2012 SUMMER PROGRAM APPLICATION GUIDELINES AND INSTRUCTIONS Program Overview: The Bridges to Success Summer Program provides you with an opportunity to begin your college-level work in the summer prior to full-time degree seeking enrollment at Valencia College in the fall. If you are selected for the program you will register and attend courses from June 15, 2012 to August 2, 2012. Bridges students receive individualized personal attention from Valencia’s faculty and staff. These individuals are dedicated to assisting students in making a successful transition from high school to college and will help build leadership skills through community service and extra-curricular activities. Upon successful completion of the Bridges to Success Summer Program you will become a Bridges to Success Achievers Scholar in the Fall (2012). Bridges Achievers Scholars receive a renewable in-state full-tuition scholarship and book stipend that will be coordinated with financial aid to cover up to the full cost of attending Valencia College.

Eligibility: Class of 2012 High School graduates from Orange and Osceola Counties are invited to apply for the Bridges to Success Summer Program. Special consideration is given to students with financial need and/or documented risk factors that may impede entry into college.

q Submit a Valencia Application for Admissions ON-LINE at valenciacollege.edu/admissions. Please print confirmation page and attach it to your application.

q Submit the 2012 Bridges to Success Summer Program application. q Have the high school send an electronic official high school transcript to Valencia College. q Attach two recommendation forms to the application in a sealed envelope. q Submit proof of Florida Residency to the Answer Center. q Submit an electronic FINAL high school transcript indicating date of graduation from high school. (Due Date: June 22, 2012)

Assessment Checklist:

q Provide your PERT scores or take the PERT at Valencia’s Assessment Office/or submit college level SAT/ACT scores (send directly to Valencia College).

q Advanced Placement (AP) scores should be sent directly from the College Board to Valencia College. If you are accepted into the Bridges to Success Program, you will be required to attend:

ACHIEVERS PROGRAM SCHOLARSHIP GUIDELINES Bridges Achievers Scholars Checklist All items identified in this checklist must be met to receive the 2012 Bridges to Success Achievers Scholarship.

q Complete and submit the 2011-2012 and 2012-2013 FAFSA (Free Application for Federal Student Aid) Online by March 1, 2012 Please Print Confirmation Page

q Submit one FINAL official high school transcripts (indicating your graduation date) by June 22, 2012 q Successfully complete the Bridges to Success Summer Program with a Grade Point Average (GPA) of 2.0 with 100% course completion rate

q Enroll for a minimum of 9 credit hours for the 2012 Fall term and 2013 Spring term (which must include SLS 1201 Personal Development and SLS 1122 Student Success courses)

q Attend four MANDATORY workshops/skill shops (Fall/Spring) q Attend two MANDATORY Group Meetings each term (Fall/Spring) q Complete 30 hours of community service per term(Fall/Spring) at an approved location

To continue as a Bridges Achievers Scholar in future terms, you must maintain a 2.5 GPA and complete at least 75% of the courses you attempt. In the event that your grade point average falls below the required 2.5 and/ or the course completion rate falls below 75% the following criteria will apply in order to maintain scholarship eligibility: • Participate in a tutorial program • Participate in the Bridges Math Lab (if required) • Use the academic Support Labs • Participate in Academic Progress Conferences every two-weeks • Participate in other activities designed to help you succeed, as assigned • Attend ALL Bridges mandatory events • Develop and submit an educational plan
