Stay ACTIVE BY CENTURY OAKS ASSISTED LIVING
We’ve all heard that staying active and keeping our minds sharp will lead to longer lives, but the concept is more than simply common sense. As we age, we begin to lose muscle mass, are likely to have some memory loss and decreased thinking skills, and we may potentially experience periods of depression. Muscle mass issues can be addressed by increasing aerobic activity with brisk walks, swimming, yoga, and stretching. Physical activity can slow the aging process and builds endorphins that add to our feeling good. The benefits of exercise are numerous, including reducing the risk for obesity, diabetes, dementia, and depression. In addition to adding exercise to our daily routine, we can also eliminate processed foods from our diets, eat more omega-3 fatty acids, drink enough water daily, and take vitamins. We can eat more fruits and vegetables, notably blueberries and mushrooms, which have been proven to slow the aging process. Not to be overlooked are ways to keep our brains active. According to the British Medical Journal, over 40 percent of adults age 65+ have some ageassociated memory loss, which comes to approximately 16 million people. While there are several studies underway of drugs to treat mild cognitive impairment in order to delay the onset of Alzheimer’s disease, other areas under study include
non-drug therapies, like puzzles and crossword puzzles, and games, such as Sudoku. A study from the University of Exeter and King’s College in London discovered that people who engage in word puzzles have brain function equal to people 10 years younger than their age. On problem-solving tests and grammar tests, people who regularly do word puzzles and grammar tests performed an average of eight years younger compared to those who don’t. The benefits of such activities include: boosting logical thinking, improving memory and recall, improving concentration, slowing Alzheimer’s, and reducing anxiety. Lastly, staying active not only encompasses physical and mental exercises but also social activities. Making time for your friends reduces loneliness. Social activities, such as singing in a church choir, joining a book club, belonging to a bridge club or sewing club, are effective ways to maintain a healthy social life. By maintaining daily or weekly social activities, we can lessen our chances of experiencing periods of depression and improve overall physical health at the same time. By staying active and keeping our minds sharp, we can slow down the aging process. We can honestly tell ourselves “We’re not getting older, we’re getting better!”

For people with Alzheimer’s or dementia, music therapy is a powerful tool because a person’s rhythmic response is tied to the motor center of the brain that requires little or no cognition or mental processing. Human response to music, particularly drumming or singing, is essentially primitive. People with Alzheimer’s or dementia who engage in music therapy often respond positively, even in the late stages of disease. APRIL–SEPTEMBER 2021 JOURNEY OF AGING
Having taught and played music most of my life, it’s been a joy bringing music’s therapeutic benefits to seniors and caregivers. I’ve heard nothing but appreciation from participants in our local Lyrics & Laughter community program, an offshoot of a program from the United Kingdom called Singing for the Brain. Lyrics & Laughter is a 45to 60-minute weekly music program
for people with dementia and their caregivers that goes beyond passive listening by incorporating singing, instruments, and action. We gather in a circle, and that cue helps focus our participants. We then begin a session of warm-ups; hello songs; rounds; rhythms; and thematic tunes on familiar topics, like the Old West, patriotism, and Irish folk tunes. Caregivers enjoy sharing in the songs too.
GENERAL | 19 An iPod listening program is another great idea for seniors whose families can help develop playlists of favorite songs. The personalized collection helps those in need of sensory stimulation, and earphone splitters can allow a senior and family member to experience music together. I’ve seen people hold hands, dance, smile, and sing or simply get a sparkle in their eyes when the music starts. Songs from a person’s young adult years, from about age 18 to 25, are the most likely to elicit dancing, toe tapping, happy facial expressions, and other forms of engagement. People with late-stage dementia especially respond to folk songs when sung in the language in which they were learned. Don’t worry if you don’t have time to create a full playlist; each time you visit, you can pull up a special song to share together. I was once sitting with an otherwise nonverbal resident who began speaking to me once she heard her songs, even
complimenting me on my blouse. Music can help elicit speech in people with advanced dementia, even if only for a short time. Facilitated drum circles brighten moods, encourage social interaction, and allow the release of pent-up emotions. People with Alzheimer’s or dementia who typically sit with their heads down may perk up and move their eyes around when group drumming starts. Drum circles are a very good fit for those who can tolerate the loud drumming because it taps into their rhythmic instinct. It’s also an affordable participatory program because drums can be handmade with simple wooden frames and duct tape, and sticks can be used for mallets. Everyone deserves music in their lives, and for those with Alzheimer’s or dementia, it can be a portal to past feelings of happiness and contentment, building connections with people
in their current social circle. It might even help them express with words and smiles, which puts a song in anyone’s heart.

A challenge for family caregivers of a person with memory loss is a loved one’s inability to communicate. In fact, the inability to understand what’s being said or talk properly may be one of the first signs of dementia. If a loved one doesn’t respond to words, it may not be because they can’t hear or are electing to not hear you, but that the disease is preventing them from understanding what you’re saying or responding in an appropriate manner. disease taking and scrambling them so what’s heard is a completely different message, resulting in an equally scrambled response. The reverse also happens—they hear and understand what’s being said, but then the disease scrambles the words so they come out wrong. When this begins, it’s easy to slowly stop communicating—to stop asking about their day or what they want to do—because the response is painful or takes too long to get out. But even
those in the end stages of a disease need interaction, daily communication, and to hear a human voice. Here are a few tips to improve communication. • Approach a loved one from the front and make eye contact. Let them know you’re talking to them and that they’re important. This helps them focus and have better responses. • Speak calmly with short sentences, and ask one question at a time. This

You may be amazed by their nonverbal responses: eye flutters, hand grasps, smiles, and frowns. • Use their life story to distract and not react. If they ask about a relative who has died, don’t get upset that they don’t recall the death, but instead help them remember a special time they had with the person.
allows your loved one to process things more easily and to not get overwhelmed. • Wait, listen, and watch. You can tell a lot by watching someone’s facial expression. Do they like what you said? Did they laugh? Did their eyes light up? You’ll get a response if you’re patient and cue into verbal and nonverbal reactions. • Don’t argue with or correct a loved one, and give compliments. One reason communication can stop is a sense of failure. It doesn’t matter
if they get something completely right, but that they’re talking with you. They need a win more than you do and to be told frequently how wonderful they are. Your job is to help them feel loved and heard. • Don’t talk about them with others while they’re present, and ask their opinions. Just because they can’t communicate as well as they used to doesn’t mean they’re not here. Even those in the end stages of a disease can hear what’s going on around them. Bring them into conversations of joy, not diagnosis
Everyone has a life story, and it’s our job to help loved one’s stories continue despite disease barriers. Sing, talk, laugh, cry, hold hands, smile, and keep the lines of communication flowing.

Lumbar SPINAL Stenosis BY DR. SETH SOHN
Lumbar spinal stenosis (LSS) is a common medical condition in the elderly population that affects over a million patients in the United States. LSS is the narrowing of the spinal canal that compresses nerve roots traveling through the back. This can be caused by degenerative disc disease; arthritic changes in the spine; and, at times, the sliding of one vertebra over another, resulting in the compression of both APRIL–SEPTEMBER 2021 JOURNEY OF AGING
the nerves and the blood supply to the area. Symptoms can include pain in the back, buttocks, and lower extremities as well as weakness, aching, cramping, fatigue, numbness, and loss of mobility. Patients with LSS report difficulty performing activities of daily living that require standing or walking and report improvement in their pain with sitting or leaning forward, such as on a shopping cart or walker.
Traditionally, this condition has been treated with physical therapy; medications; and interventional options, such as epidural steroid injections or surgical decompression for the affected levels. Recently, an FDA approved product by Vertiflex™ for the treatment of this condition was introduced in the United States called the Superion™ Indirect Decompression System, or spinous

spacer. This implantable device offers a minimally invasive option to treat mild to moderate spinal stenosis, resulting in significantly less recovery time than traditional surgical options. The spinous spacer is supported by robust research, with studies showing comparable outcomes to traditional decompression with laminectomy, which has been considered the gold standard for treatment of spinal stenosis. It’s believed that this device offers an effective treatment option for patients that have failed to see relief with conservative care but are not yet considered candidates for traditional surgical intervention or
want to avoid a significantly more invasive surgery. As a same-day outpatient procedure, the quarter-sized device is implanted under x-ray guidance through a small one- to one-and-a-halfcentimeter incision. The implant works by preventing the spinal levels treated from moving into extension, the position that causes pain when standing and walking. This device does not restrict motion in any other plane, reducing the risk of more rapid degeneration at untreated levels as seen with traditional decompression with fusion. Patient recovery typically consists of minor surgical discomfort at the incision site for a week and
light activity for four weeks. Patients typically report improvement in their symptoms within two to four weeks. Having this procedure does not affect the patient’s ability to pursue moreaggressive surgical intervention in the future, as it does not alter the anatomy of the spine in any way and can be removed if needed. The Vertiflex spinous spacer is a safe and reversible procedure for the patient with spinal stenosis looking to prevent or prolong the need for more aggressive surgical intervention. Aging brings change. Though these changes can be good, they can also be difficult. Sometimes people have to face the harsh reality that they can no longer care for the property where they lived for a large majority of their lives. When AGING in Place is Wrong Studies indicate that over 80 percent of seniors want to spend the rest of their lives in their home, a concept called aging in place. Most companies, agencies, and senior care professionals work toward that goal. However, there are two important concerns to aging in place as the desired lifestyle: safety and social. SOS, the universal call for help. The following are areas to be addressed for safety. • Forgetting to take medications, taking more than prescribed, or sharing • Forgetfulness stemming from dementia or Alzheimer’s
• Spoiled food that does not get thrown away, poor diet, or forgetting to eat • Dirty house, extreme clutter, dirty laundry piling up, decline in grooming • Missing important appointments, especially doctor’s appointments
SAFE & HEALTHY AT HOME | 43 There are times, though, when safety or social concerns make aging in place inadvisable—think of safety or social as SOS. • Unexplained bruising or continual skin tears that invite infection • Trouble getting up from a seated position or trouble getting out of bed • Difficulty with walking, balance, and mobility • Uncertainty and confusion when performing once-familiar tasks • Expenditures for unneeded repairs or purchases—changes in activities • Infrequent showering or bathing— use of perfumes to mask • Stacks of unopened mail, late payment notices, and bill collectors There are also situations that directly affect the safety of the senior. • Wandering from home or driving and not remembering how to get home • Frequent medication mistakes, especially if medications are for a disease
• Unsafe behavior like leaving the stove on or front door unlocked • Physical abuse of spouse or caregiver, or self-neglect • Lack of adequate fluids or nutrition • Increased falls • Signs of depression
can foster negative thinking. Seniors can become trapped in their home by a lack of mobility and loss of family and friends. The social interaction in communal living, such as independent and assisted-living communities, often helps.
While safety is key, depression is often a link to a need for social interaction. The National Institute on Aging reported studies that show social relationships are consistently associated with biomarkers of health. Social isolation constitutes a major risk factor for morbidity and mortality; those without adequate social interaction were twice as likely to die prematurely; social isolation impairs the immune system and boosts inflammation; and people are reluctant to admit to loneliness.
