Am I Welcome Here? PROJECT BY MICHAL OSIER, MA, LPC When I started as the LGBT Senior Advocate at OutReach LGBT Community Center in Madison, several people asked me for help in finding a safe, welcoming residence to meet the growing needs of their aging LGBT parent or partner. While Dane County has a wealth of senior residential options, including assisted living and memory care facilities, skilled nursing homes, and adult family homes, I quickly realized there was no information concerning policies about, and
knowledge of, LGBT elders’ unique needs and challenges. I had some anecdotal information, but nothing to base a recommendation on; this question had not yet been systematically asked, nor had training been offered. And more, there were no shared benchmarks for LGBT cultural competence. What would organizations need to know in order to be able to recognize that LGBT seniors’ life experiences were
SEPTEMBER 2018–FEBRUARY 2019 JOURNEY OF AGING
different in many regards from that of their heterosexual peers? It’s crucial to understand that LGBT seniors born between the 1920s and the 1950s came of age during a time when discrimination and bigotry against LGBT people was universal, unrelenting, and unquestioned. Even now, safety from prejudice remains a strong and valid concern. The new AARP study, Maintaining Dignity, found that 67 percent of LGBT respondents 45+ were concerned about neglect, 62
AGING & CAREGIVER RESOURCES | 11 percent about abuse, 61 percent about verbal/physical harassment, and 61 percent that they would be refused access to needed services, like in-home assistants and inresidential care. The survey “found high demand for long-term care providers who actively welcome the LGBT community and demonstrate awareness and knowledge of the specific needs of LGBT adults as they age. More than 8 in 10 … say they would feel more comfortable with providers who are specifically trained in LGBT patient needs (88 percent).” OutReach’s Senior Advocacy Program’s Am I Welcome Here? Project is my response. I’d hoped to reach all of the senior residences and care facilities around Dane County. When I learned there are almost 300, I decided to focus on assisted living and memory care residences because of what going into assisted living means: the aging person can no longer care for themselves in the self-sufficient way that characterizes much adult life. To need assistance in the tasks of daily living is to be dependent on other people whom you do not know in ways that you never were before. This represents a huge loss to anyone, but for LGBT people, this may well mean dependency on individuals who may disapprove of, dislike, or even be hostile toward them, just because they’re LGBT. A survey with a cover letter explaining the Am I Welcome Here? Project’s purpose and goals has been sent out twice to almost 90 facilities. It asks facility directors to assess LGBT cultural competence in three areas: overall knowledge about LGBT elders’ histories and how lifelong discrimination has
affected them, LGBT terminology and culture, and specific action steps to make their facility LGBT affirming, from adding additional options to gender and maritalstatus questions on forms to putting LGBT images and information into their décor and resource libraries to making antidiscrimination policies known to staff and residents alike. I offer staff training in each of these areas, and participating organizations are given a free listing in OutReach’s annual directory to show they have engaged in building their LGBT cultural competence. Cultural shifts take place steadily but gradually. In addition to training, my goal is to bring the discussion about LGBT elders’ needs, wants, and realities to the larger community. LGBT older adults have made countless contributions, large and small, to their communities throughout their lives. When they’re no longer able to fully care for themselves, they deserve to be able to call the place they move to their new home, not their new closet.
Michal Osier, MA, LPC, is the LGBT Senior Advocate at OutReach LGBT Community Center. She’s a recently retired community mental health psychotherapist and a lifelong writer who lives in Madison with her beloved wife, Nancy Hutson, and their excellent dog, Rosie.

The Ladies POETRY SOCIETY BY ELIZABETH HUMPHRIES, RN, MSN, CNM
Vicki Teigen has been a CNA in homecare and hospice for years. As a writer and poet, she carries her craft with her as she works to create comfort, ease, and companionship with her senior clients. This interview explores the wonder of a shared legacy that comes from lovingly caring for elders and listening to their life stories. How did writing poetry with your senior clients come about? “When we started sharing our literary history and they discovered I am a writer as well, I was quickly encouraged to start writing poetry at my visits … and last fall we
ended up with a shared collection of poems inspired by my visits. My one client now calls it ‘The Ladies Poetry Society,’ and she was also the editor and publisher.” What kinds of things are you writing about together? “In our everyday conversations about life, their adventures, relationships, their travels around the world … and my life, which was so much different than theirs … just sharing our stories.” How do you see the writing working in their lives as something not only enjoyable,

but as possibly therapeutic for them? “It’s definitely therapeutic for them because it surrounds them with creativity, which they love. They enjoy poetry, art—so many works of art that are beautiful keep them feeling alive and enjoying life.” Do you see any benefits while doing the writing with them in terms of memory and cognitive benefits for the brain? Do you think it could be beneficial in cases of memory loss and dementia care? “The writing is important because it helps one of my own clients to

But it has been a way for him to dictate his memories and stories with me, and he often adds his own lines in the poetry itself. It’s a joint effort and joy. I can see a progression of memory loss; however, the loss is lessened by the joint effort and wonderful warmth
AGING & CAREGIVER RESOURCES | 17 and friendship that we share in this joint activity we do together.” How do you see this activity being used in caregiving for others, with other seniors, or even in a group setting? “Oh, yes, it can be used with most every senior in almost any setting, writing not only poetry together, but in writing life stories, legacies, their own personal story recorded. It gives a feeling of accomplishment and a sense that things in their life are complete, whole, and recorded for future generations.” How would you help other caregivers learn this as a skill to use with their clients and family members? “I would encourage other caregivers to just sit down with their client friends or senior family members and just record some thoughts using a simple recording app on a smartphone or a recording device, and transcribe it into a poem, a story, or a journal piece. Share it with the client and form it into a piece of writing together, and start from there.” Do you see this as potentially a growing movement—a tool for people to use with aging folks that serves not only to help with life review and legacy building, but helps to stimulate the mind and memory recovery? “The process brings the memories alive again by sharing them and creating beautiful writing together. As we know, music sparks places in the brain that help to awaken the mind, and can be the only way people with advanced dementia can connect and communicate. Writing is similar, the creative process. This can be another important tool that people share to awaken the mind and improve quality of life.”
What do you say to people who complain about “those same old stories, again and again, I’ve heard them a thousand times.” “I would say this: those stories are enriching our lives. That is history and their life in story, and if we don’t hear them and get them recorded how would we ever pass them down and share them for future generations? It’s important for everyone to feel they have a voice in history.” Poems published with permission of author.

You Can Teach
AN OLD DOG NEW TRICKS BY BETH JOHNSON
Earlier this year, Science Daily reported that researchers showed for the first time that healthy older men and women can generate just as many new brain cells as younger people.2 Related research says, “White matter in the older brain is more plastic than in the younger brain.”1
Village. Neuroscientists have long believed that older people have less of the neural flexibility, or plasticity, required to learn new things.
Until the 1960s, we thought that patterns in the brain were pretty fixed and unable to create new pathways,” says Heather Donald, MS, CCC/SLP, Director of Rehabilitation Services at Oakwood
“For many, many years, we were oblivious to our tremendous creative capabilities. We had no idea that our brains were changing in response to our actions and attitudes every day of our lives. We unconsciously

and randomly shaped our brains and, thus, our future because we believed we had an immutable brain that was primarily genetically determined. “The implications of neuroplasticity are extremely significant—we have the capability to keep our brain sharp, effective, and capable of learning new skills if we protect it from damage and provide it with stimulation and beneficial fuel.” What are some ways to induce neuroplasticity in our daily lives?
ALZHEIMER’S & DEMENTIA | 25 Fortunately, there are several. According to Heather, research looked at how we can continue to change our brains and keep them strong. There are six evidencebased strategies or practices that we can incorporate into our daily living: attitude, learning, exercise, diet, socializing, and rest.
ATTITUDE Being grateful can reprogram your brain to begin to look for things to be grateful for.
LEARNING As we age, choosing what we want to learn instead of what we have to learn can ignite the brain.
EXERCISE Research shows that exercise increases blood flow to your brain and induces the growth of new brain cells.
DIET Most people know that their dietary choices affect the health of their body. Heather points out that the brain consumes about 20 percent of the nutrients we ingest. What we eat has a significant impact on our neurotransmitters, the chemicals that carry messages between neurons across synapses.
SOCIALIZING Making meaningful connections with others is not only helpful to our brains, but vital.
REST Learning to still the mind is as important as stimulating the mind. Evidence now shows that practicing mindfulness and meditation decreases cortisol, helps to control blood sugar levels, regulates metabolism, reduces inflammation, and helps to maintain a strong memory for information.

our awareness of earth’s demise that a kinder, gentler way of dying is making a comeback: the green burial. Of course, green burials are nothing new—Native American burial mounds, for example.
SEPTEMBER 2018–FEBRUARY 2019 JOURNEY OF AGING
While cremation is the fastestgrowing choice for one’s final disposition, green burials are quickly becoming recognized as an environmentally safe way to return “dust to dust.” Locally, Natural Path Sanctuary is leading the way.
END OF LIFE | 31 On 25 acres of rural land near Verona, the Sanctuary was established by the Farley Center in June 2011 as a nature preserve burial ground/cemetery with an educational component focused on promoting sustainable end-of-life care choices and burial practices. It’s the first green cemetery in Dane County and one of only a few in the country. Directed by Shedd Farley, son of Gene and Linda Farley, whose bodies are buried there, the Sanctuary continues their legacy of ecological sustainability. Uprooting himself from his life as a homebuilder in Denver for 38 years, Shedd has taken over running the Farley Center. More often than not, he can be found digging three-foot graves, the desired depth for earth to do its work. There are currently around 100 gravesites in the Sanctuary. Finding them can be difficult as above ground markers are limited in size and materials used. The Sanctuary partners with area funeral homes to prepare the body, although families may choose to do so themselves. All burial shrouds and containers are required to be biodegradable, and, of course, embalming and vaults are not allowed. The burial sites are essentially camouflaged by the abundance of nature, leaving the land as undisturbed as possible while the birth/death cycle is endlessly recreated. One cannot talk about Natural Path Sanctuary without mention of Gene and Linda, long-time family physicians who dedicated their lives to healing the world of social and economic injustice. They practiced medicine on the Navajo Reservation in rural upstate New York and in the inner cities of Rochester, NY; Denver, CO; and Kingston, Jamaica, and were
tireless advocates for healthcare coverage.
universal
Believing in the importance of locally grown agriculture, the Farleys made the land accessible to those who had little resources to purchase land. Currently, a diverse group of community-supported agriculture businesses grow produce on land adjacent to the Sanctuary, paying rent with use of equipment. Shedd also has partnered with the Department of Corrections via the Grow Academy, which provides teenagers with a labor experience as a form of community service. Contemplating our demise might be viewed as a meditation on the poetics of life and death. As life offers us more complex choices, we may decide that less is more and that there’s a simpler, environmentally friendly way of returning to our roots as conscious stewards of our planet. Photographs provided by Natural Path Sanctuary. Virginia Rose is a freelance writer of essays, poems, and songs, and a multimedia artist who intertwines music, theatre, performance art, photography, dance, and spoken word. She performs in the musical duo Half Rose Half Nelson. Currently she’s writing a book about her global thrift store shopping experiences, entitled Otra Vase, which is her mantra, philosophy, and brand. Her forte is the epiphany of the costumed body.
HOSPICE CARE Provides pain relief, comfort, symptom management, and supportive services to individuals who usually have six months or less to live. Typically, care is provided by a team consisting of a doctor, nurse, social worker, counselor, chaplain, home health aide, and trained volunteers. The team works together to focus on the dying person’s and their family’s physical, psychological, social, and spiritual needs. This level of care can be provided in one’s residence (collaborating with the home health agencies/medical care), at most hospitals (collaborating with your hospital team), at most nursing homes, and at an inpatient hospice facility.

Conversations About
END-OF-LIFE CARE BY ANN MASSIE NELSON
END-OF-LIFE CARE BEGINS WITH THE CONVERSATION Talking about our wishes for health care at the end of life remains one of the most difficult conversations. According to a recent survey by The Conversation Project, 90 percent of
people believe it’s important to talk with their loved ones about end-oflife medical treatment, yet only 30 percent have had the conversation. A number of tools exist to help people document their decisions
SEPTEMBER 2018–FEBRUARY 2019 JOURNEY OF AGING
regarding medical treatment in the event they become unable to communicate their wishes. The Wisconsin Medical Society offers sample forms for creating written advance directives and appointing a healthcare proxy, available in the
ESSENTIAL PREPARATION | 37 Honoring Choices section of the society’s website. Regrettably, these written documents are often stored in a file drawer or safety deposit box. When faced with making a decision on your behalf, your healthcare proxy may have little or no understanding of your choices or the values and goals you have for end-of-life care. The result is your wishes may not be upheld, your proxy may be unnecessarily burdened, and your family may disagree about what’s best for you. CHOOSE YOUR HEALTHCARE PROXY WISELY Ideally, the first conversation occurs before you appoint someone to act on your behalf. Often people appoint their spouse or an adult child as healthcare proxy without considering if that person will be able or willing to carry out their wishes in a crisis. Consider sharing your wishes for medical treatment and ask, “If something happens to me, will you support my decisions and speak on my behalf?” Tell other family members who you have appointed your healthcare proxy so no one is surprised. Let them know your wishes so they can support your healthcare proxy in making difficult decisions. CONVERSATION BEGINS WITH YOU Your loved ones probably will feel reluctant to bring up end-oflife topics, so you are responsible for breaking the ice. Sometimes a famous person’s death or a controversial decision regarding end-of-life medical treatment in the news triggers a conversation about our own wishes.
happens to me, I want you to know what kind of care I would like at the end of my life,” or “I want you to be able to communicate for me if I can’t speak for myself. I don’t want you to second-guess yourself.” WHAT IF YOU CHANGE YOUR MIND? Your advance-care directive isn’t written in stone. Review your advance directive annually (April 16, National Healthcare Decisions Day, is a good time) or any time a lifechanging event—death of a spouse, divorce, or a diagnosis of disease— calls for it. When your wishes change, remember to tell your healthcare proxy and loved ones. Having the conversation reduces burden of decision-making, improves family harmony, and gives everyone peace of mind. When you initiate these conversations, it’s helpful to remember that it isn’t about how you want to die, but how you want to live at the end of your life. CREATE YOUR ACTION PLAN for Successful Aging & Retirement BY SVA PLUMB FINANCIAL Growing older forces change. Changes impact areas of life such as where to reside, transportation choices, relationships and family dynamics, financial options, and healthcare choices. Making decisions, identifying choices, and conveying wishes are important parts of the road ahead, but that doesn’t always happen. 1.
According to recent Marist research, 80 million Americans are reportedly conversation avoiders, meaning they haven’t talked about any important end-of-life issues with their parents or children, or have talked about just one issue.1 Research conducted by the Home Instead Senior Care® network reveals 70
percent of family conversations about aging are prompted by an event, such as a health crisis or other emergency. Using this checklist as a first step will help you create or review your plans, and give you the opportunity to have important family conversations.

GENERAL | 49
SVA PLUMB FINANCIAL CHECK LIST RESIDENTIAL I have identified an individual, such as a family member, close friend, or person with power of attorney, to help me make decisions, and I have let that person know I will need assistance. I have discussed with my family or close friends where I would like to age in the future and where I would like to live if I could no longer live at home. I have made a list of things at home that would be difficult to give up and things that I could incorporate elsewhere if I move. I know what it costs in my area to live at home and to live in a care community. I know what makes home special for me and have documented the pros and cons of moving or staying at home. I have considered ways to adapt my home to help keep me safe. I understand what would happen if I needed help at home. I have conducted an annual safety check of my home.
FINANCIAL I have identified a credible, trusted, and certified financial planner and met with him/her. I have estimated my cost of living based on where I would like to live as I age. I have determined the kind of lifestyle I would like, including whether I will continue to work and the amount of travel and leisure time I expect. I have made a list of assets and the location of important documents, such as a will, banking accounts, pensions, and retirement plans.
My financial planner has helped me determine the amount of money I will need to live on based on my financial plan. My financial planner has helped me determine how much money I will need to cover my healthcare and long-term care expenses, and how to plan ahead. I have appointed a person with power of attorney in the event that I cannot make decisions on my own. I have a contingency plan in the event my spouse dies or my financial situation changes.
HEALTH I have assessed my current physical, mental, and spiritual health and have documented the healthy choices and decisions I will start making. I have outlined my goals for a healthy lifestyle as I age. I have considered my current daily diet and mapped out positive changes for the future in my food choices. I am up to date on medical checks and preventive screenings. I have identified activities that would help me be more socially engaged. I have logged how often I exercise and set out goals to improve upon my exercise plan. I have spoken with my doctor to make sure I am on track physically and mentally. I have let my loved ones know about my healthcare situation, including doctors’ names, healthcare issues that I am being treated for, and medications I am taking.
TRANSPORTATION I have identified in writing how important driving is to me and how I would feel if I had to give it up. I have identified medical issues that might affect my driving abilities. I have a plan to ensure my driving is assessed for safety. I have written down how I would explain my driving predicament to others as a way to enlist their help in developing resources I would need to fill the gap. I have considered what I would do if I could no longer drive. I have identified someone or some services that could help me if I could no longer drive.
END OF LIFE I have made a bucket list of the issues that I would like to resolve and the goals I would like to accomplish before my life is over. I know where I would want to be at the end of my life—such as at home or in a care community. I have defined what I consider to be quality of life and have shared that with my closest family and friends. I know who I would want to receive my inheritance and valuables, and I have made a will. I have shared my thoughts, wishes, and feelings with my family and/ or closest friends regarding end-of-life decisions and related documentation, such as advanced directives, medical power of attorney, Do-Not-Resuscitate order, and funeral plans. I have appointed a healthcare proxy. JOURNEYOFAGING.COM

LIVING WITH Chronic Pain BY DR. DOUG KEEHN
We all have aches and pains. Whether we overdo it catching up on yard work, are healing from surgery, or dealing with an arthritic shoulder related to an old high school football injury, pain is a part of our lives. But pain exists for a reason—it’s a signal from our bodies that there’s an injury, cautioning us in the use of that area. It’s also an adaptive response to guide us safely through our environment. Once pain subsides, we know an affected area has healed. This is the essential function of acute pain. Chronic pain is a different story. We expect most injuries to heal within a few weeks to a few months. Chronic
pain occurs when the injured tissue has healed, but pain persists. It can also occur as part of a disease process, such as joint destruction that occurs in arthritis, or burning pain that may occur as a complication from the nerve destruction of diabetes, manifesting as painful peripheral neuropathy. Chronic pain represents a major public health problem. According to a report from the Institute of Medicine from 2011, over 100 million Americans suffer from chronic pain each year at an annual cost of over $600 billion. This includes direct costs, such as doctor visits, medications, and procedures, and
SEPTEMBER 2018–FEBRUARY 2019 JOURNEY OF AGING
indirect costs, such as lost productivity. The goal of healthcare professionals is to reduce the impact of pain on people’s lives by providing treatment to help them feel better and participate in their lives to the fullest extent. The cornerstone of chronic pain treatment is lifestyle change, such as quitting smoking, managing body weight, and doing regular low-impact aerobic exercise. Overthe-counter medications, such as ibuprofen and acetaminophen, are commonly recommended. Despite these measures, many people continue to suffer with chronic pain.
HEALTH | 61 for Disease Control estimated there were over 63,000 deaths in the United States from drug overdose, most due to opioids. Furthermore, some studies show that chronic musculoskeletal pain is not effectively treated with opioids. Thus, serious questions arise about the safety and effectiveness of opioids for chronicpain treatment.

How do we treat the problem of chronic pain without putting patients at undue risk? Tailored treatments. In addition to lifestyle modifications, other treatments include safe, non-habit-forming medications; manipulative medicine (osteopathic or chiropractic manipulation); swimming; yoga; and implantable devices, which work at the level of the spinal cord and alter the way the brain interprets pain signals from the body.
In the late 1990s, opioids were recommended for the treatment of chronic pain. The term opioid refers to medication both naturally derived, like morphine and opium, as well as synthetically derived, like oxycodone and hydrocodone (active compounds in strong prescription drugs, such as Percocet and Vicodin respectively). Historically, these powerful medicines were reserved for the treatment of very severe pain, such as pain from surgery or pain that can come at the end of life in patients with cancer. Some people felt the safety concerns over the use of opioids, such as addiction and unintentional overdose, were overblown, and recommended they be used more liberally to treat chronic pain. Sadly, the widespread use of opioids for chronic-pain treatment has led us to a national crisis of opioid misuse. As of 2016, the national Centers

A Multifaceted Approach to
FALL PREVENTION BY PAUL MROSS
Group Health Cooperative (GHC) of South Central Wisconsin is approaching the issue of alarming high fall rates in Wisconsin in a unique way with their comprehensive Fall Prevention Clinic. GHC has been committed to health and wellness for more than 40 years as a nonprofit, member-owned cooperative that brings health
insurance and healthcare together for the benefit of the communities it serves. Staying true to a cooperative spirit that believes the good health of many makes us all better, the new Fall Prevention Clinic is a natural progression of GHC’s dedication to providing more access to health and wellness programming.
SEPTEMBER 2018–FEBRUARY 2019 JOURNEY OF AGING
One third of Americans over the age of 65 will experience a fall this year. Every 11 seconds, a Wisconsinite will visit an emergency room or hospital because of a fall-related injury. Falls are the leading cause of fatal injuries among seniors, and Wisconsin ranks second in the nation for fallrelated deaths. While Wisconsin is a leader in fall-prevention research
HEALTH | 65 and community-driven, evidencebased programming, this disturbing situation persists. GHC’s department of complementary medicine designed a 10-hour program that weaves together multiple approaches which address the reduction of fall rates. The Fall Prevention Clinic introduces the participants to programming like Stepping On, Ballroom Basics for Balance™, tai chi, yoga, and Argentine tango. It comes from the belief that if one way works well, multiple ways must work better. Teaming with community leaders from each program, the Fall Prevention Clinic is a way for the participants to broaden their exposure to these different options, have access to written materials related to fall risks, and learn some simple skills to practice at home. One unique element to the clinic is the use of staff and providers within GHC, like the clinical pharmacist who spoke with the group about medications that could affect balance and the potential to fall. The pilot Fall Prevention Clinic was completed in spring 2018. A self-reported survey revealed an increased level of confidence in balance and steadiness in situations like bending over to pick up something and walking outside on icy sidewalks. One participant stated, “[What’s] most important is the awareness to pay attention all the time.” Like all pilot programs, lessons learned only enhance the quality of programming. Increasing sustainable future endeavors will include enhancing our recruitment methods with the CDC’s fall-prevention algorithm among the nursing staff and PT/OT departments.
One of the methods introduced during the clinic was yoga. Yoga is a new player on the frontier of fallprevention research. The University of Wisconsin–Madison and ADRCs of southwest Wisconsin recently did a study that showed a 48 percent reduction in falls when comparing the six months prior to the yoga classes to the six months after. One practice used in that research was developing a home habit of noticing how the four corners (base of the big and little toes, and inner and outer edges of your heel) of the bottom of your feet feel when you stand. This was one of many skills and habits the GHC clinic participants used and developed over the course of the program. With adequate knowledge and information, we can significantly reduce the number of falls in Wisconsin. Continuing to advance research and community awareness is key. To find out more about GHC’s Fall Prevention Clinic and our next class, starting October 10, visit ghcscw.com. Photograph provided by Group Health Cooperative of South Central Wisconsin.

Finding BALANCE FOR HEALTHY LIVING BY JOSH KNOPS, AUBREE WILHELM, AND KRISTY COX
Balance. It’s an important factor in staying healthy, especially for seniors. After retirement, seniors can focus on themselves and their health first, without the external demands and pressures that come in the work place. So where does one start? There are three primary categories of personal development for supporting a balanced and healthy lifestyle: social, physical, and intellectual. Social is clear—being around people, connecting, and creating both short- and long-term
relationships. These interactions are necessary for affirming personal value, helping to keep the mind sharp, and staving off loneliness and the depression that comes with it. Meeting new people and varying routine has also been shown to keep the mind sharp and may even delay memory loss. Looking for new ways to connect? There are a variety of resources available to match up interests and explore opportunities. Check out your local senior center, community parks and recreation department,

aging and disability resource center, and chamber of commerce. They can provide lists of clubs, schedules of activities and classes, as well as local social group meeting times. Physical wellness is a joy for some and a burden for others. No matter your previous relationship with sports, exercise is an important part of healthy aging. If you don’t use it, you lose it. Exercise helps you to maintain the abilities already in place: walking, stretching, balancing. All of these activities are performed in daily tasks. Staying

physically active will ensure that daily activities can be performed with less fatigue and stress on the body. Want to keep up your golf game? Travel? Play with the grandchildren or great-grandchildren? Focus on these elements to support your physical wellness.

nutrition and choices that include plant-based diets while limiting red meats
•
Managing stress
• Avoiding tobacco Intellectual wellness affects how we think, our emotions, and our actions. Stress can cause anxiety and depression that result in bad feelings and sometimes bad choices. As Thomas Lansing Masson said, “A mind is as strong as its weakest think.” Maintaining positive intellectual health is just as important as physical health while social wellness overlaps. There are important aspects to not only feeling recognized, but also contributing actively to a purpose. How should you remain purposeful in retirement? Wasn’t the point of retirement to stop working? The answer is not so simple. Vocation does not necessarily mean a traditional job. Making meaningful contributions can be very individual and is an essential part of every person’s positive intellectual wellness. This can be as simple as listening to a friend in a tough situation. It could also be performing volunteer service in the community, or even delivering
random acts of kindness to total strangers. Fulfillment, well-being, optimism, coping skills, and relaxation: all are the rewards of a focus on intellectual wellness. How can you find volunteer opportunities or ideas for your own service? Reach out to the resources listed in the third paragraph of this article. Also search out causes that interest you. Many nonprofit and charitable organizations welcome the opportunity for volunteers to help with their mission. All three aspects of wellness are vital to healthy aging no matter where you are in the journey of life. Continually remind yourself that you are a priority and keep the balance you need to stay happy, healthy, and fulfilled.

Welcome to Fantasy Island Is Your Fantasy Self Cluttering Your Home? BY SUE LAING
Shirley is feisty. Her soulful eyes scrutinize my face when I ask if she has ever used the largest, most accessorized version of a food processor I’ve ever encountered. I knew she had not. She’s wondering if she can convince me she has diced, chopped, sliced, and everything else she fantasized about doing with this behemoth appliance that almost takes up an
entire cupboard. Shirley gives in and admits, “Well, I want to.” We both laugh uproariously. The food processor is the largest of nine kitchen appliances she hasn’t used. There’s also an ice cream maker, a bread maker, a donut maker, a panini maker, a quesadilla maker, a ninja, a hot-air popcorn machine, and a large orange wok.
SEPTEMBER 2018–FEBRUARY 2019 JOURNEY OF AGING
SAFE & HEALTHY AT HOME | 97 Shirley fantasized about making good use of all of these appliances, but the reality is that she only uses the most rudimentary of her kitchen items: 4 of the knives in a butcher block that holds 12 and 3 of the pans out of a collection of 8. She buys ice cream, bread, and donuts. She can’t remember the last time she ate popcorn. The Asian food she enjoys ordering out could never be replicated by her using the vibrant wok. There’s also a set of lovely china that has been used once. Shirley is a fantasy kitchen goddess. Here are fantasizers.

Tool Master The guy who has a workshop. You’ll find gadgets of every sort that have been used one time to fix one thing. Fortunately, tools have a nice resale value.
For the Love of Books We have eight bookcases in our 750-square-foot condo. My husband will not part with them because he is emotionally attached to them. Even though I doubt he has touched any of them in years because he reads ebooks mostly, he is soothed by the display.
The Crafting Maven The people who have yarn, fabric, patterns, paints, scrapbook supplies, kits, packages, special machinery, and gadgets to make gifts. The car drives itself to the store where these things are sold. Whole days are spent at craft fairs, wandering around getting ideas, and surfing Pinterest and Etsy. Usually it’s a whole room full of stuff to make stuff often vacated by the youngest offspring.

Office Organizer Not to be a hypocrite, office supplies are my kryptonite. I have an actual file cabinet that sits unused with complete file systems that I’m sure are better than the last ones I bought, also not used. One of these days! I don’t even want to tell you how many 2018 planners of every size I bought searching for the right one. If you can identify your fantasy self among these types it goes a long way in helping guide you where to start downsizing. Don’t be too hard on yourself—it’s usually the result of a prosperous, wellintentioned life!

Safety IS NO ACCIDENT BY NIKI ANDERSON
Falls are the leading cause of fatal injury and the most common cause of nonfatal trauma related hospitalization in people 65+. In fact, according to the Centers for Disease Control, more than a quarter of older adults fall each year. Of those falls, one in five causes a serious injury. Though only 20 percent of falls cause injuries, falling once doubles your chances of falling again. The fear of falling may lead people to cut down on everyday activities,
which can lead to an increase in weakness, increasing the chances of a fall. The best thing you can do to prevent injury is to avoid falling in the first place. There are many steps you can take to help prevent falls.
TALK TO YOUR DOCTOR Less than half of people who fall every year tell their doctor. Being open about falls and the fear of

falling with your healthcare team gives them the opportunity to evaluate your risk and recommend specific things you can do to decrease it.
MIND YOUR MEDICATIONS Some medications or combinations of medications can have side effects, such as dizziness or drowsiness, making falls more likely. Make sure each of your providers knows about the medicines
SAFE & HEALTHY AT HOME | 103 you take, including prescriptions, over the counter, vitamins, and supplements. Ask your doctor or pharmacist to review your medications for side effects or possible interactions.
EXERCISE Lower-body weakness and balance problems both contribute to falls. Work on increasing your strength through daily activities, like walking and water aerobics. In addition, choose activities that can help improve your balance, such as tai chi, chair yoga, ballroom dancing, and exercise classes that include balancespecific exercises.
KEEP YOUR VISION SHARP Have yearly eye exams and wear glasses or contact lenses with your most up-to-date prescription. Being able to see clearly will help you get around much more safely.
ELIMINATE HAZARDS AT HOME Because half of all falls happen at home, a home safety check can go a long way in reducing falls. Fixing potential issues, such as tripping hazards, clutter, and poor lighting, can help lower the chance of falling. Installing grab bars in the bathroom or railings on stairs is also a great way to decrease your risk. SSM Health at Home, in partnership with United Way of Dane County, developed the SAFE at Home program, which provides home safety assessments and medication reviews for Dane County residents ages 60+. SAFE at Home participants receive: • Basic in-home safety assessment. • Medication review by a pharmacist. • Safety aids, including a pill box and nightlight.
The best thing you can do to prevent injury is to avoid falling in the first place. • Home safety recommendations. • Information about community resources. • Six months of follow-up by a medical social worker. The program is designed to help participants identify risk factors and advise them on ways to change or modify their environment to decrease fall risk. The goal is to reduce falls, helping people live safely and independently in their own homes for as long as possible. Contact SSM Health at Home if you have questions or want to schedule an assessment. Not a Dane County resident or younger than 60? venting falls is much more effective than treating the results of them.
