TAKING THE HEALTH ASSESSMENT How to register for my health and wellness center… Getting ready Taking a health assessment is a great way to learn more about your overall health and track it over time. It will also help you understand what you can do to maintain and improve your health. It’s quick and easy! Remember, too, it’s completely confidential. Before taking the health assessment, know your numbers: blood pressure, total cholesterol, HDL cholesterol, height, weight, and waist circumference. If you received these numbers at a recent annual physical, you can use them to complete your health assessment now! Health screenings (Biometrics) will be offered at all Benefits/Wellness Fairs for those that may not know their numbers. Please see separate schedule to participate in a campus site screening. Appointments for the health screenings will be available with more information to come. Completion of the health assessment, with your most recent screening values, is part of your open enrollment process again this year. Use the instructions below to access and register for my health & wellness center (the name of the webpage where the health assessment is located). Your spouse or child (age 18 or older), who is currently on the plan, will also need to take the health assessment. They should register, create a personal account, and complete their own health assessment using the instructions below.

Go to myCIGNA.com
If you have previously registered for myCIGNA, log in with your User Name and Password
If you have not previously registered for myCIGNA (and are currently enrolled in the medical plan), select the “Register” button and complete the registration process

If you’re the policyholder, select the Take My Health Assessment link on the homepage, just look for the big red apple to scroll across the center of the screen. (A different page will appear for spouses and dependents, and they should select the "my health and wellness center" link.)

On the next page select your name
If you have already accessed my health & wellness center from myCIGNA, you may automatically go to your my health & wellness center home page
If you have not accessed my health & wellness center from myCIGNA, or you had to reset your user name or password, you will be asked if you’ve taken a health assessment or signed up for a my health & wellness center account:

Select “yes” if you took the health assessment previously or if you already registered the year before. Then enter your login information (this will be the same user name and password you used for myCIGNA) to access your my health & wellness center homepage where you can take the health assessment.

Select “no/not sure” to proceed to registration
You can now take your health assessment. Your data can be “saved” at any point, and you can return to complete the assessment within 72 hours. If you “submit,” instead of “save,” your incomplete health assessment, you will need to start over.
If you, your spouse and/or your dependents, age 18 or older, are a new CIGNA health plan member, you can complete the Health assessment after your effective date of coverage. For Open Enrollment changes, the effective date will be January 1, 2012. Any new enrollments will have fifteen (15) days after the effective date of coverage to complete a health assessment.
