Moving your loved one into a memory care community is a big change for them, and as a caregiver, it can be very emotional and challenging for you as well. But there are many things you can do to help make the transition successful.

Talk About Moving

When to Talk About an Upcoming Move

It’s important to gauge how your loved one reacts to the idea of moving. If the topic is stressful, don’t talk about it too much and move slowly. If they’re interested, take them along on tours during their best time of the day.

This is a common question. It’s usually best to wait until two to three days beforehand. You may also consult with the memory care community you’ve selected about specific concerns.

Keep the Move Simple Don’t bring too many items. It can be overwhelming for a person living with dementia to live in a cluttered environment. Narrow it down to necessary items and belongings to make the new apartment feel like home. Move items into the apartment before your loved one arrives to make it as organized and familiar as possible. Bring items such as a cozy bedspread, family photos, furniture, and favorite mementos. And, if possible, set up the apartment in a similar way to what your loved one is already used to.


ALZHEIMER’S & DEMENTIA | 15 Share History with Memory Care Staff Share stories, information, and photos with staff to help them learn your loved one’s history, including their past profession, current and past hobbies and interests, and family life. The more staff knows, the more personal the experience they can create. Staff can build a good relationship with your loved one and help facilitate friendships with other residents.

It can take time for everyone to adjust to memory care. Allow two to three weeks of some ups and downs. Throughout this time, communicate often with staff and don’t be afraid to ask for guidance along the way. Soon the move will be behind all of you, and a new normal will be established.

Everyone grieves at their own pace. There’s no right or wrong way to grieve. It’s devastating to lose someone you love, and in the current environment, it’s even harder to process all the emotions of losing a loved one. Physical and mental isolation is real and makes us all uncomfortable. For most, mourning is uncomfortable because it’s a raw and rare emotion. Couple that with the other loss—the comfort of hugs and condolences delivered in person—and mourning becomes even more complicated. Especially if we’re unable to say goodbye or spend time with our loved one before they pass.

But loss of life isn’t the only thing we’re grieving in today’s world—we’re all grieving in some way right now. All the same feelings of shock, disbelief, and denial are part of what many are experiencing in the face of a worldwide pandemic. There’s loss of employment, daily routine, and even our way of life. Each day provides an experience very different from what we’ve known, and feelings of grief come upon us unexpectedly, sometimes in ways that are unrecognizable. By now, practically everyone knows someone impacted in some way by COVID-19. Perhaps it’s a lost job or pay

cut, or a postponement or cancellation of important plans, such as a wedding, birthday, anniversary, or graduation. Looking at one on its own doesn’t seem like a reason to grieve, but when any are combined together or with the current state of affairs, feelings of loss can be just as powerful and real as the loss of someone you love. Grief is a natural response, and it’s important to recognize and acknowledge the feelings in order to make sense of them and allow us to redirect our energy in a constructive and positive way. This doesn’t mean we won’t still hurt over loss for years


END OF LIFE | 21 or perhaps the rest of our lives, but the process of grieving allows the possibility of reinvesting the energy elsewhere to obtain a sense of peace rather than a place of pain. A support network is the best way to work through the process of mourning the loss of someone or something integral in our lives. The spectrum of our emotions is so changeable, sometimes even irrational, that we may think that if we open the door we’ll be consumed with sadness or rage. Yes, rage. It’s a very common and perfectly normal reaction to loss. Reaching out for support from friends, family, or a spiritual counselor can help balance our thoughts and emotions. It’s important to spend time remembering Grandma’s pot roast dinners, laughing about how your beloved pet would wake you up at 3:00 a.m., or even the sweet luxury of meeting a group of friends for drinks and dinner.

DO I NEED A LIVING WILL? BY BETH JACOBSEN

The short answer is yes. A living will, sometimes called a directive to physicians or advanced directive, is a legal document that details medical interventions you’re comfortable with being used to prolong your life should you become unable to consent to treatment. The directives will only apply to your care if you have an incurable terminal condition. Much like estate planning documents, healthcare documents should be completed before you’re in crisis. An accident can

happen at any time, so it’s important to outline your personal choices regarding end-of-life measures. Thinking about your mortality isn’t an easy subject. Should you become incapacitated without a living will, your immediate family will be responsible for making end-of-life decisions on your behalf. By outlining your wishes, you’re eliminating the often stressful, sometimes controversial, decisions your family and healthcare providers are otherwise required to make.

Completing a living will should give you and your family peace of mind. The authority granted will expire when the grantor dies.

Drafting a Living Will Living wills can be drafted by an attorney. In Wisconsin, residents can find a free form through the Department of Health Services website, dhs.wisconsin.gov. When determining your end-of-life wishes, you’ll have to consider your personal values and/or religious beliefs.

It’s vital to discuss the medical terminology within your living will with a healthcare professional to ensure you are completing the form correctly.

Sharing Your Wishes One of the most important steps after completing a living will is sharing your wishes with your immediate family and those closest to you. It’s also recommended that you provide a copy to your primary care physician or your local hospital to keep in your file. Like all important documents, you should also keep an original copy in a safe place where it can be easily found in the event of an emergency. A living will is one of several tools that can be used to give advance directives about your end-of-life wishes. You may also consider completing a healthcare power of attorney form. By naming a healthcare power of attorney, you give another person the authority to make healthcare decisions on your behalf if you’re unable to consent to treatment. HOW HOME REMODELING CAN HELP

MULTIGENERATIONAL HOUSEHOLDS THRIVE BY JESSICA STEINHOFF

A century ago, living with a parent or grandparent was the norm in America. This changed after World War II, when it became more common for older generations to live apart from their adult children. But now the tide is turning. According to Pew Research Center, almost a quarter of American households now contain two or more adult generations. As households grow to accommodate more adults, homes must evolve to meet the needs of everyone who lives there. For many homeowners, remodeling is the best way to achieve this goal. Here are four factors to consider

when multiple generations live under one roof.

Privacy Whether your household includes an aging parent or a young adult child, privacy is a must. “Having some space to yourself is important if you want to read a book, talk on the phone, or spend time with a guest,” says Josh Fetting, UW Credit Union’s consumer lending sales manager. “This is true no matter what age you are.” This is one reason Josh added a bedroom to his home when his

mother-in-law moved in. “My family had an extra living room that wasn’t well utilized, so it made sense to turn it into a private space for her,” he says. “Plus, we realized that adding a bedroom to the house could increase its resale value.” Josh recommends a home equity line of credit, or a HELOC, for this type of upgrade and many others, from building a separate entrance for a college-age child to turning a basement into a mother-in-law suite. “A HELOC is a great fit for many remodeling projects because they

In other words, you can borrow exactly what you need and only pay interest on that.”

Accessibility Mobility often decreases as age increases, creating a need for spaces that accommodate wheelchairs and walkers. Universal design can make a home comfortable for everyone. Some examples include widening doorways to make space for a wheelchair or baby stroller, and installing faucets and drawer pulls someone with arthritis can use with ease. In addition to making a home more functional in the present, universal design can help it appeal to a wider range of buyers, including older adults and people with disabilities, when you decide to sell.

Storage When you add people to your household, you also add their belongings. This can create a pressing need for storage space. Josh remembers that this was a concern when his mother-in-law moved in. “We had to find more space for everyone’s things, so we created a new walk-in closet when we added her bedroom.” Other storage options, such as builtin shelving and customized cabinetry, are also worth considering. Unlike an off-site storage unit, these in-home solutions allow a home’s residents to use their belongings regularly. Plus, there’s no rental fee to pay.

Space Sometimes a home is too cramped for everyone to live comfortably, but moving presents too many challenges. Perhaps health or mobility issues make moving overwhelmingly difficult, or maybe it’s just too hard to find a time that six people can attend a home viewing.

In these situations, it often makes sense to increase your home’s size. This could mean constructing a second story, finishing a basement, or using an addition to enlarge a bedroom. You might even build another property on a large lot—if zoning laws allow it. Many multigenerational families find that they need more kitchen space so they can cook together and enjoy meals as a group. In addition to increasing the footprint of your kitchen, you might want to add an island, install touch-free faucets, or replace tile flooring with something less slippery. Once again, a HELOC can be a great resource. “With construction projects, the final bill is often a little more than homeowners expect, so it’s good to have a flexible funding source,” Josh says. “A HELOC gives you some wiggle room in case that happens, but it can also prevent you from borrowing more than you actually need.”

Coronavirus has made our lives everchanging. The ability to stay on top of newly released information, which feels like a daily occurrence, is challenging. We all know the basics: wash your hands frequently, wear a mask, stay home when possible, and avoid large gatherings. Sadly, these suggestions also make it difficult to stay connected to loved ones and our community.

Public transportation is integral to maintaining independence and autonomy for older adults. The Dane County transit system has been struggling to keep up with public needs and the need to keep the public and workers safe. While we’re all concerned about coronavirus exposure when we’re out and about, there are ways to mitigate the risks.

At Union Cab, we’ve developed policies to keep drivers and riders safe. We require all workers, drivers, and office staff to diligently practice good hygiene; wear masks; and practice social distancing, remaining at least six feet from others. We supply drivers with cleaning supplies to clean and sanitize their cabs after each ride, and all of our cabs are stocked with hand sanitizer, masks, and tissue.

We also require all passengers properly wear masks during their rides and only allow them to ride in the back seats to maximize social distancing. These measures reduce the risk of viral transmission, and we believe that currently traveling by cab is safer than mass transit, where passengers are exposed to more people. Some of the questions we’re frequently asked regard masks. It’s been a hard adjustment for everyone. The CDC recommends cloth masks with three layers that can be washed and reused. For people who don’t have their own masks, many businesses are providing disposable masks. Current research suggests that the least-safe options are bandanas and gator scarves. A mask helps keep the wearer’s germs more contained and closer to their body so that, when done in conjunction with social distancing, transmission is much less likely to occur. Masks reduce airborne particles, keeping everyone safer. Granted, there are numerous reasons people find wearing a mask difficult. It may be harder to breathe, glasses can fog up, and it can be hot and uncomfortable. Communicating while wearing a mask is also a tricky new skill many of us are still learning, and, sadly, masks obscure beautiful faces! The positives of wearing a mask far outweigh the negatives, especially since coronavirus can be carried by a person who shows no signs of having the virus. Every person we come into contact with is a potential carrier, so taking universal precautions are necessary to stay safe and reduce infection numbers. And when you go out, contact the businesses you’ll be visiting to find out

what precautions they’re taking to keep people safe. The more we know, the safer we remain. Don’t forget an often overlooked effect of coronavirus: losing touch and feeling alone. Call friends and family to stay connected, and be sure to ask if they need your help. And be sure to ask for help yourself if there’s anything you need or don’t understand. Doing what we can to keep ourselves and others safe, and making sure no one is left alone or feels alone, is one the kindest and most respectful things we can do for one another.

If your current heartburn treatment isn’t giving you the results you desire and you’d like to reduce or eliminate dependence on potentially risky medication, there’s another option. It’s a new minimally invasive procedure called LINX.

GERD Gastroesophageal reflux disease (GERD) occurs when the stomach contents reflux up into the esophagus. The resulting acid causes the symptoms of heartburn and regurgitation.

First Lines of Defense The first approach if you have GERD is diet modification. Avoid spicy foods, caffeine, fatty foods, alcohol, and

carbonated beverages. Secondly, prop up the head of your bed at night.

Medications that block acid production in your stomach also exist. The most common category is proton pump inhibitors (PPI). About one in five Americans suffers from GERD, and there are 20 million patients in the United States on PPI drugs, with 38 percent of patients still having symptoms, even while taking these drugs.

Incidentally, if you look at the statistics for esophageal cancer in this country, the rate has seen an approximate 600-fold increase from the time acid-blocking medications came out in the mid-1980s. The concern is that medications may be improving symptoms but not solving the core problem, resulting in patients still being subject to reflux damaging the esophagus.

Complications of Untreated GERD About 10 to 15 percent of patients with GERD will develop Barrett’s esophagus, which results in changes in the lining of the esophagus from acid damage. Patients with Barrett’s

Traditional Surgery vs. LINX In the traditional surgical approach to treat GERD, known as Nissen fundoplication, a portion of the


HEALTH | 53 stomach is wrapped around the lower esophagus. This changes the anatomy of the stomach, requires an overnight stay, and involves dietary restrictions. The newer, less-invasive LINX treatment involves putting a small beaded bracelet of titanium magnets (about the size of a quarter) around the end of the esophagus to bolster the sphincter. The procedure is done laparoscopically in an hour, with patients typically going home the same day. It’s removable and doesn’t change the anatomy of the stomach. Unlike other gastrointestinal surgeries, this procedure doesn’t require food restriction, and patients are actually encouraged to start eating right away post-surgery. Patients who have a LINX device will retain the ability to belch and vomit, which is often not the case for patients who have a Nissen fundoplication.

Who’s a Good Candidate for LINX? LINX is for any individual taking reflux medication who is still having symptoms of reflux, patients who are on a double dose, and patients who don’t have heartburn but still experience regurgitation symptoms that come with reflux.

Potential Side Effects Initially, the most common side effect of placing the LINX is dysphagia, which is a sensation that occurs when patients can feel food going through a tighter area in their esophagus upon swallowing. We recommend eating solid foods frequently after surgery to lessen dysphagia, and we typically see dysphagia disappear after about three months. Some individuals have had concerns about the device material. It won’t set off airport alarms, so you can fly per usual. It’s also considered MRI safe for the most commonly used MRI machines.

Is LINX Right for You? What I would like patients to know is that LINX is an extremely viable option for those taking PPI medications. Certainly, if you’re someone who is taking PPIs and still having symptoms, are unhappy with the way you’re feeling on these medications, or you’re having to increase the medication dose, you’re a very good candidate for the LINX procedure. Five years postoperatively, 85 percent of LINX patients are free of their medications, 88 percent of those patients no longer have heartburn, and 99 percent no longer have regurgitation. We also don’t see the bloating and gassiness that can occur with traditional surgery. I have had several patients who suffered with heartburn for decades. They’re afraid of surgery because of potential bad side effects. With LINX, there’s relief, so they don’t have to live with heartburn. Please note that people with allergies to titanium or nickel should not get this surgery.

Quality Assurance & Performance Improvement (QAPI) are familiar buzzwords. Phone calls are “recorded for quality assurance purposes.” Retailers and healthcare providers solicit feedback on our experiences, be it a haircut or an operation. But at long-term care facilities, QAPI is more than just measuring satisfactory versus unsatisfactory experiences. QAPI takes this to the next level by

helping facilities engage in proactive approaches to improving systems and delivering safe, effective, personcentered care. Capitol Lakes Health Center is part of a continuing care retirement community (CCRC) that is recognized as having achieved high QAPI standards across the continuum of care, particularly in the skilled nursing facility. Patients, family members, and providers can all benefit from QAPI initiatives.

The Centers for Medicare & Medicaid Services (CMS) requires all skilled nursing and long-term care facilities to have a QAPI plan. The plan must incorporate regular meetings, measurable results, and proof of outcomes. Capitol Lakes was the first to earn Providigm’s Advanced QAPI Accreditation. Fewer than 2 percent of skilled nursing facilities achieve this gold standard of accreditation. This recognition confirms commitment

A dynamic QAPI program promotes interdisciplinary teamwork to improve services and make residents’ experiences better. Capitol Lakes utilizes abaqis ® quality management to efficiently identify strengths and care areas needing improvement. Simply put, issues are identified earlier and thus can be fixed earlier. Since implementing abaqis ® in 2016, residents of Capitol Lakes have indicated they feel more empowered with their healthcare choices, have fewer instances of unrelieved pain, and are more engaged with the Therapeutic Recreation Department. Residents at Capitol Lakes benefit from QAPI’s evidence-based strategies to standardize, measure, and improve the patient experience across the

care continuum. Lower readmission rates, fewer complications and infections, shorter lengths of stay, and higher patient satisfaction are all interconnected. When healthcare facilities implement industry-standard training and quality methodologies, we ultimately provide better value for our patients. Memory LOSS: CREATING A PLAN B BY PAULA GIBSON

Once your loved one has been diagnosed with Alzheimer’s disease or another form of dementia, Plan A is typically to keep them in their home for as long as possible. However, there may come a time when those walls won’t support their quality of life anymore. Unfortunately, those with dementia do not recognize the need for assistance. This is why families need to formulate a Plan B for where

their loved one might live when their home situation is no longer a good fit. The first step in the Plan B creation is to ensure that the person with memory loss has healthcare and financial power of attorney (POA) paperwork. This paperwork should name a primary and a secondary person that will assist in making decisions for the

person who cannot make their own. The paperwork does not take effect until two physicians or a physician and a psychologist medically determine the person to be incompetent or unable to make medical decisions for themselves. If the POA paperwork is not in place, it may result in a court proceeding to establish guardianship, which can be costly and take valuable time.

If your loved one needs 24/7 care, they need reliable people to be there all day, every day, without excuse, absence, or delay. • Emergency and crisis situations have arisen. Your loved one falls, a caregiver is ill, or worse. Sixty-three percent of caregivers pass before the person they’re caring for. Sadly, the last item is the most common reason why a Plan B is enacted. Most families wait until a health crisis occurs to create a Plan B, and then wait until another crisis or myriad of concerns happen before enacting it. Plan A is optimal, but when the walls can’t hug or keep your loved one safe, it’s time to realize that the memories can come with them. The main thing is to ensure that your loved one is safe, supported, and smiling, and that can happen when Plan B is in place. The POA individuals should be involved in creating the Plan B and visit memory care homes that could assist a loved one now and in the future. In doing this, you ensure that if something happens to your loved one or their main care provider, Plan B is in place.

This program allows us to help homeowners with some of the most expensive and time-consuming projects. We walk you through every step of the process. The program itself is very nuanced, but the application process is super easy for homeowners.”

So in addition to the freedom, lifestyle, and general well-being adaptations offer, it can be significantly less expensive to upgrade rather than move. And as we get older, being uprooted to an unfamiliar place can be stressful; different smells, views, sounds, and neighbors are major adjustments. This may sound overly dramatic, but these are the things many older adults have told us. They are

thankful to be helped so they can stay in the homes they’ve known and loved. Project Home’s programs exist to improve living conditions and the housing stock, but it’s the human side to the work that affects us most—it’s why we do it. If repairing or adapting someone’s home allows them to retain their independence and comfort, as well as makes long-term improvements to the neighborhood, Project Home will be achieving its mission—one house at a time.

Helping Patients Manage Chronic Conditions at Home BY NIKI ANDERSON

Chronic conditions are among the most prevalent and costly medical issues facing Americans. Nearly half of all U.S. adults have a chronic condition, and one in four has two or more. For those over 65, the numbers only grow. Common chronic conditions include heart disease, COPD, and diabetes. Those suffering from chronic conditions are the most frequent users of healthcare, accounting for over 80 percent of hospital admissions and a third of healthcare spending in the country. Chronic disease management programs can help patients achieve better health outcomes and increase their quality of life as well as lower healthcare spending.

The goal of these programs is to help patients self-manage their illness, allowing for better health outcomes as well as reducing hospital admissions, readmissions, and emergency room visits related to their diagnosis. Telehealth technology can also support patients managing health conditions at home. Telehealth, or home telemonitoring, helps patients with chronic conditions by providing a consistent connection to healthcare professionals who can intervene before a small change becomes a big problem. Telehealth solutions offer devices and software that consistently and accurately manage patient vital signs in

the home. Patients living with a variety of conditions, including heart disease, diabetes, COPD, and hypertension, can benefit from the daily monitoring that telehealth services provide. As part of daily monitoring, patients complete a set of questions customized to their health condition. Healthcare providers receive vital-sign data and interview responses. If the data is outside established limits for the patient, the telehealth system alerts the healthcare provider. These alerts allow for early interventions and treatment adjustments before a more significant health event occurs, such as emergency room visits and hospitalizations.


SAFE & HEALTHY AT HOME | 91 Additional benefits of this consistent monitoring are a comprehensive view of how well a patient’s condition is being managed and objective measures of how effective a treatment is over time. Today, more than 90 percent of adults over 65 live independently, and most would prefer to remain at home if they’re able. Services like chronic disease management and home telemonitoring programs can help make that happen for those living with chronic conditions. Home health services have one goal— to help patients remain at home. They assist people in recovering from illness or injury or healing from surgery to recover at home, and specialty programs can be designed to help those living with a chronic condition better manage their illness to remain as healthy as possible at home.

During a long, successful career as a healthcare in aging executive, Christine Klotz became familiar with The Village to Village Network and had seen the positive value of community as people age. Christine and her husband, Chuck Henderson, moved to Waunakee from central New York. “I was delighted to know that one of the oldest and most successful villages was here with Sharing Active Independent Lives (SAIL),” says Christine. “I knew that finding and maintaining purpose is a critical part of

Christine offers her experience and leadership skills to assist in areas she feels she can make a difference, including as chair of the Communications Committee and as a member of the SAIL Operating Council. “We don’t need many services yet, but we feel it is appropriate to support the organization now so that it is strong when we face inevitable challenges in the future.”

Members are encouraged to share their time and talents to support fellow members and the organization. The wide range of backgrounds, talents, and experiences of its members build the strength of the organization. “Using my knowledge and experience and being a part of identifying needs within the organization now and for the future has been rewarding,” says Christine. “Other members bring different skills and perspectives; sharing those keeps us all engaged.” Members and their families find that the peace of mind provided by SAIL membership when aging in place is the right choice.
