Instructor Application Worksheet
I have three years of LEO or Corrections Experience. If you have not been sworn under Florida’s Criminal Justice Standards & Training Commission for more than 3 years, you must provide documentation of other qualifying service. Describe experience and education in the document listed below. Resume CJSTC Form 71 Instructor Certification App lication completed up to and including number 12. CMS Instructor Techniques Certificate. CJSTC Form 81 Instructor Competency Checklist completed blocks A- G. #3 & #4 signed by evaluating instructor. #6 signed and dated by Student. YOU MUST attach Instructor Evaluations completed by the students. It is recommended that you include a cover letter that briefly describes your experience and rational for approval as a FDLE Certified Instructor affiliated with the Criminal Justice Institute at Valencia Community College. When you have all the required documents, please schedule an appointment to meet with me to review your application package. High Liability Proficiency Sheet Completed during High Liability Instructor Course; Firearms -CJSTC-4, First Aid-CJSTC5, Defensive Tactics-CJSTC6, Vehicle Ops-CJSTC7. If you took your high liability instructor course at Valencia, I will pull your proficiency sheet after I receive your complete application.
