Staff training and progression Each member of staff will have specific training that is tailored toward both the needs of the people they support and their own personal development. All staff are supported to develop their skill and progress. Housing solutions We understand the importance of identifying the right housing solutions and environments for the people we support. We source and develop safe and appropriate settings with integrated solutions that promote independence. Dedicated in-house property sourcing team Collaborating with developers and housing provider partners to build bespoke properties Sourcing rental accommodation to suit specific needs; including independent and shared living Providing solutions for assistive technology and technology enabled care Undertaking adaptations Environmental checklists

We work with over 70 specialist housing providers and landlords to support people with managing their tenancy!


Housing solutions We support people to understand the options available when searching for the right home for them, including; • • •

Identifying if we have a suitable vacancy available in one of our existing locations Working with specialist housing providers and landlords to source appropriate properties Understanding if a home could be adapted to meet an individual’s requirements – our in-house property team can install specialist equipment, such as hoists and tracking, environmental controls, accessible bathrooms and assistive technologies Investing in the development of specialist purpose-built homes. We will invest our own capital or work with specialist development partners to meet the identified needs of people in a local community

Learning disabilities We ensure that every individual we support with a learning disability is able to live the life they choose. We can support them with simple things such as daily tasks and accessing the community, or assist them in finding employment or getting an education. We are committed to providing the people we support with the right care and support, how and when they choose.

Fiona Fiona loves animals and with our help, she is able to volunteer at a local stables. She enjoys riding the horses, but also helps with tasks like feeding and grooming them. Fiona also helps keep things at the stables clean and tidy. Thanks to her volunteering work, she’s now working towards an equestrian qualification!

Jenny Jenny from Flintshire really wanted to spend some time with her family by going on a holiday. With the help of staff, she was able to plan and save up the money she needed to go. Jenny loved her family holiday!


Autism Our support for autistic people is designed around their needs, interests and preferences. Our goal is to support each autistic person to achieve the best possible outcomes in their lives.

We have 33 services fully accredited by the National Autistic Society!

All of the people we support are assessed by us using autism specific tools, like envrionmental checklists both before and after their transition. Support is based around each person’s unique abilities, interests and preferences. We have a comprehensive, autismspecific learning and development pathway for staff.

In order to meet our quality standards, we work with the National Autistic Society, who set the standards of our autism support.

Case study

Alan was diagnosed with Asperger’s Syndrome, a form of autism. He wanted to reduce the level of support he needed and move towards more independent living. Working with Alan, we created a transition plan. Alan was able to make informed choices in all aspects of his progress. He even chose how his new apartment was decorated and with support, purchased furnishings. Together, we created a financial plan, allowing him to be in control.

Since moving into his new apartment, Alan is happy he now has control over his life. He is able to direct his support towards achieving clear objectives. His self-esteem has increased significantly! He knows he will continue to develop and now feels confident that he can achieve his hopes and dreams for the future.


Brain injury rehabilitation We work intensively with multi-disciplinary teams to rehabilitate and provide on-going care and support. We have a collaborative process that encourages, educates and supports people to take control of their lives after a brain injury and regain their independence. Our goal is to help your clients regain as many skills as possible, through our rehabilitation pathway. We ensure support is: • Defined based on the individual’s needs • Guided by the individual’s goals • Based in the community • Inclusive of family and support networks.


Brain Injury Rehabilitation

Case study

Darren was involved in a high speed collision and sustained an acquired brain injury in his mid-twenties. With a decompressed skull, unable to walk or swallow, he moved into one of our brain injury services where he needed two support workers.

Headway Accredited

Darren struggled to cope at the beginning of his journey but after his reconstructive skull surgery, Darren regained strength in his legs. Thanks to our physiotherapy team, he was able to take part in daily exercise. As his confidence grew, he felt comfortable trying to swallow his food, which was successful and resulted in his peg being removed. With the support of our staff and his family, Darren eventually moved back to his family home with his mother. Although he arrived home in a wheelchair, it didn’t take long for him to challenge himself and before he knew it, he was walking unaided. Darren is still living at home and flourishing, thanks to the team who supported him.

Our 16 brain injury rehabilitation services provide over 180 places for community-based rehabilitation. All of the services are accredited by Headway the brain injury association.


Prader-Willi syndrome We recognise that Prader-Willi syndrome is an integral part of the individual and we prioritise finding unique ways to manage the challenges they may face. We ensure that we provide a safe and positive environment, where the people we support are encouraged to lead a healthy and enriched lifestyle. As part of our dedication to providing bespoke care and support for each person we support living with the syndrome, we have developed our Prader-Willi syndrome specialism. This specialism means we can implement person-centred support plans, environmental assesments and behavioural support plans if required. Our Prader-Willi syndrome professionals group, made up of staff members, help drive continuous improvement in this area.

James had long term issues with his food and weight due to his Prader-Willi syndrome. Because of this, the service had to keep the fridge and cupboards in the kitchen secured using DoLS. Staff worked with James to take control of his eating and weight – promoting more independence. He kept a daily food diary and began counting the calories he had eaten. Complex nurse-led care Our nurse-led, individualised support is available 24/7 and is safe, flexible and reliable. Support can be given in the individual’s own home, or in the community. We are committed to making a difference to the lives of vulnerable children and adults with life-limiting and life-threatening illnesses and disabilities through caring clinical case management.

Case study

Benny has a learning disability, dysphagia, epilepsy and uses a wheelchair full time. He also communicates non-verbally. Staff noticed Benny was losing weight, vomiting and sometimes refusing support with eating and drinking and quickly created an action plan to help. Benny went to the first doctor and after no luck, staff spoke to a dietician and helped Benny find a second opinion. The second doctor suggested Benny go for some tests. After the tests, he was diagnosed with a hernia, gastric reflux and ulceration to his esophagus. Thanks to staff quickly noticing a change in Benny and getting him the professional help he needed, he is now on the correct medication and has a new diet plan. His weight has stabilised and he’s happier accepting support whilst eating and drinking.


Children’s Complex Care in Greater London We provide high quality, responsive, flexible support, care and /or treatment that is designed to meet individual assessed needs. We work with families to support their children’s aspirations. This supports our commitment to the philosophy of ‘every child matters’ and the five outcomes for children: be healthy, stay safe, enjoy and achieve, make a positive contribution, achieve economic well being.

Case study

Staff that supported Amy wanted to help her understand her condition so that she was able to understand the procedures that needed to take place to support her. To achieve this, a clinical manager adapted an old doll to be more relatable to Amy. She fitted it with a tracheostomy, with tapes and a gastrostomy tube. This really helped Amy get a better understanding of what procedures were needed through play. The staff were able to show her using her doll. Through giving Amy the doll, staff were able to explain how each tube helped her. For example, one would provide food and the other would help with breathing. Amy was able to relate and she’s become much more settled with her condition, knowing her doll was receiving the same treatment.


Specialist Behavioural Support Services Specialist Behaviour Support Services (SBSS) is our specialism for delivering support for people with a learning disability and/or autism who are either preparing for discharge from a long-term hospital stay or are at risk of admission due to exhibiting more complex challenging behaviour. The SBSS specialism has been developed to support the mobilisation and delivery of sustainable support for people who need enhanced services to maintain meaningful lives in their own home, away from inpatient settings. We cover: • Learning and development • Attracting and retaining the right team • Housing and property • Quality • Mobilisation

We work with health and social care professionals to develop solutions that provide the right support at the right time.

Step up support We can offer step up support for people who are at risk of admission. Step down solutions We can assist the individual with a step down solution that provides a pathway to supported living and community support.


Specialist Behavioural Support Services

Case study

Martin has lived in institutions including secure hospitals for most of his life. Because of this, Martin has had very little education and few opportunities to learn about social norms and boundaries. After being moved to one of our services, Martin was very interested in the garden. The service deputy manager noticed this and approached a gardener at a local attraction with garden. He explained Martin’s background, needs and interests and the gardener offered Martin a placement. Together, we developed risk management plans and agreed what tasks Martin would be doing at his job. While working, Martin is supported by two staff members who also take part in the tasks. Martin now works there every week and is ‘one of the family’. He feels accepted like any other staff member. He has overalls, steel toe cap boots and a high-vis jacket which has really boosted his confidence. Martin’s quality of life has increased and his behaviours have reduced to the extent that he recently went on his first ever holiday.


Transition Our proactive and person-centred approach to support ensures an individuals transition, at whatever stage of life, is positive and smooth. By working together with the individual and their support network, we can create a structured and flexible transition plan which will help to alleviate any worries. We facilitate open and honest communication throughout an individuals transition journey, providing clarity and reassurance when it is needed most. We will be there to support people in a way that suits them as they move into their new home and going forward.

Case study

Molly has a learning disability and was anxious when it was suggested she was ready to move from one of our residential services to her own flat. Our team helped Molly overcome her fears by explaining the process clearly to her. This included assuring her that her current staff team would continue to support her until her new support workers understood her needs. Molly was also told she’d be involved with recruiting her new support workers. This helped reduce her anxiety and increased her confidence. Another barrier was finding the right property for Molly, as she can be easily influenced by others. It was important the property was in a safe environment. Working with Molly, we sourced a nice residential area. Molly was also involved in choosing her furniture and decor. Molly’s confidence has increased significantly since her transition, and has even reduced her support hours.

Supported living Supported living empowers people to choose the right amount of support, letting them lead an enriching and fulfilling life. Our support is tailored to the individuals needs and we work with people to create personalised support plans based on their interests and aspirations. We work closely with specialist registered housing providers who will support the people we support with managing their tenancy. We can support people to find a home that is right for them, whether that is living alone, in shared housing or in an apartment block.

Community support We can support individuals in their own home or out in the community, and we work with them to create a tailored support plan that involves their goals, interests and preferences. The experiences people we support take part in will give them the chance to learn new skills, engage in their local community, gain a new found independence and enjoy plenty of interactive activities. Day opportunities Our passionate and caring teams will help individuals get the most of out an individuals day and their budget, so they can lead a happy life without limits. Because we are outcome based, you can expect our teams to support people with the following: Community inclusion like day trips as a group or individually or accessing leisure activities. Health and well-being such as exploring opportunities to exercise and leading a healthy lifestyle. Developing your skills and confidence in the community by supporting people to travel independently, providing training and teaching money managing skills. Case study Charlie’s supported living package was initially for 24 hours per day. His support team worked with him to develop his daily living and social skills. As a result, he now only needs 20 hours per week! During his person-centred planning, Charlie asked for help finding a job. His staff team supported him to write his CV and build his confidence. They built his skills with practice interviews and he was successful! He works 12 hours a week at a supermarket. Well done Charlie and team!


Residential care homes Our specialist residential care homes are more than somewhere to live, we offer somewhere for people we support to make their home. Our experienced staff deliver safe, personalised care and support for for the people who live in our residential services, 24 hours a day.

Our focus is on finding the right home for the people we support, within their chosen community. Our residential care homes offer long-term, safe and personalised support, 24 hours a day. Each of our homes have been carefully designed and adapted to meet the needs of the residents. We support people to do the things that are important to them, both at home and out in the community.

Residential care homes We support Dave at one of our residential care homes. Dave has physical disabilities, is non-verbal and needs full support with all of his needs. Before Dave moved to our home, his family were concerned that he wasn’t going to be out in the community or taking part in his favourite activities. Our staff supported with Dave’s transition to make it as smooth as possible. Through intensive training and continuous support, Dave’s support workers have now mastered his movements and gestures, which help them understand his needs. Dave is now at a healthier weight and is a much happier person. His family couldn’t be more pleased.

Respite Our respite care can deliver tailored support to cater for the people we support’s needs, be it an overnight stay or a placement for a few weeks. We can provide planned or emergency respite at multiple of our residential care homes for people with a variety of needs including physical disabilities and nursing care.

Staying away from home is a big step for someone and we understand and cater to that. Before their stay, they have the opportunity to visit the home and meet the staff. Visitors are always welcome and we always work collaboratively with the people we support and their loved ones to produce a support plan tailored towards them.

Case study

Rosa was living in her own flat with four care calls a day. She had severe anxiety, was lonely and extremely vulnerable in her environment as she was suffering from financial abuse. Due to her extreme loneliness, Rosa had double incontinence, a behaviour she developed to make carers return to her home. Since moving to one of our respite services, Rosa has flourished. She can now get up and visit the bathroom by herself and has even visited the hairdressers – she couldn’t remember the last time she had her hair cut! With staff on-site 24-hours a day, her anxieties have lessened and she’s able to get out and about in the community on a daily basis. Rosa has settled in well to her new, safer environment and she feels transitioning into a residential respite care home is the best thing that could’ve happened for her.


Nursing care homes Our nursing care homes provide nursing support to people with learning disabilities and significant health needs, making sure to focus on maximising the potential of each resident. We can provide 24 hour nursing care in residential, repsite and community based settings. Families can rest easy knowing that we are offering their loved one a comfortable, safe and happy environment.

We can provide support to individuals who need pressure or wound care, PEG feeding and support with their epilepsy and multiple sclerosis.


Nursing care homes

Case study

Vanessa has a learning disability and is supported by us to live independently in her own flat following a move out of nursing care services. As well as achieving this transition, she wanted to re-build relationships with the family she had not seen in 30 years. During key worker meetings and conversations, she had told staff she wanted to find her family, as she had lost contact. With Vanessa’s consent, staff contacted the social worker who provided her with relevant historical and contact information. Our staff supported her to make contact with her family, and supported her during the meetings at Vanessa’s request. Eventually, Vanessa chose to continue the visits unsupported. The relationship with her family has blossomed and continues to be maintained. Vanessa has adjusted well to her new environment, and is enjoying fulfilling family relationships.
