Dr. James Webb Specializes in Helping Complex Back Pain O

steoporosis can be as deadly as heart disease. In the 1980s, the disease was naturally considered a normal process of aging. As medicine evolved, it became evident that Osteoporosis was connected to fractures. Today, medicine shows that not everyone with Osteoporosis has low bone density and people can have ‘fragility fractures’ as a result of the disease’s progression. “There are thousands of doctors who do kyphoplasty in the U.S. and very few actually treat the underlying osteoporosis. As an interventional musculoskeletal radiologist, there are very few of us who actually focus on treating osteoporosis and not just fixing fractures. I was the first in Oklahoma to have a free standing clinic focused on treating osteoporosis, not just the fractures with kyphoplasty, but medically treating the underlying osteoporosis. There are probably less than 20 doctors around the country that I know that do this to the high degree that we do,” explained Dr. Webb. Though the majority of

Osteoporosis patients are over 60, he has treated patients as young as 18, and many in their 30s. He is helping drive research for vertebral fractures. Dr. Webb is offering free bone density scans to qualified patients to improve access to care as well as raise awareness of the disease.

“I really wanted to offer free screening to all patients who needed it--to raise awareness of bone health and to give back to this community that has been so supportive of us.” “Most people hear the word ‘osteoporosis’ and they think of a ninety-year-old grandmother with a hip fracture. Unfortunately, problems with bone health can impact younger patients and it also affects men.” Webb said. “Like heart disease or a stroke,

people think it can’t happen to them until it’s too late. Fractures from bad bone health are a common cause of misdiagnosed back pain. If you’re concerned about your bone health, or you have back pain that no one has been able to help, give us a call.” Interested participants should call the Dr. James Webb & Associates office at 918-260-9322 for a prescreening. Priority will be given to those who have fallen and broken a bone, have a family history of osteoporosis, or take certain medications proven to reduce bone density. Pre-screening is needed to make sure that the test is medically indicated as well as to avoid regulatory hurdles. “I really wanted to offer free screening to all patients who needed it--to raise awareness of bone health and to give back to this community that has been so supportive of us,” Webb says. “Unfortunately, government and insurance regulations actually won’t allow us to do that, so our focus is really for uninsured patients who need bone density testing”

Osteoporosis occurs when the body’s bones become weak from low bone density, low bone quality, or both. As a result, bones become weak and may break from a fall or, in serious cases, from something as slight as sneezing or minor bumps. Most often, osteoporosis is a silent killer until a fracture occurs. “There is hope for slowing down and even reversing the effects of bad bone health,” said Webb. “The key is to catch it as early as possible and then treat it medically to reduce the risk of future fractures.” Osteoporosis is often underdiagnosed. At least once a month he sees a patient who has been told they don’t have osteoporosis when in fact they do have the disease. One of the ways doctors screen is by using a DEXA bone density scan. This test does two things: Screens for risk in asymptomatic people; and

helps determine if a person’s bone density is low enough to be put on medication. Screening is good if you have a family history or any concern about Osteoporosis. Essentially, a bone density scan compares bone density to the bones of an average healthy young adult. The test results can help determine if you have osteoporosis, are at risk for it, and can be used to determine the overall risk for having a fracture. “Advanced CT scans and X-ray’s can miss half the fractures; and two-thirds of the unfound vertebral fractures heal before they receive medical treatment,” explained Dr. Webb. “A physical exam is the most technical way to detect a fracture. We treat the patient and not the imaging because we know that many fractures are painful for weeks before we can detect the collapse on imaging.” He specializes in innovative less invasive treatments to treat Osteoporosis and fractures before they become severe. He uses a variety of treatments including kyphoplasty, steroid injections, ablations and other pain-relieving procedures.

Today’s Science: Solutions for Back Pain What is Kyphoplasty? Does it Relieve Chronic Back Pain? Why are Vertebral Compression Fractures (VCF) important?

A vertebral compression fracture is often the first sign of osteoporosis, but they are usually missed by imaging. For example, 86% of VCFs are missed on advanced CT. VCFs are also associated with high rates of death if left untreated. A recent analysis of the entire Medicare data set showed that for patients with a VCF, only 15% will survive 10 years. However, the same analysis showed that kyphoplasty more than doubles the survival rate of these patients.

What is Kyphoplasty?

Kyphoplasty is a minimally invasive procedure that quickly repairs vertebral compression fractures (VCF) and restores structural integrity to broken vertebral bone. Kyphoplasty is an outpatient procedure that uses a cavity creation device (usually a balloon) to create a space in the broken vertebra which is then filled with a special bone cement to stabilize the fracture and reduce pain.

How does Kyphoplasty help?

The goal of kyphoplasty is to restore the broken vertebra to a more normal height and to fix the pain caused by the fracture. When a vertebral fracture occurs, it most commonly presents as an acute worsening of chronic lower back pain. Many times the patient’s chronic back pain is getting worse, but it’s actually due to a recent fracture. Many of our patients experience immediate pain relief following kyphoplasty. It can help relieve pain by stabilizing the fracture. Improved mobility results because of decreased pain levels.

What happens during the Kyphoplasty procedure?

• Needle Placement - Under IV sedation and using x-ray guidance, a

pencil-sized needle is placed through a small (quarter-inch) incision and a working channel is made in the fractured vertebral body. Cavity Created – A balloon is generally used to create a space and to reduce the fracture to more anatomic alignment. Devices Removed – The balloon is deflated, leaving a cavity in the vertebral body. Fracture Stabilized – The cavity is filled with bone cement to stabilize the fracture. Once filled, the incision is closed. End of Procedure – With the process completed, an internal cast is now in place. This stabilizes the vertebral body and provides rapid mobility and pain relief. The procedure takes about 30 minutes and has a short recovery time, usually a couple of days for most patients.

After Kyphoplasty

Patients treated with balloon kyphoplasty have significantly better outcomes in terms of pain reduction, quality of life, function and mobility both initially at one month, and throughout the months and years after treatment. 94% have complete or significant pain reduction and 67% have improved mobility. While kyphoplasty is highly effective for improving mobility and decreasing pain in patients with spinal fractures, it does not treat the underlying osteoporosis itself. It’s important for your doctor to treat the underlying bone disease (osteoporosis) causing the fractures. This decreases the risk of future fractures and generally makes spinal fractures less frequent.

Kyphoplasty vs. Vertebroplasty

Kyphoplasty and vertebroplasty are both medical procedures designed to treat pain associated with vertebral compression fractures. The main difference is vertebroplasty does not create a cavity and there is higher likelihood of cement leakage. Kyphoplasty has a significantly lower complication rate compared to verteboplasty and more than doubles the 10-year survival rate in patients over 65.

For the Tulsa Rock and Mineral Society, it’s a passion they want to share with everyone. On the weekend of July 17-18, the Society will host its annual Rock, Gem, Mineral, and Jewelry Show at the Exchange Center, Tulsa County

Fairgrounds. Saturday the show will run from 9 a.m. to 6 p.m. and Sunday 10 a.m. to 5 p.m. The show is open to the public, and will amaze and inspire visitors with gems, beads, fossils, jewelry, crystals and much more! There will be many exhibits to explore, including a kid’s zone

with interactive stations and fun activities, such as fossil excavations and growing crystals. A market with over 27 vendors will be on-site, with items available for sale including jewelry, fossils, artifacts, minerals and gems. Working exhibits will provide entertaining education while demonstrating lapidary arts (work involved in cutting, polishing, or engraving stones or gems), jewelry making, stone cutting and much more. There will be something for everyone to enjoy! “I would be shocked if someone came and was bored at the show,” said Event Director, Eric Hamshar. “There’s so much to do and see!” One of the most popular exhibits will be returning this year, the fluorescent mineral rock room. “There are certain minerals that will fluoresce under UV lights that are special for this activity,” he said. “The rocks don’t look all that special until the room is made dark and the UV lights are turned on. All of the rocks

just glow. It’s amazing.” Eric is an active member with the Tulsa Rock and Mineral Society and is the Curator at the DW Correll Museum in Catoosa. The museum mostly houses the eclectic personal collection of DW Correll and contains countless minerals, gemstones, stones, and fossils from around the world. A recent addition of a gift shop provides an opportunity for visitors to purchase stones, crystals, fossils, jewelry and more to enjoy. The Tulsa Rock and Mineral Society was organized in 1958, “to promote interest in rocks, minerals,

fossils, artifacts, gemstones, lapidary, jewelry-making, and all forms of earth science.” The show is hosted each year as a fundraiser to produce funds that provide for educational outreach and scholarships. “I do it because I love the rock club, and I want it to flourish and succeed,” said Eric. “It’s also a great way to introduce new people into it, and grow their interest.
