as a volunteer to young people and vulnerable adults. To achieve this, I WILL NOT: Touch a minor or vulnerable adult in a sexual way or other inappropriate manner. Be alone with a minor or vulnerable adult in a residence, rectory, sleeping facility, or any other closed room. Share a bed with a minor or vulnerable adult. Take an overnight trip alone with a minor or vulnerable adult. Acquire, possess or distribute pornographic images of minors under the age of 18. Introduce sexually explicit or pornographic topics, vocabulary, music, recordings, films, games, websites, computer software or entertainment to a minor or vulnerable adult. Provide alcohol, cigarettes or controlled substances to a minor or vulnerable adult. Use, possess, or be under the influence of illegal drugs. Use alcohol when engaged in ministering to a minor or vulnerable adult. Engage in physical discipline for behavior management of minors or vulnerable adults. Humiliate, ridicule, bully, or degrade another person.

Measures to Aid Observance of the Code of Conduct To help me keep the promises in the Code, I WILL: Report any suspected child abuse or abuse of a vulnerable adult to the proper authorities. Avoid physical contact when alone with a minor or vulnerable adult. Physical contact with minors or vulnerable adults can be misconstrued, especially in private settings. Avoid overnight stays with a minor or vulnerable adult unless there is another adult present in a supervisory role. Avoid providing overnight accommodations for minors or vulnerable adults in private residences or rectories. 28


VOLUNTEER REQUIREMENTS Avoid driving alone in a vehicle with a minor or vulnerable adult. Assume the full burden for setting and maintaining clear, appropriate physical and emotional boundaries in all ministerial relationships. Refrain from giving expensive or inappropriate gifts to a minor or vulnerable adult. Avoid meeting privately with minors or vulnerable adults in rooms, offices, or similar areas where there is no window or where the door cannot remain open. If oneon-one pastoral care of a minor or vulnerable adult is needed (e.g. Sacrament of Reconciliation) avoid meeting in isolated locations. Exercise caution in communicating through e-mails or the internet. Only share work/ ministry related e-mail addresses with minors and vulnerable adults. Do not participate in chat rooms with minors or vulnerable adults. Ensure that all activities (extra-curricular, catechetical, youth ministry, scouting, athletics etc.) for which you are responsible have been approved in advance by the appropriate administrator. Have an adequate number of adults present at events. A minimum of 2 adults in supervisory roles must always be present during activities for minors and vulnerable adults. Release young people only to parents or guardians, unless the parent or guardian has provided permission allowing release to another adult. Avoid taking minors and vulnerable adults away from the parish, school, or agency for field trips, etc. without another adult present in a supervisory role. Obtain written parental/guardian permission before such activities. Permission slips should include the type, locations, dates, and times of the activity and emergency contact numbers.

VOLUNTEER REQUIREMENTS Practical Suggestions These are some practical suggestions for identifying permissible and impermissible conduct.

Conduct that May Be Permissible Appropriate affection between Church personnel and minors and vulnerable adults constitutes a positive part of Church life and ministry. Nonetheless, any touching can be misunderstood and must be considered with great discretion. Depending on the circumstances, the following forms of affection are customarily (but not always) regarded as appropriate. • Verbal praise

• Sitting beside small children

• Handshakes

• Kneeling or bending down for hugs from small children

• “High-fives” • Pats on the shoulder or back • Hugs (brief)

• Holding hands during prayer • Pats on the head when culturally appropriate

• Holding hands while walking with small children

Conduct that is NOT Permissible Some forms of physical affection have been used by adults to initiate inappropriate contact with minors. In order to maintain the safest possible environment for minors and vulnerable adults, the following are examples of affection that are NOT TO BE USED: • Inappropriate or lengthy embraces • Kisses on the mouth • Holding minors over four years old on the lap • Touching buttocks, chest, legs or genital areas • Showing affection in isolated areas such as bedrooms, closets, staff-only areas or other private rooms • Wrestling or tickling minors or vulnerable adults • Piggyback rides • Any type of massage given by minor to adult, or by adult to minor. • Any form of unwanted affection • Compliments that relate to physique or body development 30

In this guide, you will find important information about our myFXW parent portal, powered by Veracross; our Learning Management System, Canvas (Grades 5-8); and our electronic medical records system, Magnus Health. myFXW PARENT PORTAL myFXW Parent Portal (Powered by Veracross) Our myFXW portal has up-to-date information on school events and activities, family and staff directories, tuition invoices and payments, links to school systems such as Magnus Health and Canvas, fundraising events and donation information, online registrations for cocurricular programs and athletics and important forms and documents. To access the portal to go to https://www.fxw.org and click on myFXW at the top.

Fill in the fields for your username and password. For forgotten passwords, click the Can’t access your account? link on the login page for assistance.

Once logged in, you should see the parent portal Dashboard. Become familiar with the navigation options.

myFXW PARENT PORTAL DASHBOARD The parent portal Dashboard includes a list of your children, the week’s calendar, and quick links to school resources. Below are some quick pointers for navigating the Dashboard. 1. Classes and Reports is where you’ll find your children’s classroom blogs in Preschool4th Grade. 2. Canvas is where you’ll find classroom announcements and assignments in 5th-8th grades. 3. My Calendar contains all events pertinent to your household. 4. School Calendar (under the Calendars dropdown in the navigation menu) lists all school events. 5. Billing is where you can view and pay invoices as well as manage your AutoPay account info.

HOUSEHOLD PROFILE UPDATE Click the Update Family Profile link. Once at the Household Profile Update page, review all sections for accuracy. At the beginning of each school year or if you have updates to your family information during the year, go to the My Information button and make the necessary updates. All families must verify their information before the beginning of the school year.

DIRECTORIES Families can search our family directories by criteria or see lists by homeroom and grade level.

myFXW PARENT PORTAL Canvas Canvas is our Learning Management System used for students in Grades 5-8 only. Teachers for these grades will post daily homework assignments in Announcements. Homework will not be posted after 4:00 p.m. every week night, and students will have assignments written in their assignment notebook. Please note parents and students will not receive a notification about homework unless they go into Notifications in Canvas and choose to have an update sent (for directions see Setting Up Notification Preferences in Canvas on the next page). To access FXW’s Canvas Site on the web, go to www.fxw.org/login and click the Canvas icon. ACCESSING CANVAS ON A LAPTOP OR DESKTOP The following information will be helpful to you when you access Canvas on a laptop or desktop computer. As of June 2019, Canvas supports the following desktop browsers and Flash versions: • Internet Explorer 11 (functional support) • Edge 42 and 44 • Chrome 73 and 74 • Safari 11 and 12 • Firefox 65 and 66 • Flash 31 and 32 (for recording or viewing audio/video and uploading files) The most up-to-date information on supported browsers can be always be found at https://goo.gl/jqtAzY *Some supported browsers may still produce a banner stating, “your browser does not meet the requirements for Canvas.” If you have upgraded your browser but you are still seeing the warning banner, try logging out of Canvas and deleting your browser cookies. CANVAS MOBILE APPS We do not recommend the use of the Canvas iOS or Android apps as they provide very limited visibility. Canvas is best experienced on the web using a PC or Mac. LOGGING INTO CANVAS THROUGH A WEB BROWSER Go to www.fxw.org/login and click the Canvas icon. You should be automatically signed into Canvas, if not, a log in box will appear (see right). Once logged-in, select “Courses” in the global navigation bar. Choose from the list 36

2. Add your desired way(s) to be notified.

3. Click on the Notifications tab on the left-hand navigation bar.

4. Set the frequency of your notifications by clicking on your chosen tab by Announcements and Grades. For example, to get a daily summary of homework postings, choice the daily option to the side of Announcements. Note, daily summaries are sent out by Canvas anytime between 5:006:00 p.m. You may modify these settings at any time.

myFXW PARENT PORTAL SETTING UP NOTIFICATION PREFERENCES IN CANVAS By default, Canvas will not send email notifications. To receive notifications from Canvas, you will have to opt in. Here’s how to set up these notifications to your liking: 1. These are your preferences. You may select how you want to receive announcements (email or text message), and how often you want to receive them. If you do not change anything, you will not receive notifications. 2. While we understand that you will select the preferences that best meets your needs, we do recommend using the following settings: • Leave all notification preferences except for Announcements set to Do not send me anything. • Set Announcements to Daily Summary (one notification sent between 5:00-6:00 p.m. each evening that is a summary of all announcements posted that day) or to Notify me right away (you will receive a notification each time an announcement is posted—this could be several times during the day). • Set Grading to your personal preference. 3. Your setting notifications will apply to all of the courses your children are enrolled in. It is not currently possible to set up different notification levels for each class or for each student. 4. myFXW PARENT PORTAL Magnus Health Magnus Health is a web-based electronic medical records system providing FXW with secure storage and up-to-date student health information. You will use Magnus Health to submit required medical forms and report information on your student’s allergies and medications. LOGGING IN TO MAGNUS HEALTH 1. Your myFXW username and password are the same for Magnus Health. 2. To log into Magnus Health after setting up your myFXW account, login to myFXW at https:// www.fxw.org/login and click the Magnus Health icon. You will be directed to the FXW specific Magnus Health login page. Use your myFXW username and password to log in. 3. Once into Magnus Health, you will see each of your students listed on the Front Desk.

4. Clicking on the Complete now tab for each student will bring you to the student health tracker page.

The Student Health Tracker page has a list of all medical documents you must provide by the specified deadline. For your reference, this list is also included below. Please read about each requirement carefully as some documents will require a physician’s signature. 39

To return these documents you have three options—choose whichever is most convenient for you: 1. You can mail them to Magnus 2. You can fax them to Magnus 3. We view food service as an integrated part of the educational experience. We aim to provide our communities not only with fresh meals and snacks they look forward to eating every day, but also with information around nutrition, cooking techniques, sustainability, and culture.

OUR APPROACH Responsible Sourcing The quality of our meals stems from the quality of our ingredients. We begin with whole, fresh foods sourced from Midwestern farms and other producers we trust. We’ve built strong relationships with our providers—ranchers who practice ethical raising, farmers who grow and harvest naturally, and millers who minimally process their grains. Sourcing locally and seasonally reduces our ecological footprint by minimizing our “food miles”—the distance traveled from harvest to plate.

From-Scratch Cooking Our chefs craft each dish using classic cooking techniques, whether they’re simmering fresh chicken stock, braising beef, sautéing seasonal vegetables, or kneading bread. Though most of our meals and snacks are made and consumed within the same day, we also extend the seasons through preservation programs that include pickling, fermenting, jamming, and dehydrating. Cooking from scratch allows our chefs total control over our food’s flavor, appearance, allergen content, and nutritional profile.

Community-Driven Programming With exceptional food and warm hospitality as our baseline, we design our offerings around the unique service requirements and educational goals of each community. Outside of the cafeteria, we love to partner with students and teachers on curriculum tie-ins related to food and nutrition. We value all ideas and welcome ongoing feedback through online surveys, focus groups, and visits to the cafeteria.

OUR COMMISSARY Our goal is to serve our handmade food as close to the time it was prepared as possible. We are excited to partner with The Frances Xavier Warde School and to set up shop in Holy Name Cathedral Campus's kitchen, where we will cook meals for both school campuses daily. For additional support, we will make use of our custombuilt, 5,000-square-foot commissary kitchen (we call it “Base Camp”) in Chicago’s Bucktown neighborhood. Customers and partners are always welcome to stop by for a tasting and to see our chefs in action!

OUR TEAM Our people are the key to our success. The HandCut Foods team holds a wealth of experience spanning fine dining, corporate food service, event planning and catering, food manufacturing, dietetics, and education. We invest in our employees, spending time to develop their technical skills, creativity, and overarching passion for responsible food.
