Goblin Time!

Points of Interest

Happy pumpkin month! Are the little goblins ready to go? My “little goblins” are now my grandkids. Savannah is 16 and McKenna is 10. During Halloween season, Savannah heads to haunted houses with her friends…and her friends know my granddaughter well. When they head off to the houses, they help Savannah get through them. Is she scared? Not at all! Savannah has earned her black belt in Tae Kwon Do. Her friends go with her to hold her arms down and make sure she doesn’t deck someone who jumps out at her! Now, that’s what I call good friends. Savannah has matured into a smart, beautiful young lady. My husband, Manny, and I were looking through pictures yesterday. I would hold up an image of Savannah and, inevitably, Manny would guess it was McKenna. They did look alike as babies and younger little girls. As they have matured, they each have taken on their own individual look. McKenna hasn’t decided what she’ll be for Halloween this year. I hope it is special, as this could be one of the last trick-or-treating experiences she has. Last year, she dressed as a werewolf—a cute one, not a nasty one! It’s hard for McKenna to look nasty because she is such a smiler. It’s so great how she goes through life with a big, contagious grin on her face. It astounds me how quickly they have grown up. I felt that way about my kids, but with the grandkids, it’s even faster. It’s kind of bittersweet. I love to see their growth—physically, emotionally, and mentally. They are such amazing people! I get a little sad when I think how fast the time has gone since I held them as babies. Onward into fall…cold nights and the beautiful, colorful changing of the leaves. Manny and I are fortunate to live in a neighborhood that has a lot of trees. The autumn colors are so incredibly beautiful. Every day, when I’m heading home, I drive through a canopy of trees that overhang the street. This time of year, it changes on almost a daily basis. On many occasions, I have been known to pull over and take pictures. I hope you all have a fantastic October. If you can, spend some time sitting outside with family and friends. If you have a fire pit, even better! Share stories, jokes, and memories with one another, and make sure the kids and grandkids are there, too. When they are our age, they will reflect on the good times they had with Mom and Dad and Grandma and Grandpa. That’s a gift that keeps on giving! We’ll talk next month.

There’s a Chill in the Air—Let’s Eat! By Kate Beiting

Apple Coffee Cake

Mix together 2 cups of flour, 1 and 1/4 cups of sugar, 1 Tablespoon of baking powder, 1 and 1/2 teaspoons of apple pie spice, 1/2 teaspoon of baking soda, and 1/2 teaspoon of salt. In another bowl, mix 2 eggs, 8 ounces of sour cream, and 1/4 cup of melted butter. Stir the egg mixture into the flour mixture, just until moistened. Add 1 apple (peeled and shredded or finely diced) and mix. Pour the batter into a greased 8-inch springform pan. Top with a mixture of 3 Tablespoons of sugar, 2 Tablespoons of flour, 1/3 teaspoon of apple pie spice, and 4 teaspoons of butter. Bake at 350 degrees for 50–60 minutes or until a tester comes clean.

Fire-Roasted Tomatoes Wash, then halve or quarter about 8 tomatoes. Place in a single layer in a casserole dish. Put 12 garlic cloves and 2 or 3 bay leaves on top. Pour 1/4 cup of olive oil over the tomatoes. Sprinkle with salt and pepper. Bake in a 425-degree oven for 45–50 minutes, until popped and sizzling. Garnish with fresh oregano and basil. Shrimp and Bowties Heat 3 Tablespoons of olive oil and 2 Tablespoons of butter in a large skillet. Add 2–3 chopped green onions and cook for 2 minutes. Add 1 cup of frozen peas and cook for another 2 minutes. Stir in 1 cup of dry white wine and about 1/8 teaspoon of saffron. Cook until reduced by half (5 minutes or so). Add 12 ounces of

fresh or frozen peeled shrimp; sprinkle with salt and pepper as desired. Cover and reduce heat to low. Cook until shrimp are done (pink). Add 2–3 cups of cooked bowties to the skillet. Thin with a bit of pasta sauce, if needed, and heat through. Taste and add additional salt and pepper if needed. Garnish with Parmesan cheese and fresh dill or oregano.

Homemade Bolognese Sauce Finely chop 1 onion, 1 carrot, and 1 stick of celery. Sauté in 2 Tablespoons of olive oil for 5 minutes. Add 2 cloves of crushed garlic and cook for another 5 minutes. Add 12 ounces of ground beef or chuck and cook until lightly browned. Stir often and break up any lumps. Pour in 2/3 cup of red wine and stir until evaporated. Add 1 cup of milk and cook, stirring often, for another 5–10 minutes. Stir in 1 can (14 ounces) of chopped tomatoes and 1 Tablespoon of tomato paste. Add salt and pepper as desired. Cover and cook on low for 1–2 hours. Freeze some for later and serve the rest with cooked tagliatelle or fettucine. Garnish with fresh basil and shredded Parmesan.

Big Red Game Day Appetizer Cook and cool 2 pounds of ground chuck; combine with 2 envelopes of taco seasoning and 3/4 cup of water. Mix 16 ounces of softened cream cheese with 1/2 teaspoon of cumin. Spread on a plate. Start layering ingredients, repeating as desired: cooked beef, chopped canned chilies, chopped onion, grated cheddar, more beef, chopped lettuce, sliced olives, hot sauce. Refrigerate and serve with corn chips or bagel toasts. Sloppy Joes Brown 1 pound of ground beef or ground chuck. Add 1/2 of a chopped onion, 2 cloves of minced garlic, 1/2 cup of tomato sauce, 1/4 cup of ketchup, 3 Tablespoons of tomato paste, 1 teaspoon of Worcestershire sauce, 2 Tablespoons of brown sugar, 1 teaspoon of prepared yellow mustard, 1 Tablespoon of apple cider vinegar, and 1/2 cup of water. Bring to simmer (or put everything in a slow cooker on low) for several hours. Serve with toasted and buttered buns. Pizza Casserole Blend together 10 ounces of cooked noodles, 1 pound of cooked and crumbled Italian (bulk) sausage, 16 ounces (or more) of tomato sauce, 1 cup of shredded mozzarella, 3 ounces of chopped pepperoni, a small can of drained mushrooms, half a can of cream of mushroom soup, a small can of sliced and drained black olives, 1 teaspoon of Italian seasoning, and salt, pepper, and garlic powder to taste. Mix everything together and pour into a greased 13x9-inch casserole dish. Top with another cup of shredded mozzarella. Bake at 350 degrees for 45 minutes, until the cheese is bubbly. Fresh Harvest Salad In a large bowl, combine 4 diced tomatoes, 1 sliced cucumber, 1/2 of a chopped red onion, 1 small seeded and chopped

green pepper, 12 pitted Kalamata olives, 12 pitted black olives, 2 Tablespoons of capers, and 1 ear of cooked corn kernels. Blend together. Mix in 1/4 cup of olive oil and 2 Tablespoons of red wine vinegar. Salt and pepper as desired. Mix in 1 teaspoon of dried oregano and crumbled Feta or other cheese. Refrigerate and eat.

Baked Acorn Squash Cut 2 acorn squash in half, remove the seeds, and bake (cut side down) for 40 or 50 minutes at 350 degrees. While the squash is baking, make the stuffing by browning a pound of ground pork sausage and adding 1/2 cup of chopped onion, 1/2 cup of chicken broth, 1 teaspoon of curry powder, and 1/2 cup of prepared chutney. Add 1/2 cup of cooked rice, if desired. When the squash is done, spoon the sausage mixture into the center of each half. Top with chopped peanuts. Cover with foil and bake at 350 degrees for another 20–25 minutes. No-Fail Apple Crumb Pie Mix 1 and 1/2 cups of flour with 1 and 1/2 teaspoons of sugar and 1 teaspoon of salt. In a small bowl, mix 1/3 cup of vegetable or canola oil and 2 Tablespoons of milk. Pour the milk mixture into the flour mixture and mix until it forms a ball. Pat the dough into a pie pan and form a crust. Chill in the refrigerator. While the crust is chilling, peel and cut 6–8 large baking apples, like Granny Smith or a blend of several types. Blend 1/2 cup of sugar with 1 teaspoon of ground cinnamon. Sprinkle half of this mixture onto the bottom of the pie crust and mix the other half into the apples. Put the apples into the pie crust. Make the crumb topping by mixing 3/4 cup of flour, 1/2 cup of sugar, and 1/4 teaspoon of cinnamon. Spread over the pie. Bake on a cookie sheet for 10 minutes at 425 degrees, then for 50–60 minutes at 350 degrees. Serve with ice cream.

Colorful Leaves and Cozy Apparel It Feels Like Fall

What image comes to mind when you think of fall? Pumpkin-scented candles? Warm color tones? Crunchy leaves underfoot? All of these will come to fruition shortly as the Midwest’s beautiful autumn takes hold. What an incredible time of year! This colorful changing of the seasons brings the opportunity for us fashionistas to upgrade last year’s fall styles with some new, on-trend looks. At the top of my list are the vests that were shown in nearly all the fall runway shows. Since fall is the time for layering, vests are a great boon to our transitional wardrobes! We can start the fall wearing a vest on its own as a top. As it gets chilly, layer it over a white top, a t-shirt, a knit top, or a light sweater. Whether it’s a preppy sweater vest or a simple classic suiting vest, this layering piece will be seen on celebrities and the mainstream population throughout the fall and winter. Don’t forget about that winter mainstay, the puffer vest! On moderate temperature days, it’s just the outerwear we’ll need to confront the outdoors. Inevitably, the moderate temperatures will pass, and the weather will get blustery and downright cold. At that time, we’ll return to the puffer coat. Predictions are that it will be more popular than ever this winter, as designers have come up with more styles and colors that make us want more, more, more. No longer just classic black or gray, puffer coat design has come a long way since last season. Bold colors like deep purple, orange, or mustard, along with bold florals and animal prints, pepper the puffer scene. You will find availability in all lengths. The extra-long versions will keep you extra-warm all season long. If the puffer is not your style, you will love the tailored coat look for fall. Classic long tailored coats are coming back for everyone, and they never fail to look smart. Mostly offered in neutral tones, this is a coat that will go forward for years. A tailored coat looks classy over a dressy outfit or jeans, and it will serve you well all winter. In fact, designers were showing entire outfits that were based on a tailored theme. Button-downs and turtlenecks, all layered under vests and paired with loose fitting trousers à la Katherine Hepburn, will keep you looking impeccable. In the Midwest, we do love our jackets. Often, they take up most of our closet real estate. Leather or faux leather jackets are always on the forefront of our favorites, and we have some new silhouettes for fall. Gone is the ever-popular moto jacket with zippers and snaps! Replace it with a longer, looser, buttery-soft style. Belted or loose, this cut will look modern and classic for years to come. This season, there are some chic fashion trends in jackets for us to enjoy, just for fun. Cozy teddy-bear fabrics, Sherpa wool, or faux fur textures continue to be on trend for the coming fall and winter. Sometimes, we feel like popping on something that gives us a certain cool vibe, and these jackets are where it’s at! Making a comeback in jackets is patchwork and quilting. I just love these homespun looks! Oversized styles are the look of the moment and are great for fall layering underneath. The blazer is 10

Independent Insurance Advisors Putting the CARE in Medicare

We all know that we need some kind of medical insurance. When we reach a certain age, we start getting lots of Medicare information in the mail. The questions arise: How do we know what coverage we need? Which carrier will cover our needs the best? What is the coverage going to cost? Can I make changes as my circumstances change? Where do I go to get answers? At least the answer to the last question is easy! Go to Insurance Advisors, where their independent agents specialize in Medicare coverage, affordable care policies (for those under 65) and life insurance. The company, co-owned by Lisa Wilsey and Michelle Scianna Panowicz, was established 18 years ago, and they’ve been at their present central Omaha location for two years. Their website states that they “put the CARE in Medicare” by listening to your needs. Because they are independent advisors, they have the ability to write coverage with multiple Medicare insurance carriers. “We aren’t paid by the client/policyholder,” explains Kristin Ehlers, an independent agent on the Insurance Advisors team. “Payment comes to our company from the carriers.”That way, the independent agents aren’t under pressure to meet a policy quota. Many of my clients become my friends.” One of the pleasures of Kristin’s job is finding just the right coverage for her clients. “Many people don’t know the range of care available, such as for some persons under 65. Medicaid might be applicable for someone who is disabled,” she notes. “There are also low-income subsidies and plans to cover dental health, mental health, and prescription drugs.” Medicare rates are regulated by the government and can’t be adjusted, she explains, but there are ways to plan ahead for this. “‘Spend Down’ accounts can be arranged to get income and assets down to a government-accepted level,” she notes. As in any industry, the insurance industry does harbor some unethical people, Kristin says. It’s important for clients to know what is allowed and what is not. “It’s against the law for anyone to charge you for medical insurance advice,” she says firmly. “Advisors are paid by the policy carrier and local agents, not clients. If you are charged a fee for medical insurance advice, you should contact the local licensing or certifying agency or the local Medicare office to report it.” She also advises us to be proactive and get a review of our coverages so we know our options. The Spooky Season Here It Comes Again!

To me, October heralds the beginning of the holiday season, and it seems only natural that we spend this moment trying to scare the wits out of unsuspecting folks. Everyone needs a little harmless excitement now and then! Besides, we follow it in November with giving thanks for good harvests and for the blessings bestowed upon us. Then we really hit the big time with Christmas festivities. Of course, we don’t stop there! We wrap it all up with over-the-top New Year’s celebrations. I guess, once we start celebrating, it’s tough to give up all that fun. Although many of the major holidays come during the last few months of the calendar year, Halloween is the one that turns us into semi-scheming kids again. We all have special memories of Halloween trick-or-treating or pranks. Whether you are the target of a prank or acting as the instigator, keeping your sense of humor is what makes the holiday fun. Is it spooky? Sure, it can be awfully spooky. In fact, it can be just as spooky as we allow it to be. Spooky is fun! Gore and terror, however, can have some bad effects on us. We have to be sure that we don’t take it too far; that way, everyone can have fun…and that’s what Halloween is all about!

In today’s world, at least as many adults as children revere and celebrate this holiday. Like most holidays, food and apparel are focal points, and themed gatherings and parties are in style. According to retailers, millions of dollars are spent each year on Halloween preparation, including costumes, food, decorations, parties, and games. Holiday celebrations don’t often have a competitive edge, but Halloween is that rare time when we vie for honors in cooking unusual dishes, wearing outrageous costumes, and entertaining ourselves and each other with games and ghostly traditions. If your trick-or-treating days are long past, you may recall dressing up in a ghostly sheet, in a cowboy or cowgirl costume,

or perhaps in a pirate outfit. We were pretty well decked out in non-scary outfits and were usually recognized by our “victims” because at least one of our parents accompanied us around the neighborhood. Parents were probably sent out with us to keep a lid on the craziness that all kids seek when dressed in a costume. Wise parents could guide us by knowing which neighborhoods offered the best treats, which areas to avoid, and when to go home. A close friend of mine just told me that she and her friends used to start their Halloween adventure two or three nights before the actual October 31st hullabaloo. Apparently, they would knock on the front door, then run around the house and knock on the back door, then return to the front door to ring the doorbell, and then hustle off to another house full of unsuspecting residents. In the final analysis, she decided long ago that this bad habit probably greatly diminished the quality and quantity of treats finally handed out to little spooks on Halloween night. Sometimes, a prank does go just a little bit far! Sounds as though there were more tricks than treats. Also, it sounds like fun. Much of today’s neighborhood trickor-treating has been replaced by organized parties and gatherings. Shopping centers and local churches often sponsor safe and sane Halloween parties indoors, away from the unpredictable elements that the Midwest is known for. Adults are always present, and many of the yummy and sticky candy has been replaced by granola, or almonds, or apples. Is it as much fun? Well, that’s up to us! Fun is what you make it. Many adults love to throw Halloween parties. Costumes are usually required, and outlandish foods are a must. Fun? You bet these parties can be lots of fun. Life should be enjoyed! It’s good to be outrageous and step out of our everyday roles once in a while. Who says an accountant can’t be a vampire for one evening? Can a plumber become an astronaut? Why not? Everyone has a special Halloween memory. Here’s mine: In the fourth grade, one of our male classmates dressed in a costume (stuffed with pillows) and a wig with glasses to make himself look like our teacher, Miss Rosemary. Miss Rosemary did not appreciate his mimicry. She grabbed him by the ear and hauled him off to the principal’s office, where he spent the rest of the day. The lesson? Breast Health Tips, Statistics, and Treatment Options By Janette Calabro

According to the American Cancer Society (ACS), breast cancer is the most common cancer among American women, and it’s the second-leading cause of death among women, after lung cancer. (Men can also get breast cancer, but fewer than 1 percent of breast cancer diagnoses are in men.) Awareness, prevention, early detection, treatment, and life after treatment play a role in all of our lives. Let’s look at the latest information about breast health. First, a few statistics about breast cancer in American women. The National Cancer Institute states that about one in eight women will develop invasive breast cancer over the course of her lifetime. In 2021, we’re expecting about 281,550 new cases of breast cancer and approximately 43,600 deaths. The good news is that death rates in women under 50 have remained steady (no notable increase) and death rates in older women have continued to drop. The decrease in deaths is likely a result of early detection and advanced treatments. What is breast cancer and why does it happen? Breast cancer starts when healthy cells change and form a tumor. The glands that produce breast milk and the ducts that carry milk to the nipple or to other tissues are prime areas for tumors to develop. It’s important to note that not all tumors are malignant or cancerous. Before you jump to conclusions, make an appointment with your gynecologist or oncologist to gain more information. Our DNA carries orders for building proteins that control the cells that make up our bodies. A wrong directive from those inherited gene mutations can increase the risk of cancer. The “why” of breast cancer is still something of a mystery, but we know that family history plays a role in about 5–10 percent of breast cancers. In other words, abnormal genes can be passed from parent to child. A woman’s risk of developing breast cancer almost doubles if her mother, sister, or daughter was diagnosed with breast cancer. On the other hand, the vast majority of breast cancers are not caused by inherited gene mutations—that is, most people who get breast cancer did not inherit it. The highest risk factors for breast cancer are simply being a woman and growing older. Early detection is vital! Like most cancers, breast cancer is more treatable in its early stages. Once it advances or spreads to different parts of the body (metastatic breast cancer), it’s harder to treat. Because breast cancer’s major risk factors cannot be changed, it’s important to maintain your overall health with diet and exercise and to keep an eye on your breast health with self-exams. As the American Cancer Society reminds us, “women should be familiar with how their breasts normally look and feel and should report any changes to a health care provider right away.” Mammograms are recommended annually for women between 45 and 54 years old; women at higher risk may want to start annual screenings at age 40. Women 55 and older at lower risk can switch to every other year. Check with your doctor for the best guidance. 20

What exactly are mammograms? They are low-dose x-rays of the breast that help detect cancer at an early stage, when treatment is most successful. Mammograms don’t catch every cancer (especially in dense breast tissue), and you may need more tests to discover whether something found on the x-ray is or isn’t cancer. The newest 3-D x-rays are higher tech and better able to detect cancers. Fortunately, most breast imaging centers use them! Annual mammograms are one of the best preventive tools for early detection (that means finding and diagnosing the disease sooner than waiting for any symptoms). Mammograms may find breast changes years before physical symptoms are noticeable. The American Cancer Society notes that “results from many decades of research clearly show that women who have regular mammograms are more likely to have breast cancer found early, are less likely to need aggressive treatment like surgery to remove the breast (mastectomy) and chemotherapy, and are more likely to be cured.” Early physical symptoms may include a lump in the breast or changes in the breast’s appearance, but such symptoms are harder to detect once cancer has advanced. Newer mammography technology in 2021 provides more comfortable exam rooms, a curved-surface x-ray machine (shaped more like a breast) for less discomfort, upright chairs for a less intimidating biopsy (a small sample of breast tissue is removed and sent to the lab), artificial intelligence detectors for breast cancer analysis, and more personalized treatment plans. Today’s treatments for breast cancer include surgery, radiation and chemotherapy (often in combination), and some oral medications that cure breast cancer in the early stages. The vast majority (99 percent) of people who undergo breast cancer treatment in the early stages live for five years or longer after diagnosis. Treatment depends on what stage of breast cancer you’re in, the size of the tumor, molecular and genetic characteristics, and your overall health. Treatment may include hormone therapy, which slows or stops the growth of hormone-sensitive tumors by blocking your ability to produce hormones or interfering with breast cancer cells. Whether administered as an oral medication or an injection, targeted therapy has made great progress in physically blocking the abnormal protein that stimulates the growth of the cancer cells. Chemotherapy can be used to treat all stages of breast cancer, even cancer that has spread. Chemo can be injected,

administered by infusion therapy, taken by mouth, or even applied to the skin to target and destroy breast cancer cells. Pain from this treatment is usually temporary. Radiation therapy is often combined with chemotherapy for a higher success rate. These energy beams use ionizing radiation to control or kill malignant cancer cells, targeting a precise point on or in your body. Radiation damages cells by destroying genetic material (both healthy and cancerous). Normal cells often repair damage caused by this treatment. There are drugs to help with nausea, among other side effects, and spectacular wigs to cover hair loss. Immunotherapy has promising benefits for breast cancer patients who have more gene mutations or tumors with higher levels of protein. This IV infusion wakes up the body’s immune system to attack cancer cells. It’s less risky and less invasive than other treatments. Your doctor should tailor the best treatment plan. Remember, there are new trial treatments and options that your doctor can explain in great detail. A lumpectomy is a surgical procedure that removes the cancerous breast tissue while leaving a large margin of healthy breast tissue behind. This allows the patient to keep most of her natural breast shape. A mastectomy removes the entire breast in order to prevent or treat breast cancer. Your doctor may recommend a mastectomy if the tumor is widespread throughout the breast. Post-mastectomy treatments include breast reconstruction, which can help patients maintain a positive body image. With breast reconstruction, a plastic surgeon can implant new breasts that mimic the feel of natural breast tissue.

Some reconstruction uses tissue from another part of the body to form new breasts. Breast reconstruction is a major surgery, so have a serious talk with your surgeon about the pros and cons. In some cases, multiple procedures are required. If you’ve had a partial mastectomy or a lumpectomy, smaller implants or fat grafting are additional options for reconstruction. Breast cancer treatment may cause menopausal symptoms like night sweats or hot flashes. It’s not uncommon for the recovery period to last a year or longer before you feel like yourself again. Treatment and recovery from surgery, radiation, and chemotherapy can cause anxiety, depression, fatigue, and other medical conditions. Talk with your doctor about gradually starting an exercise routine and ask what foods will help you navigate to your new normal. Physical therapy and exercise after cancer treatment are vital to improve strength, range of motion, and cardiovascular fitness. Don’t forget that everything physical is directly connected to your mental well-being, too. We hear this often enough to tune it out, but it’s worth repeating: Keep your weight in check, exercise regularly, avoid too much alcohol, and don’t smoke. This lifestyle helps you get through cancer treatment and recovery and can help you avoid some cancers in the first place. Good general health, being familiar with your breasts, and undergoing annual exams are the keys to great breast health! Stark Media Films Stark Media was born out of a six-year stint as IT/desktop support for a large credit card processing company. I then stumbled onto an opportunity to make “videos” for that same company. I said yes. My role was to produce in-house corporate marketing videos. Having no real experience as a videographer, I had to adopt the “fake it ‘til you make it” mentality...and it worked. Fast forward three years. The excitement was missing, I was yearning for more meaningful projects. I became complacent and uninspired. Although it did put food on the table, I yearned to produce something with more impact and meaning. Then along came another opportunity to help produce a short film about Alzheimers. Chandelier-style lighting, comfortable seating, and artful displays invite visitors to browse at their leisure. Beau Joyau carries the most extensive collection of Select Polish Pottery in Nebraska. This centuries-old style of pottery is still made and painted by hand in Poland before Pam imports it to Nebraska. Patterns range from the traditional peacock pattern created for Prussian royalty to contemporary designs featuring polka dots and stars. Disappearing Ink Removing a Tattoo

There are many reasons to get a tattoo. It may be to honor cultural norms, to make a personal statement, or simply because you favor a certain design. There are also many reasons to get a tattoo removed. Perhaps the tattoo has blurred or faded. Maybe you no longer care for the image. Maybe you simply regret a hastily-made decision. No matter what the reason, there are ways to remove that unwanted ink. Removal methods include laser, surgical, dermabrasion, topical creams, and covering up the tattoo. There are several factors to keep in mind when considering tattoo removal. Older tattoos are easier to remove than newer ones. The colors black, brown, blue, and green are easiest to remove. The larger the tattoo, the more time-consuming and costly it can be to remove. Since tattoos are meant to be permanent, there may be some scarring or skin color variation involved. You should consult with a dermatologist or plastic surgeon to discuss the removal option best suited for you. Let’s go over what each of those options entails. Laser tattoo removal is considered to be a successful and cost-effective method, and it is most effective on lighter skin. A special type of laser (called a Q-switched laser) releases a single energy pulse that heats up the ink in the skin to dissolve it. Multiple treatments are often required to lighten and fade the tattoo. Treatments are repeated every several weeks. With laser removal, the skin is numbed with a local anesthetic prior to laser application. The skin may blister or bleed

slightly. Antibacterial ointment is applied post-treatment to help with healing. The treated area will need to be kept clean and dry. Tight clothing and sun exposure should be avoided on the treated area. Surgical tattoo removal, sometimes referred to as excision tattoo removal, physically removes the tattooed skin. This is the most invasive method of removal but is the most effective way to completely remove a tattoo. It’s best utilized for smaller tattoos. The procedure is typically performed by a plastic surgeon. During the procedure, the surgeon will inject a local anesthetic into the skin before using a scalpel to remove the tattooed skin. Finally, the remaining skin will be stitched into place. The surgery may take anywhere from one hour to several hours, depending on the size of the tattoo. Allow several weeks for the skin to completely heal. As with laser removal, it’s important to keep the site clean and dry and to avoid sun exposure. As with any surgery, there will be a scar.

Following the surgeon’s instructions closely will minimize scarring. Dermabrasion is used less commonly for tattoo removal but can often remove most of an existing tattoo. It’s typically performed at a cosmetic surgeon’s office, utilizing a high-speed rotary sanding device with an abrasive wheel or brush. The top layers of the skin are sanded down and removed, allowing the ink to seep out. The tattooed area is usually chilled or numbed with a local anesthetic prior to the procedure. The time required for the procedure depends on the size and color of the tattoo. Large, multi-colored tattoos usually require more than one hour. The treated area will likely be sore and raw for several days following the procedure, and recovery may take two to three weeks. The skin may appear pink for several weeks. An antibiotic ointment is usually prescribed to reduce the chance of infection and to help the healing process. Dermabrasion is not recommended for people with sensitive skin or those who are taking blood thinners. Some scarring may result, and the treated area may have a slightly darker or lighter appearance than the surrounding skin. Tattoo removal creams are cost-effective but are unlikely to remove a tattoo entirely; in most cases, the eventual outcome is that the tattoo is faded. Creams contain a peeling chemical such as glycolic acid or trichloroacetic acid (TCA). They work by sloughing away or bleaching the top layer of the skin. These creams should be applied by a medical professional, not at home. The most artistic option is to cover up the old tattoo with a new one. With this approach, scarring and skin discoloration are of no concern. A tattoo artist will work with you to come up with a new design to hide what you already have. Your new tattoo may involve heavier lines, more shading, or unusual shapes. This is not a new concept for tattoo artists, and they are usually quite creative in coming up with a satisfactory design. No matter how or why you choose to remove that unwanted tattoo, there is a skilled professional out there who can help you make that ink disappear. Choose a theme and invite your guests to arrive dressed in character. Eat, drink, and be scary! When it’s time for trick-or-treating, some homeowners go full haunted porch. It’s also possible to have a celebration that is lower-key but still festive. Deck out your front doorstep and greet trick-or-treaters with a colorful rug and lanterns. Cast an eerie glow outdoors by stringing orange lights around windows, and hang creepy bats, spiders, or giant yellow cat eyes to peer out at everyone who passes by. Thanksgiving is a time to recognize how thankful we are, a time to share with family and friends. Decorating for this holiday allows for lots of creativity. Chiropractic care is a form of alternative medicine that treats the musculoskeletal system, especially the spine. A chiropractor is not a medical doctor. Instead, chiropractors earn an undergraduate degree and then undergo four years of chiropractic training to earn a Doctor of Chiropractic (D.C.) degree. After their training, they must obtain a license to practice. Although the practice of spinal manipulation has been used since the times of ancient Greece, chiropractic treatment as we know it was developed much more recently. In 1895, Daniel David Palmer developed chiropractic techniques to treat patients. In 1897, he opened what is now the Palmer College of Chiropractic, a school in Davenport, Iowa, that still trains chiropractors today. The list of benefits of chiropractic treatment is long—proper spine alignment affects more than you might think! Of course, adjustments can greatly reduce many types of neck pain and lower back pain. Combined with physical therapy, chiropractic care can decrease the progression of scoliosis, a condition in which the spine twists and curves to the side.

Mask Yourself for Halloween Imaginative Costumes Make the Holiday I always enjoy this time of year. The weather change, yes, but I especially love the creativity involved in devising Halloween costumes. Mom and Dad always encouraged us to make our own costumes. One year, Mom let me wear one of her old dresses with a fancy hat and purse for accessories—I became a movie star! All three of us kids had cowboy outfits (I admired Dale Evans): jeans, flannel shirt, hat, and boots. Those costumes were especially good if it was cold for Halloween. We lived in Wyoming, and it could get pretty chilly by the end of October. Nobody bought costumes then; we always came up with something from home. We did the same thing with our kids. When our oldest was about a year old, we took him to visit Grandpa and Grandma. He was in a walker, which allowed him to scoot around. We put a large paper bag over his head with a hole cut out for his face and wrote BOO! on the front in big, bright letters. Then we put him on G and G’s porch and rang the doorbell. What a surprise! It was their first Halloween with their first grandchild. As they got older, the kids were baseball or football players, ghosts, clowns, cowboys, dancers, and movie stars. Costumes have to be devised again every year…even though the kids and their friends are now adults. When they

gather for a Halloween party, having a costume is mandatory. These “kids” are clever! One guy dressed as a “biker babe” in leather chaps and a blond wig. Another year his buddy borrowed the blond wig and went as television favorite “Dog the Bounty Hunter;” his wife dressed appropriately as Dog’s wife. One couple came as “cereal” killers, attaching miniature cereal boxes with plastic knives stuck in them to their clothes. One of the boys happens to have frizzy hair; he bought a paintbrush and artist’s palette and became Bob Ross. Initially, some of their friends rented costumes…a deed they regretted when comparing the cost to homemade. They all decided that the most effective costume is one that uses your imagination. A hooded sweatshirt with the hood drawn tight hides your natural hair and covers part of your face. Change your hair color with a wig; don a hard hat to become a construction worker; wear an apron and wield a spatula to become a chef. Whether you and your kids go out or stay at home, costumes are part of the holiday fun! What costumes will you come up with this year? Use your imagination! It’s always more fun when everyone dresses up—kids and adults alike. Happy Halloween!

Fall Bulbs It’s Planting Time Again!

It is fall, the evenings are getting cooler, and gardens are starting to shut down for the season. Unlike my garden, I can’t rest—my mind is already buzzing about when I need to start planting bulbs to guarantee that the last stages of winter will be quickly wiped out by next year’s early spring blooming! The time for fall bulb planting is here. Before buying any bulbs or digging any holes, think about your color scheme. A single color will create a stunning, dramatic impact. Two complementary colors can also make a vibrant statement. Orange and purple are simply gorgeous together, as are red and yellow. If you prefer, you can just spin the color wheel and go crazy with a whole range of colors and textures. A mix of colors is always popular and beautiful. There are no hard and fast rules here—it is totally up to your imagination! Mold your garden to your tastes and preferences. If you want to be completely precise and see continuous blooms from spring until fall, you need to read up on which flowers bloom at which times of the season. Personally, I like to plant bulbs that spring to life very early (we have all seen crocuses peeking out through a late snow) and go chronologically from there. Daffodils, hyacinths, and muscari are all early bloomers. They are such a welcoming relief from winter as they

reach up out of the earth and make us smile at the wonders of nature. A little later will come tulips (my favorite), ranunculus, anemone, and another favorite of mine, Dutch iris. My gardens will always and forever have alliums habiting happily in their spots. These flowers are amazing—real showstoppers in the landscape. If you have never planted allium bulbs before, I highly recommend you give it a whirl. (You can thank me in the spring). Later in the season, you’ll see lilies, caladiums, cannas, and liatris. These summer stunners give so much character to the garden. Last is the gladiolus. They are stunning in gardens and flower arrangements, but remember—they really need the sun! I am speaking from experience here. I made the mistake of planting gladiolus bulbs in a location that got too much shade, and they never bloomed, much to my dismay. Lesson learned. Now let’s get to the planting of these bulbous wonders. A tip that I recently discovered is to soak your bulbs for two hours in warm water before planting. Make sure you place the bulbs with the pointy end up and the roots down. Occasionally, I have run across bulbs that are confusingly “unpointed” and you cannot really tell which way is up. In that case, just plant it on its side and it will find its way to the surface. A good rule of thumb is to plant your bulbs about five inches deep for optimum growing, but do check out the recommended depth for each type of bulb. Once you have them planted, be sure to add a light layer of mulch. Water them well to give them the best chance to do their winter thing. Bulbs always look best to me when they are planted generously in groups and in several locations. As a novice, I would gingerly place one bulb here and there—of course, when they bloomed there was just one lone bloom in and around the garden. Not a great look, so again, do your pre-planning to get the best results. Landscapes look more pleasing and unified when planted as multiple groupings; you will never regret doing this. There are a couple of things to take note of when shopping for bulbs. The first thing to consider is bulb size. The larger the bulb, the bigger the plant will be, with more and larger flowers. Look the bulbs over to make sure they are fresh (you do not want old, dried-out looking bulbs). Always store bulbs in a cool, dry place until you are ready to plant. A newer idea—for me, anyway— that I have totally embraced is planting bulbs in containers. You plant them the same way you do bulbs in the ground, and they look impressive as they reach for the sky. There is something very light-hearted and whimsical about them gloriously blooming out of pots when we’re accustomed to seeing them in the ground. I always plant elephant ear bulbs in a large container in the spring and place it in a shady area on my patio. They look very picturesque and unique. It’s fun to poke bulbs in the ground in the fall and then enjoy their spectacular blooms in the spring. Get planting!

Surgical Options Painful, Damaged Joints

Our joints are amazing structures! They keep us moving every day of our lives. However, with age or injury, they can cause considerable pain and limit our mobility. When non-surgical treatments do not provide relief, doctors may recommend joint replacement. Knees and hips are the most common replacement procedures, but ankles, wrists, shoulders, or elbows may also benefit from similar procedures. The baby boom generation is responsible for a large increase in the number of joint replacement surgeries. Today’s seniors are living very active lives, and they are not willing to accept limited mobility. An estimated 50 million adults have some form of arthritis— most often osteoarthritis, which is due to “wear and tear” on the joints. This type of arthritis is most commonly found in the knees and hips. Joint pain is often caused by damage to the cartilage that lines the ends of the bones. The two types of joints are hinge (like the knee) and ball-and-socket (like the hip and shoulder). In either case, joint replacement surgery removes all or part of an arthritic or damaged joint and replaces it with metal, ceramic, or plastic components. My mom had knee replacement many years ago, when the procedure was in its infancy. Her joint damage was due to rheumatoid arthritis, and surgery relieved her nearly constant pain. My husband’s more recent knee replacement was necessitated by the wear and tear damage of osteoarthritis. He had played football in high school and then spent years as an electric lineman in the

days when they actually climbed those towering light poles. Joint replacement, or arthroplasty, has come a long way since my mom’s surgery, with improvements in techniques, artificial joint materials, anesthesia, and pain management. In some cases, surgeries that once required a hospital stay of several days are now outpatient procedures. With knee replacement, it might be possible to choose between total replacement or partial replacement. Either way, damaged bone and cartilage are removed and replaced with artificial components. A partial replacement means less damage to ligaments and tendons and, thus, a quicker recovery. It is not usually feasible for rheumatoid arthritis, in which the entire joint is affected by the disease. The patient is typically up and using a walker soon after knee surgery. Upon release, a support person is vital to do the patient’s driving and assist with other daily activities. Physical therapy is generally ordered for several weeks. After three to six weeks, most routine activities can resume.

With hip replacement, the options increase, including differences in technique and also in the location of the incision. Originally, an incision was made on the posterior side of the hip joint. More recently, an anterior (front) incision is preferred by some surgeons. A third option is a lateral incision. Anterior procedures are believed to offer some advantages such as less damage to muscles, which are separated (rather than cut) to allow work on the joint. It does take special training and considerable expertise to perform this technique. Unlike knee replacement, after which physical therapy is a must to regain strength and mobility, hip replacement often does not require the services of a physical therapist after release from the hospital. At-home exercises are prescribed to encourage a gentle return to full range of motion. With either knee or hip surgery, choice of anesthesia makes a big difference. A shortterm spinal anesthesia is ideal, if the patient is a good candidate for it. Recovery is quicker than with general anesthesia, with lower risk of blood clots and nausea or vomiting. Shoulders are also candidates for replacement surgery when pain and lack of function are not relieved by non-surgical methods. Often, just an overnight stay in the hospital is required. Physical therapy or diligence performing at-home exercises (orACS - Surviving Sixth.indd both) is crucial to regaining range of motion. A soft sling is typically used for six weeks while healing takes place. With improvements, artificial joints are now expected to last 15 to 20 years. That’s an important statistic to consider when you’re deciding whether to hold off on replacement surgery or get it in an earlier stage of joint damage. Is joint replacement surgery inevitable? Hardly. We can’t prevent aging, but there are steps that can help prevent injury or early failure. High-impact sports are hard on joints. Walking, swimming, and cycling are better choices for exercise, but don’t give up exercise to save your joints! Excess weight strains joints, as well. Some foods may be especially advantageous for joint health. Legumes, oranges, Brussel sprouts, and brown rice are believed to promote healthy cartilage. We can be thankful that newer surgical techniques, better components, improved anesthesia, and better pain management assure safe, successful joint replacement for living without pain. Honest Conversations Sex Therapy and Total Wellness

Many of us hesitate to reach out to any type of psychotherapist. For some, the blockade is due to a lingering stigma around mental health issues; for others, it is burdensome to schedule personal time. Whatever the impediment, it can be even more daunting to seek the help of a sex therapist. It’s important to recognize that sexual issues affect our overall happiness, confidence, and security. Sex therapy doesn’t just deal with libido and performance issues; it also encourages fearless reflection and self-care. Whether or not your partner joins you in sessions, talking through issues with a sex therapist is a solid investment in your relationship. Like other mental health practitioners, sex therapists must be licensed psychologists, therapists, or counselors in the state in which they work, though there is no specific sex therapy licensure. Practitioners may apply for membership in AASECT (American Association of Sexuality Educators, Counselors, and Therapists), which requires a master’s degree in mental health, psychology, or a similar field, and mandates a certain number of hours of supervised clinical practice. Sex therapy sessions most commonly involve couples, but individual counseling is also available, whether you’re in a relationship and your partner is not interested in therapy or you want to work on some issues on your own. In the same way that family counseling and individual therapy can coexist, each partner is ultimately responsible for their own sexual health. Early on in your discussions with a therapist, you will be

encouraged to talk about your sexual history, including sexual orientation and gender identity, partners, trauma, and the concerns that prompted you to reach out for guidance. Be prepared to share personal details of your sexual experiences. It may seem awkward at first (TMI alert!), but that’s what your therapist is there to hear. There’s no such thing as Too Much Information when it comes to these discussions; it’s actually helpful. Many of the issues your therapist will discuss won’t be specifically about sex. Your therapist will want to know about your general mental health history, what your relationship is like with other family members, and what the dynamic is like between you and your partner outside of the bedroom. There won’t be any physical exams in sex therapy. If your counselor believes there may be an underlying physiological cause to your difficulties, they may recommend additional testing or doctor visits.

There are many causes of discomfort during sex. Some of them are physical, such as experiencing pain during intercourse, erectile dysfunction, or premature or failure to orgasm. A great many more frustrations are sourced in the mind and emotions. These include dealing with past traumas related to sex, such as rape, infidelity, abuse, or body shaming. In these cases, the overarching goals of therapy often include growing to a point of trust and safety with a sexual partner. Other frustrations may be caused by not being on the same page as your partner in areas such as frequency of sex, comfort level with fantasies or experimentation, or sexuality in the context of infertility. Sex therapists aim to help you develop skills at communicating your desires and emotional needs. They may offer frank advice for arousal and pleasure, as a couple or as individuals, including any necessary education on anatomy and anatomical function. Sometimes a shift in attitude is liberating—therapy may encourage partners to move away from viewing orgasm as the ultimate goal, instead embracing the ebb and flow of sensations. Some couples may be interested in exploring adventuresome practices such as role-play or open relationships. A sex therapist can help couples try new things while making sure the exploration is healthful. The therapist is likely to suggest setting boundaries as a couple to ensure that each partner remains a willing participant at every stage. It may feel awkward to broach such subjects at home, but a therapist’s office can serve as neutral ground on which to honestly discuss these desires. Sometimes just talking about these ideas can lead to greater intimacy. If you have been quietly concerned about your connection to your sexuality, take the first step today by contacting a sex therapist. By law, your sessions are completely confidential, and you can expect a judgment-free space in which to share your feelings and concerns. Don’t continue to disregard your anxiety. If these issues have been troubling you for months or years, your heart is trying to tell you that the subject should be addressed. A good sex therapist will work through any issues at your pace and comfort level. You owe it to yourself to be whole and happy, and a sex therapist may be able to help you on your journey. Tips for Women Buying Vehicles Information is Power

Are women at a disadvantage when buying a car? Hmm…maybe? I have an image in my head of a woman walking into a dealership, looking lost and uninformed, and I can see the salespeople thinking,“Here comes an easy sale!”How do you get the respect, deals, and fairness that you deserve? How do you get a great deal when you are not exactly a car enthusiast? It’s all about today’s fiercest weapon: information.

Fear of the unknown is just what it sounds like— fear of the unknown. If you walk into a car dealership without knowing anything about the vehicle you’re interested in or what a fair price would be, you won’t feel confident, and your lack of confidence will show. If you are informed, however, you can enter with a level of confidence that will mark you not as an easy sale but as an educated one. The first thing to establish is your budget for a vehicle. Can you pay for a car outright? What can you afford in monthly payments? If you must get a loan, what kind of interest rate can you get? Do you want to finance it through your own bank? Most dealerships have their own financial departments, but keep in mind that dealerships make big profits in their finance offices. You should have financing pre-approval secured and know your credit score before you ever enter a dealership. Would it make more sense for you to lease a car? That all depends on what your future plans are. If you like to get a new car every few years, a lease might be the way to go, especially if you anticipate having low mileage. If you intend to keep the car for quite a while, it makes more sense to buy. If getting a new vehicle means you no longer need your present car, consider selling it instead of trading it in. It is not uncommon for dealers to offer you $4,000 to $6,000 below market value for your trade. If you can sell your own car at market value, you will have more money in hand to spend on your new vehicle. Once you know what your budget will allow and have decided what kind of vehicle you’d like to get, it’s finally time to visit the dealership. When you walk in, you should be carrying a file folder that holds the pre-approval from your lender and information on dealer fees and models of the car you are interested in. I mention dealer fees for a reason. A good deal of dealer fees are not written in stone and are probably inflated. There are always undisclosed fees like dealer prep, advertising fees, and registration fees. Knowing that can give you some leverage when you are ready to 52

discuss actual pricing. Another point to ponder is whether you’d prefer a new or used vehicle. New vehicles lose a good chunk of their value as soon as you drive out of the lot with them. All dealerships now have pre-approved, previously owned vehicles that may actually be a better value for your money. Do not rely on the salesperson to provide you comparison studies on new versus used cars—that’s something you look up before you arrive. Learn what dealers pay for new and used cars so you can calculate the offer you plan to make. Dealerships love to offer extended warranties. You do not have to purchase a warranty the same day you purchase a vehicle, and you will pay too much if you buy it from a dealership. Ask for a breakdown of all fees and the “out-the-door” price of the car so you can carefully review the paperwork. Typically, car owners pay more for extended warranties than they get back in benefits. Many experts do not recommend buying extended warranties; they advise instead that you check reliability ratings before you purchase a vehicle. You can find ratings on sites like cars.com, consumerreports.org, edmunds.com, and truecar.com. With your folder in hand, you’re ready to negotiate. It will work to your advantage when the salesperson sees that you’re prepared. You are armed with multiple price quotes and can proceed on your terms! Don’t be afraid to ask lots of questions. Always be up-front about what you want, and always trust your gut (about the salesperson as well as the car). Keep the conversation professional and be prepared to leave if you feel rushed. Set a time limit for yourself when you visit a showroom; this gives you a natural opportunity to make an exit and stops you from spending all day at the dealership. Making Waves in Water Safety Little Waves Family Swim School

Little Waves Family Swim School provides a unique opportunity for families to participate in a safe and fun environment where children learn to swim for the first time or refine their skills and prepare for competition. Mike has been in the swimming business for more than 45 years. His experience ranges from teaching at Millard High School to coaching elite swimming at Swim Omaha and Millard Aquatics Club. Many of his athletes went on to compete in the Olympic Trials in Omaha and have become success stories in numerous ways. In the eight years since Mike opened the business, Little Waves has taught over 5,000 students, some as young as six months old. The experts at Little Waves believe that children should start swimming lessons “as early as possible to become safer in the water and more aware of water safety.” Little Waves offers parent-child lessons for children aged six months to three years. In these early childhood classes, an instructor guides the parents in teaching their child to be comfortable and safe in the water. “In the parent-child lessons,” Mike says, “we emphasize rolling onto the back and floating, to feel safe and to be safe.” When children are three years old, they move into lessons without their parents. “At this time, our instructors continue to encourage novice swimmers to roll onto their backs for safety,” Mike says. “The younger they start, the sooner they learn to be safe in and around the water.” Mike and his team believe in an instructional process that encourages fun while developing swim skills and focusing on water safety. Swimming can become a healthy practice in life as well as a fun form of recreation or a competitive sport. It is also a survival skill that everyone needs, no matter how young or old and no matter where life takes us. Do you follow a regular fitness routine, or do you have trouble finding time to work out? If you’d like a fitness routine that fits into your schedule and lifestyle, consider a home gym. Exercising at home offers several benefits. Time is precious, and working out at home saves time because there’s no commute.You also have the convenience of exercising on your schedule.You can work out in the privacy of your own home, at your own pace. Options for achieving a good workout routine at home range from free weights to complete home gym systems. According to the National Institute on Aging, the four types of exercise important in maintaining optimum health are aerobic, strength, balance, and flexibility. Aerobic, or cardiovascular, exercises and activities strengthen the heart and muscles, burn calories, control appetite, improve sleep quality, reduce arthritic pain and stiffness, and help prevent or manage high blood pressure, heart disease, and diabetes. Strength training increases muscle mass, develops strong bones, increases joint flexibility, helps with weight control, and can improve balance and flexibility, which reduces falls and injuries as we age. Free weights are a good option for those who want to build strength without a significant equipment investment. Because free weights are not connected to a piece of equipment, they provide freedom of movement for many exercises. Free weights include dumbbells, barbells, kettlebells, medicine balls, and sandbags. The type that you should use depends upon your goals and workout routine. Basic body movement patterns are squat, lunge, hinge, push, pull, rotate, and anti-rotate. Different exercises call for different types of weights. However, dumbbells, barbells, and kettlebells are the most versatile. Setting goals and determining the kind of exercises you want to perform will help you choose the proper free weights. Interconnected systems of equipment called “home gyms” can help you build strength even if you aren’t comfortable using free weights. These machines can help you learn proper weightlifting mechanics, target specific muscles, and help with bigger lifts. Features include weightlifting benches, stacked metal weights, arm and leg curl or press systems, pull-down bars, and pull-up bars. Home gym systems provide dozens of strength training exercises and weight options. Cardio equipment is available to match any sporting interest, whether it’s walking, running, rowing, climbing, or cycling. Treadmills offer walkers and runners a yearround option without having to deal with unpredictable or uncomfortable weather conditions. Folding treadmills are available for small spaces. Elliptical machines simulate walking and running in a gliding motion, and some models feature movable handlebars for a full-body workout. climbing machine is a vertical apparatus that replicates rock climbing. Exercise bike options abound, including upright, spin, air, and recumbent. Upright bikes are similar to regular bikes. Spin bikes simulate racing bikes with inclined adjustable handles, so riders lean forward. Their seats adjust for both height and proximity to the handlebars. Air bikes have a fan near the pedals, which adds resistance as you pedal, simulating wind outside. Recumbent bikes have seats that sit lower to the ground in a reclined position that offers back support; the pedals are out in front. Don’t let achy joints stop you from getting the exercise you need. Most cardio equipment is also low impact, which is good news for your joints. Treadmills are low impact if used for walking. Elliptical machines, rowing machines, climbing machines, and exercise bikes are also considered low impact. With the right equipment, a good cardio workout is attainable while minimizing stress and impact on joints. Many exercise machines offer numerous multimedia component options. Bluetooth and Wi-Fi connectivity allow wireless streaming to fitness trackers, cell phones, and tablets. Interactive monitors stream on-demand, live, or virtual sessions and provide performance data. Keep in mind that many streaming options are subscription-based, requiring monthly or annual membership fees. When designing your home gym, consider your fitness goals. What kind of exercise do you enjoy, and does it require machines? How often will you exercise? The space needed for your home gym depends upon your exercise interests and the amount of equipment required. Good lighting is essential for safety and atmosphere, as is choosing an inspiring paint color. Flooring such as foam tiles protects floors and your joints. A television is necessary if you stream online classes; hanging the TV will free up floor space. A well-designed, inviting home gym will keep you motivated because you’ll enjoy being in the space. As the weather gets colder, it’s a great time to set up a home gym that will help you and your family achieve your fitness goals in the comfort of your own home. For many of us, the most pressing reason to use pest control is that pests like insects and rodents give us the creepy-crawlies. Did you know that having pests in the home can also cause a risk to you and your family’s health and to the structural integrity of your house? Mice, rats, ticks, and mosquitoes can carry disease, and termites and carpenter ants will cause damage to your home by eating away at the wood inside your walls. Regular visits from a professional pest control specialist can help you detect any issues before they get out of hand. The most common household pests to keep an eye out for are cockroaches, mice, rats, termites, and ants. Cockroaches cause issues by contaminating food, destroying fabric and paper products, staining surfaces, and being potential disease carriers. Small numbers of cockroaches can be controlled by putting out traps, but large numbers of these resilient pests can only be eliminated with the help of a professional. (Let’s be honest—if you see one cockroach, there are probably more.) Mice cause trouble with their droppings, which can be a health hazard, and their tendency to destroy items to build nests. Telltale signs of termites are swarms of small, winged insects and signs of tunneling, darkening, or blistering wood. No matter how small your termite infestation or how quickly you catch it, you will need a professional to exterminate this pest. Ants are the easiest pest to spot, as they are often out in the open. Your pest control pro can figure out where they’re gaining entry to the house and make sure everything is sealed. To keep them from returning, make sure your food containers are tightly sealed and your kitchen counters are regularly wiped with soap or vinegar and water.

Oncology Associates Expert Advice for Cancer Treatment

Dr. Timothy Dorius is very optimistic about cancer treatments. As an oncologist with Oncology Associates, he dives into the newest research and helps patients navigate the system for treatment options. After diagnosis, when life feels upside down, that’s when Dr. Dorius and his team of nurses step up to help. “Our job as oncologists is to help people navigate the various treatment options available to them,” says Dr. Dorius. “A patient with stage one breast cancer is going to have different needs than a patient with stage four breast cancer.” Treatments can include a complicated combination of surgery, radiation, chemotherapy, targeted therapy, and immunotherapy. “I help people understand what their options are and guide them through their treatment decisions.” When it’s time to actually plan for treatment, Oncology Associates follow the national guidelines and all the available evidence, says Dr. Dorius. “Most oncologists know what chemo to give and what guidelines to follow,” he notes, “but I think the kind of supportive care that people receive is crucial. That’s where we stand out as a practice. We have a really great nursing staff to take care of our patients. We’re always available, on call if needed. Our patients have easy access to both oncologists and nurses to help navigate the whole cancer experience and diagnosis.” Dr. Dorius didn’t plan to be an oncologist, but he wouldn’t have it any other way. He started his career in international economics, traveling to Brazil and trying to figure out why some countries prospered and others didn’t. “I was sitting at my desk one day,” he recalls, “and it dawned on me that I wanted to get into medicine.” He was fortunate to be trained by some of the best, including working alongside one of the world’s leading oncologists. Now, Dr. Dorius is pleased to be working in oncology during a time of such great innovation in cancer treatment. “Immunotherapy is definitely the most exciting thing that’s happened in oncology in the last seven or eight years,” he says. “It has changed the game for several cancers. The survival rate we can expect from patients with advanced lung cancer is much better than it used to be.” Immunotherapy is used to treat several cancers, including melanoma, lung cancer, bladder cancer, some colon cancers, and, most recently, breast cancer—even in patients with the worst prognoses. Before recommending immunotherapy, Dr. Dorius gets as much information as possible about the pathology of the tumor. Certain markers suggest that immunotherapy might be the best treatment option; special testing is done to identify these markers. The principle of this type of therapy started over a century ago, Dr. Dorius says, 62

when a doctor tried to infect cancer with bacteria, hoping the immune system would attack the cancer along with the bacteria. Today, the results are Dr. Timothy Dorius M.D. driven by the biology of the cancer, and immunotherapy helps double life expectancy. How does it work? Immunotherapy is an IV infusion (commonly seen on commercials for Keytruda, Opdivo, and others) that is administered every two to six weeks for one or two years. These infusions gear up the immune system to help it recognize that the cancer is foreign and, therefore, should be attacked. “It takes the brakes off the immune system and allows it to get to work fighting the cancer,” says Dr. Dorius. “It’s like it removes the blindfold from the immune system.” Though it is less predictable than some other cancer treatments, immunotherapy is also less risky and causes fewer side effects. Immunotherapy is constantly improving. Targeted therapies are advancing in treatment options, too, and mutations can be treated with oral medications rather than with traditional chemotherapy. “Medicine changes rapidly, and I’m always ready to learn the latest treatments and the newest research,” says Dr. Dorius. “So many factors go into cancer. Environment and genetics— it’s hard to predict. The interesting thing about cancer is that it’s still a mystery.” Dr. Dorius feels honored to help people who are going through the toughest time of their lives, even if the outcome isn’t what everyone hopes. “It’s rewarding to work with people who have incurable cancer, helping them through something that is terribly painful but seeing the beauty that can come from the relationship they have with their families and the support they get from their friends,” Dr. Dorius concludes. “We’re all in this together.
