Why The Change? CMS has cited two main reasons:

1.

Payers cannot pay claims fairly using ICD-9, since the classification does not accurately reflect current technology. Significantly different diagnoses and procedures are assigned to a single ICD-9 code. These limitations are translating directly into limitations in the DRG groups and therefore, the resulting payments.

2.

The healthcare industry cannot accurately measure the quality of care using ICD-9.

It is difficult to evaluate the outcome of new procedures and emerging healthcare conditions when codes are not precise. Most importantly, the industry has a mission to improve its ability to provide patient care, which is limited by the incomplete data mined from ICD-9-CM.

A Brief History In 1853, the International Statistical Congress adopted a list of 139 rubrics. Although this classification was never universally accepted, the general arrangement proposed included the principle of classifying diseases by anatomical site. This survived as the basis of the International List of Causes of Death. The World Health Organization (WHO) took over the responsibility for the ICD at its creation in 1948 when the Sixth Revision, which included causes of morbidity for the first time was published. ICD-10 was endorsed by the 43rd World Health Assembly (the decision-making body of WHO) in May 1990 and came into use by WHO member states in 1994.

organization. Some staff members have expressed thoughts about what being faith-based means. Those thoughts include opportunities to pray aloud with patients and their families, chapel services and the ability to attend them, foundational use of the Ethical and Religious Directives for Catholic Health Care Services and a fully integrated spiritual care program for our patients, families and staff. Of course, being a faith-based organization is all those things! In addition, however, we have an opportunity to carry out the mission in some very personal, not so obvious ways. When we find the urge within us to care for others, we can express that privately by putting the needs of others first as noted in Philippians 2:4. It was the mission of Jesus to care deeply for those among whom he lived. We can do that by making sure before we leave a patient’s room there is nothing further we can do for the patient. We can assist other staff members who are feeling overwhelmed. These are ordinary parts of the work we do daily and may not seem connected to the mission and ministry of TMF. However, such behaviors are intricately a part of the mission and ministry. To be the people we privately would hope to be, by putting others first, may be the foremost part of our mission. Grand Rounds, Grand Learning In September 2013, Physician and Nursing Leadership kicked off our inaugural MD/RN Grand Rounds. These monthly Physician/ Nursing Grand Rounds are scheduled for the first Friday of each month, and offer an excellent opportunity for our staff to hear expert physicians and nurses co-presenting on the latest, up to the minute topics. Each month features different topics presented by physician(s) and nurse(s) who are experts in that field. With the guidance of Dr. Steven Keuer and Dr. Barbara Allen, and nursing leadership from Robert Rose, CNO, our committee works in a collaborative effort to achieve CNE/CME credits for those physicians and nurses who attend. Committee members are Deb Taylor, CNO, LPOHH; Laurie Hartwig, Director, Education; Rhonda Phillips and Shelley Welch, Staff Development Educators; ‘Tish’ Cline, Education Program Specialist and Jennifer

Hawkins, Medical Staff Credentialing. This FREE event scheduled in Wisenbaker Conference Center requires no registration. To learn more about each month’s topics and speaker information is easy, as this is communicated to TMF Physicians and Nursing Staff and our many campuses by email, flyers and Stand Up. For our campuses outside of MFH Tyler, web conferencing is now available which allows participants the ability to view and listen to events, and earn CME/CNE credits. However, since the number of participants is limited, pre-registration and set up is required prior to our event. Ms. Styles designed the first comprehensive study of nurse credentialing.

Certified Nurses Day, observed on March 19 this year, will once again honor and recognize the important achievement of nursing specialty and subspecialty certification. Human Resources News 2013 Flex Reminder As we’ve moved into the New Year, we want to remind employees it is not too late to use your 2013 Flexible Spending Account dollars. Employees who participated in the Flex Medical Reimbursement Account in 2013 have an extended service period of 14 ½ months to incur expenses. Meaning you have until March 15, 2014 to incur expenses and April 15, 2014 to file your claims for reimbursement if you still have a balance in your 2013 Flexible Spending Account. Some eligible Flex expenses include: • P hysician, Lab, Inpatient/Outpatient Co-Pays • Deductibles • P rescription Co-Pays • Vision Co-Pays

Employee Self Service Login Changes As a reminder, the format for your Employee Self Service username and password has changed. Your username is now ‘tmfhs’ followed by your 6-digit employee number, i.e. tmfhs000000. Your password must now be a minimum of 8 characters and no more than 15 characters, must contain both upper and lower case letters, a number and one special character (i.e. Newformat3*). All passwords will expire every 90 days, requiring a new password and ensuring your personal information stays private. If you have any questions please contact Human Resources at (903) 531-4472. Employee Self Service is your portal for accessing and updating your personal information. Self Service gives you access for updating or changing your name, address, phone number and email address. You can also add or change your direct deposit information on the self-service web portal. Need to change your exemptions on your W-4? This can also be completed via your Employee Self Service. Add or update your emergency contact information and much more. Log into your Employee Self Service web portal today.

Employee Engagement Survey Update We want to take a moment and thank everyone who shared their honest and open input with us. We received a 47% response rate providing us data in which to develop action plans in an effort to meet your needs and enhance the TMF employee experience. Our next steps include: 1) All assigned leaders have been facilitating mandatory meetings with their teams to review the engagement survey results and create action plans that are most important to employees. 2) Once all the action plans have been created and assigned, you should begin seeing initiatives developed and implemented based off the action plans developed. In addition, system-wide, we are also looking at initiatives that impact all employees developed by employees. Thank you again for helping make TMF a great place to work!

Cooking Heart to Heart LPOHH Food Services has joined forces with FRESH by Brookshire’s in a basic cooking lifestyle class! Pictured above are Mitch Bond, Chef, and Lauren Clark, Registered Dietician, LPOHH, along with Michael Brady, Corporate Chef for FRESH by Brookshire’s. These three personalities will be teaching, preparing and demonstrating best practices in Heart Healthy cooking. On the first Thursday of each month at 10:00 am in the Whitney Café at LPOHH, the group will be presenting Heart Healthy topics featuring cooking skills and nutritional information. The trio will be mic’d up with live video feed for audience participation and will provide up close demonstration skills of cooking techniques for different types of foods. On the third Thursday of each month at 10 am, the talented trio will take the cooking class over to FRESH by Brookshire’s, located at 6991 Old Jacksonville Highway. Topics are selected quarterly by the group that will keep Heart Healthy cooking as well as seasonal food selections close to the heart

and close at hand for our local community. These classes are free to the public. Registration is not necessary. These Heart to Heart presentations will feature a different theme each month. Our vision is to help promote a Heart Healthy lifestyle through cooking and basic nutrition education.

Women With Heart: Celebrating One of Our Own, Shannon Wilks Throughout the month of February, the LPOHH and KYTX CBS 19 are celebrating ‘Women With Heart’: women who have conquered heart disease with the help of Trinity Mother Frances. Two women from four East Texas counties (Smith, Cherokee, Wood, Henderson) were selected from their community to be honored and help raise awareness about heart disease in women. One of those women, Shannon Wilks, works for TMF Health and Fitness Jacksonville and we are so proud of her for sharing her success story with all of East Texas. Below is a brief summary of her ‘Women With Heart’ story. Shannon Wilks is a single working mother 5 TrinityTALK tribune FEBRUARY 2014

of two children, a daughter in college and her 11-year-old son, so she doesn’t have any spare time to be sick. She has survived 3 heart attacks in the past few years and calls Dr. Oscar Paniagua, Trinity Clinic Cardiology, “her angel.” Dr. Paniagua placed three stents in her heart where blockage had occurred. She has such a positive outlook and says that it is her children that keep her going, without a doubt. She knows that it is in God’s plan for her to still be here to fulfill a purpose and she makes a huge effort now to de-stress as much as possible and just enjoy every day!


Help Your Heart: Coronary Calcium Screening What is coronary calcium scoring? A simple heart scan called coronary calcium scoring can detect your risk of cardiovascular disease. The noninvasive exam takes only minutes to perform and is used to determine the presence of calcified plaque in the arteries. Plaque is composed of fat, cholesterol and calcium. The plaque in the coronary arteries—the arteries that supply blood to the heart—narrows blood flow. This condition, known as atherosclerosis, increases the likelihood of a future heart attack, heart failure and stroke. Early detection enables physicians to provide aggressive treatment that will hopefully prevent major heart attacks or even cardiac death. The result of the test is called a ‘coronary calcium score.’ The amount of calcium detected during the scan is used to calculate a score that is combined with other health information to determine your risk of coronary artery disease and heart attack.

Who should have a scan? All men and women with an intermediate risk for coronary artery disease should consider having a coronary calcium screening. You are encouraged to have a heart scan if you have one or more of the following risk factors: • Family history of heart disease • High cholesterol • High blood pressure • Current or prior tobacco usage • History of prolonged exposure to second-hand smoke •A ge (men between ages 40-65, women between ages 45-70)

It enables the physician to estimate your risk of heart attack, detect heart disease in early stages, and even treat a heart condition before the patient experiences any symptoms of the disease. Treatment may include recommendations of lifestyle changes and medical therapy to decrease your risk of

The cardiac calcium scoring scan is a simple test done with minimal radiation exposure – one-third of the radiation that we receive from natural sources annually. There are no injections or dyes involved, and no preparation or disrobing is needed; the patient simply lies on the table as the CT scanner moves you through the “doughnut” shaped opening for a few seconds. There is no disturbing noise in the room, and the patient experiences no discomfort.

How do I schedule the screening? If you meet certain health criteria* you can schedule a screening without a physician referral. Please consult with your primary care provider or call (888) 440-2344 to schedule your calcium screening for just $49. * Criteria for self-referral: Person to be screened must be a male between the ages of 40-65 or female between the ages of 45-70 AND have at least one of the following risk factors: urrent smoker •C • Obesity • F amily history of heart disease •C holesterol level greater than 160 mg/dl (LDL) • Blood pressure greater than 140/90 mmHg tmfhc.org 6


Employee Spotlight Manuel Bautista, Transportation By: Randy Thomas, Director, Food and Nutrition Someone you should get to know is Manuel Batista. Manuel came to Trinity Mother Frances on February 19, 1996. If you have ever wanted to feel welcome and wished you could see a friendly smile, look for Manuel. He constantly shows the outstanding traits that make TMF a customer service oriented culture. Manuel stays motivated by moving throughout various departments transporting our patients around the system. You will often see Manuel moving with a determined purpose, professional appearance

Mary Hamilton, Laundry By: Cassandra Cooper, PhD, TMF Organizational Effectiveness Officer Dedication, excellence, and service are just a few words to describe how Mary Hamilton has proudly worked for TMF Integrated Services in Laundry. On Tuesday, January 28, 2014, Mary Hamilton retired after 48.2 years with Trinity Mother Frances. To give this momentous occasion perspective, when Mary started on November 1st her hourly pay was 76 cents. The greatest honor when talking with Mary about her career was when she shared, “Work is what I enjoy doing. I always help Gary, CC, and Gerald (her supervisors) in doing what it takes to do a good job and help my co-workers.” Her supervisors are also quick to talk about her dedication. Cecelia Cruz, Laundry Supervisor, shared, “Mary will be hard to replace. In over 48 years she has missed less than 5 days of work.” It is also important to share about Mary that she walks to work every day and has for the past 48+ years. Gary Kennedy, Manager, Laundry, commented, “Mary walks to work in the rain, the cold and yes, even those rare times when there’s snow. Mary even arrives to work early, and tries to not miss a day and let her team down.” Gerald Cofer, Laundry Supervisor, shared, “Mary’s dedication to her co-workers is evident when she asks if there is anything else she can do to help out others. Mary exemplifies the mission and purpose of Trinity Mother Frances and she will be greatly missed.” As a system, our greatest assets are our employees. It is with great appreciation that we spotlight an employee like Mary Hamilton whose tireless dedication and commitment to Trinity Mother Frances will never go unnoticed and will forever be a legacy as to what we as employees should strive to be. and a broad smile. When Manuel is asked how he performs his duties at such a high level of quality he simply replies, “I enjoy meeting nurses, doctors, patients and greeting my coworkers daily with a smile and a ‘Good Morning’”. Manuel enjoys basketball and especially playing games with his son. Manuel’s son would like to be a paleontologist some day. Manuel and his wife both work and alternate the hours to keep the family safe and closely knit together. He works during the day and his wife works as a nurse at night. Manuel also has a daughter who enjoys dancing and watching “Dancing with the Stars”. She has aspirations to be a professional dancer one day. For now, The Barbie Show will do. When you see Manuel, be sure to return his greeting with a huge smile and salutation, giving him thanks for making a difference in all of our lives!


System News Employee Christmas Dinner Our 2013 Employee Christmas Dinner was held on December 18th in the Mother Frances Hospital Tyler Café. Approximately 3,000 meals were served and there have been great reviews on the event. Kudos go out to TMF Food Service, Randy Thomas and Perry Speaker, and their entire team for all of the time and effort to prepare the food, not to even mention their great culinary skills! Thank you! We enjoyed beautiful Christmas music during the day in the Wisenbaker Conference Center, creating a special holiday atmosphere. TMF Jacksonville and Winnsboro, the ASB and our outlying Trinity Clinic locations all celebrated Christmas by holding Employee Christmas Dinners on-site. We would like to convey our appreciation to all who volunteered to help, and to our TMF Leadership for this annual event. Thank you for your generosity and for taking the time to come serve us during the most wonderful time of the year - Christmas.

December Service Excellence Award Winner It is with great pride that we announce the December Service Excellence Award Winner: Theresa McInnis, a lab tech from TMF Canton. The committee selected Theresa because of her “giving heart,” “spirit of caring for her co-workers and patients,” and “her smile and positive attitude.” When speaking with Theresa after her celebration we were honored by her focus on her team and the dedication that everyone at TMF Canton makes. Theresa shared, “I could not receive this award without acknowledging that this is a team effort here in Canton.” So, we decided to honor Theresa by including members of the Canton team and we want to send a special “Thank You” to everyone at TMF Canton for all that you do for both each other and our patients.

System News Heart Healthy Recipe: Tomato Crostini Ingredients 2 pounds ripe tomatoes, cored, halved, seeded, chopped into 1 inch-thick slices 3 garlic cloves, 2 minced, 1 halved Sea salt and freshly ground black pepper 3 tablespoons extra-virgin olive oil plus more for drizzling 1 tablespoon red wine vinegar 1 loaf ciabatta or baguette, cut on a diagonal into 1/3 inch pieces Âź cup packed fresh basil leaves, coarsely chopped Low-fat shredded mozzarella (optional) Preparation Combine tomatoes and minced garlic in a large bowl. Season mildly with salt and pepper. Add 3 tablespoons of oil and vinegar; toss to mix well. Cover and let tomatoes marinate at room temperature to allow flavors to develop. Rub one side of toasted bread with cut sides of remaining garlic cloves; drizzle bread with oil and cut crosswise. Add basil to tomato mixture in bowl and toss well. Arrange crostini on a platter. Spoon some tomato mixture on top of each crostini. Sprinkle lightly with mozzarella if desired. Nutrition Fact Red foods provide your body with a specific anti-oxidant called anthocyanins. Anthocyanins are the anti-oxidant group that includes red, blue and purple foods. Anti-oxidants reduce the amount of free radicals in your body thus promoting good heart health. Tomatoes also include a phytonutrient called lycopene. Lycopene plays an important role in the reduction of prostate cancer. Tomatoes are even more beneficial when eaten cooked. Retention Process Improvement (PI) Working Group We are excited to share with everyone our second system-wide process improvement working group. Since December 2013, a group of dedicated employees have gathered feedback from fellow employees across the system. Their research focus has been on ways to improve retention efforts, assess work-life balance and enhance the workplace environment. I am proud to introduce you to just a few of the individuals who are working hard to share your voice and create initiatives that have a system-wide impact for Trinity Mother Frances. If you see any of the individuals below thank them but also share your voice because your voices makes a difference to this dedicated group of employees! If anyone is interested in participating in this working group or another working group, please reach out to your leader and let them know what area of interest you have. They will then communicate that to either Cassandra Cooper or Marsha Ballard and a determination will made regarding which process improvement working group would be most appropriate for you. This is another great way to add skills to your own career development tool kit.


LiveWELL Success

By: Diane Bottoms, Administrative Secretary, Mother Frances Hospital Winnsboro I have been overweight for a very long time and February 2013, my doctor diagnosed me as diabetic and that was the “Ah-Ha” wakeup call for me. I realized that I had been eating too much of everything - especially ice cream and chocolate syrup. So, I decided to make a life change and start eating the right food and especially the right amount of good food. I started walking and using my Total Gym and Ab Lounger that had been just collecting dust for a long time. One of my friends here at the hospital showed me a couple of “apps” for my phone to track my eating habits and exercise. With the great support I get from my faith, family and my wonderful friends here at the hospital, I have gone from 265 pounds in February 2013 to 180 pounds today. My goal is 150 so I am very close. All of my lab work is so much improved that my doctor wants to take my picture and hang it on the wall and show me off as his “A-plus” student. Some of my favorite things have changed. I love to get on the scale on Monday mornings to see if I have lost a pound or two. I love to go through my closet and be able to pass some of my clothes to others and I have really enjoyed being able to shop in clothing stores that I previously would not even walk into. So if you are ready, get out there and make a life change. You too can feel this thrill that I am experiencing each day.
