suny upstate Medical university, one of 140 academic medical centers in the united states, attracts outstanding Doctor of pharmacy graduates from across the nation. each pharmacy residency program draws upon the resources of our academic medical center – including a teaching hospital, four health related colleges (the College of Medicine, the College of Graduate studies, the College of health professions and the College of nursing), our research enterprise and hundreds of specialty services. each program, accredited by (or in precandidate status with) the system pharmacists (ashp), offers pharmacy residents the opportunity to interact with many other disciplines.

Upstate University Health System the upstate university health system includes upstate university hospital, upstate Community hospital, upstate Golisano Children's hospital, the upstate Cancer Center and numerous satellite sites. as with many teaching hospitals and health systems, upstate university hospital and its associated outpatient clinics serve a diverse population that includes 1
residents at suny upstate receive a competitive stipend for one year, July 1 to June 30, plus fringe benefits which include health and dental insurance and paid travel for relevant meetings.
Where do our residents go? a review of our residency graduates since 2013 indicates that about 67 percent of our residents pursue post Graduate year 2 (pGy-2) pharmacy residency or fellowship training. a majority of the remaining 33 percent go on to pursue a career in the acute care hospital setting. Many of our residents stay on as members of the upstate pharmacy family with a few pursuing academia immediately after graduation.

Research and Publications our residency program provides both pGy-1 and pGy-2 pharmacy residents the opportunity to develop proficiency in research, manuscript writing and publication. With the introduction of our innovative eight-month research Certificate program, our residents have the opportunity to gain a strong foundational knowledge to help pave the way for success with their current and future research endeavors. the upstate pharmacy services translational research team (upstart), in collaboration with the upstate pharmacy residency programs, has worked tirelessly to create a

PGY-1 - PHARMACY RESIDENCY “I am honored to serve as the PGY-1 Pharmacy Residency Program Director at Upstate. Additionally, I am very proud of the unique design and conduct of our residency program. Our training environment at Upstate is intentionally designed to make our residents successful. My goal as a program director is to train future leaders who will become distinguished candidates for any career they wish to pursue after graduation. Site: Upstate University Hospital upstate university hospital, the area's only level-1 trauma center, has a centralized pharmacy distribution paired with numerous satellite offices. the decentralized pharmacy services cover every patient care area, including the emergency Department. upstate university hospital also houses the Central new york poison Control Center, which serves 54 counties. the poison Control Center is run by pharmD clinical toxicologists and serves as one of our training sites.
internal medicine based learning experience designed as a repeating, four-week rotation, alternating with fourweek, core learning experiences and one elective rotation. the program is designed to offer pGy-1 pharmacy residents the opportunity to progressively increase their independence in a professional, friendly environment in which they are able to become proficient pharmacy clinicians by the end of their residency training.

PGY-2 - AMBULATORY CARE RESIDENCY “The PGY-2 Ambulatory Care residency is a cosponsored program between Upstate Medical University and St. John Fisher College (SJFC) – Wegmans School of Pharmacy (WSoP). The residency, which began in 2011, primarily focused on developing services in an Internal Medicine medical residency training site. However, over the last several years the outcomes achieved by the clinical pharmacist and residents have influenced expansion of collaborative pharmacist practice to other adult outpatient specialty areas, such as Inclusive Health Services, Neurology and Rheumatology. It is the philosophy of the program to develop a strong clinical practitioner with a foundation in primary care but also educate the resident on critical aspects of sustainability and the elements of developing a successful business plan. The partnership with WSoP enhances the resident experience by developing academic teaching skills and mentoring pharmacy students for future opportunities in pharmacy practice. The blend of academic and clinical pharmacy provides the resident with the skills and abilities to develop a practice in either setting. The role of academic pharmacy is to drive innovation and I believe our program accomplishes that. Program Description our pGy-2 ambulatory Care residency is offered in conjunction with st. John Fisher College-Wegmans school of pharmacy (sJFC-Wsop), in rochester, ny. it is a 12-month program that builds upon the principles and experiences of a pGy-1 residency with additional expectations that the resident gain exposure, experience and independent skill in delivering pharmaceutical care services to ambulatory adult patients. additionally, the joint venture with sJFC-Wsop offers the resident the opportunity to develop and refine academic teaching skills. upon completion of the residency, the pharmacist should be prepared to establish and maintain a practice-based clinic in the care of ambulatory adult patients and feel confident to pursue a career in academia if so desired.
Program Activities the ambulatory Care pharmacist faculty are primarily involved with the hospital-based clinics of internal Medicine, neurology, rheumatology and immune health services (ihs). our focus is on the management of high-risk disease states and high-risk medication use. our internal medicine clinic served as a pilot site for Collaborative Drug therapy Management (CDtM) practice in new york state (nys) and provided data to support CDtM legislation in nys. our pharmacist-driven CDtM services include anticoagulation, diabetes and cardiovascular risk reduction.
The PGY-2 Ambulatory Care Pharmacy Residency at Upstate University Hospital is a joint venture with St. John Fisher College - Wegmans School of Pharmacy, where the resident serves as an adjunct preceptor and teacher. This collaboration gives the resident a well-rounded experience at multiple practice settings from academia to clinical practice.

Core Longitudinal Experience Components the pGy-2 training experience in ambulatory Care has four core longitudinal experiences with an opportunity for elective rotations.
Direct Patient Care Direct patient care activities provided during either CDtM services or joint visits with the medial residents provide the resident with the skills needed to provide comprehensive medication management with focus in diabetes, anticoagulation, cardiovascular risk reduction, neurology, rheumatology and hiv. our diverse patient population provides the resident with a unique experience in patient education and motivation.

I am incredibly excited to continue my residency training in the PGY-2 Ambulatory Care Pharmacy program. The strong pharmacy presence in the adult medicine and various specialty clinics allows me to explore many facets of ambulatory care pharmacy. I have a lot of exposure to Collaborative Drug Therapy Management, which lets pharmacists in New York practice at the top of their licenses. This is augmented by the multitude of teaching and mentoring opportunities provided by the Wegmans School of Pharmacy. The academia and clinical components of this program blend together nicely to fit my needs and interests. All of the pharmacists here are amazing at what they do and they continually challenge me to improve my practice while providing plenty of support along the way. Site: Upstate Golisano Children’s Hospital the Golisano Children’s hospital is a 71bed children’s hospital with an additional 8-bed adolescent psychiatric unit located within the upstate university Downtown campus. the dedicated pediatric emergency Department services approximately 35,000 visits annually. ambulatory care services provided by hospital faculty and staff in an adjacent complex include pediatric and adolescents as well as numerous subspecialty clinics.
including in-services, clinical pharmacy rounds, pediatric grand rounds and co-precepting opportunities with pGy-1 residents and pharmacy students. Complementing the clinical experience, residents will be involved in journal club presentations, newsletter publications, drug utilization evaluations, formulary drug reviews and a structured research project. research intensity is driven by resident’s desire.

“ Upstate Golisano Children’s hospital is where I found my passion for becoming a pediatric clinical pharmacist and I am so honored to be able to continue my residency training here. The culture of the pharmacy residency program is truly unprecedented. Each and every day of my PGY1 I came in excited to learn and I owe that all to my residency program director, preceptors, and co-residents. Another major factor that went into my decision to stay at Upstate was the incredible interprofessional relationships we have within the hospital. Being able to learn alongside other medical professionals has been instrumental in my training. Upstate Golisano Children’s hospital has all of the key elements I was looking for in a PGY-2 pediatric residency. Site: Upstate University Hospital residents will have exposure to patients in inpatient areas including: iCus, emergency Department, and pediatrics as well as clinics affiliated with our infectious Diseases group.
Educational Program Overview the pGy-2 infectious Diseases residency is designed to transition pGy-1 residency graduates from generalized practitioners to independent specialized practitioners with a focus on patients with infectious diseases. residents will see patients with a variety of infectious diseases through their participation with the infectious diseases (iD) consult service, antimicrobial stewardship, and hiv clinics. additional learning experiences include critical care, pediatric infectious diseases, emergency department and microbiology laboratory. in addition to direct patient care, the

resident will complete a research project, and build leadership skills by being involved in decision making pertaining to antibiotic use through the antibiotic subcommittee and infection control committee. teaching opportunities are ample and include journal club, pharmacy rounds, iD grand rounds, iD fellow education, and co-precepting of pGy-1 residents and appe students.

PGY-2 GERIATRIC RESIDENCY “My goal for this program is to expose the pharmacy resident to every aspect of geriatric care in several advanced pharmacy practice areas. Our geriatric program strives to help the elderly patient from a pharmacotherapy perspective in many phases of care: from emergency room admission, stabilization of disease state, to outpatient management in specialized clinics. On a personal level, the relationships that develop between patient and provider make our interventions meaningful and professionally rewarding. Upon completion of the program, the resident will be well-versed in the full spectrum of care and able to carry that knowledge forward into a multitude of career settings. Site: Upstate Community Hospital residents will have exposure to a variety of unique practice settings including: the Geriatric emergency department and transitional care unit at upstate Community hospital; the Geriatric Clinic at upstate specialty services located in close proximity to the downtown hospital, and teaching through Binghamton’s school of pharmacy and pharmaceutical sciences.
Program Purpose and Description this pGy‐2 program in Geriatrics is a 12 month program that builds on Doctor of pharmacy (pharm.D.) education and the pGy-1 pharmacy residency program to contribute to the development of clinical pharmacist in the specialized area of geriatrics. the program fosters critical thinking, thereby enabling the resident to integrate their understanding of pharmacokinetics, pharmacodynamics and biomedical literature into an effective approach to optimize pharmacotherapy for geriatric patients. this program includes research in a sustainable quality improvement project or initiative for the geriatric population in a practice area of the resident’s choice. additional research opportunities are available 11
based on level of interest and are fostered by both upstate and suny Binghamton’s school of pharmacy. these research projects are designed to be publishable. this pGy-2 residency provides the resident with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care of older patients or other advanced practice settings. residents who successfully complete this accreditation eligible pGy2 pharmacy residency are prepared for advanced patient care, academic or other specialized positions, along with board certification in geriatrics.

“With the goal of becoming a geriatric clinical pharmacist involved in academia and research, I knew that Upstate University Hospital was ideal for me. Through the program’s multitude of experiences in geriatrics emergency medicine, Acute Care for the Elderly (ACE) team, transitions of care, and ambulatory care, I am confident that I will become well‐versed in caring for the older patient population in all phases of care. Upstate is geared towards meeting resident’s interests with unique elective rotations, including geriatric hematology/oncology and Healthcare for the Homeless.
