Fox Valley MEMORY PROJECT BY JILL H. GRAMBOW Fox Valley Memory Project (FVMP) is celebrating its rebirth! Originally operated as a program with Lutheran Social Services, FVMP provides activities and resources for those with forms of dementia and memory loss. FVMP applied to be an independent nonprofit agency and received approval as such in August. The new designation also brings with it a new full-time executive director, Jill H. Grambow; new office space at Goodwill Community Campus on Appleton Road in Menasha; and a new FVMP logo. However, many things about FVMP remain the same. The vision of FVMP is to ease the burden of dementia and memory loss by providing support and resources throughout the Fox Valley JANUARY–JUNE 2019 JOURNEY OF AGING
region. The bread and butter of programming continues to focus on opportunities for social connections and community engagement. • The most notable social activity is the Memory Café. There are nine FVMP cafés per month located throughout Appleton, Kaukauna, Kimberly, Menasha, Neenah, and New London. Cafés take place in community locations and are facilitated by a team of volunteers with experience in recreation/adult programs. The cafés each have a theme for the month and include a variety of activities, from games and projects to guest speakers, learning experiences, conversation, laughter, and refreshments. It’s a casual atmosphere where those
with memory loss can enjoy camaraderie and feel comfortable among friends. • Other social activities include a monthly dinner at a local restaurant as well as meet ups in the community. Many friendships are built among participants, and they enjoy getting together in casual settings throughout the Fox Valley. FVMP schedules and advertises meet ups in their newsletter. FVMP friends get together at outings, such as music programs in parks, local plays and performances, museum and art programs, and other area events. • On a Positive Note chorus is another delightful opportunity. The chorus was originally directed
ALZHEIMER’S & DEMENTIA | 13 by the talented Frank G. Rippl. Members practice together once per week and perform at local events and community celebrations. No formal experience is required—only a love for singing. • There are four unique day-long bus trips offered each year. The 2018 bus trips featured travels to Sheboygan’s Kohler Art Center, a boating excursion in Waupaca, Door County adventures, and exploring the Neville Public Museum in Green Bay. Each bus trip includes additional stops along the way as well as lunch and snacks. All activities are shared experiences; care partners and those with memory loss participate together. A regular FVMP participant recently shared her enthusiasm for the rebirth. “I am really happy for the program and for all of us who count on it. These are my friends now, and coming here with mom is good for both of us. Coming to a café where people know what you are dealing with…well, it is a relief to walk in the door.”

HOSPICE CARE Provides pain relief, comfort, symptom management, and supportive services to individuals who usually have six months or less to live. Typically, care is provided by a team consisting of a doctor, nurse, social worker, counselor, chaplain, home health aide, and trained volunteers. The team works together to focus on the dying person’s and their family’s physical, psychological, social, and spiritual needs. This level of care can be provided in one’s residence (collaborating with the home health agencies/medical care), at most hospitals (collaborating with your hospital team), at most nursing homes, and at an in-patient hospice facility.

It’s in the PLANNING BY MELLISSA HANSEN, CSW, EOLD We’re all born into this world, and will all leave it one day. And while we plan for many of life’s events—baby showers, birthdays, graduations, weddings, etc.—we frequently don’t plan for the final one. Why do we find it so hard to talk about death? Why not take control of the decisions so we can make known what’s important to us? What if your family can’t agree or remember your wishes for medical treatments? Are you willing to have a feeding tube or be placed on a respirator to keep you alive? How do you want to leave this world and have your loved ones remember you? Planning can help your end-of-life journey be peaceful, filled with joyful memories, and beautiful for you and your JANUARY–JUNE 2019 JOURNEY OF AGING
family rather than filled with anxiety, confusion, and stress. One of the most common comments I hear from seniors is “I don’t want to be a burden to my family.” Even if you haven’t begun, there’s time to plan for your death. There are many decisions to make and several important documents you need to have in place while you’re still healthy. Planning sooner means it’s easier to make good decisions because you’re more capable of understanding them. You don’t want to wait until you’re ill because it will affect how good your decisions will be. Planning ahead also takes the burden off you and your family during a crisis situation, which relieves stress and anxiety. When we’re not feeling well,
we should be visiting with loved ones, making lasting memories, and enjoying the time we have left. The final journey should be a time of comfort. I represent Final Journey Companions, LLC, in the Fox Valley area, and am honored to be an end-oflife doula. End-of-life doula services are nonmedical, holistic approaches to end-of-life care. The goal is to act as a companion so people aren’t afraid, alone, or unprepared for death. We provide a thoughtful, nonjudgmental, and proactive approach to supporting, guiding, and empowering a client to the best quality end-of-life care. Each doula’s background may be different, but all work with clients and

their families through the process in a mindful and attentive manner. End-of-life doulas do not replace homecare staff and hospice staff; we collaborate in providing care. Ask your doula if they are multicredentialed and how they can assist you. Being a state-certified social worker, I have hospice experience that enables me to help with clients’ planning stages, putting important decisions into place while they’re still healthy. Should you need an end-of-life doula’s services when death is near, that’s okay too. We can help you put into place what you need based upon what you want. As part of my program, I also extend services to significant others and family members as needed to help them adjust to life without their loved one. I’ve been helping seniors for over 15 years in the healthcare industry in a variety of venues. It’s my honor and pleasure to be included in someone’s journey. Everybody’s idea of what our end journey should look like is unique. Planning gives you the power to have your decisions in place and not someone else’s.

HEARING HEALTHCARE BY ELIZABETH LANDE
“Hearing loss can cause breakdowns in communication, leading to stress, isolation, and frustration, which impact various aspects of our lives,” says Dr. Sarah Andersen, audiologist with Avada Hearing Care Centers. There are many misconceptions, so we asked Dr. Andersen to share her insights.
Does hearing loss impact a lot of people? According to the Hearing Loss Association of America, just under 50 million Americans have some hearing loss. Hearing loss touches JANUARY–JUNE 2019 JOURNEY OF AGING
not only the individual who suffers from it, but also their family, friends, and anyone they interact with.

• Individual isn’t engaging in conversations where more than one person is speaking.
How does hearing loss impact someone’s life? Not being able to hear means certain social cues may be missed and, as a result, an individual may retreat from their social life. People with hearing loss may not realize they aren’t hearing little things, such as birds singing or the wind in the trees. Often, people with age-related hearing loss lose the ability to hear higher-pitched voices, so they miss getting full conversations.
How can you help a family member who struggles with hearing? Encourage a hearing test by an audiologist or hearing care provider. Also, use communication strategies: • Look at the individual when speaking. • Slow your rate of speech but don’t elongate or overexaggerate words; instead add pauses between each word so that what’s being said can be processed. • Rephrase a sentence rather than simply repeating it multiple times. • Speak in a well-lit environment so the individual can clearly see your face.
What can be done to help someone with hearing loss? In many cases, hearing aids and communication strategies help. Hearing aids can take time to get used to, so be patient with the individual. Communication can be challenging with even mild hearing loss, especially in noisy environments or places where a lot is going on. For important conversations,
find a quiet place where you can both focus on listening.

Give Up THE GUILT
MOVING A PARENT INTO ASSISTED LIVING BY THERESA PICHELMEYER, EdD, RN, MPA We often feel guilty over the process of planning and moving aging parents into an assisted-living facility. Family and societal pressures have wrongly characterized these moves as abandonment, disloyalty, and laziness. How can we shape a healthier, more peaceful and realistic perspective?
The Reality of Your New Role As grown children, we continue to rely on our parents as caregivers, problem-solvers, and a safe shelter in a storm. Gradually, things begin to change, and we realize roles are shifting. A home workshop project that dad used to run with JANUARY–JUNE 2019 JOURNEY OF AGING
you as his assistant now has you taking the lead. Or you’re now going to medical appointments with mom to take notes and be her advocate, rather than her doing it for you. Many adult children find this reversal unnerving. They feel vulnerable because they’ve lost some feelings of stability and normalcy. It’s a natural evolution for children to begin caretaking for their aging parents, and it would be wrong to assume otherwise. Frailty, illness, and dementia will continue regardless of your attempts to stop their progression. And as your
relationship changes, don’t try to fix it. You’ll be overwhelmed by feelings of failure and aggravation if you to try make things the way they used to be. With help, you can keep your parents’ options open. It’s difficult to predict the progression of natural aging or a chronic disease, and a fall or health emergency immediately narrows the options for getting care in a setting of their choice. A decision to move a parent into a safe, comfortable apartment is a decision to accept reality. Don’t wait for a broken hip to direct decisions for you.
RETIREMENT HOUSING & LONG-TERM CARE | 35 When it Feels Like You’re Giving Up Moving a parent may feel like an admission that you can no longer care for them. This can be especially true if others view the move as you simply lightening your caregiving load. But why shouldn’t you? Few parents want their children to give up their lives. If they do, reject that premise as unfair and unrealistic. Just as your parents cared for you when you were young, you’re doing your best with the information and resources available. The life skills your parents taught you will help you navigate this change, and your commitment to them is a result of their commitment to you. Even after the decision has been made, doubt can linger. Did you ask enough questions? Was it clean? Did you check the facility’s state record for infractions? Did you seek people’s opinions? Anyone who has been through this arduous process knows it’s not for the faintof-heart. It’s hardly giving up; it’s stepping up.

What Do I Do Now? After your parent moves into their new home, your responsibilities change but do not evaporate. Continue to advocate for your parent and be visible to other residents and staff. Visit and bring the kids and grandkids. Help decorate their new apartment with meaningful furniture and pictures from their prior home. Introduce them to residents and staff, especially if they’ve become isolated in their previous home due to illness or immobility. They may complain but try not to engage; instead, redirect the conversation. Your visits will no longer be focused on a to-do list. When you share the care, it frees you up to enjoy your parent without being physically and emotionally worn out. Joy can once again blossom in your relationship, and with this newfound joy, any guilt will slip away.
But I Promised “I promised them I’d never put them in a home.” Ask yourself what you were really saying when you made that promise. Were you saying you would always care for them, never abandon them, and keep them at the heart of the family? Your parent may have been working off a set of outdated impressions—perhaps a 1970s-style nursing home without the activities, décor, and dining options of today’s senior living communities. The decision to help your parent find a new place to live is in keeping with the spirit of the promise you made. In fact, it will go a long way to preserve a loving parentchild relationship. You won’t be

ASSISTED LIVING - CBRF Community Based Residential Facility (CBRF). Licensing renewals and dates vary—please reference dhs.wisconsin .gov for information that is updated quarterly. Designed specifically for aging adults needing more advanced care, CBRFs are appropriate for individuals with general/frail requirements. They provide up to 28 hours per week of personalized supportive services, such as meals, health monitoring, medication management, housekeeping, and laundry similar to RCACs but also includes no more than three hours per week of licensed nursing care. CBRFs can work for those with Alzheimer’srelated dementias or other ailments requiring more in-depth monitoring.

The familiarity of reaching for a countertop, dresser, or favorite chair is preprogramed safety. While it’s important to put safety above aesthetics, try to provide as much choice as possible. Remind your loved one that the goal is to keep them in their home. Arranging

help with cleaning or yard work may be nonnegotiable, but which cupboards get rearranged and what furniture is moved requires artful discussion. Look for the wins, and listen to objections. The chair you think should go may be the one remaining reminder of their spouse.
Seek Peace as a Caregiver As caregivers, we often spend a lot of time anticipating and worrying our loved one will fall or do something nonsensical. Arguments about potentially dangerous tasks, like driving, using step stools, and cooking, can be common. The worrying can be exhausting, as can the controlling. As caring families, we must remember that our loved ones are adults with the freedom to make these decisions. Acceptance of that can allow your peace, their happiness, and reduce conflict.
