ACCESS PROJECT PROGRAM INFORMATION
• *Photo Waiver: Photographs, videos and other documentation taken at school or Community Education activities may be submitted to newspapers, used on district website, social media sites or appear in publications. Check above if you do not want pictures published. • These activities have been customized for adults with developmental disabilities. • Preregistration is required for all classes. Please return the form above or call 651-403-8313 to register. • Please indicate any accommodations needed to effectively participate when registering for an activity. • Participants who require one-on-one personal attention for physical and/or behavior issues or medication administration must be supervised by a caregiver. If a behavioral incident occurs, a caregiver may be required to accompany the participant to all future activities. • Classes will be held at the date, time and location listed unless you are notified that they are canceled, changed or full. • Transportation to and from activities is the responsibility of the participant or caregiver. • Participants must be accompanied to the classroom and should check in no earlier than 15 minutes prior to the listed start time and should be picked up promptly at the listed end time. • All Access Project activities are sponsored by ISD 197 so there is no tobacco or alcohol allowed. • Refunds will only be given for cancellations received two days before an activity.

"Happy" Line Dancing Mini Golfing at Grand Slam
We will be going to Grand Slam in Burnsville for this outing. No golfing experience needed. Lunch and a drink will be provided. Please bring quarters if you are interested in playing games in the arcade. We recommend that you wear tennis shoes for this event. Our bus will depart from Branch Out at 11 a.m. sharp! $15 fee includes transportation, mini golf, and lunch. Cost for caretakers $15 to participate or $5 for transportation only. Zumba Gold Game Day!
Join us to play Left, Center, Right (LCR) - a game of dice, luck and fun! LCR is a very simple game, yet it has acquired many fans from all over the world. It is played with three dice and a handful of chips (or candy). Once a player sits in on a game, the outcome is left entirely to the three dice. Hope it's your lucky day! No cost for caretakers.
