Postmenopausal Atrophic Vaginitis
P
ostmenopausal atrophic vaginitis, or vaginal atrophy, is the thinning of the walls of the vagina caused by decreased estrogen levels. This most commonly occurs after menopause. Menopause is the time in a woman’s life, usually between ages 45 and 55, when her ovaries no longer release eggs. She also stops having menstrual periods. A woman is postmenopausal when she has not had a period for 12 months or more. Women with vaginal atrophy have a greater chance of chronic vaginal infections and urinary function problems. It can also make sexual intercourse painful. According to the American Association of Family Physicians, up to 40 percent of postmenopausal women have atrophic vaginitis. Symptoms of Vaginal Atrophy While vaginal atrophy is common, only 20 or 25 percent of women seek medical attention from their doctor. In some women, symptoms occur during perimenopause, or the years leading up to menopause. In other women, symptoms may not appear until years later, if ever. Symptoms can include: • thinning of the vaginal walls • shortening and tightening of the vaginal canal • lack of vaginal moisture (vaginal dryness) • vaginal burning (inflammation) • spotting of blood after intercourse • discomfort or pain during intercourse • pain or burning with urination • more frequent urinary tract infections • urinary incontinence (involuntary leakage) Causes of Vaginal Atrophy The cause of atrophic vaginitis is a decline in estrogen. Without estrogen, vaginal tissue thins and dries out. It be-

A decline in estrogen can occur at other times besides menopause, including: • during breast-feeding • after removal of ovaries (surgical menopause) • after chemotherapy for treatment of cancer • after pelvic radiation therapy for treatment of cancer • after hormonal therapy for treatment of breast cancer Regular sexual activity helps keep vaginal tissues healthy. A healthy sex life also benefits the circulatory system and improves heart health. Risk Factors for Vaginal Atrophy Some women are more likely than others to get atrophic vaginitis. Women who have never given birth vaginally are more prone to vaginal atrophy than women who delivered their babies vaginally. Smoking impairs blood circulation, depriving the vagina and other tissues of oxygen. Tissue thinning occurs where blood flow is decreased or restricted. Smokers are also less responsive to estrogen therapy in pill form. Potential Complications Atrophic vaginitis increases a woman’s risk of contracting vaginal infections. Atrophy causes changes in the acidic environment of the vagina, making it easier for bacteria, yeast, and other organisms to thrive. It also increases the risk of urinary system atrophy (genitourinary atrophy). Symptoms associated with atrophy-related urinary tract problems include more frequent or more urgent urination or a burning sensation during urination. Some women may also have incontinence and get more urinary tract infections. Diagnosing Vaginal Atrophy See your doctor right away if sexual intercourse is painful, even with lubrication. Kindergarten is the New 1st Grade: Is your Preschooler Ready?
W
hen you think back to your own kindergarten experience, it probably involved a half-day program and plenty of fun activities - finger painting, sand play and nap time. Likely the hardest parts were learning the alphabet and counting to 100. Fast forward to today. While kindergarten still offers a fun learning environment, it’s certainly more focused on academic achievement. Kindergartners often know the alphabet and have solid counting skills prior to the first day of school. If students aren’t reading at the beginning of the school year, they’re certainly expected to be doing so by the end. “Kindergarten is the new first grade” isn’t just an anecdote, it’s a fact. New research from the University of Virginia compares kindergarten and first-grade classrooms between 1998 and 2010, finding that kindergarten classes have become increasingly like first grade, with more time spent on academic instruction and, ultimately, higher educational expectations. Each year an estimated 4 million children enter kindergarten. To help pre-K kids improve core learning skills and get them excited about school, it’s important to find fun, age-appropriate ways to help them learn. Consider these expert ideas for preparing kids ages 3-6 for kindergarten.
Educational apps Children today have a natural knack for technology. While it’s important to monitor and balance screen time, the right technology can help educate kids. Visit your local library, stock your bookshelf and download digital books. Additionally, make time for genuine conversation with your child. Both reading and speaking help children expand their knowledge of the English language. They’ll also learn important social skills, and you’ll also find yourself developing a deeper bond with your little one. Preschool Want kids to be prepared for kindergarten? Enroll in preschool a few days a week. This experience gives children the perfect foundation to prepare them for their educational journey. Preschools provide introductory education in writing and math, but it’s a lot of fun, too. What’s more, kids learn proper social etiquette and how to follow classroom rules. Being able to sit still, listen to the teacher, raise a hand to ask a question and interact respectably among peers are all important skills that kids will need in order to thrive in an academic environment. If preschool isn’t possible, consider joining a playgroup or visiting the library for story time, and encourage social interaction with new kids at the playground. Lyme Disease

yme disease is most prominent in the summer months, typically from April through September and is caused by a tick bite from a certain tick infected with one of the three bacterial species called spirochete borrelia burgdorferi sensulato consisting of: b burgdorferi, borrelia afzelli and borelli garinii. The b burgdorferi is the common pathogen of Lyme disease in the U.S. Deer is commonly known to be the tick vector (transporter); however dogs, horses and other domesticated animals can also be carriers. 1. The tick is found in tall grass and shrubs and can attach to animals and people walking by. 2. The tick can only infect if it stays attached for at least a day. 3. The tick is now found in half of all us counties, but is more prominent in the northeast U.S. Lyme disease has 3 stages • 1st there is the stage that occurs in the 1st month, where there is the characteristic skin lesion called erythema migrans aka “bulls eye rash” which is nonpainful annular rash at the site of the tick bite within 7-14 days after a bite in the armpit, groin, behind the knee or in the belt line that has a central clearing with or without constitutional symptoms of fatigue, loss of appetite, headache and / or fever resembling symptoms of the flu. • The 2nd stage occurs days to weeks after infection. It’s the early disseminated stage with multiple tender red nodules or lumps usually in the shin called erythema nodosum and/ or neurological and cardiac findings. • The 3rd stage is called the late stage that occurs months to few years after the infection. During this stage, there will be intermittent, persistent arthritis of 1 or more joints especially affecting the knee and/ or in rare cases causing neurological problems of encephalopathy (disease affecting brain function) or polyneuropathy. In the event that you suspect you have tick bite, you should try and remove tick yourself by using fine tipped tweezers or using gloves with tissue paper. You should not remove tick with your bare hands if a tweezer is not available. You should grab tick as close to its mouth which is closest to the skin. Gently pull the tick straight out (without twisting or unscrewing) until mouth lets go of skin. Put tick in jar filled with alcohol for possible need of later identification. Then wash your skin at bite site with a lot of warm water and soap and apply topical over the counter antibiotic for example neosporin. Then watch for infection symptoms of pain, swelling, redness or warmth around bite area.
You should see a Dr if you can’t remove the tick yourself, if the tick was attached at least 36 hours or you develop symptoms of Lyme disease. Blood test aren’t necessary to start treatment because lab test can be negative in the first few weeks after infection. However, there is a blood test that can detect the Lyme antibody 2-3 weeks after exposure and there’s another lab test that can be detected several weeks after exposure and reach max levels 4-6 months and remain high levels for several yrs. Treatment includes use of antibiotics called doxycycline for 10- 21days. However, doxycycline should be avoided in pregnant women and children under 18 yrs old therefore alternative antibiotics that can be used are amoxicillin and cefuroxime. Botox to Treat Chronic FDA Approves

otox -- famous for smoothing out wrinkles on the face -- has been approved by the FDA to treat chronic migraine headaches in adults. The FDA says Botox injections have been shown to be effective in the prevention of migraines, which are debilitating headaches that cause intense pulsing or throbbing pain and affect about 12% of Americans. “Chronic migraine is one of the most disabling forms of headache,” Russell Katz, MD, of the FDA, says in a news release. “Patients with chronic migraine experience a headache more than 14 days of the month. This condition can greatly affect family, work, and social life, so it is important to have a variety of effective treatment options available.” Migraine headaches are sometimes called “sick headaches” because they often are accompanied by nausea and sensitivity to light and sound. How Botox Is Used to Treat Migraines Botox to treat chronic migraines is given at intervals of about 12 weeks as multiple injections around the head and neck to try to dull future headache symptoms, the FDA says in a statement. The FDA says it’s important that patients who suffer chronic migraines discuss with their doctors whether Botox is appropriate for them. Allergan Inc., the maker of Botox, says in a statement that the FDA’s approval applies to people with chronic migraine, which it defines as a “distinct and severe neurological disorder characterized by patients who have a history of migraine and suffer from headaches on 15 or more days per month with headaches lasting four hours a day or longer.” The company says that when treating chronic migraine, qualified medical specialists administer 31 Botox injections into seven specific head and neck sites. It says that Botox, when injected at labeled doses in recommended areas, is expected to produce results lasting up to three months depending on the individual patient.

Botox Studies “Chronic migraine is a debilitating but under-recognized neurological condition,” Scott Whitcup, MD, Allergan’s chief scientific officer, says in the company’s announcement. “Oftentimes, chronic migraine patients mistakenly self-diagnose their symptoms as headaches or infrequent migraine and treat them with drugs that provide rapid, but temporary, relief rather than seeking an evaluation, diagnosis and treatment from a qualified headache specialist.” He says with the FDA’s approval of Botox to prevent migraines, there is now a new option “to reduce the days and hours spent in pain as a result of this condition.” It says the FDA’s approval for use of Botox to fight migraines was based on the results of two studies involving 1,384 adults in North America and Europe. The studies, published in the March 2010 issue of Cephalalgia, report that patients treated with Botox experienced a major decrease in the frequency of headache days, according to the Allergan statement.■

It’s Time to Talk About Advanced Prostate Cancer
HEALTH & WELLNESS About advanced prostate cancer Testosterone, which is the primary male hormone, fuels the growth of prostate cancer cells.2 However, even when a patient is receiving treatment to help lower testosterone levels, prostate cancer can continue to progress in several ways: Castration-resistant prostate cancer (CRPC) occurs when the disease continues to progress despite low levels of testosterone.3 Metastatic prostate cancer occurs when the cancer has traveled to other parts of the body, such as the bones, lymph nodes, bladder and rectum.4 When the cancer spreads outside the prostate, and progresses despite treatment, it is called metastatic CRPC.5 Several studies have estimated that, within five years of diagnosis, 10-20 percent of men with prostate cancer will develop CRPC.6 This may seem overwhelming, but fortunately there are ways to educate yourself to be prepared for possible progression and help make well-informed treatment decisions with a physician. How can a doctor diagnose advanced prostate cancer? Unlike some other types of cancers or diseases, men living with advanced prostate cancer may not experience symptoms.2 High levels of prostate-specific antigen (PSA) may indicate cancer or other prostate health issues.2 Other symptoms of advanced prostate cancer may include bone pain or problems urinating.7 If appropriate, a doctor may recommend additional tests such as MRI, CT, PET or bone scans to confirm if the cancer has spread.4 An advanced prostate cancer diagnosis - next steps Everyone’s experience with advanced prostate cancer is unique. There is no one-size-fits-all option, so it’s important that men discuss treatment and lifestyle priorities with their doctor who can help treat the cancer in a way that’s appropriate for him. Priorities can help the patient and his doctor have detailed conversations about what’s important to him in any future treatment.

She offers this advice: 1. Get organized. Your environment can work for you or against you. Having an organized refrigerator can be the key to success on the journey to weight loss. Keep produce where it is easily visible and accessible. Storing foods like hard-boiled eggs, chicken breast and roasted vegetables at eye-level can really help to make smarter meal choices. Clear food containers will also help to keep already prepared meals top of mind and lessen food waste, which means saving money in the long run. 2. Practice superfood swaps. Don’t cut out food cravings embrace them! Eat the flavors that you crave but swap out overly processed stuff for fresh, wholesome ingredients. Avoid products with chemicals, refined sugars and flours, 14

artificial flavors and preservatives and it will naturally lead to a healthier lifestyle. When you fill the house with healthier foods, you’ll automatically eat smarter when hunger strikes. 3. Eat your “green base.” It can be difficult to make the right nutrition choices all the time. So when you are eating more decadent food like fried chicken, Chinese or pizza, put it on a base of leafy greens like spinach, spring mix or kale. That way you will get to eat what you want, but you’ll fill up more on your superfood greens and eat less of the high-calorie foods. 4. Set the table. One of the simplest ways to start naturally eating less and enjoying food more is to eat all meals and snacks at the table. When you put food on a plate, eat at a table and sit in a chair you’ll eat much less than if you were eating while working on the computer, watching TV, standing in the fridge or driving! 5. Build a better breakfast. A healthy diet starts with a nutritious breakfast. Think whole foods instead of pre-packaged foods high in calories and packed with preservatives. t’s no secret that experiences in early and middle childhood are extremely important for a child’s healthy development and lifelong learning. Yet schools - the places where kids spend the majority of their time outside of the home during the week - often lack the resources and support needed to enable and inspire students to adopt healthier lifestyles. The solution is within reach; it rests in the hands of parents and other concerned community members who make investments (even small ones) in kids inside and outside of school to help build healthier communities and a healthier world. Dominique Dawes, an Olympic Gold Medalist, three-time Olympian and child nutrition advocate, shares her tips on how caregivers can get involved in manageable, meaningful ways to impact change within their children’s schools and communities. 1. Understand you’re not alone. Look to organizations with a footprint in your community, and seek out ways to volunteer. There’s a great organization called Action for Healthy Kids. With the help of sponsors like GoGo squeeZ, their volunteer network works to improve the health and wellness of students in schools nationwide and highlight the link between nutrition, physical activity and learning. They even have an “Every Kid Healthy Week” to celebrate the great effort schools are making. Programs like these are impactful resources for parents, students and teachers alike. 2. You may be able to help out with something on a recurring basis - a wellness or athletic program of particular interest to you. Also, ask about other ways you can serve your community. Talk to the administration at your child’s school about activities and competitions to help students take ownership of their own health. Action for Healthy Kids offers free online activities to help improve physical activity and nutrition in school, but you can also encourage your school to apply for a grant to expand your local resources. hen a death occurs, it brings with it a plethora of decisions that must be made and many, many tasks. Grief is most always accompanied by considerable angst and emotions that are often overpowering. I’m a big proponent of Pre-Planning. I witness first-hand, on a regular basis how planning for this particular family crisis can help to alleviate a great deal of the force that is often associated with grief. Here in the beautiful Prince Georges County we are comprised of mostly ordinary, working class families. Rarely do I serve families who just have $10,000.00 lying around ready to fund the expense of a funeral. The average cost of funeral services in this region is $7,500. This cost DOES NOT include the cost of earth burial aka cemetery cost. Cemetery cost should be factored in as an addition to funeral cost. Aside from those two major expenses, you may want to consider the cost of death certificates, flowers, clergy, and limousines, just to name a few. Many families are choosing cremation as an option. Although cremation holds less of an economic obligation, there is still a nominal cost associated with it. My day to day observations led me to believe many families are choosing cremation because you’ve failed to Pre-Plan. Case in Point: I am serving a family, as I write to you now, who is economically unprepared. Their initial request to me over the phone was that their mother be cremated after a traditional funeral service and wake/visitation. Once in arrangement conference, I discovered that the spouse of the deceased was a military veteran currently interred in a military cemetery. Because of the veteran’s privilege to a military interment, the spouse is afforded the right of burial alongside her husband, FREE of charge. The family was not aware of this right. Once I shared this information with the family the atmosphere in the room and the tone of the funeral arrangements quickly took on new life. The family was visibly significantly RELIEVED. They were relieved because they no longer had to compromise what their hearts wanted for their mom all along. A day after the arrangement conference the family supplied me with the veteran’s form DD-214. I am happy to report that mom is currently resting well with her spouse at Quantico National Cemetery and the family is enjoying sweet peace. id you know that in an average week, 75 percent of Americans skip doing something for themselves because they didn’t adequately plan or allow enough time for it? Does looking at your to-do list feel so daunting that it discourages you from making any progress at all? You’re not alone. A new Post-it Brand productivity study found that more than 1 in 4 Americans feel completing everything in their weekly to-do list is harder than running a marathon. Juggling multiple tasks at one time, along with a never-ending to-do list, is a common challenge for everyone - even those at the top of their game. The ‘write’ way You’re 42 percent more likely to get something done if you write it down, according to research from the Dominican University of California. Creating a to-do list is an efficient way to visualize and prioritize your tasks, both short-term and long-term. While this is always a smart financial move, a little-known way to help build your net worth is to keep taxes top of mind throughout the entire year. Reducing taxes means you keep more of what you earn, according to Nick Holeman, a financial planning expert at Betterment. com. “You can’t control the stock market, but you can control some of your taxes,” Holeman said. “Knowing how your investments affect your tax bill can help you save money not just on April 15th, but for years to come.” Check to see whether your long-term investment strategy is running efficiently with these tips from Holeman. Invest your tax refund: One smart place to invest your tax refund is in an IRA. Normally, investors might divert a portion of the refund into this account as part of a well-rounded investment strategy and claim the deductions for next year’s tax time. Invest your refund, and you may get a portion of that back in tax savings. Stay in the habit of investing that refund if you can and watch those small returns add up over time. Think several moves ahead: Investing is complex and from time to time you will have to sell some of your investments; everybody does. It might be to rebalance your portfolio or maybe your goals have changed and your investments no longer match their intended purpose. Still, smart investors need to think ahead before blindly selling parts of their portfolio. This is because selling could potentially lead to taxes. By carefully choosing which investments to sell, you can help minimize that hefty tax consequence.
One way to do this is to partner with an investment company that has the tools to make this information easy to access and understand. Betterment.com, for example, offers Tax Impact Preview, which lets investors see estimated potential tax on a sale before making the trade. If you don’t think the pros outweigh the cons, don’t do it. Reorganize your investments: Another way to potentially leverage even small tax advantages into long-term growth is to build your portfolio like an energy-efficient engine, built to run for more miles with less need to refuel. You can help accomplish this by reorganizing your portfolio. Move inefficient investments like international stocks and other assets that are taxed more often into a tax-deferred account, such as an IRA or a Roth IRA. That way, you can enjoy the high growth for less tax. Then, move lesstaxed assets, such as municipal bonds, into taxable accounts. Benefit from losses: Help keep your portfolio in balance by selling off the laggards and replacing them with a similar investment. You can receive a tax deduction from your losses that can help cancel out the taxes you owe on assets that have gains. This is done automatically for investors at many automated services through a strategy called tax loss harvesting. Smart investors should always remember that investments involve risk and may result in loss. Give to a worthy cause: While it’s important to secure your future, many investors see community support as an important goal. Consider donating a to a nonprofit organization in your community. Sweet Potato Bowl INGREDIENTS Prep time: 10 mins/Cook Time: 25 mins Servings: 2 1 large sweet potato 2 teaspoons olive oil, separated salt and pepper 2 Eggland’ Best Eggs (large), poached 1 cup ground sausage ½ cup salsa 1 avocado, diced
PREPARATION: 1. Heat 1 teaspoon of olive oil in a medium pan over med/high heat and brown the ground sausage. 2. While the sausage cooks, fill a large pot with a few inches of water and place over medium/high heat until it just starts to simmer.
3. Crack eggs individually into a ramekin or cup and create a gentle whirlpool in the water. 4. Slowly pour the eggs one by one into the water and leave to cook for three minutes. 5. Remove with a slotted spoon and drain on paper towel. 6. While the eggs poach, spiralize the sweet potato using a spiralizer. 7. Heat the other teaspoon of the olive oil in a large skillet over medium heat. 8. Add in the sweet potato noodles and cook until they just begin to soften, about 5-7 minutes. 9. Season with salt and pepper. 10. Transfer the cooked sweet potato noodles into a large bowl. 11. Drain the ground sausage and add to the sweet potato noodles. 12. Toss with ½ cup of the salsa. 13. Divide the noodles and sausage between two bowls, top each with diced avocado and a poached egg!■

White Corn Gluten-Free
Chicken Strips
Ingredients: Prep time: 15 minutes/Cook time: 35 minutes/Serves: 15
1 - 10 ounce bag of Gluten-free 100% white corn tortilla chips 4 Eggland’s Best Eggs (large) 1 teaspoon Black pepper 1 teaspoon Paprika 5 large skinless/boneless chicken breasts 3 tablespoons butter (melted to coat baking pan) optional - wing sauce of choice for dipping after strips baked PREPARATION: 1. Preheat oven to 425 degrees F. Cut chicken breasts into 1/2 inch strips. Trim any fat away from strips. Break eggs in bowl large enough to dip all chicken strips; whisk until well blended. Finely crush white corn tortilla chips with rolling pin in original (opened) chip bag or place chips in zip lock gallon size plastic bag. Add pepper and paprika to crushed chips and shake bag to mix seasonings. 2. Place chicken strips in bowl of whipped eggs-be sure to coat all strips. If using the larger portion of chicken strips
from recipe use full amount of eggs in recipe. Using a fork, take chicken strips out one by one and place in the finely crushed white corn chips. Close zip lock gallon bag or close chip bag tig htly and shake until coated with chips. Empty coated white corn chicken strips back into bowl of whipped eggs if any whipped eggs remain in bowl. Re-coat with rest of whipped eggs and then place back in chip bag and re-shake. (This step can be eliminated if all whipped eggs coated the chicken strips previously.) 3. Add butter to a 9X13 baking dish; Place pan in preheated oven and melt butter-watch closely so as not to burn the butter. Remove from oven and swirl pan to evenly spread butter. Arrange chicken strips in pan. Using a fork, turn each chicken strip over so both sides will have butter on them. Pour remaining crushed white corn chips from bag over the top of all chicken strips in pan. Return pan to oven and bake for 25 minutes at 425 degrees F. Using a fork, take pan of chicken strips out of the oven and turn over each strip. Return to oven and bake an additional 10 minutes. 4. Total baking time will be 35 minutes. Remove from oven and serve with your choice of wing dipping sauce.â– June-July 2017

Corny Salmon Cakes INGREDIENTS Prep time: 10 mins/Cook Time: 10 mins Servings: 4 Two 6-ounce cans boneless, skinless pink salmon, drained and finely flaked 1 cup dried Bread Crumbs, divided 3/4 cup pre-shredded reduced-fat Cheddar Cheese 3/4 cup frozen Corn kernels, thawed 1/3 cup light Mayonnaise 2 tablespoons Ketchup
1 large Eggland’s Best egg, beaten 1 tablespoon Canola Oil PREPARATION: Combine the salmon, 1/2 cup bread crumbs, cheese, corn, mayonnaise, ketchup, and egg in a bowl and mix until combined. Shape the mixture into 8 patties and coat with the remaining 1/2 cup bread crumbs. Heat half the oil in a large nonstick skillet over medium heat. Cook the patties until golden brown, 5 minutes. Add the remaining oil to the skillet, flip the patties and cook an additional 4 to 5 minutes.â–

With the average mid-to upscale bathroom remodeling costing in the range of $30,000, homeowners are also looking for the most cost-effective way to create that spa-like feeling.
