If you don’t already have one, you can ask for a Hospital Passport Form to tell the doctors and nurses how you want to be supported. To get a form, please telephone the ward where you will be staying.
9
What will happen when you are in hospital
Please show your hospital letter to a member of staff when you arrive and they will tell you how to get to where you need to go.
10
Leaving Hospital
Leaving hospital is called being discharged. We will help you plan for this and make sure you can manage when you get home.
We might ask you to sign a consent form to say you agree to have treatment.
We might ask a social worker to find out if you need any extra help or support. They will talk to you, your family or carer about this.
There is a process for consent issues so if you are unclear please ask a member of staff.
You can fill in a feedback form to tell us what you think about the care we gave you. Ask your nurse for one, it is called the Friends and Family Test.
