The Secret to Losing Weight & Getting Fit

M

ost of us would like to become more healthy, lose fat, and gain muscle. After the age of 20, we naturally lose muscle mass. This causes our bodies to use less calories, and unless we try hard through diet and exercise, most of us naturally gain more fat and lose more muscle. Not only does this cause us distress in not looking our best, but it also increases our risk for medical problems such as heart attacks, strokes, and cancer. Most people use scales to monitor their weight, or include their height in order to calculate a measurement called their BMI (body mass index). Both of these methods of monitoring weight loss and health are misleading because they do not discriminate between muscle, fat, and water weight. When someone embarks upon an exercise and healthy eating regimen, they may not lose much weight on the scale, nor change their BMI very much, and yet could have significant decreases in fat and increases in muscle, both of which are the goals of weight loss and fitness programs. Better methods than the scales are needed in order to properly monitor fitness programs. Up until now, very few people have had access to accurate ways to monitor actual muscle and fat amounts in their bodies. Calipers that measure skin fold amounts and home scales that use electrical impedance to estimate body fat are both highly inaccurate. The most accurate way to measure body fat and body muscle mass is via a method called DEXA (dual energy x-ray absorpti-

It is so accurate, that it is the preferred method of monitoring fat and muscle in medical research studies and is “the gold standard.” This method uses a tiny amount of x-rays that accurately measures the amount of fat and muscle in various parts of the body. The amount of radiation used is tiny, amounting to less than what a person is exposed to by our own environment every day. The entire scan only takes about 5 minutes to perform, and immediately gives a person their percentage of body fat, percentage of body muscle, how much muscle is located in each arm and leg (especially helpful information for athletes), as well as calculates the person’s basal metabolic rate. Knowing this information so accurately, a person can then embark upon an exercise and eating regimen to work on decreasing fat and increasing muscle. The test can then be repeated at intervals, such as every 3-6 months, to accurately inform the person about how their program is going, and where it may need improvements. DEXA to evaluate muscle and fat amounts of the body (also called body composition) is not just for research any more. he knee is the largest joint of the body. It is a complex area where the femur, the large upper bone of the leg, connects with the tibia, the lower leg bone. The patella (knee cap) slides in a groove on the front side of the femur. Each of the bony surfaces that touch each other, inside of the knee joint, are covered by a smooth firm surface called cartilage. This allows the knee to move effortlessly, allowing us the ability to walk, stand, and kneel. The bones themselves are attached to each other by strong ligaments on the outside of the knee, as well as the menisci or meniscus (types of ligaments) on the inside of the joint. Knee pain, is one of the most common problems in people as they become older. Although pain causes distress, it may also interfere with a person’s quality of life by keeping them from doing the activities that they enjoy. There are many different potential causes of knee pain, and they each have different forms of treatment in order to feel better. Determining the cause of knee pain is performed by a physician taking a good history, a physical examination, diagnostic tests x-rays, and blood tests. Doctors who are most experienced at diagnosing and treating knee problems are rheumatologists and orthopedic surgeons. Rheumatologists are doctors who specialize in treating joint and arthritis problems non-surgically. If surgery is needed, they send patients to orthopedic surgeons, but usually as a last resort. Here at Arthritis and Pain Associates of PG County, we have 6 board certified rheumatologists, as well as a nurse practitioner and a physician assistant who also specialize in rheumatology. We use the most up to date diagnostic and treatment methods in helping people who suffer from joint problems such as knee pain. For example, our doctors are able to perform a diagnostic ultrasound examination of the knee during the physical examination at the first visit. This allows us to be able to see the structures inside of the knee without even getting an x-ray. Often times, this is enough to figure out the cause of knee pain quickly and painlessly.

“Knee pain, is one of the most common problems in people as they become older.” Once the cause of knee pain is identified, the doctor can prescribe the best therapy to treat the problem. Treatments can help the patient to feel better and resume their normal activities. One of the most common causes of knee pain is osteoarthritis. It is also called degenerative arthritis and degenerative joint disease. This is the most common type of arthritis, occurring in all of us as we get older. If medications and exercise are insufficient in relieving knee pain, our doctors are able to drain fluid out of the knee, and inject medications such as cortisone or hyaluronate to help relieve pain. These treatments are two of the safest that exist for knee pain. The medication is placed at the area needed inside the knee rather than taking pills, which can potentially cause problems in other parts of the body. Hyaluronate injections, a form of treatment that is also called viscosupplementation, are a very exciting and very safe treatment for osteoarthritis of the knee. Hyaluronate (also called hyaluronic acid) is naturally produced by normal cells that produce the lubricating fluid inside of the knee joint. Hyaluronate is the same substance that gives egg white its slick and slippery quality. You can easily imagine how this slippery substance would allow the structures of the knee to move seamlessly. In fact, the knee fluid is called “synovial” fluid, which partially comes from the Greek word “-ovi”, which means egg. This alludes to how the joint fluid is very similar to the consistency of egg white. Knee osteoarthritis causes the cells of the joint to stop making as much hyaluronate. This causes the normally thick fluid to become thin and watery, which then increases pain and stiffness in the knee.


HEALTH & WELLNESS Viscosupplmentation is a treatment where our doctors remove fluid from the knee and inject a hyaluronate medication back into the knee joint. Examples of hyaluronate include Supartz, Synvisc, Hyalgan, Euflexxa, GelSyn-3, and GelOne. The injected hyaluronate stimulates the cells of the knee to start producing more hyaluronate again, increasing the lubricating properties of the synovial fluid within the knee. These types of injections provide significant improvements in pain and stiffness in 75 - 80% of people who get the injections. When they work, they can be repeated every 6 months in order to provide ongoing pain relief. Our doctors use ultrasound to help drain fluid from the knee and do the injections. We strongly believe in providing maximum patient comfort by using freezing sprays and numbing medications to make the injections as comfortable as possible. In addition, the use of ultrasound during the procedure allows the doctors to see the needle during the injection, al-

For as Long as Possible!

ife has a habit of getting in the way of our doing what it takes to stay healthy, youthful and vibrant for as long as we can. Work, family and children, unexpected life events and stressors, have a way of displacing our doing what we deep down know we can and should do to feel well and look well for was long as possible. I see patients all day long who want to lose weight and feel better. Most of the actions that will allow for this, such as regular exercise, eating a diet with minimal sugar and refined carbohydrates and trans fats, are clear to them. But somehow they keep putting these off but plan to get to them. And there are also people who are doing these things but can’t seem to lose weight and have the kind of energy they used to have and want to reclaim. And the truth is our bodies’ change, we change as we get older. But most of us can feel well, be pain-free (at least mostly) and have our bodies support us in living the life we want. Here is my approach: 1. I sit down with my patients, understand their medical history, their family history, and listen to them to get a sense of what their life is like, especially to understand the stressors on them, as well as what they enjoy doing, and how they spend their time. Then I address these as appropriate. 2. Then I get down to the nitty gritty and address the following issues: • Are they sleeping? Is there any evidence of a sleep disorder? If so I test for this and treat appropriately. • I do comprehensive blood work looking for nutritional deficiencies, hormonal deficiency or excess, and any other imbalances that can be revealed from blood saliva and urine testing. • Are there any underlying chronic infections? I see many people who are chronically fatigued who have Lyme disease, viral infections such as Epstein Barr Virus, chronic sinusitis, chronic issues with yeast, and even hidden parasites. • What is their diet like? Are they eating fast food, sugar and depleted carbs or are they eating nutrient rich foods, using high grade supplements, and avoiding excess caffeine, artificial sweeteners? I also look at food allergies and intolerances, and address histamine intolerance which can produce chronic hives and rashes. • I aggressively treat underlying thyroid problems, autoimmune disease, and testosterone deficiencies in men and other hormonal imbalances in women. These approaches can be life changing.

• Finally I offer patients the opportunity to do a detoxification program which is a great way to rid the body of the toxins we accumulate, and reset our metabolism. While this approach requires thinking and personalization I have found it to be the only way to help each person regain their potential and feel well. Regaining Confidence & Intimacy After Childbirth

T

here’s no doubt that a woman’s life changes after having a baby, ushering a flood of emotions and deep love for her newborn. However, for some women, life after childbirth can also bring about difficult emotions, like increased insecurity around appearance and intimacy. Although the majority of women positively adapt to the physical changes their body experiences during pregnancy, they often do not retain the same level of self-confidence after childbirth. According to a recent study, more than 64 percent of new moms surveyed confessed their body image had worsened since becoming a mother. In addition to feeling self-conscious about weight gain and stretch marks, women also grapple with the functional changes that occur to their bodies after childbirth as the stretching that occurs to the vaginal wall and introitus (opening to the vagina) during childbirth often lasts well beyond delivery. According to a recent study, as many as 63 percent of women felt their vagina was looser since giving birth. Loss of tone and sensation is also common for women who gave birth naturally, rather than by cesarean section. Coupled with poor self-image, functional difficulties (such as dryness, itching and looseness) after birth can lead to women feeling self-conscious around their partners, making intimacy difficult. More than 8 in 10 women experience sexual problems in the first three months after their first delivery, and 62 percent of women surveyed attributed the negative changes in their sex life to post-partum vaginal looseness. And a year and a half after childbirth, 24 percent of women are still experiencing pain during sex. We also know that a woman’s body image correlates positively with both her and her husband’s marital satisfaction.So what can we do to solve the problem nobody wants to talk about? Advances in technology, beyond the surgical interventions of the past, have made it possible to remedy some of the func-

tional issues so many women face after giving birth, helping them rebuild self-esteem and regain intimacy with their partners, yet such solutions and interventions are not regularly discussed. Thankfully, there are several ways to relieve discomfort and gain confidence after giving birth: Address your discomfort: Vaginal looseness is a common side effect of childbirth. Clench and release exercises known as Kegels are a simple way to strengthen pelvic muscles, which may help with looseness. To address vaginal dryness, many women look to manufactured lubricants as a way to ease their discomfort. Talk to your doctor: Consider a simple, non-surgical procedure to help restore intimacy after having a child. There are non-invasive treatment options that exist. Spend time with your partner: Remember why you fell in love with your partner. Make time to get out of the house on a date night for dinner or a movie.Although intimacy and self-confidence are important parts of women’s overall happiness, many women struggle to achieve them after birth, as functional problems can last well beyond delivery. As these symptoms are common, women should feel comfortable seeking remedies which can help them rebuild their self-esteem. Help, I’m Loosing My Hair

H

ealthy and thick hair is our crown and glory. It is normal to lose 50-150 hairs per day. But when excessive shedding is noted, medical attention should be sought. According to the American Academy of Dermatology, approximately 80 million people experience androgenetic alopecia or male and female pattern baldness. is is the most common form of non-scarring alopecia and is genetic. Telogen effluvium is an acute form of hair loss usually triggered by an event. Triggers include hormonal changes as seen with birth control pills, post-partum, menopause, thyroid dysfunction, and severe stress. Other causes include medication, depression, anemia, poor nutrition, and major surgery. Alopecia areata is an autoimmune disorder where immune cells attack the hair follicles. The cause of this condition is unknown but has been associated with stress and may have a genetic component. Scarring alopecias include central centrifugal scarring alopecia, traction alopecia, discoid lupus, and lichen planopilaris. In this family of alopecia, an inciting factor results in inflammation that damages and/or destroys the hair follicles. Central centrifugal scarring alopecia and traction alopecia has been associated with tight styling practices such as weaves, corn-rolls, braids, and dreads. Discoid lupus and lichen planopilaris are autoimmune conditions of unknown etiology. Treatment for hair loss must incorporate a multifaceted approach. Blood work may be performed to determine if an underlying medical condition is the cause for the alopecia. A fungal culture may be obtained to determine if ringworm may be the culprit. FDA approved treatment for androgenetic alopecia is minoxidil for both men and women while Propecia is an approved treatment for men. Prescription strength minoxidil can be compounded in a special formulation to reduce inflammation. The most common over the counter 5% minoxidil preparation is Rogaine. When used once or twice a day for at least 4 months, dormant follicles may begin to produce hairs. Active follicles will produce thicker hairs. To maintain the benefits of minoxidil and Propecia, the medications must be used daily. The suggested treatment for telogen effluvium is observation as regrowth occurs in the majority of cases. Once the underlying medical condition has been addressed, the hair follicles cycle out of the resting phase into their growing phase. Other medical treatment options include the use of minoxidil, cortisone injections to the affected area, stress management, and adequate nutrition and sleep. The treatment for scarring alopecia is more difficult as inflammation under the skin eventually destroys the hair follicles. Early medical intervention is absolutely imperative for scarring alopecia. As once the follicles are gone, regenerative treatments

will not be effective. The goal of treatment is to slow down and/ or halt the underlying inflammatory process so that additional follicles are not damaged. In the early stages of scarring alopecia, where the follicles have not been completely destroyed, then treatment can assist in pushing follicles into their growing phase. Cortisone injections and minoxidil are also used to treat those with mild to moderate involvement. It cannot be stressed enough that early intervention is crucial. It’s Time to Talk About Advanced Prostate Cancer

HEALTH & WELLNESS About advanced prostate cancer Testosterone, which is the primary male hormone, fuels the growth of prostate cancer cells.2 However, even when a patient is receiving treatment to help lower testosterone levels, prostate cancer can continue to progress in several ways: Castration-resistant prostate cancer (CRPC) occurs when the disease continues to progress despite low levels of testosterone.3 Metastatic prostate cancer occurs when the cancer has traveled to other parts of the body, such as the bones, lymph nodes, bladder and rectum.4 When the cancer spreads outside the prostate, and progresses despite treatment, it is called metastatic CRPC.5 Several studies have estimated that, within five years of diagnosis, 10-20 percent of men with prostate cancer will develop CRPC.6 This may seem overwhelming, but fortunately there are ways to educate yourself to be prepared for possible progression and help make well-informed treatment decisions with a physician. How can a doctor diagnose advanced prostate cancer? Unlike some other types of cancers or diseases, men living with advanced prostate cancer may not experience symptoms.2 High levels of prostate-specific antigen (PSA) may indicate cancer or other prostate health issues.2 Other symptoms of advanced prostate cancer may include bone pain or problems urinating.7 If appropriate, a doctor may recommend additional tests such as MRI, CT, PET or bone scans to confirm if the cancer has spread.4 An advanced prostate cancer diagnosis - next steps Everyone’s experience with advanced prostate cancer is unique. There is no one-size-fits-all option, so it’s important that men discuss treatment and lifestyle priorities with their doctor who can help treat the cancer in a way that’s appropriate for him. a Novel Approach to the Management of Knee Pain

K

nee pain is a common complaint among many patients. It is one of the most common reasons that patients seek medical attention from a physician (general practitioners, orthopedists, rheumatologists, and pain management specialists). The cause of knee pain arises from several factors. These include factors originating from inside the knee joint, those from around the joint, and that referred from nearby structures like the hip, femur bone and tibia and fibula bones. Some of the common causes of knee pain from inside the joint include arthritis, infection, fracture, tumor, chronic dislocation of the patella, joint instability, foreign body, patellofemoral syndrome, and hemarthrosis, which is blood formation in the joint. Causes of knee pain attributable to structures around the knee joint includes bursitis, tendonitis, joint instability, muscle strain, muscle sprain, infection around the joint which does not involve the joint space, and adhesive capsulitis (frozen joint). Knee pain can also be caused from systemic disease and some of the most common cases of these are rheumatoid arthritis, gout, Reiter’s syndrome, collagen vascular disease, among others. Though less common, knee pain can also be caused from other areas of the body. These include lumbar radiculopathy (commonly referred to as sciatica), lumbar spondylosis (degeneration in the spinal discs), fibromyalgia, myofascial pain syndrome, inguinal hernia, nerve entrapment, and pelvic tumors. Knee pain can be acute or chronic. When the pain is acute in the absence of trauma, it warrants seeking medical attention as some conditions associated with acute knee pain can cause serious damage to the joint, such as bleeding into the joint especially in patients on blood thinners. On the other hand, the prevalence of chronic knee pain has increased over the years. Osteoarthritis of the knee is a common disorder among older adults. Chronic osteoarthritis of the knee is often not effectively treated with our current pharmacological and interventional treatments. As an alternative to current treatment options, a new technology has been developed to effectively treat knee pain: Genicular Nerve Radiofrequency Ablation. Genicular Nerve Radiofrequency Ablation is a new cutting edge treatment currently used to treat osteoarthritis of the knee in patients who have not been effectively treated by current treatment management, those who want to delay surgery, and those who have had knee surgery and failed to have relief of their pain. The procedure is performed in two stages. These include a diagnostic Genicular Nerve block and the Genicular Nerve Ablation. The Genicular Nerve block is a test injection that consists of injecting a small amount of local anesthetic to the Genicular Nerves. If the patient has sufficient response to the test with significant pain relief, then this justifies going forward with the Genicular Nerve Ablation. The nerve radiofrequency ablation is the therapeutic treatment in which a specialized type of radio wave is applied to the nerves to effectively relieve the pain, provide long term relief and restore function to the patient. The radiofrequency ablation works by shocking the nerves that sup-

plies pain to the knee and stop them from sending signal to the brain. While this is a neuro destructive procedure, the nerves do grow back. The same technology has been used for years in treatment of neck and back pain with excellent success. One of the advantage of this procedure is the fact that it is performed outside of the knee joint. This procedure is indicated for a number of conditions including osteoarthritis, degenerative joint disease, chronic knee pain, patients who have undergone total or partial knee surgery, patients who are not candidates for knee replacement, and those who want to avoid surgery totally. Reorganize Your Kitchen to Encourage Healthy Eating Habits

ore than half of Americans say they can judge how healthy a family’s lifestyle is by the contents of their refrigerator, and more than a quarter admit that they would eat healthier if they were better organized, according to a new national survey. They’re right to associate the state of their refrigerators with the healthfulness of their eating habits, says celebrity nutritionist and healthy cooking expert Keri Glassman. “Practicing ‘mindful eating’ can be hindered by a disorganized refrigerator and a cluttered kitchen,” says Glassman. “When you’re preparing and eating meals in an organized environment, you’re more likely to pay attention to what you’re eating and whether it really does nourish your body and mind.” And while Glassman says refrigerator organization is a first step in leading a healthier life, it’s sometimes easier said than done. In fact, independent consumer research of 1,000 adults, commissioned by kitchen appliance manufacturer LG Electronics shows that more than one-third of American families say fruits and vegetables are the hardest things to keep organized in the fridge. Glassman offers tips to get more organized in the kitchen for a healthier 2017: Take control of your refrigerator “Keeping healthy items in your refrigerator front and center can actually affect the nutrition value of what you’re preparing for your family,” Glassman says. “Today’s refrigerators are making this easier with advanced organization features, like fridges with the new LG InstaView feature that makes it simple to organize your food in a way that puts the most healthful choices in easy view and reach.” This feature, now available in more than a dozen models in a variety of sizes, styles, finishes and prices, has a tinted glass panel on the right door that illuminates with two quick knocks to allow users to see what’s inside without opening the door. “And

when you see healthy choices right in front of you, you’re more likely to choose those first,” notes Glassman. Additionally, transferring packaged- or bulk-foods into clear glass containers can help keep the fridge looking fresh, and keep your foods fresher longer - which in turn, allows the enticing colors of fresh fruits and veggies to draw your attention. “A clean fridge with colorful fruits and veggies is key to healthy eating,” Glassman says. “My favorite weekend mornings are spent on color quests at the farmer’s market gathering at least seven different colored fruits and veggies. Not only does the fridge look gorgeous, it inspires fresh cooking and will improve your eating habits!” Declutter your kitchen Kitchen counters often become the repository for lots of clutter, from kitchen electronics and small appliances, to car keys and the mail. The clutter takes away valuable cooking space and can affect how healthfully you eat. “Keep a clear counter,” Glassman says. “Designate an area for keys and mail that is away from your cooking space. Don’t keep unhealthy snacks sitting out on the counter, where their high visibility will make them even more tempting.” After a meal, and especially after dinner, clear plates and clean up in the kitchen right away. Leaving chores for later will only draw you back into the kitchen, where you might be tempted to snack even though you’ve already satisfied your body’s nutritional needs. In fact, Glassman says, consider adopting a closed-kitchen policy after dinnertime. Close the door to the room, if you have one, and if your home has an open floor plan, avoid making unnecessary trips to the kitchen. “Organize your pantry and fridge with the motto, if you see it, you’ll use it,” Glassman says. “Rearrange so the healthiest choices are most visible, and the less healthy ones aren’t in easy sight.”■

house with ample windows can win your heart with silent promises of sun-drenched rooms and warm breezes wafting through open windows. The truth, however, is that even a home with windows galore can have rooms that feel dark and cave like. Some of the most-loved aspects of modern design - open, expansive floor plans, wood flooring in darker tones, walls of windows - may leave portions of a room dazzled with sun and other parts dim and poorly lit. For example, a room with a wall full of picture windows will often lack windows on other walls. Near the windows, you’ll get plenty of sunlight and some glare, but the natural light might not make it to the far corners of the room - especially if it’s a large space like a great room. What’s more, interior spaces like powder rooms, laundry rooms, mudrooms and closets often have no windows at all, making them feel small and dark. No one wants rooms that look like caves - not even if your house has an actual “man cave” or “woman cave” in it! Fortunately, a few smart decorating tricks and some budget-friendly, high-impact enhancements can help brighten rooms and relieve that cavelike feeling. Sunlight solutions Of course, the sure-fire way to get more light into a room is to add more windows, but that’s not always a practical or cost-effective solution. Instead, look for alternative ways to bring more sunlight into a room.

For example, if you have a windowless powder room, adding a transom above the door can help the room feel bigger while borrowing light from adjacent rooms. The tactic of borrowing light can also work if you’re able to put a pass through between a closed off room and an adjacent one that has windows. Decorating tricks Finally, don’t overlook the power of decor in making a room feel bright and welcoming. Light-hued walls of white, cream or pale yellow or blue not only make a room feel bigger, they evoke the visual qualities of natural light. Large windowless rooms, like a basement play room, can benefit from large framed art. Select scenes that mimic the view you might get from a picture window, such as a seascape, mountain scene or city skyline. Filtered overhead light can also create the effect of a natural light source. For example, drape light-colored, opaque fabric above a bed, so that the cloth conceals the overhead light fixture. As the artificial light shines through the fabric, it will imitate the diffused look of natural light.■

Low Down Payment Mortgage

Y

ou would like to buy, but you can’t manage that 20 percent down payment. Does this sound familiar? The down payment is the biggest impediment to buying a home according to surveys, but in reality many individuals can qualify for a mortgage with as little as 3 percent down. It is important to compare loans and do the math. Consider your closing costs (the cash you need in-hand), the monthly mortgage payment, and if that payment will go down or up in a few years. Paying a few more dollars each month in the beginning can sometimes save borrowers money in the long term. For this exercise, we compare a $234,900 home purchase (the national median home price as of December 2016), with a 5 percent down payment and a 720 FICO score. And because calculators and loan terms vary, consider these costs as examples only. A mortgage professional can provide you with specific estimates.

Conventional loan with PMI A conventional loan is a traditional mortgage from a lender that is not insured by a government agency. With a 5 percent down payment, the borrower finances the remaining 95 percent over 30 years with a 4 percent interest rate. Private mortgage insurance (PMI) is required because of the low down payment and is $78 of the monthly bill, making the total monthly mortgage payment $1,143. Pros: A borrower can get a conventional loan with PMI with as little as 3 percent down. PMI can be cancelled once 20 percent equity in the home value is reached, which means your monthly bill decreases. Cons: For some borrowers, a 5 percent versus 3 percent down payment may be a better deal as costs may be lower. However, for many prospective homebuyers looking to lock in low interest rates, build equity and home appreciation faster, an option to get into a home with the lower down payment may be better. 26

A combo loan (aka piggyback mortgage) A piggyback involves two separate loans simultaneously. In this scenario, the first “primary” mortgage covers 80 percent of the loan with a 30-year fixed interest rate of 4 percent; the second loan is for 15 percent with 10-year fixed interest rate of 5 percent; and the remaining 5 percent is the down payment. The total monthly mortgage payment would be $1,271. Pros: The borrower will not pay PMI. Cons: It may be a more expensive as the borrower will pay closing costs on two loans. And unlike PMI, the piggyback loan doesn’t cancel, but will be paid off over the term of the mortgage. The second loan often comes with higher interest rates too. FHA loans FHA loans are mortgages insured by the government through the Federal Housing Administration. The limits for FHA loans typically are lower than conventional mortgages. However, FHA mortgage insurance cannot be cancelled and must be paid for the life of the loan. FHA has other specific requirements, like the condition of the home. In this scenario, the mortgage is set at 95 percent of the home’s value with a 30 year fixed interest rate of 3.75 percent. The total monthly mortgage payment would be $1,199.08. Pros: A borrower can get a FHA loan with as little as 3.5 percent down and a FICO score as low as 600 may qualify. Cons: FHA mortgage insurance cannot be canceled, so your monthly bill won’t be reduced the way it is with a conventional loan with PMI. Also, FHA loans are subject to an upfront fee of 1.75 percent that is financed over the life of the loan. No matter what you choose, do the math and compare so you can make an informed decision. am the very proud wife of Lt. Colonel (retired) Dewey R. Wiseman Sr. His 32 years of service to this great nation has given me a special sensitivity to all men and women who defend, protect and serve the United States of America. In my continued effort to assist our military families when death occurs, there are a few things I’d like to share with you; should the need arise to inter/inurn your soldier. In order to service military members and their families well, it is of paramount importance that you have, IN YOUR POSSESSION, the service member’s DD-214. This is the all-important, Certificate of Discharge/Service. It affords honorably discharged soldiers and their spouses the right to interment/ inurnment in a military cemetery without cost (small fee may apply for spouses). This vital document will most always be the determining factor in whether or not you have burial rights. There is a widely held belief that the funeral home is responsible for producing the DD-214. This is false information. While we are certainly more than happy to assist you in your search, it is solely the task of the family. I would advise all service members to locate this documentand keep it in a safe, quick to put your hands on, and easy-to-remember place. Additionally, prepare yourself for a delayed interment. This may not always be the case however; I currently have in my care a two-star general who passed in mid-May. Arlington National Cemetery is not able to inter him until this coming September. You may experience less wait time if you consider other area cemeteries. Most families will plan a funeral date and plan the burial at a later date if the dates are too far apart. Generally speaking, a military member who resides in Maryland will qualify for Crownsville (near Baltimore) or Maryland National Cemetery in Upper Marlboro, Maryland, better known as Cheltenham. A veteran who lives in Washington, DC will typically qualify for Quantico National Cemetery in Triangle, Virginia. Veterans in Virginia will typically be interred in Quantico, Culpepper, or Danville to name a few. You will likely be required to supply proof of residency in the form of a current utility bill with the veteran’s name on it—be prepared with that. In closing, I’d also like to share that contrary to popular belief the military rarely pays the FUNERAL expense of a veteran. There are cases in which they will however; according to Mr. Ritchie in the Veterans Affairs Department in N.E. Washington, DC, the criteria is very narrow and will almost always involve pre-existing circumstances orarrangements . I love our veterans, and it is may honor and privilege to serve you.■
