New Relief for Knee Pain May be on the Horizon

I
nnovations in modern medicine have brought many artificial joints and bionic body parts to patients. However, a prosthetic replacement for the meniscus - the tissue pad located between the thigh and shin bones - has not been available. Now two clinical trials underway across the U.S. may help make the “artificial meniscus” a reality, providing a new treatment option for millions of Americans with persistent knee pain caused by injured or deteriorated meniscus. The meniscus is highly vulnerable to tearing, both from physical activity and excess weight. Once damaged, it has a very limited ability to heal. More than 1 million partial meniscectomies to remove or repair a torn meniscus are performed in the U.S. every year - about the same as the total number of hip and knee replacement surgeries combined. However, many patients still experience persistent knee pain following meniscus surgery. New York area volunteer firefighter Tracy Cullum was diagnosed with a meniscus tear eight years ago, when the 42-year-old mother of two was experiencing a constant ache and the inability to put any weight on her left leg. Although she underwent a partial meniscectomy to treat the tear, her knee pain became progressively worse and resulted in a constant limp. “I went from working out seven days week to having to stop any exercise activities,” Cullum says. “And my work as a part-time volunteer firefighter was challenging, since it was difficult to
move any equipment and my mobility was compromised when responding to an emergency scene.” Cullum discovered a hospital near her home town is participating in the clinical trial of the NUsurface Meniscus Implant - the first “artificial meniscus” designed to replace the damaged one. “There are limited options for patients who experience persistent knee pain following meniscus surgery,” says Dr. Richard Alfred, orthopedic surgeon at Capital Region Orthopaedic Associates and VENUS study investigator. “It is our hope that the NUsurface implant alleviates pain in these patients, allowing them to return to their previous levels of activity.” Cullum received the implant in July 2016 through a small incision in her knee. She completed a six-week rehabilitation program, and she has now returned to daily leisure activities like hot yoga and her work as a volunteer firefighter. “My range of motion is improving,” Cullum says, “and I am ecstatic to be able to resume one of my favorite activities - volunteering together with my husband and children at our local fire department.” The NUsurface Meniscus Implant has been used in Europe since 2008 and Israel since 2011. “It is important to note that NUsurface is an investigational device that is limited by United States law to investigational use,” Dr. Alfred says. “Responses to the NUsurface can and do vary with each patient, so those interested in the trial should discuss their individual situations with a clinical trial physician.” To be eligible for the clinical studies, you must be between the ages of 30 and 75, have pain after medial meniscus surgery and have had meniscus surgery at least six months ago. Staying Youthful,

For as Long as Possible!
ife has a habit of getting in the way of our doing what it takes to stay healthy, youthful and vibrant for as long as we can. Work, family and children, unexpected life events and stressors, have a way of displacing our doing what we deep down know we can and should do to feel well and look well for was long as possible. I see patients all day long who want to lose weight and feel better. Most of the actions that will allow for this, such as regular exercise, eating a diet with minimal sugar and refined carbohydrates and trans fats, are clear to them. But somehow they keep putting these off but plan to get to them. And there are also people who are doing these things but can’t seem to lose weight and have the kind of energy they used to have and want to reclaim. And the truth is our bodies’ change, we change as we get older. But most of us can feel well, be pain-free (at least mostly) and have our bodies support us in living the life we want. Here is my approach: 1. I sit down with my patients, understand their medical history, their family history, and listen to them to get a sense of what their life is like, especially to understand the stressors on them, as well as what they enjoy doing, and how they spend their time. Then I address these as appropriate. 2. Then I get down to the nitty gritty and address the following issues: • Are they sleeping? Is there any evidence of a sleep disorder? If so I test for this and treat appropriately. • I do comprehensive blood work looking for nutritional deficiencies, hormonal deficiency or excess, and any other imbalances that can be revealed from blood saliva and urine testing. • Are there any underlying chronic infections? I see many people who are chronically fatigued who have Lyme disease, viral infections such as Epstein Barr Virus, chronic sinusitis, chronic issues with yeast, and even hidden parasites. • What is their diet like? Are they eating fast food, sugar and depleted carbs or are they eating nutrient rich foods, using high grade supplements, and avoiding excess caffeine, artificial sweeteners? I also look at food allergies and intolerances, and address histamine intolerance which can produce chronic hives and rashes. • I aggressively treat underlying thyroid problems, autoimmune disease, and testosterone deficiencies in men and other hormonal imbalances in women. These approaches can be life changing.

• Finally I offer patients the opportunity to do a detoxification program which is a great way to rid the body of the toxins we accumulate, and reset our metabolism. While this approach requires thinking and personalization I have found it to be the only way to help each person regain their potential and feel well. It takes time as well as responsibility from both doctor and patient but it Works!

fiber: What it is and why you need more of it in your diet
I
f your doctor didn’t give you this advice at your last checkup, she probably should have: 97 percent of Americans don’t get the recommended daily amount of dietary fiber they need to stay healthy. But what is fiber, and why is it good for you? Dietary fiber, sometimes called “roughage,” is a plant-based carbohydrate found in fruits, vegetables, beans, nuts, seeds and whole grains such as rice and wheat. Our bodies have a difficult time digesting fiber, which is actually a good thing - as fiber passes through the body undigested, it does a lot of good along the way! Fiber has many proven health benefits. It has been shown to improve heart health, lower cholesterol, control blood sugar, decrease the risk of stroke, help you feel fuller longer, prevent constipation and boost digestive health and your immune system. Experts like the Food and Drug Administration recommend we consume about 28 grams of fiber each day - which, it turns out, is a lot of food. You’d have to eat about 94 baby carrots, 47 stalks of celery, or 15 slices of whole-wheat bread to get your daily dose of fiber from food alone!

To help people get the fiber they need, the FDA has approved seven ingredients that can be taken as supplements or added to food to boost the amount of dietary fiber they contain. Achieve Their ‘hot’ Status Each Year
Y
ou see it every year - a “hot” new color begins appearing on phone cases, walls, ceilings, furnishings and accessories. Soon, virtually everyone who’s doing any kind of renovation or design work is incorporating that color into their project. Have you ever wondered how home color trends happen? “Our team of global color experts assess societal and cultural influences from around the world to forecast what colors will be popular in home decor, consumer goods, automotive and even on airplanes for the upcoming year,” says Dee Schlotter, senior color marketing manager for PPG in North America. “We also look at what major retailers are doing in home décor, and what’s happening in society, and people’s state of emotion because of these events. All this information helps us predict what colors will resonate with people in the coming year.” For example, immediately following 9/11, pinks and browns were popular because people craved comfort and security. For 2017, a blending of various cultural norms influenced the choice of purples as color of the year, Schlotter says.
“Culturally, we’re seeing a shift toward blending of gender, relationships, careers, workplaces and living spaces,” she says. “For example, male makeup models are now blending traditional views of masculine and feminine. Hotels, restaurants and homes are blending indoor and outdoor spaces, and modern offices are finding middle ground between work and play. Purple is such a blended color that it really speaks to what’s going on in the world, especially for our choices of 2017 color of the year, which combines gray, blue and purple all in one tone.” Shades of purple are the colors of the year for multiple paint brands including PPG Paints, Glidden and Olympic paints. Glidden’s Byzantine Blue and PPG’s Violet Verbena blend blue and gray to create a hue that appears more gray when paired with dark neutrals and bluish-purple when paired with whites, similar to that of a chameleon. Cloudberry from Olympic is a calm, soft violet that also straddles the blurred line between purple and gray. “Blended purples evoke a lush, pampering yet calm effect that’s a comforting foil to the harshness of the world right now,” Schlotter says. “These trending colors are sweet without being feminine, and gray without being somber.” The colors also mesh with the more bohemian lifestyles people seem to be pursuing, she notes. A trend toward spiritual exploration is driving interest in meditation and yoga, which blend elements of mental and physical exercise. “This fits perfectly with the purple trend because this hue is thought to represent the subconscious, and can be a meditative color in the right tone,” she says. While Schlotter says homeowners have historically been shy about using purple, this year’s hues will work well in a variety of settings. The muted nature of the colors makes them gender-neutral yet impactful, so they’re great statement colors when used in a dining room, on the front door or even as an accent wall in a child’s room. What’s more, the color pairs well with trending home materials, such as gray flooring, black tiles and gold décor accents. The unique blended qualities of the colors mean these purples can adapt to the surrounding environments and blend effortlessly. “The great thing about Violet Verbena, Cloudberry and Byzantine Blue is that they all have hints of other colors and neutrals in them,” Schlotter says. “Gray is popular now more than ever, but many consumers don’t want a monochromatic look. These beautiful purple hues pull in hints of blue and gray to form chameleon-like hues that adjust based on their surroundings. They’re not your typical purple, but rather can be looked at - and used - like a new neutral.”■ April - May 2017

Vegetable Cornbread Bake INGREDIENTS Prep Time: 15 minutes Cook Time : 45 minutes Serves: 8 3 medium (3 cups) zucchini, sliced 8 ounces (2 cups) Pepper Jack Cheese, shredded 1 (16-ounce) package frozen whole kernel corn, thawed 1 (16-ounce) jar thick and chunky salsa 1 (15-ounce) can black beans, rinsed, drained 1 teaspoon finely chopped fresh garlic 1 (8 1/2-ounce) package corn muffin mix 1/3 cup milk 1 Eggland’s Best Egg (large)

DIRECTIONS Heat oven to 400°F. Grease 13×9-inch baking dish; set aside. Combine zucchini, 1 cup cheese, corn, salsa, beans and garlic in bowl. Spoon into prepared dish. Bake 25-30 minutes or until heated through. Combine muffin mix, milk and egg in bowl; stir just until dry ingredients are moistened. Drop cornbread mixture by 1/4 cupfuls onto hot vegetable mixture. Continue baking 12-15 minutes or until cornbread is light golden brown. Sprinkle with remaining cheese during last 2-3 minutes of baking. Let stand 10 minutes before serving.■

Zaatar Dusted Poached Eggs with Avocado Toast Ingredients / serves 2 1 ripe medium-large avocado 1 Meyer lemon- zest and juice (or use regular lemon) 3 teaspoons olive oil - divided ¼ teaspoon salt ¼ teaspoon pepper 2 slices wholesome bread ½ cup baby arugula 4 radishes, thinly sliced 2 Eggland’s Best Eggs (large) - cracked into a small bowl pinch salt and pepper generous pinch Zaatar spice (or sub cumin, sumac, or coriander) Aleppo chili flakes (optional)
Directions 1. Set a medium pot, filled with two inches of lightly salted water with a teaspoon of white vinegar (optional) to boil on the stove. 2. Crack the eggs into a bowl, taking care to keep yolks intact, set aside. 3. Cut the avocado in half, remove the seed and scoop out the flesh into a small bowl. 4. Add 1 teaspoon zest, the juice from half a lemon, 2 teaspoons oil, salt and pepper and stir and mash with a fork to combine. It does not need to be smooth. Taste. Adjust salt, you want this to be slightly salty and lemony. 5. Toast the bread. 6. Once the water is boiling turn heat down so it’s just simmering. Give a stir, and slip the eggs into the simmering water, gently stirring in a circular motion around the eggs. 7. Cook for 2-3 minutes or until egg white is set. Turn heat off. 8. Spread ½ of the avocado mixture on the toast. Top with sliced radish. Top with a mound of baby arugula. Drizzle with olive oil, a little lemon, and salt. 9. Using a slotted spoon place the poached egg over the dressed arugula. Sprinkle the egg with salt, pepper and a generous pinch of Zaatar spice and enjoy!■ April - May 2017

The lowdown on
Low Down Payment Mortgage
Y
ou would like to buy, but you can’t manage that 20 percent down payment. Does this sound familiar? The down payment is the biggest impediment to buying a home according to surveys, but in reality many individuals can qualify for a mortgage with as little as 3 percent down. It is important to compare loans and do the math. Consider your closing costs (the cash you need in-hand), the monthly mortgage payment, and if that payment will go down or up in a few years. Paying a few more dollars each month in the beginning can sometimes save borrowers money in the long term. For this exercise, we compare a $234,900 home purchase (the national median home price as of December 2016), with a 5 percent down payment and a 720 FICO score. And because calculators and loan terms vary, consider these costs as examples only. A mortgage professional can provide you with specific estimates.
Conventional loan with PMI A conventional loan is a traditional mortgage from a lender that is not insured by a government agency. With a 5 percent down payment, the borrower finances the remaining 95 percent over 30 years with a 4 percent interest rate. Private mortgage insurance (PMI) is required because of the low down payment and is $78 of the monthly bill, making the total monthly mortgage payment $1,143. Pros: A borrower can get a conventional loan with PMI with as little as 3 percent down. PMI can be cancelled once 20 percent equity in the home value is reached, which means your monthly bill decreases. Cons: For some borrowers, a 5 percent versus 3 percent down payment may be a better deal as costs may be lower. However, for many prospective homebuyers looking to lock in low interest rates, build equity and home appreciation faster, an option to get into a home with the lower down payment may be better. 14

A combo loan (aka piggyback mortgage) A piggyback involves two separate loans simultaneously. In this scenario, the first “primary” mortgage covers 80 percent of the loan with a 30-year fixed interest rate of 4 percent; the second loan is for 15 percent with 10-year fixed interest rate of 5 percent; and the remaining 5 percent is the down payment. The total monthly mortgage payment would be $1,271. Pros: The borrower will not pay PMI. Cons: It may be a more expensive as the borrower will pay closing costs on two loans. And unlike PMI, the piggyback loan doesn’t cancel, but will be paid off over the term of the mortgage. The second loan often comes with higher interest rates too. FHA loans FHA loans are mortgages insured by the government through the Federal Housing Administration. The limits for FHA loans typically are lower than conventional mortgages. However, FHA mortgage insurance cannot be cancelled and must be paid for the life of the loan. FHA has other specific requirements, like the condition of the home. In this scenario, the mortgage is set at 95 percent of the home’s value with a 30 year fixed interest rate of 3.75 percent. The total monthly mortgage payment would be $1,199.08. Pros: A borrower can get a FHA loan with as little as 3.5 percent down and a FICO score as low as 600 may qualify. Cons: FHA mortgage insurance cannot be canceled, so your monthly bill won’t be reduced the way it is with a conventional loan with PMI. Also, FHA loans are subject to an upfront fee of 1.75 percent that is financed over the life of the loan. No matter what you choose, do the math and compare so you can make an informed decision. Caring for Our Military Veterans & Their Families
I
am the very proud wife of Lt. Colonel (retired) Dewey R. Wiseman Sr. His 32 years of service to this great nation has given me a special sensitivity to all men and women who defend, protect and serve the United States of America. In my continued effort to assist our military families when death occurs, there are a few things I’d like to share with you; should the need arise to inter/inurn your soldier. In order to service military members and their families well, it is of paramount importance that you have, IN YOUR POSSESSION, the service member’s DD-214. This is the all-important, Certificate of Discharge/Service. It affords honorably discharged soldiers and their spouses the right to interment/ inurnment in a military cemetery without cost (small fee may apply for spouses). This vital document will most always be the determining factor in whether or not you have burial rights. There is a widely held belief that the funeral home is responsible for producing the DD-214. This is false information. While we are certainly more than happy to assist you in your search, it is solely the task of the family. I would advise all service members to locate this documentand keep it in a safe, quick to put your hands on, and easy-to-remember place. Additionally, prepare yourself for a delayed interment. This may not always be the case however; I currently have in my care a two-star general who passed in mid-May. Arlington National Cemetery is not able to inter him until this coming September. You may experience less wait time if you consider other area cemeteries. Most families will plan a funeral date and plan the burial at a later date if the dates are too far apart. Generally speaking, a military member who resides in Maryland will qualify for Crownsville (near Baltimore) or Maryland National Cemetery in Upper Marlboro, Maryland, better known as Cheltenham. A veteran who lives in Washington, DC will typically qualify for Quantico National Cemetery in Triangle, Virginia. Veterans in Virginia will typically be interred in Quantico, Culpepper, or Danville to name a few. You will likely be required to supply proof of residency in the form of a current utility bill with the veteran’s name on it—be prepared with that. In closing, I’d also like to share that contrary to popular belief the military rarely pays the FUNERAL expense of a veteran. There are cases in which they will however; according to Mr. Ritchie in the Veterans Affairs Department in N.E.
