Based on your results, you may be eligible for referral to the free Healthier You NHS Diabetes Prevention Programme. The programme is transforming the lives of those at risk of developing type 2 diabetes. With personalised support from dedicated health coaches, you too can make positive changes to your diet, weight and physical activity to reduce your risk. Diabetes is a serious health condition that can cause blindness, kidney failure, heart disease, stroke and even limb amputation. But it can be prevented.
