A Different Take on SHREDDING BY JEAN LONG MANTEUFEL Do you have a handy little shredder at home? Those pint-sized shredders are perfect for your weekly shredding. Of course, it’s not always easy to keep up on, and sometimes we amass stacks of papers we tell ourselves we’ll get to later. Then when our loved ones come over, we spend that time cleaning up and getting things in order rather than enjoying the time with conversation and maybe a cocktail or two. Let’s see if we can simplify things. First off, credit card offers and outdated medical sheets you get on every doctor visit…yeah, you can shred those. And any receipts or banking deposit slips JULY–DECEMBER 2019 JOURNEY OF AGING

you’ve checked against your credit card or bank statement can be shredded as well (assuming they’re not needed for tax purposes or warranties). With all the threats to identity theft, you’ll want to be sure to shred anything with your name, address, birthdate, signature, or identifiable account or social security information on it. Now you’ve caught up and are staying ahead of that mountain of stuff that comes through your door, but why not make it easier on yourself. For bills, consider electronic payments or utilize “bill pay” through your bank or credit union—they’ll even help you set it up.

It’s great for regular bills, like insurance, water, electric, mortgage, cable, phone, newspaper, and church donations. I have a billing account at my financial institution, and with just one deposit a month, all those regular bills are taken care of. Not only does it save me work, but it’s more secure because my account information doesn’t go out on checks. The bonus is that it’s all done automatically. Thompson Center on LOURDES BY GREG BELL For decades the Thompson Senior Center in downtown Appleton provided a welcome service to the areaâ&#x20AC;&#x2122;s aging adult population until a rather sudden closing in early spring 2016. It was heartbreaking for a significant portion of Fox Cities seniors, who were left in the lurch without the place that had become so much a part of their lives. Fortunately, a small group of concerned citizens realized the vacuum closing the senior

center would create and immediately launched an effort to relocate. By April 2016, nearly all activities were reinstated at the new Thompson Center on Lourdes (TCoL), hosted in a former elementary school on the St. Bernadette Parish Center campus. TCoL agreed on a five-year renewable lease that allowed for the new senior center to plan for the future as they recruited

an advisory board of directors, hired staff, and everything else theyâ&#x20AC;&#x2122;d need for long-term success. The new 501(c) (3) nonprofit certification was granted in May 2018. Fundraising has been very successful through a very generous group of individuals, foundations, and corporations. We can now report that TCoL has over 2,000 patrons that use the venue annually, JOURNEYOFAGING.COM

In a collaboration initiative with St. Paul Elder Services, TCoL launched operation Thompson Community Services in July 2018, an outreach program designed to deliver a social serving ministry to seniors vulnerable to the hardships associated with aging and potential difficulties of living independently in private homes. The mission is dedicated to aiding in the mental, physical, and emotional wellbeing of our older adult population. Staff and volunteers share their passion for the mission and will be providing a comfortable, educational, social, and recreational environment that strives to empower and enrich the lives of community elders while enhancing their personal growth.

It’s also a place that helps keep our elders from falling into isolation and depression. We are here as a home away from home to help lift their spirits so they can enjoy life to the fullest. Test after test, doctor after doctor, and still no relief. Those with chronic pain know the cycle well. This seemingly never-ending search for relief from chronic pain can breed frustration, anger, and even outright depression. It can also strain one of the most important things in life: relationships. But, just like with every other aspect of pain, it’s all a matter of finding the right treatment.

Relationship Stressors According to Advanced Pain Management licensed psychologist Mary Papandria, “Oftentimes, the pain patient becomes fixated on their pain. That’s all they talk about, think about, focus on. Their world becomes smaller and smaller … [and] they have little time or attention for other people in their lives.” As a result, their loved ones can feel neglected and unimportant.

According to a Journal of Pain review, as pain sufferers become more isolated from those they love, and more psychosocially impaired in general, spouses can become less satisfied and begin to view their marriage as maladjusted.

tend to view treatments as not being effective and see their pain as worse.” Their negative emotional state could also lead them to focus too heavily on their pain, causing them to take notice of every twinge and spasm, and making them think treatments aren’t helping.

These extra responsibilities, oftentimes incredibly overwhelming, can create a sense of resentment toward the pain sufferer, like he or she is using their pain as an excuse. And the individual in pain can often feel like a burden on their partner.

Since pain, emotions, and relationships are inextricably linked, to effectively treat one aspect you have to treat them all. That’s where a multimodal approach comes in.

“Furthermore,” says Papandria, “who the pain patient was before the pain may have disappeared. They aren’t themselves anymore, and the spouse may feel that they don’t know them anymore.”

The Effects of Relationship Discord All of these factors can create tension, frustration, and anger as well as consequences beyond a relationship. For instance, husbands of patients with chronic pain, compared to those married to women without pain, report loneliness, high stress levels, low activity levels, and fatigue in addition to a decline in marital satisfaction. But more than that, relationship turmoil can have an impact on pain levels and the effectiveness of treatment. In a study from the journal Behaviour Research and Therapy, one year after completing a pain program, those with nonsupportive families relied more on pain medications and reported having more pain sites, while those with supportive families had less pain intensity and greater activity levels. Papandria says the negative emotions, depression, and anxiety that oftentimes accompany pain and strained relationships can “all result in a worsened perception of pain, increased disability, and lessened benefits from treatment. … People tend to be pessimistic. They focus on what’s wrong versus what’s right. As a result, they JULY–DECEMBER 2019 JOURNEY OF AGING

Treating the pain sooner rather than later through pain clinics, which now offer so many nonopioid treatments, helps to improve pain and emotions. “If patients have better pain control, then they are going to be less depressed/ anxious and less apt to isolate,” says Papandria. “They will also be less focused on the pain and will have the energy and attention to attend to other things and people.” And by addressing emotional issues and strained relationships, individuals can achieve better treatment outcomes while simultaneously learning how to communicate their fears and needs, how to identify harmful thought patterns, how to set achievable goals, and how to cope with the changing roles that pain has brought to their lives. Most Americans know something about Medicare. To the younger segment of our population, the idea of Medicare may seem abstract—it’s something politicians and newspeople constantly debate, and it’s a deduction on their paychecks. But for those who are getting older, Medicare is a very real part of our long-term health insurance planning. For that reason, it’s important to know what type of plans are available and which is best for you. Medicare is a federal health insurance plan available to people 65+, and there are special-needs plans available to younger individuals with disabilities. Once you become eligible for Medicare, you can enroll in the federal plan, which is also referred to as Original Medicare. Depending on your financial situation, you may have a monthly premium for these insurance benefits. In many cases, the government will deduct the premium co-payment from your social security or other government income. If it’s not deducted there, you may receive a bill in the mail. Original Medicare does not

Original Medicare isn’t the only way to receive benefits. There are private companies that have contracts with the government to provide Parts A, B, and D benefits directly to eligible recipients. Although each company is unique in what they offer, the advantage to choosing a company over Original Medicare may be the extra or supplemental benefits offered above the basic package. These extras may include exercise programs and gym memberships or dental, hearing, and eyewear benefits.

When reviewing options, think about what benefits may be important to you and don’t hesitate to ask questions—you have the right and should fully understand the benefits and, more importantly, costs you might incur as a plan member. Again, many insurance plans—including Medicare—involve a monthly premium payment and potential co-payments for doctor visits, laboratory work, and medications. If you’re a senior or adult with a disability and are on a fixed or very low income, you may qualify for a type of plan that has little to no premium payments or co-payments. These plans are called Dual-Eligible Special Needs Plans and are available to people who qualify for full Medicaid and Medicare benefits. While the best place to start is medicare.gov, also ask family and friends for recommendations based upon their experiences. And, again, ask questions to find the best fit for you. Your plan should match your health and financial needs. Fall is a great time to learn more—you’ll see a lot of commercials and ads for Medicare plans. Know your options and your rights to be ahead of the curve. Happy plan finding!

Once your loved one has been diagnosed with Alzheimer’s disease or another form of dementia, Plan A is typically to keep them in their home for as long as possible. However, there may come a time when those walls won’t support their quality of life any more. Unfortunately, those with dementia do not recognize the need JULY–DECEMBER 2019 JOURNEY OF AGING

for assistance. This is why families need to formulate a Plan B for where their loved one might live when their home situation is no longer a good fit. The first step in the Plan B creation is to ensure that the person with memory loss has healthcare and financial power

of attorney (POA) paperwork. This paperwork should name a primary and a secondary person that will assist in making decisions for the person who cannot make their own. The paperwork does not take effect until two physicians or a physician and a psychologist medically determine the person to be incompetent or

If your loved one doesn’t respond appropriately, they shouldn’t be living alone. • Family is unable to provide the necessary level of care. If caregivers become short with a person or if helping the loved one could harm them or the caregiver, it’s time to seek assistance. • The current level of need is too great for family or it’s too difficult to manage others to do it. If your loved one needs 24/7 care, they need reliable people to be there all day, every day, without excuse, absence, or delay. • Emergency and crisis situations have arisen. Your loved one falls, a caregiver is ill, or worse. Sixty-three percent of caregivers pass before the person they’re caring for. Sadly, the last item is the most common reason why a Plan B is enacted. Most families wait until a health crisis occurs to create a Plan B, and then wait until another crisis or myriad of concerns happen before enacting it.

The POA individuals should be involved in creating the Plan B and visit memory care homes that could assist a loved one now and in the future. In doing this, you ensure that if something happens to your loved one or their main care provider, Plan B is in place. Moving is hard, which can lead to it being pushed off. But if the following signs appear, it’s time to implement Plan B.

Plan A is optimal, but when the walls can’t hug or keep your loved one safe, it’s time to realize that the memories can come with them. The main thing is to ensure that your loved one is safe, supported, and smiling, and that can happen when Plan B is in place.
