Hearing loss can make talking on the phone difficult, but CapTel® captioned telephone helps those with hearing loss stay in touch over the phone. If you’re living with hearing loss, you aren’t alone. Nearly one in three adults between 65 and 74 years old has some degree of hearing loss, while nearly one-half of adults 75+ report being hard of hearing. One of the most common concerns for people with hearing loss is staying connected with friends and family, especially over the phone. MARCH–AUGUST 2022 JOURNEY OF AGING
Whether you want to stay in touch with faraway family members, schedule a doctor’s appointment, or plan brunch with a friend, telephone communication is key. If you can’t hear well over the phone, it’s difficult to make plans, and many hard-of-hearing people start avoiding phone conversations out of sheer frustration. Research has shown that social interaction is also an important factor in staying healthy for older adults.
Social isolation can lead to loneliness and depression. But older adults who stay in touch with friends and family are more likely to have lower blood pressure and a possible reduced risk of some types of cancer, osteoporosis, and cardiovascular problems. Staying connected is also important for the mental health of older adults. Fortunately, CapTel is available to help people with hearing loss stay connected and enjoy the telephone again.

CapTel captioned telephone works like any other phone, but it allows users to read captions of what their calls say similar to the way you read captions on television. This lets users support the hearing ability they have by reading captions on the screen to make sure they catch every word the caller says. When a person wants to make a phone call, they just press the captions button on their CapTel phone to be connected to a captioning service at no cost. Then they simply dial the number of the person being called. The call is carried out like any other phone conversation, except that the CapTel user can read what the other person is saying on the phone’s display screen. CapTel captioned telephones allow those with hearing loss to stay in touch with their family and friends and, in doing so, maintain their independence and confidence over the phone.

LET’S TALK ABOUT
CAREGIVER ANGER by Jeff Hamm, MSW, CAPSW
Caregiver anger: a topic almost as taboo as sex. We all experience it at some point in the caregiving journey— whether it’s the repetitions, the resistance, our feelings of isolation, or just the unrelenting tasks of care, we hit a wall. Some of us can stay calm and composed for hours, days, or weeks until that one final refusal to bathe, question about going home, or soaked bed. And MARCH–AUGUST 2022 JOURNEY OF AGING
then the irritability, resentment, and anger come out. Some of us feel comfortable letting the anger boil over. We hit pillows, sit alone in the car and shout, or take a brisk walk. We vent our frustration to others who know and accept us. Some of us hold in and suppress the anger. In fact, we may not even be aware of it until we
hear our irritable response; feel the heat in our face; or notice the clenched fist, stomachache, or headache. We may be as surprised by our sudden anger as the person we care for. It’s a problem when our person with dementia is the target. Our anger can be confusing and scary. If we’ve been suppressing our feelings, our angry

outburst can be sudden and out of proportion to the situation. Sometimes our anger can trigger their frustration and anger, creating the danger of escalation and even violence. In the end, we’re usually left feeling guilty and ashamed about our angry responses, realizing that our loved one is not acting out of choice. Anger is a normal and natural response to the frustrations of dementia and the strains of caregiving. We’re understandably mad at the disease for its impact on our loved one. When caring for a family member, we hate the change in roles and relationship that accompany dementia. We may be angry that others don’t help more or don’t understand the demands of caregiving. We get upset when schedules fall apart and plans go wrong. And we’re likely grieving the many losses associated with dementia, perhaps forgetting that anger is a common and even expected reaction to grief. It may be helpful to think of anger as neither good nor bad, but as a signal.
Our anger is sending a message. It might be telling us that we need a break, that we have some emotional work to do, that we need to adjust our expectations, or that we have reached a turning point in our caregiving. Being aware of our anger early is critical to decoding the message and taking effective steps forward. If we decide that anger is bad and must be ignored or suppressed, we lose the chance to learn. Here are some suggestions when we’re feeling angry.
Step Back,Take a Break Stop and catch your breath emotionally. Go to another room, get a drink, take a quick walk—whatever works. Even giving yourself 90 seconds will allow the anger-related physiological reactions to peak and subside.
Engage Your Thinking Brain Noticing, analyzing, being curious, asking questions, all these can move you out of your emotions and into a more useful problem-solving mode. What triggered the anger? Is it related

to a particular activity, situation, or time of day? Am I upset because my expectations about how things should go or when things should happen are not being met? Do I need to remind myself that I’m dealing with my loved one’s disease and not behaviors they would typically choose?
Don’t Feed the Flames Some of us keep the anger burning by replaying the situation in our minds, imagining angry things we’d like to say, or talking to ourselves about our frustrations and resentments. Instead of allowing the feelings to subside naturally, we feed them with our angry thoughts. Try to notice your thinking and make a different choice.
Get Help Help might mean respite during the day, some extended time away, or talking to a professional. It may mean starting the discussion about facility care. We often need others to remind us that limits and boundaries in caregiving are essential and that self-care is not optional. With the help of others, we can begin to interpret the messages that our caregiver anger is sending.

HOSPICE CARE Provides pain relief, comfort, symptom management, and supportive services to individuals who usually have six months or less to live. Most of us don’t want to think about long-term care; it’s not a fun topic. However, it’s to our benefit to start considering long-term care options sooner rather than later so we can receive the care we want, if it were to become necessary.
What is Long-Term Care? Long-term care is typically needed when someone requires the help of someone else to perform activities of daily living; this person will need to receive long-term care to stay safe and remain healthy for as long as possible. often provided in a patient’s home or in a facility, such as assisted living or a nursing home.
Attendance benefit. Medicare and the VA only cover a small percentage of cases, which leaves LTCI and Medicaid as the two primary sources.
Who Pays for Long-Term Care? Long-term care is very expensive. Unfortunately, a large percentage of seniors will go broke paying for care out of pocket before subsidizing their costs. Paying out of pocket may make it difficult to leave any assets to your spouse, children, or grandchildren. Long-term care costs can be subsidized by Medicare, Medicaid, long-term care insurance (LTCI), or the VA Aid and
Long-Term Care Insurance (LTCI) LTCI covers many long-term care options, like nursing homes, assisted living, and home health care. The price of LTCI has risen in recent years, but long-term care has also continued to go up; if you’re able to plan out far enough ahead, LTCI costs will actually be less than what the potential cost of a nursing home or assisted-living facility will be.
ESSENTIAL PREPARATION | 27 LTCI is best for those that: • Are able to preplan. You’re healthy and not in need of immediate care. • Want to protect their assets. You may have a large estate or many assets that you’d rather pass down to your spouse or loved ones. • Want to create a legacy. You hope to leave your children and grandchildren with financial stability with your assets.
What About Medicare? Many assume Medicare will help them pay for the care they need in the future because Medicare is only, with few exceptions, for those 65+. However, this federal program will not pay for longterm care and was designed for some medical costs for short-term stays in a hospital or nursing home for hospitalrelated care.
What about Medicaid?

Medicaid pays for over 60 percent of all long-term care costs. This federal program was designed specifically for long-term care. The only problem is that you have to fit within Medicaid’s strict guidelines. It’s important to work with a planning professional who specializes in long-term care and knows Medicaid’s rules.
This innovative product helps you shift items from the asset column into the income column in order to help you qualify for benefits. Once you’re within Medicaid limits, Medicaid will subsidize the cost of care and any at-risk assets will be considered protected.
There are strategies that seniors can use to help them achieve Medicaid eligibility without depleting all their countable assets. Legal or financial products are often involved.
• Reduce assisted-living or nursing-home costs. • Accelerate Medicaid benefits for your loved one. • Spend down excess countable assets. • Preserve your loved one’s nest egg.
Using an Irrevocable Trust If you’re planning at least five years before needing care, then an irrevocable trust is a great way to plan on using Medicaid coverages in advance. Essentially, a trust allows you to wrapper your assets and shield them from creditors, so if care becomes necessary, your asset list has already been minimized outside of the five-year look-back period.

Universal Design Principles
WILL OPTIMIZE YOUR LIVING SPACE by Chad Speight
Universal Design supports aging in place gracefully for family members and accommodations for guests who are physically challenged in some way by incorporating common sense solutions for better living. The idea is to create spaces that are easier and safer to live in, allowing all occupants to maintain maximum mobility and independence. When considering the function and design of your living spaces, it’s helpful to review the 7 Principles of Universal Design as articulated by the National Association of the Remodeling Industry (NARI). ESSENTIAL PREPARATION | 31 For example, to incorporate Universal Design principles in your bathroom, access should be as barrier free as possible. A main level floor full bathroom versus a powder room is desirable for long-term accessibility. The best option is usually a zero-clearance threshold shower with niches for storage and a secured seat option. Properly designed and installed, these showers can turn the entire room into a waterproofed area—even an overflowing toilet will simply flow down the shower drain.

Toilets can have automatic flushing mechanisms and should be situated in the larger space area versus a separate enclosed space to allow someone to assist from one side if needed. A chairheight, or comfort-height, toilet or bidet is an excellent option that requires less bending to use and is easier to get on and off of. Faucets and valves with twist handles should be replaced with single-handle levers or single-handle shower control for better manipulation. They should also be positioned for easy reach and control. A handheld shower on a vertical bar provides excellent height control and flexibility in showering options.

Select a sink vanity with a readily accessible height and plan for essential item storage within easy reach. Floating vanities can be an ideal option to allow for positioning the sink at the best height and providing space to maneuver around and under. Ensure closed storage areas have functional and decorative door and drawer handles for easier access to contents.
Remember to consider safety first. Grab bars are available in many styles/ finishes for seamless integration into a stylish bath design. It’s prudent to install blocking for grab bars when building or remodeling even if you don’t plan on installing them just yet. Surfaces should be easy to clean and durable. Take care in selecting nonslip flooring (especially where surfaces can get wet) and properly installing the flooring material to prevent trip hazards or obstacles for mobility devices.
Technology helps boost access when combined with Universal Design principles. A humidity-sensing bath vent can be a smart addition since the fan can turn on and off automatically to stop moisture from accumulating and help prevent mold. Lighting should be designed for both general access and specific tasks. Think in terms of layering your lighting with good general lighting, bright task lighting for self-care, and easy to engage low-light options for dark interior spaces or late-night usage.
If possible, the floorplan should allow for a threshold-free, wide-access doorway into the space (36-inch minimum is optimal) and enough room to turn around a mobility-assistance device. A pocket door or outward swing door can allow for better access if the door were to be accidentally

to Reduce Food Waste by Andy Gricevich
Read up on food waste in the United States and you may be shocked. Our nation leads the world in the quantity of food that ends up in the garbage. It’s estimated that 80 billion pounds of food per year—between 30 and 40 percent of the national supply—goes to waste, and the majority of that food is still edible! In a society plagued by food insecurity in many communities, the loss of such MARCH–AUGUST 2022 JOURNEY OF AGING
abundance signals clear ethical and economic problems. Food waste is also a very serious ecological issue. When food gets discarded, the resources required to produce, process, and ship it are effectively squandered. Once it’s in the landfill, food waste produces a number of the primary greenhouse gases involved in global climate change. It also releases large quantities of nitrogen, which runs off
into watersheds, encouraging blooms of algae that choke streams, rivers, lakes, and oceans. Here in Dane County, around 20 percent of what goes into landfills is food waste, and we’re in an important spot on the continent for water. Truly solving the problem will mean big changes in our food system, market economy, and society. As individual eaters, there’s a lot we

can do to minimize food waste in our own lives.

Plan Ahead
Home Composting
The simplest step may be the most effective, and also the hardest for many of us to do: plan meals and eat leftovers. Having a plan for the ingredients we buy helps us use everything that goes into the fridge. Planning for particular types of dishes can make this easier; just about anything, including last night’s leftovers, can go into omelets, soups, stews, pasta dishes, and stir-fries.
It’s pretty easy for many individuals and households to get started with composting as well. A home compost pile is a much better destination for fruit and vegetable matter, coffee grounds, and eggshells than the landfill. Learn more about composting at cityofmadison.com/streets/compost.
For soups and stews, it’s nice to have some good stock or broth on hand as well. That can be from a box or you can easily make your own. Just keep a freezer bag or tupperware container in the freezer; whenever you have vegetable scraps or animal bones, toss them in. When the container fills up, dump it into a pot, cover with water, add some salt, and simmer. After your meal, freeze the leftovers in jars or other containers (this, of course, adds thawing the stock to your meal planning). Freezing leftover
Sell By—Use By We’d also end up wasting a lot less food if we adopt a more skeptical attitude toward those dates on the packaged foods we buy. Remember that the requirements and recommendations for these dates, while based on conscientious efforts to promote food safety, are very conservative, and “best by” is distinct from “sell by” and “use by.” For foods without expiration dates, your senses and your ingenuity are enough to go on. A fruit that’s developed some darker spots won’t kill you, and in some cases, that’s a sign of
the most perfect flavor. There’s nothing dangerous about slightly wilted greens or carrots.
What We Buy What we choose to buy is also a factor in food-waste reduction. Local produce will generally be fresher and less subject to damage over the course of multiple stages of storage and shipping than product grown far away. Buying whole chickens, rather than packages of breasts, guarantees that none of an animal will go to waste, and going for less popular cuts contributes to a similar goal. We can also consciously choose products that may be less visually perfect but have lost none of their genuine flavors, textures, or nutrition, stepping up to the challenge of changing the aesthetics of the ways we value food. Food may be a huge industry, but it’s still very much dependent on the choices of individuals, and because we all have to eat, every one of us is faced with those choices in stores, at restaurants, and at home. Together, we can do a lot to help solve the problem of food waste, moving our food system toward a place of balance with nature and participation in collective well-being.

FUNNY NAME, SERIOUS FUN by Beth Johnson
If you’ve recently driven past your local tennis courts and done a double take noticing the people on the courts aren’t playing tennis, you’re not alone in asking, “What are they doing?” They’re playing pickleball, one of the fastest-growing sports around. It’s a combination of tennis, ping-pong, and badminton. Due to its booming popularity, some cities are even reconfiguring their tennis courts into pickleball courts.
Verona converted its tennis courts at Harriet Park at 414 Mary Lou Street into pickleball courts in 2021. Pickleball is played on a court the size of a badminton court with a net about two inches lower than a tennis net using what look like oversized pingpong paddles and a wiffle-type ball. Very little equipment is needed to play, which is just one reason people
are attracted to the sport. The court is smaller than a tennis court, so some people gravitate to pickleball because of the lower physical strain. It’s known for its ease of learning and appeal to all ages, offering a multigenerational activity grandparents can even play with their grandchildren. The main point of pickleball is to hit the ball back and forth over a net.
HEALTH | 51 It’s really that simple. The majority of the game is played close to the net, meaning less time running around and longer rallies. With even a basic understanding of the game, anyone can start to play immediately without lessons. The sport is not about power or speed exclusively. It’s a sport of strategy and patience.
Why did you start playing pickleball?
Many people are drawn to the sport because of its social atmosphere and camaraderie. Teammates and opponents are physically closer to one another on a smaller court, making for great interaction, jokes and wisecracks, and encouragement between competitors. In fact, it’s considered customary to commend your opponent’s shot in the middle of a game.
Why do you think it has become so popular?
LYNN: I was invited to play by a friend I golf with. I love to get exercise doing something fun and thought I’d give it a try.
KAREN: We like to stay active and heard it was a great fun way to exercise.
LYNN: As you get older, it’s harder to play the sports we loved as kids. Pickleball can be fun and competitive and is easier on the body than other active sports. You still get a good workout, but you don’t have to do a lot of running.
KAREN: It’s less stressful than tennis Lynn and Karen are both new to the sport. Here’s what they say:
When did you start playing pickleball? LYNN: I just started playing pickleball
and easier on the joints. It’s also a good activity for couples. It’s a very inexpensive sport. All you need is a pickleball paddle, a couple of pickleballs, and an opponent.

KAREN: Not hard at all. Of course there’s always YouTube to learn the rules of the game. Lots of community recreational departments offer lessons. The rules are fairly simple: if you serve, you’re the only one who can score; otherwise you lose the serve, and the other person gets a chance to score. First one to reach 11 points with a twopoint lead wins. The court design is easy to understand.
What kind of shape do you have to be in to play? LYNN: I would say you need to be in medium shape to play. Like I said earlier, it’s not all that much running, but there is exertion to the point where one can work up a good sweat. I would recommend anyone to just try playing. I like the competition, but others may want to play just for fun. Either way it gets you moving, which is good for our health.
KAREN: Anyone can play. The intensity of the play can be adapted to the players. The court is smaller than a tennis court, so there’s not as much running.

Choosing Emergency OR URGENT CARE by Tina Strandlie, RN
When you or a loved one is experiencing a sudden illness or injury, it can be confusing to know where to get care. If you’re like most people, you’d rather avoid an unnecessary trip to the hospital ER.
Urgent Care This rapidly growing area of healthcare provides easy access to care with typically shorter wait times than busy emergency departments. In fact, Stoughton Health opened our MARCH–AUGUST 2022 JOURNEY OF AGING
McFarland Urgent Care Clinic last year in addition to our Oregon and Stoughton locations, and other healthcare providers are responding similarly. Urgent care is designed to treat urgent medical needs that are not life threatening, but still require prompt attention. Urgent care clinics are frequently open during the day, but also in the evening and weekends when your doctor’s office is closed. They’re
intended to fill the gap between care provided by your physician’s office hours and the emergency department. Typical minor illnesses or injuries that are appropriate for urgent care include: • • • • •
Ear pain Eye irritation Fever or flu Minor cuts, scrapes, and bruises Minor broken bones and fractures (i.e. fingers, toes)

Emergency Care For patients with symptoms that may indicate a life-threatening illness or injury. Some examples of these are: • Alcohol or drug intoxication or overdose • Amputations or severe lacerations • Bone fractures • Facial or eye trauma • Fever in newborn (less than 3 months old) • Gunshot or knife wounds • Heavy, uncontrolled bleeding • Loss of consciousness • Moderate to severe burns • Poisoning • Pregnancy-related problems • Seizures or convulsions • Serious head, neck, or back injury • Severe abdominal pain • Severe chest pain
• Severe headache • Shortness of breath or difficulty breathing • Signs of heart attack or stroke • Suicidal or homicidal feelings Emergency care is provided in hospitals or, in some cases, standalone emergency care facilities and are open 24/7.
Level of Care If you go to a hospital that offers both emergency and urgent care services, such as Stoughton Hospital, the staff will first conduct a preliminary assessment to determine the appropriate level of care. This assessment is made based on the presenting symptoms, and charges are based on the level of care needed and delivered. For example, if the doctor feels you need to have an EKG, ultrasound, MRI, or CT scan to help diagnose your problem, this would likely make your visit an emergency care visit.
Virtual Care or Nurse Call Line Additional healthcare options, such as virtual visits and nurse call lines,
are becoming more common to help patients receive care or healthcare advice from the comfort of their own home. Don’t Put That in Your Mouth PET POISON AWARENESS by Lisa Bernard
“The biggest things we hear from pet owners is that they didn’t know something was toxic or they didn’t realize how a small amount of something could be problematic,” says Dr. Erica Hellestad, veterinarian and owner at Whole Pet Veterinary Clinic in Madison. In fact, there are many items in your home that could be problematic for your pets. Whole Pet Veterinary Clinic receives one to two calls per week about possible pet toxicity, but, depending on what and how much was ingested, not every case needs treatment. Typically, the clinic sees younger dogs and cats for pet poisoning because

they’re more likely to get into things. “Dogs are more likely to ingest things they shouldn’t,” says Erica. While medications and food seem to be the bigger vices for dogs, plants tend to be the most common culprit for cats. When a pet suffers from poisoning, you may see such signs as vomiting, lethargy or excitability, and twitching.
Whatever your pet ingested, Erica advises pet parents “please be honest with your vet. Things like marijuana toxicity have some pretty classic symptoms, and it saves a lot of time (and likely money) if you’re honest with us up front versus us having to guess at what’s going on.”
If you’re not sure about the toxicity of the item your pet ingested, call the American Society for the Prevention of Cruelty to Animals (ASPCA) Animal Poison Control Center (APCC) hotline at (888) 426-4435 or your veterinarian.
Top Toxic Offenders Each year, the APCC compiles a list of the most commonly reported pet toxins. The top two in 2020 were over-the-counter medications and human prescription medications, respectively.

To protect your pet from these dangers, keep human medications in childproof packaging in a closed cupboard your pet cannot reach. If you have medication in a purse or backpack, either keep the bag out of your pet’s reach or make sure the medicine is in a securely closed pocket of the bag. “Don’t give human meds to pets without guidance from your vet,” Erica adds. Human food ranks third on APCC’s list, with chocolate taking the fourth spot. Because the APCC handles so many cases involving chocolate exposure— nearly 76 cases per day—it earned its own spot on the list. “Lots of people are unaware something like a pack of sugar free gum can cause
big problems,” says Erica. Gum, as well as some candies, peanut butters, and baked goods, contain the sweetener xylitol. Xylitol can cause insulin release that can lead to liver failure within days, according to the ASPCA. Plants can add life and color to a room, but for some pets, they can become a stomach-upsetting temptation. “There are a lot of plants that can cause a bit of GI upset but aren’t truly toxic,” says Erica. “[However,] lilies are incredibly toxic to cats—even just ingesting some pollen can put them into kidney failure. Best to not have them in the house at all.” Make sure you check the plant’s toxicity before bringing it home or keep it in a room that your pets don’t access.
When cleaning your home, make sure the area cleaned is dry to the touch before allowing pets access to the area. Just because a cleaner says it’s “all natural” doesn’t mean it’s safe for pets. “Fumes and scents can be problematic for our pets too; they have a much more sensitive sense of smell,” says Erica. “Cats are really sensitive to certain essential oils used in some cleaners.” Depending on the oil’s concentration, essential oil diffusers could pose issues for your pet as well. But, according to the APCC, using an oil diffuser for a short period in an area your pet doesn’t access shouldn’t be an issue. Overall, the best practice in dealing with possible pet poisoning situations is prevention. Take a careful look around your house to identify potential problem items and take steps to pet proof your home.
