QHSE Achievements
Project 886200 recently celebrated the milestone achievement of 4 Million safe man- hours without any Lost Time Injury. Founder of Galfar- Dr. Mohammed Ali and Executive director Mr. Satish G Pillai took part in the celebration and appreciated the project team. Our Galfar Al Misnad has been awarded by Qatar Red Crescent for successful participation and completion of the “Workers health awareness program� which organized by QRC. Galfar has obtained this certification for the campaign held on December 2016.
QHSE Achievements
Galfar Almisnad has received a appreciation certification from Qatar Green Building Council for the participation in the campaign of “No paper day – April 2017” which was organized by the QGBC as a part of Environment initiative. More than “one ton” of paper waste has been recycled during this campaign. Dukhan- Flow line project # 7 7440 (EPIC for Route survey, Design, Construction, Installation & Hookup of Oil, Gas lift flowline, PWI spur lines in Dukhan field) Received the MEED Quality award - National winner in the Oil & Gas project category.

Q1 Safety performance was very good in terms of total injuries reduction, but there were ten first Fatality 0 0 aid cases and four medical aid cases and one lost time case, these LTIFR < 0.4 0.04 injuries taught us many lessons to improve Safety in our workplace. The above analysis shows, most of the injuries Are caused during improper material handing & hand tools operation. QHSE team is coming up with a program on Material handling & hand tool Safety, Every employee of Galfar has a role in reducing the incidents and improving the safety culture, let us together work for making our workplace safer. HSE Campaign Report
In a row of 3rd consecutive year, The â&#x20AC;&#x153;Drive safe campaign has been organized along with Plant department in January 2017. Nearly 625 driversoperators and 200 staff were attended the awareness session & the campaign covered all the sites and camp premises. 311 employees including staffs were awarded with appreciation certification and rewards for their incident free records.

Say no to Alcohol In
a sad indictment on the construction industry, trades are among those workers most likely to report their level of consuming alcohol may cause them harm on the job. Workers in construction are also among those at greatest risk of using drugs. Statistics indicate about 50 per cent of workers in the building and construction industry drink alcohol at levels that put them at risk. Among the significant negative impacts of alcohol use are: • accidents and injury • absenteeism • lower productivity • staff turnover • poor morale and working relationships. A number of factors may contribute to increased risk of alcohol use in the workplace including: • shift work and working long hours • difficult or dangerous work • work-related stress • lack of job security • younger age • male gender • availability of alcohol • Worker attitudes and beliefs towards them • poor industrial relations • limited supervision • low pay • boredom
What can we do? The most effective programs for management of alcohol and drug use are whole of workplace strategies involving management, employees and visitors. The four basic components to effective alcohol and drug programs are: • A written drug and alcohol policy should be clearly communicated to all workers. It should be developed in consultation with workers, apply to everyone, clearly state what is acceptable behavior and outline the consequences of any unacceptable behavior. (At Galfar, The policy is already in practice)
Say no to Alcohol â&#x20AC;˘ Education and training is essential to make managers and workers aware of the policy. Implementation of the policy should involve a range of education and training strategies to improve understanding of alcohol and drugs including meetings, education sessions, newsletters, posters or workplace drug testing. â&#x20AC;˘ Counselling and treatment for workers with alcohol or drug problems and referring them to general practitioners, counsellors, community agencies or employee assistance programs. â&#x20AC;˘ Regular evaluation (an annual review process) to determine if the workplace policy is reaching its intended audience and meeting its objectives. Workplace alcohol and drug programs are most effective if they are tailored to suit the specific conditions, needs and resources of individual workplaces. They should also adopt a multi-component, whole of workplace approach to create a culture that minimizes the risk of alcohol and drug-related harm.
HSE Alert Incident type Near-miss (Unsafe act & Condition)
Incident Description During a site visit at one of our jobsite, it was observed that some employees were sitting inside a make shift shed like arrangement at laydown area (plywood placed over the gap in between adjacent stacked blocks) It is revealed during the query that these workers make use of this arrangement for intermittent rest as temporary rest area during work intervals. What could have happened
Cause • Laydown area without rest shelter • In-sufficient awareness about workplace hazards (Employees)
Getting injured due to • Falling down of blocks by slippage. • Biting insects • Being hit by operating equipment at/nearby the area without knowledge of persons (operator) inside the make shift shed since it has single entry/access. Learnings and safe practice to be adopted •
Temporary facilities should be provided with rest shelters if it located away from the work-site

Employees should be educated regarding the work-place risk and their prevention measures and as well as Do and Do-not at workplace
•
The workplace should be checked thoroughly at lay down areas, before loading / unloading / material shifting by heavy equipment
•
Workplaces including laydown area should be provided with continuous supervision
Quality Assurance Vs. Quality Control
Many of us always have a confusion with the terms Quality Assurance and Quality Control. Well! Here let us discuss clearly about the terms and eventually you would understand what are its differences? Before starting, let us go through some basic terms related to the Quality. What is Quality? Quality is a measure of excellence or a state of being free from defects, deficiencies, and significant variations, brought about by the strict and consistent adherence to measurable and verifiable standards to achieve uniformity of output that satisfies specific customer or user requirements. Let’s give the differences between QA and QC. We will show samples of QA and QC actions on forth-coming tabulation. Quality Assurance • The planned and systematic activities implemented in a quality system so that quality requirements for a product or service will be fulfilled. • Means of providing confidence that quality requirements will be met. Quality Control • The observation techniques and activities use to fulfill requirement for quality. • Mechanism used, such as checks or tests that are performed to ensure that requirements are met.
Quality Assurance Vs. Quality Control Know the difference between QA Vs. QC Item Project Procurement
Quality Assurance (QA) QA provided Scope of Work, Conditions of Contract, Specification.
Quality Control (QC) QC is to adopt and follow what are the requirements in the Scope of work, Condition of Contract, Specification. And to execute the project as per such standards in no more no less. QC shall make sure that all works to be executed as per standards and drawings.
Drawing
QA ensures drawings are submitted and approved by the Local Authorities and or Engineer for site application.
QC will conduct periodical checks of revisions and issue log of drawings to staff to update them for the new revisions. QC will withdraw uncontrolled copy of drawings.
Materials
QA is to make sure materials is submitted via Material Approval Request (MAR) and shall ensure compliance of specification.
QC is to Monitor the latest revision of Material Approval Request (MAR) by acquiring material log sheet from Document Control. QC is to check materials upon arrival on site to verify compliance from the approved MAR. QC is to conduct site surveillance for the materials used on site.
Document Controller
QA to record the outgoing and incoming of the materials submittal or MAR, Drawings and Related Documents and make sure recorded it in log sheets. Document controlling process or procedure set out in the company procedures shall be followed.
QC shall make sure that such documents are stamped and filed in a manner that it can be searchable. When placed in box files make sure clearly labeled.
QA/QC Engineer
Example: Ceramic tile Material QA shall make sure that material has MAR approved.
QC shall issue a Material Inspection for checking with the Engineer. QC shall verify the size, thickness, color shade and the supplier. Requirements from approved MAR shall be verified by means of such physical check.
Site activity
QA is concern with the fabrication of Method Statement, Inspection and Test Plan including Checklist for each activity in order to meet the requirements.
QC is to check and verify the activity, material and tests by means of Inspections as detailed in the MS and ITP. Stages of activity that will satisfy standard or requirements shall be written on Checklist.
QC shall stamped drawings with "Controlled Copy" for the latest revision and otherwise stamped as "Superseded". Drawing shall Stamped "Issued for Construction" when released for site use.
What is new in ISO 9001: 2015 It is important to know some of the benefits of adopting and implementing ISO 9001:2015 standard. The new standard allows: Broaden business opportunities by demonstrating compliance - Become a more consistent competitor in market place - Better quality management helps to meet customer expectations - Efficient ways of working will save time, money and resources - Improved operational performance will prevent errors and increase profits - Motivate and engage staff with more efficient internal processes - Win more high value customers with better customer service - ISO 9001:2015 is not a revolutionary change; however, lots of evolutionary changes are made (restructure and reorganization). New standard is more aligned with Project Mgt.
Key changes from the earlier version: Standards are rewritten according to so called “High Level Structure” – to use across management system standards for improved benefits. Role top management & leadership have changed significantly - Top management - ensure -requirements of the management system - integrated - organization’s processes - policy and objectives – compatible strategic direction -organization. More emphasis given on Context of the organization - organisation - determine external and internal issues - relevant to purpose - impact on organization product - intended outcome(s) of its management system - ‘issue’ covers not only problems – relevant to mgt system market assurance and governance goals Relevant needs of interested parties is the new focus. Stakeholder Terms introduced Risk management has become the foundation of management the standard - Senior management - demonstrate an understanding of business risks - impact on Context of the customer requirements - Risk management process organization Knowledge as a - critical certification - Risk replaces preventive resource action - Organization - identify - risk arises and ensure controls. Communication and awareness - Communication both internal and external stakeholders and an Risk management organization must develop a communication plan. Awareness - All employees of organization should be aware of the quality policy, objectives, their contributions to QMS, implications of non-conformities etc. Objectives and measurement - Requirements Few terms/requirements are either deleted or around quality objectives - more detailed - reorganized or not a mandatory requirement consistent with the quality policy, measurable, No mandatory requirement of “IMS manual or IMS monitored, communicated, and updated - procedures” (needs to show the evidence that established at relevant functions and levels - requirements are met) - No mandatory requirement Objectives - include plans - how to achieve - result of :Management Representative” - The word evaluation - determination of responsible person - “Preventive Action” has been deleted from the delivery of the objectives - resources required - Standard. However, purpose of preventive action What & When to be done should cover through Risk management.
