Grandparents Raising Grandchildren ‘Information Exchange Tour’ WESTFIELD — On Thursday, October 6 from 6 to 8 p.m., the Westfield Senior Center will be one of the Western Massachusetts host sites for the Grandparents Raising Grandchildren ‘Information Exchange Tour.’

The ‘tour’ is being conducted by the State Attorney General’s Office and the Commission on the Status of Grandparents Raising Grandchildren. This is an ambitious project with eight stops throughout Massachusetts. The goal is to share information with grandparents raising grandchildren about resources for them and their families, both locally and statewide, as well as to listen and learn from them about their concerns as kinship caregivers with a particular focus on how the opioid epidemic is impacting their families. With the information gathered throughout the fall from each event, the Commission and the Attorney General’s Office hopes to improve resources and advocate for changes to help these amazing families. A variety of local organizations will be at the event with brochures, flyers, and helpful information for participants. Please register by calling the Senior

Making Hilltown Businesses More Visible And More Viable HILLTOWNS — The 2017 edition of the Hilltown Business Directory is now soliciting listings.

The Directory has been published

by Hilltown CDC for 30 years to help make hundreds of Hilltown businesses more visible and more viable. It will be delivered in January to every one of the 14,600 households in 20 towns.

Each year, over 300 Hilltown businesses list themselves in the directory. They urge their neighbors to “support local business” and “shop Hilltowns first.” Businesses listed last year said “The hilltown directory is the best thing we ever did!” and “I had a tremendous response last year. I will definitely advertise again!”

The Directory includes businesses in six towns in eastern Berkshire County (Dalton, Hinsdale, Peru, Becket, Washington and Windsor), nine towns in western Hampshire County (Plainfield, Cummington, Goshen, Williamsburg, Chesterfield, Westhampton, Huntington, Worthington and Middlefield) plus Chester, Blandford, Russell and Montgomery in Hampden County and Ashfield in Franklin County.

Breast Cancer Awareness Month The De-escalation of Cancer Treatment “Less is more” was the theme of an educational session at the San Antonio Breast Cancer Symposium last December, which looked at strategies for minimizing or “de-escalating” therapies for breast cancer.

Dr. Grace Makari-Judson, chair of the Baystate Health Breast Network, attended the symposium.

“When we talk about de-escalation of cancer treatment, we mean trying to accomplish the same goal to be cancer-free, but using treatments with fewer side effects that are carefully selected for each individual. That means limiting surgery, omitting or limiting radiation, and omitting chemotherapy whenever possible,” said the Baystate Regional Cancer Program oncologist.

Until recently, it was standard practice to perform axillary lymph node dissection – a surgical procedure to remove lymph nodes from under the arm

– for any woman with cancer found in her sentinel nodes. The logic of performing axillary dissections for a woman with cancer found in her sentinel nodes was that other forms of cancer treatment, such as radiation, chemotherapy, and hormone therapy, are not as effective as surgery at killing off every cancer cell in an affected lymph node.

However, two large clinical trials found that there was no significant difference in the recurrence rates, either under the arm or elsewhere in the body, between women who had more surgery and those who did not. As a result, it has become the standard of care to now omit axillary dissection for women – those who have not undergone chemotherapy before surgery - with early stage breast cancer and 1-2 positive sentinel nodes.

As for breast radiation, standard treatment in the United States has traditionally been performed over 5-6 weeks, while in Britain and Canada the standard has been a 3-4 week course for several decades.

“Recent long term trial results from both these countries have shown this shorter course of radiation, using fewer treatments at a higher dose but the same total dose, to be just as effective and less toxic for appropriately selected women – those with early stage breast cancer not involving the lymph nodes, who have not undergone mastectomy or been given chemotherapy. However, it is not advised for women under the age of 50,” said radiation oncologist Dr. Seth Kaufman at Baystate Medical Center.

And, thanks to molecular testing, doctors are now prescribing 20-30 percent less chemotherapy compared to 10 years ago.

Molecular testing now helps medical oncologists to identify individuals with early stage breast cancers who really need chemotherapy and which individuals have an excellent prognosis with hormone therapy alone. Recent studies have validated these tests and demonstrate that women with low scores do very well with hormone treatment alone.

Signs and Symptoms of Breast Cancer

Widespread use of screening mammograms has increased the number of breast cancers found before they cause any symptoms. Still some breast cancers are not found by mammograms, either because the test was not done or because even under ideal conditions mammograms do not find every breast cancer.

Breast Lump or Mass

The most common symptom of breast cancer is a new lump or mass. A mass that’s painless, hard, and has irregular edges is more likely to be cancer, but breast cancers can be tender, soft, or rounded.
