Applications open for scholarship program Application is open to male and female athletes aged between 13 – 18 in 2015, who live in the Southern Sports Academy region, and who have been selected for State (NSW only) or National level representation in their chosen sport for the Southern Sports Academy Individual Athletes Scholarship Program (IAP). Scholarships will be offered based on recent performances, ranking and recommendations from respective state sporting organisations. Applicants will be assessed by the Southern Sports Academy IAP selection panel. Personal interviews may be called for. IAP athletes receive individual cash grants and access to the Southern Sports Academy sporting consultants. The IAP scholarship is designed to assist athletes to meet costs associated with training and competing at state or National level, as well as provide relevant information from our range of consultants, that can contribute to improved performance. Athletes selected in the program are required to: maintain monthly communication with the Southern Sports Academy office; attend the induction; attend workshops with the range of Southern Sports Academy consultants; Fulfill a community based component of their scholarship, which involves work in

a voluntary capacity within their local communities eg. Coaching at junior level, involvement in sport club working bees etc. Avail themselves for media opportunities; Attend the Southern Sports Academy Graduation Night. The Athlete Benefits from the scholarship include: z Financial support to the value of $750 to assist with expenses incurred in gaining access to higher quality competition, training, facility access, gym fees etc. z Southern Sports Academy uniform including a full tracksuit and polo shirt z One ticket to the Academy Graduation Night z Marketing opportunities. Warialda hosts NSW showjumping champs Pony club showjumpers from around NSW and Queensland will converge on Warialda for the 2015 Pony Club NSW State Showjumping Championships. The Championship event is being held at Warialda Showground at the beginning of this month and will showcase riders between the ages of 10 and 25 years battling it out for the title of Pony Club State Showjumping Age Champion. Around 130 riders have entered in the State Championship, with competitors from as far as Burra District Pony Club in Urila, NSW making the 10 hour drive with their horses. The youngest rider to compete over the weekend, Lucy McLaughlin from Nyngan Pony Club will be hoping for some birthday luck as she celebrates her 10th Birthday on the first day of competition. Lucy’s mother Abigail McLaughlin said “Lucy has been going on and on about having to celebrate her birthday at State Showjumping because it happens to her nearly every year,” she said. “This is the first year she will be competing which should hopefully make it a lot more fun”. Lucy’s older sister Alexandra won the Under 13 years Age Champion at the 2014 PCA NSW State Showjumping Championships in Condobolin last year riding the family’s

home bred Thoroughbred cross Australian Stockhorse, Jimmy McElroy. “Jimmy is a distinctive 16 hand-high grey with the most exceptional temperament, and this year Alex has given him to Lucy to ride at the championship,” Mrs McLaughlin said. “We will probably have a cake around the campfire at Warialda to celebrate Lucy turning 10, I hope that Jimmy takes her around safely and that she comes back with a smile on her face.” Pony Clubs in Zone 8 will be hosting the Championship and have 27 riders of their own competing from Garah-Boomi,

Warialda, North Star, Moree and Gravesend Pony Clubs. The Zone 8 Chief Instructor, Julie Morgan said preparation of the team had been a work in progress for many months starting with the regional showjumping school held in early March. “Then the first round of our zone showjumping competition was held at Moree and continuing on with Zone 8 riders making up our Area 3 showjumping team to compete at this year’s Sydney Royal and running a very close second,” she said. Competitors and their families will camp at the grounds for the duration of the championship. The competition will commence on Saturday from 8:30am and conclude Sunday afternoon with the mounted presentation ceremony. Entry into the cChampionship is free to the public and Julie Morgan encouraged locals to see what it’s all about. “Spectators can expect to see some of the best youth riders from around the state showcase their horses with high quality showjumping,” she said. The next state event is the NSW Mounted Games Championship to be held at Bungendore from the 10-11th October 2015. Vaccinating against strangles es, whatever their age, husbandry, usage, and management may have been. There is no doubt that the disease is extremely contagious, and certainly was very much a serious health risk to animals that become affected, and, inevitably, the great majority of horses in any group would eventually become infected. Today, it is relatively rare to diagnose the condition and even when it happens, seemingly many horses in a group remain healthy. Either the bacterium that causes the disease has changed, or, much more likely, we have seen a development of relative immunity among the general horse population. What I suspect has changed, although I have no solid supporting facts or information, is the makeup of the strangles vaccine. Strangles vaccination has always been available, but the earlier versions that we had seemed relatively ineffective in stimulating immunity in the vaccinated animal, and even then, any immunity that developed was relatively short lived. Worse still, the effects of the vaccine on the animal were major and some started to think the potential “cure” was almost worse than the disease. In particular, a large number of animals had significant reactions

Strangles is one of the oldest and most contagious diseases affecting horses. Thirty years ago, strangles was a very common problem throughout the horse population and was feared by horse owners. Incredibly, today it is a relatively rare occurrence to see the disease, particularly in outbreak form, in the pleasure and performance industry. Make no mistake, the disease is still very much alive and well and it remains a particular problem, particularly in the larger breeding farms where it commonly affects foals and younger horses that have a reduced or compromised immune system. The very nature of the operational necessities of these establishments means that there is an inevitable mass movement of mares and foals between farms, agistments, and owners and so the probability of dissemination of the diseased through the wider horse community is ever present. The incredible relative lower incidence of the disease these days in the general horse population amazes me. In the 70s and 80s, we used to see the disease almost weekly and it seemed to affect all classes of hors-

Strangles is highly contagious outcome; however, I suspect they are relatively near the mark. My belief is that most foals and young horse owners are fairly diligent in their vaccination protocols as everyone loves foals and young horses need a lot of maintenance and attention. Also, new owners almost always will vaccinate their horses when they acquire a horse so effectively they receive a booster. After that, vaccinations often become more sporadic. What are our recommendations? Unless there is solid evidence that any particular has very definitely received a proper priming course of vaccine, we feel that all horses should receive a proper priming course. That means either two doses of tetanus toxoid, four weeks apart, or more ideally, three doses of “Two in One” at two weekly intervals. This ensures that your new horse has received proper stimulation. Obviously all foals would fall into this category, but remember foals should be 12 weeks old before the course is commenced. (This is because of the effect of the mare’s colostrums interfering with the vaccine and the foal’s immune system being “old” enough to react properly). All horses should then receive a booster 12 months after the priming course. Certainly for tetanus, this seems to provide a very good and seemingly long lasting immunity. Seemingly, now, one would have to suspect that the strangles immunity is now also fairly good as evidence by the low incidence. Complacency is very dangerous and if the horse industry in general started to “forget” to booster, I suspect that certainly strangles may eventually regain a foothold. I believe all horses should receive a booster every 12 months, certainly up until they are 10 to 15 years old. Most horses are valuable to their owners and the cost of a booster is so small compared to the serious consequences if their animal caught the disease. Breeding animals should certainly receive more frequent strangles boosters, and all pregnant mares should be bolstered approximately six weeks prior to foaling. Next month, I will describe the actual disease in more detail.

to the disease and would become extremely stiff, sore, and lethargic. The vaccine available today is relatively innocuous in terms of apparent side effects on the horse, and seemingly, would have to be given a large amount of credit for the relatively low incidence of the disease today. This would suggest that the formulation and make up of today’s vaccine has become far more effective in stimulating a long lasting resistance to the disease. Somehow, someway, there appears to have been a development of an innate “herd” resistance to strangles in the general population of horses, relative to 30 or 40 years ago. Most owners are acutely aware of tetanus and strangles and almost always diligently administer vaccines to their horses, particularly in the new phases of ownership. Unfortunately, the vaccine is now quite expensive, and human nature being what it is, the frequency of repeat vaccinations often dwindles. Still the disease remains relatively rare. Tetanus in particular, is extremely rare. The vaccine has always been very effective in protecting the animal. Most horses receive a reasonable course of vaccine as foals, and then for whatever reasons, most will receive tetanus boosters along the way, for example when colts are gelded or when they sustain some wound, and it would appear that this is enough to maintain some protection. (Please be assured I am not advocating no need to booster tetanus. Far from it as tetanus is deadly and I believe all horses should receive regular boosters). Strangles vaccine has always been a little different. Initially, horses needed to receive three doses of the vaccine, at two week intervals. This made it a bit more bother for owners and so compliance was not always great, and in older days, many vaccinated horses still managed to contract the disease, and so there developed an indifference to the vaccine. Fortunately nowadays, the manufacturers have formulated a combined “Two In One” vaccine that does both strangles and tetanus in the one syringe. Even more fortuitously, they have been able to make the cost of “Two in One” almost the same as either of the individual singular vaccines, so naturally the tendency is to use the combined vaccine. So, where are we with vaccination protocols? Reports from the manufacturer’s surveys of the industry suggest that the uptake of vaccine relative to the size of the horse industry is a little low. These surveys may well be a little biased and slanted towards a particular

Equine nutritional therapist and medical herbalist It is absolutely essential that you seek professional advice before using any herbs as a remedy for pain relief. I prefer to use a group of ingredients to achieve a positive result rather as generally herbs or other nutrients on their own are not very effective. Bad temperament which is just behavioral is not common in horses but can be caused by pain. General pain in horses is very common and can also cause tension. Pain is debilitating and is undoubtedly the most common problem to go unnoticed and can also interfere with performance. Finding a way to manage any of the symptoms associated with pain or to treat a complaint is achievable with the use of many complimentary medicines and treatments and may also include conventional medicine. When we suffer any form of pain or discomfort we would normally seek some type of remedy for relief, whether it is drugs or complimentary treatments. The prevalence of pain in horses is more common than we think and it is essential for us to ensure that preventative measures are put in place. Always make sure that you seek professional advice when thinking about using any of the following herbs as part of your treatment. Remember just because it is a herb does not mean it is safe to use. The occurrence of pain takes place as a result of the activation of at least two major neurological networks in the mind/ body. Pain is an extremely complex experience given the complex association within and between these neurological networks and between the pain-sensing systems and other psychoneuro endocrine networks. A number of botanical medicines have proven effective for relieving different forms of pain observed and in clinical trials. The methods of action of many anodyne (analgesic) herbs are not always understood, though some have been well defined. Basic Neurobiology of Pain The more ancient slow pain system begins

with unmyelinated type C fibres in the skin or other tissues, C fibres are found in the peripheral nerves of the somatic sensory system. The C fibres synapse in the spinal cord, often first with small inter neurons then with second order neurons that carry the incoming pain signal to various brainstem nuclei. The signal is then relayed, usually by multiple short neurons, to the thalamus and only indirectly to the cerebral cortex. The recently evolved fast pain system begins with thinly myelinated A-delta fibres. These synapse in the spinal cord and the second order neurons ascend through the spinal cord to the thalamus. Third order neurons transfer the information to the cerebral cortex. Visceral pain (i.e. pain from any of the large interior organs) is sensed only by the slowtype pain system, while pain from the skin can be sensed by fast- and slow-type pain systems. Both slow and fast pain can be sensed without the cerebral cortex, though the cortex clearly plays a role in integrating and modulating various pain signals. Pain is often referred from one area to another. This is due to spinal neurons receiving input from the viscera and deep musculoskeletal structures and also receiving input from the skin. In addition, tissues with the same embryological origin may input signals to the same places in the spinal cord and inputs from one region often synapse at multiple levels in the spinal cord. Pain is also often poorly localised, particularly slow pain, possibly because of the multiple synapses in both pain networks' neuron

chains and the effects of simultaneous inputs from tactile sensation modulating pain signals. C fibres utilise substance P as their major neurotransmitter. Substance P is released from the distal terminus of a C fibre when it is activated. This in turn causes mast cells to release histamine, which triggers a signal to travel from the peripheral to the central nervous system. Substance P builds up and breaks down slowly. This phenomenon, combined with the unmyelinated nature of C fibres, explains the slowness of the slow pain network and why slow pain can linger even after the initial trigger is removed. A-delta fibres utilise aspartate, glutamate, substance P, and calcitonin gene-related product (CGRP) as their primary neurotransmitters. The myelination of A-delta fibres combined with faster neurotransmitters for the speed of the fast pain network. The mind/body has an endogenous analgesic system consisting of neurons in the spinal column, brain stem, and periaqueductal grey regions. These neurons produce endogenous opioid neurotransmitters such as enkephalins, endorphins and dynorphin which act on four subtypes of opioid receptors with a variety of effects, including the reduction of pain sensation. Serotonergic neurons in the brainstem are also believed to modulate endogenous opioid secretion. Finally, endogenous cannabinoids in the cerebral cortex and possibly other sites also modulate pain sensation and responses Botanical Analgesics Salicylate-containing Herbs Among the best researched and longest used botanical analgesics are plants containing salicylate glycosides, particularly Salix spp. (willow) bark, the bark of Populus tremuloides (quaking aspen) and other Populus species and Betula spp. (birch) bark. The example of willow will be discussed primarily here, as it has been best studied. The degree to which this information applies to other salicylate-rich herbs is unknown, though all have been used effectively with some patients.Willow and aspen are generally considered more specific to the musculoskeletal system while birch is more specific to the genitourinary tract. ‰ Continued page 15

Research proves vital with herb use ‰ From page 14 Though Salix alba is often considered the standard willow bark for clinical use, it actually contains relatively low salicylate levels. Much higher levels are present in S.daphnoides, S. purpurea and other species. Willow bark extracts standardised to provide 120-240 mg per day of salicin (often referred to as salicoside) have been the subject of several randomised clinical trials investigating their efficacy in patients with lumbalgia and various arthritides (arthritic conditions). A four-week, double-blind trial involving 210 patients with idiopathic chronic low back pain compared two doses of salicinstandardised willow bark extract with placebo. There was a dose-dependent superiority of willow bark over placebo in relieving low back pain. The higher dose showed moderately greater effectiveness compared to the lower dose, particularly as on average the higher dose relieved pain a full week earlier than the lower dose. The analgesic drug tramadol was used as a rescue pain-reliever significantly more often in the placebo group than in either willow bark group. There was only one case of allergy in this trial that could be attributed to willow bark. A follow-up study recently found that willow standardised extract (providing 120 mg per day of salicin) was cheaper than non-steroidal anti-inflammatory drug (NSAID) therapy with similar efficacy.

Willow bark standardised extract (equivalent to 240 mg per day of salicin) was approximately as expensive as the drugs. The authors noted that willow bark probably contains active constituents other than salicin and it may act by nonsalicylatemediated mechanisms. Exaggerated focus on any one constituent including salicin may ultimately crowd other important molecules out of willow bark extracts, actually reducing efficacy. A product combining 100 mg willow bark, 40 mg Guaiacum officinalis (Guaiacum) resin, 17 mg of a 7:1 extract of poplar bark (Populus spp.), 25 mg of a 4:1 extract of Smilax spp. (sarsaparilla) root/rhizome and 35 mg Cimicifuga racemosa (black cohosh) rhizome per tablet has been investigated in a two month, doubleblind, placebocontrolled trial for patients with osteoarthritis or rheumatoid arthritis. The dose was not clearly defined but two tablets were taken at a time. The herbal formula was superior to placebo at relieving arthritis pain, though the clinical significance of the change was marginal. Concomitant NSAID consumption did not change in either group during the trial. It is possible that an insufficient dose of the product was used. The author of a small doubleblind, placebo-controlled trial involving patients with osteoarthritis of the knee and/or hip joint concluded that the analgesic effect of willow bark extract could not be attributed to the salicin derivatives alone. As other constituents of willow

bark (such as flavonoids and other phenolic compounds) must contribute to the overall activity exaggerated focus on any one constituent including salicin may lead to reduced efficacy. Herbs containing salicylates may work primarily by interfering with production of bradykinin and cytokines during tissue damage and inflammation. These mediators are particularly likely to activate the slow pain network (fast type pain is not generally induced by tissue destruction). To some extent salicylate-containing herbs work by treating the cause of the pain and by interfering with pain transmission. These herbs are generally not as effective for psychogenic pain or pain not related to inflammation or tissue destruction. Salicylates may also have activity in the central nervous system to reduce pain sensation. Although the synthetic salicylate derivative acetylsalicylic acid (aspirin) clearly interferes with platelet aggregation and hence blood fluidity, natural salicylates appear not to have this effect to any clinically significant degree. Whilst there is little concern in combining salicylate-containing botanicals with anticoagulants or synthetic salicylates, patients on this therapy warrant supervision. Salicylate-containing herbs cannot be substituted for aspirin for prevention stroke or myocardial infarction. Salicylate sensitivity is an absolute contraindication to use of salicylatecontaining botanicals. Anti-inflammatory Analgesics Other anti-inflammatory botanicals may also reduce inflammation-associated pain. They may also have direct analgesic properties, though this has been poorly researched. Harpagophytum procumbens (devil's claw) tuber is the best researched of this group of botanicals. This native southern African plant contains, among many other compounds, a variety of bitter, anti-inflammatory and analgesic iridoid glycosides. Numerous uncontrolled and double-blind trials have shown devil's claw extracts to be effective for reducing the pain of osteoarthritis, lower back pain

and rheumatism. There is evidence that stomach acid degrades the activity of the devil's claw constituents, which might explain why some studies show devil's claw lacks efficacy. Devil's claw preparations should be administered between meals when gastric activity is at its lowest. However, more recent experiments with simulated stomach conditions found that harpagoside was stable. Devil's claw is particularly handy when the patient suffers dyspepsia, hypochlorhydria or other syndromes of impaired digestive function because of the herb's bitterness. However, these effects also make it contraindicated in acute diarrhoea, active peptic ulcers and hyperchlorhydria. Usual doses of fluid extracts/tincture (1:2-1:5) are 4-5 mL three times daily, though for more acute pain higher doses (up to 10 ml 4-5 times daily) may be required for 1-2 days. Tanacetum parthenium (feverfew) leaf is another established anti-inflammatory botanical. The major active compounds are believed to be sesquiterpene lactones (parthenolides) which have shown effects including inhibition of platelet aggregation, inhibition of phospholipase A2, spasmolytic activity and other effects in vitro and in animals. Several double-blind trials have proven that powdered crude or freeze-dried feverfew can prevent and/or reduce the severity of migraine headaches. A trial using feverfew extracted in 90% ethanol and granulated was not successful. Further trials are warranted but this form of preparation may not be efficacious. If fluid extracts or tinctures are used, an ethanol content lower than 90% is recommended. The one available double-blind trial on use of feverfew in rheumatoid arthritis patients was negative, casting uncertainty on the use of feverfew as an analgesic and anti-inflammatory in conditions other than migraine. Nevertheless, feverfew is frequently used for other conditions traditionally, and higher doses may be necessary for longer periods of time for best effects.

RDANSW conducts Dressage Development Program Riding for Disabled NSW Dressage offers registered RDANSW riders an opportunity to compete in Dressage competition. Each year the RDA holds a video competition and encourages RDA centres to host a protocol day and regional competitions in the lead up to the RDANSW State Dressage Championships. Riders who do not ride with an RDA centre however have a disability are encouraged to join RDANSW as an Associate Rider thus allowing them to participate in these events with their own horse and coach. Contact the RDA NSW office for more details. RDANSW Dressage Competitions are based on the five Para-Equestrian Classification grades. I A – Walk Only 1B – Walk and some trot 2 – Walk and trot tests 3 – Walk, trot and canter 4 – Walk, trot & canter with lateral work RDANSW centres or regions are encouraged to host a Dressage development day. These days are designed to encourage RDANSW riders and coaches to start Dressage or to provide opportunities for development of coaches and riders by having local experienced Dressage coaches or judges attend the centre to conduct a training day. Centres/regions will be provided with a starter pack with information including Dressage test plans, video examples, contact details for local Dressage coaches/judges and a Dressage mentor (from the Dressage committee) to help with organising the day. Ground work - a pre ride checklist By TANJA KRAUS There are a multitude of reasons to do groundwork with our horses and I do ground work with all horses that I work with - from an unstarted colt as part of the starting process, to my advanced clinic horse - yes I still do groundwork with him! As mentioned, there are a multitude of reasons for doing ground work with our horses and I thought I’d take a look at them over the course of a few different newsletters, and I thought I would start with the most obvious and probably the biggest reason people do ground work with their horses as a pre riding check. But we do need to take a look at how we do our groundskills before we ride and more importantly we need to know what we are looking for. It is not enough to simply run through our ground skills and expect that the horse is ready to ride - we must read the horse. Sometimes we may do five minutes of ground skills and realise that our horse is more than ready to get on, or we may do 20 minutes and realise he is not ready yet. And by ready, I mean not at a point that he is ready to mount and I know he is going to keep him, and his rider, as safe as possible. So what are we looking for ? Softness - we want our horse to be soft to the halter, to the hand, and to the cues. Willingness - we want to see that the horse is willing to do what we are asking of him.

The horse should be actively performing the skills that we are asking him to do - not reacting or running away.

Bend - we want to see that the horse can bend laterally on both sides without being defensive.

We are looking for combinations of all of the above in all of our groundskills. If they aren't all there then we don't consider the horse ready to ride. This often can be a difficult call. All too often we are on a time limit or we

have people waiting, or we are simply impatient to get on and often we get away with it. The horse can be ridden that day with no mishaps but sometimes we miss signs, or ignore signs, and we get on anyway, and it can end badly. In future articles I will cover using our groundskills for performance training, to vary our training, and to train advanced manoeuvres.

Check your attitude towards your horse By TANJA KRAUS “Your attitude determines your altitude”. I love this quote! Often I will say at clinics, or when someone is having trouble with their horse, that they need to look to themselves to see if there is an issue causing the issue that they are complaining about. For example, someone might come to me and say that their horse rushes in its gaits, or perhaps it is braced when being ridden. Most of the time I see this reflected in the owner - the owner tilted forward, causing the weight to come down on the horse’s shoulders, causing the horse to lose balance and then rush. Or I see the owner bracing in their own body and I see the horse bracing while being ridden by that person. Body position and riding skill is one thing - but I wanted to take a look at this from another angle - our attitude. What is your attitude like when it comes to your horse? How much of your baggage are you putting on him before you even get started? When I am preparing for a clinic, I always plan for it to be the best clinic that I have ever done. I want everyone to leave a clinic excited, pumped and inspired about the future with their horse. This is an easy mindset for me and it goes a long way towards my attitude when I first walk into that arena at 9am and start coaching. But what about your attitude? I know that a lot of people come to clinics with

A positive and constructive attitude towards your horse will help build a much better and stronger relationship with it. a problem horse or a horse that has a few issues or a horse that they need to work on but their success is quite dependent on their attitude towards the horse and the clinic. Often people are able to tell me how their horse is going to behave at a clinic before they even load up and get there - they will send me a message and say 'he will go crazy', 'he doesn't like other horses', he gets nervous etc etc. Now of course some of these things we say due to past experiences - but your horse isn't bringing those experiences with him to the clinic and so neither should you.

Just because your child has one bad day at school - does that mean the rest of the year is going to be terrible? No! Each day is different - and the same goes for your horse. But if you decide that you know how the horse is going to be at a clinic, then that’s the way you will treat him and that’s the way he will be. What about your attitude to the clinic? Are you heading in thinking 'oh I’ve done clinics with this person before, they are all the same, I won’t learn anything new' - of course you won’t! If that’s what you think going in then you aren't even going to be listening for things you don't know (and frankly you are the type of student that will ruin the experience for everyone else with your negativity). How about your attitude to external things that you cant control 'oh I don't like that other student, I'm going to have a bad weekend' or 'oh no its raining / too hot / too cold, that means that I’m not going to learn much at this one', what about 'oh no there are heaps of beginners at this clinic, it will be really basic and I won’t get anything'. Can you see how you choose how much you get out of a clinic or a lesson ? The coach can only 'put it out there' - it’s up to you how much you are going to reach out and take. Every single time I am at a clinic or having a lesson, or event fence sitting at an event I make a conscious decision to learn. It takes a conscious commitment and the right attitude, to be learning consistently. Hendra virus confirmed in NSW NSW Department of Primary Industries (DPI) has confirmed Hendra virus as the cause of death of a horse in Murwillumbah on the NSW north coast. DPI Chief Veterinary Officer Ian Roth said the dead horse had been buried and the property was placed in quarantine. The gelding died at the end of last month after showing typical Hendra symptoms, including lethargy, for two to three days prior," Mr Roth said. "Samples from the horse were sent for laboratory analysis and results confirmed the Hendra virus. "An additional Murwillumbah property that received a gelding which had been in contact with the sick horse was also quarantined." Tracing is underway to confirm movements on or off the property, while two other horses and two dogs on the infected property which were closely monitored for any warning signs of the virus. Staff from Local Land Services were working closely with the property owners. NSW Health had been informed and assessed a number of people who had contact with the dead horse. This is the first Hendra case in NSW this year and DPI confirmed the horse had not been vaccinated for Hendra virus. NSW DPI has been encouraging horse owners to see their veterinarians and work out their vaccination strategy against Hendra virus. "Winter is the season when horses have been infected with Hendra in NSW in the past – so now is the time to get a vaccine booster for your horse," Mr Roth said "Vaccination is the single most effective way of reducing the risk of Hendra virus infection in horses." Mr Roth said people in contact with horses

needed to practice good biosecurity and personal hygiene measures even if a horse is vaccinated against Hendra virus. "Horses should be kept away from flowering and fruiting trees that are attractive to bats," Mr Roth said. "Do not place feed and water under trees and cover feed and water containers with a shelter so they cannot be contaminated from above.

"People should avoid touching sick horses but if they need to come into direct contact ensure they are wearing gloves and a protective mask and avoid any contact with secretions from the animal." Horse owners and vets are encouraged to download the latest information on Hendra virus from the DPI website and if a horse becomes sick, owners should contact their vet immediately.

Waiting for Hendra test results If your veterinarian considers your horse may have Hendra virus, they will take samples for testing at a government laboratory. Initial test results are usually available one to two working days later. Caring for horses while waiting for Hendra results: Although only a few people have ever been infected with Hendra virus, all of these people caught the infection from horses. So strict precautions must be taken when you are caring for your horses while waiting for Hendra results. While you are waiting for test results: • If possible, avoid going within five metres of the sick horse until Hendra virus has been ruled out. • Do not allow children or minors to be within five metres of the sick or dead horse or its body fluids or wastes. • If it is necessary to go within five metres of Page 28

the sick or dead horse, keep the number of people having contact to a minimum and take the precautions outlined under How to reduce the risk of people becoming infected and Personal Protective Equipment as listed in this release. • Move sick horses away from areas that can be accessed by the public. • Isolate the sick horse from other animals if it is safe to do so. Ideally, leave the sick horse where it is and move other animals to a different area of the property. • If you need to feed and water the sick horse keep your distance from them. • Notify your veterinarian immediately of any change in the health status of any horse on the property. Managing a carcase: If the horse under investigation dies or is euthanased, avoid contact with the carcass where possible.

The owner or person in charge of the horse is responsible for the disposal of the carcase. Have a plan for the method of disposal of the carcase, e.g. deep burial on the property, burning on the property or transport to a disposal site. Restrict access to the carcase until it can be disposed of e.g. cover with tarpaulin and tape of area. Advise anyone disposing of the carcass that the horse is under investigation for Hendra virus and to take the precautions outlined under How to reduce the risk of people becoming infected and Personal Protective Equipment. Caring for other animals while waiting for Hendra results: Some other species of animals, such as dogs, cats and pigs are known to be susceptible to Hendra virus infection. Continued page 29

If they become infected with Hendra virus they may pose a risk to humans. All pets should be locked up to prevent contact with the sick horse, its carcase or its body fluids and wastes. Always practise strict hygiene when handling any animal, especially those that may have had contact with the infected animal or its body fluids and wastes. How to reduce the risk of people becoming infected Do not approach within five metres of a horse that may be infected with Hendra virus unless it is necessary. Waiting for Hendra test results: If you must closely approach a sick or dead horse and Hendra virus has not been ruled out, always take the following precautions: • Cover cuts and abrasions with a waterresistant dressing. • Put on all personal protective equipment (PPE) as outlined below. • After handling the horse, remove and decontaminate or dispose of the PPE carefully, making sure there is no contact with your face, particularly your eyes, mouth and nose. • Carefully remove any clothing contaminated with a sick horse’s body fluids. • Immediately wash your hands with soap and water and dry them or use hand wipes and waterless hand hygiene solution. • If you have handled a sick horse, follow these steps before having contact with other horses: o Wash off any contamination with plenty of soap and water. o Shower and wash your hair. o Change your clothes and footwear. o Disinfect footwear and wash clothes in soapy water. • Arrange your activities so that you handle unaffected horses first and have contact with the sick horse last. Personal protective equipment: Personal protective equipment must be fitted and worn correctly. The following personal

protective equipment is required if you need to be within five metres of a horse that may be infected with Hendra virus: • P2 respiratory protection (mask). People who have beards require specialised respiratory protection provided by powered rebreathing units. • Rubber boots • Protective clothing e.g. disposable overalls with full head protection • Eye protection (non-vented goggles or included in respiratory protection) • Rubber gloves/double gloved. Human health concerns: If you have concerns about a person’s health at any time, seek medical advice. Contact your general practitioner, local hospital emergency department or local public health unit if you have concerns about possible exposure of people to a horse infected with Hendra virus. The Hendra virus symptoms in horses Hendra virus can cause a range of symptoms in horses. Usually there is a sudden fever and either respiratory or neurological illness and rapid death. In some cases the onset of illness is gradual . SYMPTOMS CAN INCLUDE: * laboured breathing * frothy &/or blood stained nasal discharge * a temperature (usually but not always higher than 40°C) * Neurological changes, including tilting of the head, loss of vision, abnormal muscle twitching, weakness and loss of balance * Colic like discomfort. Most cases in horses are fatal but occasionally a horse will survive the infection. The reported mortality rate in infected horses is greater than 70%.

The Hendra virus symptoms in people If you have been in contact with an infected horse please seek medical attention immediately. Symptoms typically develop between five and 21 days after contact with an infectious horse. Fever, cough, sore throat, headache and tiredness are common initial symptoms. Meningitis or encephalitis (inflammation of the brain) can then develop, causing headache, high fever, and drowsiness, convulsions and coma. Hendra virus infection can be fatal. THAT TALKS Did you know that horses talk? You only need to listen to the signs they show you to understand what they’re saying. They could talk to you by hanging the tongue out, getting the tongue over the bit or rolling the tongue back. These are all signs of bit pain and are their way of telling you. Those aren’t the only signs: many may chew, grind, grab or chomp on the bit because they are trying to hold the bit pressure off their tongue. Some will lean on the rein, hang on one rein bad, pull the reins through your hands, head toss, run through the bit, or bolt, while others will rear and buck to get rid of the pain or the rider at the end of the reins. Horses will open their mouth to give them more room, enabling them to roll their tongue back or hang it out to escape the bit pain. Others will rub their face on their leg or the rider at the end of the ride - this is them saying “Please get the bit out of my mouth! It has hurt me!” There is extensive research behind the Winning Tongue Plate Bit, and combined with input from leading trainers and riders the design of the bits has helped to overcome these problems. These riders have stated it’s like having power steering and the difference the WTP bit makes is comparable to wearing comfortable slippers all day instead of high heels.

Individual responses to herbs vary ‰ From page 15 Typical doses of capsules of crude herb are 25-100 mg daily for prevention and up to double this amount acutely. Typical doses of 1:2-1:3 are 3-5 mL three times daily for prevention and up to 10-12 mL five times daily acutely. Side effects may occur in patients allergic to feverfew or members of the Asteraceae/Compositae (daisy) family. Numerous other anti-inflammatory herbs might be considered in patients with inflammationrelated pain. Zingiber officinale (ginger) rhizome and its relative Curcuma longa (turmeric) tuber, and Boswellia serrata (Boswellia) gum resin are some systemic anti-inflammatory. Topically, creams or other preparations of Arnica montana (Arnica) flower might give some relief. Ginger is a well-established antiinflammatory with anti-nausea, digestive stimulating, anticancer and antimicrobial properties. A double-blind trial has established the efficacy of ginger, combined with Alpinia galanga (galangal) root, in relieving symptoms in patients with knee osteoarthritis. Case studies suggest it may also help alleviate pain and inflammation in patients with rheumatoid arthritis, though controlled clinical trials are needed to confirm these observations. Ginger, turmeric, and galangal are all members of the Zingiberaceae family. Turmeric contains anti-inflammatory and antioxidant compounds known as curcuminoids including curcumin itself. Curcumin has been shown to reduce postoperative inflammation as effectively as phenylbutazone and more effectively than placebo in one double-blind trial, though pain was equally relieved by all three treatments (curcumin, phenylbutazone and placebo). Boswellia is a fragrant Middle Eastern herb documented in initial clinical trials to alleviate inflammation in patients with conditions such as ulcerative colitis and chronic non-ulcerative colitis. Its effects on pain in such settings has not been specifically investigated. One trial has looked at a combination formula containing Page 36

Boswellia, ginger, turmeric and Withania somnifera (ashwagandha), an immunomodulator. The extract used was poorly described; the daily dose was said to be 444 mg. This 16-week, double-blind trial randomised 165 patients in India with rheumatoid arthritis to the herbal formula or placebo. There was no difference between the herb and placebo groups in terms of pain, though joint swelling was modestly decreased in the herb group. Underdosing, low-quality herbal extracts, and failure to concomitantly remove disease provoking influences, such as food allergens, may explain the poor outcomes. A review of clinical trials conducted in Germany up to 1996, of which two were double-blind, suggested that Boswellia was helpful for rheumatoid arthritis patients, although since that time a negative double-blind trial has been published. Topical treatment of painful, inflamed joints, muscles, or other structures may be beneficial using some herbal extracts. Unfortunately, there are no published clinical trials demonstrating an anti-inflammatory or analgesic effect for topical Arnica, despite widespread clinical use. The European Scientific Cooperative on Phytotherapy (ESCOP) suggests topical Arnica may be helpful in treating sprains, myalgia, and rheumatic pain and inflammation. While a few drops of tincture (1:10) can be applied topically, it is most typical to use a cream or gel at least three times per day. Hypnotic Analgesics Most of the herbal medicines used for improving sleep quality and duration also have analgesic activity. They are often most useful when pain is spasmodic in nature, although generalised reduction of even non spasmodic pain is often observed. The mechanism of action of these herbs has received minimal research attention, so most of this information is based on centuries of clinical experience. The root of valerian (Valeriana officinalis and other species) has been investigated the most for its hypnotic activity. Valerian is not sedative in most humans, meaning that it will not

produce sleepiness unless taken when one is trying to fall asleep. In fact, most studies show that valerian administration actually enhances alertness when taken on a regular basis. These results are in stark contrast with benzodiazepines, which blunt daytime wakefulness and damage sleep microarchitecture and quality. Valerian has been recommended for relieving pain in general, nervous headache, and spasmodic pains by the Eclectics. Animal studies have largely failed to show a centrally-mediated, general analgesic effect for valerian extracts. Valerian constituents may act primarily on GABAergic neurons, possibly including the interneurons that help regulate transmission of pain signals in the central nervous system, though the evidence is certainly not definitive on the mechanisms of action. Considering the existing research it is difficult to reconcile the frequently held opinion of clinicians that valerian has analgesic activity. Even if valerian only helps people indirectly compensate for acute or chronic pain by improving their sleep quality, it could still be of value. Its primary indications are improving sleep, especially that disturbed by pain, and relief of smooth muscle spasm. Typical doses of 1:2-1:3 are 5-10 mL at bedtime and 3-5 mL three times daily for ongoing pain treatment.

Other than the occasional individual who has an opposite reaction to valerian and becomes over stimulated, valerian is extremely safe. Other botanicals that may have similar benefits to valerian are Passiflora incarnata (passionflower) aerial parts, Scutellaria lateriflora (skullcap) herb, Eschscholzia californica (California poppy) aerial parts, Lactuca virosa (wild lettuce) leaf, and Piscidia piscipula (Jamaica dogwood) root bark. None of these herbs has been the subject of clinical trials for relief of pain of any type in humans to date, though at least passionflower and California poppy have documented hypnotic effects in preliminary clinical trials. Passionflower has been suggested as a treatment specifically for neuralgia. Skullcap was regarded by Felter as primarily allowing restful sleep to occur and thereby only indirectly relieving pain. California poppy is a well-established traditional, very safe analgesic in American herbalism. The latex of wild lettuce, known as lactucarium, was formerly utilised by the Eclectics as a weak substitute for opium. Jamaica dogwood is spoken of as a potent hypnotic, anodyne and spasmolytic in the Eclectic literature, effects borne out in modern practice. It is surprising the lack of clinical research on these potentially useful herbs. ‰ Continued page 37

Botanical analgesicslcan be extremely effective ‰ From page 36 Central-acting Analgesics Some of the most powerful botanical analgesics have surprisingly received almost no attention in modern research at all. All are believed to act in the central nervous system, though peripheral activity cannot be excluded based on existing knowledge. Clinically, these herbs, when applied in safe doses, can be extremely effective. Four that will be discussed are Bryonia alba (white bryony) root, Pulsatilla vulgaris (Pulsatilla) aerial parts, Gelsemium sempervirens (Gelsemium, yellow jasmine) root, and Corydalis yanhusuo (yanhusuo, Corydalis) tuber. (A number of species of Corydalis including C. ambigua, C. yanhusuo and C. turtschaninovii have been used as the traditional Chinese medicine yanhusuo.) For greatest safety, heavy machinery should not be operated while taking herbs in this category. Dried plant Pulsatilla is milder than fresh plant tinctures but should be taken within the recommended dosage range. White bryony root has been used by both herbalists and homoeopaths. I t is usually recommended for pain and inflammation in serous membranes, particularly pleurisy, made worse by even small movements. White bryony is probably interchangeable with Bryonia cretica ssp. dioica (red bryony). Cucurbitacins from both plants have been investigated as immunomodulating and antiatherosclerotic agents, but have not been looked at as analgesics. Early signs of toxicity include gastrointestinal distress. The usual dose of tincture (1:10) is one drop diluted in approximately 120 mL water three or more times daily. White bryony is contraindicated in children, during pregnancy or lactation and in patients with hepatic or renal failure. Pulsatilla or pasque flower herb contains potent glycosides such as ranunculin. The aglycone of this molecule is protoanemonin which spontaneously dimerises to form the milder compound anemonin.

Drying appears to promote dimerisation. Ethanol may also promote dimerisation and reduction of toxicity. The fresh herb is recommended for use by the Eclectics, but it is likely more toxic than the dried plant. Felter recommended Pulsatilla for deep eye pain, pain in general with debility, headache, neuralgia, and toothache. It is considered particularly for pain emanating from reproductive organs. Other indications included tension headache. Modern clinical trials have not investigated its efficacy. Even excessive doses of dried Pulsatilla preparations can cause serious side effects including severe gastrointestinal irritation. A typical dose of fresh plant tincture (1:10) is 1-5 drops t.i.d. or as needed for acute pain. A typical dose of dried plant tincture (1:10) is 0.5-3 mL t.i.d. (1.5-9 mL per day). Pulsatilla is contraindicated in pregnancy, lactation and for use in children. Gelsemium root shares some features with Pulsatilla, though it has its unique features as well. Like Pulsatilla, it has been recommended for pain in the genitourinary tract, particularly when related to "nervous tension". Gelsemium is more sedating than Pulsatilla and has been used successfully by many practitioners for pain induced insomnia, insomnia not relieved by milder therapies and severe anxiety. Typical doses of tincture (1:10) are 1-5 drops three times daily or as needed. Overdose may provoke neuromuscular toxicity and death can occur in extreme cases due to paralysis of the diaphragm. Gelsemium is contraindicated in pregnancy, lactation and children. Yanhusuo (Corydalis) is a Chinese herb traditionally utilised to relieve pain including dysmenorrhoea, angina and epigastric pain. It may also help for mild cancer pain. It has a relaxing and sedating effect. The main active constituents are alkaloids including tetrahydropalmatine. Yanhusuo extracts have shown analgesic activity approximately 1% as strong as opium in animal studies. Animal research suggests yan-

husuo acts on dopamine receptors in the central nervous system, and possibly via serotonergic and noradrenergic neurons.47 There are many other species of Corydalis, and not all share the same properties, so substitutions should be made carefully if at all. The usual dose of tincture (1:3-1:5) is 0.25-1 mL three times daily. In traditional Chinese medicine, 4.5-12 g of the crude herb is recommended daily, generally in divided doses as a decoction. Yanhusuo is contraindicated in pregnancy due to possible abortifacient properties. Adverse effects are uncommon at proper doses, though headache or fatigue may occur. Topical Analgesic A botanical medicine that has proven to have an utterly unique mechanism of pain relief is the resinous compound capsaicin extracted from the fruit of Capsicum minimum and other species (cayenne). Capsaicin gives cayenne its spiciness. Capsaicin is a powerful and basically safe tool for relieving a variety of forms of pain when applied topically. However, since it does not treat the cause of pain, it must be seen as a supportive measure at best, and other efforts instituted whenever possible to eliminate the cause of the pain. Capsaicin has a unique mechanism of action. It binds to vanillinoid receptors and strongly promotes release of substance P, neurokinin, somatostatin, and calcitonin from peripheral nerve fibres, particularly C fibres in the slow pain network.

This initially provokes worsening of pain and itching. However, with a few repeated applications, the C fibres are depleted of these neurotransmitters, and are no longer able to transmit pain or itch signals.With continued use, this effect can be sustained indefinitely. Conditions in which topical capsaicin has proven effective in double-blind trials include various neuralgias, diabetic neuropathy, dialysis-induced pruritus, psoriatic pruritus, fibromyalgia, osteoarthritis, stump pain, postmastectomy pain and postherpetic neuralgia. Another interesting though initially very uncomfortable use is intranasal application of capsaicin for relieving cluster or migraine headaches. Various commercial capsaicin creams are available, generally in 0.025% and 0.075% strengths. Typically one to three applications are needed daily although best results come with multiple use in a day. The milder form is tried first, and the stronger resorted to only if the milder is not sufficient. It is imperative that patients either wear disposable gloves or wash very thoroughly with hot water and soap after applying capsaicin. If capsaicin is inadvertently transferred to mucous membranes, the result is intense burning pain and discharge. There are no other known adverse effects of capsaicin use.
