Assistive Technology FOR AGING AT HOME

Aging at home can present a unique set of challenges for older adults. Loss of hearing, sight, or mobility can impact daily life and prevent feeling independent at home and within a community. Assistive technology (AT) can aid in performing activities that are otherwise difficult, helping people with
disabilities regain their independence and live more active lives.
AT is any piece of equipment, software program, or product used to increase, maintain, and improve functional capabilities. It can be as high tech as a computer or as low tech as a

special writing utensil. AT helps build confidence and independence by empowering people in doing things for themselves.
As an example, Roger has persistent hand tremors in both hands that make eating and cooking more challenging. He’d like to continue cooking for himself, particularly his famous stirfry. An example of AT that could help Roger is a unique cutting board that has a chef’s knife pivot mounted directly to the cutting board. There are suction cups to secure the board to the counter, so cutting only requires the use of one hand and simple motions. And, with the addition of a stabilizer spoon to compensate for his hand tremors, more of Roger’s stir-fry masterpiece will make it into his mouth.
Tasha’s daughter just started a new job, and Tasha wants to provide support and encouragement to her through phone calls. But Tasha has trouble hearing incoming calls and the speaker on the phone. An amplified telephone would boost the sound for Tasha. A signaling device could be added to alert her that someone is calling by flashing a light or playing a loud tone. Some of these
phones also have large print dial pads or picture dialing capabilities to make it easier to place outgoing phone calls as well.
With just a few simple pieces of technology, Roger and Tasha can combat obstacles that were hindering their independent-living journey. With thousands upon thousands of AT options available, older adults should feel encouraged to find the support they need so they can live their best lives at home.

MAKING THE DECISION ABOUT FACILITY CARE

The decision to move a loved one with Alzheimer’s or dementia into facility care can be a very difficult one to make for care partners and families. One longtime Alzheimer’s & Dementia Alliance of Wisconsin client and frequent volunteer shared that he remembers two dates vividly: the day he and his wife were married and the day he placed her in facility care. Caregivers frequently struggle with guilt, regret,
anger, fear, and sadness throughout the process.
There’s no consistent or standard right time for a move. The timing depends on individual circumstances, and everyone’s story and situation is different. Sometimes family members who can’t imagine providing personal care for their loved one find themselves transitioning naturally into a caregiving
role. Others who may have committed to keeping the person at home end up overwhelmed with unexpected care demands, safety concerns, and health issues. Moving a loved one to a care facility may be based upon one or multiple reasons.
Safety. The loved one forgets or refuses to use safety equipment, such as wheelchairs, walkers, and grab bars.

Frequently, spoiled food is found in the individual’s refrigerator, their stove burners are on, or they leave cigarettes unattended. Regular falls or wandering away from home are key reasons to consider a move.
Poor hygiene. A combination of urinary or fecal incontinence plus a refusal to bathe can lead to serious health problems.
Inadequate nutrition. The loved one is unable to prepare food, has problems eating and swallowing, or forgets to eat entirely.
Serious behavior problems. Examples include wandering and regular verbal or physical aggression.
Increased need for care. When a caregiver is no longer able to provide the level of care needed, the individual requires additional assistance with daily living activities, such as toileting; bathing; dressing; eating; moving around the home; and transferring between bed, chair, and toilet.
The individual requires 24/7 care and supervision.
Inability to balance care with other responsibilities. A caregiver is unable to provide care while managing their own employment and family responsibilities.
Crisis. Situations, such as illness or injury, arise for the loved one or caretaker. It’s not unusual for persons with dementia to go from a hospital to a care facility.
Caregiver burnout. Caregivers can become physically, emotionally, and mentally exhausted. Unfortunately, they often don’t recognize the toll of the caretaking until they’ve broken down or the loved one has moved into a facility.
Optimally, the decision to move an individual to a facility is made in consultation with others, including the individual. Ask others to take on the caregiving role for a few days so fresh eyes can help evaluate the caregiving
demands. Adult children often recognize the burden to one parent while caring for the other; their advice can be invaluable. Consider convening a family meeting that includes family, friends, neighbors, clergy, and others who are involved in supporting the individual.
Caregivers may be surprised to find that a move to a care facility improves the quality of life for their loved one. Increased social interactions, support of caring staff, a daily routine, regular meals, improved sleep, and engaging activities can lead to increased well-being and a more satisfying life. This possibility can often remain unconsidered.
While there may be no clear right time for placement, many professionals suggest it’s never too early to start planning. Thinking about a care facility location, size, quality indicators, and funding can bring peace of mind even if a loved one is able to remain at home. A solid plan protects the caregiver and loved one in the case of sudden crisis or rapid disease progression.

WHY AN END-OF-LIFE DOULA?

There are many end-of-life practitioners, some more medical in nature, some less so. Gerontology, palliative medicine, and hospice care represent medical practitioners. Hospice describes a dying person as a “patient” who is “admitted” to hospice with a “diagnosis” and will receive a hospice physician, nurse, and
nursing assistants. They arrive at the dying person’s home in medical scrubs with the usual medical accoutrements, such as stethoscopes; oxygen saturation monitors (the little gizmo they put on your finger); and medical supplies, including pads, bandages, specialized lotions, ointments, and soaps, which are helpful for all sorts of skin ailments that can happen when one is bed ridden. The patient receives a care plan, and everything
proceeds within the paradigm of illness and treatment.
While illness may bring about the end of life, dying is not a disease nor is aging; it’s a natural human process. There’s no treatment to prevent aging. We grow old. There’s no treatment to prevent dying. We all die. Something is missing in the medical approach to death, though, and some of us are creating a new approach—returning

to an ancestral view of death that acknowledges death as a natural and often beautiful transition. It can necessitate medications for pain or anxiety, but not always.
Physicians, nurses, and hospice offer wonderful and often essential endof-life care. But it’s the illusion of a cure, or at least of control, that causes us to lose so much of the grace, beauty, and gift of aging and, finally, of dying. This is why a dying person or family member reaches out for an experienced end-of-life doula, midwife, or practitioner.
Death doulas provide advocacy with hospice and other professionals, expertise in legal and financial preparation for end of life, knowledge of options in funeral and burial arrangements, and wisdom and guidance through the entire process. Death can be terrifying, and for most, death and dying are the last things we want to pay attention to. A death doula can make all the difference.
How do you choose the right doula for you and your loved one? Reach out to the National End-of-Life Doula Association to access contact information for
death doulas and practitioners in your area then interview a number of them before choosing who you’d like to work with. There are three qualities you should seek.
Experience. Does the doula offer the wisdom and depth of life experience to provide you with a knowledgeable and resourceful companion during this difficult transition time.
Heart. Does the doula have heart for the work. A doula should not lecture or instruct you to do something a specific way; they’re a guide and are present for all family members and their needs and perspectives. They can sit with those feeling anger, regret, grief, and fear and with those feeling joy, resolution, gratitude, and wonder. They provide a safe and accepting space, and do so with deep respect. They don’t pretend to know all the answers, but want to help open the way family members need to journey. They have the gift to create a sacred space—not in a religious sense, but in a human sense. They’ll do all of this with the utmost care by walking the dying one home and being at their side.
Time. Does the death doula have time for you.

SPENDING LESS on Groceries

As grocery prices rise, buying food and ingredients cuts more and more into your budget. We may all know about clipping coupons and buying products on sale to save money, but there are other tips you can use to lessen the financial burden.
Write a shopping list and stick to it. Flashy product displays and your own hunger can cause you to make unintended purchases.
Buy sale items. Grocery stores will often put sale items in the entryway or at the ends of aisles. Look for flyers or sign up to receive them via email.
Avoid prepackaged and prepared foods. Salad kits, shredded cheese, snack packs, sandwiches, and other preportioned and prepared foods can save time, but you’re paying for that convenience. You can save money by buying the components and assembling them yourself.
Buy when it’s a good deal and store what you can’t use soon. If a larger version of a product you were going to buy is on sale, consider buying it, using what you need now, then storing the rest. For example, it can be cost effective to buy fruit in season and freezing or canning it to enjoy later in the year. Keep a dry-erase board on

your freezer to remind yourself what’s inside so that you use the food and don’t waste the money you saved! Be careful not to buy a large quantity in hopes that you’ll figure out how to use it later; you may end up spending more than you would have otherwise.
Look at the unit cost. Usually, a larger quantity of product will cost less per ounce than a smaller quantity—for example, a gallon of milk compared to a quart of milk. Sometimes, though, the savings is negligible, and you’re better off buying the smaller quantity to ensure the product remains fresh and that you use it all. Compare two different product sizes by looking at the unit cost on the price tag.
Buy generic brands. Generic brands often source their products from recognizable national brands. For any generic products at a notably cheaper price, consider buying a small quantity to see if you like it.
Buy smaller quantities if possible. Many stores have bulk bins with candy, nuts, and coffee; some stores have additional products. You can buy the quantity that you’ll use before the product quality decreases, saving you from discarding the rest later. Check
the unit cost, though, to ensure that you’re getting a good deal compared to the packaged version of the product. If possible, buy a few apples rather than a full bag unless you have plans to use the larger amount.
Look for an imperfect produce section. Stores are increasingly offering very ripe, blemished, or otherwise imperfect fruit and vegetables for a lower cost. Depending on how you plan to use the produce, it might be worth buying from this section.
Buy produce in season. When fruit and vegetables have to travel
farther and in reduced supply, they’re more expensive.
Make extra portions. It often costs you less and takes you less time to bake two pies, roast two chickens, or make two side dishes at once. You can freeze the second, eat leftovers, or give it to a friend.
Sign up for the loyalty program or, for a co-op, look at the member benefits. The savings might be worth it!
The idea isn’t to use all of these tips, but to find a handful that work for you. Oftentimes, you’ll find you end up with a heavier wallet and a healthier lifestyle.

The Volunteer Factor–AGING WITH GRACE AND COMMUNITY

Visions of retirement and aging vary for everyone—some spend their work life looking forward to a retirement of leisure and travel; others hope to pursue their passions and hobbies more extensively. No matter how you imagine your golden years, volunteering can be an integral part of a successful aging plan. Older adult volunteers experience improved physical and mental health, increased life satisfaction and self-esteem, and
increased social interactions and relationship developments that come from sharing interests.
Throughout her career, Sharing Active Independent Lives (SAIL) Volunteer Coordinator Becky Rogers has observed all of the above when interacting with volunteers. “The best part of working with volunteers is the joy that people find once they are really involved and
to see what a difference they make for someone else. Some people volunteer with one goal in mind and then discover there are broader purposes.”
Volunteering takes all forms, from providing occasional assistance to a neighbor to a more formal role within a church, local nonprofit, school, or business. Some find satisfaction in sharing previous work knowledge

and experience while others seek to explore new things. Someone who had a career in finance may volunteer to do bookkeeping at a nonprofit. A person can volunteer for a local tourism agency helping others to enjoy the community they love; at a hospital or hospice; by reading a book to someone with low vision; or using a talent, such as sewing, to assist those who can no longer do something for themselves.
SAIL Member and Volunteer Ken Martin recalls a CEO he worked with sharing a Muhammad Ali quote: “Service to others is the rent you pay for your room here on earth.” It resonated with Ken, and in 2014, shortly after his retirement, he heard AgeBetter Executive Director Ann Albert speak at a retiree luncheon. While Ken wasn’t in need of assistance, he recognized he might be in the future because his family wasn’t nearby. “I’ve given people rides, flipped mattresses, hung pictures, and moved boxes from upstairs to downstairs,” says Ken, who enjoys meeting people. “I like to listen to their stories—we all have stories. One day, I’ll be a consumer of the services I’m providing now.”
Similar to Ken, Nancy Winton joined SAIL after she heard Ann’s presentation
at her church in 2005. Nancy was recently retired and wondered what she’d do with her time. “I told Ann, ‘I’m not ready for services now, but I’d like to volunteer,’ and I became a volunteer driver. As time passed, I started using the same services I had offered to others.” The pay it forward mentality is at the heart of village model organizations, like SAIL, and a generally good way for older adults to volunteer and then feel comfortable accepting assistance when they need it.
Those who can’t or don’t want to leave home can still volunteer and experience the benefits of avoiding isolation and loneliness, remaining mentally active, and feeling a sense of purpose. Many organizations need volunteers to assist with office work that can be completed from home, such as making check-in phone calls, sending personal notes, and using crafting skills to make décor for fundraising events.
The list of possibilities is endless and the rewards valuable for volunteers and recipients. Are you looking for a possible volunteer opportunity? A good place to start may be Madison Community Foundation, which maintains a directory of 501(c)3 organizations in Greater Dane

PALLIATIVE CARE
A specialized area of healthcare that focuses on relieving and preventing the suffering of patients. Unlike hospice care, palliative medicine is appropriate for patients in all stages, including those undergoing treatment for curable illnesses and those living with chronic
diseases, as well as patients who are nearing the end of life. Palliative medicine utilizes a multidisciplinary approach to patient care, relying on input from allied health professionals in formulating a plan of care to relieve suffering in all areas of a patient’s life. Palliative programs may vary according to the provider. Benefits of ACTIVE AGING

We all know the importance of eating well and staying physically active as you age. The benefits are undeniable. But active aging involves a lot more than exercise and a healthy diet, it’s also a mindset where people of any age, especially older adults, open themselves up to trying new things and experiences, meeting new people, and embracing what’s possible.
Qualities of happiness, optimism, and positive mental attitude may be just
what make the difference between active agers and those less active and infirm, especially in times when healthrelated issues for seniors are becoming more the norm than the exception. Bobby McFerrin was onto something when he wrote the song “Don’t Worry, Be Happy.”
For seniors, loneliness is a primary contributor to mental and physical health decline. While many are determined to stay in their homes forever, studies show
that this can often result in increased feelings of loneliness, isolation, and anxiety that can contribute to other health issues, such as heart disease, depression, and cognitive decline.
Active senior living communities offer a true respite from loneliness through their lifestyle programming, providing seniors with a plethora of activities and opportunities to socialize, exercise, and try new things. While senior living may not be for everyone, the increased

activity that these communities offer stimulates minds and is proven to result in higher levels of happiness and increased quality of life.
Lifestyle enrichment staff within senior communities are committed to engaging seniors and keeping them as active and involved as they choose to be. This is done in a variety of ways, such as offering fitness classes, arts and crafts, happy hours, local field trips, musical entertainment, community sing-a-longs, woodworking shop, Bible studies, card groups, cooking classes, and so much more. Many communities also offer transportation to local stores, theatres, and museums; provide outdoor activities to promote getting out into the great outdoors; and host vibrant social events that also invite friends and family to join in the fun.
Meghan Davis, director of lifestyle enrichment for Capri Communities, explains, “We strive to ingrain our signature enjoy life programming into our communities’ day-to-day lives. It’s our commitment to inspire our residents to branch out, try new things, and engage in positive and meaningful
relationships with peers, staff, family, and their larger communities. In 2021, we launched a new enjoy life adventure series that has been very well received by residents and families. It brings people from multiple communities together to interact with each other and take on new adventures outside of normal day-to-day programming, including special events, such as a summer bocce ball league, line dancing class, boat cruises, historic train ride to East Troy, winery tours, field trips to visit other local attractions, and educational offerings. It has been very gratifying to see our residents engage with each other, form new friendships, and have a great time in the process! We see this as a real difference maker
for our residents that’s enhancing their quality of life.”
So while the concept of active aging may not be rocket science, the fundamental belief in promoting healthy living by feeding mind, body, and soul with stimulating activity, exercise, and socialization is something that cannot be underscored enough in helping people age gracefully and live long, fulfilling lives.

Finding the Right MEAL PREPARATION SERVICE

Finding the right meal service can be challenging. One family caregiver shared, “My mom is no longer cooking for herself, and her health is suffering as a result. Something’s got to change, but I’m not sure which option would work best for her.”
Meal delivery services are convenient and affordable, but can’t be customized and sometimes aren’t very healthy. Meal kit companies offer variety, but require online ordering, and you have to do the cooking and cleanup. Having a homecare aide prepare freshly made meals also offers companionship, but they’re typically not trained cooks and their services are expensive.
Personal chef services provide highquality, fully customized meals and companionship, but they can be more expensive. The same caregiver said, “What would be nice is unbiased information about food options for seniors.”
Meal Delivery
• Tend to be more affordable, charging $8 to $14 per meal plus shipping, which is approximately $20 per order.
• Order meals online for a cooler full of frozen meals to be delivered in three to seven days. Meals can be heated at your convenience.
• Meals are prepared in a factory setting, so exact calorie counts and other nutrient breakdowns are available, which is helpful for dietary concerns.
• Meals are prepared in a central location then shipped, so you can order from anywhere in the United States.
• Many seniors have dietary concerns and food preferences, but companies cannot accommodate because meals are mass produced.
• Most deliver a cooler of massproduced IQF (individual quick

freezing) frozen meals, which aren’t as fresh and tasty as other options.
• Meals might not be eaten—instead, simply being stacked in the freezer.
• Excess waste because of disposable containers.
Meal Kit Companies
• Blue Apron, Hello Fresh, and similar companies offer online ordering. Meal kits are mailed with recipes and cooking instructions packaged in materials that keep food a certain temperature.
• Many have a wide meal selection, including vegetarian and gluten-free options.
• While many have a large entrée selection, they’re unable to customize for dietary restrictions and food preferences.
• Requires online ordering, leaving some seniors at a disadvantage if they don’t have a computer or internet services.
• Meal kits provide ingredients for meal preparation. You cook and clean up.
Homecare Aide Prepared Meals
• Meals are prepared in client’s home, which offers a great opportunity for companionship.
• Aides typically don’t prepare makeahead meals for the week, instead preparing one meal at a time and plating it for client.
• The highest cost option. The average hourly rate is $25, so two meals per day with a one-hour prep time per meal is $350 per week plus groceries.
• May not have much cooking experience, which can affect quality and be limited for special diets.
• Typically not ServSafe certified, so not trained in proper food preparation and storage procedures, which could lead to food safety issues.
Personal Chef
• Many have professional cooking experience and culinary degrees, so meal quality is typically far better.
• Customizes meals to meet dietary concerns and food preferences, and can work with your dietitian and doctor to ensure you’re eating in accordance with recommendations.
• Meals are prepared in your home, providing companionship as well as a fun experience.
• Most are ServSafe certified, so they’re knowledgeable about proper food handling and storage procedures, helping to avoid the potential for foodborne illness.
• After food preparation, will clean the kitchen.
• Most are affordable, but more expensive than meal delivery services, although there are exceptions.
• The personal chef industry is relatively new, so not available everywhere. Most located in and around major metropolitan areas.
Finding the right meal service is an important part of aging well at home. When considering options, reach out to a variety of companies to discuss specific needs. Be sure to consider customization and companionship when weighing costs as they add great value to meal service.
Lisa Allman is co-founder of Chefs For Seniors, which charges $125 plus groceries ($40 to $80) for approximately 12 meal servings. Cost is often less than the cost of a takeout meal or meal delivery. Flexible plans allow client to use the service as often and long as they choose.

ADULT DAY CENTERS
These are nonresidential facilities providing activities for the elderly and special-needs individuals. Most centers operate 10 to 12 hours a day and provide meals, social/recreational activities, and general supervision. These centers enable caregivers to go to work or to obtain a break from their responsibilities.
